Abstract:Vaccination is an important means to fight against the spread of the SARS-CoV-2 virus and its variants. In this work, we propose a general susceptible-vaccinated-exposed-infected-hospitalized-removed (SVEIHR) model and derive its basic and effective reproduction numbers. We set Hong Kong as an example and calculate conditions of herd immunity for multiple vaccines and disease variants. The model shows how the number of confirmed COVID-19 cases in Hong Kong during the second and third waves of the COVID-19 pandemic would have been reduced if vaccination were available then. We then investigate the relationships between various model parameters and the cumulative number of hospitalized COVID-19 cases in Hong Kong for the ancestral, Delta, and Omicron strains. Numerical results demonstrate that the static herd immunity threshold corresponds to one percent of the population requiring hospitalization or isolation at some point in time. We also demonstrate that when the vaccination rate is high, the initial proportion of vaccinated individuals can be lowered while still maintaining the same proportion of cumulative hospitalized/isolated individuals.
Impacts of vaccination on the variant selection of SARS-CoV-2 and the subsequent case hospitalization rate: insights from the first U.S. Omicron wave

Abstract:Background COVID-19 vaccines, while providing protection against hospitalization, could inadvertently increase selection pressure on new immune-escape variants, impacting case hospitalization rate (CHR). Methods Using epidemiological and genomic sequence data, we calculated daily state-level CHR and the proportion of Omicron mutations in the United States during the first Omicron wave (between December 11, 2021, and March 22, 2022). We derived mathematical formulas to link evolution responses to an increasing population immunity with lagged regression models. Using mediation analysis, together with generalized linear mixed models and distributed lag nonlinear models, we assessed how natural selection, shaped by vaccine coverage, impacts CHR. Results The model showed that increasing vaccination coverage from 45% to 70% contributed to a reduction in CHR from 5.8% to 4.4%. Part of the reduction resulted from direct vaccine protection (OR: 0.85, p-value=0.012). However, the higher vaccination coverage was correlated with a 20% increase in the proportion of BA.1/BA.1.1-associated mutations. As the Omicron variants were less severe than their predecessors (Delta), CHR further reduced (OR: 0.87, p-value<0.001). Marginally, this could reduce CHR from 5.8% to 5.1% via the adaptation of Omicron variants as marginal effect without accounting for direct vaccine protection. Conclusions The study offers new insight into vaccine strategies for reducing hospitalization risk by shortening [or maintaining] the circulation of more [or less] virulent variants among infectious diseases. Continuous monitoring of variant evolution, including their virulence, is critical.

Abstract:Abstract After the adoption of a dynamic zero-COVID strategy in China for nearly two years, whether and for how long this policy can remain in place is unclear. The debate has thus shifted towards the identification of mitigation strategies capable to prevent the disruption of the healthcare system, should a nationwide epidemic caused by the SARS-CoV-2 Omicron variant start to unfold. To this aim, we developed a mathematical model of SARS-CoV-2 transmission tailored to the unique immunization and epidemiological situation of China. We find that the level of immunity induced by the current vaccination campaign would be insufficient to prevent overwhelming the healthcare system and major losses of human lives. Instead, a synergetic strategy would be needed and based on 1) a heterologous booster vaccination campaign, 2) treating 50% of symptomatic cases with an antiviral with an 80% efficacy in preventing severe outcomes, and 3) the adoption of non-pharmaceutical interventions (NPIs) capable of reducing Rt to ≤2. Protecting vulnerable individuals by ensuring accessibility to vaccines and antivirals, and maintaining a certain degree of NPIs should be emphasised in a future mitigation policy, possibly supported by strengthening critical care capacity and the development of highly efficacious vaccines with long-lasting immunity.
Estimating long-term vaccine effectiveness against SARS-CoV-2 variants: a model-based approach

Abstract:With the ongoing evolution of the SARS-CoV-2 virus, variant-adapted vaccines are likely to be required. Given the challenges of conducting clinical trials against a background of widespread infection-induced immunity, updated vaccines are likely to be adopted based on immunogenicity data. We extended a modelling framework linking immunity levels and protection and fitted the model to vaccine effectiveness data from England for three vaccines (Oxford/AstraZeneca AZD1222, Pfizer-BioNTech BNT162b2, Moderna mRNA-1273) and two variants (Delta and Omicron) to predict longer-term effectiveness against mild disease, hospitalisation and death. We use these model fits to predict the effectiveness of the Moderna bivalent vaccine (mRNA1273.214) against the Omicron variant using immunogenicity data. Our results suggest sustained protection against hospitalisation and death from the Omicron variant over the first six months following boosting with the monovalent vaccines but a gradual waning to moderate protection after 1 year (median predicted vaccine effectiveness at 1 year in 65+ age group: AZD1222 38.9%, 95% CrI 31.8%-46.8%; BNT162b2 53.3%, 95% CrI 49.1%-56.9%; mRNA-1273 60.0%, 95% CrI 56.0%-63.6%). Furthermore, we predict almost complete loss of protection against mild disease over this period (mean predicted effectiveness at 1 year 7.8% for AZD1222, 13.2% for BNT162b2 and 16.7% for mRNA-1273). Switching to a second booster with the bivalent mRNA1273.214 vaccine against Omicron BA.1/2 is predicted to prevent nearly twice as many hospitalisations and deaths over a 1-year period compared to administering a second booster with the monovalent mRNA1273 vaccine. Ongoing production and administration of variant-specific vaccines are therefore likely to play an important role in protecting against severe outcomes from the ongoing circulation of SARS-CoV-2.
Impact of immune evasion, waning and boosting on dynamics of population mixing between a vaccinated majority and unvaccinated minority

Abstract:Background We previously demonstrated that when vaccines prevent infection, the dynamics of mixing between vaccinated and unvaccinated sub-populations is such that use of imperfect vaccines markedly decreases risk for vaccinated people, and for the population overall. Risks to vaccinated people accrue disproportionately from contact with unvaccinated people. In the context of the emergence of Omicron SARS-CoV-2 and evolving understanding of SARS-CoV-2 epidemiology, we updated our analysis to evaluate whether our earlier conclusions remained valid. Methods We modified a previously published Susceptible-Infectious-Recovered (SIR) compartmental model of SARS-CoV-2 with two connected sub-populations: vaccinated and unvaccinated, with non-random mixing between groups. Our expanded model incorporates diminished vaccine efficacy for preventing infection with the emergence of Omicron SARS-CoV-2 variants, waning immunity, the impact of prior immune experience on infectivity, “hybrid” effects of infection in previously vaccinated individuals, and booster vaccination. We evaluated the dynamics of an epidemic within each subgroup and in the overall population over a 10-year time horizon. Results Even with vaccine efficacy as low as 20%, and in the presence of waning immunity, the incidence of COVID-19 in the vaccinated subpopulation was lower than that among the unvaccinated population across the full 10-year time horizon. The cumulative risk of infection was 3–4 fold higher among unvaccinated people than among vaccinated people, and unvaccinated people contributed to infection risk among vaccinated individuals at twice the rate that would have been expected based on the frequency of contacts. These findings were robust across a range of assumptions around the rate of waning immunity, the impact of “hybrid immunity”, frequency of boosting, and the impact of prior infection on infectivity in unvaccinated people. Interpretation Although the emergence of the Omicron variants of SARS-CoV-2 has diminished the protective effects of vaccination against infection with SARS-CoV-2, updating our earlier model to incorporate loss of immunity, diminished vaccine efficacy and a longer time horizon, does not qualitatively change our earlier conclusions. Vaccination against SARS-CoV-2 continues to diminish the risk of infection among vaccinated people and in the population as a whole. By contrast, the risk of infection among vaccinated people accrues disproportionately from contact with unvaccinated people.

Abstract:Importance: While a substantial fraction of the US population was infected with SARS-CoV-2 during December 2021-February 2022, the subsequent evolution of population immunity against SARS-CoV-2 Omicron variants reflects the competing influences of waning protection over time and acquisition or restoration of immunity through additional infections and vaccinations. Objective: To estimate changes in population immunity against infection and severe disease due to circulating SARS-CoV-2 Omicron variants in the United States from December 2021 to October 2022, and to quantify the protection against a potential 2022-2023 winter SARS-CoV-2 wave. Design, setting, participants: Bayesian evidence synthesis of reported COVID-19 data (diagnoses, hospitalizations), vaccinations, and waning patterns for vaccine- and infection-acquired immunity, using a mathematical model of COVID-19 natural history. Main Outcomes and Measures: Population immunity against infection and severe disease from SARS-CoV-2 Omicron variants in the United States, by location (national, state, county) and week. Results: By November 10, 2022, 94% (95% CrI, 79%-99%) of the US population were estimated to have been infected by SARS-CoV-2 at least once. Combined with vaccination, 97% (95%-99%) were estimated to have some prior immunological exposure to SARS-CoV-2. Between December 1, 2021 and November 10, 2022, protection against a new Omicron infection rose from 22% (21%-23%) to 63% (51%-75%) nationally, and protection against an Omicron infection leading to severe disease increased from 61% (59%-64%) to 89% (83%-92%). Increasing first booster uptake to 55% in all states (current US coverage: 34%) and second booster uptake to 22% (current US coverage: 11%) would increase protection against infection by 4.5 percentage points (2.4-7.2) and protection against severe disease by 1.1 percentage points (1.0-1.5). Conclusions and Relevance: Effective protection against SARS-CoV-2 infection and severe disease in October 2022 was substantially higher than in December 2021. Despite this high level of protection, a more transmissible or immune evading (sub)variant, changes in behavior, or ongoing waning of immunity could lead to a new SARS-CoV-2 wave.

Abstract:At the end of 2021, the Omicron variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) took over from the preceding Delta variant as the dominant strain, causing a huge wave of infection and death due to its high transmissibility and immune escape. The Omicron wave was the fifth of the pandemic in Hong Kong Special Administrative Region, China. Given the huge variability in the mortality and vaccination rates associated with coronavirus disease (COVID-19) across age groups in the fifth wave, we constructed a two-age-group transmission model with time-varying transmission, vaccination, and death rates to assess the dynamics of the Omicron variant in two age groups (a younger group aged 0-59 years and a group of older adults aged ≥ 60 years). We calibrated the model to eight sets of epidemic data from Hong Kong using the Markov Chain Monte Carlo algorithm to disentangle the transmission degree and disease severity for the two age groups. The calibrated model was then used to explore counterfactual scenarios in which the booster vaccination campaign was modified to assess the direct and indirect effects of booster doses in the two age groups. We also performed sensitivity analyses by varying key model parameters so as to evaluate the uncertainty of some parameters on the epidemic trend. The results indicated a high degree of assortativity (intra-group contact) in the two age groups, and the intra-group transmission rate was much higher than the inter-group transmission rate. Younger individuals showed a slightly higher transmission advantage in the fifth of the pandemic in Hong Kong. Moreover, the average mortality rate in the older group was 87 times higher than that in the younger group. We confirmed that administering booster doses of vaccines offered substantial protection to the target population, particularly against death. There are vast infection-suppression benefits of vaccinating younger individuals that play an important role in transmission. Meanwhile, vaccination in the older individuals has vast benefits in reducing deaths. Vaccination campaigns targeting a particular age group are far less effective than boosting wider segments of the population. This study provides a retrospective understanding of Hong Kong's fifth wave of the pandemic, which is crucial for future projections and the planning of interventions.

Modelling herd immunity requirements in Queensland: impact of vaccination effectiveness, hesitancy and variants of SARS-CoV-2

Abstract:Long-term control of SARS-CoV-2 outbreaks depends on the widespread coverage of effective vaccines. In Australia, two-dose vaccination coverage of above 90% of the adult population was achieved. However, between August 2020 and August 2021, hesitancy fluctuated dramatically. This raised the question of whether settings with low naturally derived immunity, such as Queensland where less than 0.005% of the population is known to have been infected in 2020, could have achieved herd immunity against 2021's variants of concern. To address this question, we used the agent-based model C ovasim . We simulated outbreak scenarios (with the Alpha, Delta and Omicron variants) and assumed ongoing interventions (testing, tracing, isolation and quarantine). We modelled vaccination using two approaches with different levels of realism. Hesitancy was modelled using Australian survey data. We found that with a vaccine effectiveness against infection of 80%, it was possible to control outbreaks of Alpha, but not Delta or Omicron. With 90% effectiveness, Delta outbreaks may have been preventable, but not Omicron outbreaks. We also estimated that a decrease in hesitancy from 20% to 14% reduced the number of infections, hospitalizations and deaths by over 30%. Overall, we demonstrate that while herd immunity may not be attainable, modest reductions in hesitancy and increases in vaccine uptake may greatly improve health outcomes.
Epidemiologic Characteristics of SARS-CoV-2 Omicron BA.5.1.3 Variant and the Protection Provided by Inactivated Vaccination

Abstract:Omicron variants have become the dominant SARS-CoV-2 variants due to their increased transmissibility and immune-escape ability. An outbreak of the Omicron variant BA.5.1.3 occurred in August 2022 in Sanya, China. Studying Omicron variants can promote the understanding of them and further contribute to managing the SARS-CoV-2 prevalence. This retrospective study analyzed the data of 258 patients with asymptomatic or mild SARS-CoV-2 admitted to the First Cabin Hospital of Sanya, China, between August 14 and September 4, 2022. The 258 patients comprised 128 males and 130 females with a mean age of 36.6 years and mean length of medical observation (LMO) of 10.1 days. Multiple linear regression analysis indicated that LMO was positively and negatively associated with age (p=0.036) and vaccination status (p=0.004), respectively. A Cox proportional-hazards model revealed that age (hazard ratio [HR]=0.99, p=0.029) and vaccination (HR=1.23, p=0.023) were risk and protective factors for LMO, respectively. Causal mediation analysis indicated that vaccination suppressed the effect of prolonging LMO caused by increasing age. Recovery times became longer with increasing age, which could be counterbalanced by vaccination. The present results indicate that vaccination interventions, even those developed through inactivated approaches, can still provide protection against Omicron variants.

Abstract:Omicron and its sublineages are currently predominant and have triggered epidemiological waves of SARS-CoV-2 around the world due to their high transmissibility and strong immune escape ability. Vaccines are key measures to control the COVID-19 burden. Omicron BA.2 caused a large-scale outbreak in Shanghai since March 2022 and resulted in over 0.6 million laboratory-confirmed infections. The vaccine coverage of primary immunization among residents aged 3 years and older in Shanghai exceeded 90%, and inactivated COVID-19 vaccines were mainly delivered. In the context of high vaccine coverage, we conducted a cohort study to assess vaccine effects on reducing the probability of developing symptoms or severity of disease in infections or nonsevere cases. A total of 48,243 eligible participants were included in this study, the majority of whom had asymptomatic infections (31.0%) and mild-to-moderate illness (67.9%). Domestically developed COVID-19 vaccines provide limited protection to prevent asymptomatic infection from developing into mild-to-moderate illness and durable protection to prevent nonsevere illness from progressing to severe illness caused by Omicron BA.2. Partial vaccination fails to provide effective protection in any situation. The level of vaccine effects on disease progression in the elderly over 80 years old was relatively lower compared with other age groups. Our study results added robust evidence for the vaccine performance against Omicron infection and could improve vaccine confidence.

Abstract:The highly infectious and low-mortality Omicron variant of COVID-19 was identified in Botswana and reported to the WHO by South Africa. Spread dynamics is often used in the study of infectious diseases. Seasons, human contact, vaccination and other factors are the important factors affecting the spread of diseases. To better study the dynamical characteristics and predict the epidemic trend of the Omicron variants, we propose modified infectious disease models based on the SEIAR model, and examine the spread of the Omicron variants using mathematical modeling. We modify the traditional SEIAR model with incorporating a new factor: the number of vaccinations. The first modified model is referred to as the IARD model, which is applicable to the period before the outbreak of Omicron. Given the high infectivity of the Omicron variants and the role of vaccinations in reducing the spread risk during the outbreak of Omicron, we further add a factor for vaccination booster and construct a new dynamical model different from IARD model, called the IARD model. Through stability analysis, we confirm the existence of the local asymptotic stability of the disease-free equilibrium point and the local equilibrium point under the given conditions for both IARD model and IARD model. Additionally, sensitivity analysis is performed on the basic regeneration number of both models. Parameter estimation and numerical simulations are conducted using the epidemic data from Tokyo, Japan. Through sensitivity analysis, we find that increasing vaccination rates and reducing human contact can reduce the number of infections and alleviate medical pressure. To prevent and control the epidemic, the government can minimize human contact and promote the necessity of vaccination to the public, as they can effectively improve individual immunity, reduce the risk of virus infection, and limit the virus spread.

Abstract:COVID-19 is a respiratory disease triggered by an RNA virus inclined to mutations. Since December 2020, variants of COVID-19 (especially Delta and Omicron) continuously appeared with different characteristics that influenced death and transmissibility emerged around the world. To address the novel dynamics of the disease, we propose and analyze a dynamical model of two strains, namely native and mutant, transmission dynamics with mutation and imperfect vaccination. It is also assumed that the recuperated individuals from the native strain can be infected with mutant strain through the direct contact with individual or contaminated surfaces or aerosols. We compute the basic reproduction number, , which is the maximum of the basic reproduction numbers of native and mutant strains. We prove the nonexistence of backward bifurcation using the center manifold theory, and global stability of disease-free equilibrium when , that is, vaccine is effective enough to eliminate the native and mutant strains even if it cannot provide full protection. Hopf bifurcation appears when the endemic equilibrium loses its stability. An intermediate mutation rate leads to oscillations. When increases over a threshold, the system regains its stability and exhibits an interesting dynamics called endemic bubble. An analytical expression for vaccine-induced herd immunity is derived. The epidemiological implication of the herd immunity threshold is that the disease may effectively be eradicated if the minimum herd immunity threshold is attained in the community. Furthermore, the model is parameterized using the Indian data of the cumulative number of confirmed cases and deaths of COVID-19 from March 1 to September 27 in 2021, using MCMC method. The cumulative cases and deaths can be reduced by increasing the vaccine efficacies to both native and mutant strains. We observe that by considering the vaccine efficacy against native strain as 90%, both cumulative cases and deaths would be reduced by 0.40%. It is concluded that increasing immunity against mutant strain is more influential than the vaccine efficacy against it in controlling the total cases. Our study demonstrates that the COVID-19 pandemic may be worse due to the occurrence of oscillations for certain mutation rates (i.e., outbreaks will occur repeatedly) but better due to stability at a lower infection level with a larger mutation rate. We perform sensitivity analysis using the Latin Hypercube Sampling methodology and partial rank correlation coefficients to illustrate the impact of parameters on the basic reproduction number, the number of cumulative cases and deaths, which ultimately sheds light on disease mitigation.

Evaluating the Impact of SARS-CoV-2 Variants on the COVID-19 Epidemic and Social Restoration in the United States: A Mathematical Modelling Study

Abstract:Background: Multiple SARS-CoV-2 variants are still rampant across the United States (US). We aimed to evaluate the impact of vaccination scale-up and potential reduction in the vaccination effectiveness on the COVID-19 epidemic and social restoration in the US. Methods: We extended a published compartmental model and calibrated the model to the latest US COVID-19 data. We estimated the vaccine effectiveness against the variant and evaluated the impact of a potential reduction in vaccine effectiveness on the epidemics. We explored the epidemic trends under different levels of social restoration. Results: We estimated the overall existing vaccine effectiveness against the variant as 88.5% (95% CI: 87.4–89.5%) with the vaccination coverage of 70% by the end of August, 2021. With this vaccine effectiveness and coverage, there would be 498,972 (109,998–885,947) cumulative infections and 15,443 (3,828–27,057) deaths nationwide over the next 12 months, of which 95.0% infections and 93.3% deaths were caused by the variant. Complete social restoration at 60, 65, 70% vaccination coverage would increase cumulative infections to 1.6 (0.2–2.9) million 0.7 (0.1–1.2) million, and 511,159 (110,578–911,740), respectively. At same time it would increase cumulative deaths to 39,040 (5,509–72,570), 19,562 (3,873–35,250), 15,739 (3,841–27,638), respectively. However, if the vaccine effectiveness were reduced to 75%, 50% or 25% due to new SARS-CoV-2 variants, there would be 667,075 (130,682–1,203,468), 1.7 (0.2–3.2) million, 19.0 (5.3–32.7) million new infections and 19,249 (4,281–34,217), 42,265 (5,081–79,448), 426,860 (117,229–736,490) cumulative deaths to occur over the next 12 months. Further, social restoration at a lower vaccination coverage would lead to even greater secondary outbreaks. Conclusion: Current COVID-19 vaccines remain effective against the SARS-CoV-2 variant, and 70% vaccination coverage would be sufficient to restore social activities to a pre-pandemic level. Further reduction in vaccine effectiveness against SARS-CoV-2 variants would result in a potential surge of the epidemic. Multiple measures, including public health interventions, vaccination scale-up and development of a new vaccine booster, should be integrated to counter the new challenges of new SARS-CoV-2 variants.

Emerging Omicron subvariants evade neutralizing immunity elicited by vaccine or BA.1/BA.2 infection

Abstract:The newly emerging severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Omicron BA.2.75 and BA.2.76 subvariants contained 35 and 29 additional mutations in its spike (S) protein compared with the reference SARS-CoV-2 genome, respectively. Here, we measured the evasion degree of the BA.1, BA.2, BA.4, BA.5, BA.2.75, and BA.2.76 subvariants from neutralizing immunity in people previously infected with the Omicron BA.1 and BA.2, determined the effect of vaccination on immune evasion, and compared the titers of neutralizing antibodies in serums between acute infection and convalescence. Results showed that the neutralization effect of serums from patients with different vaccination statuses and BA.1/BA.2 breakthrough infection decreased with the Omicron evolution from BA.1 to BA.2, BA.4, BA.5, BA.2.75, and BA.2.76. This study also indicated that the existing vaccines could no longer provide effective protection, especially for the emerging BA.2.75 and BA.2.76 subvariants. Therefore, vaccines against emerging epidemic strains should be designed specifically. In the future, we can not only focus on the current strains, but also predict and design new vaccines against potential mutant strains. At the same time, we can combine the virus strains' infection characteristics to develop protective measures for virus colonization areas, such as nasal protection spray. Besides, further studies on the Y248N mutation of BA.2.76 subvariant were also necessary to explore its contribution to the enhanced immune evasion ability.

Abstract:This study analyzes the co‐evolution of SARS‐CoV‐2 mutation, ACE2 receptor binding, and NAb responses across various variants using SARS‐CoV‐2 broad neutralizing antibody (bNAb) assay. The results reveal the link between vaccine efficacy and vaccine type, dosing, and the hybrid vaccination combining the WT inactivated vaccine with the Omicron BA.4/5 mRNA vaccine may provide broad protection against both non‐Omicron variants and Omicron variants. Host immunity is central to the virus's spread dynamics, which is significantly influenced by vaccination and prior infection experiences. In this work, we analyzed the co‐evolution of SARS‐CoV‐2 mutation, angiotensin‐converting enzyme 2 (ACE2) receptor binding, and neutralizing antibody (NAb) responses across various variants in 822 human and mice vaccinated with different non‐Omicron and Omicron vaccines is analyzed. The link between vaccine efficacy and vaccine type, dosing, and post‐vaccination duration is revealed. The classification of immune protection against non‐Omicron and Omicron variants is co‐evolved with genetic mutations and vaccination. Additionally, a model, the Prevalence Score (P‐Score) is introduced, which surpasses previous algorithm‐based models in predicting the potential prevalence of new variants in vaccinated populations. The hybrid vaccination combining the wild‐type (WT) inactivated vaccine with the Omicron BA.4/5 mRNA vaccine may provide broad protection against both non‐Omicron variants and Omicron variants, albeit with EG.5.1 still posing a risk. In conclusion, these findings enhance understanding of population immunity variations and provide valuable insights for future vaccine development and public health strategies.

Abstract:In a population with ongoing vaccinations, the trajectory of a pandemic is determined by how the virus spreads in the unvaccinated, vaccinated without boosters, and vaccinated with boosters, which will exhibit distinct transmission dynamics based on different levels of natural and vaccine-induced immunity. We found that enhancing the use of face masks in a partially vaccinated population is associated with a reduction of new infections, hospitalizations, and deaths. We highly recommend the use of a face mask with at least a 50% efficiency, such as improved cloth and surgical face masks, due to its effectivity and cost ratio. Our simulations indicated that there may be two upcoming Omicron waves (in the last months of 2022 and in May 2023). The magnitude of these waves will be 75% and 40% lower than their prior wave. Moreover, the size of these waves is heavily influenced by immunity parameters like waning immunity and cross-immunity protection. Hence, we recommend continuing the use of face masks to decrease transmission because we are not developing sterilizing immunity if we get infected by a prior sublineage, meaning that we can still get infected regardless of the acquired immunity.

Abstract:Over the past several years, the emergence of novel SARS-CoV-2 variants has led to multiple waves of increased COVID-19 incidence. When the Omicron variant emerged, there was considerable concern about its potential impact in the winter of 2021-2022 due to its increased fitness. However, there was also considerable uncertainty regarding its likely impact due to questions about its relative transmissibility, severity, and degree of immune escape. We sought to evaluate the ability of an agent-based model to forecast incidence in the context of this emerging pathogen variant. To project COVID-19 cases and deaths in Indiana, we calibrated our model to COVID-19 hospitalizations, deaths, and test-positivity rates through November 2021, and then projected COVID-19 incidence through April 2022 under four different scenarios that covered the plausible ranges of Omicron's severity, transmissibility, and degree of immune escape. Our initial projections from December 2021 through March 2022 indicated that under a pessimistic scenario with high disease severity, the peak in weekly COVID-19 deaths in Indiana would be larger than the previous peak in December 2020. However, retrospective analyses indicate that Omicron's severity was closer to the optimistic scenario, and even though cases and hospitalizations reached a new peak, fewer deaths occurred than during the previous peak. According to our results, Omicron's rapid spread was consistent with a combination of higher transmissibility and immune escape relative to earlier variants. Our updated projections starting in January 2022 accurately predicted that cases would peak in mid-January and decline rapidly over the next several months. The performance of our projections shows that following the emergence of a new pathogen variant, models can help quantify the potential range of outbreak magnitudes and trajectories. Agent-based models are particularly useful in these scenarios because they can efficiently track individual vaccination and infection histories with multiple variants with varying degrees of cross-protection.

Abstract:Background: The Covid-19 pandemic has been characterized by the emergence of novel SARS-CoV-2 variants, each with distinct properties influencing transmission dynamics, immune escape, and virulence, which, in turn, influence their impact on local populations. Swift analysis of the properties of newly emerged variants is essential in the initial days and weeks to enhance readiness and facilitate the scaling of clinical and public health system responses. Methods: This paper introduces a two-variant metapopulation compartmental model of disease transmission to simulate the dynamics of disease transmission during a period of transition to a newly dominant strain. Leveraging novel S-gene dropout analysis data and genomic sequencing data, combined with confirmed Covid-19 case data, we estimate the epidemiological characteristics of the Omicron variant, which replaced the Delta variant in late 2021 in Philadelphia, PA. We utilized a grid-search method to identify plausible combinations of model parameters, followed by an ensemble adjustment Kalman filter for parameter inference. Results: The model successfully estimated key epidemiological parameters; we estimated the ascertainment rate of 0.22 (95% credible interval 0.15-0.29) and transmission rate of 5.0 (95% CI 2.4-6.6) for the Omicron variant. Conclusions: The study demonstrates the potential for this model-inference framework to provide real-time insights during the emergence of novel variants, aiding in timely public health responses.

Abstract:Having adopted a dynamic zero-COVID strategy to respond to SARS-CoV-2 variants with higher transmissibility since August 2021, China is now considering whether, and for how long, this policy can remain in place. The debate has thus shifted towards the identification of mitigation strategies for minimizing disruption to the healthcare system in the case of a nationwide epidemic. To this aim, we developed an age-structured stochastic compartmental susceptible-latent-infectious-removed-susceptible model of SARS-CoV-2 transmission calibrated on the initial growth phase for the 2022 Omicron outbreak in Shanghai, to project COVID-19 burden (that is, number of cases, patients requiring hospitalization and intensive care, and deaths) under hypothetical mitigation scenarios. The model also considers age-specific vaccine coverage data, vaccine efficacy against different clinical endpoints, waning of immunity, different antiviral therapies and nonpharmaceutical interventions. We find that the level of immunity induced by the March 2022 vaccination campaign would be insufficient to prevent an Omicron wave that would result in exceeding critical care capacity with a projected intensive care unit peak demand of 15.6 times the existing capacity and causing approximately 1.55 million deaths. However, we also estimate that protecting vulnerable individuals by ensuring accessibility to vaccines and antiviral therapies, and maintaining implementation of nonpharmaceutical interventions could be sufficient to prevent overwhelming the healthcare system, suggesting that these factors should be points of emphasis in future mitigation policies.
