Abstract:OBJECTIVE To investigate the curative effect of Xuan Bai Cheng Qi Tang and mirabilite combined with pulse high‐volume hemofiltration (PHVHF) in treatment of patients with severe acute pancreatitis (SAP) ,in or‐der to provide references for clinical treatment of SAP .METHODS A total of 13 SAP cases were selected from Jan . 2015 to Oct .2015 in hospital intensive medicine department .On the basis of conventional western medicine treat‐ment ,the patients were given Xuan Bai Cheng Qi Tang and mirabilite ,enema and external ,and then combined with PHVHF therapy .The changes of life signs ,abdominal symptoms ,blood biochemical indexes ,blood gas anal‐ysis ,acute physiology and chronic health conditions and APACHEII score and Ranson score before and after the treatment of patients were observed .RESULTS The body temperature ,heart rate and respiratory rate of patients were significantly relieved than those before treatment (P<0 .05) .The hemodynamics of the survival of 12 pa‐tients were stable ,and the average arterial pressures were (42 ± 3) and (74 ± 7)mmHg ,which were significantly increased after treatment(P<0 .05) .The serum levels of alanine aminotransferase ,serum amylase and total bili‐rubin were significantly decreased after treatment (P<0 .05) .The arterial oxygen partial pressure ,arterialcarbon dioxide partial pressure and oxygenation index were significantly higher than those before treatment (P< 0 .05) . CONCLUSION The clinical effects of Xuan Bai Cheng Qi Tang and mirabilite combined with PHVHF in treatment of patients with severe acute pancreatitis is significant ,which can be used as one of the methods in treatment of SAP .
Continuous Blood Purification on Immune Homostasis in Patients with Severe Acute Pancreatitis

Abstract:Objective:To investigate the effects of continuous blood purification(CBP)on immune homostasis in patients with severe acute pancreatitis(SAP).Methodology:Twelve SAP patients,all males,diagnosed as SIRS,were enrolled in this prospective cohort study.Among them,seven patients were proved to have bacteria infection.Those patients were divided into two groups randomly.They were control group(four patients who received conventional treantment)and CBP group(eight patients who received conventional treantment and CBP treatment for 72 hours).The clinical prognosis was compared,and the blood samples were taken from those patients at 0,6,12,24,48 and 72hr during CBP to measure the levels of plasma cytokines profile with antibody chip.Meanwhile,effects of CBP treatment on peripheral monocytes,HLA-DR expression were also studied.Results:(1)After CBP treatment for 72hrs,APACHE-Ⅱ score and α-amylase in patients was significantly lowered than those before CBP,and renal function became recovery(P0.05).While there was no marked improvement of patients in control group except that plasma α-amylase in these patients were lowered to some extents(P0.05).(2)Compared with healthy individuals,the plasma levels of cytokines in patients with SAP were great higher except for IL-4(P0.05).After CBP treatment for 72hrs,the plasma levels of the IFN-γ,IL-1,IL-2 and IL-5,IL-10,IL-13 were markedly decreased,the levels of TNF-α and IL-6 were also reduced but without significant difference(P0.05).The ratio of IL-13/TNF-α and IL-13/IL-1 drop was lower than those before treatment.With CBP treatment,the HLA-DR expression and monocytes was markedly elevated(P0.05),while the peripheral CD4+/CD8+ T cells were increased insignificantly(P0.05).In contrast,no change was shown in control patients,cytokines in some patients were even elevated a little.Conclusion:CBP treatment can not only effectively improve the prognosis of patients with SAP,ameliorate acid-base balance and remove metabolic waste,but also can decrease the production of cytokines,improve HLA-DR expression and restore immune homeostasis.After CBP treatment,IL-13/TNF-α and IL-13/IL-1 could be lower which suggests that the CBP clearance ability to Th2 cytokines is more effective.

Patients and Methods: A total of 155 SAP patients were collected and followed up for 6 months. The participants were divided into control (49 cases) and BP group (106 cases) according to whether they received BP treatment or not. The primary outcomes were 6-month mortality, length of hospital stay, and hospitalization costs. Propensity score matching (PSM) analysis was performed based on various factors such as gender, age, etiology, SOFA score, JSS score, and creatinine value on day 1. Results: There were significant differences in all baseline data between BP and control groups ( p < 0.05). However, there was a significant difference in the mortality, length of hospital stay, hospital costs and infection aggravation rate the in outcome data for 6-months (all p < 0.05). BP was not considered a death factor in any adjusted models, with p -values ranging from 0.81 to 0.93. The results of subgroup analysis after PSM showed that BP mode had no significant impact on prognostic indicators, but the length of ICU stay and total costs were significantly increased (all p < 0.001). There was no significant difference in mortality among the cases that did not require early intervention after 6 months ( p =0.487). However, the patients in BP group had longer ICU stays ( p =0.001) and higher hospitalization costs ( p < 0.001) compared to the control group. Conclusion: The utilization of BP therapy did not decrease the 6-month mortality in SAP patients. Additionally, BP therapy has a significant impact on the duration of ICU stay or hospitalization expenses. However, the effectiveness and cost-efficiency of this therapy are unsatisfactory, and early intervention does not enhance survival benefits. Furthermore, there was no substantial variation in survival benefits between continuous veno-venous hemofiltration (CVVH) alone and compound BP. Keywords: severe acute pancreatitis, blood purification, long-term efficacy, cost-efficiency Acute pancreatitis (AP) is an inflammatory reaction induced by the state of the pancreas caused by factors such as gallstones, alcohol, hyperlipidemia, etc. Globally, the incidence of severe acute pancreatitis is 34/100,000, and it increases constantly. 1,2 Severe acute pancreatitis (SAP) is the common acute abdominal diseases. It refers to the dysfunction of one or more organs other than the pancreas for more than 48 hours. 3 It is characterized by rapid progression, development of multiple complications, and high mortality rates. Despite the recent advances in the treatment of SAP, the mortality rate due to multiple organ dysfunction syndrome (MODS), and secondary infections is still high (20%–40%). 4 Surviving patients suffer from various secondary diseases which affect their quality of life. For example, 40% of the patients experience an abnormal glucose tolerance or type 3 diabetes after the acute phase, 5 25% experience an impairment of the pancreatic exocrine function, 6 50% of the patients with necrotizing pancreatitis experienced mobility impairment one year after the onset, and 18% of patients experienced recurrence, while 8% developed chronic pancreatitis. 7 Blood purification (BP), is a non-surgical treatment method that is vital in the treatment of SAP patients. It can effectively lower the concentration of inflammatory mediators in pancreatitis patients, shorten hospital stay, and reduce mortality rate. 8–10 However, due to the differences in study design, enrollment population, BP parameters, as well as the lack of clinical trials that investigate the effects of BP on the long-term survival and quality of life of SAP patients, there is still much debate regarding the use of this technology in inflammatory diseases, particularly pancreatitis, and sepsis. Abstract:Background: To retrospectively analyze the effects of routine treatment and blood purification combined with antibiotics on the extravascular lung water index (EVLWI), inflammatory factors, and treatment outcomes in patients with severe acute pancreatitis (SAP) complicated with acute respiratory distress syndrome (ARDS). Methods: A total of 131 SAP patients admitted to the intensive care unit of Suzhou Ninth Hospital Affiliated to Soochow University from January 2019 to December 2020 were retrospectively enrolled in this study. Patients were divided into two groups according to the treatment methods. In addition to conventional treatment, 60 patients in group A received continuous blood purification (CBP) treatment and 71 patients in group B did not. The EVLWI, inflammatory factors, remission time of clinical symptoms, curative effect, and patient outcomes were recorded at admission and after 1, 3, 5, and 7 days of treatment. Results: There was a statistically significant difference in the clinical symptom relief time and the clinical efficacy between the two groups of patients (P<0.05). The mortality rate of patients in group A was 3.33%, which was significantly lower than the 14% mortality rate observed in patients in group B (P<0.05). The EVLWI, as well as the C-reactive protein (CRP), interleukin (IL)-6, IL-8, and tumor necrosis factor (TNF)-α levels were significantly lower in group A patients compared to group B patients on day 1, 3, and 7 post-treatment (P<0.05). There was also a significant difference in APACHE II scores between the two groups (P<0.05). The incidence of adverse reactions in group A was 6.7%, which was significantly lower than the 22.5% incidence observed in group B (P<0.05). Conclusions: Continuous blood purification combined with antibacterial drugs is safe and has a significant effect on the treatment of SAP patient complicated with ARDS, including effectively relieving clinical symptoms and signs, reducing the level of inflammatory factors, and promoting early disease outcomes.

Abstract:This study aimed to evaluate the therapeutic efficacy and effect of blood purification (BP) therapy on severe acute pancreatitis (SAP). Information on 305 patients (BP group 68, control group 237) diagnosed with SAP was retrieved from the Medical Information Mart for Intensive Care IV (MIMIC IV) database. Firstly, the influence of BP treatment was preliminarily evaluated by comparing the outcome indicators of the two groups. Secondly, multiple regression analysis was used to screen the mortality risk factors to verify the impact of BP on the survival outcome of patients. Then, the effect of BP treatment was re-validated with baseline data. Finally, cox regression was used to make the survival curve after matching to confirm whether BP could affect the death outcome. The results indicated that the BP group had a lower incidence of shock ( p = 0.012), but a higher incidence of acute kidney injury (AKI) (p < 0.001), with no differences observed in other outcome indicators when compared to the control group. It was also found that the 28-day survival curve of patients between the two groups was significantly overlapped ( p = 0.133), indicating that BP treatment had no significant effect on the survival outcome of patients with SAP. Although BP is beneficial in stabilizing hemodynamics, it has no effect on short- and long-term mortality of patients. The application of this technology in the treatment of SAP should be done with caution until appropriate BP treatment methods are developed, particularly for patients who are not able to adapt to renal replacement therapy.

Continuous Hemofiltration Reduces Mortality in Severe Acute Pancreatitis: A Meta-Analysis

Abstract:Background. Severe acute pancreatitis (SAP) is a deadly condition, with a mortality rate ranging from 15% to 30%. Recently, blood purification therapy has been adopted in administrating SAP patients. The present study aimed at evaluating the effect of continuous hemofiltration therapy for SAP. Methods. A systematic search of Cochrane Library, PubMed, and Embase was carried out until October 1st, 2019. Prospective studies comparing outcomes for SAP patients between continuous hemofiltration and standard therapy were enrolled. Results. Continuous hemofiltration therapy was associated with lower level of PACHE II score (MD = −1.49; 95% CI: −2.69 to −0.29, P=0.02), CRP (MD = −1.56 mg/L; 95% CI: −2.64 to −0.47, P=0.005), Cr (MD = −3.57 umol/L; 95% CI: −5.50 to −1.65, P=0.003), and Bun (MD = −3.63 mmol/L; 95% CI: −6.07 to −1.20, P=0.003) at 72 h after onset of treatment. Continuous hemofiltration therapy was associated with shorter length of abdominal pain relief time (MD = −1.82 hours; 95% CI: −2.93 to −0.71, P=0.001), lower surgery rate (OR = 0.15; 95% CI: 0.03 to 0.78, P=0.02), and mortality rate (OR = 0.54; 95% CI: 0.37 to 0.77, P=0.0007). Conclusions. continuous hemofiltration therapy could effectively alleviate SAP as early as 72 hours after onset of treatment, lowering the level of Bun, Cr, CRP, and APACHE II scores. Continuous hemofiltration therapy could confer SAP patients with lower mortality rates.
Endothelial Dysfunction in Patients with Severe Acute Pancreatitis: Improved by Continuous Blood Purification Therapy

Abstract:Objectives. Endothelium dysfunction is one of the critical pathophysiologic disorders in patients with severe acute pancreatitis (SAP). To investigate the effect of continuous blood purification (CBP) on endothelial function, we conducted a prospective study of 20 patients with SAP, 9 of whom had evidence of sepsis. Methods. All patients underwent CVVH for 72 h. Soluble E-selectin (sE-selectin), soluble thrombomodulin, permeability of the endothelial monolayer, and endothelial intracellular calcium ([Ca2+]i) levels were used as the markers for the assessment of endothelial function and the effect of CBP therapy in patients with SAP. Blood samples were taken from the patients at 0, 2, 12, 24, 48, and 72 h during CVVH therapy. sE-selectin and thrombomoduiln were measured by ELISA. The endothelial permeability and activation were evaluated using cultured endothelial monolayer and intracellular Ca2+ concentration. Results. The results showed that during CVVH treatment, the hemodynamics and mean arterial pressure (MAP) were stable. The Acute Physiology and Chronic Health Evaluation (APACHE) II score was improved significantly after CVVH. Endothelial dysfunction was evident in patients with SAP as compared to normal controls. Patients with SAP had increased levels of sE-selectin, endothelial permeability and intracellular [Ca2+]i, which was higher in patients with sepsis than in those without sepsis. The level of thrombomodulin showed a tendency to increase; however, these changes were not significant between SAP patients and controls. After CBP treatment, sE-selectin levels substantially decreased in all patients. CBP treatment also significantly diminished the endothelial permeability and decreased the intracellular [Ca2+] concentration. Conclusions. These data demonstrate that endothelial dysfunction is present in patients with SAP and the degree of endothelial damage may be correlated with the disease severity. CBP therapy can not only improve the general conditions, as measured by the APACHE II score, but also effectively improve endothelial dysfunction.

Abstract:Objective:To study the effects of continuous blood purification(CBP)in the treatment of 1 692 critical patients retrospectively and analyze the influencing factors of prognosis.Methodology:From Jan.1998 to Mar.2007,a total of 1 692 critical patients were treated with CBP in Jingling Hospital.The clinical data was collected:(1)General conditions:sex,age and original disease;(2)Clinical status prior to CBP:indications of CBP,APACHE Ⅱ score,SAPS Ⅱ score,number of organ dysfunction,complications,and the percentage of patients with vasoactive agents and mechanical ventilation;(3)Technique and method of CBP:modality,dosage,and vascular access of CBP,category of anticoagulant and membrane of hemofilter used in CBP;(4)Outcomes:the individual predicted risk of death was calculated form the SAPS Ⅱ scores of each patient.Results:(1)They were 1 088 males and 604 females,with an average of(46.9±19.6)years old(3.5-94,median 46).1 107 cases(65.43%)were admitted for medical reasons and 585(34.57%)for surgical reasons.(2)Three main indications of CBP were azotemia(60.46%),oliguria/anuria(39.07%)and refractory organ edema(especially lung)/nutritional support in patients with or at risk of pulmonary edema/ARDS(16.13%).(3)Continuous high-volume hemofiltration(HVHF)(66.55%)and continuous veno-venous hemofiltration(CVVH)(27.30%)were the main modality of CBP.A double-lumen catheter was used in most of patients through the internal jugular vein(64.72%)and femoral vein(30.91%).Polysulfone(78.72%)and polyacrylonitrile(15.77%)were the most common used membranes.57.21% of cases were anticoagulated with low molecular weight heparin(LMWH)combined with citrate acid,and 23.40% with LMWH only.(4)Except for 94 cases(5.56%)quitted for economic reasons,800 cases(47.28%)were cured and 458(27.07%)were improved.The total effective(cure and improve)rate was 74.6%,340 cased died,the mortality was 20.1%,which was lower than that the predicted in-hospital mortality of 29.5%.(5)The M/F ratio was much higher in the non-survivals,with an average of(55.8±21.1)years old vs(44.8±18.8)in the survivals(P0.01).The clinical condition of non-survivals were more severe before CBP:with higher APACHE Ⅱ scores and SAPS Ⅱ scores,more patients complicated with hypotension/shock,bleeding and infection/sepsis,more patients treated with vasoactive agents and mechanical ventilation,and more organ function were injured.Conclusion:The outcome of critical ill patients was significantly improved by CBP,the mortality was decreased.
Pulmonary Artery Catheter in Patients with Severe Acute Pancreatitis: A Single-Center Retrospective Study

Abstract:Objective: It is still uncertain what effects pulmonary artery catheter (PAC)-guided resuscitation has on outcomes for patients with severe acute pancreatitis (SAP). Therefore, we aimed to investigate the effect of PAC on hospital mortality in patients with SAP. Methods: We collected the data of patients with a diagnosis of SAP from January 10, 2017, to July 30, 2019. Patients were divided into a PAC group and a control group. The primary outcome measured was the day-28 mortality. Secondary outcomes included day-90 mortality, duration of ICU and hospital stay, ventilation days, usage of renal support and vasoactive agents, incidences of acute abdominal compartment syndrome, infusion volumes, and fluid balance and hemodynamic characteristics measured by the PAC. Kaplan-Meier analysis was applied to estimate survival outcomes. Complications related to PAC were also analyzed. Results: There was no significant difference between the PAC group and the control group for day-28 mortality (22.7% vs. 30%, odds ratio, 0.69; 95% CI 0.31-1.52; P = 0.35). The duration of ICU stay in the PAC group was shorter (P = 0.00), and the rate of dependence on renal support treatment was lower in the PAC group than in the control group (P = 0.03). There was no difference in other secondary outcomes and no significant difference in the survival curve between the two groups (log-rank P = 0.72, X2 = 0.13). However, SAP patients inserted PAC within 24 h ICU admission showed that duration of renal support therapy in PAC patients within 24 h ICU admission (mean days, 1.60; standard deviation, 0.14) was shorter than those with 24-72 h ICU admission (mean days, 2.94; standard deviation, 0.73; P = 0.03). The organ failure rates (1 organ, 2 organs and 3 organs) were all lower in PAC patients within 24 h ICU admission than with 24-72 h ICU admission (P = 0.02, P = 0.02, P = 0.048, respectively). Conclusion: In patients with severe acute pancreatitis, PAC-guided fluid resuscitation shortened the duration of ICU stay, and patients in the PAC group had a lower rate of dependence on renal support, while no benefit in terms of mortality was observed. However, SAP patients inserted PAC within 24 h ICU admission showed shorter duration of renal support therapy and lower organ failure rates than those with 24-72 h ICU admission, indicating that early use of PAC, especially within 24 h, might be better for SAP patients.
THE EFFECT OF PERCUTANEOUS CATHETER DRAINAGE COMBINED WITH SOMATOSTATIN ON INFLAMMATION AND PLASMA THROMBOXANE 2, PROSTACYCLIN I2 LEVELS IN PATIENTS WITH SEVERE PANCREATITIS

Abstract:Objective: Explore the effectiveness of ultrasound-guided percutaneous catheter drainage (PCD) combined with somatostatin in the treatment of severe pancreatitis (SAP) patients. Methods: A retrospective study method was adopted to select 95 patients with SAP who were treated in our hospital from January 2018 to June 2022 for clinical research. Among them, 48 patients received routine treatment+somatostatin (control group), and other 47 patients received ultrasound guided PCD treatment on the basis of the control group (research group). The differences in the peripheral white blood cells (WBC), procalcitonin (PCT), interleukin-6 (IL-6), and Tumor Necrosis Factor- α(TNF- α), prostacyclin I2 (PGI2), plasma thromboxane 2 (TXA2), blood amylase, serum albumin (ALB), acute physiological function and chronic health score (APACHE II), sequential organ failure score (SOFA), and clinical efficacy were compared. The ICU treatment time, hospital stay, and incidence of complications were also recorded for the two groups of patients. Results: Before treatment, there was no statistically significant difference in APACHE II score and SOFA score between the research group and the control group (P>0.05); The APACHE II scores of the research group after 14 days of treatment and 28 days of treatment were lower than those of the control group. The SOFA scores of the research group after 28 days of treatment were lower than those of the control group, and the differences were statistically significant (P<0.05); Before treatment, there was no statistically significant difference in WBC, PCT, IL-6, TNF-α, PGI2, TXA2, blood amylase, and ALB levels between the research group and control group (P>0.05); PCT, IL-6, TNF-α, TXA2 and blood amylase levels in the research group after 28 days of treatment were lower than that in the control group, and the differences were statistically significant (P<0.05); The ICU treatment time and hospitalization time of the research group were lower than those of the control group, and the differences were statistically significant (P<0.05); After 28 days of treatment, clinical efficacy evaluation was conducted, and the overall efficacy of the research group patients was better than that of the control group, with statistically significant differences (P<0.05); The complication rate of the research group was 27.66%, and that of the control group was 47.92%, which was significantly lower in the research group than in the control group (P<0.05). Conclusion: Ultrasound guided PCD combined with somatostatin treatment for SAP patients can more effectively alleviate the degree of inflammatory response, effectively alleviate the severity of the patient's condition, reduce the occurrence of related complications, and improve clinical treatment effectiveness.

To evaluate the efficacy of continuous high-volume hemofiltration for the treatment of severe acute pancreatitis patients and the impact of acute physiology and chronic health evaluation (APACHE) II score on the efficacy of high-volume hemofiltration. Material and methods. A total of 63 patients diagnosed with severe acute pancreatitis between January 2005 and July 2007 were retrospectively analyzed: 34 accepted adjunctive continuous high-volume hemofiltration (HVHF group); and 29 only accepted conventional recommended treatments (control group). Results. There were no differences in physiological characteristics between the two groups when entering the intensive care unit. After treatment, the percentages of patients successfully weaned from mechanical ventilation (p = 0.004) and who experienced renal function recovery (p = 0.046) were significantly higher in the HVHF group than in the control group. The 28-day survival rate was 91.2% in the HVHF group, compared with 65.5% in the control group (p = 0.014). For patients with APACHE II scores > 15, survival was significant higher in the HVHF group than in the control group (87.5% vs 50%; p = 0.044). No difference in survival was found in patients with APACHE II scores <= 15 between the two groups (94.4% vs 76.5%; p = 0.177). After HVHF therapy, APACHE II score, body temperature, urine volume and laboratory indices, including serum creatinine, base excess and blood calcium, were significantly improved. Conclusions. Adjunctive continuous HVHF was beneficial for the survival of severe acute pancreatitis patients. Survival improvement was significant in patients with APACHE II score > 15.

Abstract:Objective:To investigate the therapeutic effects of extended daily hemofiltration(EDHF) on severe acute pancreatitis(SAP).Methods:The clinical data of 62 patients with SAP were retrospectively reviewed,including life vital signs,biochemical indexes and prognosis within 72 h.30 cases with an average acute physiology and chronic health evaluation II(APACHE II) score of(16.3±4.3) underwent EDHF on the basis of traditional treatment;another 32 cases with an average APACHE II score of(15.2±3.6) were treated traditionally.Results: From the beginning to 72 h after treatment,the vital signs,the biochemical indexes(such as body temperature,heart rate) and the biochemical indexes(such as C-response protein,creatinine,blood albumim,abdominal pain and abdominal distension) were improved more obviously in patients who received EDHF treatment than those who only received traditional treatment.Patients treated with EDHF needed shorter hospitalization time than patients treated conventionally[(12.3±7.1) d vs.(19.2±8.4) d,P<0.05].There was no significant difference in mortality rate between EDHF treatment group and conventional treatment group(16.7%vs.28.1%,P>0.05).Conclusions: EDHF treatment can improve the symptoms of SAP patients,such as body temperature and heart rate;and it can also improve some of the biochemical indexes,such as C-response protein,abdominal pain and abdominal distension.Moreover,EDHF can shorten hospitalization time,but EDHF has no significant effects on mortality rate.
Effect of Baicalin on Inflammatory Mediator Levels and Microcirculation Disturbance in Rats with Severe Acute Pancreatitis

Abstract:Abstract Objective: To investigate the effect of Bacailin on inflammatory mediator levels and microcirculation disturbance in severe acute pancreatitis (SAP) rats and explore its therapeutic mechanism on this disease. Methods: SAP model rats were randomly divided into model control group and Baicalin treated group, 45 rats in each group. The same number of normal rats were included in sham-operated group. These groups were further subdivided into 3 h, 6 h and 12 h subgroups, respectively (15 rats in each subgroup). At 3, 6 and 12 hours after operation, rats were killed to conduct the following experiments: (1) to examine the mortality rates of rats, the ascites volume and pancreatic pathological changes in each group; (2) to determine the contents of amylase, PLA~2~, TXB~2~, PGE~2~, PAF and IL-1[beta]; in blood as well as the changes in blood viscosity. Results: (1) Compared to model control group, treatment with Baicalin is able to improve the pathological damage of the pancreas, lower the contents of amylase and multiple inflammatory mediators in blood, decrease the amount of ascitic fluid and reduce the mortality rates of SAP rats; (2) at 3 hours after operation, the low-shear whole blood viscosity in Baicalin treated group was significantly lower than that in model control group;at 12 hours after operation, both the high-shear and low-shear whole blood viscosity in Baicalin treated group were also significantly lower than those in model control group. Conclusion: Baicalin, as a new drug, has good prospects in the treatment of SAP since it can exert therapeutic effects on this disease through inhibiting the production of inflammatory mediators, lowering blood viscosity, improving microcirculation and mitigating the pathological damage of the pancreas.

Abstract:BACKGROUND Non-surgical methods such as percutaneous drainage are crucial for the treatment of patients with severe acute pancreatitis (SAP). However, there is still an ongoing debate regarding the optimal timing for abdominal paracentesis catheter placement and drainage. AIM To explore the influence of different timing for abdominal paracentesis catheter placement and drainage in SAP complicated by intra-abdominal fluid accumulation. METHODS Using a retrospective approach, 184 cases of SAP complicated by intra-abdominal fluid accumulation were enrolled and categorized into three groups based on the timing of catheter placement: group A (catheter placement within 2 d of symptom onset, n = 89), group B (catheter placement between days 3 and 5 after symptom onset, n = 55), and group C (catheter placement between days 6 and 7 after symptom onset, n = 40). The differences in progression rate, mortality rate, and the number of cases with organ dysfunction were compared among the three groups. RESULTS The progression rate of group A was significantly lower than those in groups B and groups C (2.25% vs 21.82% and 32.50%, P < 0.05). Further, the proportion of patients with at least one organ dysfunction in group A was significantly lower than those in groups B and groups C (41.57% vs 70.91% and 75.00%, P 0.05). At postoperative day 3, the levels of C-reactive protein (55.41 ± 19.32 mg/L vs 82.25 ± 20.41 mg/L and 88.65 ± 19.14 mg/L, P < 0.05), procalcitonin (1.36 ± 0.51 ng/mL vs 3.20 ± 0.97 ng/mL and 3.41 ± 0.98 ng/mL, P < 0.05), tumor necrosis factor-alpha (15.12 ± 6.63 pg/L vs 22.26 ± 9.96 pg/L and 23.39 ± 9.12 pg/L, P < 0.05), interleukin-6 (332.14 ± 90.16 ng/L vs 412.20 ± 88.50 ng/L and 420.08 ± 87.65ng/L, P < 0.05), interleukin-8 (415.54 ± 68.43 ng/L vs 505.80 ± 66.90 ng/L and 510.43 ± 68.23ng/L, P < 0.05) and serum amyloid A (270.06 ± 78.49 mg/L vs 344.41 ± 81.96 mg/L and 350.60 ± 80.42 mg/L, P < 0.05) were significantly lower in group A compared to those in groups B and group C. The length of hospital stay in group A was significantly lower than those in groups B and group C (24.50 ± 4.16 d vs 35.54 ± 6.62 d and 38.89 ± 7.10 d, P < 0.05). The hospitalization expenses in group A were also significantly lower than those in groups B and groups C [2.70 (1.20, 3.55) ten-thousand-yuan vs 5.50 (2.98, 7.12) ten-thousand-yuan and 6.00 (3.10, 8.05) ten-thousand-yuan, P < 0.05). The incidence of complications in group A was markedly lower than that in group C (5.62% vs 25.00%, P 0.05). CONCLUSION Percutaneous catheter drainage for the treatment of SAP complicated by intra-abdominal fluid accumulation is most effective when performed within 2 d of onset.

Abstract:The aim of the study was to explore the CT images of the iterative reconstruction algorithm to evaluate the curative effect of continuous blood purification combined with nursing intervention in the treatment of severe acute pancreatitis (SAP). A total of 100 patients with SAP treated by the bedside continuous venous hemofiltration purification method in a hospital were selected. The control group (n = 50) was given a routine treatment, and the observation group (n = 50) was treated with the continuous blood filtration mode for blood purification based on the routine treatment. In the CT image scanning of periodontitis patients, the iterative reconstruction algorithm was introduced to reduce image noise, and the CT values under the algorithm were statistically analyzed. The results showed that IL-1, IL-6, and IL-8 after treatment were significantly lower than those before treatment ( P < 0.05 ). The symptoms effectively improved with continuous blood purification combined with nursing intervention in patients with SAP. After the use of the iterative reconstruction algorithm, the image quality, image information, and image MSE significantly improved. The image noise with 50% dose reduction was the lowest, but the reconstruction algorithm improved the low contrast resolution ( P < 0.05 ). CT images based on the reconstruction algorithm can clearly display the lesion characteristics of the patients, and the reconstruction algorithm is feasible to improve the spatial resolution of CT images.

Abstract:BACKGROUND: To investigate the effects of Baicalin and Octreotide on inflammatory mediators and pancreatic acinar cells apoptosis of rats with severe acute pancreatitis (SAP).METHODS: SD rats were randomly divided into sham operated group (I group), model control group (II group), Baicalin treated group (III group) and Octreotide treated group (IV group). Each group was also divided into subgroup of 3, 6 and 12 h (n = 15). The mortality rate, ascites/body weight ratio as well as the level of endotoxin, NO and ET-I in blood were measured. The pathological severity score of pancreas, apoptotic indexes, and expression levels of Bax and Bcl-2 proteins in each group were investigated.RESULTS: The survival rate of III and IV group has a significant difference compared with II group (P-12 h < 0.05). The ascites volume, contents of inflammatory mediators in blood and pathological severity score of pancreas of III and TV group declined at different degrees compared to II group (P < 0.055 P < 0.01 or P < 0.001). Apoptotic index in III group was significantly higher than that in II group at 3 and 6 h (P-3,P- 6 h < 0.05). Apoptotic index in TV group was significantly higher than that in II group at pancreatic tail at 6 h (P-6 h < 0.05). Expression level of Bax in III group was significantly higher than that in II group (pancreatic head P-3 h,P-6 h < 0.01, pancreatic tail P-3 h < 0.0001).CONCLUSIONS: Compared with Octreotide in the treatment of SAP, the protective mechanisms of Baicalin include reducing the excessive inflammatory mediators' release, inducing the pancreatic acinar cells apoptosis.
Serum Cystatin C Level Monitoring for Intervention Opportunity of CBP in Children with Severe Sepsis

Abstract:Objective: The aim of this study is to investigate the instruction value of the serum cystatin C (Cys C) level monitoring for intervention opportunity of continuous blood purification technology (CBP) in children with severe sepsis. Methods: 67 children with severe sepsis in the pediatric intensive care unit (PICU) with CBP treatment were retrospectively selected from May 2016 to April 2020. According to the time intervals between the time point of serum Cys C level began to increase (>15 mg/L) and the time point of CBP began, all children were divided into group A (<24 h, 29 cases), group B (24-48 h, 22 cases), and group C (>48 h, 16 cases). The children's general characteristics, vital signs, biochemical parameters, acute physiology and chronic health evaluation (APACHE II), and sequential organ failure assessment (SOFA) scores were evaluated. The influence factors of prognosis of children with severe sepsis were analyzed by multivariate regression analysis. Results: The intervals between the time point of PICU hospitalization and the time point of CBP began and the times of CBP in group A were significantly more than those in group B and C (P < 0.05). There was no statistically significant duration of CBP among three groups (P > 0.05). After follow-up of 28 d, there was no significant difference on the occurrence of coagulation disorders and hypovolemic shock induced by CBP among three groups (P > 0.05). However, the mortality of children in group A was lower than that in group C (P < 0.05). Children in group A had lower APACHE II scores, SOFA scores, serum K+, blood urea nitrogen (BUN), serum creatine (SCr), partial pressure of carbon dioxide (PCO2), and higher partial pressure of oxygen (PO2) than those of children in group C after CBP. (P < 0.05). SOFA scores ≥5 after CBP treatment and the time intervals between the time point of serum Cys C level began to increase (>15 mg/L) and the time point of CBP began ≥24 h were the independent influence factors on the prognosis by multivariate regression analysis. Conclusion: There are significant evidences that continuous blood purification technology within 24 h of serum Cys C level may better control the condition of children with severe sepsis.
Treatment of Early Phase Severe Acute Pancreatitis in Intensive Care Units: a Retrospective Multicenter Study

Abstract:Objective To analyse the experience and treatment of early phase severe acute pancreatitis (SAP) in intensive care units (ICU).Methods A multicenter retrospective study was done on patients with SAP treated in three major teaching hospitals (Beijing Hospital,Peking University First Hospital and Peking University Shenzhen Hospital) in China from Jan.2001 to Dec.2011.Results There were 188 patients who were enrolled in the study,including 121 males and 67 females.The age ranged from 19 to 104 (51.0±18.2) years.The mean APACHE Ⅱ score was (22.2±4.6).84.0％ of patients survived,the mortality was 10.1％ in the early phase and 5.9％ in the late phase.The most common systemic complications were acute renal injury (46.3 ％),acute respiratory distress syndrome (35.6％),and septic shock (17.6％).The local complication rate was 47.3％,which included acute peripancreatic fluid collections (32.8％),acute necrotic collection and walled-off necrosis (48.4 ％) and pseudocyst (18.8 ％).The conservative treatments included intensive care,fluid resuscitation,mechanical ventilation,continuous renal replacement therapy,antibiotics,glucose control,inhibition of pancreatic enzyme activity and secretion,and nutritional support.Surgical intervention included endoscopic retrospective cholangio-pancreatography and endoscopic sphincterectomy,B ultrasound or CT guided puncture and drainage,and surgical drainage and debridement of necrosis.Conclusions The early phase of SAP was characterized by systemic inflammatory response syndrome and multiple organ dysfunction syndrome which accounted for the first peak in mortality.Intensive care therapy and multi disciplinary comprehensive combined strategy were very important for these patients with systemic and local complications.ICU treatment in the early phase was preferred for patients with SAP.
Early Ultrasound-Guided Percutaneous Catheter Drainage in the Treatment of Severe Acute Pancreatitis with Acute Fluid Accumulation

Abstract:The clinical effect of early percutaneous ultrasound guided percutaneous catheter drainage (PCD) in treating severe acute pancreatitis complicated with acute fluid accumulation in the abdominal cavity was analyzed. A total of 178 patients with severe acute pancreatitis complicated with acute fluid accumulation in peritoneal cavity admitted from January, 2011 to January, 2015 to Chuiyangliu Hospital were retrospectively analyzed. Based on the treatment, patients were divided into the following groups: PCD group and conservative treatment control group. Time-period of systemic inflammatory response (SIRS), time-period of abdominal pain, bowel sounds recovery time, dietary recovery time, hospitalization days, white blood cell count, serum amylase, C-reactive protein, serum calcium and complications in both groups were observed and compared. The measurement data between the two groups were presented as mean ± standard deviation (±SD), and analyzed by t-test. Classification data were analyzed by the Chi-square test, with P<0.05 indicating a statistically significant difference. Time-period of systemic inflammatory response (SIRS), time-period of abdominal pain, bowel sounds recovery time, dietary recovery time and hospitalization days were shorter in the PCD group than those in the control group (P=0.001). Improvements of white blood cell count, serum amylase, C-reactive protein and serum calcium were better than those of the control group (P<0.001), the rate of transferring to surgical department in the PCD group was lower than that of the control group (P=0.042), and complications of severe acute pancreatitis were not significantly different in the two groups (P>0.05). In this study, 6 adverse events occurred in the PCD group, accounting for 7.9% (6/76), including 1 case of puncture bleeding and 5 cases of obstruction. In conclusion, early ultrasound-guided PCD in treating severe acute pancreatitis is effective and safe.

Abstract:OBJECTIVE:To investigate the feasibility and clinical value of the step-up approach for severe acute pancreatitis (SAP).METHODS:Clinical data of 121 SAP patients admitted between January 2002 and December 2011 were retrospectively analyzed. Fifty-eight patients (37 males and 21 females, aged from 20 to 72 years, mean 47.6 years) in the group of direct open necrosectomy from January 2002 to December 2006 were performed laparotomy through removal of all necrotic tissue. Sixty-three patients (42 males and 21 females, aged from 19 to 78 years, mean 46.2 years) of step-up approach from January 2007 to December 2011 underwent percutaneous catheter drainage through retroperitoneum or omental bursa guided by B-type ultrasonography for the first therapy, and then, according to the pathogenetic condition, if necessary, followed by a small incisional necrosectomy along the drainage tube. The two groups were compared for the rates of postoperative complications, death, transfusion and length of stay, medical costs.RESULTS:The rates of total postoperative complications, organ dysfunction, alimentary tract fistula and incisional hernia in step-up approach group were significantly lower than those of direct open necrosectomy group (31.7% vs. 62.1%, 14.3% vs. 37.5%, 6.3% vs. 19.0%, 9.5% vs. 29.3%; χ(2) = 4.43 to 11.17, P = 0.001 to 0.035). The other complications had no significant differences between the two groups (P > 0.05). Patients in step-up approach group had a lower rates of transfusion (44.4% vs. 70.7%, χ(2) = 8.488, P = 0.004), fewer medical costs of transfusion and hospital stay, compared with those in direct open necrosectomy group ((2525 ± 4573) yuan vs. (4770 ± 6867) yuan, t = 2.131, P = 0.035; (171 213 ± 50 917) yuan vs. (237 874 ± 67 832) yuan, t = 2.496, P = 0.014). There were no significant differences of length of stay and mortality between two groups (P > 0.05).CONCLUSION:Step-up approach for SAP which can reduce the rates of postoperative complications, transfusion and medical costs has significant feasibility and great clinical value.
