Abstract:Programmed cell death receptor 1 ligand 1 (PD-L1) expression in various tumors, including hematologic malignancies, has recently become a research topic of great interest. We performed a meta-analysis to evaluate the prognostic and clinicopathological value of PD-L1 expressed in tumor cells of patients with diffuse large B-cell lymphoma (DLBCL). Relevant studies were identified from PubMed, EMBASE, Web of Science and the Cochrane Library. The hazard ratio (HR) and 95% confidence interval (95% CI) were used for analyzing survival outcomes, and the odds ratio (OR) was used for analyzing clinicopathological parameters. Pooled results showed that tumor cell PD-L1 expression is associated with poor overall survival (OS) (HR = 2.128, 95% CI: 1.341–3.378, P = 0.001), the non-germinal center B-cell-like subtype (OR = 2.891, 95% CI: 2.087–4.003, P < 0.000), high international prognostic index score (3–5) (OR = 1.552, 95% CI: 1.111–2.169, P = 0.010), B symptoms (OR = 1.495, 95% Cl: 1.109–2.015, P = 0.008), positive MUM1 expression (OR = 3.365, 95% Cl: 1.578–7.175, P = 0.002) and negative BCL6 expression (OR = 0.414, 95% Cl: 0.217–0.792, P = 0.008). Sensitivity analysis showed that there was no publication bias among these studies. Our meta-analysis supported the idea that tumor cell PD-L1 expression may represent a promising biomarker for predicting poor prognosis and is associated with adverse clinicopathologic features in DLBCL patients.
Serum levels of soluble programmed death ligand 1 predict treatment response and progression free survival in multiple myeloma

Abstract:Immune checkpoint signaling plays an important role in immunosuppression in multiple myeloma (MM). Blood levels of soluble programmed death-ligand 1 (sPD-L1), a checkpoint-relevant protein, might predict treatment response and survival outcomes in MM patients. We used an enzyme-linked immunosorbent assay to measure serum sPD-L1 levels in 81 newly diagnosed MM patients. We found that myeloma patients had higher sPD-L1 concentrations than healthy controls. The best sPD-L1 cutoff value for predicting disease progression risk was 2.783 ng/mL. The overall response rate to treatment was higher in low sPD-L1 patients than in high sPD-L1 patients. The 3-year progression free survival (PFS) and overall survival (OS) rates for all patients were 16% and 64%, respectively. Multivariate survival analysis including Eastern Cooperative Oncology Group performance status score, treatment response, and sPD-L1 level showed that a less than partial treatment response (PR) and higher sPD-L1 levels (>2.783 ng/ml) were independent prognostic factors for shorter PFS; neither factor was predictive of OS. The serum sPD-L1 level is a valuable biomarker for predicting treatment response and an independent prognostic factor for PFS. PD-1/ PD-L1 blockade may be a promising novel immune-based therapeutic strategy in MM.

Abstract:Abstract Circulating soluble programmed death-1 ligand (sPD-L1) is measurable in the serum of cancer patients. This study aimed to investigate the significance of sPD-L1 in cancer patients receiving immune checkpoint inhibitor therapy. Blood samples were obtained before and after immune checkpoint inhibitor therapy (January 2015 to January 2019). The study cohort consisted of 128 patients who were diagnosed with non-small cell lung cancer (n = 50), melanoma (n = 31), small cell lung cancer (n = 14), urothelial carcinoma (n = 13), and other cancers (n = 20). Patients with a high level (> 11.0 pg/μL) of sPD-L1 were more likely to exhibit progressive disease compared with those with a low level (41.8% versus 20.7%, p = 0.013). High sPD-L1 was also associated with worse prognosis; the median PFS was 2.9 (95% confidence interval [CI] 2.1–3.7) months versus 6.3 (95% CI 3.0–9.6) months (p = 0.023), and the median OS was 7.4 (95% CI 6.3–8.5) months versus 13.3 (95% CI 9.2–17.4) months (p = 0.005). In the multivariate analyses, high sPD-L1 was an independent prognostic factor for both decreased PFS (HR 1.928, p = 0.038) and OS (HR 1.788, p = 0.004). sPD-L1 levels did not correlate with tissue PD-L1 expression. However, sPD-L1 levels were positively correlated with neutrophil to lymphocyte ratios and negatively correlated with both the proportion and the total number of lymphocytes. We found that high pretreatment sPD-L1 levels were associated with progressive disease and were an independent prognostic factor predicting lower PFS and OS in these patients.

Abstract:Hypomethylating agents (HMAs) are widely used in patients with higher-risk MDS not eligible for stem cell transplantation. However, the general response rate by HMAs is lesser than 50% in MDS patients, while the relapse rate is high. Emerging evidence indicates that demethylating effects committed by HMAs may facilitate the up-regulation of a range of immune checkpoints or cancer suppressor genes in patients with MDS, among which the programmed death protein 1 (PD-1) and its ligands are demonstrated to be prominent and may contribute to treatment failure and early relapse. Although results from preliminary studies with a limited number of enrolled patients indicate that combined administration of PD-1 inhibitor may yield extra therapeutic benefit in some MDS patients, identifications of this subgroup of patients and optimal timing for the anti-PD-1 intervention remain significant challenges. Dynamics of immune checkpoints and associated predictive values during HMA-treatment cycles remained poorly investigated. In this present study, expression levels of immune checkpoints PD-1 and its ligands PD-L1 and PD-L2 were retrospectively analyzed by quantitative PCR (Q-PCR) in a total of 135 myelodysplastic syndromes (MDS) cohort with higher-risk stratification. The prognostic value of dynamics of these immune checkpoints during HMA cycles was validated in two independent prospective cohorts in our center (NCT01599325 and NCT01751867). Our data revealed that PD-1 expression was significantly higher than that in younger MDS patients (age ≤ 60) and MDS with lower IPSS risk stratification (intermediate risk-1). A significantly up-regulated expression of PD-1 was seen during the first four HMA treatment cycles in MDS patients, while similar observation was not seen concerning the expression of PD-L1 or PD-L2. By utilizing binary logistic regression and receiver operating characteristic (ROC) models, we further identified that higher or equal to 75.9 PD-1 expressions after 2 cycles of HMA treatment is an independent negative prognostic factor in predicting acute myeloid leukemia (AML) transformation and survival. Collectively, our data provide rationales for monitoring the expression of PD-1 during HMA treatment cycles, a higher than 75.9 PD-1 expression may identify patients who will potentially benefit from the combined therapy of HMA and PD-1 inhibitors.

Abstract:Abstract Myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML) are clonal hematopoietic stem cell diseases arising from the bone marrow (BM), and approximately 30% of MDS eventually progress to AML, associated with increasingly aggressive neoplastic hematopoietic clones and poor survival. Dysregulated immune microenvironment has been recognized as a key pathogenic driver of MDS and AML, causing high rate of intramedullary apoptosis in lower-risk MDS to immunosuppression in higher-risk MDS and AML. Immune checkpoint molecules, including programmed cell death-1 (PD-1) and programmed cell death ligand-1 (PD-L1), play important roles in oncogenesis by maintaining an immunosuppressive tumor microenvironment. Recently, both molecules have been examined in MDS and AML. Abnormal inflammatory signaling, genetic and/or epigenetic alterations, interactions between cells, and treatment of patients all have been involved in dysregulating PD-1/PD-L1 signaling in these two diseases. Furthermore, with the PD-1/PD-L1 pathway activated in immune microenvironment, the milieu of BM shift to immunosuppressive, contributing to a clonal evolution of blasts. Nevertheless, numerous preclinical studies have suggested a potential response of patients to PD-1/PD-L1 blocker. Current clinical trials employing these drugs in MDS and AML have reported mixed clinical responses. In this paper, we focus on the recent preclinical advances of the PD-1/PD-L1 signaling in MDS and AML, and available and ongoing outcomes of PD-1/PD-L1 inhibitor in patients. We also discuss the novel PD-1/PD-L1 blocker-based immunotherapeutic strategies and challenges, including identifying reliable biomarkers, determining settings, and exploring optimal combination therapies.

Abstract:Abstract This work investigated the clinical prognostic implications and biological function of plasma soluble programmed cell death ligand 1 in breast cancer patients. Plasma sPD-L1 levels of recurrent/metastatic breast cancer patients were determined, and the association of sPD-L1 levels and metastatic progression-free survival and metastatic overall survival was assessed. The PD-L1 expression on breast cancer cells was analyzed by flow cytometry, and the level of sPD-L1 in the supernatant of breast cancer cells was determined by enzyme-linked immunosorbent assay. Furthermore, the effect of sPD-L1 on the proliferation and apoptosis of T lymphocytes was detected by WST-1 assay and flow cytometry. The plasma sPD-L1 levels in 208 patients with recurrent/metastatic breast cancer before receiving first-line rescue therapy were measured. The optimal cutoff value of plasma sPD-L1 for predicting disease progression was 8.774 ng/ml. Univariate and multivariate analyses identified high sPD-L1 level (≥ 8.774 ng/ml) and visceral metastasis were independent factors associated with poor prognosis. Relevance analysis showed that the plasma sPD-L1 level was weakly associated with some systemic inflammation markers, including white cell count (WBC), absolute monocyte count, and absolute neutrophil count. Furthermore , we found sPD-L1 could be found in supernatant of culture with breast cancer cell line expressing PD-L1 on the cell surface and inhibit T lymphocyte function, playing a negative regulatory role in cellular immunity. sPD-L1 was a good tumor predictive maker in breast cancer and it may play a potentially important role in immune tolerance.

Abstract:The immune checkpoint pathway consisting of the cell membrane-bound molecule programmed death protein 1 (PD-1) and its ligand PD-L1 has been found to mediate negative regulatory signals that effectively inhibit T-cell proliferation and function and impair antitumor immune responses. Considerable evidence suggests that the PD-1/PD-L1 pathway is responsible for tumor immune tolerance and immune escape. Blockage of this pathway has been found to reverse T lymphocyte depletion and restore antitumor immunity. Antagonists targeting this pathway have shown significant clinical activity in specific cancer types. Although originally identified as membrane-type molecules, several other forms of PD-1/PD-L1 have been detected in the blood of cancer patients, including soluble PD-1/PD-L1 (sPD-1/sPD-L1) and exosomal PD-L1 (exoPD-L1), increasing the composition and functional complications of the PD-1/PD-L1 signaling pathway. For example, sPD-1 has been shown to block the PD-1/PD-L immunosuppressive pathway by binding to PD-L1 and PD-L2, whereas the role of sPD-L1 and its mechanism of action in cancer remain unclear. In addition, many studies have investigated the roles of exoPD-L1 in immunosuppression, as a biomarker for tumor progression and as a predictive biomarker for response to immunotherapy. This review describes the molecular mechanisms underlying the generation of sPD-1/sPD-L1 and exoPD-L1, along with their biological activities and methods of detection. In addition, this review discusses the clinical importance of sPD-1/sPD-L1 and exoPD-L1 in cancer, including their predictive and prognostic roles and the effects of treatments that target these molecules.

Abstract:Programmed death protein 1 (PD-1) interaction with PD-L1 deliver immunosuppressive environment for tumor growth, and its blockade with directed monoclonal antibodies (anti-PD-1/anti-PD-L1) has shown remarkable clinical outcome. Lately, their soluble counterparts, sPD-1 and sPD-L1, have been detected in plasma, and elevated levels have been associated with advanced disease, clinical stages, and worst prognosis for cancer patients. Elevated plasma levels of sPD-L1 have been correlated with worst prognosis in several studies and has displayed a persistent outlook. On the other hand, sPD-1 levels have been inconsistent in their predictive and prognostic ability. Pretherapeutic higher sPD-1 plasma levels have shown to predict advanced disease state and to a lesser extent worst prognosis. Any increase in sPD-1 plasma level post therapeutically have been correlated with improved survival for various cancers. <i>In vitro</i> and <i>in vivo</i> studies have shown sPD-1 ability to bind PD-L1 and PD-L2 and block PD-1/PD-L1 interaction. Local delivery of sPD-1 in cancer tumor microenvironment through local gene therapy have demonstrated an increase in tumor specific CD8+ T cell immunity and tumor growth reduction. It had also exhibited enhancement of T cell immunity induced by vaccination and other gene therapeutic agents. Furthermore, it may also lessen the inhibitory effect of circulating sPD-L1 and enhance the effects of mAb-based immunotherapy. In this review, we highlight various aspects of sPD-1 role in cancer prediction, prognosis, and anti-cancer immunity, as well as, its therapeutic value for local gene therapy or systemic immunotherapy in blocking the PD-1 and PD-L1 checkpoint interactions.

Abstract:Objective: Myelodysplastic Syndrome (MDS) is a special myeloid neoplasm with still limited treatment choices and poor survival. Although current therapies including HMAs have been shown to prolong survival, only 40-50% of patients respond to therapy, and responses are universally transient, with loss of response within 2 years. With the increasing understanding of the pathogenesis of MDS in recent years, MDS was found closely associated with the bone marrow microenvironment, including immune microenvironment (IME). More and more evidences show that the immune microenvironment of MDS is related with its disease characteristics and prognosis. Knowing more of the IME of MDS may facilitate the diagnose and treatment on MDS. However, till now little was known about the bone marrow immune microenvironment of MDS. Previous studies mostly applied multiparameter flow cytometry and based on the PB samples or frozen bone marrow fluid samples, less was known about the immune landscape of MDS bone marrow biopsies. Methods: We use multispectral imaging of multiplex immunofluorescence (mIF) to analyze the bone marrow biopsy slides and explore the bone marrow immune microenvironment of MDS patients. A total of 20 de novo MDS patients, 20 post-treatment MDS patients and 20 normal controls were included in this study. In HMAs treated group, 10 patients were response to HMA and another 10 patients were not response. HMAs were either decitabine with a dose at 20mg/m2 d1-5 or 5-Azacytidine with a dose at 75mg/m2 d1-7. The multiplex staining panel could differentiate the distinct cell subpopulations into CD34+CD117+ cells, CD8+ cells, PD1+ cells, CTLA4+ cells, CD8+PD1+ cells and CD8+CTLA4+ cells, CD8-PD1+ cells and CD8-CTLA4+ cells, respectively. We analyzed the proportion of these cells in MDS patients and normal controls, and analyzed the proportion of these cells in MDS patients with different IPSS risk stratifications, and we also assessed the degree of myelofibrosis in MDS patients and analyzed the relationship between myelofibrosis and immune cells. Results: We found that compared with the normal control, the proportion of CD8+ cells in de novo MDS patients increased, and CD8+PD1+ T cells increased too, but the CD8+CTLA4+ T cells decreased. Compared with the lower-risk groups, CD8+ T cells as well as CD8+PD1+ T cells increased in higher-risk groups, while CD8+CTLA4+ T cells decreased. We also found a higher CD8+ T cells as well as CD8+/PD1+ T cells in MDS-F patients, especially in MDS-F patients with higher IPSS risk levels. We also found that compared with the normal control, the proportion of CD8+ T cells per CD34+CD117+ cell reduced, and was lower in IPSS int-2 risk/high-risk group than that in low-risk/int-1 risk group. But the proportion of CD8+PD1+/CD34+CD117+ increased compared with normal control. To analyze the bone marrow immune microenvironment after HMAs treatment, CD8+PD1+ T cells decreased in patients who are effective to HMAs, but still remain high in patients who had no response to HMAs, so did the proportion of CD8+PD1+/CD8+ and CD8+PD1+/CD34+CD117+. However, such findings were not found on CD8+/CTLA4+ T cells. Conclusions: Our research findings provide a new insight on the bone marrow IME of MDS. The results confirmed that the immune checkpoint molecule PD-1 positive CD8+ T cells are abundant in MDS IME and associated with the risk. However, CTLA4 may not be a major molecule interfere with the IME of MDS. Our result also suggest that PD-1 inhibitor combines with HMAs might be used as a first-line therapy for de novo MDS patients to cover those patients who may not have response to HMAs. [Expression and Significance of PD-1/PD-L1 in MDS Blast Cells, T Lymphocyte Cell Subsets and Treg Cells]

Abstract:Objective: To investigate the expression and significance of PD-1/PD-L1 in MDS blast cells, T lymphocyte cell subsets and Treg cells. Methods: Eighty-eight MDS patients and 19 AML patients were collectd as the study subjects, and Iron deficiency anemia and healthy bone marrow donors were used as control group. The expression of PD-1/PD-L1 in MDS/AML blast cells, T lymphocyte cell subsets and Treg cells was detected by flow cytometry, and the expression level of Th1/Th2/Th17-related cytokines in peripheral serum was detected. Results: The expression of PD-1/PD-L1 in blast cells, T lymphocyte cell subsets and Treg cells in low risk MDS group were lower than that in control group, medium and high risk MDS group and AML group（all P < 0.01）, and Th1/Th17 type cytokines were dominant. The expression of PD-1/PD-L1 in blast cells, T lymphocyte cell subsets and Treg cells of intermediate and high risk MDS group and AML group were higher than that of control group and low risk MDS group (all P < 0.01), and Th2 type and Treg type (IL-10、TGF-β) cytokines were dominant. After treatment, the differences of PD-1/PD-L1 expression were not statisticatly significant in blast cells, T lymphocyte cell subsets and Treg cells between the MDS remission group and the control group (all P >0.05). The expression of PD-1/PD-L1 in blast cells, T lymphocyte cell subsets and Treg cells in MDS non-remission group were significantly higher than that in remission group and control group (all P < 0.01). Conclusion: The high expression of PD-1/PD-L1, dominance of Treg (IL-10、TGF-β) and Th2-related cytokines and inhibition of effector T lymphocyte cells in patients with MDS is conducive to tumor cell proliferation and immune escape, which may promote the progression of MDS disease.

Abstract:BackgroundThe programmed cell death protein 1 (PD‐1) and its ligand 1 and 2 (PD‐L1/PD‐L2) regulates the immune system and the checkpoint pathway can be exploited by malignant cells to evade anti‐tumor immune response. Soluble forms (sPD‐1/sPD‐L1/sPD‐L2) exist in the peripheral blood, but their biological and clinical significance is unclear. MethodTime‐Resolved Immunofluorometric Assay (TRIFMA) and Enzyme‐Linked Immunosorbent Assay (ELISA) were used to measure sPD‐1, sPD‐L1 and sPD‐L2 levels in serum from 131 lymphoma patients and 22 healthy individuals. ResultsPatients had higher sPD‐1 and sPD‐L2 levels than healthy individuals. In Diffuse Large B‐cell Lymphoma, patients with high International Prognostic Index score had higher sPD‐1 levels and sPD‐L2 levels correlated with subtype according to cell of origin. Compared to other lymphoma types, follicular lymphoma displayed higher sPD‐1 and lower sPD‐L1 levels along with lower ligand/receptor ratios. ConclusionThis is the first study to simultaneously characterize pre‐therapeutic sPD‐1, sPD‐L1 and sPD‐L2 in a variety of lymphoma subtypes. The relation between higher sPD‐1 levels and adverse prognostic factors as suggests a possible biological role and potential clinical usefulness of sPD‐1. Moreover, the reverse expression pattern in follicular lymphoma and T‐cell lymphoma/leukemia may reflect biological information relevant for immunotherapy targeting the PD‐1‐pathway.

Abstract:Immune checkpoint inhibitors (ICIs) bring prognostic benefits to patients with malignancies. However, there is a substantial number of patients whose lesions are not improved by ICIs. In addition, ICIs may cause immune‐related adverse events (irAEs), which could lead to an unfavorable prognosis with fatal consequences. Therefore, we conducted a retrospective study to evaluate the utility of circulating sPD‐L1 (soluble programmed cell death 1 ligand 1) as a biomarker in patients with advanced melanoma treated with anti–PD‐1 (programmed cell death 1 protein) antibodies. Sera from 31 consecutive patients were prospectively collected before and after anti–PD‐1 antibody treatment and the serum level of sPD‐L1 was evaluated. We found that high sPD‐L1 levels before treatment were associated with better prognosis, and this association was observed only in patients with a low tumor burden. We also found that sPD‐L1 levels were elevated in patients who developed severe irAEs after treatment, and the patients with severe irAEs had significantly higher fluctuations in sPD‐L1 (delta sPD‐L1) than those without severe irAEs. Our study suggests that serum sPD‐L1 level is a useful biomarker to predict tumor response and irAE development in patients with advanced melanoma treated with anti–PD‐1 antibodies.

Abstract:Background: Elevated levels of soluble PD-L1 (sPD-L1) have been associated with worse prognosis in renal cell carcinoma and multiple myeloma. However, the regulatory roles and function of sPD-L1 in advanced melanoma are not fully understood. This study was designed to evaluate the association between circulating sPD-L1 expression and prognosis in patients with advanced acral and mucosal melanoma. Methods: Totally 102 untreated advanced acral and mucosal melanoma patients from Peking University Cancer Hospital between Jan 2012 and Dec 2015 were enrolled in the present study. Peripheral blood samples were obtained from 40 healthy donors as control. Circulating sPD-L1 expression was tested by enzymelinked immunosorbent assay (ELISA). Results: The advanced melanoma cohort includes 58 acral melanoma and 44 mucosal melanoma. Concentrations of sPD-L1 (2.91ng/mL) were elevated in the plasma of prior to treatment advanced melanoma patients in comparison with healthy donors (0.59ng/mL). The expression of sPD-L1 in serum was found to be highly up-regulated in 39 (38.2%) of 102 cases. The sPD-L1 concentration appeared to be significantly related with subtype (arcal 3.14 vs. mucosal 2.60 ng/mL P = 0.004). No significant association was observed between serum sPD-L1 level and other clinicopathological variables as: BRAF mutation, LDH level, tumor burden and peripheral blood CD4+/CD8+. There were no associations between sPD-L1 and chemotherapy clinical responses in our cohort. But the overall survival rates were statistically estimated with the expression of sPD-L1. The OS in this cohort with high and low up-regulated sPD-L1 expression levels was 8.50 months and 11.64 months, respectively (p = 0.022). Conclusions: sPD-L1 was elevated in advance acral and mucosal melanoma patients and may play an important role in patients prognosis. Abstract:PD-L1 expression in malignant tumor tissues is a rational biomarker to predict the efficacy and prognosis of anti-PD-1/anti-PD-L1 treatment, but few studies focus on the role of blood PD-L1 expression. Fifty-one paired tissue samples and blood samples, as well as clinicopathologic features, were collected from patients with diverse malignancies to investigate the correlation among tissue PD-L1 (tPD-L1) expression, plasma PD-L1 mRNA expression, soluble PD-L1 (sPD-L1) expression and clinicopathologic features. Tissue PD-L1 were measured by immunohistochemistry. PD-L1 mRNA and self-designed plasma external reference PLACON were measured by quantitative real-time PCR. Soluble PD-L1 were detected by ELISA kit. Then, dynamic changes of blood PD-L1 expression (at baseline and within 2 months) were measured to evaluate the efficacy of patients with malignancies (n=24) who received anti-PD-1/anti-PD-L1 treatment. Moderate correlation between tPD-L1 and PD-L1 mRNA (r=0.62, P<0.001), weak correlation between tPD-L1 and sPD-L1 (r=0.37, P=0.007) and weak correlation between PD-L1 mRNA and sPD-L1 (r=0.32, P=0.02) were found. Most clinicopathologic features had no significant correlation with PD-L1 mRNA and sPD-L1 expression. Interestingly, patients without metastasis had higher PD-L1 mRNA and sPD-L1 expression than counterparts. Further, patients with over 2.03-fold PD-L1 mRNA increase (n=11) during treatment experienced improved progression-free survival (PFS) than those with less than 2.03-fold increase (n=13), these patients also had higher best overall response (bOR) rate (45.45% vs. 7.69%). By comparison, the dynamic changes of sPD-L1 expression had no significant correlation with PFS and bOR. Our study demonstrates that plasma PD-L1 mRNA expression was significantly correlated with tissue PD-L1 expression, and provides proof for the application of plasma PD-L1 mRNA as a predictor for anti-PD-1/anti-PD-L1 treatment.

Abstract:PURPOSE:Immune checkpoint proteins in the tumor microenvironment can enter the blood circulation and are potential markers for liquid biopsy. The aims of this study were to explore differences in immune checkpoint protein expression between patients with nasopharyngeal carcinoma (NPC) and healthy controls and to investigate the prognostic value of the soluble form of programmed death-ligand 1 (sPD-L1) in NPC.METHODS:In total, 242 patients were included in the disease group. Plasma samples from 23 NPC patients and 15 healthy control were used for immune checkpoint protein panel assays. Samples from 219 patients with NPC including 30 paired pre-treatment and post-radiotherapy samples were evaluated by enzyme-linked immunosorbent assay to determine sPD-L1 levels.RESULTS:A total of 14 immune checkpoint proteins, including sPD-L1were upregulated in 23 patients with NPC (all p<0.001) compared with 15 healthy controls. Among 219 patients, the median follow-up time was 50 months (7-82 months). Based on the optimal cutoff value of 93.7 pg/mL, patients with high expression of sPD-L1 had worse distant metastasis-free survival (87.5% vs 74.0%, p=0.006) than those of patients with low expression. Multivariate analysis showed that sPD-L1 (HR=1.99, p=0.048) and EBV-DNA (HR=2.51, p=0.030) were poor prognostic factors for DMFS. In the group with high EBV-DNA expression, DMFS was worse for patients with high sPD-L1 expression than those with low sPD-L1 expression (56.4% vs 82.6%, p=0.002).CONCLUSION:Plasma immune checkpoint protein expression differed significantly between patients with NPC and healthy donors. Plasma sPD-L1 levels are a candidate prognostic biomarker, especially when combined with EBV-DNA.

Abstract:Molecular pathology and clinical characteristics play a crucial role in guiding treatment selection and predicting the prognosis of diffuse large B-cell lymphoma (DLBCL). The programmed cell death protein 1 (PD-1) and its ligand (PD-L1), have emerged as pivotal regulators of immune checkpoints in cancer. The objectives of this study are to investigate the correlation between the expression levels of PD-1 and soluble PD-L1 (sPD-L1) in the peripheral blood of DLBCL patients, analyze their clinicopathological characteristics, and identify the optimal beneficiary group for PD-1/PD-L1 blockade. Peripheral blood samples were collected from 36 DLBCL patients before their initial treatment at Shandong Cancer Hospital between December 2018 and July 2019. The expression levels of PD-1 and sPD-L1 were measured using flow cytometry and enzyme-linked immunosorbent assay (ELISA), respectively. The clinicopathological characteristics, including age, sex, Ann Arbor stage, International Prognostic Index (IPI) score, response to treatment, etc., were recorded for each patient. The surface expression of PD-1 on peripheral blood T cells was significantly higher in DLBCL patients compared to healthy controls. There was a significant association between elevated PD-1 expression levels and the advanced Ann Arbor stage (P=0.0153) as well as the B group (P=0.0184). Higher sPD-L1 levels were associated with the GCB subtype according to Hans's classification (P=0.0435). The expression levels of PD-1 and sPD-L1 in the peripheral blood of DLBCL patients are significantly correlated with advanced disease stage, B group, and GCB subtype according to Hans's classification. This suggests that the PD-1/PD-L1 axis play a critical role in specific subgroups of DLBCL. Targeting this axis could serve as a potential therapeutic strategy to enhance the clinical outcomes of DLBCL patients. Further studies are necessary to explore the prognostic implications of PD-1 and sPD-L1 expression levels in DLBCL patients.

Abstract:Abstract Backgroud The specific recurrent cytogenetic and molecular abnormalities are significantly associated with prognosis and have been used to establish risk stratification which directs the therapeutic strategy in AML. Whether the immune surveillance contributes to the prognosis according to the cytogenetic and molecular risk status is unknown. The checkpoint PD-1/PD-L1 pathway is involved in the tumor surveillance. Down-regulation of PD-1/PD-L1 pathway caused a slower AML progression in mouse models through anergy of CD8+ CTL cells. Clinical trials of PD-1/PD-L1 inhibitor-based regimens in patients with AML are ongoing and seem to have promising results. But few studies investigated clinical significance of PD-L1 expression on leukemic cells in AML patients. Method We prospectively observed 120 consecutive adult patients with non-APL AML. PD-L1 mRNA of bone marrow mononuclear cells at diagnosis was detected by RT-PCR and the specific fluorescence indices (SFI) of PD-L1 expression on bone marrow blast cells was determined by flow cytometry. Cytogenetic and molecular genetic risks were assessed according to NCCN AML guideline (2016 version 2). Patients with age >60 years or failure of karyotype analysis were excluded. All remaining 95 patients were treated with "3+7" idarubicine and cytarabine induction regimen. Patients with response of ≥PR were administered the same regimen as the initial chemotherapy, and then consolidated with intermediate-dose cytarabine for four cycles followed by MA, AA, HA and AE regimens subsequently. Patients at poor risk or with unsatisfied controlled minimal residual disease were recommended allogeneic transplantation (allo-HCT). Among the patients with indication of allo-HCT, those who had both donor and desire received allo-HCT after 3 cycles of consolidation in their first CR (CR1). Result The diagnoses according to morphology included AML-M0 (n=1), AML-M1 (n=5), AML-M2 (n=55), AML-M4 (n=19), AML-M5 (n=13) and AML-M6 (n=2). Twenty patients were at favorable risk, 43 were at intermediate risk and 32 were at poor risk. We found a correlation between SFIs of PD-L1 and PD-L1 mRNA (r=0.413, P<0.001). After the base 10 logarithm transformation, quantity of PD-L1 mRNA in mononuclear cells of bone marrow were different among all risk groups (P=0.025). Post Hoc multiple comparisons showed PD-L1 mRNA of poor-risk group was higher than that of favorable-risk group (P=0.008). PD-L1 mRNA of the intermediate-risk group was also higher when compared to that of favorable-risk group (P=0.029). SFIs of PD-L1 on leukemic cells was higher in poor-risk group than in either intermediate-risk (P=0.040) or favorable-risk group (P=0.042), although no statistic difference was found among three groups (P=0.057). With ROC curves, we found PD-L1 mRNA could well predict both the early CR defined as the CR achieved after the initial chemotherapy (AUC=0.736, P=0.050) and the final CR defined as CR1 achieved after any cycles of induction chemotherapy (AUC=0.911, P=0.006). SFI of PD-L1 on leukemic cells could only predict the final CR (AUC=0.889, P=0.009). With both the best specificity of 0.833 and sensitivity of 1.000 for predicting final CR and specificity of 0.667 and sensitivity of 0.917 for predicting early CR, the cutoff value of PD-L1 mRNA was identified as 0.0681. SFI of PD-L1 less than 1.852 was a good predictor for achieving final CR with the sensitivity of 0.800 and specificity of 1.000. PD-L1 mRNA≥0.0681 was the only independent poor-risk factor for the early CR (HR=12.697, 95%CI 2.710-59.481, P<0.001). Conclusion AML cells with poorer-risk cytogenetic and molecular abnormalities seemed to have a higher PD-L1 expression which may compromise the leukemic immune surveillance by CTL suppression. Higher PD-L1 expression on AML cells was associated with poorer response to IA-based induction regimens. Disclosures No relevant conflicts of interest to declare.

Abstract:The contribution of this work is to propose novel balanced bandpass filters (BPFs) using the half-mode substrate integrated waveguide (HMSIW) and its size-reduced form, a folded HMSIW structure. The size of an HMSIW is about half of the substrate integrated waveguide (SIW) and the size of the folded HMSIW further reduces to 25%. Besides compactness, the proposed BPFs are attractive because the thru-via array needed for providing the side-wall effect is no longer required. In addition, the common-mode (CM) rejection of the HMSIW BPF is achieved from the non-coupling slots, caused by the PMC. The CM rejection of the folded HMSIW BPF is obtained by up-shifting the CM operating frequency far away from the differential-mode (DM) passband. Prototypes of the proposed circuits are fabricated and measured to present CM rejection levels larger than 40 dB over a wide frequency range and DM insertion losses around 2 dB in the passbands. Agreements between measured and simulated data are observed to validate the circuit design and performance.

Abstract:OBJECTIVE The study of the relation between treatment costs and disability of psychiatric patient groups. DESIGN Perspective assessment of costs and disability of 1371 adult psychiatric patients in charge at two Operative Psychiatric Units (OPU), followed during an average period of 9 months. Data are related to all OPU's psychiatric services, including ambulatory, full or half-residential and psychiatric departments of acute hospital services. SETTING OPUs of Magenta (MI) and Desio (MI). METHOD The disability level has been measured by Health of the Nation Outcome Scales (HoNOS) filled in at the inclusion of the patient in the study and every three-months on average thereafter. Besides other HoNOSs have been filled in both at admission and discharge from psychiatric departments of acute hospitals, Residential Centres of psychiatric Therapies and Rehabilitation and Guarded Communities. All patients have been grouped using the main psychiatric diagnosis (first digit ICD-10) and the maximum disability level shown in the whole period of the study. Direct costs of publicly financed psychiatric services have only been considered. Their attribution to each patient has been made applying standard costs or tariffs (diagnostic procedures) to the data perspectively collected by the regional Register and a purposely designed protocol. RESULTS Total cost of 1371 patients has been 9771.1 million lire with a cost per patient of 7,127,000 lire (sd 19,499,000) and a cost per "day in charge" of 27,172 lire (sd 68,358). The cost per day has been found unrelated with the length of observed time frame. At the inclusion the mean level of disability has been 4.26 points (sd 3.73) and 3.19 points (sd 3.26) at the end of the study. Its value, measured at maximum level shown by each problem in the whole period of study, has been 6.00 points (sd 4.64). Disability and treatment cost of each patient did result directly related (r = 0.626, p = 0.0001). All patients have been grouped in 12 classes with a significant (p = 0.0001) overall difference on both their disability level and treatment cost. CONCLUSIONS All adult psychiatric patients could be grouped in disability related classes which sometimes have also a different treatment cost. A study on a greater number of patients is needed to confirm these results. It may also provide a more reliable basis for a new financing system of psychiatric services.
