Abstract:Objective: A cyclin-dependent kinase inhibitor, p271(kip1), is recognized as a negative regulator of the cell cycle. To clarify whether immunohistochemical detection of p27 might provide prognostic information, Foe investigated the expression of p27 in laryngeal squamous cell carcinoma (SCC), Study Design: A retrospective study of patients was performed in 109 cases of laryngeal SCC, In addition, we investigated the expression of p53 and granulocyte colony-stimulating factor receptor (GCSF-R) to examine the prognostic significance of them in the same samples. Methods: Immunohistochemical staining by specific monoclonal antibodies was performed using the avidin-biotin-peroxidase complex technique, Results: Advanced tumor size and clinical stage and the occurrence of lymph node metastasis were associated with the absence of p27 expression, but not correlated with p53 expression and GCSF-R expression. The overall B-year survival rate in the p27-positive group was significantly higher than that in the p27-negative group. In the Cox proportional hazard model, p27 was demonstrated to be the most powerful prognostic factor among gender, tumor size, lymph node metastasis, stage of disease, and p53 and GCSF-R expression. Conclusions: We concluded that assessment of p27 expression is useful as a prognostic factor for laryngeal SCC and of value in selecting patients with laryngeal SCC for aggressive therapy.

Abstract:Objective To investigate the possible relationship between expression of Ki67 antigen and clinicopathological features and prognosis of laryngeal squamous cell carcinoma(LSCC).Methods The sections of paraffin embedded blocks were examined by Envision immunohistochemical method for the expression of Ki67 proliferation marker in 40 cases of primary LSCC and 5 cases of normal laryngeal tissues.Results The difference of Ki67 expression between carcinous and adjacent normal tissues was statistically significant(P(0.01)).The expressions of Ki67 in cervical metastasis group,squamous cell histological types Ⅱ group,and advanced stages group(stage 3,4) were higher than those in no cervical metastasis group,squamous cell histological types I group,early stages group(stage 1,2) respectively(P(0.05)).The expressions in survival group without recurrence were lower than those in the recurrent or died group(P(0.01)).There was no significant difference between supraglottic group and glottic group(P(0.05)).Conclusion The expressions of Ki67 antigen are closely correlated with lymph node metastasis,histological grade and clinical stages,but not correlated with tumor location.The expression of Ki67 antiogen is one of the important biologic characteristics in LSCC and therefore a prognostic index of this tumor.
Expression And Significance Of Glucose Transporter-1, P-Glycoprotein, Multidrug Resistance-Associated Protein And Glutathione S-Transferase-Pi In Laryngeal Carcinoma

Abstract:Increasing glucose transporter-1 (GLUT-1) activity is one of the most important ways to increase the cellular influx of glucose. We previously demonstrated that increased GLUT-1 expression was an independent predictor of survival in patients with laryngeal carcinoma. Thus, GLUT-1 may present a novel therapeutic target in laryngeal carcinoma. In this study, the expression of GLUT-1, P-glycoprotein (P-gp), multidrug resistance-associated protein 1 (MRP1) and glutathione S-transferase-pi (GST-pi) in laryngeal carcinomas was investigated by immunohistochemistry. Additionally, possible correlations between GLUT-1 and P-gp, MRP1 and GST-pi and various clinicopathological parameters were analyzed. In this study, 52.9% (18/34), 58.8% (20/34), 20.6% (7/34) and 58.8% (20/34) of the laryngeal carcinomas were positive for GLUT-1, P-gp, MRP1 and GST-pi, respectively. The expression of GLUT-1, P-gp, MRP1 and GST-pi was higher in laryngeal carcinoma specimens when compared with laryngeal precancerous lesions (P<0.05). Pearson's correlation analysis showed correlations between GLUT-1 and P-gp (r=0.364; P=0.034), GLUT-1 and MRP1 (r=0.359; P=0.037) and P-gp and GST-pi (r=0.426; P=0.012). GLUT-1 expression was found to significantly correlate with tumor-node-metastasis classification (P=0.02) and clinical stage (P=0.037). Furthermore, P-gp was found to significantly correlate with clinical stage (P=0.026). Univariate analysis showed that MRP1 expression was significantly associated with poor survival (c(2)=5.16; P=0.023). Multivariate analysis revealed that lymph node metastasis (P=0.009) and MRP1 overexpression (P=0.023) were significant predictors of poor survival. In the present study, the expression of GLUT-1, P-gp, MRP1 and GST-pi in laryngeal carcinomas was investigated, as well as the correlations between these proteins. P-gp was found to significantly correlate with clinical stage, while MRP1 overexpression was significantly associated with poor survival.
Abnormal P16 Expression and Prognostic Significance in Esophageal Squamous Cell Carcinoma.

Abstract:BACKGROUND:The purpose of this study was to analyze p16 expression status and evaluate whether abnormal p16 expression was associated with prognosis in a large-scale esophageal squamous cell carcinoma (ESCC) cohort of patients.METHODS:We retrospectively evaluated p16 expression status of 525 ESCC samples using immunohistochemistry. Associations between abnormal p16 expression and survival were analyzed.RESULTS:P16 negative, focal expression and overexpression were found in 87.6%, 6.9% and 5.5% of ESCC patients. No significant association was observed between abnormal p16 expression and age, sex, tumor site and location, differentiation, vessel and nerve invasion, T stage and lymph node metastasis. In all patients, the survival of p16 focal expression group tended to be better compared with negative group (disease free survival/DFS P=0.040 and overall survival/OS P=0.052) and overexpression group (DFS P=0.201 and OS P=0.258), and there was no survival difference between negative group and overexpression group. The multivariate analysis for OS and DFS found that only clinical stage was a significantly independent prognostic factor (P<0.001). When patients were divided into I-II stage (n=290) and III-IVa stage (n=235), the survival of focal expression group was better compared with negative group (DFS P=0.015 and OS P=0.019), and tended to be better compared with overexpression group (DFS P=0.405 and OS P=0.432) in I-II stage ESCC, which was not found in III-IVa stage ESCC.CONCLUSION:P16 overexpression or negative expression tend to be associated with unfavorable outcomes, especially in I-II stage ESCC. Our study will help to identify a subgroup of ESCC patients with excellent prognosis after surgical therapy.
[The expressions of Ki67 and VEGF in squamous cell carcinoma of larynx and the correlation between the two marks]

Abstract:Objective: To investigate the expression of Ki67 and VEGF in squamous cell carcinoma of larynx and the correlation between Ki67 and VEGF. Method: The expression of Ki67 and VEGF in squamous cell carcinoma of larynx tissues of 40 cases were studied by immunohistochemical staining. Result: The expression of Ki67 and VEGF in squamous cell carcinoma of larynx tissues were higher than those in mucosa of laryngeal cavity (P<0.01). The expression of the two marks in squamous cell carcinoma of larynx with the metastasis of lymph nodes were higher than those without metastasis (Ki67: P<0.05, VEGF: P<0.01). Age and sex did not change the expression obviously (P>0.05). There was a positive correlation between T-stage and immunohistochemical staining (P<0.05). There was also a positive correlation in staining intensity between Ki67 and VEGF (r = 0.823, P<0.01). Conclusion: Ki67 and VEGF are objective marks to estimate the behaviors of squamous cell carcinoma of larynx, and they can do help to the diagnosis, the judgment of stages and the treatment of carcinoma of larynx.

Abstract:Objective To investigate the relationship between Ki-67 and P16 expression levels after neoadjuvant chemotherapy, and the clinicopathological characteristics and prognosis of patients with locally advanced cervical cancer. Methods Patients with FIGO stage IB2 or IIA2 cervical cancer, who underwent neoadjuvant chemotherapy combined with radical hysterectomy at the First Affiliated Hospital of Chongqing Medical University between January 2015 and December 2019, were identified retrospectively to correlate postoperative Ki-67 and P16 expression levels with clinicopathological factors. The optimal threshold for predicting recurrence was analysed using receiver operating characteristic (ROC) curves for the Ki-67 index, and univariate and multi-factorial Cox regression analysis were used to investigate the association between clinicpathological features including Ki-67 and P16 and recurrence-free survival. Results In total, 334 patients were included after screening. The cut-off value of Ki-67 for determining recurrence was 67.5 % according to the ROC curve. On multi-factorial Cox analysis, lymphatic vascular space ( p = 0.003) and Ki-67 index ( p = 0.005) were shown to increase the risk of recurrence, and were independent prognostic factors for recurrence, while the expression of P16 was not significantly associated with the risk of recurrence ( p = 0.097, odds ratio = 0.319). Patients with cervical cancer in the high Ki-67 expression group (Ki-67 ≥ 67.5 %) had lower recurrence-free survival and overall survival than patients in the low Ki-67 expression group (Ki-67 < 67.5 %) ( p = 0.001 and 0.036, respectively). Conclusion The expression levels of Ki-67 and P16 after neoadjuvant chemotherapy for locally advanced cervical cancer correlated with tumour differentiation. High expression of Ki-67 (Ki-67 ≥ 67.5 %) may indicate poorer recurrence-free survival and overall survival.

Abstract:OBJECTIVE: To investigate the expression of Caveolin-1 and its correlation with HPV in human Laryngeal squamous cell carcinomas(LSCC) tissues,and analyze their influences on the biological behavior of human LSCC.METHODS: Expressions of Caveolin-1 and HPV16/18 were measured by immunochemical staining or in-situ hybridization in LSCC and Para-cancerous tissues.RESULTS: The positive rates of Caveolin-1 in LSCC and Para-cancerous tissues were respectively 71.2% and 9.5%(P0.05).Expression rate of Caveolin-1 was significantly associated with tumor differentiation,TNM stage,and metastasis.The frequency of HPV expression in laryngeal carcinoma and Para-cancerous tissues were 55.1% and 21.4% respectively(P0.05) and there was no relation with differentiation and metastasis.The expressions of Caveolin-1 and infection of HPV in the LSCC had a positive correlation(r=0.62,χ2=10.40,P0.05).CONCLUSIONS: Expression of Caveolin-1 is significantly higher in LSCC than that in Para-cancerous tissues and it has significant relations to tumor invasion and metastasis.The pathogenesis of LSCC is closely related with HPV infection.And the mechanism may be that Caveolin-1 mediates the infection of HPV in tumors.
HPV Infection and Anemia Status Stratify the Survival of Early T2 Laryngeal Squamous Cell Carcinoma.

Abstract:Objective. This study was designed to investigate the potentially prognostic indicators of early laryngeal squamous cell carcinomas (LSCCs), including human papillomavirus (HPV) status.Methods. A total of 336 patients with T2N0-1M0 LSCC were included in this study. Clinical data were collected from archival documents, and HPV infection and p16(INK4A) expression were detected.Results. A total of 32/318 cases of high-risk HPV infection and 10/336 cases of p16(INK4A) overexpression were found. Three hundred eighteen tumors were classified into a three-class model according to HPV infection and p16(INK4A) expression: class I, HPV+/p16+; class II, HPV+/p16-; and class III, HPV-/p16-. Class III had a trend of decreased overall survival (OS) (P = 0.076) and a markedly low relapse-free survival (RFS) (P = 0.022) compared with class I and class II. HPV-positive cases (class I plus class II) had a significantly longer OS (P = 0.038) and RFS (P = 0.006). In multivariate analysis, HPV-positive (P = 0.020), nonanemia (P = 0.011), and N0 stage (P = 0.005) were significant predictors for high RFS. But only HPV-positive (P = 0.047) and nonanemia (P < 0.001) were significant predictors for superior OS.Conclusion. A trend of discrete survival among HPV+/p16+, HPV+/p16-, and HPV-/p16 classes was found in early LSCCs. We suggest that HPV infection and hemoglobin level are the potential factors that can stratify outcome of early LSCCs.

Abstract:Laryngeal squamous cell carcinoma (LSCC) is a common malignancy that, despite scientific advancements, has not seen an improvement in its prognosis in the last decades. Few promising predictive markers have been found and none are relevant in clinical practice. p16ink4a, an oncosuppressor protein involved in cell cycle arrest, with a prognostic impact on other cancers, has been widely used in the head and neck region as a surrogate marker of HPV infection. Published papers and recent meta-analyses seem to minimize the biological role of HPV in the context of LSCC's cancerogenesis, and to disprove the reliability of p16ink4a as a surrogate prognostic marker in this context, while still highlighting its potential role as an independent predictor of survival. Unfortunately, the available literature, in particular during the last two decades, is often not focused on its potential role as an independent biomarker and few relevant data are found in papers mainly focused on HPV. The available data suggest that future research should focus specifically on p16ink4a, taking into account both its potential inactivation and overexpression, different patterns of staining, and immunohistochemistry cutoffs, and should focus not on its potential role as a surrogate marker but on its independent role as a predictor of survival.

Detection and Significance of Human Papillomavirus, CDKN2A (p16) and CDKN1A (p21) Expression in Squamous Cell Carcinoma of the Larynx

Abstract:Although a strong etiologic relationship between human papillomavirus (HPV) and a majority of oropharyngeal squamous cell carcinomas has been established, the role of HPV in non-oropharyngeal head and neck carcinomas is much less clear. Here, we investigated the prevalence and clinicopathologic significance of HPV and its reported biomarkers, CDKN2A(p16) and CDKN1A(p21), in laryngeal squamous cell carcinomas in patients treated either with primary surgery and postoperative radiation or with definitive radiation-based therapy. Nearly all of 76 tumors were keratinizing and none displayed the nonkeratinizing morphology that is typically associated with HPV infection in the oropharynx. However, CDKN2A(p16) immunohistochemistry was positive in 21 cases (28%) and CDKN1A(p21) in 34 (45%). CDKN2A(p16) and CDKN1A(p21) status strongly correlated with each other (P=0.0038). Yet, only four cases were HPV positive by DNA in situ hybridization or by reverse transcriptase PCR E6/E7 mRNA (all four were CDKN2A(p16) and CDKN1A(p21) positive). Unexpectedly, 9 additional tumors out of 20 CDKN2A(p16) positive cases harbored high-risk HPV DNA by PCR. For further investigation of this unexpected result, in situ hybridization for E6/E7 mRNA was performed on these nine cases and all were negative, confirming the absence of transcriptionally active virus. Patients with CDKN1A(p21)-positive tumors did have better overall survival (69% at 3 years) than those with CDKN1A(p21)-negative tumors (51% at 3 years) (P=0.045). There was also a strong trend towards better overall survival in the CDKN2A(p16)-positive group (P=0.058). Thus, it appears that the role of HPV is more complex in the larynx than in the oropharynx, and that CDKN2A(p16) and CDKN1A(p21) expression may not reflect HPV-driven tumors in most cases. Because of this, CDKN2A(p16) should not be used as a definitive surrogate marker of HPV-driven tumors in the larynx.

Expression of P16 in Non-Small Cell Lung Cancer and Its Prognostic Significance: a Meta-Analysis of Published Literatures.

Abstract:The prognostic value of p16 for survival of patients with non-small cell lung cancer (NSCLC) remains controversial. we performed a meta-analysis of the literatures in order to clarify its impact. Published studies in English were identified using an electronic search in order to aggregate the available survival results. To be eligible, a study had to have dealt with p16 protein assessment in NSCLC patients on the primary site and have reported survival data according to p16 expression. Twenty trials, comprising 1995 patients, provided sufficient information for the meta-analysis. Seventeen assessed any non-small cell lung cancer subtype, three assessed adenocarcinoma only. Eight identified high p16 expression as a favourable prognostic factor and one linked it with poor prognosis, Eleven trials were not significant. The overall combined hazard ratio (HR) calculated using a random-effects model suggested that high p16 expression has a favourable impact on survival in all NSCLC [0.69, 95% CI: 0.59–0.81]; The studies were categorized according to histology, disease stage and laboratory technique. The aggregated survival data showed a poor survival prognosis in squamous cell cancer with lower p16 expression [0.34, 95% CI: 0.13–0.91]. The adenocarcinoma subgroup had an HR of 0.91 [95% CI 0.76–1.10] without statistical significance. In early stage NSCLC (I–II), the aggregated HR was 0.42 [95% CI: 0.28–0.63], showing a worse survival for NSCLC with abnormal p16 expression; Results were significant with the HR of 0.61 [95% CI: 0.45–0.82] for five studies detecting p16 by immunohistochemistry with antibody clone G175–405. In conclusion, our meta-analysis shows that the p16 expression status is an independent prognostic factor in NSCLC, and this tendency is also found in the subgroups of squamous cell lung cancer and early stage NSCLC (I–II), but not in lung adenocarcinoma.
Expression and significance of P16INK4a and P14ARF proteins in laryngeal squamous cell carcinoma

Abstract:Objective To study the expression and clinical significance of P16INK4a and P14ARF proteins in laryngeal squamous cell carcinoma. Methods The expression of P16INK4a and P14ARF proteins was detected in 71 carcinoma samples by immunohistochemistry (EnVision method). Results The negative expression of P16INK4a and P14ARF was detected in 35 samples(49.3 %)and 29 samples(40.8 %) respectively. While 15 cases(21.1 %)showed simultaneous loss in both proteins. Rank correlation analysis revealed that there was no correlation between negative expression of P16INK4a and P14ARF proteins in carcinoma(P>0.05). The negative expression rate of P16INK4a or P14ARF was unrelated to the clinical data including gender,age and clinical stage. The overpositive proportion of all P14ARF positive cases in stage I to II was 41.7% and 11.1% in stage III to IV. Conclusions The aberrance of P16INK4a and P14ARF gene might be associated with the oncogenesis of carcinoma. There was no correlation between negative expression of P16INK4a and P14ARF proteins in carcinoma. Moreover,the loss of P14ARF protein occurred in the early stage of carcinoma.

Abstract:Inflammatory bowel disease among the elderly is common, with growing incident and prevalence rates. Compared with younger IBD patients, genetics contribute less to the pathogenesis of older-onset IBD, with dysbiosis and dysregulation of the immune system playing a more significant role. Diagnosis may be difficult in older individuals, as multiple other common diseases can mimic IBD in this population. The clinical manifestations in older-onset IBD are distinct, and patients tend to have less of a disease trajectory. Despite multiple effective medical and surgical treatment strategies for adults with Crohn's disease and ulcerative colitis, efficacy studies typically have excluded older subjects. A rapidly ageing population and increasing rates of Crohn's and ulcerative colitis make the paucity of data in older adults with IBD an increasingly important clinical issue.
Prognostic heterogeneity of Ki67 in non‐small cell lung cancer: A comprehensive reappraisal on immunohistochemistry and transcriptional data

Abstract:Abstract In the present study, the debatable prognostic value of Ki67 in patients with non‐small cell lung cancer (NSCLC) was attributed to the heterogeneity between lung adenocarcinoma (LUAD) and lung squamous carcinoma (LUSC). Based on meta‐analyses of 29 studies, a retrospective immunohistochemical cohort of 1479 patients from our center, eight transcriptional datasets and a single‐cell datasets with 40 patients, we found that high Ki67 expression suggests a poor outcome in LUAD, but conversely, low Ki67 expression indicates worse prognosis in LUSC. Furthermore, low proliferation in LUSC is associated with higher metastatic capacity, which is related to the stronger epithelial‐mesenchymal transition potential, immunosuppressive microenvironment and angiogenesis. Finally, nomogram model incorporating clinical risk factors and Ki67 expression outperformed the basic clinical model for the accurate prognostic prediction of LUSC. With the largest prognostic assessment of Ki67 from protein to mRNA level, our study highlights that Ki67 also has an important prognostic value in NSCLC, but separate evaluation of LUAD and LUSC is necessary to provide more valuable information for clinical decision‐making in NSCLC.

Abstract:Purpose: Immunotherapy may be a potential alternative for patients with sinonasal squamous cell carcinoma (SNSCC). Data regarding potential immunotherapy targets, such as programmed death-ligand 1 (PD-L1) and tumor-infiltrating lymphocytes (TILs), in SNSCC are limited. In this study, we assessed the prevalence and prognostic value of PD-L1 expression and TILs in p16-negative and p16-positive SNSCC. Patients and methods: Tissues from 96 patients with SNSCC were stained using immunohistochemistry against PD-L1, CD8, and Foxp3 to assess the immune environment. The correlations between PD-L1 expression, TILs, and p16 status were analyzed. Additionally, PD-L1, CD8, and Foxp3 expressions, as well as p16 status, were analyzed in relation to patient clinicopathological variables and prognosis. Results: Twenty-nine (30.2%) patients with SNSCC showed PD-L1 expression in >5% of tumor cells. PD-L1 expression was significantly correlated with poor differentiation and a high level of TILs. PD-L1 expression and the CD8+ and Foxp3+ T-cell infiltrates in p16-negative patients (n=78, 81.2%) and p16-positive patients (n=18, 18.8%) were not significantly different. PD-L1 expression and p16 status were not associated with overall survival (OS) and disease-free survival (DFS). Patients with high CD8+ or Foxp3+ cell infiltration had better clinical outcomes. A multivariate analysis confirmed that CD8 TILs were a significant independent and favorable prognostic factor for OS (p=0.023) and DFS (p=0.008). Conclusion: TILs can play a prognostic role in SNSCC. We did not find differences in immune marker expression between p16-positive and p16-negative SNSCC tissues. The high correlation between PD-L1 expression and TILs indicates that the PD-1/PD-L1 pathway is a promising immunotherapeutic target for SNSCC.

Abstract:Objective To study the expression of p16 and Ki-67 in cervical epithelial neoplasia, analyze the clinical diagnostic value.Methods100 cases of cervical intraepithelial neoplasia patients were selected as the research object, 50 healthy volunteers were treated as control objects, to compare the expression of p16 and Ki-67 in different groups.Results p16 and Ki-67 in cervical intraepithelial neoplasia in patients with lesions of the tissue expression positive rate were significantly higher than those in healthy control group, and the expression positive rate was increased with increase of the CIN grade of and grading in patients, the different was significant (P<0.05).Conclusion It can provide a reliable basis for the diagnosis and grading of cervical intraepithelial neoplasia patients according to the expression of Ki-67 and p16 in cervical tissues.
Expression of P63 and CK5/6 in Early-Stage Lung Squamous Cell Carcinoma is Not Only an Early Diagnostic Indicator but Also Correlates with a Good Prognosis

Abstract:BackgroundNon-small cell lung cancer (NSCLC) accounts for 80% of lung cancers, and lung squamous cell carcinoma (SQCC) is one of the main types. Advances in the treatment of lung SQCC are lacking when compared to lung adenocarcinoma. The main treatment for early-stage SQCC is surgery. However, factors affecting the efficacy of surgical treatments for early-stage lung SQCC remain unclear. In this study, we examined the significance of commonly used lung SQCC diagnostic markers p63, p40, and cytokeratin (CK)5/6 in prognosis.MethodsSeventy-six cases of early-stage lung SQCC (N-0) were obtained from our lung cancer database (January 2000 to December 2009). Tissue microarray and immunohistochemical (IHC) staining were used to detect the expression of p63, p40, and CK5/6. The effect of the expression level of each marker on patients' survival was examined.ResultsSensitivity and specificity of each marker for detecting lung SQCC was 87.0% and 81.0% for p63, 75.9% and 97.9% for p40, and 78.9% and 97.7% for CK5/6. Survival rates of patients with high expression levels of p63 or CK5/6 or both were higher than in patients with low expression levels (P < 0.05). Expression levels of p40 had no effect on survival (P > 0.05). Multivariate analysis showed that high levels of p63 expression p63+CK5/6 co-expression were independent prognostic factors for good survival.ConclusionIHC staining detection of p63 and CK5/6 in specimens should be routinely performed in postoperative early-stage lung SQCC patients. Its significance lies not only in differential diagnosis, but also in determining prognosis.
Evaluation of Risk Factors for Laryngeal Squamous Cell Carcinoma: A Single-Center Retrospective Study

Abstract:Background The survival rate of patients with laryngeal squamous cell carcinoma (LSCC) is correlated with several factors. However, the independent prognostic factors of patients with LSCC remain unclear. Thus, we sought to identify prognostic factors affecting LSCC outcomes in the Chinese population. Methods The survival and potential prognostic factors of 211 patients with LSCC between April 2011 and July 2019 were retrospectively analyzed. Overall survival (OS) and progression free survival (PFS) were estimated by the Kaplan Meier method, and a log-rank test was used to compare the possible prognostic factors between different groups. The Cox proportional hazard model was used to perform multivariable analysis of significant covariants. Results A total of 211 LSCC patients were included, of which 164 (77.7%) were male and 47 (22.3%) were female. Mean age was 62.19 ± 8.328 years. A univariate analysis showed that seven factors including pathological differentiation, clinical stage, tobacco consumption, alcohol consumption, T stage, N stage, and concurrent chemoradiotherapy were correlated with survival ( P <0.05). Cox proportional hazards regression analyses revealed that clinic stage (hazard ratio=3.100, p=0.048), pathological differentiation (hazard ratio = 2.538, p=0.015), alcohol consumption (hazard ratio = 8.456, p =0.004) were associated with OS in LSCC. Pathological differentiation (hazard ratio =5.677, p=0.000), alcohol consumption (hazard ratio =6.766, p=0.000) were associated with PFS in LSCC. Conclusions Pathological differentiation, alcohol consumption, are independent prognostic factors and predictors of recurrence in LSCC. These factors could help inform guidelines for clinical treatment and prognosis.

Abstract:Objective Systemic inflammatory response index (SIRI) values and tumor-infiltrating lymphocytes (TILs) are associated with the prognosis of various tumors. There is minimal evidence of those two as prognostic markers in laryngeal squamous cell carcinoma (LSCC). In this study, we aimed to examine the predictive value of SIRI and tumor-infiltrating CD3 + /CD4 + /CD8 + T cells in the prognosis of patients who underwent partial or total laryngectomy. Study design A total of 78 patients with LSCC who underwent total or partial laryngectomy at the Eye, Ear, Nose, and Throat Hospital of Fudan University between 2013 and 2015 were retrospectively analyzed. Methods The tumor tissues of 78 LSCC patients were retrospectively evaluated using immunohistochemical staining for CD3+ /CD4+ /CD8+ -cells. The overall survival (OS) and disease-free survival (DFS) rates were recorded using the Kaplan–Meier method. Results Patients with high immunoscore (IS) (3–4) had prolonged survival ( P < 0.001 for OS). High SIRI values were independently associated with poorer OS and DFS ( P = 0.018 for OS; P = 0.016 for DFS). CD8 + TILs and SIRI values showed a- negative association ( P < 0.01). Patients with low SIRI values and high IS had better 5-year OS and DFS than those with high SIRI values and low IS ( P < 0.001 for OS; P = 0.0014 for DFS). Patients with ‘hot’ tumor had a higher 5-year OS than those with ‘excluded’ or ‘cold’ phenotype. Conclusions The SIRI values and the density of TILs may help predict LSCC patients' outcomes after surgery. The combination of SIRI and IS may be a new component of the tumor, nodes, and metastases (TNM) classification of cancer and prognostic factor for T-cell-target immunotherapy.
