Abstract:Objective: This study aimed to assess the efficacy of erbium-doped yttrium aluminum garnet (Er:YAG) laser, pastes containing casein phosphopeptide amorphous calcium phosphate (CPP-ACP) with and without fluoride and their combination for prevention of white spot lesions in the enamel. Materials and methods: This in vitro experimental study was conducted on 90 extracted sound premolars. The teeth were then randomly divided into six groups of 15: (1) Control, (2) laser, (3) CPP-ACP with fluoride (GC MI Paste, Recaldent™ 900 ppm as NaF), (4) CPP-ACP without fluoride (GC Tooth Mousse Recaldent™), (5) laser + CPP-ACP with fluoride, and (6) laser + CPP-ACP without fluoride. In each group, enamel surface was exposed to the remineralizing agent. The teeth were then subjected to pH cycling for 14 days. The teeth were then sectioned, polished, and underwent cross-sectional microhardness testing at 20-160 μ depth quantitatively. Using the Simpson's rule, the amount of mineral loss was calculated in each group. Statistical Analysis Used: ANOVA was used for the comparisons, and Tukey's test was applied for pairwise comparisons. Results: The highest mean volume percentage of microhardness at 20-60 μ depth belonged to the group laser + CPP-ACP with fluoride and the lowest belonged to the control group (P = 0.001). The differences were not significant at 80-120 μ depth (P > 0.05). These finding are confirmed according to △Z (mineral loss). Conclusion: Based on these results, Er: YAG laser was able to decrease demineralization and was a potential alternative to preventive dentistry and was more effective when combined with CPP-ACP products.
Effect of elevated blood tyrosine on intellectual development of premature infants

Abstract:Blood tyrosines were followed from birth to nursery discharge in 71 premature infants fed a high-protein formula supplemented by 60 mg/day of ascorbic acid. In 89% tyrosine concentrations were abnormal, and in 38% the maximum observed was 5.0 mg% or higher. Maximum blood tyrosine correlated with gestational age (p < 0.05) but not with birth weight.On a follow-up study performed at 15 months, infants with high tyrosine levels had no increase in neurological abnormalities. Between 7 and 8 years a second follow-up was done on 64 children. This included a WISC and tests for psychomotor function. Two children had died in the interval and six others were too retarded for full testing. The full scale IQ of all children correlated with birth weight (p < 0.01). The mean IQ of high and low tyrosine subjects was 85.9 and 86.2, respectively. When infants were grouped by birth weight a significant difference was detected in subjects weighing 2000+ gm.Significant differences were recorded in scores on object assembly (p < 0.05), picture assembly (p < 0.05), and picture completion (p < 0.10). We observed no effect of high tyrosine levels on intellectual performance of smaller infants, who, on the whole, are a greater risk for other complications of prematurity.
Efficacy of fluoride varnish containing casein phosphopeptide-amorphous calcium phosphate application and diode laser irradiation on white spot lesions remineralization: An in vitro study

Abstract:Background/objectives: The prevention of white spot lesions (WSLs) during orthodontic treatment with fixed appliances is of paramount importance to orthodontists. Numerous non-invasive techniques have been extensively researched to effectively manage WSLs. The objective of this study was to investigate the efficacy of MI varnish application, diode laser irradiation and their combination on remineralization of WSLs. Materials and methods: In this in vitro study, 40 enamel samples were divided randomly into 4 groups based on the applied treatment after demineralization. In Group I, MI varnish was applied to the enamel samples for a period of 7 days. In Group II, samples were irradiated with diode laser. In Group III, MI varnish was applied for 7 days, followed by diode laser irradiation. Whereas Group IV (control group) received no treatment and samples were stored in artificial saliva for 7 days. Microhardness assessments were performed at baseline, after demineralization and following the different treatment protocols. A representative sample from each group was randomly selected for scanning electron microscopy (SEM) analysis. One-way ANOVA, repeated measures ANOVA, and Tukey's Post Hoc tests were used for statistical analysis, with significance set at P≤0.05. Results: All treatment groups demonstrated significantly higher mean microhardness values compared to the control group (P<0.05). Group 1 (MI varnish) had a final microhardness mean value of 193.50 (P<0.001), Group 2 (diode laser) recorded the highest final mean of 214.20 (P=0.018), and Group 3 (MI varnish+diode laser) had a mean of 203.93 (P=0.011). SEM observations supported these findings by showing enhanced surface morphology in the treatment groups. Conclusions: MI varnish application, laser irradiation, and their combination demonstrated enhanced microhardness of artificially demineralized enamel, highlighting their effectiveness in remineralization of WSLs.

Abstract:Aim: Fluoride-Amorphous Calcium Phosphate and Biomimetic Nano-Hydroxyapatite for Enamel Remineralization; An In-Vitro Study of Surface Microhardness and Composition. Material and methods: Ninety-six extracted human premolars with sound buccal surface were divided using a randomization computer-generating software into four groups; Group I (control) sound untreated enamel, Group II (demineralized) demineralized to create white spot lesions, Group III (biom-n-HA) demineralized and then treated with biomimetic nanohydroxyapatite cream, and Group IV (F-ACP) demineralized and then treated using Fluoride-Amorphous Calcium Phosphate varnish. Each group was divided into two subgroups; subgroup "A" evaluated for mineral content using energy dispersive x-ray spectroscopy (EDX) and for surface microhardness using the Vickers microhardness test and Subgroup "B" evaluated for white spot lesion depth using a polarized light microscope (PLM). Results: The highest microhardness (VHN) was found in the (F-ACP) group (mean=428.61±54.43) and then in the (Biom-n-HA) group (mean=408.11±70.16) followed by the (Control) group (mean=402.13±53.40) with no significant difference between them and finally in the significantly different (Demineralized) group (mean=256.99±45.83). The weight percentage of Ca (30.29±1.04 and 33.44±1.07) and Ca/P ratio (1.87±0.06 and 2.03±0.05) were significantly different between Group III and Group IV respectively. PLM measurements in Group II (198.83μm), Group III (60.17μm), and Group IV (26.33μm) were significantly different. Conclusions: Both the (Biom-n-HA) cream and the (F-ACP) varnish showed promising results for enamel remineralization. The increased enamel surface microhardness was consistent with the mineral content and the changes in the birefringence.

Abstract:Background White spot lesions (WSLs) are common early indicators of enamel demineralization, particularly in pediatric orthodontic patients. Effective remineralization of these lesions is crucial for preventing further dental decay. This study aimed to evaluate the three-dimensional remineralization efficacy of two commercial toothpastes, calcium sucrose phosphate (CaSP) and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), using micro-computed tomography (micro-CT). Objectives To compare the remineralization efficacy of CaSP and CPP-ACP on artificially created WSLs in human premolar enamel using micro-CT assessment. Materials and methods Freshly extracted, caries-free human premolars were used for the purpose of the study. Teeth with any defects, including caries, non-carious lesions, fractures, or hypocalcifications, were excluded. Teeth were stored in a saline solution with 0.1% thymol at +4°C until experimentation. Enamel slabs (N = 18) of 3 x 3 x 1.5 mm were prepared from the buccal surfaces of the premolars and polished to a mirror-like finish. The slabs were divided into three groups (n=6 each): control, CaSP, and CPP-ACP. Specimens were demineralized in a demineralization solution for 72 hours and then treated with the respective toothpastes or remineralizing solutions for 13 days. Micro-CT scanning was performed to assess changes in enamel volume and mineral density. Statistical analysis included Shapiro-Wilk tests, paired t-tests, and one-way ANOVA with post-hoc Tukey's HSD tests. Results Enamel volume changes were significant between groups (p<0.01), with the CaSP group showing the largest remineralization effect. Enamel mineral density changes were also significant (p=0.004), with the CPP-ACP group showing the greatest improvement in mineral density. Conclusions CaSP and CPP-ACP are both effective in remineralizing artificial enamel lesions. While CaSP shows comparable efficacy to CPP-ACP, further research is needed to confirm these findings in clinical settings. CaSP paste can be considered a viable, cost-effective alternative for enamel remineralization.

Abstract:Background: World-wide, the contribution of dental caries to the burden of oral diseases is about 10 times higher than that of periodontal disease, the other common oral condition. Owing to its globally high prevalence, dental caries is a "pandemic" disease characterized by a high percentage of untreated carious cavities causing pain, discomfort and functional limitations. Untreated carious cavities; furthermore, have a significant impact on the general health of children and on the social and economic well-being of communities. A surgical approach to the elimination of carious lesion was developed a century ago; this approach was necessary at that time, because there was no valid alternative. The focus in caries has recently shifted to the development of methodologies for the detection of the early stages of caries lesions and the non-invasive treatment of these lesions. The non-invasive treatment of early lesions by remineralization has the potential to be a major advance in the clinical management of the disease. Remineralization of white-spot lesions may be possible with a variety of currently available agents containing fluoride, bioavailable calcium and phosphate and phosphate. This concept bridges the traditional gap between prevention and surgical procedures, which is just what dentistry needs today. Aims and objectives: The aim of this in vitro study was to evaluate and to compare the remineralization potential of casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) and casein phosphopeptide-amorphous calcium fluoride phosphate (CPP-ACFP) on artificial white spot enamel lesions using the quantitative light fluorescence (QLF). Materials and methods: A total of 45 caries-free extracted maxillary first premolars were embedded in acrylic resin. The samples were randomly divided into three groups namely control group, CPP-ACP group and CPP-ACFP group with 15 samples in each group. The samples of each group were subjected to demineralization process for a period of 96 h. The samples were then mounted in the artificial mouth model and subjected to remineralization and pH cycling for a period of 21 days. QLF readings were recorded at the end of demineralization (1st, 7th, 14th and 21st day) and were statistically analyzed. Results: As compared with artificial saliva both CPP-ACP and CPP-ACFP produced significant amount of remineralization of the artificial enamel white spot lesion (P < 0.001), however when the remineralizing effect of CPP-ACP was compared with the remineralizing effect of CPP-ACFP there was no significant difference. Significant amount of remineralization was produced by CPP-ACP and CPP-ACFP only after the 7th day. After the 14th day, the remineralization produced by both CPP-ACP and CPP-ACFP as compared to artificial saliva was non-significant.
Comparative evaluation of remineralization potential of novel bioactive agents on eroded enamel lesions: A single-blinded in vitro study

Abstract:Objective: The escalating prevalence of noncarious tooth wear stands as a critical concern in the backdrop of evolving lifestyles and dietary patterns. Dental erosion, a progressive condition induced by both endogenous and exogenous acidic influences, directly impacts enamel integrity, resulting in surface loss. The contemporary surge in carbonated beverage consumption further exacerbates this erosive milieu, underscoring the urgency for dental practitioners to adopt meticulous treatment strategies. Existing literature underscores a noteworthy 94% reduction in tooth erosion risk for individuals abstaining from sweetened soft beverages, emphasizing the imperative for a well-devised remineralization protocol to counter demineralized surfaces. Methodology: Seventy-three enamel specimens were taken. Forty samples were subjected to pre-operative hardness testing, and five samples were subjected to baseline EDX evaluation followed by grouping of samples (Group 1 = control Group; Group 2 = casein phosphopeptide-amorphous calcium phosphate fluoride [CPP-ACPF] Group; Group 3 = Biomin F Group; and Group 4 = self-assembling peptide [SAP] P-114 Group). A demineralization-remineralization cycle was carried out for 5 days followed by testing through Vickers Microhardness Tester, EDX Evaluation, and Scanning Electron Microscopy (SEM) Imaging. Statistical analysis was performed using one-way analysis of variance followed by intergroup analysis using Tukey's post hoc test with SPSS software 25.0 version. Results: The mean percentage change in microhardness values was 30.05% in Group 1, 24.21% in Group 2, 18.85% in Group 3, and 12.08% in Group 4. The mean Ca/P ratio of samples tested through EDAX was 2.20 at baseline, 1.40 in Group 1 (Control Group), 1.62 in Group 2 (CPP-ACPF), 1.82 in Group 3 (Biomin F), and 2.01 in Group 4 (SAP-P114). Postintervention values were statistically significant from baseline values in both parameters. Conclusion: Curodont Protect exhibits superior efficacy, offering valuable insights for future in vivo studies and clinical applications. The multifaceted evaluation, encompassing microhardness testing, SEM analysis, and EDXS assessment, contributes to a nuanced interpretation of the agents' impact, paving the way for informed decisions in clinical practice and future research endeavors.

Abstract:Aim: Different remineralizing pretreatments Casein phosphopeptide-amorphous calcium phosphate fluoride (CPP-ACPF), tricalcium phosphate fluoride (TCP-F), self-assembling peptide (SAP) P11-4 and 10 % Nanohydroxyapatite (nHA) gel activation via invisible infrared light on the dentin microhardness (MH) and micro shear bond strength (µSBS) of composite restoration. Methods: Seventy-five human molar teeth were collected and the dentinal surface of all the samples was exposed to different demineralizing solutions. (n = 15) Group 1 (demineralized dentin), Group 2 (CPP ACP), Group 3 (TCP-F), Group 4 (SAP P11-4), Group 5 (nHA gel activation via invisible infrared light). MH assessment was performed using Vickers hardness. Each group of 10 samples was subjected to composite restoration buildup and µSBS were tested. The debonded samples were then observed under a stereo-microscope for failure analysis. ANOVA was conducted, along with Tukey's post hoc analysis, to examine the µSBS of composite and MH of the remineralized surface. Results: nHA gel activation via invisible infrared light pretreated specimens showed the maximum outcomes of surface hardness (331.2 ± 77.3) and bond strength (10.38 ± 2.77). However, Group 4 (SAP P11-4) (148.3 ± 29.2) remineralized dentin displayed minimum scores of MH and µSBS (5.88 ± 1.01). Conclusion: Remineralizing pretreatment nHA gel activation via invisible infrared light and casein phosphopeptide-amorphous calcium phosphate fluoride seem to improve the dentin MH and µSBS of the composite restoration.

Abstract:Objectives: The main purpose of this study was to compare the remineralizing effects of casein phosphopeptide amorphous calcium phosphate fluoride (CPP-ACPF) and tricalcium phosphate (TCP) on artificially induced enamel white spot lesions (WSLs). Materials and Methods: In this in vitro study, 45 sound extracted premolars were immersed in a demineralizing solution (pH=4.5) for 96 hours, and were randomly divided into 3 groups of TCP, MI Paste Plus, and control. They were exposed to the remineralizing agents for 5 minutes once a day for 30 days. After mounting the teeth in resin blocks and polishing, they underwent a microhardness test at 3 different depths from the enamel surface. Data were analyzed by Prism software, two-way ANOVA, and Tukey’s test (α=0.05). Results: The volume percentage of mineral content (VPM) was significantly different among the three groups at 30-, 60- and 90μm depths (P<0.0001). At 30μm depth, CPP-ACPF was significantly more effective than TCP (P 0.05). Conclusion: Both CPP-ACPF and TCP had significant efficacy for remineralization of artificially induced enamel WSLs under in vitro conditions.
Comparative Evaluation of Influence of Nd:YAG Laser (1064 nm) and 980 nm Diode Laser on Enamel around Orthodontic Brackets: An In Vitro Study

Abstract:(1) Background: The prevention of demineralizing lesions at the enamel structure level continues to represent a challenge in daily dental practice. When bacteria influence the pH level, this will decrease below the threshold for remineralization and the dissociation of hydroxyapatite will occur with a high percentage of phosphate and calcium loss. These elements continue to be studied by many authors in order to obtain a working protocol that will lead to their stabilization at the level of the enamel structure, thus preventing the demineralization process. The aim of this study is to evaluate and compare the influence of two types of laser wavelengths on the surface morphology and mineral components of the enamel through an examination with scanning electron microscopy (SEM) and energy-dispersive X-ray spectrometry (EDX). (2) Methods: Thirty permanent human incisors extracted for periodontal reasons from patients aged 25−40 years old were selected for this study. Metallic brackets (SS Standard 022 Slot, OC Orthodontics, McMinnville, OR, 97128, USA) were bonded onto each tooth. The buccal surface was randomly assigned three sections: Section A—negative control (no treatment), section B—treated with 980 nm Gallium−Aluminum−Arsenide diode laser (a 300 µm optic fiber was used with 0.8 W output power, energy density of 5.33 J/mm2, in continuous mode, for 30 s, oriented perpendicularly to the enamel surface in contact mode) (KaVo GENTLEray 980 Diode Laser, Kaltenbach & Voigt GmbH, Biberach, Germany), and section C—treated with Nd:YAG laser (a 300 µm fiber was carried out at a 1 mm distance from the enamel surface with 0.75 W power, 75 mJ pulse energy, pulse repetition rate of 10 Hz, 5 J/mm2 fluency, average exposure time of 30 s, and water cooling assisted) (LIGHTWALKER AT S, M021-5AF/1 S, Fotona d.o.o, Ljubljana, Slovenia). The elements evaluated in this study were calcium (Ca), phosphate (P), oxygen (O), and carbon (C). A one-way analysis of variance, paired t-tests, and independent t-tests were carried out to evaluate the results using the SPSS 19 IBM Statistical package software for Microsoft. (3) Results: The evaluation of the data indicated that both wavelengths produced an increase in Ca wt% (for diode laser the mean of Ca wt% before irradiation was 21.06, while that after treatment reached 28.24; and for Nd:YAG laser, the mean of Ca wt% before irradiation was 21.31, while that after treatment reached 33.88); as well, the 980 nm diode laser decreased P wt% (from 17.20 before irradiation to 16.92 after irradiation) and the Nd:YAG laser increased P wt% (from 17.46 before irradiation to 18.28 after irradiation). These results showed a statistically significant difference at the p < 0.05 level. (4) Conclusions: It can be concluded that the best improvement of enamel chemical composition was obtained with Nd:YAG irradiation.
In vitro controlled elemental analysis to evaluate the combined effect of acidulated phosphate fluoride with Er:YAG and diode lasers

Abstract:Background: Acidulated phosphate fluoride (APF) application is used for dental caries prevention. Combining APF with different laser wavelengths increases acid resistance in enamel. Aim: To assess the effects of Er:YAG (2940 nm) laser and diode (940 nm) laser after the application of APF on morphology and mineral content of enamel surfaces of human teeth in vitro. Materials and methods: Twenty (20) freshly extracted third molar crowns were collected and split longitudinally into halves. Ten (10) halves were given to each of the following groups: group 1 (control), group 2 (application of 1.23% APF gel only), group 3 (diode laser; 1 W for 60 sec, contact mode after APF gel), and group 4 (Er:YAG; 80 mJ, 0.8 W, 10 Hz; in pulsed mode for 10 sec after APF gel). Specimens were morphologically examined by scanning electron microscope (SEM), and mineral content was analysed by energy dispersive X-ray (EDX). Collected data were statistically analysed by ANOVA. Results: Morphologic results were superior in group 3 followed by group 2 and group 1 with the least change in group 4. EDX of calcium (Ca), phosphorous (P), and Ca/P ratio weight % showed the highest value with no statistical significance between groups 2 and 1, and higher with statistical significance in groups 3 and 4 in descending order. Carbon weight % showed highest results in group 4 then 3, 2, and 1. Fluoride weight % showed highest statistical results in group 4 then 2, 3, and 1. Conclusion: The diode laser showed superior morphological results after APF application compared to the Er:YAG laser. The Er:YAG increased fluoride absorption on outer enamel surfaces.

Abstract:Introduction Interproximal reduction (IPR) damages the caries protective superficial layer of the enamel, making the enamel surface prone to caries because of the increase in surface roughness. Remineralizing solutions can help in preventing these undesirable side effects. Therefore, this study aimed to compare the effect of nanohydroxyapatite (nHAp) and sodium fluoride (NaF) application on enamel remineralization after IPR and to evaluate changes in surface roughness, composition, and microhardness of the treated enamel. Methods A total of 25 patients with Angle's Class I malocclusion, requiring 4 premolar extractions, were selected and randomly divided into 5 groups (n = 5). Group 1 served as the control. In group 2, the extraction of premolars was done immediately after IPR, and in group 3, the extraction of premolars was done 3 months after IPR. In group 4, the extraction of premolars was performed 3 months after IPR with weekly application of nHAp serum. In group 5, the extraction of premolars was performed 3 months after IPR, along with once-a-month application of NaF varnish. The proximal reduction of premolars in all the groups was done using Strauss IPR burs (Strauss Diamond Instruments, Palm Coast, Fla). The extracted teeth were sectioned, and the enamel surfaces were subjected to energy-dispersive x-ray spectroscopy to evaluate elemental composition. Vicker's microhardness test was used to evaluate enamel hardness and atomic force microscopy for enamel surface roughness. Descriptive statistics were calculated for the 5 groups using a 1-way analysis of variance, and Tukey's multiple post-hoc test was used for intergroup comparison. Results Calcium-to-phosphorous ratio, enamel microhardness, and surface roughness were found to be closest to untouched enamel in patients treated with nHAp, followed by patients who were treated with NaF. A lower calcium-to-phosphorous ratio and weakened and roughest enamel surface was seen in teeth, which were extracted immediately after IPR. Conclusions Among the remineralizing agents tested, nHAp serum can be recommended for better remineralization of enamel surfaces after IPR.

Abstract:Introduction: Development of white spot lesions on enamel is a significant and common problem during the fixed orthodontic treatment. Aim: This study was aimed to assess the effect of two types of erbium lasers (Er: YAG) laser, (Er, Cr: YSGG) laser and light cure fluoride on the enamel demineralization and surface microhardness around metal orthodontic brackets. Material and Methods: In vitro study, 90 freshlyextracted human premolars were randomly allocated to six groups (n =15) of control (neither fluoride nor laser was used), light cure fluoride varnish (Clinpro XT) on the enamel surfaces, Er: YAG laser (2.94 μm Er: YAG irradiation of the teeth), Er: YAG laser then varnishTM XT, Er Cr:YSGG laser (2.78 μm Er Cr: YSGG irradiation of the teeth), Er Cr: YSGG laser then varnish TM XT. PH cycling for 14 days through a daily procedure of demineralization and remineralization for 6 h and 18 h, respectively. Microhardness values of enamel were evaluated with Vickers test. Two sample of each group was prepared for SEM (scanning electron microscopy) and the data from the remaining teeth were analyzed with F-test (ANOVA). Results: Microhardness mean values from high to low were as follow: varnish TM XT, Er Cr: YSGG laser then varnishTM XT, Er:YAG laser then varnish TM XT, control, Er Cr: YSGG laser, Er :YAG laser. Conclusion: Fluoride varnish alone showed the best preventive measure against enamel demineralization. There was no different between the two types of erbium laser on the effect of enamel surface microhardness.
Micro-Raman spectroscopy analysis of dentin remineralization using eggshell derived nanohydroxyapatite combined with phytosphingosine

Abstract:The aim of this study was to assess the remineralization efficacy of chicken eggshell-derived nano-hydroxyapatite (CEnHAp) combined with phytosphingosine (PHS) on artificially induced dentinal lesions. PHS was commercially procured whereas CEnHAp was synthesized using microwave-irradiation method and characterized using X-ray diffraction (XRD), Fourier transform infrared spectroscopy (FT-IR), high-resolution scanning electron microscopy-energy-dispersive X-ray spectroscopy (HRSEM-EDX), and transmission electron microscopy (TEM). A total of 75 pre-demineralized coronal dentin specimens were randomly treated with one of the following test agents (n = 15 each): artificial saliva (AS), casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), CEnHAp, PHS, and CEnHAp-PHS under pH cycling for 7, 14, and 28 days. Vickers microhardness indenter, HRSEM-EDX, and micro-Raman spectroscopy methods were used to assess the mineral changes in the treated dentin samples. Data were submitted to Kruskal-Wallis and Friedman's two-way analyses of variance (p < 0.05). HRSEM and TEM analysis depicted irregular spherical structure of the prepared CEnHAp with a particle size of 20-50 nm. The EDX analysis confirmed the presence of Ca, P, Na and Mg ions. The XRD pattern showed the characteristic crystalline peaks for hydroxyapatite and calcium carbonate that are present in the prepared CEnHAp. Dentin treated with CEnHAp-PHS revealed highest microhardness values along with complete tubular occlusion compared to other groups at all test time intervals (p < 0.05). Specimens treated with CEnHAp showed increased remineralization than those treated with CPP-ACP followed by PHS and AS groups. The intensity of mineral peaks, as observed in the EDX and micro-Raman spectra, confirmed these findings. Further, the molecular conformation of the collagen's polypeptide chains, and amide-I and CH2 peaks attained peak intensities in dentin treated with CEnHAp-PHS and PHS whereas other groups revealed poor stability of collagen bands. Microhardness, surface topography, and micro-Raman spectroscopy analyses revealed that dentin treated with CEnHAp-PHS have an improved collagen structure and stability as well as highest mineralization and crystallinity.

Abstract:Background: White spot lesions occur when the pathogenic bacteria have broken through the enamel layer. White spot lesions (WSLs) can be treated using a complex approach. The most crucial step is to stop demineralization and biofilm formation and use assorted strategies for remineralization of lesions, thinning, microabrasion, erosion infiltration, adhesive composite resin restorations, and the bonded facets. Aim: To evaluate and compare the fluoride, calcium, and phosphorus ion release, remineralizing efficacy, and microhardness of zwitterionic material, self-assembling peptide, and bioactive glass (BGA) incorporated with MI Varnish. Materials and methods: The original study was conducted on 60 extracted premolars; the sample size calculated was 10 per group. All samples were divided into four groups-group A, MI Varnish (control), group B, MI Varnish + zwitterionic material, group C, MI Varnish + self-assembling peptide, and group D, MI Varnish + BGA. All these groups were further evaluated and compared for fluoride, calcium, and phorphorus ion release, remineralizing efficacy, and surface microhardness (SMH). Results: Zwitterionic material, when incorporated with MI Varnish showed high fluoride and calcium ion release and high remineralizing efficacy under polarized light microscopy (PLM). BGA, when incorporated with MI Varnish showed high phosphorus ion release and higher values in the evaluation of SMH, followed by zwitterionic material and self-assembling peptide. Conclusion: MI varnish alone had remineralizing properties of WSLs, but when novel materials like zwitterionic ion, self-assembling peptide, and BGA are incorporated, its efficacy increases. Among all zwitterionic ions showed superior results for fluoride and calcium ion release and remineralization and BGA for phosphorus ion release and SMH. Rehardening and the Protective Effect of Gamma-Polyglutamic Acid/Nano-Hydroxyapatite Paste on Surface-Etched Enamel

Abstract:Many revolutionary approaches are on the way pertaining to the high occurrence of tooth decay, which is an enduring challenge in the field of preventive dentistry. However, an ideal dental care material has yet to be fully developed. With this aim, this research reports a dramatic enhancement in the rehardening potential of surface-etched enamels through a plausible synergistic effect of the novel combination of γ-polyglutamic acid (γ-PGA) and nano-hydroxyapatite (nano-HAp) paste, within the limitations of the study. The percentage of recovery of the surface microhardness (SMHR%) and the surface parameters for 9 wt% γ-PGA/nano-HAp paste on acid-etched enamel were investigated with a Vickers microhardness tester and an atomic force microscope, respectively. This in vitro study demonstrates that γ-PGA/nano-HAp treatment could increase the SMHR% of etched enamel to 39.59 ± 6.69% in 30 min. To test the hypothesis of the rehardening mechanism and the preventive effect of the γ-PGA/nano-HAp paste, the surface parameters of mean peak spacing (Rsm) and mean arithmetic surface roughness (Ra) were both measured and compared to the specimens subjected to demineralization and/or remineralization. After the treatment of γ-PGA/nano-HAp on the etched surface, the reduction in Rsm from 999 ± 120 nm to 700 ± 80 nm suggests the possible mechanism of void-filling within a short treatment time of 10 min. Furthermore, ΔRa-I, the roughness change due to etching before remineralization, was 23.15 ± 3.23 nm, while ΔRa-II, the roughness change after remineralization, was 11.99 ± 3.90 nm. This statistically significant reduction in roughness change (p < 0.05) implies a protective effect against the demineralization process. The as-developed novel γ-PGA/nano-HAp paste possesses a high efficacy towards tooth microhardness rehardening, and a protective effect against acid etching.

Abstract:Objective: To evaluate and compare clinically the effectiveness of a nano-hydroxyapatite (n-HAP) paste (Sesensibilize™ Nano-P, FGM) and a tri-calcium phosphate fluoride (TCP-F) varnish (Clinpro™ White Varnish, 3M ESPE) in remineralization of white spot lesions (WSLs) on young permanent teeth. Methods: 20 patients having at least one pair of teeth with WSL were randomly selected to this study according to specific criteria. Each patient received two treatments of WSL; one treatment using n-HAP paste on one side of the arch (n-HAP group) and a second one using TCP-F varnish on the contralateral or opposing side of the arch (TCP-F group). A total of 80-teeth with WSLs were included in this randomized clinical trial. The reminaralizing agents were applied over the tooth surface with a WSL as per manufacturer recommendations. Four application sessions were completed during the first month on a weekly basis. DIAGNODENT PEN (DD) (Kavo Dental –Germany) was used to assess the degree of remineralization. DD readings were recorded at baseline, after one, three and six-months of the reminaralizing agents’ application. All patients attended the treatment sessions and remained throughout the follow-up periods. Results: There was a statistically significant difference between the WSLs mean DD readings over time for both groups (Friedman, P<0.001). The mean DD reading was 22.05 (±3.250) in n-HAP group and 21.33 (±3.467) in TCP-F group at the baseline, decreased to 10.80 (±1.7423) and 9.675 (±2.693) at one-month, 8.30 (±1.880) and 10.95(±2.726) at the 3-month, 7.875 (± 1.555) and 13.725 (±2.745) at the 6-month follow-up in n-HAP group and TCP-F group respectively. Upon comparing the groups, the WSLs had a similar mean DD reading at baseline (P>0.05). There was no statistically significant difference in the mean DD reading at one-month between the two groups (P=0.032). However, the mean DD readings were significantly lower in the n-HAP group at 3- and 6-months with high statistical significant level (P <0.001). Conclusion: The nano-hydroxyapatite paste (nano-P) and the tri-calcium phosphate varnish (clinpro) were effective in WSLs’ remineralization. Both treatment protocols could be advocated to promote remineralization of WSLs after orthodontic treatment. The nano-hydroxyapatite paste appears to demonstrate better stability than the tri-calcium phosphate varnish in its remineralization effect over the 6-month follow-up.
Evaluation of Surface Properties of Two Remineralizing Agents after Modification by Chitosan Nano Particles: An In vitro Study

Abstract:Background: Calcium phosphate-based systems have been introduced as promising bio-mimetic materials due to their close resemblance to the enamel. Chitosan and its derivatives have been an emerging biomaterial due to their additional antibacterial effect and promising re-mineralizing ability. Aim: The aim of this study is to evaluate the effect of chitosan nanoparticles on the remineralization of the demineralized enamel surface after being added to nano-hydroxyapatite and nano-calcium phosphate materials. Setting and design: This was in vitro study. Materials and methods: Twenty specimens of extracted permanent molars were collected, and then immersed in demineralizing solution, then distributed into four groups according to the remineralizing material. Group 1: Treated with Nano-beta-tricalcium phosphate (Nβ-TCP) gel, Group 2: Treated with Nβ-TCP with chitosan gel, Group 3: Treated with Nanohydroxyapatite (NHA) gel, and Group 4: Treated with NHA with chitosan gel. The surface hardness of the teeth was measured at baseline, after demineralization, and after remineralization. The structural changes in each group were analyzed using the scanning electron microscopy. Statistical analysis: Shapiro-Wilk's test, one-way ANOVA followed by Tukey's post hoc test was used. Results: In all groups, there was a significant difference in mean Vickers hardness number (VHN) at different intervals, with the highest value found after treatment (301.64-395.65) VHN, followed by the baseline (236.97-276.15) VHN, while the lowest value was detected after demineralization (121.23-124.39) VHN. It was also indicated that baseline treatment, the Hardness percentage change (%) of the nano NHA + Chitosan group showed the highest significant value (55.10%), while the Nβ-TCP group exhibited the lowest significant value (9.56%). Conclusions: It can be concluded that NHA and NΒ-TCP modified by chitosan NPs as remineralizing agents of enamel surface hold promising results.
The Effect of Formulated Dentin Remineralizing Gel Containing Hydroxyapatite, Fluoride, and Bioactive Glass on Dentin Microhardness: An In Vitro Study

Abstract:Objectives: This study aimed to develop a gel with dentin-remineralizing properties, integrating nano-hydroxyapatite (nHA), sodium fluoride (NaF), and bioactive glass (BG). Materials and methods: The enamel layer of 40 bovine incisors was removed. The samples were allocated into four groups of 10 each, based on varying concentrations of nHA, BG, and NaF in the gel compositions (wt%): (1) 2.5%-7.5%-0.05%, (2) 5%-5%-0.05%, (3) 7.5%-2.5%-0.05%, and (4) a control group with a base gel lacking remineralizing agents. After 8 hr of demineralization, the dentin surface microhardness was measured at depths of 30, 60, and 140 µm. After a 20-day pH cycling, the percentage of surface microhardness recovery (SMHR%) was measured and compared among the groups using the ANOVA and Tukey HSD post hoc tests (α = 0.05). Scanning electron microscopy analysis evaluated each specimen's superficial morphology. Results: At all depths, the SMHR% of the Group 2 and Group 3 was significantly higher than the control group (p < 0.05). The SMHR% Group 1 (67.39% ± 29.34%) was significantly higher than the control group (-21.24% ± 51.72%) only at the depth of 30 μm (p = 0.047). Group 3 had higher SMHR% than Group 2 at all depths; however, the difference was not statistically significant. Moreover, the SMHR% of Group 3 was significantly higher than that of Group 1 at depths of 30 μm (187. 94% ± 68.95% vs. 67.39% ± 29.34%; p = 0.005) and 60 μm (179.55% ± 75.96% vs. 64.34% ± 41.96%; p = 0.043). Surface deposition and tubule occlusion were observed in the Groups 2 and 3 samples, which was more prominent in the latter. Conclusions: Combining 7.5% nHA, 2.5% BG, and 0.05% NaF could potentially remineralize primary carious lesions.
