Abstract:Objective To investigate the influence of donor diabetes on the overall prognosis of patients with acute-chronic liver failure（ACLF） after liver transplantation. Methods A retrospective study was conducted in 839 ACLF patients who received liver transplantation from January 1,2013 to December 31,2013 in Scientific Registry of Transplant Recipients（SRTR） database. Then, the recipients were divided into two groups according to whether they used diabetic donors（93 cases were in DM group, 746 cases were in non-DM group）. Donor and recipient baseline characteristics were compared between two groups, and overall survival rates were compared according to different recipient ACLF grades. Univariate and multivariable analysis were further used. Results The overall survival rates were comparable in patients with ACLF1 and ACLF2. However, in patients with ACLF3, the 1, 3, 5-year overall survival rates were 84.3%, 78.3%, 72.4% in the non-DM group,it was significantly better than that in the DM group, which were 70.4%, 55.6%, 51.9%, respectively（P = 0.016）. It showed statistically significant differences in ACLF3. In multivariate analysis, the age of the recipient（P = 0.001） and the diabetes of the donor（P = 0.007） were independent predictors for post-transplant overall survival for patients with ACLF3. Conclusion In patients with ACLF3, donor diabetes is an independent predictor for inferior overall survival after liver transplantation.

Abstract:Objective To analyze the impact of allograft category on the result of living related donor kidney transplantation (LRKT)and to evaluate the predominant donors. Methods A retrospective analysis of 104 recipients receiving LRKT from Apr. 2004 to Mar. 2008 was performed. Based on donor resource, all the recipient-donor pairs were divided into four groups: spousal donation group,parental donation group, sibling donation group and cousinly donation group. The observational parameters were selected for analysis, such as average post-transplant hospitalization dates, time for serum creatinine (Scr) back to normal level, Scr levels of every observational time point, incidence of major complications (infection, rejection, DGF) and recipient/graft survival rate. Results Recipient/graft survival rate of sibling donation group seemed higher. Recipients of sibling donation group seemed to have fewer post-transplant hospitalization dates, but higher rates of infection, while those of parental donation group seemed to have higher rates of rejection. Rates of rejection and infection of spousal donation group were lower than supposed. There was no statistically significant difference in time for Scr back to normal level and Set levels of every observational time point among these four groups. Conclusions The result of sibling donor renal transplantation is better, while short-term outcome of spouse donor renal transplantation is ideal, which is similar with parent or cousin donor renal transplantation. Except for human leukocyte antigen, aspects such as quality of donor kidney, predominance during operation and self-administration post-transplant are also the guarantee for the success.

Abstract:Objective To assess the physical impact on the donors and recipients of the live related renal transplantation.Methods Six live donor renal transplantations were taken from Jan.2000 to Dec.2002 and the donors and recipients were all followed up.The perioperation complications and the renal functions were dynamically observed.Results Among the 6 recipients,1 case failed in the cadaveric donor renal transplantation occurred the hyperacute rejection this time and continued in the hemodialysis status.5 recipients got the triple immunosuppressive therapy with normal renal functions.1 case occurred the acute rejection and another case occurred subclinical rejection,both recovered after antirejection therapy.No other complications occurred.Conclusion Live related renal transplantation provides significant advantages in terms of improved patients and graft survival with little impacts on donors.We should think highly of the preoperation physical evaluation and the postoperation follow-up.
The Effect of Donor-Recipient Body Surface Area Ratio on Donor Age and Donor Glomerular Filtration Rate in Chinese Patients Undergoing A Living-Donor Kidney Transplant

Abstract:Objectives: The purpose of this study was to evaluate the effect of donor-recipient body surface area ratio on donor age and donor glomerular filtration rate in living-donor kidney transplant.Materials and Methods: This retrospective study included 254 rejection-free patients who underwent their first living-donor kidney transplant at our center between April 2007 and April 2011. We performed multivariate linear regression and receiver operating characteristic curve analyses to determine independent associations and the cumulative effects on posttransplant graft function and outcomes in persons in China who had a living-donor kidney transplant.Results: In multivariate linear regression, donor age, donor estimated glomerular filtration rate, and donor-recipient body surface area ratio were independent predictors of 1-year graft function. Linear regression showed that correcting donor age by donor-recipient body surface area ratio increased the strength of the correlation between donor age and 1-year graft function. In the older group (donor age >= 45 y), the effect of donor-recipient body surface area ratio on graft function was stronger. By considering the 1-year donor estimated glomerular filtration rate in 2 groups (< 60 or >= 60 mL/min/1.73 m(2)), the cutoff values for corrected donor age was 55 years and donor estimated glomerular filtration rate before surgery was 113 mL/min/1.73 m(2).Conclusions: By correcting for donor-recipient body surface area ratio, donor age accurately predicted and correlated better with 1 year graft function. During preoperative evaluation donor and recipient body surface area matching may be useful.
Analysis on spouse donor renal transplantation

Abstract:Objective To analyze clinical outcomes of the spouse donor kidney transplantation for evaluating the security.Methods Sixty-three cases of living-renal transplantation were divided into two groups, spouse donor group (12 cases) and the other related donor group (53 cases). Twelve cases of spouse-donor(SD)renal transplantation were summarized, that were compared with the nospouse transplantation cases of the other living-related renal transplantation in the same period, which were similar in basic conditions and in immunosuppressant scheme. The observational parameters included average hospitalization time, rate of acute renal necrosis, acute rejection incidence within 1 year, serum creatinine at 7 d, 30 d and 1 year after transplantation.Results The recipient age of the 2 groups was (39±3)years and (37±3)years(P=0.05), dialysis time was (4.7±3.2)months and (4.4±2.9)months(P=0.78), the average hospitalization time was (20.9±8.3)d and (23.0±7.8)d(P=0.41). There was no significant difference between the spouse donor group and the no spouse related donor group. The acute rejection incidence within 1 year was 33.3%(4/12) in spouse group and 3. 9%(2/51) in the other related group, there was significant difference between the 2 groups(P＜0.05). The rate of acute renal necrosis was 16.7%(2/12) in spouse group and 3.9%(2/15)in no spouse related group, there was no significant different between the 2 groups(P＞0.05).SCr was (206.47±47.22)μmol/L and (163. 75±25.91)μmol/L in spouse group at post-operation 7 d and 30 d, and was (142.79±89.42)μmol/L and (119. 99±15.03)μmol/L in no spouse group. There was significant difference between the groups(P=0.02, P=0.00). One year after operation, SCr was (133. 40±6. 11)μmol/L in spouse group and (121. 00±34.12)μmol/L in no spouse group,there was no significant difference between the 2 groups (P=0.25).Conclusions Preoperative comprehensive assessment of the spouse donors and recipients renal transplantation is the guarantee for the success.Short-term outcomes of spouse donor renal transplantation is ideal, but rate of acute rejection within 1 year is higher than that of the other living-relative donor kidney transplantation, which dose not influence the long-time survival of spouse recipients.
Comparison of Graft Outcome Between Donation After Circulatory Death and Living-Donor Kidney Transplantation.

Abstract:BACKGROUND:Donation after cardiac death (DCD) and living-donor (LD) kidney transplantation are the main kidney transplantation types in China. But the outcome of DCD kidney transplantation compared with LD kidney transplantation remains unclear. METHODS:In this study, 325 DCD and 409 LD kidney transplantations were included. We retrospectively compared 3-year graft survival, death-censored graft survival, recipient survival, and graft function. All kidneys of the DCD group were procured from voluntary donation after the citizens' death by the Organ Procurement Organization (OPO) in the presence of the Red Cross, and the transplantation application was approved by the Organ Transplant Ethics Committee. RESULTS:The graft function at year 3 in the DCD group was superior to that of the LD group (eGFR: 71.14±22.28 vs 64.29±16.76 mL/min/1.73 m2; P < .001). After matching donor age, there was no significant difference between the paired DCD and LD group (eGFR: 62.22±18.50 vs 66.99±17.81 mL/min/1.73 m2; P = .068). The 3-year graft survival (94.7% vs 97.4%; P = .041) and recipient survival (97.2% vs 99.5%; P = .011) were a little worse in the total DCD group. However, once the DCD kidney transplantation recipients survived more than 2 months, graft and recipient survival rates were similar between the DCD and LD groups (97.7% vs 97.4%, P = .866 and 99.5% vs 99.0%, P = .466). These results were confirmed in an age-paired groups study. Severe infection was the main cause of graft loss and recipient death in the early stage of DCD transplantation. CONCLUSIONS:Medium- and long-term graft function of DCD kidney transplantation were comparable to LD kidney transplantation. Our results supported the continued use of DCD kidneys.

Abstract:Introduction This study identifies the effect of individual donor and recipient characteristics on graft survival in living-donor kidney transplantation (LDKT) using a recently described novel measure, kidney life years (KLYs). Materials and Methods The Organ Procurement and Transplantation Network/United Network for Organ Sharing database was used to identify first-time kidney-only LDKT recipients between 1987 and 2020 who did not experience death with a functioning graft (DWFG) and were not missing relevant information (n = 87,290). Patient characteristics were evaluated using Cox and multiple regression analyses, with the dependent variable being KLYs. An equation for expected KLYs based on patient characteristics was created using regression coefficients. The equation was validated using bootstrapped Pearson correlations and then applied to the DWFG group for comparison. Results Based on statistical significance from Cox and multiple linear regression analyses, 9 of the original 18 variables were selected for inclusion in the equation. Variables with notable impact included HLA match points (0.021 KLYs; 95% CI: [0.019,0.024]; P ≤ .001), Donor Age (-0.030 KLYs; 95% CI: [-0.035,-0.025]; P ≤ .001), and Donor African American Ethnicity (-2.356 KLYs; 95% CI: [-2.552,-2.159]; P ≤ .001). Equation validation was supported, given a negative correlation (r = -0.071; P ≤ .001) between expected KLY change and observed graft failure. Expected KLY change was found to be greater in those who eventually DWFG when compared with all other LDKTs (t = -5.735, P ≤ .001). Conclusions Increasing HLA match points may be more beneficial for graft longevity than minimizing donor age in comparisons using realistic between-donor differences. Additionally, greater average expected KLYs in those who ultimately DWFG may illustrate an opportunity for improved donor-recipient matching.

Risk Factors of Kidney Re-Transplantation Recipients and Graft Survival During the Long-Term Follow-Up

Abstract:Objective To investigate the risk factors of kidney re-transplantation recipients and graft survival during the long-term follow-up. Methods The clinical data of kidney transplant donors and recipients performed at the Kidney Disease Center of the First Affiliated Hospital of Zhejiang University from January 1, 1991 to December 31, 2017 were retrospectively analyzed. A total of 5 374 cases of first kidney transplantation and 37 cases of re-transplantation were included. In addition, the recipients of re-transplantation were divided into long-term survival group (19 cases, >5 years) and short-term survival group (18 cases, ≤5 years). The age of donor and recipient, the interval between first and second kidney transplantation, HLA mismatch and cold/warm ischemia time of donor and recipient in long-term and short-term survival groups were compared by group t test. Chi-square test was used to compare the gender of recipients, type of donor kidney, rate of positive panel reactive antibodies before and after re-transplantation, initial graft nephrectomy, immune induction before re-transplantation, delayed graft function (DGF) and acute rejection in two groups. The 1, 5 and 10-year patient/graft survival rates were calculated by Kaplan-Meier method. The Cox proportional hazard model was used to analyze the possible risk factors on long-term survival of transplanted kidney. P<0.05 was statistically significant. Results As of March 1, 2018, the median follow-up time of re-transplantation group was 152 months (11-323 months), 2 recipients died, 18 reipients suffered from renal allograft dysfunction and 17 patients had stable renal transplant function. The median follow-up time of the first transplantation group was 108.9 months (0.1-350.0 months). 459 cases died and 1 343 cases suffered from renal transplant failure. The 1, 5 and 10-year recipient/graft survival rates in the re-transplantation group were 86%/81%, 86%/62% and 82%/36%, and the 1, 5 and 10-year recipient/graft survival rates in the first transplantation group were 99%/98%, 93%/89% and 88%/80%. The 1, 5 and 10-year recipients survival rates of re-transplantation group were lower than those in the first transplantation group (χ2= 60.816, 25.110 and 43.900, P<0.05), and the 1-year recipients survival rate in the re-transplantation group was lower than the first transplantation group (χ2= 40.409, P<0.05). Univariate analysis showed that there were significant differences in DGF and acute rejection rates between long-term and short-term survival groups (χ2=4.039 and 4.748, P<0.05). Cox regression analysis showed that DGF and acute rejection were independent risk factors for long-term survival of transplanted kidneys in re-transplanted recipients (RR=4.317 and 4.571, P<0.05). Conclusions The survival rate of re-transplant recipients is lower than that of the first renal transplant recipients. DGF and acute rejection are independent risk factors affecting the survival of re-transplant recipients. Abstract:Objective To evaluate the effect of gender matching on the outcomes of livingdonor renal transplantation.Methods A total of 419 cases of living-donor renal transplantation in our center were divided into male-donor-male-recipient (MDMR) group,male-donor-female-recipient (MDFR) group,female-donor-male-recipient (FDMR) group,female-donor-female-recipient (FDFR)group.The outcomes including graft and patient survival,acute rejection and renal function were analyzed retrospectively.Results Compared to MDMR group,MDFR group and FDFR group had lower Scr [(80.7±17.9),(87.4±21.9) μmol/L vs (120.3±72.5) μmol/L,all P ＜ 0.05] and uric acid (UA) [(318.1 ± 86.4),(303.5 ± 66.9) μmol/L vs (358.4 ± 77.8) μmol/L,P ＜ 0.05] 6 months after operation.Compared to MDFR group,FDMR group had higher Scr[(117.7±27.4) μmol/L vs (80.7±17.9) μmol/L,P ＜ 0.01],UA [(371.0±92.4) μmol/L vs (318.1±86.4) μmol/L,P ＜ 0.05] and lower glomerular filtration rate (GFR) [(70.4± 17.8) ml/min vs (79.6± 18.9) ml/min,P ＜ 0.05].Compared to FDMR group,FDFR group had lower Scr [(87.4±21.9) μmol/L vs (117.7±27.4) μmol/L,P ＜ 0.01] and UA [(303.5±66.9)μmol/L vs (371.092.4) μmol/L,P＜ 0.01].Compared to MDFR group,FDFR group showed lower GFR [(72.4±25.3) ml/min vs (82.7± 18.7) ml/min,P ＜ 0.05] 1 year after operation.Compared to MDMR group,FDFR group showed lower UA [(322.9±69.7) μmol/L vs (376.0±66.2) μmol/L,P ＜ 0.05] 2 years after operation.Compared to FDMR group,FDFR group showed lower Scr [(88.7 ±27.0) μmol/L vs (112.7±27.8) μmol/L,P ＜ 0.05] and UA [(318.3 ±61.2) μmol/L vs (396.2± 100.3) μmol/L,P ＜ 0.05] 3 years after operation.5 years after operation,there were no significant differences in above indexes,the incidence of slow graft function,acute rejection and survival of graft and patient among groups.Conclusions Male recipients of female donors have the worst renal function while female recipients have better outcomes after operation.
Impact of Donor Age on Liver Transplant Outcomes in Patients with Acute-on-Chronic Liver Failure: A Cohort Study.

Abstract:Background/Aims:Liver transplantation is the most effective treatment for the sickest patients with acute-on-chronic liver failure (ACLF). However, the influence of donor age on liver transplantation, especially in ACLF patients, is still unclear. Methods:In this study, we used the data of the Scientific Registry of Transplant Recipients. We included patients with ACLF who received liver transplantation from January 1, 2007, to December 31, 2017, and the total number was 13,857. We allocated the ACLF recipients by age into group I (donor age ≤17 years, n=647); group II (donor age 18-59 years, n=11,423); and group III (donor age ≥60 years, n=1,787). Overall survival (OS), graft survival, and mortality were compared among the three age groups and the four ACLF grades. Cox regression was also analyzed. Results:The 1-, 3-, and 5-year OS rates were 89.6%, 85.5%, and 82.0% in group I; 89.4%, 83.4%, and 78.2% in group II; and 86.8%, 78.4%, and 71.4% in group III, respectively (p<0.001). When we analyzed the different effects of donor age on OS with different ACLF grades, in groups II and III, we observed statistical differences. Finally, the cubic spline curve told us that the relative death rate changed linearly with increasing donor age. Conclusions:Donor age is related to OS and graft survival of ACLF patients after transplantation, and poorer results were associated with elderly donors. In addition, different donor ages have different effects on recipients with different ACLF grades.

Abstract:Liver transplantation (LT) is the most effective way to save patients with acute-on-chronic liver failure (ACLF). However, the impact of donor diabetes mellitus (DM) on LT outcomes in patients with ACLF has not been fully investigated. We retrospectively analyzed data from the Scientific Registry of Transplant Recipients (SRTR) between January 1st, 2008 to December 31st, 2017 in this study. All the patients were divided into donors with DM and without DM group (DM: 1,394; non-DM: 11138). We compared the overall survival (OS) and graft survival (GS) across different estimated ACLF (estACLF) grades between two groups. There were 25.10% estACLF-3 patients in the entire cohort. And in estACLF-3 patients, 318 patients had DM donors. The estACLF-3 associated 5-year OS rate in the non-DM group was 74.6%, significantly better than that in the DM group, with corresponding survival rate at 64.9% (P < 0.001). Donor DM was an independent predictor for OS in the entire cohort as well as in estACLF-3 patients. Donor DM was associated with inferior outcomes of LT in patients with estACLF-3. However, the differences were not obvious in recipients with other estACLF grades.
The Safety Analysis of Living-Related Kidney Donors in Short Term after Transplantation

Abstract:OBJECTIVE:To evaluate the safety of living related donors in short term after transplantation.METHODS:Two hundred and fifty-one cases of living related donor kidney transplantation from May 2000 to July 2007 were analysed retrospectively. There were 117 male and 134 female aged from 22 to 72 years old, with a mean of 46.6 years old. The indexes were compared including serum creatinine (SCr), creatinine clearance (CCr), glomerular filtration rate (GFR) and quality of life before and after donation. Surgical complications were followed-up.RESULTS:Donors' SCr was (75.9 +/- 17.2) micromol/L before donation, (107.4 +/- 21.2) micromol/L on 7 d after donation, (130.4 +/- 58.2) micromol/L at the 1(st) month and (116.1 +/- 24.1) micromol/L at the 3(rd) month. There were significant difference between any 2 time points (P < 0.01). CCr was (94.4 +/- 17.5) ml/min before donation and (63.5 +/- 17.8) ml/min on 10 d after donation (P < 0.01). In 62 donors, total GFR was (82.4 +/- 21.8) ml/min before donation. On 10 d after donation, GFR of remaining kidney was (57.4 +/- 14.1) ml/min which was 34.7% higher than GFR of this kidney before donation (42.6 +/- 11.8) ml/min. There was no significant difference in quality of life before living related donors and non-donor populations (P = 0.116). Surgical complications included splenic rupture in 1 case, descending colon rupture in 1 case and wound infection in 5 cases.CONCLUSION:Living donor kidney transplantation is safe for donors, although part of indexes would vary within normal range during the early time after donation.
Safety Analysis of Living Related Live Donors in Kidney Transplantation

Abstract:Objective To analyze the clinical outcomes and evaluate the safety of living related live donors in kidney transplantation.Methods One hundre and thirty-two cases of living related donors were studied retrospectively for psychological and physiological parameters. The parameters including life quality, urinalysis, serum biochemistry tests, glomerular filtration rate (GFR) and endogenous creatinine clearance rate (CCr) were analyzed.Results There was no significant difference between living related donors and normal people in the aspect of life quality. In living related donors,SCr was (78.33±15.94)μmol/L before operation and was (108.49±19.88)μmol/L at 7 days postoperation, P＜0.05. SCr was (112.47±20. 38)μmol/L at 6 months post-operation. There was no significant difference in SCr levels between 7 days and 6 months post-operation (P=0. 109). CCr was (95.80±20.92)ml/min in pre-operation and was (57.36±14. 92)ml/min at 7 days post-operation,P＜0.05. CCr was (65. 49±8. 25) ml/min at 6 months post-operation. There was no significant difference in CCr between 7 days and 6 months post-operation.The pre-operative total GFR was(74.08±18.51)ml/min. Of which, the right kidney GFR was (38.43±10.33)ml/min. The residual right kidney GFR was (56. 49±13. 01 ) ml/min 6 months after operation, which decreased 17. 59 ml/min (23.8%) compared with pre-operative total GFR (P＜0.05) and increased 18. 06 ml/min (47.0%) compared to the pre-operative right kidney GFR. Surgical complications included 1 case of splenectomy, 1 case of descending colon rupture and 5 cases of wound fat liquefactions.Conclusion Pre-operatively systemic psychological and physiological evaluation on living related donors, detailed consent consultation, standardized operating techniques, careful perioperative management and strict follow-up can improve the safety of living related donors.

Abstract:Objective To evaluate the efficacy of living related kidney transplants (LRKT). Methods Retrospectively clinical results of 25 LRKT were reviewed and analyzed. Results Living related donors underwent nephrectomy without any complications and the donors have had normal renal function on follow up studies.All the renal grafts survived.Two recipients experienced delayed graft function recovery,Both the patients and the grafts have been surviving. Conclusions The patient and graft survival rates of LRDT are better than cadaveric donor transplantation.Living related donor serves as an alternate way of kidney source.

Abstract:Introduction: The demographics of donor and recipient candidates for kidney transplantation (KT) have substantially changed. Recipients tend to be older and polymorbid and KT to suboptimal recipients is associated with delayed graft function (DGF), prolonged hospitalization, inferior long-term allograft function, and poorer patient survival. In parallel, donors are also older, suffer from several comorbidities, and donations coming from circulatory death (DCD) predominate, which in turn leads to early and late complications. However, it is unclear how donor and recipient risk factors interact. Methods: In this retrospective cohort study, we assess the impact of a KT from suboptimal donors to suboptimal recipients. We focused on: 1) DGF; 2) hospital stay and number of dialysis days after KT and 3) allograft function at 12 months. Results and discussion: Among the 369 KT included, the overall DGF rate was 25% (n = 92) and median time from reperfusion to DGF resolution was 7.8 days (IQR: 3.0-13.8 days). Overall, patients received four dialysis sessions (IQR: 2-8). The combination of pre-KT anuria (<200 ml/24 h, 32%) and DCD procurement (14%) was significantly associated with DGF, length of hospital stay, and severe perioperative complications, predominantly in recipients 50 years and older.

Abstract:INTRODUCTION: In Japan, approximately 90% of kidney transplantations involve living donors who are relatives. Selection of a living donor from potential family member donors could affect the entire family. However, reports focusing on preliving-related kidney transplant (LRKT) family functioning are lacking. Family functioning comprises ways that family members communicate and cooperate with each other. The Family Assessment Device (FAD) was used to measure family functioning from the perspective of donors and recipients just prior to LRKT.METHOD: A total of 122 donor-recipient pairs (244 participants in total) who planned to have LRKT were recruited consecutively from July 2020 to July 2021 and included in the analysis.RESULTS: There was no significant difference in FAD scores between donors and recipients, with approximately 20% of both groups reporting poor family functioning. Differences in family functioning according to types of relatives were shown in recipients. The rate of poor family functioning was significantly lower in the spouse group than in the parent-to-child group (recipient: 6.6%, 29.3%; donor: 8.2%, 34.1%, respectively). However, agreement regarding good or poor family functioning assessment was high in the parent-to-child pairs and low in the spouse pairs.DISCUSSION: Most LRKT donors and recipients reported good family functioning; however, some perceived poor family functioning. Evaluations by donors did not always align with that of recipients, especially among spouse pairs. It is important to treat them as independent entities. Abstract:Objectives To investigate the safety and feasibility of living relative kidney transplantation(LRKT)and evaluate the clinical effects of LRKT.Methods The clinical data of 12 LRKT were retrospectively analyzed.Results Living relative donors underwent nephrectomy without any complications.They were discharged at 7 to 9 days postoperation.One recipient experienced delayed graft function recovery.Acute rejection occurred in one recipient.The renal function in all recipients recovered well.In 8recipients with 6 months follow up,BUN,Cr and creatinine clearance were 8.2 mmol/L,97.2μmol/Lkidney from living relative donor is higher than that from cadaveric kidney.

Abstract:Introduction: Short-term renal graft survival has been increased dramatically. However, it is still not clear if long-term graft survival improved over time. Methods: We performed a retrospective graft survival analysis of kidney transplant patients registered in the United Network of Organ Sharing (UNOS) registry from 1988 to 2007 with follow-up to 2012. Only primary solitary adult deceased donor kidney transplants were selected. Three eras of transplantation were divided: era1(1988-1994), era2(1995-2001) and era3(2002-2007). Results: There were proportionately more older recipients and donors, African American recipients and donors, and higher recipient body mass index, HLA mismatch and PRA in recent cohorts.Table: No Caption available.The one-year death-censored graft survival increased from 80.1% to 94.4% and five-year survival from 78.0% to 86.4%. Ten-year (1988-2002) survival increased from 55.9% to 65.6%. Compared to era1, the one-year and five-year death-censored graft survival of era3 increased significantly 9% and 7.2% respectively.Figure: No Caption available.It is worth to be noted that the one-year survival improved mainly before 1997-11.1% of total 14.3%. And the five-year and ten-year (1988-2002) survival stayed stable before 1996 and elevated 7.2% and 9.2% respectively after that. This phenomenon was consistent with the evolvement of immunosuppressant. Death with graft function (DWGF) was the most important reason for graft failure that accounted for 42%. Conclusions: The long-term survival of kidney grafts was underestimated for long time. It is a feasible path to improve further long-term outcome by multidisciplinary cooperation to reduce DWGF.
