Abstract:Recently we demonstrated the necessity of microglial activation, and subsequent secretion of proinflammatory cytokines (PIC) within the brain, in peripheral-angiotensin-infusion hypertension. Here, we examine the involvement of microglial activation in a model of low-renin hypertension. Mice expressing human renin under a neuron-specific synapsin promoter were crossed with mice expressing human angiotensinogen under its own promoter. Resulting double-transgenic (“sRA”) mice exhibit moderate hypertension (30 mmHg), extreme polydipsia and polyuria (8x controls), elevated basal metabolic rate (1.25x controls), and increased sodium intake due to elevated brain renin-angiotensin system (RAS) activity (2x controls) and suppressed circulating RAS activity (0.3x controls). sRA mice exhibited enhanced microglial activation and significant increases in angiotensin type 1 receptor (1.6-fold, P<0.05) and interleukin-1β (1.5-fold, P<0.05) mRNA levels in the hypothalamic paraventricular nucleus when compared to littermate control mice. Together, our data suggest that increased brain-RAS signaling is associated with mciroglial activation and PIC, independent of the status of the circulating RAS. Ongoing studies are examining the necessity of mciroglial activation and PIC in the blood pressure, hydromineral, and metabolic phenotypes of the sRA model. Abstract:Purpose: Our previous studies have established that activation of microglial cells and production of proinflammatory cytokines (PIC) within the PVN are two critical events in the establishment of chronic angiotensin (Ang) II-induced neurogenic hypertension. These have led us to hypothesize that brain microglial cells are a primary target of Ang II, initiating a sequence of cellular and physiological events resulting in sympathetic nervous system activation and hypertension. Methods: Microglial cells from the hypothalamus of Sprague-Dawley rats were established in primary culture. Their phenotype was confirmed by the presence of IBa1 immunoreactivity and high levels of CD11b mRNA, markers for microglia. They were used to determine the effects of, Ang II, prorenin and aldosterone (ALDO) on cytokine production. Neurogenic hypertensive mice were generated using decoxycorticosterone acetate (DOCA)-salt model (DOCA 50 mg s.c., 0.15 M NaCl in drinking water for 3 weeks). The PVN was dissected to determine PIC expression at the end of the protocol. Results: Treatment of cultures with Ang II (100 nM) or ALDO (1 nM) for 24 hr increased the expression of PIC mRNAs, e.g. for tumor necrosis factor (TNF)α. Ang II and ALDO elicited respective 2- and 3-fold increases (P<0.05). More strikingly, we found that prorenin (20 nM, 24 hr) elicited highly significant 300- and 400-fold increases in the expression of TNFα and interleukin (IL)-1β mRNAs, respectively (P<0.05). These effects of prorenin were associated with respective 16- and 14-fold increases in NFκb1 and NFκbia mRNA levels, which are two subunits of the transcription factor NFκb complex. We previously demonstrated that DOCA mice exhibit a similar hypertensive phenotype as double transgenic hypertensive sRA mice, which display increased levels of PIC and activation of microglia in the PVN. Consistently, DOCA-salt mice exhibit respective 1.5, 1.08 and 1.09 fold increases in the expression of mRNAs for IL-1β, TNFα and IL-6 in the PVN compared to control mice (n=3-4). Conclusion: These data indicate that brain microglial cells are the primary target of RAAS components in promoting PIC production, which in turn contribute to hypertension development.

Abstract:MicroRNAs (miRs) are short RNAs that primarily reduce protein abundance by base-paring with their target mRNA. The role of most miRs in the development of hypertension remains unknown. We performed a deep sequencing analysis of miR expression in human kidney biopsies with hypertensive nephrosclerosis or without any significant injury. miR-192-5p was one of the most abundant miRs detected and was down-regulated in hypertensive nephrosclerosis. Previous studies have shown that miR-192-5p targets the beta 1 subunit of Na/K-ATPase which drives renal tubular reabsorption. We hypothesized that miR-192-5p in the kidney protects against hypertension. We used the Dahl salt sensitive (SS) rat and a congenic rat SS.13BN26 (L26) with reduced salt sensitivity as well as Mir192 knockout mice (KO) to test this hypothesis. SS rats had a decreased level of abundance of miR-192-5p in the renal cortex compared to the L26 rats (n=9, p<0.05). The protein abundance of the beta 1 subunit of Na/K-ATPase was higher in the SS rat compared to the L26 rat (n= 3, p<0.05). Treatment with anti-miR-192-5p, delivered directly into the kidney through renal artery injection, in uninephrectomized L26 rats significantly exacerbated hypertension. Mean arterial blood pressure (MAP) of L26 rats treated with anti-miR-192-5p reached 151+/-5 mmHg at day 14 post anti-miR treatment and 4% NaCl (HS) diet, which was significantly higher than L26 rats treated with a control anti-miR and HS (135+/-5 mmHg, n=6 and 8, p<0.05). Tissues were collected in additional groups of rats at 9 days after anti-miR injection, which was just before MAP became significantly different between the two groups, for analysis of Na/K-ATPase activity. Na/K-ATPase activity was increased in the renal cortex of rats treated with anti-miR-192-5p compared to control anti-miR (9.8 +/-1.8μmole/min/μg vs 7.2+/-1.3μmole/min/μg, n=5 and 6, p<0.05). Furthermore, Mir192 KO mice treated with 1μg/Kg/min of Angiotensin II and HS for 14 days exhibited an increased MAP compared to wild-type (WT) mice (190+/-4 mmHg vs 167+/-12 mmHg; n= 3 WT and 5 KO, p<0.05). In conclusion, miR-192, particularly miR-192-5p in the kidney, confers significant protection against the development of hypertension.

Abstract:Chronic kidney disease (CKD) has a strong genetic component; however, the underlying pathways are not well understood. Dahl SS/Jr rats spontaneously develop CKD with age and are used to investigate the genetic determinants of CKD. However, there are currently several genetically diverse Dahl SS rats maintained at various institutions, and the extent to which some exhibit age-related CKD is unclear. We assessed glomerulosclerosis (GS) and tubulointerstitial fibrosis (TIF) in 3- and 6-month-old male and female SS/JrHsdMcwi, BN/NHsd/Mcwi (Brown-Norway (BN)), and consomic SS-Chr 1BN/Mcwi (SS.BN1) rats, in which chromosome 1 from the BN rat was introgressed into the genome of the SS/JrHsdMcwi rat. Rats were fed a 0.4% NaCl diet. GS (31±3 vs. 7±1%) and TIF (2.3±0.2 vs. 0.5±0.1) were significantly greater in 6- vs. 3-month-old SS/JrHsdMcwi rats, and CKD was exacerbated in males. GS was minimal in 6- and 3-month-old BN (3.9±0.6 vs. 1.2±0.4%) and SS.BN1 (2.4±0.5 vs. 1.0±0.3%) rats, and neither exhibited TIF. In SS/JrHsdMcwi and SS.BN1 rats, mean arterial blood pressure (BP) was significantly greater in 6- vs. 3-month-old SS/JrHsdMcwi (162±4 vs. 131±2 mmHg) but not SS.BN1 (115±2 vs. 116±1 mmHg) rats. In 6-month-old SS/JrHsdMcwi rats, BP was significantly greater in females. RNAseq analysis revealed that inflammatory pathways were upregulated in isolated medullary thick ascending (mTAL) tubules in 7-week-old SS/JrHsdMcwi rats, prior to the development of tubule pathology, vs. SS.BN1 rats. In summary, SS/JrHsdMcwi rats exhibit robust age-related progression of mTAL abnormalities, CKD, and hypertension, and gene(s) on chromosome 1 have a major pathogenic role in such changes.

Abstract:BACKGROUND:In spite of extensive study, the mechanisms for salt sensitivity of BP in humans and rodent models remain poorly understood. Several microRNAs (miRNAs) have been associated with hypertension, but few have been shown to contribute to its development. METHODS:We examined miRNA expression profiles in human kidney biopsy samples and rat models using small RNA deep sequencing. To inhibit an miRNA specifically in the kidney in conscious, freely moving rats, we placed indwelling catheters to allow both renal interstitial administration of a specific anti-miR and measurement of BP. A rat with heterozygous disruption of the gene encoding endothelial nitric oxide synthase (eNOS) was developed. We used bioinformatic analysis to evaluate the relationship between 283 BP-associated human single-nucleotide polymorphisms (SNPs) and 1870 human miRNA precursors, as well as other molecular and cellular methods. RESULTS:Compared with salt-insensitive SS.13BN26 rats, Dahl salt-sensitive (SS) rats showed an upregulation of miR-214-3p, encoded by a gene in the SS.13BN26 congenic region. Kidney-specific inhibition of miR-214-3p significantly attenuated salt-induced hypertension and albuminuria in SS rats. miR-214-3p directly targeted eNOS. The effect of miR-214-3p inhibition on hypertension and albuminuria was abrogated in SS rats with heterozygous loss of eNOS. Human kidney biopsy specimens from patients with hypertension or hypertensive nephrosclerosis showed upregulation of miR-214-3p; the gene encoding miR-214-3p was one of several differentially expressed miRNA genes located in proximity to human BP-associated SNPs. CONCLUSIONS:Renal miR-214-3p plays a functional and potentially genetic role in the development of hypertension, which might be mediated in part by targeting eNOS.
Sodium-Independent Dietary Effects on Renal Immune Cell Infiltration in Salt-Sensitive Hypertension.

Abstract:Recent studies have shown sodium-independent dietary effects to be important in the development of Dahl SS hypertension and renal disease. Dahl SS/JrHsdMcwiCrl (SS/Crl) rats fed a grain-based diet (Teklad 5L2F) were less susceptible to salt-induced blood pressure elevation (116.5±1.2 vs 141.9±14.4 mmHg) and albuminuria (21.7±3.5 vs 162.9±22.3 mg/day) compared to SS/JrHsdMcwi (SS/Mcw) rats fed a 4.0% high salt casein-based diet (AIN-76A) for 3 weeks (n=6-8/group). Given the role of immunity in hypertension, SS/Crl rats displayed significantly less total CD45+ leukocyte infiltration in the kidney than SS/Mcw (6.1±0.9 vs 11.6±2.6 x10 6 cells/kidney). Reductions were observed in all subsequent immune cell populations, including CD3+ T cells (6.8±0.8 vs 14.4±2.2 x10 5 ), CD45R+ B cells (1.5±0.4 vs 6.7±2.2 x10 5 ), and CD11b/c+ antigen presenting cells (5.0±0.7 vs 9.2±2.1 x10 6 cells/kidney) in SS/Crl versus SS/Mcw. The SS/Crl immune system appears to be less activated, as demonstrated by a respective 44.8% and 66.8% reduction in CD4+CD25+ and CD8+CD25+ T cells, as well as a 40.4% decrease in TNFα-producing CD4+ T cells. Upon taking a discovery approach, RNA sequencing data (n=4 pools of 3 rats each) revealed genes related to hematopoietic cell lineage, the complement and coagulation cascade, B cell signaling, and primary immunodeficiency. Specifically, RNA expression of components of the NLRP3 inflammasome pathway were upregulated, including TLR4 (44.0%), NLRP3 (33.8%) and caspase-1 (26.4%), with a similar trend for IL-1β (25.4%), in the renal medulla of SS/Mcw rats fed high salt. This coincided with increased protein expression of NLRP3 (1.5-fold) and ASC (3.2-fold), together demonstrating increased NLRP3 inflammasome mRNA and protein expression in SS/Mcw rats after high salt challenge. Interestingly, RNA expression of these NLRP3 inflammasome proteins were significantly reduced in SS/Crl versus SS/Mcw rats fed high salt, including TLR4 (39.2%), NLRP3 (31.2%), caspase-1 (32.8%) and IL-1β (39.2%). These data indicate that the NLRP3 inflammasome may be a key mechanistic step in determining how these non-sodium components of the diet affect immunity, and warrant further studies to elucidate its role on salt-sensitive hypertension and renal damage.

Abstract:Chronic kidney disease (CKD) mediates pathological changes in heart and vascular tissues, yet many molecular mechanisms by which renal injury directly affects cardiovascular pathology (CVD) are unknown. miRNAs are proposed to be powerful regulators of pathophysiological signaling networks; having the potential to radically change the landscape of mRNA transcript expression and protein abundance in the context of health and disease. We have profiled the expression of miRNAs in a rat model of CKD – the 5/6 nephrectomy (5/6Nx) – with the objective to identify miRNAs with strong effects on CKD-mediated CVD. One such miRNA, miR-146b-5p (miR-146b), is highly upregulated in 5/6Nx rats compared to sham-operated controls at 7-weeks post-5/6Nx in both the kidney (5.4 fold increase; p<0.05) and the heart (1.6 fold increase, p<0.05), a time-point at which we see maximal renal and cardiac pathology. To examine the effects of miR-146b upregulation in our model of CKD, we performed RNA sequencing on cardiac tissue at 7 weeks post-5/6Nx in both wild-type (WT) Sprague-Dawley (SD) rats and a unique CRISPR-Cas9-mediated miR-146b knockout strain (miR-146b -/- ) created on the SD background. Of 3,018 genes differentially regulated by more than 1.5-fold (absolute value) in miR-146b -/- vs. WT rats after 5/6Nx, 135 had significantly altered expression (adjusted p-value <0.05). We filtered all differentially expressed genes through Ingenuity Pathway Analysis to identify affected signaling networks in miR-146b -/- vs. WT rats that correlate with observed pathological phenotypes. The expression pattern of differentially regulated genes in these networks indicates predicted activation of pathways that contribute to tissue fibrosis and mineralization, reactive oxygen species signaling, and the inflammatory and immune responses. These pathways are enriched with predicted miR-146b target genes and correlate closely with observed phenotypes in miR-146b -/- 5/6Nx rats; including increased renal fibrosis and mineralization of cardiovascular tissue, and the well-known role of miR-146b in negative feedback regulation of NFκB signaling. These preliminary analyses uncover unique networks of genes regulated by miR-146b that may contribute to CKD-mediated CVD.

Abstract:The Dahl salt-sensitive (SS) rat is an established model of SS hypertension and renal damage. In addition to salt, other dietary components were shown to be important determinants of hypertension in SS rats. With previous work eliminating the involvement of genetic differences, grain-fed SS rats from Charles River Laboratories (SS/CRL; 5L2F/5L79) were less susceptible to salt-induced hypertension and renal damage compared with purified diet-fed SS rats bred at the Medical College of Wisconsin (SS/MCW; 0.4% NaCl, AIN-76A). With the known role of immunity in hypertension, the present study characterized the immune cells infiltrating SS/MCW and SS/CRL kidneys via flow cytometry and RNA sequencing in T-cells isolated from the blood and kidneys of rats maintained on their respective parental diet or on 3 weeks of high salt (4.0% NaCl, AIN-76A). SS/CRL rats were protected from salt-induced hypertension (116.5±1.2 versus 141.9±14.4 mm Hg), albuminuria (21.7±3.5 versus 162.9±22.2 mg/d), and renal immune cell infiltration compared with SS/MCW. RNA-seq revealed >50% of all annotated genes in the entire transcriptome to be significantly differentially expressed in T-cells isolated from blood versus kidney, regardless of colony or chow. Pathway analysis of significantly differentially expressed genes between low and high salt conditions demonstrated changes related to inflammation in SS/MCW renal T-cells compared with metabolism-related pathways in SS/CRL renal T-cells. These functional and transcriptomic T-cell differences between SS/MCW and SS/CRL show that dietary components in addition to salt may influence immunity and the infiltration of immune cells into the kidney, ultimately impacting susceptibility to salt-induced hypertension and renal damage.

Abstract:Renal medullary fibrosis is a pathological hallmark of Dahl salt-sensitive (SS) rats fed a high-salt diet. The development of fibrosis is localized and progressive. We examined alterations of microRNA expression in medullary thick ascending limbs (mTALs) during the progression of fibrosis within a kidney. mTALs were isolated using laser capture microdissection from fibrotic and non-fibrotic regions of the same kidney in SS rats fed a 4% NaCl diet for 14 days (n=5) or from SS rats maintained on a 0.4% NaCl diet (n=2). microRNA expression profiles were examined using next-generation sequencing. microRNA abundance profiles were highly reproducible across rats (r 2 =0.93 to 0.99). Detectable microRNAs included 130 known rat microRNAs, 64 microRNA homologs, 89 newly predicted rat microRNAs, and 13 newly predicted microRNA homologs. Four known rat microRNAs, 4 newly predicted rat microRNAs, and 1 microRNA homolog exhibited p values of less than 0.1 (paired t-test) between mTALs isolated from fibrotic and non-fibrotic regions in rats fed the high-salt diet (n=5). Seventeen microRNAs exhibited p values of less than 0.05 (unpaired t-test) in mTALs isolated from non-fibrotic kidney regions of SS rats fed the high-salt diet (n=5) compared to mTALs from SS rats fed the 0.4% NaCl diet (n=2). Thirteen microRNAs were identified when mTALs isolated from fibrotic regions were compared to SS rats fed the 0.4% NaCl diet, 6 of which overlapped with microRNAs identified from non-fibrotic regions. The 4 known rat microRNAs identified from the comparison of fibrotic and non-fibrotic regions within a kidney included 2 members of the miR-30 family (miR-30a and miR-30b-5p). Both miR-30’s were down-regulated in mTALs isolated from fibrotic regions compared to non-fibrotic regions. We previously found that miR-30b was down-regulated in human kidney epithelial cells treated with the pro-fibrotic factor TGFβ1. miR-30a or miR-30b was not identified in comparisons of SS rats on the 4% and 0.4% NaCl diets. The study was the first to examine genome-wide molecular profiles in tissue regions in different stages of injury progression within a kidney. The result suggests that down-regulation of the miR-30 family in mTALs might contribute to the progression of renal medullary fibrosis in SS rats.
Single-nucleus Sequencing Reveals Endothelial Cell-specific Dysregulation in the Kidney in Multiple Models of Hypertension

Abstract:The kidney plays a crucial role in the onset and progression of hypertension. To investigate the roles of different renal cell types in hypertension, we applied single-nucleus sequencing and spatial transcriptome sequencing to kidney samples from three well-established hypertension models: C57BL/6 mice treated with Ang II, the Dahl salt-sensitive (SS) rat on high-salt diets, and the spontaneously hypertensive rat (SHR), along with their respective controls including Sprague-Dawley (SD) and Wistar Kyoto (WKY) rats. Our dataset comprised 179,637 nuclei from 24 samples across 12 experimental conditions, identifying 17 major cell types including various epithelial cells, endothelial cells, vascular smooth muscle cells, fibroblasts, and immune cells. At baseline, differentially expressed genes between the SS and SD groups were specifically enriched in endothelial cell pathways related to vascular development. This included expression deficiencies in genes involved in angiogenesis in SS rats, such as Arhgap24 and Nox4. Compensatory expression changes in these genes were observed in the SS rats after a high-salt diet and in 26-week-old SHR, but not in their respective controls. These genes were also associated with multiple blood pressure-associated single-nucleotide polymorphisms (SNPs). Furthermore, changes in endothelial cell communication were most prominent upon stimuli, with impaired PTPRM (Protein Tyrosine Phosphatase Receptor Type M) signaling being a robust feature in hypertension models, involving angiogenic endothelial cells in the SS model and proliferating capillary endothelial cells in the SHR model. Our study highlights the critical role of renal endothelial cell angiogenic dysfunction in the pathogenesis of hypertension and suggests a link between specific renal endothelial cell subtype dysregulation and hypertension development.

Abstract:Background: Chronic kidney disease (CKD) is a significant risk factor for pulmonary hypertension (PH), a complication that adversely affects patient prognosis. However, the mechanisms underlying this association remain poorly understood. A major obstacle to progress in this field is the lack of a reliable animal model replicating CKD-PH. Methods: This study aimed to establish a stable rat model of CKD-PH. We employed a combined approach, inducing CKD through a 5/6 nephrectomy and concurrently exposing the rats to a high-salt diet. The model's hemodynamics were evaluated dynamically, alongside a comprehensive assessment of pathological changes in multiple organs. Lung tissues and serum samples were collected from the CKD-PH rats to analyze the expression of angiotensin-converting enzyme 2 (ACE2), evaluate the activity of key vascular components within the renin-angiotensin-aldosterone system (RAAS), and characterize alterations in the serum metabolic profile. Results: At 14 weeks post-surgery, the CKD-PH rats displayed significant changes in hemodynamic parameters indicative of pulmonary arterial hypertension. Additionally, right ventricular hypertrophy was observed. Notably, no evidence of pulmonary vascular remodeling was found. Further analysis revealed RAAS dysregulation and downregulated ACE2 expression within the pulmonary vascular endothelium of CKD-PH rats. Moreover, the serum metabolic profile of these animals differed markedly from the sham surgery group. Conclusions: Our findings suggest that the development of pulmonary arterial hypertension in CKD-PH rats is likely a consequence of a combined effect: RAAS dysregulation, decreased ACE2 expression in pulmonary vascular endothelial cells, and metabolic disturbances.
High-Salt Diet Aggravates Endothelial-to-Mesenchymal Transition in Glomerular Fibrosis in Dahl Salt-Sensitive Rats

Abstract:Background: Both diabetic and hypertensive nephropathy eventually progress to glomerulosclerosis. Previous studies revealed a potential role of endothelial-to-mesenchymal transition (EndMT) in the pathophysiology of glomerulosclerosis in diabetic rats. Therefore, we hypothesized that EndMT was also involved in the development of glomerulosclerosis in salt-sensitive hypertension. We aimed to explore the effects of high-salt diet on endothelial-to-mesenchymal transition (EndMT) in glomerulosclerosis in Dahl salt-sensitive (Dahl-SS) rats. Methods: Eight-week-old male rats were fed high-salt (8%NaCl; DSH group) or normal salt (0.3%NaCl; DSN group) for eight weeks, with systolic blood pressure (SBP), serum creatinine, urea, 24-hour urinary protein/sodium, renal interlobar artery blood flow, and pathological examination measured. We also examined endothelial-(CD31) and fibrosis-related protein(α-SMA) expressions in glomeruli. Results: High-salt diet increased SBP (DSH vs. DSN, 205.2 ± 8.9 vs. 135.4 ± 7.9 mm Hg, P < 0.01), 24-hour urinary protein (132.55 ± 11.75 vs. 23.52 ± 5.94 mg/day, P < 0.05), urine sodium excretions (14.09 ± 1.49 vs. 0.47 ± 0.06 mmol/day, P < 0.05), and renal interlobar artery resistance. Glomerulosclerosis increased (26.1 ± 4.6 vs. 7.3 ± 1.6%, P < 0.05), glomerular CD31 expressions decreased while α-SMA expression increased in DSH group. Immunofluorescence staining showed that CD31 and α-SMA co-expressed in glomeruli of the DSH group. The degree of glomerulosclerosis negatively correlated with CD31 expressions (r = -0.823, P < 0.01) but positively correlated with α-SMA expressions (r = 0.936, P < 0.01). Conclusions: We demonstrated that a high-salt diet led to glomerulosclerosis involving the EndMT process, which played an essential role in glomerulosclerosis in hypertensive Dahl-SS rats.

Abstract:A high-salt diet often leads to a local intrarenal increase in renal hypoxia and oxidative stress, which are responsible for an excess production of pathogenic substances. Here, Wistar Kyoto/spontaneous hypertensive (WKY/SHR) rats fed a high-salt diet developed severe proteinuria, resulting from pronounced renal inflammation, fibrosis and tubular epithelial cell apoptosis. All these were mainly non-pressure-related effects. Hsp90β, TGF-β, HIF-1α, TNF-α, IL-6 and MCP-1 were shown to be highly expressed in response to salt loading. Next, we found that Hsp90β might play the key role in non-pressure-related effects of salt loading through a series of cellular signalling events, including the NF-κB, p38 activation and Bcl-2 inactivation. Hsp90β was previously proven to regulate the upstream mediators in multiple cellular signalling cascades through stabilizing and maintaining their activities. In our study, 17-dimethylaminoethylamino-17-demethoxygeldanamycin (17-DMAG) or Hsp90β knockdown dramatically alleviated the high-salt-diet-induced proteinuria and renal damage without altering blood pressure significantly, when it reversed activations of NF-κB, mTOR and p38 signalling cascades. Meanwhile, Co-IP results demonstrated that Hsp90β could interact with and stabilize TAK1, AMPKα, IKKα/β, HIF-1α and Raptor, whereas Hsp90β inhibition disrupted this process. In addition, Hsp90β inhibition-mediated renal improvements also accompanied the reduction of renal oxidative stress. In conclusion, salt loading indeed exhibited non-pressure-related impacts on proteinuria and renal dysfunction in WKY/SHR rats. Hsp90β inhibition caused the destabilization of upstream mediators in various pathogenic signalling events, thereby effectively ameliorating this nephropathy owing to renal hypoxia and oxidative stress.

Abstract:BACKGROUND/AIMS:To investigate the change of intrarenal renin-agiotensin system (RAS) and its role in high-salt induced hypertension. METHODS:Wistar rats were divided into normal-salt (NS), high-salt diet (HS) and high-salt diet with Losartan group (HS+L), for 6 weeks. Systolic blood pressure (SBP) was monitored. Blood and urine samples were collected every 2 weeks. Angiotensinogen (AGT) was measured by ELISA. AGT mRNA and protein were measured by real-time PCR and immunohistochemistry. Renin activity and angiotensin II (Ang II) were measured by radioimmunoassay. RESULTS:HS versus NS group, SBP increased from 2(nd) week (P<0.05), urinary protein increased at 6(th) week (P<0.05). Although plasma renin, AGT and Ang II had no significant changes (P>0.05), renal cortex renin, AGT, and Ang II increased significantly in HS (P<0.05). In HS+L, Losartan failed to reduce SBP (P>0.05) but abolished the increase of proteinuria (P<0.01), renal cortex renin, AGT, Ang II and urinary AGT reduced (P<0.05) while plasma renin, AGT and Ang II enhanced (P<0.05) when compared with HS. Urinary AGT was positively correlated with renal AGT (r=0.592, P <0.01) and Ang II (r=0.726, P <0.01). CONCLUSION:Inappropriate response of the renal RAS to a high salt diet may contribute to hypertension and renal damage, and urinary AGT could reflect intrarenal RAS activity.
Crowding Out of Long-Term Care Insurance: Evidence from European Expectations Data

Abstract:Long-term care (LTC) is the largest insurable risk that old-age individuals face in most western societies. However, the demand for LTC insurance is still ostensibly small in comparison with the financial risk. One explanation that has received limited support is that expectations of either 'public sector funding' and 'family support' crowd out individual incentives to seek insurance. This paper aims to investigate further the aforementioned motivational crowding-out hypothesis by developing a theoretical model and by drawing on an innovative empirical analysis of representative European survey data containing records on individual expectations of LTC funding sources (including private insurance, social insurance, and the family). The theoretical model predicts that, when informal care is treated as exogenously determined, expectations of both state support and informal care can potentially crowd out LTC insurance expectations, while this is not necessarily the case when informal care is endogenous to insurance, as happens when intra-family moral hazard is integrated in the insurance decision. We find evidence consistent with the presence of family crowding out but no robust evidence of public sector crowding out.

Abstract:NLRP3 inflammasome is formed by NLRP3, the adaptor ASC, and caspase-1, functions as a sensor of danger signals and triggers processing and release of interleukin-1beta (IL-1β). Recent animal studies demonstrate that NLRP3 inflammasome promotes renal inflammation. However, little is known on the role of NLRP3 in hypertension. We have investigated the effect of NLRP3 inflammasome on blood pressure, plasma renin activity and concentration (PRA and PRC), renal and cardiac hypertrophy in NALP3 and ASC deficient mice on which the two-kidney, one clip (2K1C) and the deoxycorticosterone-acetate (DOCA)/salt models were applied. MBP, PRA, PRC, and HW/BW were significantly increased at week 12 in WT-2K1C mice compared to the sham. Neither NLAP3-KO nor ASC-KO 2K1C treated mice developed hypertension and had lower circulating levels of PRA and PRC and serum amyloid A (SAA) and IL6 compared to the control. RNA levels of SAA, NLRP3, IL1β and IL1α were increased in the ischemic kidney in C57BL/6J mice. Administration of anti-IL1β antibody to the WT mice attenuated the increases of blood pressure and renin in 2K1C mice. With chronic administration of DOCA/salt, MBPs in both NLRP3-KO and WT mice were comparable and not significantly increased compared to tap water group. PRA and PRC in both NALP3-KO and WT mice were significantly suppressed by DOCA/salt. HW/BW and KW/BW in both DOCA/salt treated NALP3-KO and WT mice were significantly increased. DOCA/salt induced hypokalemia was comparable between Nlrp3 KO and WT groups. However, the heart and kidney index in DOCA/salt NLRP3-KO mice was significantly lower than that in DOCA/salt WT mice. Our data show that NLRP3 mediated IL1β is linked to development of renovascular hypertension and suggest that a novel target for the treatment of hypertension. The results also implicate that NLRP3 contributes to the development of cardiac and renal hypertrophy independently of blood pressure in the DOCA/salt model.

Abstract:Clinical trials of renal denervation (RDN) have shown unanticipated changes in non-renal sympathetic activity. It has been hypothesized that this is due to ablation of sympathoexcitatory afferent renal nerves, which are overactive under conditions of renal inflammation. This study sought to investigate the interaction of the inflammatory cytokine, IL-1, and afferent renal nerves in the pathogenesis of hypertension. To test this hypothesis, we compared the effect of afferent RDN (ARDN) to the administration of the IL-1R antagonist anakinra, on the pathogenesis DOCA-salt hypertension. In addition, the combination of ARDN and anakinra treatments in DOCA-salt mice was administered to assess the degree of mechanistic overlap. 10-week-old male C57BL6/J mice were implanted with radio-telemetry probes in the carotid artery for measurement of mean arterial pressure (MAP). DOCA was administered via subcutaneous implantation of a silicon pellet containing 50 mg of DOCA. Sham mice received a drug free silicon pellet. Uni-nephrectomy, pellet insertion, salt treatment, ARDN were all performed or started on the same day. ARDN was performed through peri-axonal application of capsaicin. IL-1R antagonist anakinra was used to suppress activation of IL-1R. Anakinra (75mg/kg) was delivered via intraperitoneal injection daily 10 days post induction of hypertension. Animals in the untreated or monotherapy groups received sham RDN and/or vehicle injections. Baseline MAP was measured 3 days prior to induction of DOCA-salt hypertension by radiotelemetry. MAP increased in untreated DOCA-salt mice by 42 ± 2 mmHg (n=7) from baseline in the final week of the study. In comparison, this response was attenuated by 42% by ARDN (+24 ± 2 mmHg, n=7). IL-1α and IL-1β were both increased in DOCA-salt kidneys (112.4 ± 19.2 pg/mg and 2.1 ± 0.4 pg/mg respectively) compared to sham kidneys (31.98 ± 4.4 pg/mg and 0.36 ± 0.06 pg/mg respectively). The DOCA-salt induced increase of MAP in mice treated with anakinra alone (+23 ± 3 mmHg, n=9), or the combination of ARDN and anakinra (+26 ± 3 mmHg, n=5) was similar to ARDN monotherapy (+24 ± 2 mmHg, n=7). All treatment groups showed statistically significant changes in MAP compared to the SHAM and Vehicle treated group with 2 Way ANOVA and Tukey test for multiple comparisons. The finding that ARDN and anakinra combination therapy provides no additional benefit beyond ARDN or anakinra alone, suggests a common mechanism. We propose that antagonism of IL-1R blocks IL-1 mediated activation of afferent renal nerves resulting in decreased central sympathetic tone and decreased MAP. Further investigation into the mechanisms of cytokine mediated sensory fiber activation in the kidney are the subject of ongoing studies.
