Abstract:Microneurographic recordings of muscle sympathetic nerve activity (MSNA) and the succeeding changes in beat-to-beat blood pressure (i.e., sympathetic transduction) provide important insights into the neural control of the circulation in humans. Despite its widespread use, the reliability of this technique remains unknown. Herein, we assessed the intra- and inter-day test-retest reliability of signal-averaging sympathetic transduction to blood pressure. Data were analyzed from 15 (9M/6F) young, healthy participants who completed two baseline recordings of fibular nerve MSNA separated by 60 minutes (intra-day). The inter-day reliability was obtained in a subset of participants ( n=13, 9M/4F) who completed a follow-up MSNA study. Signal-averaging sympathetic transduction was quantified as peak change in diastolic (DBP) and mean arterial pressure (MAP) following a burst of MSNA. Analyses were also computed considering different MSNA burst sizes (quartiles of normalized MSNA), and burst patterns (singlets, couplets, triplets, and quadruplets+), as well as non-burst responses. Intraclass-correlation coefficients (ICC) were used as the main reliability measure. Peak changes in MAP [intra-day: ICC=0.76(0.30-0.92), P=0.006; inter-day: ICC=0.91(0.63-0.97), P<0.001] demonstrated very good to excellent reliability. Sympathetic transduction of MSNA burst size displayed moderate to very good reliability, though the reliability of MSNA burst pattern was poor to very good. Non-burst responses revealed poor intra-day [ICC=0.37(-1.05-0.80), P=0.21], but very good inter-day [ICC=0.76(0.18-0.93), P=0.01] reliability. Intra-day reliability measures were consistently lower than inter-day. Similar results were obtained using DBP. Collectively, these findings provide evidence that the burst-triggering signal-averaging technique is a reliable measure of sympathetic transduction to blood pressure in young, healthy adults.

Abstract:Background: Heart failure with reduced ejection fraction (HFrEF) is associated with reduced cardiac β-adrenergic signal transduction in response to chronic elevations in neurally released and circulating norepinephrine. Whether elevations in muscle sympathetic nerve activity (MSNA) are accompanied by attenuated α-adrenoceptor-mediated vasoconstriction remains unclear. Therefore, the objective of the current work was to compare transduction of sympathetic firing into blood pressure (BP) in treated patients with HFrEF and healthy controls. Methods: Twenty-three treated patients with HFrEF (4 females, left ventricular ejection fraction: 28±2%) and 22 healthy controls (6 females) underwent a 7-minute resting measurement of continuous beat-to-beat BP (finger photoplethysmography), heart rate (electrocardiography), and MSNA (microneurography). Sympathetic-BP transduction was quantified using both signal averaging, whereby the BP response to each MSNA burst was serially tracked over 15 cardiac cycles and averaged to derive the peak change in BP, and cross-spectral analysis of low-frequency (0.04-0.15 Hz) MSNA and BP oscillations. Results: Compared with controls, patients with HFrEF had less sympathetic-BP transduction (0.7±0.3 versus 0.2±0.3 mm Hg; P<0.01), and lower low-frequency oscillations in MSNA (120±56 versus 64±32 arbitrary units2; P<0.01) and BP (3.1±1.6 versus 2.0±1.7 mm Hg2; P<0.01). In subgroup analysis, resting sympathetic-BP transduction was lower in patients with HFrEF with normal resting MSNA compared to healthy controls (0.7±0.3 versus 0.4±0.3 mm Hg; P=0.01) and further attenuated (0.1±0.1 mm Hg; P=0.03) in patients with HFrEF with elevated resting MSNA. Conclusions: Treated HFrEF is associated with lower sympathetic-BP transduction, even when MSNA is not elevated, and diminishes further with disease progression. These adaptations may serve to limit the adverse consequences of oscillatory surges in sympathetic vasoconstrictor discharge on stroke volume.

Sympathetic transduction is the study of how impulses of sympathetic nerve activity (SNA) affect end‐organ function. Recently, the transduction of resting bursts of muscle SNA (MSNA) has been investigated and shown to have a role in the maintenance of blood pressure through changes in vascular tone in humans. In the present study, we investigate whether directly recorded resting cardiac SNA (CSNA) regulates heart rate (HR), coronary blood flow (CoBF), coronary vascular conductance (CVC), cardiac output (CO) and mean arterial pressure. Instrumentation was undertaken to record CSNA and relevant vascular variables in conscious sheep. Recordings were performed at baseline, as well as after the infusion of a β‐adrenoceptor blocker (propranolol) to determine the role of β‐adrenergic signalling in sympathetic transduction in the heart. The results show that after every burst of CSNA, there was a significant effect of time on HR (n = 10, ∆: +2.1 ± 1.4 beats min–1, P = 0.002) and CO (n = 8, ∆: +100 ± 150 mL min–1, P = 0.002) was elevated, followed by an increase in CoBF (n = 9, ∆: +0.76 mL min–1, P = 0.001) and CVC (n = 8, ∆: +0.0038 mL min–1 mmHg–1, P = 0.0028). The changes in HR were graded depending on the size and pattern of CSNA bursts. The HR response was significantly attenuated after the infusion of propranolol. Our study is the first to explore resting sympathetic transduction in the heart, suggesting that CSNA can dynamically change HR mediated by an action on β‐adrenoceptors. Key points Sympathetic transduction is the study of how impulses of sympathetic nerve activity (SNA) affect end‐organ function. Previous studies have examined sympathetic transduction primarily in the skeletal muscle and shown that bursts of muscle SNA alter blood flow to skeletal muscle and mean arterial pressure, although this has not been examined in the heart. We investigated sympathetic transduction in the heart and show that, in the conscious condition, the size of bursts of SNA to the heart can result in incremental increases in heart rate and coronary blood flow mediated by β‐adrenoceptors. The pattern of bursts of SNA to the heart also resulted in incremental increases in heart rate mediated by β‐adrenoceptors. Abstract:Purpose Patients with chronic kidney disease (CKD) are more than twice as likely to die from a cardiovascular event than those with normal kidney function. Although CKD may increase resting sympathetic activity, quantification of resting sympathetic outflow alone does not account for the ensuing vasoconstriction, and blood pressure (BP) change (i.e., sympathetic transduction). Patients with CKD have been reported to exhibit elevated α-adrenergic receptor sensitivity, which may predispose this population to greater sympathetic transduction. We tested the hypothesis that patients with CKD have augmented sympathetic transduction to BP. Methods In 16 patients with CKD, 17 bodyweight-matched (BWM) controls, and 11 lean controls of a similar age muscle sympathetic nerve activity (MSNA) and beat-to-beat BP were continuously recorded during quiet supine rest. Signal averaging was used to quantify changes in mean arterial pressure (MAP) and total vascular conductance (TVC) following spontaneous bursts of MSNA. Results Peak increases in MAP following MSNA bursts were not different among patients with CKD and the control groups (CKD: 2.3 ± 1.1 mmHg; BWM controls: 2.1 ± 1.0 mmHg; lean controls: 1.7 ± 0.9 mmHg; P = 0.28). Likewise, nadir reductions in TVC following all bursts of MSNA were not different among patients with CKD and either control group ( P = 0.69). Both patients with CKD and controls had graded increases in MAP and decreases in TVC with increasing burst size, which were not different among groups (all P > 0.05). Conclusion In summary, these data indicate that patients with CKD do not have augmented sympathetic transduction to BP.

Abstract:What is the purpose of sympathetic neuronal action potential (AP) discharge and recruitment patterns for human vascular regulation? This study tested the hypothesis that sympathetic neuronal discharge and recruitment patterns regulate neuropeptide Y (NPY) bioavailability. We used microneurography to record muscle sympathetic nerve activity and a continuous wavelet transform to detect sympathetic APs during a baseline condition and intravenous dexmedetomidine infusion (α2-adrenergic agonist, 10-min loading infusion of 0.225 µg·kg-1; maintenance infusion of 0.1-0.5 µg·kg·h-1) in six healthy individuals (5 females, 27 ± 6 yr). Arterial blood samples provided NPY (enzyme-linked immunosorbent assay) and norepinephrine (liquid chromatography-tandem mass spectrometry) levels at baseline and the dexmedetomidine maintenance infusion. Linear mixed-model regressions assessed the relationships between AP discharge, recruitment, and neurotransmitter levels. Across baseline and the dexmedetomidine condition, NPY levels were positively related to mean arterial pressure (β = 1.63 [0.34], P = 0.002), total AP clusters (β = 0.90 [0.22], P = 0.005), and AP frequency (β = 0.11 [0.03], P = 0.003). Norepinephrine levels were not related to mean arterial pressure (β = 0.03 [0.02], P = 0.133) but were positively related to total AP clusters (β = 19.50 [7.07], P = 0.030) and AP frequency (β = 2.66 [0.81], P = 0.014). These data suggest that sympathetic neuronal discharge and recruitment patterns regulate NPY and norepinephrine bioavailability in healthy adults. As such, sympathetic neuronal firing strategies are important for human vascular regulation.NEW & NOTEWORTHY The purpose of sympathetic neuronal discharge and the recruitment of neuronal subpopulations for human circulatory homeostasis remains unknown. Abstract:Hypertension, Ahead of Print. Background:We tested the hypothesis that patients with heart failure with preserved ejection fraction (HFpEF) would have greater muscle sympathetic nerve activity (MSNA) at rest and sympathetic reactivity during a cold pressor test compared with non–heart failure controls. Further, given the importance of the baroreflex modulation of MSNA in the control of blood pressure (BP), we hypothesized that patients with HFpEF would exhibit a reduced sympathetic baroreflex sensitivity.Methods:Twenty-eight patients with HFpEF and 44 matched controls (mean±SD: 71±8 versus 70±7 years; 9 men/19 women versus 16 men/28 women) were studied. BP, heart rate, and MSNA (microneurography) were measured during 6 to 10 minutes of supine rest and the 2-minute cold pressor test. Spontaneous sympathetic baroreflex sensitivity was assessed during supine rest.Results:Patients with HFpEF had higher resting MSNA burst frequency (39±14 versus 31±12 bursts/min;P=0.020) and lower sympathetic baroreflex sensitivity (−2.83±0.76 versus −3.57±1.19 bursts/100 heartbeats/mm Hg;P=0.019) than controls, but burst incidence was not different between groups (56±19 versus 50±20 bursts/100 heartbeats;P=0.179). During the cold pressor test, increases in MSNA indices did not differ between groups (P=0.135–0.998), but patients had a smaller increase in diastolic BP (Δ4±6 versus Δ14±11 mm Hg;P<0.001) compared with controls.Conclusions:Despite augmented resting MSNA burst frequency, burst incidence was not significantly different between groups, and sympathetic baroreflex sensitivity was reduced in patients with HFpEF. Furthermore, patients had preserved sympathetic reactivity but attenuated diastolic BP responses during the cold pressor test. These data suggest that, during physiological stress, sympathetic reactivity is intact, but the peripheral pathway for sympathetic vasoconstriction may be impaired in HFpEF.

Abstract:Sir : In the November 1997 issue of Histopathology, Bussolati et al. describe the restoration of endogenous biotin reactivity following antigen retrieval (using pressure cooking and to a lesser extent microwave retrieval). The authors suggest that blocking of endogenous biotin by sequential avidin–biotin treatment prevents the erroneous detection of biotin. Intra-nuclear biotin inclusions have been previously misinterpreted to be those of Herpes simplex virus (HSV), using the streptavidin–biotin complex (StrepABC). We have also described biotin intranuclear inclusions in gestational endometria closely resembling HSV inclusions. This error can be easily avoided by paying careful attention to the negative control. We have noted the negative control to be invariably positive, due to the presence of endogenous biotin, with the StrepABC immunodetection system. This false positive immunoreaction should be the clue to the alert pathologist that there is endogenous biotin reactivity. Despite attempts to block endogenous biotin with free avidin and biotin as recommended, we have demonstrated residual positivity in the negative control, using the StrepABC system. We have also compared the alkaline-phosphatase– anti-alkaline phosphatase (APAAP) and the peroxidase– anti-peroxidase (PAP) detection systems and found that whilst the negative control was negative with the former, there was aberrant immunoreactivity with the PAP system. Curiously this was eliminated with microwave pretreatment prior to immunohistochemistry. These curious results serve to underscore the complexities of antigen retrieval. We use retrieval methods such as pressure cooking, microwave technology and even ultrasound, barely understanding the mechanisms of antigen retrieval. It is inevitable that aberrant and puzzling results are obtained. It is imperative that diagnostic pathologists interpret immunohistochemical findings in the light of positive and negative controls, and also with full knowledge of the pitfalls and limitations of the varying detection systems used in their laboratories. We advocate that the optimal detection system for immunohistochemistry in the tissues containing endogenous biotin, as so comprehensively demonstrated by Bussolati et al. is either the APAAP or PAP immunodetection system (the latter with microwave pretreatment), rather than the StrepABC method. Abstract:Two erbium doped fibers (EDFs) with different radiation sensitivities were used in the photo-annealing tests, and their photo-annealing efficiencies were compared by using 532-nm and 976-nm lasers. For both EDFs, the 532-nm laser with 10 mW showed better efficiency than 976-nm laser with 290 mW. The radiation induced attenuation (RIA) could nearly be diminished in the wavelength range from 900 nm to 1700 nm by the photo-annealing of 532-nm laser. The half-time of recovery was 5.6 seconds by using 532-nm laser, and was about 40 minutes by using 976-nm laser whose annealing rate was similar to that of purely thermal annealing at about 330°C. For γ-irradiation tests of superfluorescent fiber sources (SFSs) exposed to a dose rate of 129.2 krad/hr, the rate of output power loss could be reduced by the 976-nm pump laser, but the output power loss could not be recovered. A most radiation-tolerant case was an SFS with 2.8-meter EDF#1 co-pumped by both the 532 nm and the 976 nm lasers. The output power loss could be recovered with a rate of 0.009 dB/min.

Abstract:Professors Peter Feaver, Christopher Gelpi, and Jason Reifler’s theory of the determinants of public support for war has received a great deal of attention among academics, journalists, and policymakers. They argue that support for war hinges on initial support for military action and the belief in the success of the war. In this review, we take a critical and constructive view of their work, focusing on methodological concerns. We discuss the dependent variable used by the authors—individual casualty tolerance—and argue that it is an insufficient measure of war support. We also make the case that their independent variables of interest—initial support for war and evaluation of war success—may, in fact, be best understood as indicators of latent support for the war more generally. Finally, we discuss the need for more research into the determinants of support for war, focusing on core values and elite rhetoric as potential variables for continued and future study. The question of whether politicians, particularly the president, attend to public opinion when crafting foreign policy has long intrigued social scientists. For many years, conventional wisdom suggested that the public lacked the sophistication needed to form coherent foreign policy attitudes. From this point of view politicians should—and do—ignore public opinion. More recently, however, scholars have countered this perspective, producing considerable evidence that the public holds consistent opinions on foreign ADAM J. Differential control of sympathetic outflow to muscle and skin during physical and cognitive stressors

Abstract:Purpose Sympathetic nerve activity towards muscle (MSNA) and skin (SSNA) regulates various physiological parameters. MSNA primarily functions in blood pressure and flow, while SSNA operates in thermoregulation. Physical and cognitive stressors have been shown to have effects on both types of sympathetic activity, but there are inconsistencies as to what these effects are. This article aims to address the discrepancies in the literature and compare MSNA and SSNA responses. Methods Microelectrode recordings were taken from the common peroneal nerve in 29 participants: MSNA ( n = 21), SSNA ( n = 16) and both MSNA and SSNA ( n = 8). Participants were subjected to four different 2-min stressors: two physical (isometric handgrip task, cold pressor test) and two cognitive (mental arithmetic task, Stroop colour–word conflict test), the latter of which saw participants separated into responders and non-responders to the stressors. It was hypothesised that the physical stressors would have a greater effect on MSNA than SSNA, while the cognitive stressors would operate conversely. Results Peristimulus time histogram (PSTH) analysis showed the mental arithmetic task to significantly increase both MSNA and SSNA; the isometric handgrip task and cold pressor test to increase MSNA, but not SSNA; and Stroop test to have no significant effects on changing MSNA or SSNA from baseline. Additionally, stress responses did not differ between MSNA and SSNA in participants who had both sets of data recorded. Conclusions This study has provided evidence to support the literature which claims cognitive stressors increase sympathetic activity, and provides much needed SSNA data in response to stressors.

Abstract:The purpose of this study was to clarify sex differences in the inhibition of sympathetic vasomotor outflow which is caused by the loading of cardiopulmonary baroreceptors. Ten young males and ten age-matched females participated. The participants underwent a passive leg raising (PLR) test wherein they were positioned supine (baseline, 0o), and their lower limbs were lifted passively at 10o, 20o, 30o, and 40o. Each angle lasted for 3 min. Muscle sympathetic nerve activity (MSNA) was recorded via microneurography of the left radial nerve. Baseline MSNA was lower in females compared to males. MSNA burst frequency was decreased during the PLR in both males (− 6.2 ± 0.4 bursts/min at 40o) and females (− 6.5 ± 0.4 bursts/min at 40o), but no significant difference was detected between the two groups ( P = 0.61). These results suggest that sex has minimal influence on the inhibition of sympathetic vasomotor outflow during the loading of cardiopulmonary baroreceptors in young individuals.

Abstract:SIR,-While agreeing with Dr. H. J. G. Bloom and others (25 July, p. 181) that the results of radical treatment for medullary carcinoma of the breast are remarkably good we cannot accept the validity of two of their conclusions. Firstly, there is insufficient evidence to support their view that conservative treatment would not have achieved similar results as compared with a radical approach. In their non-randomized retrospective study only 14 patients out of 104 had either a simple mastectomy or wedge excision. Of these 10 also had postoperative radiotherapy. No attempt was made to explain why the policy of radical surgery was abandoned in these cases. We believe that it is not possible to reach a reasonable conclusion on the results of conservative surgery alone on the basis of four patients. Secondly, we feel that it is very difficult for the authors to make judgements on the importance of the role of possible local defence mechanisms in medullary carcinoma of the breast. This would entail a prospective randomized trial comparing the results of radical treatment, which would involve ablation of the lymph nodes by surgery or radiotherapy, with conservative surgery by itself, which would result in the least interference with regional lymph nodes. Taking cancer of the breast as a whole there is increasing evidence of the importance of local defence mechanisms.' Noninvasive Assessment of Temporal Dynamics in Sympathetic and Parasympathetic Baroreflex Responses

Abstract:Background: The baroreflex system is crucial for cardiovascular regulation and autonomic homeostasis. A comprehensive assessment requires understanding the simultaneous temporal dynamics of its multiple functional branches, which traditional methods often overlook. Objective: To develop and validate a noninvasive method for simultaneously assessing the temporal dynamics of sympathetic and parasympathetic baroreflexes using pulse contour analysis and the sequence method. Methods: Beat-to-beat blood pressure and ECG recordings were analyzed from 55 preoperative cardiothoracic surgery patients in the supine position and 21 subjects from the EUROBAVAR dataset in both supine and standing positions. Systolic arterial pressure (SAP), interbeat interval (IBI), cardiac output (CO), myocardial contraction (dP/dtmax), and systemic vascular resistance (SVR) were estimated using pulse contour analysis. Baroreflex sensitivity (BRS) was calculated via the sequence method and correlated with hemodynamic and heart rate variability (HRV) parameters. Results: Parasympathetic BRS for IBI was correlated with the root mean square of successive differences of ECG RR intervals (RMSSD-HRV) at 0-beat delay. Sympathetic BRS for SVR strongly correlated with SVR, CO, and RMSSD-HRV, particularly at 3-beat delay, and was uniquely associated with SAP at 1-beat delay. Sympathetic BRS for dP/dtmax correlated with dP/dtmax at 1-beat delay. In contrast, BRS for CO correlated with CO and SVR at 0- and 3-beat delays. Postural changes mainly affected parasympathetically-mediated BRS for IBI and, to a lesser extent, the sympathetic vascular and myocardial branches. Conclusions: This method effectively captures multiple baroreflex responses and their temporal dynamics, revealing distinct autonomic mechanisms and the impact of postural changes. Further validation is warranted.

The amplitude (Amp, uV) of lower limbs in patients with stage 1 hypertension, were significantly lower than that in patients with normal BP, stage 2 and 3 hypertension (P < 0.05). Conclusion: Sympathetic activity, evaluated by SSR, were different in patients with different stage hypertensions.

Abstract:Multiunit recordings of postganglionic sympathetic outflow to muscle yield otherwise imperceptible insights into sympathetic neural modulation of human vascular resistance and blood pressure. This Corcoran Lecture will illustrate the utility of microneurography to investigate neurogenic cardiovascular regulation; review data concerning muscle sympathetic nerve activity of women and men with normal and high blood pressure; explore 2 concepts, central upregulation of muscle sympathetic outflow and cortical autonomic neuroplasticity; present sleep apnea as an imperfect model of neurogenic hypertension; and expose the paradox of sympathetic excitation without hypertension. In awake healthy normotensive individuals, resting muscle sympathetic nerve activity increases with age, sleep fragmentation, and obstructive apnea. Its magnitude is not signaled by heart rate. Age-related changes are nonlinear and differ by sex. In men, sympathetic nerve activity increases with age but without relation to their blood pressure, whereas in women, both rise concordantly after age 40. Mean values for muscle sympathetic nerve activity burst incidence are consistently higher in cohorts with hypertension than in matched normotensives, yet women's sympathetic nerve traffic can increase 3-fold between ages 30 and 70 without causing hypertension. Thus, increased sympathetic nerve activity may be necessary but is insufficient for primary hypertension. Moreover, its inhibition does not consistently decrease blood pressure. Despite a half-century of microneurographic research, large gaps remain in our understanding of the content of the sympathetic broadcast from brain to blood vessel and its specific individual consequences for circulatory regulation and cardiovascular, renal, and metabolic risk.

Abstract:Purpose Alterations of autonomic cardiovascular control are implicated in the origin of chronic low blood pressure (BP) (hypotension), but comprehensive analysis of baroreflex function is still lacking. This study explored baroreflex function in its cardiac, vascular and myocardial branches Methods Continuous BP was recorded at rest and during a mental arithmetic task in 40 hypotensive and 40 normotensive participants. Assessed cardiovascular variables included stroke volume (SV) (calculated by the Modelflow method), heart rate (HR), cardiac output (CO), total peripheral resistance (TPR) and heart rate variability (HRV). Baroreflex sensitivity (BRS) was calculated using the spontaneous sequence method. Results Hypotensive participants exhibited greater BRS in the three baroreflex branches, in addition to lower SV, HR and CO and higher HRV and TPR. Reactivity for BP, HRV and CO during the stress task was reduced in hypotensive individuals. The greater cardiac BRS can explain the lower HR and higher HRV observed in hypotension, suggestive of increased vagal cardiac influences. The higher vasomotor BRS may contribute to the greater TPR observed in the hypotensive participants. Abnormal associations between myocardial BRS and SV arose, suggesting aberrant autonomic control of myocardial contractility in hypotension. Conclusion The results indicate that hemodynamic deficits in hypotension are related to preload factors, probably triggered by hypovolemia and reduced unstressed blood reserves, resulting in lower venous return, ventricular preload and SV. In contrast, afterload mechanisms seem to work appropriately.

Abstract:In the pathophysiological study of vasovagal syncope, the nature of the interaction between baroreceptor sensitivity (BS), sympathetic withdrawal, and parasympathetic activity has yet to be ascertained. Altered BS may predispose toward abnormal sympathetic and parasympathetic responses to orthostasis, causing hypotension that may progress to syncope if there is sympathetic withdrawal. To examine this hypothesis, we monitored blood pressure (BP), heart rate (HR), BS, forearm blood flow, and muscle nerve sympathetic activity (MNSA) continuously in 18 vasovagal patients during 60° head-up tilt, syncope, and recovery. Results were compared with those of 17 patients who were able to tolerate tilt for 45 min. During early tilt, BP was maintained in both groups by an increase in HR and MNSA from baseline ( P < 0.01), but BS decreased more in the syncopal group ( P < 0.05). At the start of presyncope (mean 2.7 ± 0.2 min before syncope and 15.2 ± 12 min after tilt), when BP fell, HR and sympathetic activity remained increased from baseline ( P< 0.01). Thereafter, BP and HR correlated directly with sympathetic activity and regressed in linear fashion until syncope ( P < 0.001), whereas BS increased to baseline. At syncope, BP, HR, and sympathetic activity fell below baseline ( P < 0.01, P < 0.05, and P < 0.01, respectively), but BS did not increase. During recovery, sympathetic activity increased to baseline and BS increased ( P < 0.05), whereas HR and BP remained low ( P < 0.01 and P < 0.05, respectively). The mechanism for the initiation of hypotension during presyncope remains unknown, but BS may contribute. Vasodilatation and bradycardia during presyncope appear to be more closely related to withdrawal of sympathetic activity than to increased parasympathetic cardiac activity.

Abstract:Objective: To investigate the influence of blood pressure variability on the parameters of arterial load and ventriculo-arterial coupling in apparently healthy individuals. Design and method: We screened 193 persons in a primary prevention program recording risk factors, blood pressure and ultrasound data: LV systolic/diastolic dimensions and volumes, septal/posterior wall thickness (SWT/PWT), left ventricle mass (LVM), ejection fraction, Doppler parameters, brachial artery Flow Mediated Dilatation (FMD), carotid artery Intima-Media Thickness (IMT). We calculated: systemic arterial compliance (C=stroke volume/pulse pressure), vascular resistance/cardiac circle length ratio (R/T where R=mean blood pressure/(stroke volume x heart rate)), effective arterial elastance (Ea=0.9 x systolic BP/stroke volume), as an index of artery load. The modified single-beat method was used to estimate left ventricular end-systolic elastance (Ees(sb)) and the ratio of Ea/Ees(sb) was computed as an index of ventriculo-arterial coupling. All subjects underwent a 24-hour Ambulatory Blood Pressure Monitoring (ABPM) on a working day, performing usual daily activities and avoiding heavy physical exercise. Blood pressure and heart rate readings were obtained every 15 minutes during daytime and every 30 minutes during nighttime. We calculated BP variability (BPV) as the Standard Deviation (SD) of mean blood pressure. Results: After data collection, we investigated the correlations (Pearson's r) between systolic BPV values and the parameters of arterial load and ventriculo-arterial coupling. We found a weak correlation between BPV and Ea/Ees(sb) ratio (r = 0,257,p < 0,001). We did not find any significant correlations between BPV values and arterial compliance, vascular resistance, effective arterial elastance and left ventricular end-systolic elastance. When we examined the correlations between systolic BPV values and the other markers of subclinical cardiovascular disease we found a moderate correlation with brachial artery FMD (r = -0,399,p < 0,001) and with carotid artery IMT(r = 0,314, p < 0,001) and a weak correlation with LV mass index (r = 0,170, p = 0,019). Conclusions: Our study indicates that blood pressure variability differences measured in apparently healthy individuals generate an adaptation of the ventriculo-arterial coupling parameters and correlate with markers of cardiac and vascular remodeling.

[Variation of Doppler ultrasonography of carotid arteries].

Abstract:The variation of ultrasound examination of the carotid arteries performed at two different hospitals was examined by retrospective analysis. Eighty-one patients were primarily examined by ultrasound at their local hospital on the suspicion of carotid stenosis. Following referral, they underwent a further scanning at the Dept. of Vascular Surgery, Rigshospitalet. Results for 143 carotid arteries were available for comparison. The overall agreement between the two examinations was only 59%. In nine of 40 (22%) cases an originally diagnosed minor stenosis was found to be significant and 3/19 (16%) with shown occlusion were found to be patent at the later examination. In order to draw clinical conclusions validation of each individual laboratory performing ultrasound examination of extracranial cervical arteries is necessary.
