Abstract:Background Seasonal patterns of preterm birth were identified in previous studies, but the effect of conception season on preterm birth has not been extensively studied. Based on the notion that the etiological roots of preterm birth lie in the beginning of pregnancy, we did a population-based retrospective cohort study in Southwest China to examine the effects of season of conception and month of conception on preterm birth. Methods We did a population-based retrospective cohort study in women (aged 18–49) who participated in the NFPHEP from 2010 to 2018, and had a singleton livebirth in southwest China. According to the time of the last menstruation reported by the participants, month of conception and season of conception were then ascertained. We used multivariate log-binomial model to adjust the potential risk factors for preterm birth and obtained adjusted risk ratio (aRR) and 95% confidence intervals (95%CI) for conception season, conception month and preterm birth. Results Among 194 028 participants, 15 034 women had preterm birth. Compared with pregnancies that were conceived in the summer, pregnancies that were conceived in the spring, autumn and winter had the higher risk of preterm birth (Spring: aRR = 1.10, 95% CI: 1.04–1.15; Autumn: aRR = 1.14, 95% CI: 1.09–1.20; Winter: aRR = 1.28, 95% CI: 1.22–1.34) and also had a higher risk of early preterm birth (Spring: aRR = 1.09, 95% CI: 1.01–1.18; Autumn: aRR = 1.09, 95% CI: 1.01–1.19; Winter: aRR = 1.16, 95% CI: 1.08–1.25). Pregnancies in December, and January had a higher risk of preterm birth and early preterm birth than pregnancies that were conceived in July. Conclusions Our study found that preterm birth was significantly related to season of conception. Preterm and early preterm birth rates were the highest among pregnancies that were conceived in winter, and the lowest among pregnancies in summer.

Abstract:Objective: To examine the associations between season of birth and factors as age at menarche, age at menopause and reproductive span. Methods: A total of 285 186 female from the China Kadoorie Biobank, with complete data on critical variables and had menarche at 9-18 years old, were included. A total of 132 373 female with natural menopause were included for the analysis on age at menopause and reproductive span. Multiple linear regression models were used to assess the associations of birth season and the age at menarche, menopause, and reproductive span. Subgroup analyses were performed on birth cohorts and urban/rural residence. Results: Compared with the Spring-born (March, April, and May), participants who were born in Summer (June, July, and August), Autumn (September, October, and November), and Winter (December, January, and February) appeared late on both age at menarche and menopause. Multivariable-adjusted coefficients (95% CI) appeared as 0.14 (95% CI: 0.13-0.16), 0.26(95% CI: 0.24-0.27), 0.10 (95% CI: 0.08-0.12) for age at menarche respectively and 0.14 (95%CI: 0.08-0.20), 0.18 (95%CI: 0.12-0.24), 0.09 (95%CI: 0.03-0.16) for age at menopause respectively. No statistically significant association was found between the season of birth and reproductive span. The association was consistent between urban and rural residents and across the birth cohorts. Conclusions: female born in spring showed both earlier age on both menarche and menopause, compared to the ones born in other seasons. Our findings suggested that exposures in early life with some degree of seasonal variation might influence the development of female reproductive system.
Role of temperature, influenza and other local characteristics in seasonality of mortality: a population-based time-series study in Japan

Abstract:Objectives To investigate the extent to which temperature and influenza explained seasonality of mortality in Japan and to examine the association of the seasonality with prefecture-specific characteristics. Design We conducted time-series analysis to estimate the seasonal amplitude before and after adjusting for temperature and/or influenza-like illness (ILI). Next, we applied linear mixed effect models to investigate the association of seasonal amplitudes with each indicator on prefecture-specific characteristics on climate, demographic and socioeconomic factors and adaptations. Setting 47 prefectures in Japan Participants Deaths for all-cause, circulatory, and respiratory disease between 1999 and 2015. Outcome measures Peak-to-trough ratio (PTR, a measure of seasonal amplitude). Results The nationwide unadjusted-PTRs for all-cause, circulatory and respiratory mortality were 1.29 (95% CIs: 1.28 to 1.31), 1.55 (95% CI: 1.52 to 1.57) and 1.45 (95% CI: 1.43 to 1.48), respectively. These PTRs reduced substantially after adjusting for temperature but very little after a separate adjustment for ILI. Furthermore, seasonal amplitudes varied between prefectures. However, there was no strong evidence for the associations of PTR with the indicators on prefecture-specific characteristics. Conclusions Seasonality of mortality is primarily driven by temperature in Japan. The spatial variation in seasonal amplitudes was not associated with prefecture-specific characteristics. Although further investigations are required to confirm our findings, this study can help us gain a better understanding of the mechanisms underlying seasonality of mortality.

Temperature- one of the key season defining factors, has been consistently linked with increasing mortality. However, little is known about its impact on seasonality of mortality. Investigating seasonality and the effect of temperature on it will provide important information for health risk management in different seasons. Aims: To assess seasonality of mortality with temperature adjustment in Japan and its temporal changes Method: Daily mean temperature and daily death cases for all-cause, circulatory and respiratory mortality from 1972 to 2015 were collected for 47 prefectures in Japan. A generalized linear model with quasi-Poisson distribution was used, with a one-dimensional cyclic spline function with 4 degrees of freedom for the calendar day. The long term trend and day of week were controlled. A cross-basis function, with natural cubic B-spline functions and extended lag period to 21 days, was used for temperature. The ratio of estimated mortality counts of peak versus trough day (RR) was estimated with and without temperature adjustment, by treating trough day as a reference day. RR was estimated for whole country over 44 years first and then for each single year to investigate the temporal changes. All estimates were conducted for all-cause, circulatory and respiratory mortality, respectively. Results: U-shaped relationships were observed for both adjusted and unadjusted seasonality, with peaks in cold seasons and troughs in warm seasons. Seasonality became less obvious after adjusting for temperature. Both adjusted and unadjusted RR showed a decreasing trend for all-cause and respiratory mortality. No significant trend was observed for circulatory mortality. Conclusions: Seasonality risk was higher in cold seasons. Temperature may be an important driver. Additionally, seasonality of mortality decreased over 44 years. Its underlying reasons should be investigated in the future.

Abstract:BACKGROUND:Ambient fine particle (PM2.5) pollution is an important public health problem in China. Short-term ambient PM2.5 exposure is associated with increased mortality of respiratory diseases. However, few evidence was available on the effect of exposure to ambient PM2.5 on the years of life lost (YLL) from respiratory diseases in the elderly. Furthermore, birth season which is frequently applied as a proxy for environmental exposure in early life may influence the health outcome in the later life. Nevertheless, the modification effect of birth season on the relationship of PM2.5 exposure and respiratory health need to be explored.METHODS:A time-stratified case-crossover design was used to analyze YLL from respiratory diseases in the elderly related to ambient PM2.5 exposure between 2013 and 2016 in Ningbo, China. The modification effect of birth season was explored by subgroup comparisons between different birth seasons.RESULTS:Each 10 μg/m3 increase in daily ambient PM2.5 was associated with an increment of 1.61 (95% CI 0.12, 3.10) years in YLL from respiratory diseases in the elderly population. Individuals who were born in winter had significantly higher YLL from respiratory diseases associated with ambient PM2.5 exposure than those who were born in other seasons.CONCLUSIONS:Birth season which reflects the early-life PM2.5 exposure level that may influence the lung development has a potential effect on the disease burden of respiratory diseases related to ambient PM2.5 exposure in later life. The results would provide theoretical basis to protect vulnerable population defined by birth season when exploring the adverse effects of ambient PM2.5 in the respiratory health.

Abstract:Abstract A series of medical and biological studies have identified a strong link between the season during which a child is born and early childhood development and adult health. While providing an explanation for the seasonal determinants of birth remains one of the key challenges for demographers, understanding seasonal birth peaks and troughs can inform both public health planning and efforts to prevent childhood diseases. The seasonality of births varies across and within countries. Nonetheless, there has been scant research into the seasonality of births in China. This study takes an in depth look at the seasonal fluctuations in China’s births since 1960 and investigates the effects on seasonality of several key factors such as parity, residence (urban or rural area), and changes to fertility policies. Traditionally, winter has been the peak season of births in China, while spring and summer have been lean seasons, a pattern which is quite different than patterns in Europe and the United States. After 1990, the sweeping implementation of one-child policy and the wider use of modern contraceptives among Chinese women prompted a fundamental shift in the seasonality of births, with the deviation of births from month to month gradually diminishing over time. People's self-control may serve as a key determinant accounting for such seasonal changes. First births exhibit pronounced and steady seasonality, while second and subsequent births follow a vague seasonal pattern, suggesting that later births are more likely affected by self-choice and self-intervention. The analysis also reveals that the monthly distribution pattern of second births experienced a dramatic change following the roll-out of the selective-two-child policy and the Universal Two-child Policy in 2014 and 2016, changes that reflects the adjustments people made to achieve their fertility plans under the new policy.

Abstract:Background Ambient temperature may contribute to seasonality of mortality; in particular, a warming climate is likely to influence the seasonality of mortality. However, few studies have investigated seasonality of mortality under a warming climate. Methods Daily mean temperature, daily counts for all-cause, circulatory, and respiratory mortality, and annual data on prefecture-specific characteristics were collected for 47 prefectures in Japan between 1972 and 2015. A quasi-Poisson regression model was used to assess the seasonal variation of mortality with a focus on its amplitude, which was quantified as the ratio of mortality estimates between the peak and trough days (peak-to-trough ratio (PTR)). We quantified the contribution of temperature to seasonality by comparing PTR before and after temperature adjustment. Associations between annual mean temperature and annual estimates of the temperature-unadjusted PTR were examined using multilevel multivariate meta-regression models controlling for prefecture-specific characteristics. Results The temperature-unadjusted PTRs for all-cause, circulatory, and respiratory mortality were 1.28 (95% confidence interval (CI): 1.27–1.30), 1.53 (95% CI: 1.50–1.55), and 1.46 (95% CI: 1.44–1.48), respectively; adjusting for temperature reduced these PTRs to 1.08 (95% CI: 1.08–1.10), 1.10 (95% CI: 1.08–1.11), and 1.35 (95% CI: 1.32–1.39), respectively. During the period of rising temperature (1.3 °C on average), decreases in the temperature-unadjusted PTRs were observed for all mortality causes except circulatory mortality. For each 1 °C increase in annual mean temperature, the temperature-unadjusted PTR for all-cause, circulatory, and respiratory mortality decreased by 0.98% (95% CI: 0.54–1.42), 1.39% (95% CI: 0.82–1.97), and 0.13% (95% CI: − 1.24 to 1.48), respectively. Conclusion Seasonality of mortality is driven partly by temperature, and its amplitude may be decreasing under a warming climate.

Abstract:To describe the seasonal growth patterns in Shanghai infants and to investigate the long based effect of birth season on postnatal early human growth. This community based longitudinal study was carried out in Fenglin community,Shanghai,China.The study series consisted of 4 128 full term children,born between 1st January 1980 and 31 st December 1990.Children was followed up monthly from 1 to 6 months,three monthly from 6 to 12 months,six monthly from 12 to 24 months.The year was divided into four seasons:spring,summer,autumn and winter.Weight gain,length gain and changes in body mass index(BMI) were calculated over the seasons of the year. [Conclusions] There is a clear seasonality in growth in Shanghai infants.Birth month has some association with attained size,but this is reduced during the first two years of life.Seasonality in infant growth represents a significant potential confounding factor in growth studies,such as nutritional clinical trials.

Abstract:Study Objectives: We assessed the yearly seasonal, environmental effects on birth pattern in Chinese patients later diagnosed with narcolepsy and cataplexy and explored if this effect persisted in patients with symptoms onset date before, following, and after the 2009 H1N1 pandemic. Methods: A total of 1,942 patients with birth data information and diagnosed narcolepsy with cataplexy were included in this study. The birth month and seasonal effect of 1,064 patients born from 1970 to 2000 were compared to controls (n = 2,028,714) from the general population. Furthermore, birth season effect in 1,373 patients with definite disease onset month were compared among patients with onset date before (n = 595), following (from January 2010 to December 2010) (n = 325), and after (n = 453) the H1N1 pandemic. Results: Patients with narcolepsy and cataplexy had a significantly different seasonality from the general population (P = .027). The monthly distribution of birth month yielded a peak in November (odds ratio =1.23 [95% confidence interval, 1.01-1.49], P = .042) and a trough in April (odds ratio = 0.68 [95% confidence interval, 0.52-0.88], P = .004). No significant difference was observed in the birth month across patients with symptom onset dates before, following, and after the 2009 H1N1 pandemic (P = .603). Conclusions: This finding across many years of seasonal effect in Chinese narcolepsy cataplexy supports a role for early-life environmental influences on disease development.

Abstract:Month of birth influences adult life expectancy at ages 50+. Why? In two countries of the Northern Hemisphere–Austria and Denmark–people born in autumn (October–December) live longer than those born in spring (April–June). Data for Australia show that, in the Southern Hemisphere, the pattern is shifted by half a year. The lifespan pattern of British immigrants to Australia is similar to that of Austrians and Danes and significantly different from that of Australians. These findings are based on population data with more than a million observations and little or no selectivity. The differences in lifespan are independent of the seasonal distribution of deaths and the social differences in the seasonal distribution of births. In the Northern Hemisphere, the excess mortality in the first year of life of infants born in spring does not support the explanation of selective infant survival. Instead, remaining life expectancy at age 50 appears to depend on factors that arise in utero or early in infancy and that increase susceptibility to diseases later in life. This result is consistent with the finding that, at the turn of the last century, infants born in autumn had higher birth weights than those born in other seasons. Furthermore, differences in adult lifespan by month of birth decrease over time and are significantly smaller in more recent cohorts, which benefited from substantial improvements in maternal and infant health.
Effect of Birth Month on Educational Attainment and Adult Cognition in Rural China

Abstract:The association of season of birth with cognitive development, academic performance and educational attainment has been well documented in western countries. However, little research has been conducted in developing countries to examine the seasonality of cognitive achievement. In this study, we analyzed data of a nationally representative sample of over 8500 rural adults born in the 1960s and early 1970s from the 2010 China Family Panel Studies survey. The results of logistic regression models demonstrate a significant association of birth month with educational attainment even with relevant individual and parental information controlled. Specially, we found that rural respondents born in the months of March and April are much less likely to obtain a high school degree than those born late in the year from October to December. The seasonal pattern is also found to significantly affect current cognitive achievements of the respondents even after their educational attainment and family social economic conditions are taken into account. Finally, we discussed the merits of age position theory and fetal origin interpretation as potential mechanisms for the seasonal pattern of education attainment, and suggest that seasonal changes of nutritional intake for pregnant mothers are the most likely cause.
Seasonality and ambient temperature at time of conception in term-born individuals – influences on cardiovascular disease and obesity in adult life

Abstract:BACKGROUND: The influence of environmental conditions early in life - including temperature and season - on health later in life has so far not attracted much attention.OBJECTIVE: Using data from the Helsinki Birth Cohort Study of 13,345 men and women, the influence of temperature and season at month of conception on birth weight, and on cardiovascular diseases and obesity-related traits in later life was studied.DESIGN: Linear regressions were fitted to examine the relationship between birth weight/obesity-related variables/hypertension and alternatively month of conception and average temperature of month of conception. The incidence of both coronary heart disease and cerebrovascular disease was assumed to follow a Weibull hazard model, and was modelled accordingly using survival analysis techniques.RESULTS: In women, unusually cold temperatures at month of conception predicted lower body mass index (BMI) and fat percentage, and protected from obesity. Warmer temperatures at month of conception were associated with higher risk for hypertension. In men, warmer temperatures around conception predicted lower BMI. No seasonal influences were detected on obesity-related variables, nor were there seasonal or temperature mediated influences on birth weight, coronary heart disease or cerebrovascular disease observed.CONCLUSIONS: We suggest that ambient temperature has an influence on obesity-related outcomes and hypertension. This merits further study, also with regard to other health outcomes and from a global perspective.

Association Between Winter Season and Risk of Death from Cardiovascular Diseases: a Study in More Than Half a Million Inpatients in Beijing, China

Abstract:Background Seasonal associations of cardiovascular mortality have been noted in most populations of European origin years ago, but are not well evaluated in Asian populations recently. Methods Utilizing the electronic Hospitalization Summary Reports (HSRs) from 32 top-ranked hospitals in Beijing, China, we evaluated the association between winter season and the risk of cardiovascular death among hospitalized individuals. General additive models and logistic regression models were adjusted for confounding factors. Results Older patients who were admitted to the hospital in the winter months (January, February, November and December) had a death risk that was increased by approximately 30% to 50% ( P < 0.01) over those who were admitted in May. However, younger patients did not seem to experience the same seasonal variations in death risk. The excess winter deaths among older patients were associated with ischemic heart disease (RR = 1.22; 95% CI 1.13 to 1.31), pulmonary heart disease (RR = 1.42; 95% CI 1.10 to 1.83), cardiac arrhythmias (RR = 1.67; 95% CI 1.36 to 2.05), heart failure (RR = 1.30; 95% CI 1.09 to 1.54), ischemic stroke (RR = 1.30; 95% CI 1.17 to 1.43), and other cerebrovascular diseases (RR = 1.78; 95% CI 1.40 to 2.25). The risks of mortality were higher in winter months than in the month of May, regardless of the presence or absence of respiratory disease. Conclusions Winter season was associated with a substantially increased risk of cardiovascular death among older Chinese cardiovascular inpatients.
Seasonal Variation in Mortality and the Role of Temperature: a Multi-Country Multi-City Study

We aimed to assess seasonal variation in mortality and to examine the contribution of temperature. Methods We compiled daily data on all-cause, cardiovascular and respiratory mortality, temperature and indicators on location-specific characteristics from 719 locations in tropical, dry, temperate and continental climate zones. We fitted time-series regression models to estimate the amplitude of seasonal variation in mortality on a daily basis, defined as the peak-to-trough ratio (PTR) of maximum mortality estimates to minimum mortality estimates at day of year. Meta-analysis was used to summarize location-specific estimates for each climate zone. We estimated the PTR with and without temperature adjustment, with the differences representing the seasonal effect attributable to temperature. We also evaluated the effect of location-specific characteristics on the PTR across locations by using meta-regression models. Results Seasonality estimates and responses to temperature adjustment varied across locations. The unadjusted PTR for all-cause mortality was 1.05 [95% confidence interval (CI): 1.00-1.11] in the tropical zone and 1.23 (95% CI: 1.20-1.25) in the temperate zone; adjusting for temperature reduced the estimates to 1.02 (95% CI: 0.95-1.09) and 1.10 (95% CI: 1.07-1.12), respectively. Furthermore, the unadjusted PTR was positively associated with average mean temperature. Conclusions This study suggests that seasonality of mortality is importantly driven by temperature, most evidently in temperate/continental climate zones, and that warmer locations show stronger seasonal variations in mortality, which is related to a stronger effect of temperature.

The Geneva Papers on Risk and Insurance Issues and Practice

Abstract:Initiated in 1995, Taiwan's National Health Insurance (NHI) programme now covers over 99.6% of its residents, ensuring widespread medical access. Despite this, regional disparities in medical resource allocation persist. This study investigates the potential urban–rural divide in life expectancy and healthcare utilisation. Drawing data from the Ministry of the Interior (population registration records), NHI Research Database (medical utilisation) and Ministry of Health and Welfare (leading causes of death), we employ spatial analysis, visualisation tools and the standardised mortality ratio for assessing regional disparities. Our findings reveal distinct regional mortality differences in Taiwan, with lower rates in northern counties and higher ones in mountainous regions. However, healthcare utilisation shows no significant regional variations. Notably, patterns of overall mortality rates and primary death causes demonstrate spatial clustering.

Abstract:There are suspected links between an animal's diet, differentiation status of a target tissue, and sensitivity to chemically induced cancer. We have demonstrated that rats fed AIN93G diets made with soy protein isolate (SPI) or whey protein hydrolysate (WPH) had a lower incidence of 7,12-dimethylbenz(a)anthracene (DMBA)-induced adenocarcinoma than rats fed the same diet made with casein (CAS). The current study was conducted to determine the differentiation status of the mammary glands during development. Offspring of rats (n = 5-10/group) were fed diets made with SPI, WPH, or CAS throughout life (beginning on gestation day 4) and were sacrificed on postnatal day (PND) 21, PND 33, PND 50 or on metaestrous between PND 48 and PND 51. There were no significant differences between the numbers of mammary terminal end buds (TEBs) or lobuloalveoli (LOB) between any of the diets groups at PND 21 or PND 33, but at PND 50 there was an 75% decrease in the mean numbers of TEBs/mm(2) in the SPI- or WPH-fed rats, compared with the CAS-fed rats (p = 0.09 and p = 0.06, respectively). In rats sacrificed in metaestrous, there were no significant differences in the proliferation index (PI) in the TEBs or LOB between any of the diet groups. In metaestrous rats, there were twice as many cells expressing estrogen receptor beta (ERbeta; approximately 60%) compared with estrogen receptor alpha (ERalpha; approximately 30%) in the LOB and 1.5 times more ERbeta (approximately 60%) compared with estrogen receptor alpha (ERalpha, approximately 40%) in the TEBs. There were no diet-dependent differences in expression of ERalpha and ERbeta. Similarly, there were no differences between the diet groups in progesterone receptor (PR) expressing LOB cells. However, in the TEBs there was a diet-dependent difference in PR positive cells with a 34% increase (p < 0.05) in the SPI-fed rats and a 38% increase (p < 0.05) in the WPH-fed rats compared with the CAS-fed rats. These results show that the type of dietary protein alters the phenotype of mammary epithelia in the TEBs. The SPI- and WPH-dependent changes in mammary differentiation may contribute to the reduced sensitivity to DMBA-induced mammary cancer in rats fed these proteins.
How Birth Season Affects Vulnerability to the Effect of Ambient Ozone Exposure on the Disease Burden of Hypertension in the Elderly Population in a Coastal City in South China.

Abstract:Birth season is an important factor that reflects prenatal nutritional conditions during early development, and which might have lifelong impacts on health. Moreover, ambient ozone pollution has been considered an important environmental risk factor for hypertension. However, whether birth season affects vulnerability to the effect of ambient ozone exposure on late-life hypertension is still unknown. A flexible case–crossover design was used to explore the effect of ambient ozone exposure on the disease burden of hypertension using years of life lost (YLL) in the elderly population in a coastal city in South China from 2013 to 2016. The influence of birth season was also explored. Ozone exposure was significantly associated with increased YLL from hypertension. The association was higher in the elderly individuals who were born in autumn than in those born in other seasons. Specifically, every 10 μg/m3 increase in ozone was associated with 0.68 (95% CI: 0.27, 1.10) YLL from hypertension in the elderly population born in autumn, while nonsignificant associations were found for those born in other seasons. The birth season, which affects the nutritional condition during early development, could affect vulnerability to the effect of ambient ozone exposure on the disease burden of hypertension in late life. The findings highlighted the importance of taking birth season into consideration when exploring the hypertensive effects of ozone exposure.

Abstract:Regional cerebral blood flow (rCBF) was measured in 13 patients with various cerebral disease by N-isopropyl-p-[123I]iodoamphetamine (IMP) SPECT with the microsphere model (rCBF (CbST)), which was compared with 133Xe inhalation SPECT (rCBFXe). Continuous withdrawal of arterial blood was performed for 5 minutes and SPECT image was obtained from 25 min to 55 min after the tracer injection. The reconstructed counts at 5 min was corrected using entire cerebral counts collected at 5 min, 20 min, and 60 min after the tracer injection. As compared with rCBFXe, rCBF (CbST) was underestimated in high flow areas and overestimated in low flow areas. Regional CBF was also measured using one minute short time SPECT images at 5 min (rCBF (Cb5)), 20 min (rCBF (Cb20)), and 60 min (rCBF (Cb60)) after the injection. Regional CBF tended to be underestimated with time from the tracer injection due to the wash-out from brain tissue to blood. In comparison with rCBF (Cb5), rCBF (CbST) was underestimated in high flow areas and overestimated in low flow areas. This error was considered to be due to the correction of reconstruct counts using entire cerebral counts, because of the kinetic behavior of 123I-IMP was different in each region of the brain.
