Abstract:The incidence of gliomas is approximately 3–5/100,000, with high-grade gliomas accounting for approximately 30–40% of these tumors. Surgery is a confirmed positive factor in prolonging the survival of these patients, and a larger resection range means a longer survival time. Therefore, surgery for high-grade glioma patients should aim to maximize the extent of resection while preserving neurological function to achieve a better quality of life. There is consensus regarding the need to lengthen progression-free survival (PFS) and overall survival (OS) times. In glioma surgery, methods such as intraoperative computed tomography (ICT), intraoperative magnetic resonance imaging (IMRI), navigation, 5-aminolevulinic acid (5-ALA), and intraoperative ultrasound (IOUS) are used to achieve an expanded resection during the surgical procedure. IOUS has been increasingly used in the surgery of high-grade gliomas and various tumors due to its convenient intraoperative use, its flexible repeatability, and the relatively low cost of operating room construction. With the continuous upgrading of ultrasound equipment, IOUS has been able to better assist surgeons in achieving an increased extent of resection. This review aims to summarize the application of ultrasound in the surgery of high-grade gliomas in the past decade, its improvement in patient prognosis, and its prospects.

Intraoperative ultrasound and magnetic resonance comparative analysis in brain tumor surgery: a valuable tool to flatten ultrasound's learning curve

Abstract:Background: Intraoperative ultrasound (IOUS) is a profitable tool for neurosurgical procedures' assistance, especially in neuro-oncology. It is a rapid, ergonomic and reproducible technique. However, its known handicap is a steep learning curve for neurosurgeons. Here, we describe an interesting postoperative analysis that provides extra feedback after surgery, accelerating the learning process. Method: We conducted a descriptive retrospective unicenter study including patients operated from intra-axial brain tumors using neuronavigation (Curve, Brainlab) and IOUS (BK-5000, BK medical) guidance. All patients had preoperative Magnetic Resonance Imaging (MRI) prior to tumor resection. During surgery, 3D neuronavigated IOUS studies (n3DUS) were obtained through craniotomy N13C5 transducer's integration to the neuronavigation system. At least two n3DUS studies were obtained: prior to tumor resection and at the resection conclusion. A postoperative MRI was performed within 48 h. MRI and n3DUS studies were posteriorly fused and analyzed with Elements (Brainlab) planning software, permitting two comparative analyses: preoperative MRI compared to pre-resection n3DUS and postoperative MRI to post-resection n3DUS. Cases with incomplete MRI or n3DUS studies were withdrawn from the study. Results: From April 2022 to March 2024, 73 patients were operated assisted by IOUS. From them, 39 were included in the study. Analyses comparing preoperative MRI and pre-resection n3DUS showed great concordance of tumor volume (p < 0,001) between both modalities. Analysis comparing postoperative MRI and post-resection n3DUS also showed good concordance in residual tumor volume (RTV) in cases where gross total resection (GTR) was not achieved (p < 0,001). In two cases, RTV detected on MRI that was not detected intra-operatively with IOUS could be reviewed in detail to recheck its appearance. Conclusions: Post-operative comparative analyses between IOUS and MRI is a valuable tool for novel ultrasound users, as it enhances the amount of feedback provided by cases and could accelerate the learning process, flattening this technique's learning curve.

Abstract:INTRODUCTION: Intraoperative imaging and neuronavigation aid in achieving maximum tumor resection. However, tissue removal during surgery can cause brain shift, which alters the relative anatomy and “de-calibrates” the standard approaches. While intraoperative MRI (iMRI) is the gold-standard to correct for brain shift, it is expensive, time-consuming, and logistically difficult. As an alternative, we investigated an approach which combines intraoperative ultrasound (ioUS) technology with preoperative imaging to allow for real-time compensation of brain shift and assessment of extent of resection (EOR). METHODS: We prospectively enrolled patients undergoing neurosurgical tumor resection at Massachusetts General Hospital beginning April 2023. Patients were registered with Brainlab Ultrasound Navigation prior to surgery. ioUS sweeps were acquired before cortex incision, mid-resection, and post-resection with bkActiv and fused to preoperative MRIs. Four points around the tumor were longitudinally tracked to assess brain shift. Data included sweep duration, image processing speed, extent of brain shift as determined by intraoperative markers, and EOR assessed from the ioUS compared with postoperative MRIs. RESULTS: Seven patients were enrolled (average age 62 ± 15, 75% male) with frontal (n = 5), temporal (n = 1), and occipital (n = 1) gliomas (n = 4) and metastases (n = 3). All patients exhibiting total resection as determined by ioUS also demonstrated total resection on post-operative MRI, evaluated independently by a neuroradiologist. The average time of scan and image processing (n = 19) was 5.7 ± 1.9 and 27.8 ± 6.5 seconds respectively. CONCLUSIONS: Navigated ioUS offers accurate and efficient assessment of EOR during surgery for tumor resections. Furthermore, when brain shift is detected, navigated ioUS can allow for compensation and continued utilization of navigation via the acquisition of a brand new, navigable, intraoperatively acquired dataset.

Abstract:Brain glioma, which is highly invasive and has a poor prognosis, is the most common primary intracranial tumor. Several studies have verified that the extent of resection is a considerable prognostic factor for achieving the best results in neurosurgical oncology. To obtain gross total resection (GTR), neurosurgery relies heavily on generating continuous, real-time, intraoperative glioma descriptions based on image guidance. Given the limitations of existing devices, it is imperative to develop a real-time image-guided resection technique to offer reliable functional and anatomical information during surgery. At present, the application of intraoperative ultrasound (IOUS) has been indicated to enhance resection rates and maximize brain function preservation. IOUS, which is promising due to its lower cost, minimal operational flow interruptions, and lack of radiation exposure, can enable real-time localization and precise tumor size and form descriptions while assisting in discriminating residual tumors and solving brain tissue shifts. Moreover, the application of new advancements in ultrasound technology, such as contrast-enhanced ultrasound (CEUS), three-dimensional ultrasound (3DUS), noninvasive ultrasound (NUS), and ultrasound elastography (UE), could assist in achieving GTR in glioma surgery. This article reviews the advantages and disadvantages of IOUS in glioma surgery.

Abstract:OBJECTIVE The use of intraoperative techniques to detect residual tumors has recently become increasingly important. Intraoperative MRI has long been considered the gold standard; however, it is not widely used because of high equipment costs and long acquisition times. Consequently, real-time intraoperative ultrasound (ioUS), which is much less expensive than MRI, has gained popularity. The aim of the present study was to evaluate the capacity of ioUS to accurately determine the primary tumor volume and detect residual tumors. METHODS A prospective study of adult patients who underwent surgery for intra-axial brain tumors between November 2017 and October 2020 was performed. Navigated intraoperative ultrasound (nioUS) of the brain was used to guide tumor resection and to detect the presence of residual disease. Both convex (5–8 MHz) and linear array (6–13 MHz) probes were used. Tumor volume and residual disease were measured with nioUS and compared with MR images. A linear regression model based on a machine learning pipeline and a Bland-Altman analysis were used to assess the accuracy of nioUS versus MRI. RESULTS Eighty patients (35 females and 45 males) were included. The mean age was 58 years (range 25–80 years). A total of 88 lesions were evaluated; there were 64 (73%) gliomas, 19 (21.6%) metastases, and 5 (5.7%) other tumors, mostly located in the frontal (41%) and temporal (27%) lobes. Most of the tumors (75%) were perfectly visible on ioUS (grade 3, Mair grading system), except for those located in the insular lobe (grade 2). The regression model showed a nearly perfect correlation (R 2 = 0.97, p < 0.001) between preoperative tumor volumes from both MRI and nioUS. Ultrasonographic visibility significantly influenced this correlation, which was stronger for highly visible (grade 3) tumors (p = 0.01). For residual tumors, the correlation between postoperative MRI and nioUS was weaker (R 2 = 0.78, p < 0.001) but statistically significant. The Bland-Altman analysis showed minimal bias between the two techniques for pre- and postoperative scenarios, with statistically significant results for the preoperative concordance. CONCLUSIONS The authors’ findings show that most brain tumors are well delineated by nioUS and almost perfectly correlated with MRI-based measurements both pre- and postoperatively. These data support the hypothesis that nioUS is a reliable intraoperative technique that can be used for real-time monitoring of brain tumor resections and to perform volumetric analysis of residual disease.

Abstract:INTRODUCTION: Intraoperative MRI (iMRI)-guided glioma surgery, has been assumed to enable reliable resection control to eliminate the effect of brain shift on extent of resection (EOR). Most previous studies are retrospective, so that at best level 2C evidence can be provided. We hypothesize that the use of high-field iMRI will increase EOR, reduce the morbidity and achieve more improved progress free survival (PFS) and quality of life than conventional neuronavigation. METHODS: A single center prospective randomized triple-blind controlled clinical trial was designed to assess the effect of iMRI on surgical efficacy of malignant gliomas. The estimated sample size was larger than 300 subjects in the intent-to-treat analysis to give 80% complete power (ie, probability of rejecting all false null hypotheses) with an experiment-wise type I error of 0.05. Randomization was conducted when the blinded surgeons deemed that the surgical goal was met, to avoid potential treatment bias. The blinded people included the patients, surgeons, the assessment personnel, and the statisticians. The protocol has been registered at ClinicalTrials.gov (NCT1479686). RESULTS: The primary endpoints were EOR (volumetric analysis) and surgical morbidity. The secondary endpoints were PFS and overall survival (OS). The first 78 patients were analyzed here. 3 patients with glioma of WHO I were excluded. Finally, 38 patients in the iMRI group and 37 ones in the control group were analyzed. Patient characteristics were balanced between both groups. The results between the 1st iMRI and control group are similar as expected. Complete resection rates in iMRI group and control group are 84% and 51% (P = .0907), respectively. Specifically, for high grade glioma, the rates are 88.24% and 66.67% (P = .1596); while for low grade glioma, they are 80.95% and 44% (P = .0090). CONCLUSION: We provided the first level 1 evidence for the application of 3.0T iMRI in glioma surgery, both in low-grade and high-grade glioma. Stryker Neuro-Oncology Award

Methods:Eighty-six consecutive patients with small, deep-seated, or ill-deifned intracerebral lesions were studied prospectively. An improved intraoperative imaging technique and surgical setup were practiced during the surgery. IOUS was performed in three orthogonal imaging planes (horizontal, coronal and sagittal). Results:Histopathological diagnoses of these 86 cases included cavernomas, metastases, hemangioblastomas, gliomas, and radiation necrosis. Forty-seven of the 86 lesions (54.7%) were small and deep-seated, 34 (39.5%) were ill-deifned, and 5 (5.8%) were small, deep-seated, and ill-deifned. Sonograms in the horizontal plane were obtained in all 86 cases. Sonograms in the sagittal plane and in the coronal plane were obtained only in 52 cases and in 46 cases, respectively, due to technical limitation. In 13 cases, sonograms in all three orthogonal planes were available. All lesions were successfully identiifed and localized by IOUS. Total resection was performed in 67 lesions (77.9%) and partial resection was performed in 19 lesions (22.1%). Conclusion:We propose IOUS to be performed in three orthogonal planes when surgery is planned for small, deep-seated, or ill-deifned brain lesions. By applying this simple, improved technique, surgeons can perform resection of these lesions precisely.
Full-course resection control strategy in glioma surgery using both intraoperative ultrasound and intraoperative MRI

Abstract:Background: Intraoperative ultrasound(iUS) and intraoperative MRI (iMRI) are effective ways to perform resection control during glioma surgery. However, most published studies employed only one modality. Few studies have used both during surgery. How to combine these two techniques reasonably, and what advantages they could have for glioma surgery are still open questions. Methods: We retrospectively reviewed a series of consecutive patients who underwent initial surgical treatment of supratentorial gliomas in our center. We utilized a full-course resection control strategy to combine iUS and iMRI: IUS for pre-resection assessment and intermediate resection control; iMRI for final resection control. The basic patient characteristics, surgical results, iMRI/iUS findings, and their impacts on surgical procedures were evaluated and reported. Results: A total of 40 patients were included. The extent of resection was 95.43 ± 10.37%, and the gross total resection rate was 72.5%. The median residual tumor size was 6.39 cm 3 (range 1.06–16.23 cm3). 5% (2/40) of patients had permanent neurological deficits after surgery. 17.5% (7/40) of patients received further resection after the first iMRI scan, resulting in four (10%) more patients achieving gross total resection. The number of iMRI scans per patient was 1.18 ± 0.38. The surgical time was 4.5 ± 3.6 hours. The pre-resection iUS scan revealed that an average of 3.8 borders of the tumor were beside sulci in 75% (30/40) patients. Intermediate resection control was utilized in 67.5% (27/40) of patients. In 37.5% (15/40) of patients, the surgical procedures were changed intraoperatively based on the iUS findings. Compared with iMRI, the sensitivity and specificity of iUS for residual tumors were 46% and 96%, respectively. Conclusion: The full-course resection control strategy by combining iUS and iMRI could be successfully implemented with good surgical results in initial glioma surgeries. This strategy might stabilize resection control quality and provide the surgeon with more intraoperative information to tailor the surgical strategy. Compared with iMRI-assisted glioma surgery, this strategy might improve efficiency by reducing the number of iMRI scans and shortening surgery time.

Analysis of Changes in KPS Scores and Imaging Signs in Deep Brain Gliomas by DWI Combined with Intraoperative Ultrasound Resection.

Abstract:Objective:This study investigates the efficacy of DWI combined with intraoperative ultrasound for deep brain glioma treatment, analyzing changes in Karnofsky performance status (KPS) scores and imaging signs. Objectives include elucidating the approach's advantages, addressing knowledge gaps, and contributing insights into its effectiveness for enhancing deep brain glioma management. Methods:In this retrospective study, we analyzed a total of 346 patients with deep brain glioma who underwent surgical treatment at our hospital from July 2015 to January 2022. After applying inclusion and exclusion criteria, 310 patients were selected and categorized into a control group (n = 150) and an observation group (n = 160) based on different auxiliary techniques of surgical treatment. The degree of resection and Karnofsky performance status (KPS) scores were assessed at 1 day preoperatively, 1 week, and 1 month postoperatively for both groups. Additionally, we conducted a comprehensive analysis of DWI and ultrasound imaging signs among patients with different grades of deep brain glioma. The study duration covered the specified period, and statistical analyses were performed to evaluate the outcomes. Results:In our study, the observation group demonstrated significantly improved resection degrees, with a total resection rate of 82.50% compared to the control group's 65.33%. Preoperative Karnofsky performance status scores showed no significant difference between groups (P > .05), but postoperative scores at 1 week and 1 month were significantly higher in the observation group (P < .05). Intraoperative ultrasound and DWI revealed distinct imaging signs differentiating low-grade and high-grade patients. These results highlight the efficacy of DWI combined with intraoperative ultrasound resection in enhancing resection outcomes and influencing postoperative Karnofsky performance status. Conclusions:DWI combined with intraoperative ultrasonic resection in deep brain glioma has a significant effect, with specific imaging signs, which can effectively improve the total resection rate and KPS score, and is worthy of clinical promotion. DWI combined with intraoperative ultrasound has important clinical significance in the resection of deep brain gliomas. The better resection results and improved postoperative Karnofsky performance-status score that we observed suggest a possible benefit in patient outcomes, which could influence treatment strategies. The precise imaging signs identified by this method provide valuable guidance for targeted and effective tumor resection.
3.0T Imri-Guided Resection of Eloquent High-Grade Gliomas: Preliminary Results of A Randomised Controlled Trial

Abstract:BackgroundHigh-field intraoperative magnetic resonance imaging (iMRI) has been studied in two single-centre randomised trials with patients who have glioblastoma and is hypothesised to enable a greater extent of tumour resection and prolong survival in patients with gliomas. iMRI for glioblastomas has also been shown to be significantly superior to 5-ALA and white-light surgery. However, the real value of iMRI is still a controversial issue. Given that use of this technology is relatively expensive, a high-quality prospective randomised trial is necessary before the approach can be made widely available. We have undertaken a single-centre prospective, randomised controlled trial to assess the clinical efficacy of 3.0T iMRI-guided resection of malignant gliomas compared with conventional neuronavigation only. Here we present the interim analysis of a long-term follow-up of patients with high-grade gliomas.MethodsEligible patients were aged 18–70 years with suspected (as assessed by consaltant surgeon), newly diagnosed malignant supratentorial glioma, met the Karnofsky performance scale 70 or above, and for whom gross total resection was intended. Patients were excluded if they had: tumour of the midline, basal ganglia, cerebellum, or brain stem; MRI contraindications; inability to give informed consent; renal or hepatic insufficiency; history of malignant tumours of other systems; or pathology other than astrocytoma, oligodendroglioma, or oligoastoryctoma (WHO grade II to IV). All participants were randomly assigned to either the trial group (iMRI group) or control group (conventional neuronavigation group) after giving written consent. All surgical protocols including conventional neuro-navigation are identical between the two groups; iMRI is specifically used in the trial group. The estimated sample size was 303 patients in the intent-to-treat analysis to give 80% complete power with a type I error of 0·05. Computational randomisation was done when maximal safe resection was achieved by the masked surgeons. The primary endpoint was extent of tumour resection. The secondary endpoints were progression-free survival and overall survival. This trial is registered with ClinicalTrial.gov, number NCT01479686. This study was approved by the Huashan Institutional Review Board. Written informed consents were given by patients or their attorneys before surgery.FindingsBetween March, 2012, and August, 2015, we enrolled 202 patients, 11 of whom were subsequently excluded because of unbefitting pathology. 177 patients were followed-up more than 6 months and subsequently analysed. 190 patients had low-grade gliomas (51 males vs 39 females, mean age 38·55 years [SD 9·56]), and 87 patients had high-grade gliomas (56 males vs 31 females, mean age 50·42 years [12·18). 43 of the patients with high-grade gliomas were enrolled in the iMRI group, and 44 patients with high-grade gliomas were enrolled in the control group. The extent of tumour resection (iMRI median 100% [IQR 70·87–100] vs control median 98·76% [51·81–100]) and the rate of gross total resection (86% vs 45%) were higher in the iMRI group than that in the control group (p<0·0001). Patients in the iMRI group who had eloquent high-grade gliomas had significantly longer progression-free survival (median not reached vs 13·2 months, p=0·012) and overall survival (median not reached vs 21·5 months, p=0·003) than patients in the control group. Abstract:Background: Glioma resection aims for maximal tumor removal while preserving neurological function. Neuronavigation systems (NS), with intraoperative imaging, have revolutionized this process through precise tumor localization and detailed anatomical navigation. Objective: To assess the efficacy and breadth of neuronavigation and intraoperative imaging in glioma resections, identify operational challenges, and provide educational insights to medical students and non-neurosurgeons regarding their practical applications. Methods: This systematic review analyzed studies from 2012 to 2023 on glioma patients undergoing surgical resection with neuronavigation, sourced from MEDLINE (PubMed), Embase, and Web of Science. A database-specific search strategy was employed, with independent reviewers screening for eligibility using Rayyan and extracting data using the Joanna Briggs Institute (JBI) tool. Results: The integration of neuronavigation systems with intraoperative imaging modalities such as iMRI, iUS, and 5-ALA significantly enhances gross total resection (GTR) rates and extent of resection (EOR). While advanced technology improves surgical outcomes, it does not universally reduce operative times, and its impact on long-term survival varies. Combinations like NS + iMRI and NS + 5-ALA + iMRI achieve higher GTR rates compared to NS alone, indicating that advanced imaging adjuncts enhance tumor resection accuracy and success. The results underscore the multifaceted nature of successful surgical outcomes. Conclusions: Integrating intraoperative imaging with neuronavigation improves glioma resection. Ongoing research is vital to refine technology, enhance accuracy, reduce costs, and improve training, considering various factors impacting patient survival.

Abstract:Background: Intraoperative magnetic resonance imaging (iMRI) enables real-time optimized extent of tumour resection (EOR) in glioma patients. However, there is no high-level evidence on its benefits. We conducted a clinical trial to assess the efficiency of iMRI-guided glioma resection.Methods: A randomized, parallel-controlled trial was launched enrolling patients with newly diagnosed cerebral glioma, aged 18-70 years, with intention to undergo gross total resection (GTR). Patients were randomly assigned to either the iMRI group or the conventional neuronavigation group (control group). The primary endpoint was the rate of GTR and the secondary endpoints were progression free survival (PFS) and overall survival (OS).Findings: A total of 321 patients were enrolled From March, 2012 to August 2018 with 161 patients randomly assigned to the iMRI group and 160 to the control group. GTR was achieved more in the iMRI group than in the control group (83.85% vs 50.00%, P = 0.0000). In the 188 high-graded glioma (HGG) patients, the rate of GTR of the iMRI group was higher than the control group (87.37% vs 56.99%, P = 0.0005). Correspondingly, the PFS of HGG patients in the iMRI group was significantly prolonged (P = 0.0017). Subgroup analysis showed that in patients with eloquent HGG, rate of GTR of the iMRI group was higher than that of the control group (88.14% vs 53.33%, P = 0.0006), which resulted in significantly longer OS in the iMRI group than in the control group (Median, 2.79 years vs 1.76 years; p=0.0282)Interpretation: This trial confirms that iMRI increases the EOR of gliomas, leading to prolonged survival of HGG, particularly in surgically challenging eloquent HGG. Effect of High-field iMRI Guided Resection in Cerebral Glioma Surgery: A Randomized Clinical Trial

Abstract:Background Extent of resection (EOR) in glioma contributes to longer survival. The purpose of NCT01479686 was to prove whether intraoperative magnetic resonance imaging (iMRI) increases EOR in glioma surgery and benefit survival. Methods Patients were randomized (1:1) to receive the iMRI (n=161) or the conventional neuronavigation (n=160). The primary endpoint was gross total resection (GTR); secondary outcomes reported were progression-free survival (PFS), overall survival (OS), and safety. Results 188 high-grade gliomas (HGGs) and 133 low-grade gliomas (LGGs) were enrolled. GTR was 83.85% in the iMRI group vs. 50.00% in the control group (P<0.0001). In 321 patients, the median PFS (mPFS) was 65.12 months in the iMRI group and 61.01 months in the control group (P=0.0202). For HGGs, mPFS was improved in the iMRI group (19.32 vs. 13.34 months, P=0.0015), and a trend of superior OS compared with control was observed (29.73 vs. 25.33 months, P=0.1233). In the predefined eloquent area HGG subgroup, mPFS, and mOS were 20.47 months and 33.58 months in the iMRI vs. 12.21 months and 21.16 months in the control group (P=0.0098; P=0.0375, respectively). From the exploratory analyses of HGGs, residual tumor volume (TV) <1.0 cm 3 decreased the risk of survival (mPFS: 18.99 vs. 9.43 months, P=0.0055; mOS: 29.77 vs. 18.10 months, P=0.0042). LGGs with preoperative (pre-OP) TV>43.1 cm 3 and postoperative (post-OP) TV>4.6 cm 3 showed worse OS (p=0.0117). Conclusions It showed that iMRI significantly increased EOR and indicated survival benefits for HGGs, particularly eloquent HGGs. Residual TV in either HGGs or LGGs is a prognostic factor for survival.

Abstract:OBJECTIVE This study aimed to assess the clinical efficiency of combined awake craniotomy with 3-T intraoperative MRI (iMRI)-guided resection of gliomas adjacent to eloquent cortex performed at a single center. It also sought to explore the contribution of iMRI to surgeons' learning process of maximal safe resection of gliomas. METHODS All patients who underwent an awake craniotomy and iMRI for resection of eloquent area glioma during the 53 months between January 2011 and June 2015 were included. The cases were analyzed for short- and long-term neurological outcome, progression-free survival (PFS), overall survival (OS), and extent of resection (EOR). The learning curve was assessed after dividing the cohort into Group A (first 27 months) and Group B (last 26 months). Statistical analyses included univariate logistic regression analysis on clinical and radiological variables. Kaplan-Meier and Cox regression models were used for further analysis of OS and PFS. A p value < 0.05 was considered statistically significant. RESULTS One hundred six patients were included in the study. Over an average follow-up period of 24.8 months, short- and long-term worsening of the neurological function was noted in 48 (46.2%) and 9 (8.7%) cases, respectively. The median and mean EOR were 100% and 92%, respectively, and complete radiographic resection was achieved in 64 (60.4%) patients. The rate of gross-total resection (GTR) in the patients with low-grade glioma (89.06% ± 19.6%) was significantly lower than that in patients with high-grade glioma (96.4% ± 9.1%) (p = 0.026). Thirty (28.3%) patients underwent further resection after initial iMRI scanning, with a 10.1% increase of the mean EOR. Multivariate Cox proportional hazards modeling demonstrated that the final EOR was a significant predictor of PFS (HR 0.225, 95% CI 0.070-0.723, p = 0.012). For patients with high-grade glioma, the GTR (p = 0.033), the presence of short-term motor deficit (p = 0.027), and the WHO grade (p = 0.005) were independent prognostic factors of OS. Performing further resection after the iMRI (p = 0.083) and achieving GTR (p = 0.05) demonstrated a PFS benefit trend for the patients affected by a low-grade glioma. Over time, the rate of performing further resection after an iMRI decreased by 26.1% (p = 0.005). A nonsignificant decrease in the rate of short-term (p = 0.101) and long-term (p = 0.132) neurological deficits was equally noted. CONCLUSIONS Combined awake craniotomy and iMRI is a safe and efficient technique allowing maximal safe resection of eloquent area gliomas with possible subsequent OS and PFS benefits. Although there is a learning curve for applying this technique, it can also improve the surgeon's ability in eloquent glioma surgery.
Establishment of Different Intraoperative Monitoring and Mapping Techniques and Their Impact on Survival, Extent of Resection, and Clinical Outcome in Patients with High-Grade Gliomas—A Series of 631 Patients in 14 Years

Abstract:BACKGROUND: The resection of brain tumors can be critical concerning localization, but is a key point in treating gliomas. Intraoperative neuromonitoring (IONM), awake craniotomy, and mapping procedures have been incorporated over the years. Using these intraoperative techniques, the resection of eloquent-area tumors without increasing postoperative morbidity became possible. This study aims to analyze short-term and particularly long-term outcomes in patients diagnosed with high-grade glioma, who underwent surgical resection under various technical intraoperative settings over 14 years. METHODS: A total of 1010 patients with high-grade glioma that underwent resection between 2004 and 2018 under different monitoring or mapping procedures were screened; 631 were considered eligible for further analyses. We analyzed the type of surgery (resection vs. biopsy) and type of IONM or mapping procedures that were performed. Furthermore, the impact on short-term (The National Institute of Health Stroke Scale, NIHSS; Karnofsky Performance Scale, KPS) and long-term (progression-free survival, PFS; overall survival, OS) outcomes was analyzed. Additionally, the localization, extent of resection (EOR), residual tumor volume (RTV), IDH status, and adjuvant therapy were approached. RESULTS: In 481 patients, surgery, and in 150, biopsies were performed. The number of biopsies decreased significantly with the incorporation of awake surgeries with bipolar stimulation, IONM, and/or monopolar mapping (p < 0.001). PFS and OS were not significantly influenced by any intraoperative technical setting. EOR and RTV achieved under different operative techniques showed no statistical significance (p = 0.404 EOR, p = 0.186 RTV). CONCLUSION: Based on the present analysis using data from 14 years and more than 600 patients, we observed that through the implementation of various monitoring and mapping techniques, a significant decrease in biopsies and an increase in the resection of eloquent tumors was achieved. With that, the operability of eloquent tumors without a negative influence on neurological outcomes is suggested by our data. However, a statistical effect of monitoring and mapping procedures on long-term outcomes such as PFS and OS could not be shown.

Abstract:OBJECTIVE: We describe a shared-resource intraoperative magnetic resonance imaging (MRI) design that allocates time for both surgical procedures and routine diagnostic imaging. We investigated the safety and efficacy of this design as applied to the detection of residual glioma immediately after an optimal image-guided frameless stereotactic resection (IGFSR).METHODS: Based on the twin operating rooms (ORs) concept, we installed a commercially available Hitachi AIRIS II, 0.3-tesla, vertical field, open MRI unit in its own specially designed OR (designated the magnetic resonance OR) immediately adjacent to a conventional neurosurgical OR. Between May 1998 and October 1999, this facility was used for both routine diagnostic imaging (969 diagnostic scans) and surgical procedures (50 craniotomies for tumor resection, 27 transsphenoidal explorations, and 5 biopsies). Our study group, from which prospective data were collected, consisted of 40 of these patients who had glioma (World Health Organization Grades II-IV). These 40 patients first underwent optimal IGFSRs in the adjacent conventional OR, where resection continued until the surgeon believed that all of the accessible tumor had been removed. Patients were then transferred to the magnetic resonance OR to check the completeness of the resection. If accessible residual tumor was observed, then a biopsy and an additional resection were performed. To validate intraoperative MRI findings, early postoperative MRI using a 1.5-tesla magnet was performed.RESULTS: Intraoperative images that were suitable for interpretation were obtained for all 40 patients after optimal IGFSRs. In 19 patients (47%), intraoperative MRI studies confirmed that adequate resection had been achieved after IGFSR alone. Intraoperative MRI studies showed accessible residual tumors in the remaining 21 patients (53%), all of whom underwent additional resections. Early postoperative MRI studies were obtained in 39 patients, confirming that the desired final extent of resection had been achieved in all of these patients. One patient developed a superficial wound infection, and no hazardous equipment or instrumentation problems occurred.CONCLUSION: Use of an intraoperative MRI facility that permits both diagnostic imaging and surgical procedures is safe and may represent a more cost-effective approach than dedicated intraoperative units for some hospital centers. Although we clearly demonstrate an improvement in volumetric glioma resection as compared with IGFSR alone, further study is required to determine the impact of this approach on patient survival.

The Use of Intraoperative MRI for the Treatment of Glioblastoma Multiforme

Abstract:Outcome for patients with glioblastoma multiforme (GBM) is improved with aggressive surgical resection followed by standard chemotherapy and radiation therapy. Intraoperative MRI (iMRI) is a recent surgical advancement that gives neurosurgeons direct radiographic feedback throughout a surgical procedure and thus enables maximal surgical resection of such lesions. We reviewed our series of 35 cases in 29 patients who underwent surgical resections of their GBM's using iMRI. The average extent of resection was 94% with only three patients having resections of less than 90%. However, even with such technology complete resection may not always be possible because of tumor location near critical brain structure or involvement of surrounding vascular structures. The addition of iMRI to the surgical armamentarium for treatment of patients with GBM improves the degree of tumor resection while minimizing complications and thus may positively impact survival in this patient population.

Abstract:Objective To summarize our experience of application of intraoperative magnetic resonance imaging (iMRI) in glioma surgery and to investigate its clinical efficiency.Methods A retrospective analysis was conducted to evaluate the extent of resection (EOR) and outcomes of 366 glioma patients who underwent craniotomy under guidance of iMRI from December 2010 to December 2016 at Department of Neurosurgery,Huashan Hospital,Fudan University.Regular follow-up and head MRI were performed for all patients.Overall survival (OS) and progression free survival (PFS) curves were analyzed by Kaplan-Meier method.Log-rank tests and Cox proportional hazards model were used for univariate and multivariate comparisons,respectively.Results A total of 366 glioma patients were enrolled into this study.The rate of gross total resection (GTR) was 67.2％.The GTR rate of glioblastoma multiform (GBM) (73.2％) was significantly higher than that of grade Ⅱ (58.5％,P ＜ 0.05) or grade Ⅲ glioma (61.9％,P ＜ 0.05).There was no significant difference in the GTR rate between grade Ⅱ and grade Ⅲ gliomas(P ＞ 0.05).Following the initial iMRI scan,90 patients underwent further resection with a significant improvement of mean EOR from 81.9％ to 95.9％ (P ＜0.001).Our results revealed that the gender,GTR and radio-chemotherapy were all independent prognostic risk factors in GBM patients.Age and location of tumor had no significant influence on the prognosis of GBM patients (all P ＞ 0.05).Conclusion The application of iMRI could efficiently increase the EOR of glioma surgery and improve the prognosis of glioma patients.
