Abstract:Abstract Despite progressive improvements over the decades, the rich temporally resolved data in an echocardiogram remain underutilized. Human assessments reduce the complex patterns of cardiac wall motion, to a small list of measurements of heart function. All modern echocardiography artificial intelligence (AI) systems are similarly limited by design – automating measurements of the same reductionist metrics rather than utilizing the embedded wealth of data. This underutilization is most evident where clinical decision making is guided by subjective assessments of disease acuity. Predicting the likelihood of developing post-operative right ventricular failure (RV failure) in the setting of mechanical circulatory support is one such example. Here we describe a video AI system trained to predict post-operative RV failure using the full spatiotemporal density of information in pre-operative echocardiography. We achieve an AUC of 0.729, and show that this ML system significantly outperforms a team of human experts at the same task on independent evaluation.

A Novel Method for Estimating Right Atrial Pressure With Point-of-Care Ultrasound

Abstract:Background: Current noninvasive estimation of right atrial pressure (RAP) by either bedside jugular venous pressure exam or inferior vena cava measurement during a comprehensive echocardiogram offers imprecise estimates of actual RAP. Methods: We enrolled 41 patients in a prospective, blinded study to validate a novel point-of-care ultrasound method using direct right atrial depth (RAD) measurement and jugular venous ultrasound to estimate RAP. Two subjects were excluded, and 39 were included in the final analysis. A parasternal long-axis view was obtained, and the depth of the noncoronary cusp attachment to the posterior left ventricular outflow tract was recorded as the RAD. This was added to an estimate of the jugular venous pressure obtained during a jugular vein ultrasound to calculate an estimated RAP (RAPUS). The RAPUS was compared to the RAP measurement during right heart catheterization (RAPcath) both as measured and as corrected for where the catheter was zeroed. Results: The correlation coefficient between RAPcath and RAPUS was +0.75; regression R2, 0.56; and bias, -0.49 mm Hg (95% CI, -1.42 to +0.43 mm Hg), with the limits of agreement -5.56 to +7.24 mm Hg and accuracy of 3 mm Hg or less in 29 (74%) of the subjects. For the RAPUS corrected for the catheter zero point, the correlation coefficient between RAPcath and RAPUS was +0.72; regression R2, 0.52; and bias, -0.60 mm Hg (95% CI, -1.60 to +0.39 mm Hg), with the limits of agreement -5.56 to +7.24 mm Hg and accuracy of 3 mm Hg or less in 26 (67%) of the subjects. Conclusion: This simple ultrasound evaluation of RAD and the right jugular vein correlates well with actual RAP and can accurately estimate RAP within 3 mm Hg in most patients. This has the potential to improve our bedside volume status exam, as well as improve the accuracy of RAP estimation during comprehensive echocardiogram.

Abstract:OBJECTIVE: Echocardiography, a critical tool for assessing left atrial (LA) volume, often relies on manual or semi-automated measurements. This study introduces a fully automated, real-time method for measuring LA volume in both 2-D and 3-D imaging, in the aim of offering accuracy comparable to that of expert assessments while saving time and reducing operator variability.METHODS: We developed an automated pipeline comprising a network to identify the end-systole (ES) time point and robust 2-D and 3-D U-Nets for segmentation. We employed data sets of 789 2-D images and 286 3-D recordings and explored various training regimes, including recurrent networks and pseudo-labeling, to estimate volume curves.RESULTS: Our baseline results revealed an average volume difference of 2.9 mL for 2-D and 7.8 mL for 3-D, respectively, compared with manual methods. The application of pseudo-labeling to all frames in the cine loop generally led to more robust volume curves and notably improved ES measurement in cases with limited data.CONCLUSION: Our results highlight the potential of automated LA volume estimation in clinical practice. The proposed prototype application, capable of processing real-time data from a clinical ultrasound scanner, provides valuable temporal volume curve information in the echo lab.

Abstract:Introduction: Echocardiography-based screening for rheumatic heart disease (RHD) in asymptomatic children in resource-limited settings results in early diagnosis, which, in combination with effective secondary prevention, may reduce morbidity and mortality. Physician-led screening, however, is not feasible in much of the developing world. Automated echocardiographic diagnosis via a novel deep learning approach may obviate the need for a physician, enabling more widespread echocardiographic screening, early diagnosis, and improved outcomes. In this study, we hypothesized that a deep learning model is capable of identifying mitral regurgitation (MR) in appropriate views. Methods: Echocardiogram clips were labeled by clip for view and presence of MR. Using the computational ability of a Nvidia GTX1080TI graphics processing unit, training dataset clips were used to build two convolutional neural networks to perform the stepwise tasks of classifying the clips 1) by view and 2) by the presence/absence of MR in appropriate color Doppler views. We trained the views classification model using 1857 clips of all views and the MR detection model using 686 parasternal long axis, parasternal short axis, and apical four chamber color Doppler clips. We then evaluated the performance of our models using a test dataset. Results are reported as an F1 score (an F1 score of 1 suggests perfect accuracy of the model). Results: Figure 1. a) View Classification Model Learning Curve, b) MR Detection Model Learning Curve Discussion: Our results suggest that a deep learning model is capable of discerning MR on echocardiogram clips. This is an encouraging step toward automated diagnosis of RHD, which will enable less skilled echocardiogram operators to make a real-time diagnosis of RHD.

Abstract:Background: It is unclear whether artificial intelligence (AI) can provide automatic solutions to measure right ventricular ejection fraction (RVEF), due to the complex RV geometry. Although several deep learning (DL) algorithms are available to quantify RVEF from cardiac magnetic resonance (CMR) images, there has been no systematic comparison of these algorithms, and the prognostic value of these automated measurements is unknown. We aimed to determine whether RVEF measurements made using DL algorithms could be used to risk stratify patients similarly to measurements made by an expert. Methods: We identified from a pre-existing registry 200 patients who underwent CMR. RVEF was determined using 3 fully automated commercial DL algorithms (DL-RVEF) and also by a clinical expert (CLIN-RVEF) using conventional methodology. Each of the DL-RVEF approaches was compared against CLIN-RVEF using linear regression and Bland-Altman analyses. In addition, RVEF values were classified according to clinically important cutoffs: <35%, 35-50%, ≥50%, and rates of disagreement with the reference classification were determined. ROC analysis was performed to evaluate the ability of CLIN-RVEF and each of the DL-RVEF based classifications to predict major adverse cardiovascular events (MACE). Results: The CLIN-RVEF and the three DL-RVEFs were obtained in all patients. We found only modest correlations between DL-RVEF and CLIN-RVEF (figure). The DL-RVEF algorithms had accuracy ranging from 0.59 to 0.78 for categorizing RV function. Nevertheless, ROC analysis showed no significant differences between the 4 approaches in predicting MACE, as reflected by respective AUC values of 0.68, 0.69, 0.64 and 0.63. Conclusions: Although the automated algorithms predicted patient outcomes as well as the CLIN-RVEF, the agreement between DL-RVEF and the clinical expert’s measurements was not optimal. DL approaches need further refinements to improve automated assessment of RV function.

Abstract:Background: Theoretically, artificial intelligence can provide an accurate automatic solution to measure right ventricular (RV) ejection fraction (RVEF) from cardiovascular magnetic resonance (CMR) images, despite the complex RV geometry. However, in our recent study, commercially available deep learning (DL) algorithms for RVEF quantification performed poorly in some patients. The current study was designed to test the hypothesis that quantification of RV function could be improved in these patients by using more diverse CMR datasets in addition to domain-specific quantitative performance evaluation metrics during the cross-validation phase of DL algorithm development. Methods: We identified 100 patients from our prior study who had the largest differences between manually measured and automated RVEF values. Automated RVEF measurements were performed using the original version of the algorithm (DL1), an updated version (DL2) developed from a dataset that included a wider range of RV pathology and validated using multiple domain-specific quantitative performance evaluation metrics, and conventional methodology performed by a core laboratory (CORE). Each of the DL-RVEF approaches was compared against CORE-RVEF reference values using linear regression and Bland-Altman analyses. Additionally, RVEF values were classified into 3 categories: ≤ 35%, 35-50%, and ≥ 50%. Agreement between RVEF classifications made by the DL approaches and the CORE measurements was tested. Results: CORE-RVEF and DL-RVEFs were obtained in all patients (feasibility of 100%). DL2-RVEF correlated with CORE-RVEF better than DL1-RVEF (r = 0.87 vs. r = 0.42), with narrower limits of agreement. As a result, DL2 algorithm also showed increasing accuracy from 0.53 to 0.80 for categorizing RV function. Conclusions: The use of a new DL algorithm cross-validated on a dataset with a wide range of RV pathology using multiple domain-specific metrics resulted in a considerable improvement in the accuracy of automated RVEF measurements. This improvement was demonstrated in patients whose images were the most challenging and resulted in the largest RVEF errors. These findings underscore the critical importance of this strategy in the development of DL approaches for automated CMR measurements.

Abstract:Abstract Background In patients with systolic chronic heart failure (CHF) clinical signs of congestion cannot always be evident at clinical examination. Right atrial pressure (RAP) measured by right heart catheterization (RHC) is an accurate and reproducible marker of blood volume. A non-invasive accurate tool to identify CHF patients with normal RAP would be desirable to tailor therapy. Purpose To validate an ultrasound (US)-assessed internal jugular vein distensibility (JVD) ratio to identify patients with normal mean RAP (defined as 7 mmHg or less) measured by RHC. Methods We first identify the JVD ratio that allows the most accurate identification of patients with normal RAP in a prospective calibration cohort of 100 patients with systolic CHF. Then, we tested the JVD ratio threshold to identify patients with normal RAP in a validation cohort of 101 consecutive patients with systolic CHF. All patients had a left ventricular ejection fraction (LVEF)<50% and underwent RHC in the setting of heart transplant work-up. At the time of jugular vein puncture, we recorded the internal jugular vein diameter by conventional linear probes. JVD ratio was calculated as the ratio between maximum diameter (during Valsalva maneuver) and rest diameter of the vein (FIGURE). Finally, we assessed the prognostic value of the JVD ratio in the follow up of the first 100 patients. Results In the calibration cohort (mean age 53 years, 13% female; median LVEF 25%, 81% in NYHA class III/IV) we define the best threshold of the JVD ratio to identify patients with normal RAP that has 1.6 with an area under the curve (AUC of 0.74; p<0.0001). Based on this JVD ratio threshold we defined patients with low JVD ratio (≤1.6; n=58; median RAP 8 mmHg) and patients with high JVD ratio (>1.6, n=42; median RAP 4 mmHg). High JVD ratio and low JVD ratio groups had similar clinical and laboratory characteristics. In the validation cohort (mean age 55 years, 13% female; median LVEF 25%; 56% in NYHA class III/IV) using the previously defined 1.6 JVD ratio threshold, we identified 51 patients with low JVD ratio (median RAP 8 mmHg) and 50 patients with high JVD ratio (median RAP 3 mmHg; p<0.0001) The JVD ratio threshold has an accuracy to identify patients with a normal RAP with an AUC of 0.82 (p<0.0001); a predictive positive value of 0.94, negative predictive value of 0.51, specificity of 0.90, and sensitivity of 0.65. Finally, in the calibration cohort, the CHF patients with low JVD ratio (≤1.6) had a higher cumulative incidence of overall death, heart transplant, or left ventricular assist device (42.7% vs. 16.1% in the high JVD ratio group, p log-rank 0.006) at a median of 13-month follow-up. Conclusions We found that US-assessed JVD ratio is a convenient and accurate diagnostic tool to identify patients with advanced systolic CHF with normal vs. increased RAP. Deep Learning Phenotyping of Tricuspid Regurgitation for Automated High Throughput Assessment of Transthoracic Echocardiography

Abstract:Background and Aims: Diagnosis of tricuspid regurgitation (TR) requires careful expert evaluation. This study developed an automated deep learning pipeline for assessing TR from transthoracic echocardiography. Methods: An automated deep learning workflow was developed using 47,312 studies (2,079,898 videos) from Cedars-Sinai Medical Center (CSMC) between 2011 and 2021. The pipeline was tested on a temporally distinct test set of 2,462 studies (108,138 videos) obtained in 2022 at CSMC and a geographically distinct cohort of 5,549 studies (278,377 videos) from Stanford Healthcare (SHC). Results: In the CSMC test dataset, the view classifier demonstrated an AUC of 1.000 (0.999–1.000) and identified at least one A4C video with colour Doppler across the tricuspid valve in 2,410 of 2,462 studies with a sensitivity of 0.975 (0.968–0.982) and a specificity of 1.000 (1.000–1.000). In the CSMC test cohort, moderate-or-severe TR was detected with an AUC of 0.928 (0.913–0.943) and severe TR was detected with an AUC of 0.956 (0.940–0.969). In the SHC cohort, the view classifier correctly identified at least one TR colour Doppler video in 5,268 of the 5,549 studies, resulting in an AUC of 0.999 (0.998–0.999), a sensitivity of 0.949 (0.944–0.955) and specificity of 0.999 (0.999–0.999). The AI model detected moderate-or-severe TR with an AUC of 0.951 (0.938–0.962) and severe TR with an AUC of 0.980 (0.966–0.988). Conclusions: We developed an automated pipeline to identify clinically significant TR with excellent performance. This approach carries potential for automated TR detection and stratification for surveillance and screening.

Similarly, for the uncorrected RAPU measurement, the correlation coefficient was +0.75, regression R2 0.56, bias -0.49 mmHg (95% CI, -1.42 to +0.43 mmHg) with the limits of agreement -5.56 to +7.24 mmHg, and 3 mmHg accuracy of 29 (74%). This simple bedside evaluation of right atrial depth and the right jugular vein correlates with actual right atrial pressure better than traditional IVC parameters, and can accurately estimate RAP within 3mmHg in most patients.
Machine learning algorithm using publicly available echo database for simplified “visual estimation” of left ventricular ejection fraction

Abstract:BACKGROUND: Left ventricular ejection fraction calculation automation typically requires complex algorithms and is dependent of optimal visualization and tracing of endocardial borders. This significantly limits usability in bedside clinical applications, where ultrasound automation is needed most.AIM: To create a simple deep learning (DL) regression-type algorithm to visually estimate left ventricular (LV) ejection fraction (EF) from a public database of actual patient echo examinations and compare results to echocardiography laboratory EF calculations.METHODS: A simple DL architecture previously proven to perform well on ultrasound image analysis, VGG16, was utilized as a base architecture running within a long short term memory algorithm for sequential image (video) analysis. After obtaining permission to use the Stanford EchoNet-Dynamic database, researchers randomly removed approximately 15% of the approximately 10036 echo apical 4-chamber videos for later performance testing. All database echo examinations were read as part of comprehensive echocardiography study performance and were coupled with EF, end systolic and diastolic volumes, key frames and coordinates for LV endocardial tracing in csv file. To better reflect point-of-care ultrasound (POCUS) clinical settings and time pressure, the algorithm was trained on echo video correlated with calculated ejection fraction without incorporating additional volume, measurement and coordinate data. Seventy percent of the original data was used for algorithm training and 15% for validation during training. The previously randomly separated 15% (1263 echo videos) was used for algorithm performance testing after training completion. Given the inherent variability of echo EF measurement and field standards for evaluating algorithm accuracy, mean absolute error (MAE) and root mean square error (RMSE) calculations were made on algorithm EF results compared to Echo Lab calculated EF. Bland-Atlman calculation was also performed. MAE for skilled echocardiographers has been established to range from 4% to 5%.RESULTS: The DL algorithm visually estimated EF had a MAE of 8.08% (95%CI 7.60 to 8.55) suggesting good performance compared to highly skill humans. The RMSE was 11.98 and correlation of 0.348.CONCLUSION: This experimental simplified DL algorithm showed promise and proved reasonably accurate at visually estimating LV EF from short real time echo video clips. Less burdensome than complex DL approaches used for EF calculation, such an approach may be more optimal for POCUS settings once improved upon by future research and development.

Abstract:BACKGROUND: Echocardiography-based screening for valvular disease in at-risk asymptomatic children can result in early diagnosis. These screening programs, however, are resource intensive and may not be feasible in many resource-limited settings. Automated echocardiographic diagnosis may enable more widespread echocardiographic screening, early diagnosis, and improved outcomes. In this feasibility study, the authors sought to build a machine learning model capable of identifying mitral regurgitation (MR) on echocardiography.METHODS: Echocardiograms were labeled by clip for view and by frame for the presence of MR. The labeled data were used to build two convolutional neural networks to perform the stepwise tasks of classifying the clips (1) by view and (2) by the presence of any MR, including physiologic, in parasternal long-axis color Doppler views. The view classification model was developed using 66,330 frames, and model performance was evaluated using a hold-out testing data set with 45 echocardiograms (11,730 frames). The MR detection model was developed using 938 frames, and model performance was evaluated using a hold-out testing data set with 42 echocardiograms (182 frames). Metrics to evaluate model performance included accuracy, precision, recall, F1 score (average of precision and recall, ranging from 0 to 1, with 1 suggesting perfect precision and recall), and receiver operating characteristic analysis.RESULTS: For the parasternal long-axis view with color Doppler, the view classification convolutional neural network achieved an F1 score of 0.97. The MR detection convolutional neural network achieved testing accuracy of 0.86 and an area under the receiver operating characteristic curve of 0.91.CONCLUSIONS: A machine learning model is capable of discerning MR on transthoracic echocardiography. This is an encouraging step toward machine learning-based diagnosis of valvular heart disease on pediatric echocardiography.

Fully Automated Mitral Inflow Doppler Analysis Using Deep Learning

Abstract:Echocardiography (echo) is an indispensable tool in a cardiologist's diagnostic armamentarium. To date, almost all echocardiographic parameters require time-consuming manual labeling and measurements by an experienced echocardiographer and exhibit significant variability, owing to the noisy and artifact-laden nature of echo images. For example, mitral inflow (MI) Doppler is used to assess left ventricular (LV) diastolic function, which is of paramount clinical importance to distinguish between different cardiac diseases. In the current work we present a fully automated workflow which leverages deep learning to a) label MI Doppler images acquired in an echo study, b) detect the envelope of MI Doppler signal, c) extract early and late filing (E and A wave) flow velocities and E-wave deceleration time from the envelope. We trained a variety of convolutional neural networks (CNN) models on 5544 images of 140 patients for predicting 24 image classes including MI Doppler images and obtained overall accuracy of 0.97 on 1737 images of 40 patients. Automated E and A wave velocity showed excellent correlation (Pearson R 0.99 and 0.98 respectively) and Bland Altman agreement (mean difference 0.06 and 0.05 m/s respectively and SD 0.03 for both) with the operator measurements. Deceleration time also showed good but lower correlation (Pearson R 0.82) and Bland-Altman agreement (mean difference: 34.1ms, SD: 30.9ms). These results demonstrate feasibility of Doppler echocardiography measurement automation and the promise of a fully automated echocardiography measurement package.

Abstract:Abstract Aims In patients with chronic advanced heart failure (HF) signs of congestion are not always evident on clinical examination. We aim to validate the ultrasound (US)-assessed jugular vein distensibility (JVD), as a non-invasive tool to identify patients with normal right atrial pressure (RAP≤7 mmHg). Methods and Results In a single-center prospective study, we assessed chronic HF patients with a left ventricular ejection fraction (LVEF)<50% who underwent a pulmonary artery catheterization in the setting of advanced HF therapies workup. We first identify the JVD threshold (Valsalva/rest ratio of the vein diameter) of 1.6 that allowed the most accurate discrimination between patients with RAP≤7 vs. >7 mmHg (area under the curve [AUC] of 0.74; p<0.0001) in a calibration cohort of 100 patients (mean age 53 years, median LVEF 25%). Based on this JVD threshold, we defined patients with low JVD (≤1.6; n=58; median RAP 8 mmHg) and high JVD (>1.6, n=42; median RAP 4 mmHg). Then, we tested the threshold in 101 patients (validation cohort), where we found comparable results (AUC of 0.82; p<0.0001). The JVD threshold had 0.86 and 0.94 predictive positive values to identify patients with RAP≤7 mmHg in the calibration and validation cohorts. Finally, the low JVD vs. high JVD group had a 42.7% vs. 16.1% incidence of major cardiac events at 2 years (log-rank p=0.006), showing its prognostic value. Conclusion US-assessed JVD is an accurate diagnostic tool to identify advanced HF patients with normal RAP. This tool could be tested in the ambulatory setting to modulate diuretic/vasodilator therapies.

An Artificial Intelligence-Based Automated Echocardiographic Analysis: Enhancing Efficiency and Prognostic Evaluation in Patients With Revascularized STEMI

Abstract:Background and objectives: Although various cardiac parameters on echocardiography have clinical importance, their measurement by conventional manual methods is time-consuming and subject to variability. We evaluated the feasibility, accuracy, and predictive value of an artificial intelligence (AI)-based automated system for echocardiographic analysis in patients with ST-segment elevation myocardial infarction (STEMI). Methods: The AI-based system was developed using a nationwide echocardiographic dataset from five tertiary hospitals, and automatically identified views, then segmented and tracked the left ventricle (LV) and left atrium (LA) to produce volume and strain values. Both conventional manual measurements and AI-based fully automated measurements of the LV ejection fraction and global longitudinal strain, and LA volume index and reservoir strain were performed in 632 patients with STEMI. Results: The AI-based system accurately identified necessary views (overall accuracy, 98.5%) and successfully measured LV and LA volumes and strains in all cases in which conventional methods were applicable. Inter-method analysis showed strong correlations between measurement methods, with Pearson coefficients ranging 0.81-0.92 and intraclass correlation coefficients ranging 0.74-0.90. For the prediction of clinical outcomes (composite of all-cause death, re-hospitalization due to heart failure, ventricular arrhythmia, and recurrent myocardial infarction), AI-derived measurements showed predictive value independent of clinical risk factors, comparable to those from conventional manual measurements. Conclusions: Our fully automated AI-based approach for LV and LA analysis on echocardiography is feasible and provides accurate measurements, comparable to conventional methods, in patients with STEMI, offering a promising solution for comprehensive echocardiographic analysis, reduced workloads, and improved patient care.
Automated estimation of mitral annular plane systolic excursion by artificial intelligence from 3D ultrasound recordings

Abstract:Perioperative monitoring of cardiac function is beneficial for early detection of cardiovascular complications. The standard of care for cardiac monitoring performed by trained cardiologists and anesthesiologists involves a manual and qualitative evaluation of ultrasound imaging, which is a time-demanding and resource-intensive process with intraobserver- and interobserver variability. In practice, such measures can only be performed a limited number of times during the intervention. To overcome these difficulties, this study presents a robust method for automatic and quantitative monitoring of cardiac function based on 3D transesophageal echocardiography (TEE) B-mode ultrasound recordings of the left ventricle (LV). Such an assessment obtains consistent measurements and can produce a near real-time evaluation of ultrasound imagery. Hence, the presented method is time-saving and results in increased accessibility. The mitral annular plane systolic excursion (MAPSE), characterizing global LV function, is estimated by landmark detection and cardiac view classification of two-dimensional images extracted along the long-axis of the ultrasound volume. MAPSE estimation directly from 3D TEE recordings is beneficial since it removes the need for manual acquisition of cardiac views, hence decreasing the need for interference by physicians. Two convolutional neural networks (CNNs) were trained and tested on acquired ultrasound data of 107 patients, and MAPSE estimates were compared to clinically obtained references in a blinded study including 31 patients. The proposed method for automatic MAPSE estimation had low bias and low variability in comparison to clinical reference measures. The method accomplished a mean difference for MAPSE estimates of (-0.16±1.06) mm. Thus, the results did not show significant systematic errors. The obtained bias and variance of the method were comparable to inter-observer variability of clinically obtained MAPSE measures on 2D TTE echocardiography. The novel pipeline proposed in this study has the potential to enhance cardiac monitoring in perioperative- and intensive care settings.

Abstract:Abstract INTRODUCTION patients undergoing planned cardioversion have high risk of left atrial appendage (LAA) trombosis. Transesophageal echocardiography (TEE) is usually performed to rule out LAA thrombosis. PURPOSE building a model for predicting the risk of thrombosis of LAA prior to TEE in patients with atrial fibrillation (AF) planned to cardioversion based anamnesis, clinic and transthoracic echocardiography (echo) using deep learning algorithms. METHODS From 12.2018, 100 patients with AF included in the hospital registry or planned cardioversion. All patients underwent echo and TEE . Other clinical data was collected from medical records, echo and TEE results. Deep learning neural network was constructed in R v. 3.5.3 for risk assessment of the LAA trombosis prior to TEE. The model was trained / verified for 70% / 30% included patients. RESULTS Among included, 47.0% were women, mean age was 67.8 ± 11.9 years. LAA thrombosis rate was 37.0%. The paroxysmal form of AF diagnosed in 22.0%, the rest had persistent form. Arterial hypertension was observed in 77.0%, diabetes mellitus - in 14.0%. Previous stroke was diagnosed in 5.0% of patients, previous myocardial infarction — in 3.0%, heart failure - in 51.0% of patients. Median of CHA2DS2VASc score was 3 (2; 4) points. The mean indexed volume of left atrium was 50.1 ± 13.7 ml/m2, for LAA - 4.0 ± 1.8 ml/m2. Mean ejection velocity in the LAA is 3.5 ± 1.3 cm/s. Before admission 24.0% of patients did not receive any anticoagulant therapy. The group was divided into «training» and «test» parts in the proportion of 70%/30%. Both groups were statistically comparable by cardiovascular deseases, risk factors, demography, age and treatment before admission. Neural network was trained on the following factors of the «training» group: gender, previous infarction, heart failure, score for CHA2DS2VASc, creatinine level, left atrial indexed volume and linear dimensions, transmitral E and mitral annulus e`, peak and mean pressures and square of mitral regurgitation, pulmonary veins systolic and diastolic flows, anamnestic anticoagulation and duration of its intake. 10-fold cross-validation was performed. Verification (comparison of the real results with the predicted by model) was made for the «test» group: the Cohen’s kappa was 0.68, 95%CI for positive predictive value was 65.3 - 98.6%, for negative predictive value - 35.9 - 99.6%, for model accuracy 67.6 - 97.3% , ROC analysis showed 91.7% for area under curve (95%CI 79.3 - 99.9%). CONCLUSION deep learning neural net for assessing the risk of thrombosis of LAA using only anamnestic and echo data can be used for experimentally prediction for pretest probability of thrombosis prior to TEE. Further verification of the model with more data is required.

Abstract:Circulation: Heart Failure, Ahead of Print. BACKGROUND:Clinical evaluation of central venous pressure is difficult, depends on experience, and is often inaccurate in patients with chronic advanced heart failure. We assessed the ultrasound-assessed internal jugular vein (JV) distensibility by ultrasound as a noninvasive tool to identify patients with normal right atrial pressure (RAP ≤7 mm Hg) in this population.METHODS:We measured JV distensibility as the Valsalva-to-rest ratio of the vein diameter in a calibration cohort (N=100) and a validation cohort (N=101) of consecutive patients with chronic heart failure with reduced ejection fraction who underwent pulmonary artery catheterization for advanced heart failure therapies workup.RESULTS:A JV distensibility threshold of 1.6 was identified as the most accurate to discriminate between patients with RAP ≤7 versus >7 mm Hg (area under the receiver operating characteristic curve, 0.74 [95% CI, 0.64–0.84]) and confirmed in the validation cohort (receiver operating characteristic, 0.82 [95% CI, 0.73–0.92]). A JV distensibility ratio >1.6 had predictive positive values of 0.86 and 0.94, respectively, to identify patients with RAP ≤7 mm Hg in the calibration and validation cohorts. Compared with patients from the calibration cohort with a high JV distensibility ratio (>1.6; n=42; median RAP, 4 mm Hg; pulmonary capillary wedge pressure, 11 mm Hg), those with a low JV distensibility ratio (≤1.6; n=58; median RAP, 8 mm Hg; pulmonary capillary wedge pressure, 22 mm Hg;P<0.0001 for both) were more likely to die or undergo a left ventricular assist device implant or heart transplantation (event rate at 2 years: 42.7% versus 18.2%; log-rankP=0.034).CONCLUSIONS:Ultrasound-assessed JV distensibility identifies patients with chronic advanced heart failure with normal RAP and better outcomes.REGISTRATION:URL:https://www.clinicaltrials.gov; Unique identifier: NCT03874312.

Abstract:The authors defined esophageal anatomy and evaluated esophageal cancer staging in a pilot group by comparing endoscopic magnetic resonance (MR) imaging results with pathologic and endoscopic ultrasonographic (US) results when available. A porcine esophagus, one volunteer, and 23 patients suspected of having esophageal cancer were imaged at 0.5 T. MR imaging was successful in 21 patients. Eight of these patients underwent esophagectomy (one after chemotherapy, which invalidated comparison with MR imaging; another did not undergo lymphadenectomy) and one underwent laparoscopy and nodal staging only; eight underwent US. When verified with pathologic staging, endoscopic MR imaging was accurate in six of seven patients (T stage) and five of six patients (N stage; nodal areas too obscured by artifact for comparison in one case). MR imaging and US results concurred in seven of eight (T stage) and five of eight (N stage) patients. No complications were observed. Endoscopic MR imaging is safe and probably comparable to endoscopic US, but with a tendency to overstage the disease.
