The association of third molars with mandibular angle fractures: a meta-analysis.

Abstract:OBJECTIVE To estimate the relative risk of mandibular angle fractures among people with a lower third molar compared with those without a lower third molar. METHODS Data for a case-control meta-analysis were obtained by performing a literature search in MEDLINE and EMBASE to identify suitable observational studies. To be included, studies had to present data on patients with mandibular fractures, incorporate cross-classified information about the presence of a lower third molar and indicate whether the fracture was a mandibular angle fracture on the ipsilateral side. RESULTS Six studies, involving 3,002 patients with mandibular fractures, met the inclusion criteria. Crude relative risk estimates for an angle fracture, comparing patients with a third molar with those without, ranged from 1.2 to 12.7. There was evidence of heterogeneity across the 6 studies (p = 0.001), but when 2 studies with less methodologic rigour were excluded, a test of homogeneity was no longer statistically significant (p = 0.22). The estimated relative risk across the remaining 4 studies was 2.4 (95% CI 1.9 to 3.0). CONCLUSIONS The presence of a lower third molar may double the risk of an angle fracture of the mandible. This could have a bearing on any clinical decision on whether to extract the molar.
Laparoscopic Management of Cholecystoduodenal and Cholecystocolic Fistula: A Clinical Case Report

Abstract:Biliary fistula is a rare (less than 8%) cholecystectomy complication, internal fistulae being the most common of them (mainly colonic and duodenal). However, the presence of two fistulas at the same time is extremely rare, with a small number of cases reported in the literature to date. Symptoms tend to be non-specific, leading to a difficult preoperative diagnosis. The standard treatment for bilioenteric fistulas is open cholecystectomy and subsequent closure of the fistula. Nonetheless, modern techniques including laparoscopic and endoscopic approaches have been reported lately for their treatment with favorable results. We present a case of concomitant cholecystoduodenal and cholecystocolic fistula successfully treated with subtotal cholecystectomy and primary closure of the fistulous tracts by laparoscopic approach in a female Hispanic patient.

Abstract:In this work, we numerically designed and then experimentally verified a metamaterial perfect absorber based on artificial dielectric "atoms". This metamaterial absorber is composed of dielectric ceramic material (SrTiO3) "atoms" embedded in a background matrix on a metal plate. The dielectric "atoms" couple strongly to the incident electric and magnetic fields at the Mie resonance mode, leading to the narrow perfect absorption band with simulated and experimental absorptivities of 99% and 98.5% at 8.96 GHz, respectively. The designed metamaterial perfect absorber is polarization insensitive and can operate in wide angle incidence.

Abstract:Cholecystoenteric fistula is a rare complication of gallbladder disease, with an incidence ranging from 0.06% to 0.14% among patients with biliary disease. This condition is most discovered intraoperatively during cholecystectomy. The duodenum is the most frequent site for these fistulas, followed by the colon. Patients with this complication have increased morbidity and mortality, making early recognition crucial. We hereby present a unique case of cholecystocolonic fistula identified during a routine outpatient colonoscopy.

Abstract:A middle-aged woman presented to us with complaints of purulent discharge which was coming from the epigastric port site for two years following laparoscopic cholecystectomy, which was performed at an outside hospital for asymptomatic gall stone disease. Computed tomography and magnetic resonance imaging (MRI) confirmed the fistulous sinus track communicating with the anterior body wall of the stomach. At re-laparotomy, the sistulous sinus track was found and excised with primary repair of the defect in the stomach wall. The patient had an uneventful post-operative recovery and was doing well on follow-up. This was the second such case reported in literature where a common surgical procedure like laparoscopic cholecystectomy has led to the formation of gastrocutaneous fistula, which is a rare complication and was successfully managed.

Abstract:Background: Cholecystoenteric fistulae are rare complications of gallstone disease, with a reported incidence of 0.5% to 0.9% of cholecystectomies. Cholecystoduodenal is the most common fistula followed by cholecystocolonic fistulae. Summary: We report a case of pneumobilia resulting from a combined cholecystoduodenal and cholecystocolonic fistulae treated with a laparoscopic subtotal cholecystectomy and open repair of the enteric fistulae. Conclusion: Combined cholecystoduodenal and cholecystocolonic fistulae are an extremely rare complication of gallstone disease, and meticulous preoperative planning and operative dexterity are needed to safely manage these unusual fistulae.

Abstract:Introduction: Cholecystocolonic fistula is a very rare complication of chronic cholecystitis, often diagnosed intraoperatively during interventions for the complications that it could cause. Clinical presentation is nonspecific, and no consensus exists on the management of fistula. Methods: A case report and literature review of 14 articles published between 2009 and 2020 were included in this study. Most of the articles included were a combination of case report with literature review, and the remainder were solely case reports or literature reviews. Results: History, physical examination, and radiologic studies have been unsuccessful in the preoperative diagnosis of cholecystocolonic fistula. Treatment choice is balanced on the risks and benefits and is case specific; however, it is always directed toward resolving the urgent complication. Conclusion: Even though cholecystocolonic fistula is a rare condition, it should be considered in elderly patients with a hx of cholelithiasis and diverticulosis presenting with bowel obstruction. This case report was prepared following the CARE Guidelines.

Abstract:Introduction: Gallstone with acute cholecystitis is one of the most common diseases in the clinic. If the disease is serious, gallbladder gangrene, perforation, and sepsis may be caused. Gallbladder diseases rarely cause thoracic-related complications, especially pleural fistula, which is very rare in clinical practice. Patient concerns: A 52-year-old male patient was admitted to the emergency department for 1 month with recurrent right middle and upper abdominal pain. Diagnosis: Computed tomography diagnosis: cholecystitis and peri-inflammation, small abscess around the base of the gallbladder, local peritonitis, and bilateral pleural effusion. Interventions: After admission, conservative treatment was given. On the 4th day of admission, the symptoms worsened, and an emergency catheter drainage was performed on the right thoracic cavity to extract 900 mL of dark yellow effusion. After the operation, a large amount of bili-like fluid was continuously drained from the thoracic drainage tube. After the iatrogenic biliary fistula caused by thoracic puncture was excluded, cholecystopleural fistula was considered to be cholecystopleural fistula. On the 6th day of admission, endoscopic retrograde cholangiopancreatography (ERCP) + cholecystography + Oddi sphincterotomy + laminating biliary stent was performed in the emergency department, and cholecystopleural fistula was confirmed during the operation. Outcomes: The patient recovered well after surgery, computed tomography examination on the 20th day after surgery indicated that pleural effusion was significantly reduced, and the patient was cured and discharged. The patient returned to the hospital 8 months after the ERCP operation to pull out the bile duct–covered stent. The patient did not complain of any discomfort after the postoperative follow-up for 3 years, and no recurrence of stones, empyema, and other conditions was found. Conclusion: Cholecystopleural fistula is one of the serious complications of acute cholecystitis, which is easy to misdiagnose clinically. If the gallbladder inflammation is severe, accompanied by pleural effusion, the pleural effusion is bili-like liquid, or the content of bilirubin is abnormally elevated, the existence of the disease should be considered. Once the diagnosis is clear, active surgical intervention is needed to reduce the occurrence of complications. Endoscopic therapy (ERCP) can be used as both a diagnostic method and an important minimally invasive treatment.

Abstract:The cholecystocolonic fistula (CCF) is an atypical variant of biliary disease, and it is the second most common intestinal fistula after cholecystoduodenal fistula. Intraoperative diagnosis is frequent, which implies challenging surgical management, especially in patients, often aged, with comorbidities. The rarity of this condition, atypical and various presentation, diagnostic and management complexity, makes it a unique surgical entity. We report our experience of an 84-year-old man with a history of chronic cholecystitis who presented with nonspecific symptoms. The imaging tests aroused the suspicion of gallbladder-colic fistula in the preoperative diagnosis, facilitating the subsequent surgical treatment that confirmed the diagnosis.

Abstract:Background: Cholecystoenteric fistula (CEF) involves the formation of a spontaneous anomalous tract between the gallbladder and the adjacent gastrointestinal tract. Chronic gallbladder inflammation can lead to tissue necrosis, perforation, and fistulogenesis. The most prevalent cause of CEF is chronic cholelithiasis, which rarely results from malignancy. Because the symptoms and laboratory findings associated with CEF are nonspecific, the condition is often misdiagnosed, presenting a challenge to the surgeon when detected intraoperatively. Therefore, a preoperative diagnosis of CEF is crucial. Case summary: We present the case of a 57-year-old male with advanced gallbladder cancer (GBC) who arrived at the emergency room with persistent vomiting, abdominal pain, and diarrhea. An abdominopelvic computed tomography scan revealed a contracted gallbladder with bubbles in the fundus connected to the second portion of the duodenum and transverse colon. We suspected that GBC had invaded the adjacent gastrointestinal tract through a cholecystoduodenal fistula (CDF) or a cholecystocolonic fistula (CCF). He underwent multiple examinations, including esophagogastroduodenoscopy, an upper gastrointestinal series, colonoscopy, and magnetic resonance cholangiopancreatography; the results of these tests confirmed a diagnosis of synchronous CDF and CCF. The patient underwent a Roux-en-Y gastrojejunostomy and loop ileostomy to address the severe adhesions that were previously observed to cover the second portion of the duodenum and hepatic flexure of the colon. His symptoms improved with supportive treatment while hospitalized. He initiated oral targeted therapy with lenvatinib for further anticancer treatment. Conclusion: The combination of imaging and surgery can enhance preoperative diagnosis and alleviate symptoms in patients with GBC complicated by CEF.

Abstract:Xanthogranulomatous cholecystitis is a rare chronic inflammatory disease of the gallbladder associated with complications such as perforation, dense adhesions and fistulation. We present a case of a female patient in her 20s who presented with three episodes of recurrent cholecystitis over 3 months. Her medical history included an endoscopic retrograde cholangiopancreatography for obstructive jaundice due to choledocholithiasis. As there was no possibility of performing an urgent cholecystectomy during her initial admission, she was listed for an elective operation. Laparoscopic inspection revealed a pericholecystic mass involving the omentum, transverse colon, duodenum and liver and findings suspicious of a cholecystoappendiceal fistula. Laparoscopic cholecystectomy and appendicectomy were performed. Early index admission laparoscopic cholecystectomy should be performed in patients with acute cholecystitis to prevent higher grades of operative difficulty and associated complications. This case presentation-cholecystoappendiceal fistula associated with xanthogranulomatous cholecystitis-emphasises the necessity of complying with national guidelines in managing acute calculous cholecystitis.

Abstract:Pseudomonas sp. 101 carboxyl proteinase (PCP) and Xanthomonas sp. T-22 carboxyl proteinase (XCP), the first and second unique carboxyl proteinases from prokaryotes to be isolated and characterized, are not inhibited by the classical carboxyl proteinase inhibitor pepstatin. In this study, we elucidated their subsite preferences by using a series of synthetic chromogenic substrates, Lys-Pro-Ile(P3)-Glu(P2)-Phe*Nph-Arg(P2')-Leu(P3') (Nph is p-nitrophenylalanine, Phe*Nph is the cleavage site) with systematic substitutions at the P3, P2, P2', and P3' positions. Among 45 substrates tested, the best substrate for PCP had a Leu replacement at the P2 position (kcat = 27.2 s-1, Km = 4.22 microM, kcat/Km = 6.43 microM-1.s-1), and that for XCP had an Ala replacement at the P3 position (kcat = 79.4 s-1, Km = 6.05 microM, kcat/Km = 13.1 microM-1. s-1). PCP and XCP preferred such charged amino acid residues as Glu, Asp, Arg, or Lys at the P2' position. This suggested that the S2' subsites of PCP and XCP are occupied by hydrophilic residues, similar to that of pepstatin-insensitive carboxyl proteinase from Bacillus coagulans J-4 [Shibata et al. (1998) J. Biochem. 124, 642-647]. In contrast, the S2' subsite of pepstatin-sensitive carboxyl proteinases (aspartic proteinases) is hydrophobic in nature. Thus, the hydophilic nature of the S2' subsite appears to be a distinguishing feature of pepstatin-insensitive carboxyl proteinases.

Abstract:Aim To report our experience regarding management of cholecystoenteric fistula (CEF) and identify the most effective diagnostic methods and surgical treatment. Methods In total, 10,588 patients underwent laparoscopic cholecystectomy for cholecystolithiasis from January 2000 to December 2014 at the Research Institute of General Surgery, Jinling Hospital (Nanjing, China). Twenty-nine patients were diagnosed with CEF preoperatively or intraoperatively. Data were retrospectively collected on demographics, preoperative diagnostics, intraoperative findings, laparoscopic procedures, complications, and follow-up. Results Twenty-nine patients (female/male ratio, 2.2; mean age, 68.7 years) with CEF were evaluated. Twenty-three (79.3%) patients had a cholecystoduodenal fistula (CDF), four (13.8%) had a cholecystocolonic fistula (CCF), one (3.4%) had a cholecystogastric fistula, and one (3.4%) had a CDF combined with a CCF. Only nine (31.0%) patients obtained a preoperative diagnosis. All patients initially underwent laparoscopic treatment, but five (17.2%) underwent conversion to open surgery; three of these five developed postoperative morbidity or mortality, and the other two had an uneventful postoperative course. Among patients managed successfully by laparoscopy, the hospital stay ranged from 3 to 6 days (mean, 4 days). All patients were asymptomatic at a mean follow-up of 13 months (range, 3-21 months). Conclusion Ultrasound and computed tomography can provide valuable diagnostic clues for CEF. Laparoscopic management of CEF in experienced hands is safe, feasible, and associated with rapid postoperative recovery.

Abstract:Enterocutaneous fistula is an abnormal communication between two epithelialized surfaces, one of which is a hollow viscous organ. It most commonly occurs as a surgical complication. Other causes are trauma, malignancy, inflammatory bowel disease, ischemia. The great majority of enterocutaneous fistula are iatrogenic (75-85%) and rest (15-25%) occur spontaneously. Enterocutaneous fistula is the one of the most challenging conditions managed by General surgeon. The mortality rates vary in different series for patients, with enterocutaneous fistula and remains 5% to 15%.Here we report a rare presentation of enterocutaneous fistula.
