Abstract:Purpose: The purpose of this study was to evaluate the remineralization potentials of different agents on demineralized enamel surfaces. Methods: Four hundred and sixty extracted human molars with artificial carious lesions were divided into six groups: (1) control; (2) acidulated phosphate fluoride (APF); (3) Curodont Repair (CR); (4) silver diamine fluoride (SDF); (5) ammonium hexafluorosilicate (SiF); and (6) SiF plus cetylpiridinium chloride (SiF+CPC). They were subdivided according to immersion periods (seven out of 30 days). After demineralization and remineralization procedures, microhardness test (VHN), energy-dispersive X-ray spectroscopy analysis, and confocal laser scanning microscopy evaluation were performed. The data were statistically analyzed. Results: By the 30-day remineralization, the mean differences in VHN values were ranked as follows, in order: (1) CR; (2) APF; (3) SiF; (4) SiF+CPC; (5) SDF; and (6) control (P<0.05). The calcium (Ca) content and calcium/phosphate ratio for all groups were significantly higher after 30 days (P<0.05). The greatest lesion depth changes were observed in the CR, APF, SiF, and SDF groups (P<0.05), while the greatest fluorescence changes were observed in the APF, SiF, and CR groups (P<0.05). Conclusions: The remineralization was most successful in the CR, APF, and SiF groups, with higher values than for those of the other treatments.

Abstract:4 hairdresser apprentices had eczematous dermatitis on both hands. The first symptoms were diffuse erythema and miliary sized vesicles with itching, which appeared a few months after they began work, followed by hyperkeratosis and swelling. Itchy eczematous lesions appeared on the back and legs which had never been exposed to hairdressing chemicals, although they disappeared shortly after returning to work. Patch testing of all agents used at their beauty parlors was performed. Cold permanent wave lotions, shampoos and rinse lotions were tested by the open patch test method, and the rest by the 48 h closed patch test method. In all 4 patients, a positive reaction was seen to the cold permanent wave lotion containing 5% ammonium thioglycolate, which did not disappear within 96 h (Table I). PPD was also positive. However, a cold permanent wave lotion containing cysteine gave a negative reaction; tests with the neutraliser solution and the other agents were negative. Open patch tests were performed using 5 % and 2 % ammonium thioglycolate. Positive reactions were seen to both solutions in all 4 patients, and the reactions persisted for more than 96 h as in the patch test using the permanent wave solution containing ammonium thioglycolate. The results of open patch tests with 5 % ammonium thioglycolate in 18 healthy subjects and 2 hairdressers without dermatitis were negative. Abstract:Objectives: to investigate whether baseline mineral distribution modulates the ability of silver diammine fluoride (SDF) to remineralize and stain enamel caries lesions. Methods: This laboratory study followed a 3 [treatment: SDF/fluoride varnish (FV)/deionized water (DIW)] ×3 [lesion protocol: methylcellulose (MeC)/hydroxyethylcellulose (HEC)/Carbopol 907 (C907)] factorial design. Lesions were created in bovine enamel specimens (n = 20). Treatments were applied and lesions remineralized in artificial saliva. Digital transverse microradiography (TMR-D) was used to analyze lesions. Lesion color was monitored spectrophotometrically. The effects of lesion protocol and treatment on changes in lesion depth (ΔLD), mineral loss (ΔΔZ), maximum mineral density at the surface zone (ΔSZmax), and color changes related to remineralization (ΔL*remin) were analyzed using two-way ANOVA. Results: The treatment×lesion protocol interaction was significant for ΔΔZ (p < 0.01) and ΔL*remin (p < 0.01), however not for ΔLD (p = 0.23) or ΔSZmax (p = 0.91). There were no differences in ΔΔZ between treatments in HEC and C907 lesions. However, DIW resulted in more remineralization than both SDF (p < 0.01) and FV (p = 0.01) in MeC lesions. Considering changes from lesion baseline after remineralization in MeC lesions, SDF treatment resulted in the highest mineral gain in the surface zone. However, DIW revealed the highest mineral gain after remineralization in the lesion body. SDF stained lesions with the intensity increasing after remineralization in C907 lesions, whereas staining decreased in MeC and HEC lesions. Conclusion: High fluoride treatments can interfere with continuous remineralization of caries lesions due to partial arrest. Baseline lesion mineral distribution affects SDF's ability to enhance remineralization and the staining caused by SDF. Clinical significance: SDF is being used to arrest active caries lesions extending into dentin and to treat dentin hypersensitivity. This study shed light on SDF's effect on an isolated process in dental caries only, remineralization. It achieved this by examining enamel caries lesions with differing mineral distributions and assessing their staining properties.

Abstract:Objectives: Evaluation of efficacy of bioactive glass (BAG), tricalcium phosphate, and ozone remineralizing agents on artificial carious lesion. Materials and Methods: Freshly extracted 48 human premolar teeth were included for this in vitro study. Remineralization of demineralized teeth was done with respective remineralizing agents (Group A – calcium sodium phosphosilicate [BAG], Group B – tricalcium phosphate [Clinpro Tooth Crème], Group C – ozone remineralizing agents, and control group: Group D – deionized water) three times a day for 12 days for 4 min. The degree of demineralization and remineralization was evaluated with Vickers hardness number. Results: There was a decrease in microhardness from baseline to demineralization in all the groups. The difference in microhardness values from demineralization to remineralization in all the three test groups was found to be statistically highly significant. Conclusion: BAG and Tricalcium phosphate had higher remineralizing capacity, which can be used clinically to treat early carious lesions.

Abstract:This study aimed to evaluate chitosan (CS)-based formulations loaded with 5% sodium fluoride (NaF) and/or 10% nanohydroxyapatite (nHA) to remineralize the demineralized primary tooth enamel surface. Ninety enamel blocks were demineralized and were divided into six groups (n = 15): (1) CS-based hydrogel, (2) CS-based hydrogel loaded with NaF, (3) CS-based hydrogel loaded with nHA, (4) CS-based hydrogel loaded with NaF and nHA, (5) 5% NaF varnish, and (6) negative control with no intervention. After intervention, the specimens were pH cycled by 2 h immersion in demineralizing solution and 22 h immersion in remineralizing solution for 8 days. The remineralization effects were evaluated by Vickers microhardness measurements and field emission scanning electron microscopy coupled with energy-dispersive X-ray spectrometry (FESEM-EDS). The best mean ± SD percentage microhardness recovery in remineralized enamel (%REMH) was found in group 4 (56.90 ± 5.49). The %REMH of groups 2 (30.74 ± 3.51) and 5 (29.23 ± 5.65) were statistically the same (p = 0.943). FESEM images confirmed partial coverage of the porous demineralized enamel with a newly formed mineralized layer. Based on EDS findings, the Ca/P ratio values of the treated enamel surfaces with CS-based hydrogels ranged between 1.71 and 1.87, and the highest F content was noticed in group 2 (1.02 ± 0.03). Although, all tested CS-based hydrogels demonstrated the potential to repair demineralized enamel, nHA- and NaF-containing CS-based hydrogel showed the highest remineralization effect. We infer that this new hybrid hydrogel is a potentially useful dental material for tooth biomineralization.

Abstract:OBJECTIVES: To compare remineralization potential of topical fluoride varnishes with added calcium phosphate-based delivery systems on artificial enamel caries.METHODS: Human enamel specimens (n = 60) were randomly divided into six groups: Group 0: Control (no varnish), Group 1: Duraphat® , Group 2: MI Varnish™ , Group 3: Embrace™ Varnish, Group 4: Enamel Pro® Varnish, and Group 5: Clinpro™ White Varnish. Specimens were immersed in demineralizing solution for 96 h to create artificial caries lesion. Each specimen was then immersed in artificial saliva for 6 h after fluoride varnish application and pH cycling was performed for 8 days. Specimens were evaluated using Knoop surface microhardness (KHN) and transverse microradiography (TMR). One-way anova test was performed to examine the effect of fluoride varnishes on change in percentage of surface hardness recovery (%SHR), change in lesion depth (ΔLD), and change in mineral loss (ΔΔZ) with the significance limit set at 5%.RESULTS: %SHR of enamel following treatment with Group 1 was significantly higher than Group 3 and Group 4. Conversely, ΔLD and ΔΔZ of Group 1 were significantly higher than Group 2, Group 3, and Group 4. No significant differences in %SHR, ΔLD, and ΔΔZ were found between Group 1 and Group 5.CONCLUSIONS: Duraphat® alone achieves significant remineralization of enamel carious lesions. Clinpro™ White Varnish had similar enamel remineralization effect as Duraphat® .

The Effect of Formulated Dentin Remineralizing Gel Containing Hydroxyapatite, Fluoride, and Bioactive Glass on Dentin Microhardness: An In Vitro Study

Abstract:Objectives: This study aimed to develop a gel with dentin-remineralizing properties, integrating nano-hydroxyapatite (nHA), sodium fluoride (NaF), and bioactive glass (BG). Materials and methods: The enamel layer of 40 bovine incisors was removed. The samples were allocated into four groups of 10 each, based on varying concentrations of nHA, BG, and NaF in the gel compositions (wt%): (1) 2.5%-7.5%-0.05%, (2) 5%-5%-0.05%, (3) 7.5%-2.5%-0.05%, and (4) a control group with a base gel lacking remineralizing agents. After 8 hr of demineralization, the dentin surface microhardness was measured at depths of 30, 60, and 140 µm. After a 20-day pH cycling, the percentage of surface microhardness recovery (SMHR%) was measured and compared among the groups using the ANOVA and Tukey HSD post hoc tests (α = 0.05). Scanning electron microscopy analysis evaluated each specimen's superficial morphology. Results: At all depths, the SMHR% of the Group 2 and Group 3 was significantly higher than the control group (p < 0.05). The SMHR% Group 1 (67.39% ± 29.34%) was significantly higher than the control group (-21.24% ± 51.72%) only at the depth of 30 μm (p = 0.047). Group 3 had higher SMHR% than Group 2 at all depths; however, the difference was not statistically significant. Moreover, the SMHR% of Group 3 was significantly higher than that of Group 1 at depths of 30 μm (187. 94% ± 68.95% vs. 67.39% ± 29.34%; p = 0.005) and 60 μm (179.55% ± 75.96% vs. 64.34% ± 41.96%; p = 0.043). Surface deposition and tubule occlusion were observed in the Groups 2 and 3 samples, which was more prominent in the latter. Conclusions: Combining 7.5% nHA, 2.5% BG, and 0.05% NaF could potentially remineralize primary carious lesions.

Abstract:Aim: To investigate the cariostatic and remineralizing effect of two commercial silver diamine fluoride (SDF) preparations on enamel and dentinal caries using a bacterial plaque model.Materials and methods: A total of 32 extracted primary molars were divided into two groups (n = 16), group I (FAgamin), and group II (SDF). Plaque bacterial model was used to induce caries on enamel and dentin. Preoperative evaluation of samples was done using confocal laser microscopy (CLSM) and energy-dispersive X-ray spectroscopy-scanning electron microscope (EDX-SEM). All samples were treated with test materials and evaluated for postoperative remineralization quantification.Results: Energy-dispersive X-ray spectroscopy (EDX) revealed that mean preoperative levels (in weight %) of silver (Ag) and fluoride (F2) in carious enamel lesions were 0.0 and 0.0, which increased postoperatively to 11.40 and 31.05 for FAgamin and 13.61 and 31.87 for SDF, respectively. For dentinal caries, EDX revealed mean preoperative levels (in weight %) of Ag and F2 were 0.0 and 0.0, which increased to 11.47 and 48.71 for FAgamin and 10.16 and 47.82 for SDF, respectively postoperatively. Both the groups showed evident demineralization with exposed collagen under SEM. The mean value of enamel lesion depth for the group I and II were 38.64 and 39.30 µm, that reduced to 28.02 and 28.70 µm while for dentinal caries, the mean depth from 38.05 and 38.29 µm that reduced significantly to 28.96 and 30.10 µm, respectively (p < 0.001). Caries depth declined significantly after the application of both FAgamin and SDF (p < 0.001).Conclusion: FAgamin and SDF show similar cariostatic and remineralization potential for dental caries. The bacterial plaque model used in this study is an efficient method to induce artificial carious lesions in teeth.Clinical significance: A comparative evaluation of these two cariostatic and remineralizing agents will aid in identifying the efficacy of both commercial products in treating initial caries lesions in an effective noninvasive and child-friendly manner.How to cite this article: Kale YJ, Misal S, Dadpe MV, et al. Combined Effect of a Bioinspired Self-Assembling Peptide and Fluoride Varnish on Remineralisation of Artificial Early Enamel Caries Lesion: An in vitro Study

Abstract:Introduction: This study aimed to investigate the remineralisation effect of combined use of a bioinspired self-assembling peptide (P26) and fluoride varnish on artificial early enamel caries lesions. Methods: Bovine enamel blocks with artificial early enamel caries lesions were prepared. The blocks were randomly allocated to four experimental groups to receive the following treatments: A = P26 + fluoride varnish, B = P26, C = fluoride varnish, and D. distilled water (negative control). The treated blocks were subjected to pH cycling. Enamel blocks were collected at time points of 7 days (d7) and 21 days (d21). The mineral gain, elemental analysis and crystal characteristics of the caries lesion were assessed by micro-computed tomography, scanning electron microscopy with energy dispersive X-ray and X-ray diffraction (XRD), respectively. Results: The mean ± standard deviation of mineral gain of group A to D were 17.4 ± 4.2%, 10.7 ± 2.2%, 10.1 ± 1.2%, and 6.8 ± 0.5% at d7, respectively, and 15.2 ± 2.6%, 8.7 ± 3.1%, 9.7 ± 1.2%, and 7.8 ± 2.3% at d21, respectively. A significant higher mineral gain was observed in group A when compared to other groups at both d7 and d21 ( p < 0.05). The calcium-to-phosphate ratio remained consistent across all groups, ranging between 1.2 and 1.4. XRD analysis indicated that crystal composition on the surfaces was apatite for all groups. Conclusion: In conclusion, the present study provided a first indication of better remineralisation effects of the combined use of the bioinspired self-assembling peptide P26 and fluoride varnish compared to the effects of the respective individual uses of P26 or fluoride varnish.

Abstract:BACKGROUND A variety of natural and synthetic agents have recently been used in clinical trials to arrest dentin caries. OBJECTIVES The aim of the present study was to explore the remineralization and antibacterial effect of natural (propolis, hesperidin) vs. synthetic (silver diamine fluoride (SDF)) agents on deep carious dentin. MATERIAL AND METHODS A total of 64 human molar teeth with Class I caries were randomly distributed into 4 groups: control group; propolis group; hesperidin group; and SDF group. The cavities were prepared using the stepwise caries removal technique, and then covered with the materials to be tested. The samples were taken from the carious lesions before and after treatment to evaluate the antibacterial effect. Then, the teeth were restored with a glass ionomer cement (GIC). Digital X-rays were taken to assess remineralization and the antibacterial effect after 6 and 12 weeks. RESULTS The highest value of radiodensity was observed in the propolis group (46.44 ±9.65 HU), while the lowest value was noted in the hesperidin group (12.62 ±5.86 HU). The bacterial count in the propolis group was 1,280.00 ±1,480.54 CFU/mL at baseline, which was not significantly higher than the value measured after 6 weeks (574.00 ±642.48 CFU/mL; p = 0.153), whereas in the hesperidin group, the mean value of the bacterial count at baseline (3,166.67 ±1,940.79) was not much higher as compared to the value obtained at 6 weeks (2,983.33 ±1,705.77) (p = 0.150). CONCLUSIONS In comparison with SDF, propolis and hesperidin agents showed promising effects in terms of remineralization of carious dental tissue and hindering the progression of caries.

Abstract:ABSTRACT Background: White lesions, also known as white spots, are the earliest signs of tooth decay. At this stage, noninvasive preventive treatments, such as fluoride application, can help to slow down or even reverse the progression of white lesions. For decades, fluoride has held the spotlight as the go-to agent in oral preventive care, demonstrating significant remineralizing power. Recent scientific literature reveals a growing interest in alternative products as natural substances that demonstrate potential remineralizing effects on tooth enamel. This in vitro study aimed to evaluate the potential remineralizing effect of calcium sodium phosphosilicate (CSP), ginger, and turmeric. Materials and Methods: This in vitro study was designed to evaluate the remineralizing effects of various materials on the enamel of extracted human teeth. The aim was to compare the effectiveness of these different substances in promoting enamel remineralization. Enamel blocks were prepared and were divided into seven experimental groups: CSP, fluoride toothpaste, nonfluoride toothpaste, turmeric, ginger, fluoride varnish, and distilled water. In this study, sample mineralization is assessed through the quantitative photo-induced fluorescence test before demineralization, after demineralization, and after remineralization. Statistical analysis of the data was conducted using a one-factor analysis of variance test. The significance level was set at P < 0.05. Results: The best results were obtained by groups treated with turmeric ( P < 0.0001), Enamelast fluoride varnish ( P < 0.0001), and NovaMin ( P < 0.01), as compared to other remineralizing treatments. Conclusion: While fluoride has undeniably revolutionized oral preventive care, the exploration of alternative products as natural substances and innovative compounds in recent scientific literature signals a shift in focus toward safer and potentially more diverse options.
A Scanning Electron Microscope Evaluation of the Efficacy of Different Fluoride-releasing Dental Restorative Materials to Prevent Enamel Demineralization: An In Vitro Study

Abstract:Aim: Aim of the present research was to investigate the effectiveness of various fluoride-releasing dental restorative agents in preventing demineralization of enamel. Materials and methods: Eighty human mandibular permanent molar teeth constituted the study group. All samples were subjected to storage in thymol, after which they were taken out to prepare alike proximal box in each. Inductions of artificial enamel surface lesions were done by placing the teeth in demineralizing solution for 96 hours. Subsequently, all 80 molars were randomly assigned to any of the four groups (i.e., 20 in every individual group) according to the restoration as group A: giomer (composite resin containing surface pre-reacted glass-ionomer fillers), group B: compomer (polyacid-modified composite resin), group C: resin-modified glass-ionomer cement (RMGIC), group D: fluoride-releasing composite. After this, the pH cycling was performed, and the samples were subjected to examination beneath scanning electron microscope (SEM). Results: Higher mean areas of remineralization were noted when RMGIC (96.34 ± 0.06) was used followed by the compomer (109.52 ± 0.17), giomer (118.39 ± 0.82), and the fluoride-releasing composite group (129.27 ± 0.31) in that order. A statistically significant difference was seen amid the investigational groups that utilized different restorative agents (p <0.001). A pairwise evaluation that was performed revealed that except for the giomer group and the compomer group, a statistically significant difference (p <0.001) was found among the experimental groups. Conclusion: This research infers that the RMGIC-treated samples exhibited significantly superior performance in preventing enamel demineralization in comparison to compomer, giomer as well as fluoride-releasing composites. Clinical significance: One among the highly frequently employed anticariogenic materials is fluorides. Owing to this characteristic, they are integrated into numerous restorative substances. Nevertheless, the quantity and speed of fluoride release differ in different agents, which translates to the efficacy of the restorative agent in avoiding demineralization about the restoration.

Abstract:Dental remineralization represents the process of depositing calcium and phosphate ions into crystal voids in demineralized enamel, producing net mineral gain and preventing early enamel lesions progression. The aim of the present study was to qualitatively and quantitatively compare the remineralizing effectiveness of four commercially available agents on enamel artificial lesions using Scanning Electron Microscopy (SEM) combined with Energy Dispersive Spectroscopy (EDS) techniques. Thirty-six extracted third molars were collected and randomly assigned to six groups (n = 6), five of which were suspended in demineralizing solution for 72 h to create enamel artificial lesions, and one serving as control: G1, treated with a mousse of casein phosphopeptide and amorphous calcium–phosphate (CPP-ACP); G2, treated with a gel containing nano-hydroxyapatite; G3, treated with a 5% SF varnish; G4, treated with a toothpaste containing ACP functionalized with fluoride and carbonate-coated with citrate; G5, not-treated artificial enamel lesions; G6, not demineralized and not treated sound enamel. G1–G4 were subjected to pH cycling over a period of seven days. Analyses of the specimens' enamel surfaces morphology were performed by SEM and EDS. Data were statistically analyzed for multiple group comparison by one-way ANOVA/Tukey's test (p < 0.05). The results show that the Ca/P ratio of the G5 (2.00 ± 0.07) was statistically different (p 0.05) between G1, G2 and G6 and between G4 (2.01 ± 0.06) and G5. We concluded that G1 and G2 showed better surface remineralization than G3 and G4, after 7 days of treatment.

Assessment of Remineralization Ability of Different Fluoride Varnishes on Artificial Enamel Lesion of Primary Teeth - A Comparative Study

Abstract:Aim: The current study's aim was to evaluate the potential of various fluoride varnishes to remineralize artificial enamel lesions on primary teeth. Materials and methods: For the study, 90 deciduous molar teeth that were implicated for extraction were taken. A window region with a size of 3 × 3 mm was located in the middle of the tooth's coronal surface. The remaining portion was given a coat of nail polish and given time to dry. A demineralizing solution was used to create the artificial enamel lesion. The samples were randomized into three experimental groups at random, with 30 samples in each group. Group 1: Application of Clinpro White Varnish, Group 2: Application of Duraphat Varnish, and Group 3: Application of MI Varnish. After 14 days, samples from each group were examined under scanning electron microscopy at a magnification of 1000× to determine whether remineralization had occurred. Microhardness was also determined using a microhardness tester. Result: Before the intervention, the mean demineralized enamel lesion in the Clinpro White Varnish group was 134.44 ± 0.04, in the Duraphat Varnish group was 133.76 ± 0.12, and in the MI Varnish group was 133.89 ± 0.08. After intervention, the maximum remineralized area was found in the MI Varnish group (82.74 ± 0.18) followed by the Clinpro White Varnish group (101.43 ± 0.16) and the Duraphat Varnish group (104.11 ± 0.10). After the intervention, there was a statistically significant difference found between the groups. After the intervention, the maximum microhardness mean value improved in the MI Varnish group (257.03 ± 1.09 to 261.18 ± 1.20) followed by Clinpro White Varnish (258.78 ± 1.64 to 260.10 ± 1.22) and Duraphat Varnish group (255.24 ± 1.51 to 258.02 ± 0.89). Conclusion: According to the findings of the current investigation, all of the varnishes utilized in this in vitro experiment are capable of correcting early enamel defects on primary teeth. However, MI Varnish group was very effective compared to the Clinpro White Varnish group and the Duraphat Varnish group.
Evaluation of Surface Properties of Two Remineralizing Agents after Modification by Chitosan Nano Particles: An In vitro Study

Abstract:Background: Calcium phosphate-based systems have been introduced as promising bio-mimetic materials due to their close resemblance to the enamel. Chitosan and its derivatives have been an emerging biomaterial due to their additional antibacterial effect and promising re-mineralizing ability. Aim: The aim of this study is to evaluate the effect of chitosan nanoparticles on the remineralization of the demineralized enamel surface after being added to nano-hydroxyapatite and nano-calcium phosphate materials. Setting and design: This was in vitro study. Materials and methods: Twenty specimens of extracted permanent molars were collected, and then immersed in demineralizing solution, then distributed into four groups according to the remineralizing material. Group 1: Treated with Nano-beta-tricalcium phosphate (Nβ-TCP) gel, Group 2: Treated with Nβ-TCP with chitosan gel, Group 3: Treated with Nanohydroxyapatite (NHA) gel, and Group 4: Treated with NHA with chitosan gel. The surface hardness of the teeth was measured at baseline, after demineralization, and after remineralization. The structural changes in each group were analyzed using the scanning electron microscopy. Statistical analysis: Shapiro-Wilk's test, one-way ANOVA followed by Tukey's post hoc test was used. Results: In all groups, there was a significant difference in mean Vickers hardness number (VHN) at different intervals, with the highest value found after treatment (301.64-395.65) VHN, followed by the baseline (236.97-276.15) VHN, while the lowest value was detected after demineralization (121.23-124.39) VHN. It was also indicated that baseline treatment, the Hardness percentage change (%) of the nano NHA + Chitosan group showed the highest significant value (55.10%), while the Nβ-TCP group exhibited the lowest significant value (9.56%). Conclusions: It can be concluded that NHA and NΒ-TCP modified by chitosan NPs as remineralizing agents of enamel surface hold promising results.

Abstract:Aim: To assess the effects of SDF and SDF+KI treatment on enamel and dentin carious lesions in primary teeth using x-ray Microtomography (XMT) and back scattered scanning electron microscopy (BSE-SEM). Methods: Artificial enamel caries of 3 caries free primary teeth were created by immersion of the samples in 50 ml demineralization solution for 72 h. Three other teeth with natural dentin caries were selected. Both groups were divided into 3 subgroups: EC-Enamel Control; ES-Enamel with SDF application; ESK-Enamel with SDF followed by KI application; DC-Dentin Control; DS-Dentin with SDF application; DSK-Dentin with SDF followed by KI application. Each tooth was imaged using XMT at 3 time points: (1) Pretreatment; (2) after immersion in remineralization solution for 120 h, with or without SDF or SDF+KI; (3) after subsequent immersion in demineralization solution for 72 h. The change of radiopacities of the lesions in these time points were assessed from the XMT images. After the XMT scans, all teeth were investigated microscopically using BSE-SEM. Results: In EC, no change in linear attenuation coefficient (LAC) was observed after remineralization, but LAC reduction was observed after subsequent demineralization. For ES, thin layer of high LAC material was deposited on the enamel surface after remineralization, and further reduction of LAC was observed after demineralization. In ESK, the surface layer was lost after SDF+KI, and small reduction of LAC was observed after demineralization. In DC, no LAC change was observed after remineralization, but reduction of LAC was detected after demineralization. In DS, high LAC material was formed on the carious dentin surface and randomly inside the lesion. No further LAC change was found after demineralization. In DSK, thick layer of high LAC material was deposited on the carious surface and inside the dentinal tubules. No further LAC reduction was found after subsequent demineralization. Conclusion: SDF and SDF+KI did not protect artificial enamel under acid attack even though Ag products were deposited in the porous enamel. However, SDF and SDF+KI shows protective properties against acid challenges and Ag products are deposited in carious dentin lesion without tubular structure randomly; and within dentinal tubules when these structures are retained.

Materials

Abstract:Silver diamine fluoride (SDF) is a strong fluoride agent for caries control, remineralization, and reducing the incidence of dental caries. This study used 38% SDF with potassium iodide (KI) on enamel remineralization and compared it with the currently used 38% SDF and 5% sodium fluoride (NaF) varnish to treat artificial caries. Bovine incisors were demineralized for 120 h and randomly distributed for treatment by the remineralization agents for 120 h as follows: control (n = 15), SDF/KI (n = 15), SDF (n = 15), and NaF (n = 15). Colorimetric analysis was performed using the ΔE value. The Vickers microhardness (VHN) was determined to evaluate the enamel remineralization of the specimens. Polarized light microscopy (PLM) and scanning electron microscopy (SEM) were performed to confirm the surfaces precisely and histologically. SDF/KI caused no significant difference in discoloration between the control and NaF groups. There was also no significant difference in the VHN after remineralization, but SDF/KI exhibited the highest mean microhardness. PLM revealed that SDF/KI had the greatest remineralization ability. In conclusion, SDF/KI is effective for dental enamel remineralization even with KI, which reduces discoloration.
