Abstract:Objective: To investigate the effects of sociodemographic and working condition variables, as well as the coping strategies used by nurses, on their occupational stress during the COVID-19 pandemic. Methods: A cross-sectional study was conducted with 104 nurses who worked in intensive and emergency care at a public hospital in the state of São Paulo, Brazil. Data collection was performed in person and online using a questionnaire assessing sociodemographic and occupational characteristics, the Nursing Stress Inventory, and the Occupational Coping Scale. Results: The participants had a high level of stress (median = 132), especially in the 'interpersonal relations' domain (median = 63), and made little use of occupational coping strategies (median = 87). Income (p = 0.027), work shift (p = 0.028), being on leave from work (p = 0.020), number of hospitals with employment ties (p = 0.001), and relationship with management were independently associated with the levels of stress among the nurses. Conclusion: In the present study, the high levels of stress among nurses were influenced by financial and work-related factors as well as interpersonal relationships. No significant association was found between stress among the nurses and the use of occupational coping strategies.

Abstract:ABSTRACT Objective: To identify the presence of burnout syndrome among nursing professionals in the emergency room and intensive care unit for adults of the University Hospital of Maringá. Methods: This is an exploratory and descriptive research study with a quantitative approach. It was developed by applying a questionnaire containing 22 questions from the Maslach Burnout Inventory instrument, which identifies the symptomatology dimensions of the burnout syndrome. Data analysis of the Maslach Burnout Inventory instrument was performed by adding up each dimension (Emotional Exhaustion, Depersonalization and Professional Fulfillment) of each questionnaire separately, according to the nursing professional's answers to each question. The values obtained were compared to the reference values of the Nucleus for Advanced Studies on Burnout Syndrome. Results: It was found that 31.36% of the nursing professionals at the University Hospital of Maringá emergency room had high Emotional Exhaustion, 30.92% had low Professional Fulfillment, and 39.25% had high Depersonalization. Regarding the nursing professionals in the Intensive Care Unit for Adults, 36.36% had high Emotional Exhaustion, 36.36% had low Professional Fulfillment, and 22.73% had high Depersonalization. Conclusion: The findings suggest that the Intensive Care Unit for Adults in the morning shift is the highest stressor and with a greater probability of the professionals developing burnout syndrome.

Abstract:Considering the importance and lack of data of toxicogenomic approaches on occupational exposure to anaesthetics, we evaluated possible associations between waste anaesthetic gases (WAGs) exposure and biological effects including oxidative stress, DNA damage, inflammation, and transcriptional modulation. The exposed group was constituted by anaesthesia providers who were mainly exposed to the anaesthetics sevoflurane and isoflurane (10 parts per million ‐ ppm) and to a lesser degree to nitrous oxide (150 ppm), and the control group was constituted by physicians who had no exposure to WAGs. The oxidative stress markers included oxidised DNA bases (comet assay), malondialdehyde (high‐performance liquid chromatography ‐ HPLC), nitric oxide metabolites (ozone‐chemiluminescence), and antioxidative markers, including individual antioxidants (HPLC) and antioxidant defence marker (ferric reducing antioxidant power by spectrophotometry). The inflammatory markers included high‐sensitivity C‐reactive protein (chemiluminescent immunoassay) and the proinflammatory interleukins IL‐6, IL‐8 and IL‐17A (flow cytometry). Telomere length and gene expression related to DNA repair (hOGG1 and XRCC1), antioxidant defence (NRF2) and inflammation (IL6, IL8 and IL17A) were evaluated by real‐time quantitative polymerase chain reaction. No significant differences (P > 0.0025) between the groups were observed for any parameter evaluated. Thus, under the conditions of the study, the findings suggest that occupational exposure to WAGs is not associated with oxidative stress or inflammation when evaluated in serum/plasma, with DNA damage evaluated in lymphocytes and leucocytes or with molecular modulation assessed in peripheral blood cells in university anaesthesia providers. However, it is prudent to reduce WAGs exposure and to increase biomonitoring of all occupationally exposed professionals.This article is protected by copyright. All rights reserved.

Relationship between depressive symptoms, burnout, job satisfaction and patient safety culture among workers at a university hospital in the Brazilian Amazon region: cross-sectional study with structural equation modeling

Abstract:BACKGROUND: Workplaces can be sources of mental distress. In healthcare services, this can also affect patients.OBJECTIVE: To assess the prevalence of and factors associated with depressive symptoms, burnout, job satisfaction and patient safety culture and the relationships between these constructs, among healthcare workers.DESIGN AND SETTING: Cross-sectional study in a university hospital in Manaus, Brazil.METHODS: Randomly selected workers were interviewed based on Brazilian-validated tools. We calculated the prevalence ratio (PR) and 95% confidence interval (CI) of depressive symptoms and burnout using Poisson regression with robust variance; and the β-coefficient of safety culture and job satisfaction using linear regression. Outcome relationships were assessed using partial least-squares structural equation modeling.RESULTS: 300 professionals were included; 67.3% were women. The prevalence of depressive symptom was 19.0% (95% CI: 14.5; 23.5%) and burnout, 8.7% (95% CI: 5.2; 12.3%). Lack of work stability increased depression (PR = 1.88; 95% CI: 1.17; 3.01) and burnout (PR = 2.17; 95% CI: 1.03; 4.57); and reduced job satisfaction (β = -11.93; 95% CI: -18.79; -5.07). Depressive symptoms and burnout were positively correlated, as also were job satisfaction and safety culture (P < 0.001); job satisfaction was negatively correlated with burnout (P < 0.001) and depression (P = 0.035).CONCLUSION: Impermanent employment contracts increased depression and burnout and reduced job satisfaction. Job satisfaction reduced poor mental health outcomes and increased safety culture. Job satisfaction and safety culture were directly proportional (one construct increased the other and vice versa), as also were depression and burnout. Better working conditions can provide a virtuous cycle of patient safety and occupational health.

Abstract:Aims: This study aimed to (1) assess the relationship between intensive care unit (ICU) nurses' burnout, organizational commitment and turnover intention, (2) examine the moderating effect of the organizational commitment on the relationship between ICU nurses' burnout and turnover intention, and (3) explore the prevalence and influencing factors of hospital workplace violence among ICU nurses. Design: Cross-sectional study design. Methods: Data were collected from August to October 2020 and a convenience sample of registered nurses was recruited. To control common method biases, one-month temporal separation, Harman's single-factor analysis and method of latent variables were adopted. The moderating effect was tested by SPSS Hayes PROCESS Macro. Chi-square and logistic regression were used to examine workplace violence data. Results: Organizational commitment (β = -.23, 95% confidence interval -.45 to -.03) and continuance commitment (β = -.15, 95% confidence interval -.24 to -.16) have negative moderation effects on the relationship between emotional exhaustion and turnover intention. 77.7% of ICU nurses experienced workplace violence, male and staff nurses, and nurses with lower professional titles and shorter working years have greater odds of experiencing this violence.
Prevalence and risk factors of Burnout syndrome among intensive care unit members during the second wave of COVID-19: a single-center study

Abstract:Objective: To evaluate the prevalence of burnout among the intensive care unit team of a university hospital after the second wave of COVID-19 and identify the key factors associated with its development. Methods: This single-center study included 395 employees from a multidisciplinary team. The participants completed a questionnaire based on the Maslach Burnout Inventory. Multivariate analysis was used to identify the factors associated with burnout. Results: Of 395 participants, 220 responded to the questionnaire (response rate: 56%). The prevalence of Burnout syndrome, defined as a severe score in at least one dimension, was 64.5% (142/220). Emotional distress was the most prevalent dimension, with a severe score affecting 50.5% (111/220) of the participants, followed by depersonalization at 39.1% (86/220). Only 5.9% (13/220) had severe scores in all three dimensions. Multivariate analysis revealed that being a physician was significantly associated with severe burnout symptoms in at least one dimension (odds ratio (OR), 1.32; 95% confidence interval (95%CI): 1.57-9.05; p=0.003). Additionally, having two or more jobs was associated with burnout in the three dimensions (OR=1.65; 95%CI=1.39-19.59; p=0.01). Conclusion: This study highlights the alarming prevalence of burnout among intensive care unit teams, particularly among physicians, following the second wave of COVID-19. This emphasizes the need for targeted interventions and support systems to mitigate burnout and reduce its negative impact on healthcare professionals' well-being and patient care.

Abstract:Burnout is most commonly defined as a syndrome characterized by emotional exhaustion, cynicism, and ineffectiveness, which occurs in response to chronic stressors at work. It can adversely affect health workers' physical and mental health, and the quality of care provided. The COVID-19 pandemic increased stressors and could impact burnout prevalence in this group. There is a lack of information regarding the prevalence of burnout among hospital health workers in Brazil. A newer definition of burnout has been proposed that considers three different clinical profiles: the frenetic, underchallenged and worn-out subtypes. This differentiation could lead to interventions tailored for each subtype. The present study aimed to estimate the prevalence of burnout, its subtypes, and associated factors in workers of a public hospital network in Brazil, during the pandemic. A total of 143 randomly selected participants answered an online form that included sociodemographic and occupational items, and the Burnout Clinical Subtypes Questionnaire, a summarized version. This questionnaire evaluates three burnout dimensions (overload, lack of development, neglect) that can be used to discriminate the three burnout subtypes (frenetic, underchallenged, worn-out, respectively); higher scores indicate higher burnout levels. The prevalence of burnout was high (53.85%), similar to other studies during the pandemic. The most common subtypes were 'frenetic' (34.97%), characterized by increased efforts to meet work demands, to the point of neglecting personal needs, and 'lack of development' (23.78%), characterized by a sense that work is uninteresting and does not contribute to personal development, and a perfunctory behavior towards tasks. Age was associated with burnout: workers with less than 51 years presented higher levels of burnout. These findings indicate the need for effective interventions to prevent and/or treat burnout. The assessment of burnout subtypes can allow managers to better understand the processes affecting employees, and inform actions to improve workforce health.

[Association of empathy and occupational stress with burnout among primary health care professionals].

Abstract:There are indications that burnout syndrome affects primary health care (PHC) professionals. The scope of this study was to investigate the association of empathy and occupational stress with the burnout of PHC professionals. A total of 348 workers filled out an an online assessment including occupational stress, empathy, and burnout scales. The empathetic-concern and perspective-taking empathy dimensions were negatively correlated with burnout (p<.01), while personal anguish was positively related to it (p<.01). The reward and effort occupational stress dimensions revealed greater weight in explaining the job disillusion and psychic exhaustion burnout subscales (p<.001), respectively, while empathetic concern and personal anguish revealed the greatest weight for the explanation of indolence and guilt (p<.001). Reward was the significant predictor of Profile 1 (p=.008), the less severe form of the syndrome, while personal anguish (p=.028) and effort (p=.012) revealed the same weight in the model for Profile 2, namely the most severe level of burnout. The results suggest interventions that focus on both work stressors and the empathy of the professionals involved.

Abstract:Introduction: Burnout syndrome results from a response to chronic work stress and is responsible for causing symptoms related to three dimensions: emotional exhaustion, reflecting work overload; depersonalization, characterized by professional detachment and cynicism; and reduced professional accomplishment, related to low productivity at work. Burnout is often associated with jobs that require professionals to have direct contact with users, such as health professionals. Primary Health Care is the assistance level that greatest contact with the community and requires teamwork, thus exposing workers to potential psychosocial stressors. Objectives: To identify the prevalence of symptoms of burnout syndrome among Primary Health Care professionals in Toledo, state of Paraná, Brazil. Methods: This was a descriptive, quantitative, cross-sectional study. A sociodemographic questionnaire and the Maslach Burnout Inventory, Human Services Survey, were used to assess the outcomes. Results: The prevalence of high risk for the development of burnout syndrome was 10.6%, and, when dimensions were evaluated separately, it was found that 29.8, 52.1 and 22.3% of participants showed a high level of symptoms in the dimensions emotional exhaustion, reduced professional accomplishment, and depersonalization, respectively. Previous use of psychiatric medication due to another condition had a significant correlation with high risk for burnout. Conclusions: The results of this research corroborated other similar studies, contributing to knowledge on the subject in a region of the state of Paraná where there was still no research on the syndrome.
[Assessment of occupational stress among workers in rehabilitation medicine]

Abstract:Emerging evidence confirms the risk of distress in staff involved in the Helping Professions resulting in the possibility of reduced efficacy and efficiency of the structure which offers health services. The aim of the present study was to reveal the presence of burnout and its relationships with environmental and organizational factors of health care professionals enrolled in medical rehabilitation. 88 health care professionals underwent a psychometric assessment by filling the Organizational Checkup System Questionnaire (OCS) for measuring burnout and associated organizational factors. Data analysis demonstrated the presence of major emotional distress (p = 0.05) in the group who had an executive role (social-assistants and assistant nurses), especially regarding organization factors linked with: Overwork (p = 0.009), lack of Leadership (p = 0.01), lack of Comprehension of modifications offered by the structure (p = 0.0001) and Professional Development (p = 0.016). OCS revealed to be a sensitive psychometric instrument, especially for the identification of organizational weaknesses, which can stimulate the application of strategies to prevent the burnout syndrome and to improve coping skills.

Abstract:Introduction: Precariousness of medical work, with loss of autonomy and devaluation, in addition to unstable and non-guaranteed employment bonds, has caused health problems in professionals, hampering the assistance provided. Research shows a high prevalence of stress, burnout, depression, and suicide among physicians. This study investigated aspects of mental health in physicians from the state of Paraná, Brazil. Objectives: We aimed to evaluate indicators of stress, burnout, and work engagement measured by inventories specifically designed for each one: Stress Symptoms Inventory, Burnout Syndrome Inventory, Utrecht Work Engagement Scale, respectively. Methods: The professionals answered the questionnaires remotely, after accepting the invitation for the study and signing the consent form. Results: A total of 1,201 physicians answered the questionnaires, with mean age of 37 years; 53.9% of participants were women; 63.5% graduated in Paraná. Of the total number of participants, 97.5% and 93.4% presented psychological and physical symptoms of stress, respectively. According to the Inventário da Síndrome de Burnout, the prevalence of diagnosis of burnout was estimated at 59.4%. As for work engagement, 40% of participants showed very high levels in the overall score of the construct. Conclusions: Most physicians showed signs of stress; burnout rates were high; negative organizational conditions prevailed in the work environment; work engagement was frequent.

Abstract:Background: Intensive care unit (ICU) nurses are highly prone to occupational stress and burnout, affecting their physical and mental health. The occurrence of the pandemic and related events increased nurses' workload and further exacerbated their stress and burnout. This work investigates occupational stress and burnout experienced by ICU nurses working with COVID and non-COVID patients. Method: A prospective longitudinal mixed-methods study was conducted with a cohort of ICU nurses working in medical ICU (COVID unit; n = 14) and cardiovascular ICU (non-COVID unit; n = 5). Each participant was followed for six 12-h shifts. Data on occupational stress and burnout prevalence were collected using validated questionnaires. Physiological indices of stress were collected using wrist-worn wearable technologies. Participants elaborated on the causes of stress experienced each shift by completing open-ended questions. Data were analyzed using statistical and qualitative methods. Results: Participants caring for COVID patients at the COVID unit were 3.71 times more likely to experience stress ( p < 0.001) in comparison to non-COVID unit participants. No differences in stress levels were found when the same participants worked with COVID and non-COVID patients at different shifts ( p = 0.58) at the COVID unit. The cohorts expressed similar contributors to stress, based in communication tasks, patient acuity, clinical procedures, admission processes, proning, labs, and assisting coworkers. Conclusion: Nurses in COVID units, irrespective of whether they care for a COVID patient, experience occupational stress and burnout.

Abstract:Objective: to analyze the risk of exposure to occupational stress among primary healthcare professionals during the COVID-19 pandemic and their perception regarding their experience. Method: mixed-methods sequential explanatory study with 50 primary care professionals. Sociodemographic, clinical, and labor questionnaires, Job Stress Scale, and semi-structured interviews were used. Quantitative data were submitted to descriptive and analytical statistical analysis; qualitative data were submitted to Thematic Content Analysis. Results: 66% of professionals were exposed to occupational stress. Doctors were associated with highly demanding work (p<0.001); nurses, nursing technicians, and dental professionals with active work (p<0.001); and dentists with lower psychological demand (p<0.001). Professionals with more than sixteen years of experience had better conditions to deal with stressful factors, compared to those with less than five years (p<0.03). Data integration showed implications of the pandemic in life, work, and interfaces with psychological symptoms. Conclusion: professionals worked under high psychological demands and a high risk of exposure to stress during the COVID-19 pandemic. Self-control and high social support may contribute to reducing these risks, as well as professional training and experience.

This study aimed to investigate Burnout Syndrome (BS) and associated factors in nursing professionals working in intensive care units (ICU) of the public service during the COVID-19 pandemic. 157 professionals were evaluated regarding sociodemographic, occupational and working conditions variables, and the Maslach Burnout Inventory (MBI) was used. The prevalence of BS was 45.2%, with some professionals suffering from more than one factor of the syndrome: emotional exhaustion (28.7%), depersonalization (3.8%) and low professional fulfillment (24.8%). Logistic regression analysis in the final model showed that female gender, not having children, statutory bond, professionals who had COVID-19 and declared wanting to leave the ICU environment had a higher risk of BS. The results showed BS in nursing professionals and that new risk factors were added with the advent of the pandemic.

Abstract:Introduction This is a descriptive cross-sectional clinical study with professionals from the Nursing Team (Nursing Assistant, Nursing Technician and Nurse). Objectives To assess the psychological impact of the Covid-19 pandemic on nursing staff professionals. Methods A descriptive, quantitative, cross-sectional study will be applied to a structured interview aimed at collecting sociodemographic and occupational data, Mental Health Scales evaluating professional exhaustion - Oldenburg Burnout Inventory and Beck's Anxiety Rating Scale to assess the state of anxiety. Results About 13,587 nursing professionals were interviewed, including nurses, technicians and nursing assistants. They were evidenced through the behavior indexes related to insomnia, the desire to cry and appetite variation may be related to the long working hours, the fear of contamination and the consequent absence from work, as well as the fear of getting sick may be related to the fact that the professional stops being a caregiver and starts to be cared for. Conclusions the study denotes the importance and need for interventions to promote and prevent mental well-being in health professionals exposed to COVID-19, these need to be implemented immediately, for nursing professionals, as they are on the front line, demanding attention Special. In this sense, the Nursing Council of the State of São Paulo created and implemented some bills such as the Obligation of Rest Rooms in Health Units, the Cuidando de Quem Cuida Program and the Yellow September Campaign in Allusion to actions for Nursing professionals for the prevention and promotion in mental health category. Prevalence, Risk Factors and Burnout Levels in Intensive Care Unit Nurses: A Systematic Review and Meta-Analysis

Abstract:Nursing is considered to be an at-risk profession of burnout due to daily exposure to difficult situations such as death and pain care. In addition, some units such as the intensive care unit (ICU), can be stressful due to high levels of morbidity and mortality and ethical dilemmas. Burnout causes a deterioration in quality of care, increasing the risk of mortality in patients due to poor performance and errors in the healthcare environment. The aim of this study was to analyse the levels, prevalence and related factors of burnout in ICU nurses. A systematic review and meta-analysis were carried out in the Medline, Scopus and CINAHL databases. Fifteen articles were found for the systematic review and four for the meta-analysis. With a sample of n = 1986 nurses, the meta-analytic estimate prevalence for high emotional exhaustion was 31% (95% CI, 8–59%), for high depersonalization was 18% (95% CI, 8–30%), and for low personal accomplishment was 46% (95% CI, 20–74%). Within the dimensions of burnout, emotional exhaustion had a significant relationship with depression and personality factors. Both sociodemographic factors (being younger, single marital status, and having less professional experience in ICU) and working conditions (workload and working longer hours) influence the risk of burnout syndrome.

Abstract:Introduction: Community health workers play a prominent role in the primary care context in Brazil. Burnout syndrome is an important work-related condition whose consequences affect job satisfaction. Objectives: To evaluate the extent of burnout syndrome and job satisfaction among community health workers in a city in southern Santa Catarina. Methods: This analytical, individual, cross-sectional study was conducted in Family Health Units. The participants were selected through random probability sampling, with a sampling error of 10%. After sociodemographic data collection, the Maslach Burnout Inventory and a job satisfaction scale were applied. Results: All 66 included workers were women. According to the analyses, there was a moderate positive correlation between satisfaction with colleagues and satisfaction with supervisors. There were weak positive correlations between emotional exhaustion and depersonalization and between professional fulfillment and satisfaction with promotions. However, there was a weak negative correlation between emotional exhaustion and both professional fulfillment and satisfaction with salary. Conclusions: The results indicate that the conditions of these workers are sufficiently satisfactory to deal with the demands of the job. Nevertheless, there was substantial dissatisfaction with salary, which can be a demotivator and trigger work-related depression.

Abstract:This report presents statistics on the utilization of non-Federal short-stay hospitals based on data collected through the National Hospital Discharge Survey from a national sample of the hospital records of discharged inpatients. Estimates are provided by the demographic characteristics of patients discharged, conditions diagnosed, and surgical and nonsurgical procedures performed, and by geographic region, bed size, and ownership of hospitals that provided inpatient care. Measurements of hospital utilization are given by frequency, rate, percent, and average length of stay.
