Abstract:The skin accumulation of therapeutic agents affects the efficiency of topical drug delivery. In this study, in vitro distribution of finasteride of ethosomes and liposomes in human cadaver skin after percutaneous delivery were investigated. Experiments were performed using modified Franz diffusion cells. Finasteride ethosomes, liposomes or hydroethanolic solutions were used as donor medium. Drug distribution at different skin layers and depths were studied by hotplate separation and frozen horizontal slicing technique. The result showed that the accumulation of finasteride in skin ranged from 9.7-24.3g/cm(2) at 12 or 24 hours. The ethosomes demonstrated better enhancing ability to deliver finasteride into the dermis layer than liposomes did. The finasteride concentration in the dermis layer from ethosomes was more than sevenfold higher than from liposomes. The finasteride accumulation in ethosomes group showed a distinctive reversed distribution profile. This distinctive reversed distribution profile is meaningful for exerting a favorable pharmacological effect for finasteride. The drug distribution profile in skin layers showed no significant difference between 12 and 24 hours application (p>0.05). The study demonstrated that finasteride can be accumulated at target site more effectively and maintained at higher level through the application of novel ethosomal carriers.
High mortality after pelvis and lower limb fractures in ESRD

Abstract:Sir, The high mortality rate associated with hip fracture in the general population, as well as in the end-stage renal disease (ESRD) population, is well described. However, the mortality associated with pelvis and lower limb fractures in ESRD patients remains unknown. We reviewed the medical records of all chronic haemodialysis or peritoneal dialysis patients hospitalized at Maisonneuve-Rosemont Hospital for a fracture from 1 July 1995 to 1 February 2007. Femoral neck, pelvis, femoral condyle, tibial plateau, patella, tibia and malleolus fractures were considered. Femoral neck and pelvis fractures were analysed separately, while the other fractures were analysed together as lower limb fractures. All patients who died during the study period were identified and mortality rates were calculated. During the 127-month-long study period, 60 dialysis patients were hospitalized for a total of 68 fractures. Seven patients had two fracture episodes, and one patient had three fracture episodes. The three most common fracture sites were hip (40 cases), pelvis (13 cases) and malleolus (8 cases) (Table 1). The 1-year mortality rate after a hip fracture was 42.5% (17 patients), with an in-hospital mortality rate of 20%. Two patients died during their hospitalization for a pelvis or lower limb fracture. One-year mortality rates were 30.8% after a pelvis fracture and 20% after a lower limb fracture. Median survival during the observation period was 427 days after a pelvis fracture and 581 days after a lower limb fracture. Hip fracture is associated with a 1-year mortality rate of 24% in the general population [1]. In ESRD, the 1-year mortalityrateafterahipfractureapproximates50%[2].Our results are consistent with previously reported mortality rates. Few studies have described the mortality associated with pelvis or lower limb fractures in the general population. In a British study, the 1-year mortality rates were 8.7% after pelvis fracture and 2.4% after a lower limb fracture [3]. In our ESRD cohort, the 1-year mortality rates for pelvis and lower limb fractures were increased 3.5- and 8.3-fold, respectively. In fact, the impact of pelvis and lower limb fractures on mortality in ESRD patients appears very similar to the impact of hip fracture in the general population. Like hip fractures [4,5], pelvis and lower limb fractures are probably markers of poor nutritional and general health status in this population. With a 1-year mortality rate similar to that of hip fracture in the general population, our data delineate for the first time the poor outcomes associated with pelvis and lower limb fractures in ESRD patients. The incidence, risk factors, clinical and functional outcomes of such fractures in ESRDpatientsneedtobeevaluatedinlarger,registry-based studies.
Quantification of nanoparticle uptake into hair follicles in pig ear and human forearm

Abstract:Drug delivery via the hair follicle (HF) especially with nanoparticles (NP) recently gained attention due to a depot effect and facilitated absorption conditions within the lower HF. With the prospect of transdermal drug delivery, it is of interest to optimize the follicular uptake of NP. In this study, a method was developed to quantify NP uptake into HF and applied in vitro in a pig ear model and in vivo in human volunteers. The influence of NP material on HF uptake was investigated using fluorescence-labeled NP based on poly(D,L-lactide-co-glycolide) (PLGA). All NP had similar hydrodynamic sizes (163-170 nm) but different surface modifications: (i) plain PLGA, (ii) chitosan-coated PLGA (Chit.-PLGA), and (iii) Chit.-PLGA coated with different phospholipids (PL) (DPPC (100), DPPC:Chol (85:15), and DPPC:DOTAP (92:8). Differential stripping was performed, including complete mass balance. The samples were extracted for fluorescence quantification. An effect of the PL coating on follicular uptake was observed as DPPC (100) and DPPC:DOTAP (92:8) penetrated into HF to a higher extent than the other tested NP. The effect was observed both in the pig ear model as well as in human volunteers, although it was statistically significant only in the in vitro model. An excellent in vitro-in vivo correlation (IVIVC, r(2)=0.987) between both models was demonstrated, further supporting the suitability of the pig ear model as a surrogate for the in vivo situation in humans for quantifying NP uptake into HF. These findings may help to optimize NP for targeting the HF and to improve transdermal delivery.

Abstract:Introduction: Follicular delivery is one of the targeted drug delivery methods aiming to target the hair follicles. The accumulation and retention time of targeted drugs is enhanced when nanoparticles are used as drug carriers. Particle size is one of the important factors affecting the penetration and accumulation of particles in the hair follicles, and there is a controversy in different studies for the best particle size for follicular delivery. Mouse models are mostly used in clinical trials for dermal, transdermal, and follicular delivery studies. Also, it is essential to investigate the reliability of the results between human studies and mouse models. Methods: Curcumin-loaded nanostructured lipid carriers (NLCs), as a fluorescent agent, with three different particle size ranges were prepared using the hot homogenization method and applied topically on the mouse and human study groups. Biopsies were taken from applied areas on different days after using the formulation. The histopathology studies were done on the skin biopsies of both groups using confocal laser scanning microscopy (CLSM). We compared the confocal laser scanning microscope pictures of different groups, in terms of penetration and retention time of nanoparticles in human and mouse hair follicles. Results: The best particle size in both models was the 400 nm group but the penetration and accumulation of particles in human and mouse hair follicles were totally different even for the 400 nm group. In human studies, 400 nm particles showed good accumulation after seven days; this result can help to increase the formulation using intervals. Conclusion: The best particle size for human and mouse follicular drug delivery is around 400 nm and although mouse models are not completely suitable for follicular delivery studies, they can be used in some conditions as experimental models.

Abstract:Hair follicles (HFs) represent a route of interest to drug delivery for treating several skin conditions. Iontophoresis, on the other hand, is a physical method to enhance drug permeation by applying a low electrical current to the formulation. HFs can be targeted following topical iontophoretic application, as they represent a pathway of lower electrical resistance, as well as a drug reservoir, in particular useful for nanoparticles (NPs), which can preferably accumulate in these structures. Combining both strategies may provide optimal results, but the literature still lacks evidence of the ideal NP characteristics for the iontophoretic drug delivery targeting the HFs. Here, we aimed to evaluate the effect of gelatin NPs' size and charge under iontophoresis application on NPs' deposition into the HFs. Four gelatin NP formulations were produced with varying gelatin concentrations and gelatin types (positively charged type A and negatively charged type B), with sizes ranging from 220 to 770 nm. A fluorescent dye, TRITC-dextran 150 kDa, was encapsulated for monitoring NPs deposition. Cutaneous penetration experiments were performed in vitro with and without iontophoresis for 6 h with pig ear skin. The deposition profile was assessed by confocal laser scanning microscopy. Photomicrographs showed a higher accumulation of the larger positively charged NPs (AL), reaching deeper portions of HFs, and showed iontophoresis further increased their deposition, resulting in the highest signal. In conclusion, these findings shed light on the applications of NPs and bring novel treatment opportunities for several diseases compromising the hair follicles.

Abstract:An efficient penetration and long-term storage of topically applied substances is important for drug delivery in medical treatment and cosmetics. It has recently become apparent that the hair follicles represent an efficient and long-term reservoir for topically applied substances. It was found that particles sized 300-600 nm penetrate more efficiently into the hair follicles than smaller or larger particles. In the present paper, the hair surface structure of human and porcine hairs was analyzed by electron microscopy. It could be observed that the thickness of the cuticula corresponds to the optimal size of the nanoparticles for penetration into the hair follicles. Additionally, it could be demonstrated that the cuticula of human vellus and terminal hairs were of similar thickness (approx. 530 nm), while the thickness of the cuticula obtained from porcine ear bristles were slightly thinner (approx. 320 nm).

Abstract:When applied to skin, particulate matter has been shown to accumulate in hair follicles. In addition to follicles, the skin topography also incorporates trench-like furrows where particles potentially can accumulate; however, the furrows have not been as thoroughly investigated in a drug delivery perspective. Depending on body site, the combined follicle orifices cover up to 10% of the skin surface, while furrows can easily cover 20%, reaching depths exceeding 25 µm. Hence, porous particles of appropriate size and porosity could serve as carriers for drugs to be released in the follicles prior to local or systemic absorption. In this paper, we combine multiphoton microscopy, scanning electron microscopy, and Franz cell diffusion technology to investigate ex-vivo skin accumulation of mesoporous silica particles (average size of 400-600 nm, 2, and 7 µm, respectively), and the potential of which as vehicles for topical delivery of the broad-spectrum antibiotic metronidazole. We detected smaller particles (400-600 nm) in furrows at depths of about 25 µm, also after rinsing, while larger particles (7 µm) where located more superficially on the skin. This implies that appropriately sized porous particles may serve as valuable excipients in optimizing bioavailability of topical formulations. This work highlights the potential of skin furrows for topical drug delivery.

Size-Dependent Penetration of Nanoemulsions into Epidermis and Hair Follicles: Implications for Transdermal Delivery and Immunization

Abstract:Nanoemulsions have been widely applied to dermal and transdermal drug delivery. However, whether and to what depth the integral nanoemulsions can permeate into the skin is not fully understood. In this study, an environment-responsive dye, P4, was loaded into nanoemulsions to track the transdermal translocation of the nanocarriers, while coumarin-6 was embedded to represent the cargoes. Particle size has great effects on the transdermal transportation of nanoemulsions. Integral nanoemulsions with particle size of 80 nm can diffuse into but not penetrate the viable epidermis. Instead, these nanoemulsions can efficiently fill the whole hair follicle canals and reach as deep as 588 μm underneath the dermal surfaces. The cargos are released from the nanoemulsions and diffuse into the surrounding dermal tissues. On the contrary, big nanoemulsions, with mean particle size of 500 nm, cannot penetrate the stratum corneum and can only migrate along the hair follicle canals. Nanoemulsions with median size, e.g. 200 nm, show moderate transdermal permeation effects among the three-size nanoemulsions. In addition, colocalization between nanoemulsions and immunofluorescence labeled antigen-presenting cells was observed in the epidermis and the hair follicles, implying possible capture of nanoemulsions by these cells. In conclusion, nanoemulsions are advantageous for transdermal delivery and potential in transcutaneous immunization.
Investigations on a triple mode waveguide horn capable of providing scanned radiation patterns

Abstract:Feedhorn reflector antenna assemblies have been widely used for both satellite communications and Radar applications [1]. A dual mode feedhorn is presented in [2–3] where TE<inf>11</inf> and TM<inf>01</inf> modes are excited to generate multiple phase centers when employed as a feed source for a reflector antenna. These papers [2–3] presented a technique of beam scanning in the Φ=90° cut plane only, thereby generation of multiple phase centers in the Φ=90° cut plane by control of the amplitude and phase of the modes. In [4] authors presented a tri-mode feedhorn design that employed TE<inf>11</inf>, TM<inf>11</inf>, and TE<inf>21</inf> modes, but the effort was to reduce the cross-polarization in the far-field pattern of a linearly polarized offset parabolic reflector antenna. In this paper, we present investigation results of a triple mode (TE<inf>11</inf>, TM<inf>11</inf>, and TE<inf>21</inf>) circular waveguide horn antenna with corrugated chokes which is capable of providing radiation pattern beam scanning in the range of θ=±24° in both Φ =0° and 90° cut planes similar to the case of a conventional phased array antenna. This is achieved by exciting all the three modes simultaneously in proper amplitude and phase. Though not discussed here, it is capable of providing multiple phase centers in both the Φ=0° and 90° cut planes. The simulation design of the horn was performed using finite element method (FEM) based full wave analysis software Ansoft HFSS. The geometry and design of the horn is discussed in section II. Simulation results of the horn are discussed in section III. Finally, section IV presents conclusion and future study.
Hair removal and bioavailability of chemicals: Effect of physicochemical properties of drugs and surfactants on skin permeation ex vivo

Abstract:Cosmetic hair removal procedures are everyday routines in our society. However, it is unclear if such routines lead to increased uptake of applied substances such as drugs or formulation compounds, potentially resulting in skin irritation or sensitization. The aim of this study was to elucidate the effect of common depilation and epilation methods on skin penetration of two surfactants and four model drugs of different physicochemical properties using the porcine ear model. It should be elucidated whether the substances' skin penetration behavior would be affected by hair removal procedures and if potential effects would be related to their polarity. Confocal Raman spectroscopy revealed no effect of hair removal on total penetration depths of SDS and sulfathiazole. Significantly higher relative penetrated amounts within 0-6 µm of stratum corneum depth were found for SDS after dry shaving, depilatory cream and waxing and for sulfathiazole after all depilation methods and partly after epilation. ATR-FTIR spectroscopy revealed no effect of hair removal on the penetration depth of lecithin LPC80, but higher relative amounts at the skin surface after wet shaving and electric epilation. Diffusion cell experiments using a lecithin-based microemulsion as carrier system for fluconazole, fludrocortisone acetate and flufenamic acid showed higher cumulative amounts, higher drug fluxes and shorter lag times for the more lipophilic drugs for some of the methods, but only shorter lag times in some cases for fluconazole. In summary, the observed effects appeared to depend on drug polarity and experimental setup.
Skin permeation of curcumin nanocrystals: effect of particle size, delivery vehicles, and permeation enhancer

Abstract:Nanocrystals are characterized by high drug loading, low carrier toxicity, and great structural stability. Therefore, they are a promising and versatile strategy for enhancing the local delivery of insoluble drugs. They achieve this by improving skin adhesion, concentration gradients, and hair follicle accumulation, as well as generating corona diffusion (which forms through the overlap of dissolved drug molecules around a nanocrystal). The development of suitable formulations for enhancing the passive diffusion and/or follicular targeting of nanocrystals is of great importance to clinical practice. We sought to elucidate the influence of particle size, a penetration enhancer, and delivery vehicles on the follicular accumulation and passive dermal permeation of nanocrystals. For this purpose, curcumin nanocrystals (particle size: 60, 120, and 480 nm) were incorporated into xanthan gum gels (delivery vehicles) with propylene glycol (penetration enhancer). This evaluation was performed in a porcine skin model. The results showed that xanthan gum reduced the follicular penetration and passive skin accumulation of curcumin nanocrystals. The propylene glycol enhanced the skin penetration and retention of curcumin nanocrystals in vitro for 24 h. The curcumin nanocrystals of smaller particle size (i.e., 60 and 120 nm) displayed higher passive skin penetration versus those with larger particle size (i.e., 480 nm); however, the latter type showed deeper follicular accumulation. In conclusion, the delivery vehicles, penetration enhancer, and particle sizes examined in this study affect the dermal penetration and accumulation of curcumin nanocrystals. Hence, their effects should be adequately considered when designing formulations of such nanocrystals.

Abstract:Recent advances in the field of dermatotherapy have resulted in research efforts focusing on the use of particle-based drug delivery systems for the stimuli-responsive release of drugs in the skin and skin appendages, i.e. hair follicles and sebaceous glands. However, effective and innocuous trigger mechanisms which result in the release of the drugs from the nanocarriers upon reaching the target structures are still lacking. For the first time, the present study demonstrated the photo-activated release of the model drug fluorescein isothiocyanate (FITC) from topically applied gold nanoparticle-doped bovine serum albumin (AuNPs-doped BSA) particles (approx. 545nm) using water-filtered infrared A (IRA) radiation in the hair follicles of an ex vivo porcine skin model. The IRA radiation-induced plasmonic heating of the AuNPs results in the partial decomposition or opening of the albumin particles and release the model drug, while control particles without AuNPs show insignificant release. The results demonstrate the feasibility of using IRA radiation to induce release of encapsulated drugs from plasmonic nanocarriers for the targeting of follicular structures. However, the risk of radiation-induced skin damage subsequent to repeated applications of high infrared dosages may be significant. Future studies should aim at determining the suitability of lower infrared A dosages, such as for medical treatment regimens which may necessitate repeated exposure to therapeutics. Statement of significance: Follicular targeting using nanocarriers is of increasing importance in the prophylaxis and treatment of dermatological or other diseases. For the first time, the present study demonstrated the photo-activated release of the model drug fluorescein isothiocyanate (FITC) from topically applied gold nanoparticle-doped bovine serum albumin (AuNPs-doped BSA) particles using water-filtered infrared A (IRA) radiation in the hair follicles of an ex vivo porcine skin model. Abstract:The transfollicular administration of pharmacologically active molecules is of current therapeutic interest, mainly with regard to delivery to specific sites of the hair follicle (HF) and the reduction of hepatic metabolism and systemic toxicity. HF are privileged pathways for specific molecules depending on formulations, which enter faster into these shunts than through the stratum corneum. The aim was to optimize the delivery of fluorescent microspheres into the HF, thereby, developing a standardized protocol for follicular targeting with microspheres. The number of HF showing penetration, as well as the depth of penetration, was determined. Freshly excised skin samples with terminal HF were divided into groups, with or without prior treatment with cyanoacrylate skin surface stripping-technique (CSSS). Thereafter microspheres at a size of 0.75-6.0 microm were applied according to the developed standardized protocol. Skin biopsies were obtained, shock-frozen, and sectioned in 5 microm slices. We demonstrated a selective penetration route of the microspheres into the HF. Optimal microsphere size proved to be approximately 1.5 microm, with a 55% rate of all HF, and with a maximum penetration depth of >2300 microm. Without previous CSSS treatment of the skin, the transfollicular microsphere penetration was below 27% with a maximum penetration depth of 1000 microm. Thus, the basis for follicular targeting of essential structures containing stem cells for keratinocytes, melanocytes, and mast cells has been laid.

Following the 1963 World Health Organization guidelines on the value of cytodiagnosis in the early detection of cancer and the need of respective education in cytology of medical students, clinicians, pathologists and other professionals, Erik Hauptmann, an internist (as the first head) and Ante Zimolo, a pathologist, initiated foundation of the Postgraduate Study in Medical/Clinical Cytology (PSMCC) in 1967. The study curriculum has since been revised twice, i.e. in 1996 with the introduction of professional studies and in 2005 as a specialist study. During the 40 years of its existence, 31 postgraduate studies have been organized, with 407 students enrolled, 380 students that completed the study and 119 students having acquired the MS degree. The efforts invested by the PSMCC students and lecturers contributed to the development of clinical cytology. The Section of Cytology and Cytodiagnosis (now Croatian Society of Clinical Cytology) of the Croatian Medical Association was founded in 1970. However, the good theoretical basis provided by PSMCC but inadequate practical work pointed to the need of the respective residency, founded in 1974. Then, PSMCC became an integral part of residency in clinical cytology. In the scope of the Croatia's pursuit of EU membership, the Task Force for Cytology of the Ministry of Health and Social Welfare of the Republic of Croatia has prepared the Residency Curriculum Proposal within the frame of clinical morphology professions with a common basis and directed towards cytopathology. In 1968, only one year of the PSMCC foundation, Inga Crepinko organized the first form of additional training for cytotechnicians. Accordingly, the foundation of PSMCC had driven a number of factors that have resulted in 48 units of clinical cytology currently active in 21 Croatian towns, employing 128 clinical cytologists, 13 other specialists and 122 cytotechnologists.
Enhanced transdermal delivery of curcumin nanosuspensions: A mechanistic study based on co-localization of particle and drug signals

Abstract:<p>Nanosuspensions have received much attention in enhanced transdermal delivery. However, the corresponding mechanisms have not been clarified. In particular, whether nanosuspensions can directly penetrate across the stratum corneum (SC) and what is the transdermal route for the enhanced penetration. Therefore, curcumin (CUR) was adopted in this study as a model drug, while an aggregation-caused quenching (ACQ) probe was physically embedded in CUR nanosuspensions, i.e., the CUR hybrid nanosuspensions (CUR-HNSs), for bioimaging. The ACQ properties enable identification of intact CUR-HNSs. The co-localization of particle and CUR signals was exploited to outline the translocation profiles of intact nanosuspensions as well as the cargoes. Three sizes of CUR-HNSs are prepared, which are spherical and amorphous. CUR is poor in transdermal transport even in propylene glycol solution, which was enhanced by nanosuspensions. Although 400 nm CUR-HNSs present higher steady state flux than 140 nm and 730 nm ones, the cumulative amount of permeated CUR is yet less than 2 % of the applied dose at 12 h. Co-localization of CUR and ACQ probe signals indicates that CUR-HNSs can infiltrate into the SC layer and accumulate in the hair follicles. The intact CUR-HNSs cannot enter into the skin. On the contrary, CUR molecules diffuse into the whole skin tissues following dissolution of CUR-HNSs in the SC and the hair follicles. In conclusion, nanosuspensions are advantageous for transdermal delivery of poor permeable drugs by filtrate into the SC and accumulate in hair follicles.</p>
