Abstract:BACKGROUND Endoscopic submucosal dissection (ESD) under general anesthesia in left lateral position may lead to transient impairment of pulmonary function. We used electrical impedance tomography (EIT), an increasingly implied non-invasive instrument for bedside real-time monitoring regional changes in ventilation, to assess the changes of regional lung aeration and ventilation in patients undergoing ESD. METHODS Twenty-two patients scheduled for elective ESD under mechanical ventilation in left lateral position were studied. We acquired 2-min EIT records at four time points: (M1) baseline, before induction of anesthesia, (M2) after the start of mechanical ventilation and before surgery, (M3) after surgery and before extubation, and (M4) after extubation and before leaving operation room, respectively. To quantify regional changes in lung ventilation, we calculated the ventilation proportion of left and right lung regions. Possible changes in lung aeration were detected by changes in end-expiratory lung impedance (EELI). Global inhomogeneity index (GI) was also analyzed. RESULTS After tracheal intubation in the left lateral position, left lung showed a lower ventilation proportion (M1, 49.6% vs. M2, 36.2% P<0.05), a reduction in EELI {∆EELI -87 [-809; 253]} and a higher GI index value (M1, 0.29±0.09 vs. M2, 0.41±0.12, P<0.05), while right lung showed a higher ventilation proportion (M1, 50.4% vs. M2, 63.8%, P<0.05) and an increase in EELI {∆EELI 161 [-952; 1,905]}. During ESD operation, no changes in either regional ventilation distribution or GI index were observed. After extubation, the GI values in right and left lung were both returned to the level before anesthesia. CONCLUSIONS In patients with left lateral position undergoing ESD, left lung was characterized by decreased ventilation and more inhomogeneity while right lung was opposite after intubation. ESD procedure with carbon dioxide insufflation did not lead to significant changes in either regional ventilation or homogeneity. And the change of lung inhomogeneity during ESD procedure is transient.
Prostate Cancer Role of Dynamic Contrast-Enhanced Magnetic Resonance ( MR ) Imaging and Proton MR Spectroscopic Imaging in the Detection of Local Recurrence after Radical Prostatectomy for Prostate Cancer

Abstract:Objectives: To assess the accuracy of magnetic resonance (MR) spectroscopic imaging (1H-MRSI) and dynamic contrast-enhanced MR (DCEMR) in the depiction of local prostate cancer recurrence in patients with biochemical progression after radical prostatectomy (RP). Materials and methods: 1H-MRSI and DCEMR were performed in 70 patients at high risk of local recurrence after RP. The population was divided on the basis of the clinical validation of MR results with the use of a transrectal ultrasound biopsy examination in a group of 50 patients (group A) and the prostate-specific antigen (PSA) serum level restitution after external beam radiotherapy, in a group of 20 patients (group B). Results: In group A, 1H-MRSI analysis alone showed a sensitivity of 84% and a specificity of 88%; the DCEMR analysis alone, a sensitivity of 71% and a specificity of 94%; combined 1HMRSI-DCEMR, a sensitivity of 87% and specificity of 94%. Areas under the receiver operating characteristic (ROC) curve for 1HMRSI, DCEMR, and combined 1HMRSI /DCEMR were 0.942, 0.93,1 and 0.964, respectively. In group B, 1HMRSI alone showed a sensitivity of 71% and a specificity of 83%; DCEMR, a sensitivity of 79% and a specificity of 100%; combined 1HMRSI and DCEMR, a sensitivity of 86% and a specificity of 100%. Areas under the ROC curve for each of these groups were 0.81, 0.923, and 0.94, respectively. Conclusion: Our results show that combined 1H-MRSI and DCMRE is an accurate method to identify local prostate cancer recurrence in patients with biochemical progression after RP. # 2007 European Association of Urology. The effects of prone and supine positions on the regional distribution of ventilation in infants and children using electrical impedance tomography

Abstract:Background: Positioning of ill children is often used to optimise ventilation-perfusion matching, thereby improving oxygenation. Objectives: To determine the effects of supine and prone positions, and different head positions, on the distribution of ventilation in healthy, spontaneously breathing infants and children between the ages of 6 months and 9 years. Methods: Electrical impedance tomography measurements were recorded from participants in supine and prone positions. Head positions included the head turned to the left and right in supine and prone positions, and in the midline in the supine position. Distribution of ventilation was described using end-expiratory-end-inspiratory relative impedance change. Results: A total of 56 participants (boys = 31 [55%]; girls = 25 [45%]) were studied. The dorsal lung was significantly better ventilated than the ventral lung (P < 0.001) in both body positions. The majority of participants (83%) had greater ventilation in the dorsal lung in both positions, whilst five participants (10%) demonstrated consistently better ventilation in the non-dependent lung in both positions. Head position had no effect on the distribution of ventilation. Conclusions: This study demonstrates that the distribution of ventilation in healthy, spontaneously breathing infants and children in supine and prone positions is not as straightforward as previously thought, with no clear reversal of the adult pattern evident.

Abstract:RATIONALE: General anesthesia and mechanical ventilation have negative impacts on the respiratory system, causing heterogeneous distribution of lung aeration, but little is known about the ventilation patterns of postoperative patients and their association with clinical outcomes.OBJECTIVES: To clarify the phenotypes of ventilation patterns along a gravitational direction after surgery by utilizing electrical impedance tomography (EIT) and to evaluate their association with postoperative pulmonary complications (PPCs) and other relevant clinical outcomes.METHODS: Adult postoperative patients at high risk for PPCs, receiving mechanical ventilation on ICU admission (N=128) were prospectively enrolled between November 18, 2021 and July 18, 2022. PPCs were prospectively scored until hospital discharge and their association with phenotypes of ventilation patterns was studied. The secondary outcomes were the times to wean from mechanical ventilation and oxygen use and the length of ICU stay.MEASUREMENTS AND MAIN RESULTS: Three phenotypes of ventilation patterns were revealed by EIT: phenotype1 [32%(N=41), a predominance of ventral ventilation], phenotype2 [41%(N=52), homogeneous ventilation], and phenotype3 [27%(N=35), a predominance of dorsal ventilation]. The median PPC score was higher in phenotype1 and phenotype3 than in phenotype2. The median time to wean from mechanical ventilation was longer in phenotype1 vs. phenotype2. The median duration of ICU stay was longer in phenotype1 vs. phenotype2. The median time to wean from oxygen use was longer in phenotype and phenotype3 than in phenotype2.CONCLUSIONS: Inhomogeneous ventilation patterns revealed by EIT on ICU admission were associated with PPCs, delayed weaning from mechanical ventilation and oxygen use, and a longer ICU stay.

Prolonged Continuous Monitoring of Regional Lung Function in Infants with Respiratory Failure

Abstract:Rationale: Electrical impedance tomography (EIT) allows instantaneous and continuous visualization of regional ventilation and changes in end-expiratory lung volume at the bedside. There is particular interest in using EIT for monitoring in critically ill neonates and young children with respiratory failure. Previous studies have focused only on short-term monitoring in small populations. The feasibility and safety of prolonged monitoring with EIT in neonates and young children have not been demonstrated yet. Objectives: To evaluate the feasibility and safety of long-term EIT monitoring in a routine clinical setting and to describe changes in ventilation distribution and homogeneity over time and with positioning in a multicenter cohort of neonates and young children with respiratory failure. Methods: At four European University hospitals, we conducted an observational study (NCT02962505) on 200 patients with postmenstrual ages (PMA) between 25 weeks and 36 months, at risk for or suffering from respiratory failure. Continuous EIT data were obtained using a novel textile 32-electrode interface and recorded at 48 images/s for up to 72 hours. Clinicians were blinded to EIT images during the recording. EIT parameters and the effects of body position on ventilation distribution were analyzed offline. Results: The average duration of EIT measurements was 53 ± 20 hours. Skin contact impedance was sufficient to allow image reconstruction for valid ventilation analysis during a median of 92% (interquartile range, 77-98%) of examination time. EIT examinations were well tolerated, with minor skin irritations (temporary redness or imprint) occurring in 10% of patients and no moderate or severe adverse events. Higher ventilation amplitude was found in the dorsal and right lung areas when compared with the ventral and left regions, respectively. Prone positioning resulted in an increase in the ventilation-related EIT signal in the dorsal hemithorax, indicating increased ventilation of the dorsal lung areas. Lateral positioning led to a redistribution of ventilation toward the dependent lung in preterm infants and to the nondependent lung in patients with PMA > 37 weeks. Conclusions: EIT allows continuous long-term monitoring of regional lung function in neonates and young children for up to 72 hours with minimal adverse effects. Our study confirmed the presence of posture-dependent changes in ventilation distribution and their dependency on PMA in a large patient cohort. Abstract:Background: Previous research has suggested correlations between the end-tidal partial pressure of carbon dioxide (PETCO2) and the partial pressure of arterial carbon dioxide (PaCO2) in mechanically ventilated patients, but both the relationship between PETCO2 and PaCO2 and whether PETCO2 accurately reflects PaCO2 in neonates and infants are still controversial. This study evaluated remote sampling of PETCO2 via an epidural catheter within an endotracheal tube to determine the procedure's clinical safety and efficacy in the perioperative management of neonates. Methods: Abdominal surgery was performed under general anesthesia in 86 full-term newborns (age 1-30 days, weight 2.55-4.0 kg, American Society of Anesthesiologists class I or II). The infants were divided into 2 groups (n = 43 each), and carbon dioxide (CO2) gas samples were collected either from the conventional position (the proximal end) or a modified position (the distal end) of the epidural catheter. Results: The PETCO2 measured with the new method was significantly higher than that measured with the traditional method, and the difference between PETCO2 and PaCO2 was also reduced. The accuracy of PErCO2 measured increased from 78.7% to 91.5% when the modified sampling method was used. The moderate correlation between PETCO2 and PaCO2 by traditional measurement was 0.596, which significantly increased to 0.960 in the modified sampling group. Thus, the PETCO2 value was closer to that of PaCO2. Conclusion: PETCO2 detected via modified carbon dioxide monitoring had a better accuracy and correlation with PaCO2 in neonates. [Effects of general anesthesia for cesarean section on infants].

Abstract:OBJECTIVE:To determine the safety of general anesthesia during cesarean section for neonates. METHODS:Singleton pregnant women for whom elective cesarean sections were planned after 37 gestation weeks were allocated to general anesthesia or epidural anesthesia. After preoxygenation, general anesthesia was induced with fentanyl (2.0 microg/kg), propofol (1.5 mg/kg) and vencuronium (0.08 mg/kg) followed by tracheal intubation. Oxygen-nitrous oxides (50%) was inhaled before delivery. Epidural anesthesia was performed at L(1 - 2) or L(2 - 3) on the left lateral decubitus position with 1.73% lidocaine carbonate (including 1:200 000 adrenaline). We studied arterial samples withdrawn from the infants 30 min after birth for the pH, PaO(2), PaCO(2), SPO(2) and Hct. Neonatal behavioral neurological assessments (NBNA) 3 - 5 days after birth were recorded. RESULTS:(1) The pH, PaO(2), PaCO(2), SPO(2) and Hct in the general anesthesia group were 7.34 +/- 0.08, (40 +/- 11) mm Hg, (73 +/- 17) mm Hg, (96.8 +/- 1.0)%, and (53 +/- 5)%, while in the epidural anesthesia group were 7.35 +/- 0.05, (41 +/- 8) mm Hg, (71 +/- 17) mm Hg, (96.6 +/- 1.0)%, and (54 +/- 6)%. (2) NBNA scores: In the general anesthesia group, neonatal adaptive capacity, passive tone, active tone, primary reflexes and general status were 12.6 +/- 0.7, 7.2 +/- 0.7, 7.4 +/- 0.6, 5.6 +/- 0.8, and 5.9 +/- 0.3 respectively. While in the epidural anesthesia group they were 13.4 +/- 0.8, 7.3 +/- 0.5, 7.3 +/- 0.8, 5.6 +/- 0.6, and 5.9 +/- 0.3 respectively. No significant difference was seen (P > 0.05) between the general anesthesia group and the epidural anesthesia group in NBNA scores, mean pH, PaO(2), PaCO(2), SPO(2) and Hct of newborn infants. CONCLUSION:It study suggests that general anesthesia for the elective cesarean section does not influence significantly the outcome of the newborn infants.

Abstract:The very many ways that have been tried to reproduce in animals lesions comparable to those found in human arteries indicate the diverse views on the causation of arteriosclerosis. In furthering such views the authors do not hesitate to proclaim the similarity of the expeiimental lesions with types of arteriosclerosis. Hence, one is soon struck by the great variety of lesions, both in the human and animal arteries, all of which are claimed to represent some form of arteriosclerosis. This we believe is not amiss, for it will permit the use of the term arteriosclerosis broadly, representing a group of lesions, and not an individual histological variety. This generic use of the term does not, however, appeal to all, for there are still such who would limit its use to a type unique in its mode of origin, or in its architectural character. There remain, however, few who after investigating the problem closely can see only one causative factor. So many different agents have been demonstrated associated in the human lesions or by experimental means that we can no longer deny the harmful effects of diverse states or substances. It is futile to minimize the harmful effect upon the arteries of bacteria, intoxications, high blood pressure, changes in the character of the blood, and fatigue or " wear and tear." Abstract:Substitution of the Phe3 aromatic ring in H-Tyr-Ticpsi[CH2-NH]Phe-Phe-OH with cyclohexylalanine (Cha) has been reported to result in a compound, H-Tyr-Ticpsi[CH2-NH]Cha-Phe-OH (TICP[psi]), showing substantially increased delta-opioid antagonist potency and high delta selectivity. TICP[psi] was radiolabeled by catalytic tritiation of its precursor Tyr(3',5'-I2)1TICP[psi]. Binding characteristics of the new tritiated pseudopeptide were determined using the radioligand binding assay in rat brain membranes. On the basis of the results of saturation binding studies performed at 25 degrees C, an equilibrium dissociation constant (Kd) of 0.35 nM and a receptor density (Bmax) of 112 fmol/mg protein were calculated. This new tritiated ligand exhibits high affinity for delta-opioid receptors, whereas its binding to mu and kappa receptors is weak. A study of [H3]TICP[psi] binding displacement by various receptor-selective opioids showed the following rank order of potency: delta > kappa = mu. These receptor binding characteristics of the ligand, together with its high specific radioactivity (41.3 Ci/mmol) and stability, makes it a useful tool for labeling delta-opioid receptors, both in vitro and in vivo.
Premature Infants Show Consistently Good Lung Compliance During Conventional Mechanical Ventilation

Abstract:Introduction Many extremely small preterm infants need to be intubated and mechanically ventilated during their intensive care stay. Animal studies indicate that lung compliance can deteriorate rapidly under conventional ventilation. This study investigated whether this presumed deterioration in compliance actually occurs in extremely small preterm infants. Methods Data from 56 conventionally ventilated preterm infants born from 2016 to 2022 at 25.22 weeks' gestation (±1.47) and 678.71 g (±138.14) birth weight were retrospectively analysed. This study investigated how dynamic compliance changed over the course of ventilation. Results The infants were conventionally ventilated from 7.25 days of life (±5.59) for 2.68 days (±2.35). Compliance at the beginning was 0.37 mL/cmH2O/kg (±0.17), after 1 h 0.38 mL/cmH2O/kg (±0.16), after 3 h 0.40 mL/cmH2O/kg (±0.20), after 6 h 0.44 mL/cmH2O/kg (±0.23), after 24 h 0.46 mL/cmH2O/kg (±0.17) and after 48 h 0.43 mL/cmH2O/kg (±0.13). The increase in compliance compared to baseline was statistically significant after 12 (p = 0.016) and 24 h (p = 0.042). Ventilation was performed with a PEEP of 7.51 cmH2O (±1.16) and a peak pressure of 20.64 cmH2O (±2.73). All received surfactant after birth and 21 (37.5%) also at the start of conventional ventilation. Conclusion During conventional ventilation in premature infants after administration of surfactant and with basically recruited lungs, no deterioration in compliance was observed either in the short or long term. The PEEP of almost 7.5 cmH2O used may have contributed to the fact that the deterioration described in the animal model did not occur in the preterm infants.

Changes in lung volume estimated by electrical impedance tomography during apnea and high-flow nasal oxygenation: A single-center randomized controlled trial

Abstract:Background: Previous studies concerning humidified, heated high-flow nasal oxygen delivered in spontaneously breathing patients postulated an increase in functional residual capacity as one of its physiological effects. It is unclear wheter this is also true for patients under general anesthesia. Methodology: The sincle-center noninferiority trial was registered at ClinicalTrials.gov (NCT NCT03478774). This secondary outcome analysis shows estimated differences in lung volume changes using electrical impedance tomography between different flow rates of 100% oxygen in apneic, anesthetized and paralyzed adults prior to intubation. One hundred and twenty five patients were randomized to five groups with different flow rates of 100% oxygen: i) minimal-flow: 0.25 l.min-1 via endotracheal tube; ii) low-flow: 2 l.min-1 + continuous jaw thrust; iii) medium-flow: 10 l.min-1 + continuous jaw thrust; iv) high-flow: 70l.min-1 + continuous jaw thrust; and v) control: 70 l.min-1 + continuous video-laryngoscopy. After standardized anesthesia induction with non-depolarizing neuromuscular blockade, the 15-minute apnea period and oxygen delivery was started according to the randomized flow rate. Continuous electrical impedance tomography measurements were performed during the 15-minute apnea period. Total change in lung impedance (an estimate of changes in lung volume) over the 15-minute apnea period and times to 25%, 50% and 75% of total impedance change were calculated. Results: One hundred and twenty five patients completed the original study. Six patients did not complete the 15-minute apnea period. Due to maloperation, malfunction and artefacts additional 54 measurements had to be excluded, resulting in 65 patients included into this secondary outcome analysis. We found no differences between groups with respect to decrease in lung impedance or curve progression over the observation period. Conclusions: Different flow rates of humidified 100% oxygen during apnea result in comparable decreases in lung volumes. The demonstrated increase in functional residual capacity during spontaneous breathing with high-flow nasal oxygenation could not be replicated during apnea under general anesthesia with neuromuscular blockade.
Numerical analysis of mechanical ventilation using high concentration medical gas mixtures in newborns

Abstract: When administered in relatively high concentrations the mechanical properties of inhaled gas can become significantly different from air. This fact has implications in mechanical ventilation where adequate respiration and injury to the lungs or respiratory muscles can worsen morbidity and mortality. Here we use an engineering pressure loss model to analyze the administration of medical gas mixtures in newborns. The model is used to determine the pressure distribution along the gas flow path. Numerical experiments comparing medical gas mixtures with helium, nitrous oxide, argon, xenon, and medical air as a control, with and without an endotracheal tube obstruction were performed. The engineering pressure loss model was incorporated into a model of mechanical ventilation during pressure control mode, a ventilator mode that is often used for neonates. Results are presented in the form of Rohrer equations relating pressure loss to flow rate for each gas mixture with and without obstruction. These equations were incorporated into a model for mechanical ventilation resulting in pressure, flow rate, and volume curves for the inhalation-exhalation cycle. In terms of accuracy, published values of airway resistance range from 50 to 150 cmH2O/L per second for a normal 3 kg infant. With air, the current results are 55 to 80 cmH2O/L per second for 0.3 to 5 L/min. It is shown that density through inertial pressure losses has a greater influence on airway resistance than viscosity in spite of relatively low flow rates and small airway dimensions of newborns. The results indicate that the high-density xenon mixture can be problematic during mechanical ventilation. On the other hand, low density heliox (a mixture of helium and oxygen) provides a wider margin of safety for mechanical ventilation than the other gas mixtures. The argon or nitrous oxide mixtures considered are only slightly different from air in terms of mechanical ventilation performance.

Abstract:Background: This study hypothesized that patients with extubation failure exhibit a loss of lung aeration and heterogeneity in air distribution, which could be monitored by chest EIT and lung ultrasound. Patients at risk of extubation failure were included after a successful spontaneous breathing trial. Lung ultrasound [with calculation of lung ultrasound score (LUS)] and chest EIT [with calculation of the global inhomogeneity index, frontback center of ventilation (CoV), regional ventilation delay (RVD) and surface available for ventilation] were performed before extubation during pressure support ventilation (H0) and two hours after extubation during spontaneous breathing (H2). EIT was then repeated 6 h (H6) after extubation. EIT derived indices and LUS were compared between patients successfully extubated and patients with extubation failure. Results: 40 patients were included, of whom 12 (30%) failed extubation. Before extubation, when compared with patients with successful extubation, patients who failed extubation had a higher LUS (19 vs 10, p = 0.003) and a smaller surface available for ventilation (352 vs 406 pixels, p = 0.042). After extubation, GI index and LUS were higher in the extubation failure group, whereas the surface available for ventilation was lower. The RVD and the CoV were not different between groups. Conclusion: Before extubation, a loss of lung aeration was observed in patients who developed extubation failure afterwards. After extubation, this loss of lung aeration persisted and was associated with regional lung ventilation heterogeneity. Global and regional heterogeneity of lung aeration in neonates with different respiratory disorders: a physiological, observational study

Abstract:Background. Aeration heterogeneity affects lung stress and influences outcomes in adults with acute respiratory distress syndrome (ARDS). We hypothesize that aeration heterogeneity may differ between neonatal respiratory disorders and is associated with oxygenation, so its evaluation may be relevant in managing respiratory support. Methods. Observational, prospective study. Neonates with respiratory distress syndrome (RDS), transient tachypnea (TTN), evolving bronchopulmonary dysplasia (BPD) and neonatal ARDS (NARDS) were enrolled. Quantitative lung ultrasound and transcutaneous blood gas measurements were simultaneously performed. Global aeration heterogeneity (with its intra- and inter-patient components) and regional aeration heterogeneity were primary outcomes; oxygenation metrics were the secondary outcomes. Results. 230 (50 RDS, TTN or evolving BPD and 80 NARDS) patients were studied. Intra-patient aeration heterogeneity was higher in TTN (mean: 61% [standard deviation: 33%]) and evolving BPD (mean: 57% [standard deviation: 20%], p<0.001), with distinctive aeration distributions. Inter-patient aeration heterogeneity was high for all disorders (Gini-Simpson index: between 0.6 and 0.72) except RDS (Gini-Simpson index: 0.5) whose heterogeneity was significantly lower than all others (p<0.001). NARDS and evolving BPD had the most diffuse injury and worst gas exchange metrics. Regional aeration heterogeneity was mostly localized in upper anterior and posterior zones. Aeration heterogeneity and total lung aeration had an exponential relationship (p<0.001; adj-R 2=0.62). Aeration heterogeneity is associated with greater total lung aeration (i.e., higher heterogeneity means a relatively higher proportion of normally aerated lung zones, thus greater aeration; p<0.001; adj-R 2=0.83) and better oxygenation metrics upon multivariable analyses. Conclusions. Global aeration heterogeneity and regional aeration heterogeneity differ amongst neonatal respiratory disorders. TTN and evolving BPD have the highest intra-patient aeration heterogeneity. TTN, evolving BPD and NARDS have the highest inter-patient aeration heterogeneity, but the latter two have the most diffuse injury and worst gas exchange. Higher aeration heterogeneity is associated with better total lung aeration and oxygenation.

Abstract:Background: Individualised bedside adjustment of mechanical ventilation is a standard strategy in acute coma neurocritical care patients. This involves customising positive end-expiratory pressure (PEEP), which could improve ventilation homogeneity and arterial oxygenation. This study aimed to determine whether PEEP titrated by electrical impedance tomography (EIT) results in different lung ventilation homogeneity when compared to standard PEEP of 5 cmH2O in mechanically ventilated patients with healthy lungs. Methods: In this prospective single-centre study, we evaluated 55 acute adult neurocritical care patients starting controlled ventilation with PEEPs close to 5 cmH2O. Next, the optimal PEEP was identified by EIT-guided decremental PEEP titration, probing PEEP levels between 9 and 2 cmH2O and finding the minimal amount of collapse and overdistension. EIT-derived parameters of ventilation homogeneity were evaluated before and after the PEEP titration and after the adjustment of PEEP to its optimal value. Non-EIT-based parameters, such as peripheral capillary Hb saturation (SpO2) and end-tidal pressure of CO2, were recorded hourly and analysed before PEEP titration and after PEEP adjustment. Results: The mean PEEP value before titration was 4.75 ± 0.94 cmH2O (ranging from 3 to max 8 cmH2O), 4.29 ± 1.24 cmH2O after titration and before PEEP adjustment, and 4.26 ± 1.5 cmH2O after PEEP adjustment. No statistically significant differences in ventilation homogeneity were observed due to the adjustment of PEEP found by PEEP titration. We also found non-significant changes in non-EIT-based parameters following the PEEP titration and subsequent PEEP adjustment, except for the mean arterial pressure, which dropped statistically significantly (with a mean difference of 3.2 mmHg, 95% CI 0.45 to 6.0 cmH2O, p < 0.001). Conclusion: Adjusting PEEP to values derived from PEEP titration guided by EIT does not provide any significant changes in ventilation homogeneity as assessed by EIT to ventilated patients with healthy lungs, provided the change in PEEP does not exceed three cmH2O. Thus, a reduction in PEEP determined through PEEP titration that is not greater than 3 cmH2O from an initial value of 5 cmH2O is unlikely to affect ventilation homogeneity significantly, which could benefit mechanically ventilated neurocritical care patients.

Abstract:Mechanical ventilation is a life-saving technology, but it can also inadvertently induce lung injury and increase morbidity and mortality. Currently, there is no easy method of assessing the impact that ventilator settings have on the degree of lung inssflation. Computed tomography (CT), the gold standard for visually monitoring lung function, can provide detailed regional information of the lung. Unfortunately, it necessitates moving critically ill patients to a special diagnostic room and involves exposure to radiation. A technique introduced in the 1980s, electrical impedance tomography (EIT) can non-invasively provide similar monitoring of lung function. However, while CT provides information on the air content, EIT monitors ventilation-related changes of lung volume and changes of end expiratory lung volume (EELV). Over the past several decades, EIT has moved from the research lab to commercially available devices that are used at the bedside. Being complementary to well-established radiological techniques and conventional pulmonary monitoring, EIT can be used to continuously visualize the lung function at the bedside and to instantly assess the effects of therapeutic maneuvers on regional ventilation distribution. EIT provides a means of visualizing the regional distribution of ventilation and changes of lung volume. This ability is particularly useful when therapy changes are intended to achieve a more homogenous gas distribution in mechanically ventilated patients. Besides the unique information provided by EIT, its convenience and safety contribute to the increasing perception expressed by various authors that EIT has the potential to be used as a valuable tool for optimizing PEEP and other ventilator settings, either in the operative room and in the intensive care unit. The effects of various therapeutic interventions and applications on ventilation distribution have already been assessed with the help of EIT, and this document gives an overview of the literature that has been published in this context.

Abstract:The scope and impact of sustained assisted ventilation were investigated within a cohort of 11,061 babies born to a geographically defined community. 76 (0.7%) normally formed newborn infants and 3 with major anomalies had respiratory failure potentially treatable by mechanical ventilation. 31 (0.3%) weighed less than 1000 g. 28 infants underwent more than 8 hours' assisted ventilation, which was initiated as an emergency in 14 cases, as an elective measure to minimise the hazards of transport in 10, and as a therapeutic option for apnoea in 4. Pneumothorax requiring drainage complicated the ventilation of 10 infants, 7 of whom died and 2 were subsequently disabled. Of the 14 ventilated survivors examined at school age, 2 had serious persisting disabilities associated with the ventilation and 2 with other perinatal events; of the 10 considered normal, 5 had had emergency ventilation.

Abstract:Objectives: A pilot study to compare pulmonary mechanics in a neonatal intensive care unit (NICU) cohort, specifically, comparing lung elastance between male and female infants in the NICU. Hypothesis: Anecdotally, male infants are harder to ventilate than females. We hypothesize that males have higher model-based elastance (converse: lower specific compliance) compared to females, reflecting underlying stiffer lungs. Study design: A clinically validated, single-compartment model is used to identify specific elastance (inverse of specific compliance) and resistance for each breath. Specific elastance accounts for weight differences when comparing male and female infants. Relative percent breath-to-breath variability (%ΔE) in specific elastance is also compared. Level of asynchrony was also determined. Patient-subject selection: Ten invasively mechanically ventilated patients from Christchurch Women's Hospital. Methodology: Airway pressure and flow data from 10 invasive mechanical ventilation (MV) infants from Christchurch Women's Hospital Neonatal Intensive Care Unit, New Zealand was prospectively recorded under standard MV care. Model-based specific elastance and resistance are identified for each breath, as well as relative percent breath-to-breath variability (%ΔE) in specific elastance. Results: Male infants overall had higher specific elastance compared to females infants (P ≤ .01), with median (interquartile range) for males of 1.91 (1.33-2.48) cmH2 O·kg/mL compared to 1.31 (0.86-2.02) cmH2 O·kg/mL in females. Male infants had lower variability with %ΔE of -0.03 (-7.56 to 8.01)% vs female infants of -0.59 (12.56-12.86)%. Males had 14.75% asynchronous breaths whereas females had 17.54%. Conclusion: Overall, males had higher specific elastance and correspondingly lower breath-to-breath variability. These results indicate male and female infants may require different MV settings, mode, and monitoring.

Abstract:Objective. The variation in pulmonary gas content induced by ventilation is not uniformly distributed in the lungs. The aim of our study was to characterize the differences in spatial distribution of ventilation in two transverse sections of the chest using electrical impedance tomography (EIT).Approach. Twenty adult never-smokers, 10 women and 10 men (mean age ± SD, 31 ± 9 years), were examined in a sitting position with the EIT electrodes placed consecutively in a caudal (6th intercostal space) and a cranial (4th intercostal space) chest location. EIT data were acquired during quiet breathing, slow and forced full expiration manoeuvres. Impedance variations representing tidal volume (VT), vital capacity (VC), forced expiratory volume in 1 s (FEV1) and forced vital capacity (FVC) were calculated at the level of individual image pixels and their spatial distribution was determined using the following EIT measures: the centres of ventilation in ventrodorsal (CoVvd) and right-to-left direction (CoVrl), the dorsal and right fractions of ventilation, the coefficient of variation (CV) and the global inhomogeneity (GI) index.Main results. The sums of pixel ventilation-related impedance variations reproduced reliably the volumetric dissimilarities amongVT, VC, FEV1and FVC, with no significant differences noted between the two examination planes. Significant differences in ventilation distribution were found between the planes during tidal breathing and slow full expiration, mainly regarding the ventrodorsal direction, with higher values of CoVvdand dorsal fraction of ventilation in the caudal plane (p< 0.01). No significant differences in the spatial distribution of FEV1and FVC were detected between the examination planes.Significance. The spatial distribution of ventilation differed between the two examination planes only during the relaxed (quiet breathing and slow VC manoeuvre) but not during the forced ventilation. This effect is attributable to the differences in thoracoabdominal mechanics between these types of ventilation.
