Abstract:In this article, I looked at the life of Yun Il-sun, a representative medical scientist of modern Korea, and examined the following problems. First, I took note of the position of the Korean people in the academic system of the Japanese colonial empire and restored the life of Yun Il-sun as specifically as possible. Yun was educated among Japanese people from elementary school to university. Although he received the best education at Old System High School and Imperial University and grew to be a prominent medical scientist, he could not overcome his identity as a colonized. Yun Il-sun, who moved from Keijo Imperial University to Severance Union Medical College, involved in activities founding of the Korean Medical Association and the Korean Medical Journal. Second, I the meaning of 'culture' to the intellectuals in the periphery. Old System High School and Imperial University where Yun Il-sun was educated were the hotbed of 'culturalism.' Yun's college days were the heyday of Taisho Democracy, and students were attracted to Marxism, Christian poverty movement, Buddhist cultivation movement and so on. Yun sought to overcome the ideological of young people through the acquisition of 'culture.' The 'culture' emphasized by Yun had an enlightenment characteristic that emphasized education, but it also functioned as a'identity culture of educated elites.' Third, I used the concept of 'colonial academism' and examined the aspects and characteristics of the colonial-periphery academic field, focusing on medicine. Yun Il-sun was a Korean professor at the Keijo Imperial University. He founded an academic society and published an academic journal for Koreans. He attempted to reproduce scholarship by doctoral dissertations. At the same time, several facts show that he was also in the affected area of 'colonial academism': the fact that he was kicked out of the Keijo Imperial University, the fact that the Korean Medical Association and the Korean Medical Journal were banned by Governor General, the fact that his students asked for doctoral degrees from Kyoto Imperial University where he studied. Yun Il-sun crossed the limits of 'colonial academism' and acted as the agent of empire. This was made possible by the characteristics of the academic discipline of medicine, the environment of the Severance Union Medical College, and personal traits of superior ability and indifference to politics. I the postcolonial evolution of the 'colonial academism' and 'culturalism.' The mix of continuity and discontinuity from 'colonial academism' and the hybrid of Japanese academism and American academism, the Korean characteristics of 'postcolonial academism.' Yun tried to harmonize the American academism with the Japanese academism and the purity of academism. This effort was revealed as an emphasis on basic medicine and natural sciences. As combined with culturalism and indifference to politics, he was recognized as the symbol of ivory tower and academism.

Abstract:Collagens in the extracellular matrix are thought to play an important role in regulating inflammatory responses by affecting cell adhesion and migration. The contact between collagens and cells is established mainly by alpha1beta1, alpha2beta1 and alpha11beta1integrin receptors. Here, we analyzed the contact hypersensitivity (CHS) reaction in mice that were genetically deficient in the collagen receptor alpha2beta1. Integrin alpha2beta1 is widely expressed and has been suggested to play an important role in mediating inflammatory responses. CHS was induced by applying dinitrofluorobenzene to abdominal skin and challenging with the same reagent on ear skin. Macroscopically and histologically, ear swelling in alpha2beta1-deficient mice did not differ from that in wild-type control mice. Immunohistological detection of infiltrated T lymphocytes, neutrophils and mast cells in inflamed ear skin revealed similar numbers in controls and integrin alpha2beta1-deficient animals. Our results suggest that the adhesive functions of integrin alpha2beta1 are dispensable for the CHS response; they may be compensated for by the collagen receptor alpha1beta1 or other collagen receptors.
Overcoming Barriers to Research Competency: a nationwide mixed-method study on residency training in the field of Korean medicine

Abstract:Objectives: This study aimed to analyze the educational needs of interns and residents in Korean medicine as the first step in developing an education program to improve their research competencies. Methods: A mixed-method design, incorporating both quantitative and qualitative data collection methods, was used to investigate the educational needs for research competencies among interns and residents working in Korean medicine hospitals nationwide. Data were collected through online surveys and online focus group discussions (FGDs), and processed using descriptive statistical analysis and thematic analysis. The study results were derived by integrating survey data and FGD outcomes. Results: In total, 209 interns and residents participated in the survey, and 11 individuals participated in two rounds of FGDs. The majority of participants felt a lack of systematic education in research and academic writing in postgraduate medical education and highlighted the need for nationally accessible education due to significant disparities in the educational environment across hospitals and specialties. The primary barrier to learning research and academic writing identified by learners was the lack of knowledge, leading to time constraints. Improving learners' research competencies, relationship building, autonomy, and motivation through a support system was deemed crucial. The study also identified diverse learner types and preferred educational topics, indicating a demand for learner-centered education and coaching. Conclusion: This study provides foundational data for designing and developing a program on education on research competencies for interns and residents in Korean medicine and suggests the need for initiatives to strengthen these competencies.

Abstract:The chirp of single-cycle microwave fields after interaction with pairs of parallel diffraction gratings is measured in a phase-sensitive terahertz spectrometer. Novel transmission grating pairs recently proposed by Tournois, which combine diffraction and refraction effects, yield an almost linear group delay over a bandwidth that exceeds that of conventional reflection gratings by a factor of ~3.5. The experimentally measured group delay shows good agreement with theoretical predictions for both types of grating. The applicability of the novel transmission gratings for stretching and compression of 1–2-fs pulses in the optical regime and the limitations set by material dispersion are discussed.
An Exploration into Life, Body, Materials, Culture of Mediaeval East Asia: Focusing on Emergency Medicine Recipes in Local Medicinals of Koryŏ Dynasty

Abstract:The Emergency Medicine Recipes in Local Medicinals (Hyang'yak Kugŭpbang) (c. 14th century) is known to be one of the oldest Korean medical textbooks that exists in its entirety. This study challenges conventional perceptions that have interpreted this text by using modern concepts, and it seeks to position the medical activities of the late Koryŏ Dynasty (918-1392) to the early Chosŏn Dynasty (1392-1910) in medical history with a focus on this text. According to existing studies, Emergency Medicine Recipes in Local Medicinals is a strategic compromise of the Korean elite in response to the influx of Chinese medical texts and thus a medical text from a "periphery" of the Sinitic world. Other studies have evaluated this text as a medieval publication demonstrating stages of transition to systematic and rational medicine and, as such, a formulary book that includes primitive elements. By examining past medicine practices through "modern" concepts based on a dichotomous framework of analysis - i.e., modernity vs. tradition, center vs. periphery, science vs. culture - such conventional perceptions have relegated Emergency Medicine Recipes in Local Medicinals to the position of a transitional medieval publication meaningful only for research on hyangchal (Chinese character-based writing system used to record Korean during the Silla Dynasty [57 BC-935 AD] to the Koryŏ Dynasty). It is necessary to overcome this dichotomous framework in order to understand the characteristics of East Asian medicine. As such, this study first defines "medicine", an object of research on medical history, as a "special form of problem-solving activities" and seeks to highlight the problematics and independent medical activities of the relevant actors. Through this strategy (i.e., texts as solutions to problems), this study analyzes Emergency Medicine Recipes in Local Medicinals to determine its characteristics and significance. Ultimately, this study argues that Emergency Medicine Recipes in Local Medicinals was a problem-solving method for the scholar-gentry from the late Koryŏ Dynasty to the early Chosŏn Dynasty, who had adopted a new cultural identity, to perform certain roles on the level of medical governance and constitute medical praxis that reflected views of both the body and materials and an orientation distinguished from those of the socalled medicine of Confucian physicians, which was the mainstream medicine of the center. Intertwined at the cultural basis of the treatments and medical recipes included in Emergency Medicine Recipes in Local Medicinals were aspects such as correlative thinking, ecological circulation of life force, transformation of materiality through contact, appropriation of analogies, and reasoning of sympathy. Because "local medicinals" is understood in Emergency Medicine Recipes in Local Medicinals as referring to objects easily available from one's surroundings, it signifies locality referring to the ease of acquisition in local areas rather than to the identity of the state of Koryŏ or Chosŏn. As for characteristics revealed by this text's methods of implementing medicine, Korean medicine in terms of this text consisted largely of single-ingredient formulas using diverse medicinal ingredients easily obtainable from one's surroundings rather than making use of general drugs as represented by materia medica or of multipleingredient formulas. In addition, accessible tools, full awareness of the procedures and processes of the guidelines, procedural rituals, and acts of emergency treatment (first aid) were more important than the study of the medical classics, moral cultivation, and coherent explanations emphasized in categorical medical texts. Though Emergency Medicine Recipes in Local Medicinals can be seen as an origin of the tradition of emergency medicine in Korea, it differs from medical texts that followed which specializing in emergency medicine to the extent that it places toxicosis before the six climatic factors in its classification of diseases.

Abstract:In recent years competition among retailers has led to intense competition among supply chains. The rise of the Internet of Things provides a basis for retail enterprises to have detailed knowledge of their supply chain and hence to enhance its management. Using the Internet of Things, retail enterprises are able to increase the quality of their supply chain management, so as to offer a more intelligent and efficient service to consumers. In this paper, the authors briefly consider the Internet of Things and then the research status of the Internet of Things as applied to supply chain management. A supply chain analysis model is built under the Internet of Things environment using Jingdong Mall as a case study. The results show that the Internet of Things improved the quality of information available to Jingdong, enhanced its management efficiency and improved customers' satisfaction; it also reduced the cost of supply chain management whilst creating more new value. Finally, some problems encountered in the process of the application of the Internet of Things technology to Jingdong Mall are analyzed and corresponding solutions are proposed.

Abstract:Japan annexed Korea in 1910 and with the promulgation of the Chosen Kyoyuk Ryong (the Korea Education Decree) in 1911, it began to conduct education for the Korean people. However, this was only a matter of formality and a policy for liquidating the spirit of the Korean people. It finally resulted in the uprising of March 1, 1919 of the Korean people that has a cultural tradition of a high level. This event served as a cause of widely spread censure among the Korean people as well as the people of the whole world. Even in Japan voice of criticism rose high. Such being the situation, Japan amended the Korean educational law under the pretext of shifting to a so-called civil-rule policy. The Japanese authorities adopted the same educational system as was practiced in Japan proper, for primary and middle school education. As for higher education, they placed under a strict control the educational facilities already established by Korean people and foreign missionaries, suppressing even minor expansions of existing facilities. However, the movement by some Korean educators to establish a private university and efforts by some missionaries to integrate the existing educational organizations into a university made it inevitable for the Japanese authorities to set up a university of their own in Korea. Thus, they hurriedly established the Kyonsong Imperial University in which was included a medical college that was an indispensable organization for colonial education. They professed that the medical college was established for the purpose of providing equal opportunities and privileges to Korean and Japanese students, but, on the contrary, the operation of the college was done strictly under their colonial policy. The system of the Kyongsong Imperial University was enforced acording to the Japanese Imperial University Law, and all the faculty members and the administratial officials were Japanese. As for Koreans, a few graduates of the university was named nonpaid deputy assistants, and graduates of other colleges were employed as nonpaid subdeputy assistants. In most cases, Koreans, finally handicapped compared with Japanese, could not continue their study and research. A few of them who could conduct continued study and research were placed under strict restraints and, accordingly, their opportunities to achieve academic and social promotion were quite limited. During the history of 20 years of the Kyongsong Imperial University Medical College Yun Il Son and Ko Yong Sun served as assistant professors for 13 months and three days respectively. In addition, there were 12 Koreans who worked as temporary assistants, the periods of their service varying from two days to 10 years. The rate of graduates for Koreans was less than 30% and the rate for Koreans who received a degree of doctor of medical science was only 25%. From 1940, the course of the medical college was shortened to three years and same months under the war-time system, and with Japan's unconditional surrender to the Allied Forces on August 15, 1945, the entire faculty of the medical college was discharged by the U.S. Military Government on November 5, 1945.
[Takeki Kudoh's Research on Modern Medical Science and Japanized Confucianism in Colonial Korea (Chosŏn)]

Abstract:This paper reviews Kudoh Takeki's activity critically during the colonial Korea period, regarding his research on Medical Science and Japanized Confucianism. He managed 'The Seoul Gynecological Hospital'for approximately 35 years in the Chosŏn period as a Japanese resident with Chosŏn status. He published medical knowledge about obstetrics through more than 280 articles, and attempted to improve the hygiene and health of 'Korean Women'. He tried to complete his will toward 'One Unity of Chosŏn and Japan'by terminating the Chosŏn culture 'gene'as an intention. The purpose would enlighten Chosŏn by Japanese blessing. This paper aims to confirm his intention by two aspect of analysis by 'Medical Science'as an occupation and 'Confucianism'and the background of his thought. The content of Kudoh Takeki's research in Chosŏn regarding Medical Science-Confucianism is described as below. First, the purpose and mission of Kudoh Takeki regarding Chosŏn was analyzed. The papers revealed the Kudoh Takeki mentioned only the 'HusbandMurders of Corean Women', which was defined by Kudoh Takeki as 'A Special Crime of Corea'. This paper examined his intensions. Second, writings by Kudoh Takeki were listed to verify the 'medical'field and 'non-medical'field according to the subject. No list of contents was found for his more than 280 articles or essays in magazines/newspapers/ publications, and these papers only described the Kudoh paper "A Special Crime of Corea"and studied the separate book publication by Kudoh THE GYNAECOLOGICAL RESEARCH OF HUSBANDMURDERS OF COREAN WOMEN, A SPECIAL CRIME OF COREA. Third, the genealogy of Confucianism of Kudoh Takeki was analyzed as his background of mental·thought by his hometown and the school he graduated from. The people from Kumamoto and Seiseiko school who were influenced by 'Yi Toegye'of Chosŏn Confucianism were more active than general Japanese. Fourth, the practical activity of Kudoh Takeki in Chosŏn was described. The paper revealed that his brother Tadaske and Shigeo also stayed in Chosŏn to act as an important assistants for the Colonial Chosŏn Government-general. Kudoh was an important man in Japanese society in Chosŏn, acting as a member of 「Group of Same Origin」 and 'Chosŏn Association of great Asia'which was an important organization assisting Colonial Chosŏn Government-general and was a representative position in Seoul district of Bukmichang-jeong(now Bukchang-dong) Fifth, Kudoh Takeki's precise activity to terminate Chosŏn cultural 'gene'and lead to enlightenment was analyzed by an examination of his Medical Science as an occupation and Confucianism as a background of his thought. Even he attempted to enlighten the brutal Chosŏn people in cultural aspects but it was only a tool to assist the colonial policy of Japan by emphasizing 'Kyoikuchokugo(Imperial Rescript on Education)'to implant the Kodo-Seishin(Imperial Spirit). Analyzing the relationship of Kumamoto Practical Party with Yi Toegye, the intention of a deep connection toward 'One Unity of Japan and Chosŏn'by colonial policy was revealed. In conclusion, the paper revealed the Japanese modernization frame to complete 'One Unity of Japan and Chosŏn'and 'Make people to obey the Japan Emperor'by enlightening the dark Chosŏn and merging them with Japan as Kudoh intended.

Abstract:Background While clinical competency is crucial for traditional East-Asian medical education, available studies on the educational system for fostering clinical performance are scarce. This study aimed to review the educational system, curriculum, facilities, and management of current traditional East-Asian medicine in a well-established university of Korea and develop a Best Practice Framework (BPF) of clinical competency education. Methods The clinical competency education system in Pusan National University School of Korean Medicine was systematically described through 5 steps of governance of the educational system, competency of the graduates, educational resources, assessment strategies and tools, and gaps in the curriculum. We also reviewed the experiences in education and the points to be improved. Results The Office of Traditional Korean Medicine Education governs the development, implementation, and evaluation of the educational curriculum for cultivating students' clinical competency. Medical students have undertaken 39 modules of clinical biomedicine and 21 of traditional medicine during the clinical clerkship courses in an affiliated hospital, Clinical Skill Practice Center, clinical research center, practice lab for medical herb, and other locations. After training, 15 modules of simulated clinical training using standardized patients, students' clinical competency are evaluated by a Clinical Performance Test using a Clinical Performance Examination (CPX) and an Objective Structured Clinical Examination (OSCE) for biomedical and traditional medical skills. Conclusions A clinical competency framework is required for a qualified physician of traditional East-Asian medicine. This study reviewed the current well-organized educational system of Korean traditional medicine in detail, which can be used for the BPF of competency-based clinical education. We expect the current study to be a representative reference for establishing an educational system of traditional medicine such as acupuncture and medical herbs in other countries.

Working to Change Systems: Repatriated U.S. Trained Japanese Physicians and the Reform of Generalist Fields in Japan

Abstract:Phenomenon: Internationally, efforts to produce an adequate supply of effective generalist physicians commonly encounter resistance. Achieving reform requires changes in educational and clinical practice cultures, and clinician educators play a central role in championing change. In Japan, training in generalist fields has historically been lacking, but for decades the government has advocated alignment with Western models. Meanwhile, some Japanese physicians have pursued U.S. training in generalist fields with intention to help change the clinical education and practice systems back in Japan. This study examines the endeavors of repatriated Japanese International Medical Graduates and provides a lens to understanding national challenges with reform and insights into strategizing next steps. Approach: Individual, semi-structured interviews were conducted with 19 purposively sampled Japanese IMGs who had repatriated across Japan after completing U.S. clinical residency in generalist fields. Iterative data collection and thematic analyses were performed using constant comparison. Findings: Participants identified Japanese medical universities and public sectors as steeped in traditions with systemic inertia. In turn, participants described well-informed career decision making involving connections and teammates, which commonly resulted in employment at new or smaller hospitals. Education-related efforts prioritized direct clinical work with physician trainees in the hope of building expansive lineages of educators. Main challenges were Japanese structural and cultural incongruences with Western generalist-based clinical practice. Participants described a competitive relationship with the long-standing ikyoku-based postgraduate education model and associated organ-based organization of clinical practice. Insights: Japanese IMG championing of clinical education and practice in generalist fields is largely marginalized within Japan's clinical education and practice landscape. National-level reform will require transforming or displacing the structurally and culturally rooted traditional infrastructure. Specific measures must be culturally nuanced but likely include those proven effective for similar reforms elsewhere. Based on Japan's national cultural characteristics, sustained leadership is anticipated to be particularly important.
Clinical pathways for Korean medicine: An implementation approach to impact on the clinical process and association with attitudes

Abstract:Background: South Korea's Ministry of Health and Welfare has developed clinical pathways for Korean Medicine (KM-CPs). As part of this initiative, a panel comprising Korean Medicine doctors (KMD) was assembled. This implementation study aimed to preliminarily explore how KM-CP implementation affects the appropriateness and efficiency of the clinical process and its relation to attitude. Methods: Through random sampling, 311 KMDs were recruited as panelists to participate in two surveys. The surveys included information regarding the KM clinical environment and KM-CP implementation. A panel management program and educational materials were provided to KMDs between the two survey periods. Only 262 KMDs who responded to both surveys were included in the analysis. Three analyses were conducted: 1) descriptive analysis of the study variables, 2) panel analysis using the ordered logit regression model to elucidate the impact of KM-CP on the appropriateness and efficiency of the clinical process, and 3) ordered logit regression analysis of the association between KM-CP implementation and attitude. Results: More than two-thirds of the KMDs attempted to adopt KM-CP, with mostly positive perception expressed by these doctors. However, expectations and concerns coexist with the standardization of KM-CP. Cases in which KM-CPs were partially and mostly implemented respectively had negative and positive effects on the appropriateness and efficiency of the clinical process compared to those in which KM-CPs were not implemented. Compared to neutral attitude, positive and very positive attitudes tended to be associated with increased implementation of KM-CP. However, statistical significances were not observed. Conclusions: The impact of KM-CP on the clinical process and its association with attitude were found to be statistically unclear or inconsistent. Considering the study limitations and implications, we suggest a policy and academic strategies aimed at fostering improvement to enhance its utility.

[Traditional medicine under Japanese rule after 1930s]

Abstract:Japan, which occupied Korea from 1910 through the end of World War II, transformed traditional medicine. Japanese colonialists propagandized the "benefits of modern civilization such as western medicine" and rejected the advantages of traditional medicine. This bias against Korean traditional medicine mirrored the government's rejection of its own traditional medicine. So, Korean traditional medicine was marginalized in the national health care system: traditional doctors were excluded from public institutions and references to traditional medicine were purged from school textbooks and newspapers. The wars that Japan waged between 1931 and 1944 effected a favorable change toward traditional medicines, however. The wars created a severe shortage of drugs and medical personnel. Thus the colonial government was eager for Koreans to cultivate and gather herbal drugs; it also built a large research institute for herbalism at the Keijo Imperial University in 1938. The colonial government made pharmacopoeia for traditional herbal drugs including plant and animal drugs from 1937 to 1942, independently from Japan. Under these conditions, the prestige of traditional medicine was greatly improved. Influential newspapers and magazines covered the traditional medicine and public lectures on traditional medicine drew large audiences. The wartime government abandoned its opposition to traditional medicine, and appointed a traditional practitioner to the staff of the public hospital in 1934. Moreover, the government allowed the association of the traditional medical doctors in Seoul to train three hundred more practitioners between 1937 and 1942. Japanese colonial policy toward traditional medicine reflected the contradiction between modernizing ideology and the reality of poor colonial medical care. Japanese propaganda promised that the colonial regime would provide more advanced medicine to Korea, but the promise was an empty one. In this situation, traditional medical doctors and herbalists once again shouldered the main responsibility for the health of the Korean people.

Abstract:Background No previous research has targeted educators regarding educational practice and the achievements of students in terms of the learning objectives of clinical clerkships in university general medicine departments of Japan. We aimed to clarify the characteristics of clinical clerkships in Japanese general medicine departments using a questionnaire administered to chairpersons of university general medicine departments. Methods This was a descriptive questionnaire-based study using Google Forms (Google, Inc., Mountain View, CA, USA). We asked the chairpersons of general medicine departments in Japanese universities the following questions, with responses given on a 5-point Likert scale: Question 1: How well are primary symptoms in the national model core curriculum for undergraduate medical education taught in clinical clerkships in university general medicine departments? Question 2: How successfully can students achieve the learning objectives of the national model core curriculum for undergraduate medical education through clinical clerkships in general medicine departments of university hospitals? Question 3: How successfully can students achieve the learning objectives of the national model core curriculum for undergraduate medical education through clinical clerkships in other community clinics or hospitals? The results of the questionnaire responses are described as mean±standard deviation. Results Of the 71 Japanese universities with general medicine departments, 43 were included in the analysis. For Question 1, the symptoms and pathophysiologies with a mean score of 4 points or higher were fever, general malaise, anorexia, weight loss or gain, edema, abdominal pain, lymphadenopathy, and headache. All those symptoms require basic medical competencies. For Questions 2 and 3, the intramural clinical clerkship of general medicine departments had a higher mean score than the extramural clinical clerkship for diagnostic reasoning that emphasizes medical history and physical examination and a comprehensive approach to patients with multiple health problems. In contrast, the extramural clinical clerkship, in which medical students can build experience with community-integrated care, had a mean score of 3 points or higher for all items. Conclusions The clinical clerkship in general medicine departments of Japanese universities provides students with chances to acquire clinical competencies regarding primary symptoms and pathophysiologies. Additionally, the extramural clinical clerkship provides experience in community-based integrated care, including home medical care, collaboration, health and welfare, and long-term care.
Characteristics of Using Oriental Medicine Clinics during the Japanese Occupation: An Analysis of the 1931 Seoul Bochun Clinic Account Book

Abstract:This article analyzes the "account book" of Kim Young-hoon (1882-1974), which summarizes information about patients at his Bochun Clinic in Seoul (then Gyeongseong) in 1931. Kim Young-hoon was a pivotal figure in the medical scene throughout the Korean Empire, the Japanese occupation, and the early years of the Republic of Korea. He left behind a large amount of documentation during his 60 years of practice at the Bochun Clinic, which he opened in the spring of 1909. In particular, the 1931 "account book" offers an insight into the daily life histories of his patients. Among the patient-visitors recorded in the account book, there were many influential people of the time, ranging from privileged individuals to anti-Japanese independence activists, from those in political and economic fields to those in academic and entertainment fields. At the same time, a significant number of lower-class people also visited the Clinic. Geographically, patients were centered in the city center of the capital, Gyeongseong, but were also widely distributed throughout the country. There are indications that those from the rural areas stayed in the homes of their acquaintances in Seoul. As such, the account book provides a tangible, concrete picture of the clinic's management for the year 1931, including visitor demographics, visiting diagnoses, telephone consultations, and the total cost of medicinal prescriptions. Because the account book is a one-year statistic, it has its limitations; however, it is the smallest unit that can be analyzed statistically. It provides insights into how many people came in over the course of a year and how much they spent. The expenditures are kept per individual family. The patient's name, prescribed medication, and the price of the medicine are mandatorily included, and in many cases, the place of residence and family relationships are also noted. The account book shows several layers of householders, servants, and employees in the extended family; it also shows people in various occupations. A few privileged families accounted for nearly half of the total expenditures, and the powerful visited frequently, utilizing Oriental medicine for many of their daily needs. For some, the Bochun Clinic is reminiscent of the royal temples of the dynasties. Patients come from the center and suburbs of Seoul, as well as from all over the country. In one year, more than one thousand types of prescriptions are issued and the total cost of medicines is about 33 seom (≒180 liters of rice). Although there is a concentration of high-frequency prescriptions, more than a thousand prescriptions are prescribed only once, which shows that the practice is specialized for each individual. Patient visits, consultations, and telephone use are observed, and the use of new drugs, quinine, and special ginseng as one-herb medication (danbang) are also noticeable. The statistical analysis of the 1931 Bochun Clinic "account book" can serve as a milestone for comparative analysis of the patterns of herbal medicine use before and after that year. Meanwhile, the Bochun Clinic "account book" shows the continuation of traditional practices of herbal medicine by both the powerful and the masses. On the one hand, Koreans responded to the coercive tide of modernity symbolized by the Imperial Governorate of Japan, but on the other hand, they were unwilling to let go of tradition and their own authority. While actively embracing the tide of civilization, Koreans also internalized their own rationality and sought to open a new path forward, a sentiment discernible between the lines of the "account book."

Abstract:Heo Joon is one of the best-known physicians of the Chosun Dynasty, the last imperial dynasty (1392~1910) of Korea. He had served King Seonjo () during his practice, and has produced many publications on medicine. Then, how did he actually treat the patients? So far, other than the case when he treated Gwanghaegun's smallpox, it is not clearly known how and when he attended and treated the ill. In his most famous book, the Treasured Mirror of Eastern Medicine, he details the physiopathological mechanisms, diagnoses, treatments or prescriptions, and treatment cases, however, it is not clear if they're from his own clinical experiences. Nevertheless, based on the written method, the original information is reconstituted according to its respective editors of the TMEM, a particular case being included may be considered as an agreement and acceptance of an actual treatment executed. This research analyzes what type of medicinal theory that the main writer Heo Joon employed in his real treatments, as well as how he diagnosed and treated diseases. After analyzing the complete series of the TMEM, we found a total of 301 clinical cases. Here, one may wonder, why does the Section of Inner and External Bodily Elements, that deal with diseases and the structure of the body, have far outnumber cases than the Section of Miscellaneous Disorders? Why does the TMEM introduce the various types of disease experiences and treatment cases, medical cases, simple treatments, nurturing life, materia medica, and also include supernatural phenomena? Why does the TMEM include the experiences and cases from the book published in the Song, Jin, Yuan dynasty of China, moreover in the Ming Dynasty of its time. These questions can be answered to the extent that Heo Joon and the others who participated in completing the book sought to justify the new clinical medicine practices, and because it had to be acceptable to the Confucius beliefs which dominated the society, and also because the book came to light in a time when tensions between the pre-existing Chosun medicine and the newly introduced Chinese medicine were evident. Among the clinical cases in the TMEM, there are only 41 cases that can be considered as Medical Cases which include the pathology and treatment mechanism. After analyzing these mechanisms, we were able to discover that they cover not only the theories of the 4 great physicians of Jin-Yuan Dynasty, but also the theories of the Danxi's Medical Current, a big trend in the Early Ming Dynasty, and some of the most recent clinical cases that had been just reported at the time. However, Heo Joon did not lean towards a particular theory of medicine; rather, he insisted on establishing a classical medicine based on the traditional medicinal scriptures such as the Yellow Emperor's Inner Canon or Shennong's Classic of Materia Medica, and had created his own Body-Viscera medicine, as Shin Dongwon's recent research. Moreover, he successfully secured his own right to be a clinical physician by customizing the amount of medication in prescriptions for the people of Chosun. Heo Joon was one of the chief physicians for the Royal Family of the Chosun Dynasty. Despite the tendency of traditional medicine to lean towards Taoism or Fangshu, for him the most important thing was the actual treatment of diseases. As a result, Heo Joon successfully treated smallpox by utilizing traditional medicinal methods, by breaking the taboo of not using medication on such diseases, as well as he was able to treat an unknown disease, scarlet fever, by discovering the pathological mechanism of the illness. Also he made bold decisions on altering existing prescriptions to treat diseases more efficiently. The TMEM consists of not only justified methods that integrate the different and scattered medicinal and clinical practices, which many insisted their originality, but also was backed with Heo Joon's such credible and endeavored clinical medicine.

Abstract:Since medical education programs in Korea and Japan seem to mutually influence each other, this review article provides a history of Japanese medical education, focusing on the way in which it influenced and was influenced by Korean medical education. In the late 19th century, the University of Tokyo established the core medical school, disseminating its scholarship and system to other medical schools. In the early 20th century, the balance between the quality and quantity of medical education became a new issue; in response, Japan developed different levels of medical school, ranging from imperial universities to medical colleges and medical vocational schools. After World War II, all of Japan's medical schools became part of the university system, which was heavily regulated by the Ministry of Education (MOE) Standard for the Establishment of Universities. In 1991, MOE deregulated the Standard; since 2000, several new systems have been established to regulate medical schools. These new approaches have included the Model Core Curriculum, 2-year mandatory postgraduate training, and a medical education accreditation system. Currently, most medical schools are nervous, as a result of tighter regulatory systems that include an accreditation system for undergraduate education and a specialty training system for postgraduate education.
