Abstract:The research addresses zoonotic sporotrichosis in Brazil, particularly caused by Sporothrix brasiliensis, highlighting its epidemiological severity. Transmission occurs through contact with sick animals, especially felines, and diagnosis in humans is challenging due to the low fungal load in the lesions. The study analyzed data from Information System for Notifiable Diseases (SINAN) and Zoonosis Surveillance Unit (UVZ) from January 2017 to March 2023, carried out in Contagem, Minas Gerais. Geospatial tools and statistical analysis revealed a significant increase in cases, peaking in 2021 for felines and 2022 for humans. The geospatial analysis highlighted areas of higher incidence, suggesting a correlation between human and feline populations. The research contributes to the understanding of sporotrichosis in Contagem, emphasizing the importance of integrated approaches for surveillance and control strategies, aiming to mitigate impacts on the local community.

Abstract:BackgroundSporotrichosis is a mycotic infectious disease that is generally acquired by traumatic inoculation of contaminated materials especially from plant debris or through bites and scratches from diseased animals, such as domestic cats. It affects the skin, lymphatic system, and other organs in the warm-blooded host. Etiological agents are embedded in the plant-associated order Ophiostomatales. With essential differences between possible outbreak sources and ecological niche, host-environment interactions are classic determinants of risk factors for disease acquisition. Sporotrichosis outbreaks with zoonotic transmission, such as those that are ongoing in southern and southeastern Brazil, have highlighted the threat of cross-species pathogen transmission. Sporothrix brasiliensis has emerged as a human threat owing to the intimate contact pattern between diseased cats and humans in endemic areas.ResultsWe describe the recent emergence of feline sporotrichosis in the metropolitan region of São Paulo, Brazil, with an overwhelming occurrence of S. brasiliensis as the etiological agent. A phylogenetic and a haplotype approach were used to investigate the origin of this epidemic and the impact of feline transmission on genetic diversity. During the last 3-year period, 163 cases of feline sporotrichosis were reported in São Paulo with proven S. brasiliensis culture. The haplotype diversity of feline S. brasiliensis isolates revealed the expansion of a clonal population with low genetic diversity. Haplotype analysis confirmed that isolates from São Paulo shared the haplotype originated in the long-lasting outbreak of cat-transmitted sporotrichosis in Rio de Janeiro, which differed from the haplotype circulating in the Rio Grande do Sul epidemic.ConclusionsThe fast spread of sporotrichosis in a short period of time highlights the potential for outbreaks and suggests that the mycosis may affect an urban population with a high concentration of susceptible felines. The feline sporotrichosis epidemic shows no signs of slowing, and this epidemiological pattern may require specific public health strategies to control future outbreaks.

Profile and temporal dynamics of the feline sporotrichosis epidemic in southern Brazil: A forecasting analysis

Abstract:A detailed clinical-epidemiological analysis of feline sporotrichosis was conducted, and 288 cases reported between the years 2007 and 2018 were analyzed. The studied cases primarily involved mongrel cats (240/260), males (212/282), and adults (121/200). The main objectives were to identify the risk factors, calculate the monthly incidence rates, and establish a predictive model using the seasonal autoregressive integrated moving average (SARIMA) approach. The statistical analysis revealed significant associations (p < 0.05) between prolonged lesion evolution times and factors such as respiratory signs, prior treatments, and lesion contact. Empirical treatment was identified as a significant risk factor for disease progression. Moreover, the number of cases demonstrated an increasing trend over the study period, with annual peaks noted in disease incidence. The SARIMA model proved to be an effective tool for forecasting the incidence of sporotrichosis, offering robust support for epidemiological surveillance and facilitating targeted public health interventions in endemic regions. The predictive accuracy of the developed model underscored its utility in enhancing disease monitoring and supporting proactive health measures for the effective management of sporotrichosis.
Emerging Cases of Cat-Transmitted Sporotrichosis Driven by Sporothrix brasiliensis in Northeast Brazil

Abstract:Cat-transmitted sporotrichosis is caused by the emerging fungal pathogen Sporothrix brasiliensis and constitutes a significant public health issue that affects people living in resource-poor urban centers in Brazil. The lack of knowledge about transmission dynamics makes it difficult to propose public health policies to contain the advance of sporotrichosis. We describe the recent emergence of 1,176 cases of sporotrichosis in cats (2016 to 2021) in the metropolitan region of Recife, Brazil, leading to significant zoonotic transmission and an overwhelming occurrence of S. brasiliensis as the etiological agent. Most cases were from cats in the cities of Olinda (408/1,176; 34.70%), Jaboatão dos Guararapes (332/1,176; 28.23%), and Recife (237/1,176; 20.15%). Molecular typing using amplified fragment length polymorphism (EcoRI-GA/MseI-AG) revealed low polymorphic information content ( PIC = 0.2499) and heterozygosity ( H = 0.2928), typical of an outbreak scenario. Dendrogram and multivariate cluster analysis revealed that isolates from Pernambuco are closely related to Rio de Janeiro isolates. We report a substantial occurrence of MAT1-2 idiomorphs in the metropolitan region of Recife (0:60 ratio; χ 2 = 60.000, P < 0.0001). The limited population differentiation and genetic diversity of the isolates from Pernambuco suggest a recent introduction, possibly via a founder effect, from the parental population in Rio de Janeiro. Our findings emphasize the critical importance of molecular surveillance of S. brasiliensis for outbreak response. A comprehensive one-health strategy is mandatory to control the spread of cat-transmitted sporotrichosis driven by S. brasiliensis , encompassing sanitary barriers, quick diagnosis, and treatment.

Abstract:Sporotrichosis is caused by fungi belonging to the genus Sporothrix, which saprophytically are found in plants and organic matter. However, cats are highly susceptible to contamination with fungal spores and, when they become sick, they can transmit it to other animals and to man. The objective of this study is to carry out a systematic review on the emergency, diagnosis, clinical symptoms, therapeutics, and control of zoonotic sporotrichosis. Published data covering the last 50 years using a combination of keywords were selected to answer the question: Why has the zoonotic sporotrichosis been a neglected disease up to now? A total of 135 studies were included in this review. The studies emphasize that in recent decades, Brazil has experienced an unprecedented zoonotic outbreak of sporotrichosis. Advances on the genus Sporothrix allowed one to associate thermotolerance, capacity for melanin synthesis, potential for adhesion to tissue macromolecules, ergosterol peroxide production, and expression of virulence proteins as tools for infection and invasion in S. brasiliensis, the main species involved, although cases with S. schenckii or S. lurei were also reported. Correct diagnosis, early treatment, basic educational measures that emphasize responsible ownership of animals and reproductive control programs for felines can contribute to the control of zoonosis.

Abstract:Sporotrichosis is a traumatic mycosis affecting the skin or subcutaneous tissues caused by Sporothrix dimorphic fungus. The fungal complex includes several pathogenic species, out of which S. brasiliensis and S. schenckii are predominant in Brazil. In Mato Grosso do Sul (MS) state, the first human and animal cases were reported in 2016 in Corumbá and Ladário cities. Accordingly, we present the first occurrences of feline sporotrichosis detected in the state capital Campo Grande, MS, by the Zoonoses Control Service (ZCS) of the Municipal Public Health Department. The study included four allochthonous cases of feline sporotrichosis originating from Corumbá, MS, attended by the ZCS. All four cats presented classical clinical signs of sporotrichosis, as ulcerative nodular cutaneous lesions. Three slides tested positive by direct microscopy and PCR, followed by Sanger sequencing confirmed Sporothrix brasiliensis in two samples. The initial suspicion and diagnosis of feline sporotrichosis at the ZCS highlights the importance of accurate surveillance of sporotrichosis in non-endemic areas to enhance the capacity to prevent, detect and respond to emerging diseases in Campo Grande.

Abstract:Sporotrichosis is a fungal anthropozoonosis that has become a major public health problem in tropical countries. With that in mind, this study analyzed the relationship between this disease and demographic, socioeconomic and public health issues in Belém, State of Pará, Brazil, from 2020 to 2022. This ecological and cross-sectional study used data from the Belém Zoonosis Control Center, the Brazilian Institute of Geography and Statistics and the Health Ministry. Descriptive and spatial analyses were performed employing significance statistical, kernel, buffer and Moran techniques. One hundred sporotrichosis cases in cats and 49 in humans were analyzed. The results showed that the individuals most affected were women (61.22 %), adults (87.76 %), with the cutaneous form (95.92 %), diagnosed histopathologically (38.78 %), still undergoing treatment (46.94 %) and that the form of contagion was through cat scratches or bites (73.47 %). The profile also showed quantitative significance of ignored data related to treatment (65.31 %) and cat presence at home (63.27 %). The disease had a non-homogeneous distribution with very high densities in Campina de Icoaraci, Águas Negras and Parque Guajará. Those neighborhoods presented a very low Living Conditions Index and precarious services and health centers. The spatial dependence between the environmental and socioeconomic studied variables evidenced the establishment of an active transmission circuit for sporotrichosis in peripheral areas of the city, related to health inequalities with an underlying possible epidemiological silence, suggesting the need for expanding One Health public policies, aiming the sustainable development.

Abstract:Over the past two decades, zoonotic sporotrichosis transmitted by naturally infected cats has become hyperendemic in Rio de Janeiro, Brazil. Sporothrix brasiliensis is the main agent involved. However, there are other forms of transmission of sporotrichosis. The aim of this study was to evaluate and associate the epidemiological, clinical and therapeutic data and the susceptibility of Sporothrix spp. to antifungal drugs in 43 non-zoonotic sporotrichosis cases. Forty-three clinical strains of Sporothrix were identified by partial sequencing of the calmodulin gene. An antifungal susceptibility test of amphotericin B, terbinafine, itraconazole, posaconazole and isavuconazole was performed according to the broth microdilution method. Most patients were male (55.8%). Regarding the source of infection, 21 patients (48.8%) reported trauma involving plants and/or contact with soil. Sporothrix brasiliensis was the predominant species (n = 39), followed by S. globosa (n = 3) and S. schenckii (n = 1). Sporothrix brasiliensis was associated with all the sources of infection, reinforcing previous data showing the presence of this species in environmental sources, as well as with all the clinical forms, including severe cases. One clinical strain of Sporothrix brasiliensis was classified as a non-wild-type strain for amphotericin B and another for itraconazole. S. schenckii was classified as non-WT for all the antifungals tested. In this context, it is important to emphasize that non-zoonotic sporotrichosis still occurs in the state of Rio de Janeiro, with S. brasiliensis as the main etiological agent, primarily associated with infections acquired after traumatic inoculation with plants and/or soil contact, followed by S. globosa and S. schenckii. In addition, non-WT strains were found, indicating the need to monitor the antifungal susceptibility profile of these species. It is crucial to investigate other natural sources of S. brasiliensis to better understand this fungal pathogen and its environment and host cycle.

Abstract:Background Sporotrichosis is a neglected tropical disease and the most common subcutaneous mycosis, mainly caused by Sporothrix species, particularly S. brasiliensis, S. schenckii and S. globosa, which exhibit varying biological behaviours and virulence. The epidemic of sporotrichosis in Brazil, initiated in Rio de Janeiro in the late 1990s, rapidly spread to other states, including Amazonas in 2021. This study aimed to identify the specific Sporothrix species responsible for the human sporotrichosis outbreak in the Brazilian Amazon. Methods A cross‐sectional study was conducted by enrolling clinically suspected cases of sporotrichosis attended at a reference dermatologic centre, in Manaus (Brazil). Biological material was collected from their skin lesions for culture (Mycosel) and for species identification (qPCR). Results Sporothrix cultures were obtained from 150 cases. Sporotrichosis predominantly affected females (67.3%), aged 44.5 years on average, with lymphocutaneous lesions (72.7%). Sporothrix brasiliensis was identified in 89.3% of patients. Up to 83.3% of these patients reported contact with cats previously to the skin lesion, and the time‐spatial progression of the human cases followed the notification of cases in cats, in the metropolitan region of Manaus. Conclusion Sporothrix brasiliensis is the dominant species in the zoonotic outbreak of human sporotrichosis in the Brazilian Amazon, with cats identified as the primary vectors. Effective sanitary control measures, education and responsible pet ownership are crucial to mitigating zoonotic sporotrichosis' impact in Brazil and preventing its spread to neighbouring Latin American cities.

Abstract:Twenty-five years have passed since the initial observation of endemic zoonotic sporotrichosis in Rio de Janeiro, Brazil. Since then, this disease has spread throughout South America. Accompanying the emergence of this mycosis, some progress has been made, including the expansion of a research network in this field and higher visibility of sporotrichosis within government authorities and funding agencies. However, there are still some challenges to curbing the expansion of this disease in the coming years. These include the development of rapid and accurate diagnostic tests, new antifungal drugs, particularly for the treatment of extracutaneous manifestations of sporotrichosis, and more comprehensive care for cats with sporotrichosis. Including these actions in the sporotrichosis research agenda is required so as to change the development of this disease in the years to come.

Abstract:Background Zoonotic sporotrichosis caused by Sporothrix brasiliensis has become the main subcutaneous mycosis in Brazil. Minas Gerais (MG) is located in southeast Brazil and since 2015 has experienced an epidemic of zoonotic sporotrichosis. Objectives This study aimed to reconstruct the epidemiological scenario of sporotrichosis from S. brasiliensis in recent epizooty in the Metropolitan Region of Belo Horizonte (MRBH), MG. Methods A total of 95 Sporothrix spp. isolates (Sporothirx brasiliensis n = 74, S. schenckii n = 11 and S. globosa n = 10) were subjected to Amplified Fragment Length Polymorphism (AFLP) genotyping and mating‐type analysis to determine genetic diversity and population structure. Of these, 46 S. brasiliensis isolates were recovered from animals (cats n = 41 and dogs n = 5) from MRBH. Results Our study describes the high interspecific differentiation power of AFLP‐based genotyping between the main phylogenetic Sporothrix groups. S. brasiliensis presents high genetic variability and pronounced population structure with geographically focused outbreaks in Brazil. The genetic groups include older genotypes from the prolonged epidemic in Southeast (Rio de Janeiro and São Paulo), South (Rio Grande do Sul), Northeast (Pernambuco) and new genotypes from the MRBH. Furthermore, we provide evidence of heterothallism mating strategy in pathogenic Sporothrix species. Genotypes originating in Rio de Janeiro and Pernambuco carry the predominant MAT1‐2 idiomorph as opposed to genotypes from Rio Grande do Sul, which have the MAT1‐1 idiomorph. We observed an overwhelming occurrence of MAT1‐1 among MRBH isolates. Conclusion Our study provides clear evidence of the predominance of a genetic group profile circulating in animals in Minas Gerais, independent of that disseminated from Rio de Janeiro. Our data can help us understand the genetic population processes that drive the evolution of this fungus in Minas Gerais and contribute to future mitigation actions for this ongoing epidemic.

Abstract:Zoonotic sporotrichosis caused by the fungus Sporothrix brasiliensis is usually severe in cats. This study investigated the associations between clinical features, fungal load, coinfections, histological skin changes, and response to itraconazole in cats with sporotrichosis caused by S. brasiliensis. Fifty-two cats with skin lesions and a definitive diagnosis of sporotrichosis were treated with itraconazole for a maximum period of 36 weeks. The animals were submitted to clinical examination and two subsequent collections of samples from the same skin lesion for fungal diagnosis and histopathology, as well as serology for feline immunodeficiency (FIV) and leukaemia (FeLV) viruses. Thirty-seven (71%) cats were clinically cured. Nasal mucosa lesions and respiratory signs were associated with treatment failure. Cats coinfected with FIV/FeLV (n = 12) had a lower neutrophil count in the lesion. A high fungal load in skin lesions was linked to young age and treatment failure, as well as to a longer time of wound healing, poorly formed granulomas and fewer neutrophils, macrophages and lymphocytes in these lesions. These results indicate that itraconazole is effective, but nasal mucosal involvement, respiratory signs and high fungal loads in skin lesions are predictors of treatment failure that will assist in the development of better treatment protocols for cats.

During the study period, 6,827 histopathological examinations (4,090 necropsies and 2,737 biopsies) of companion animals were reviewed, and fungal infections were diagnosed in 54 cases. Of these, 32 cases were diagnosed in cats and 22 in dogs. Sporotrichosis was the most prevalent mycosis (19/54), followed by dermatophytosis (9/54), cryptococcosis (8/54), candidiasis (7/54), and phaeohyphomycosis (4/54). Other diseases had two cases each, including aspergillosis, mucormycosis, and histoplasmosis, whereas coccidioidomycosis occurred once. Mycoses were diagnosed through microscopic lesions associated with the histomorphological characteristics of the agent, as evidenced by routine and special histochemical stains. They were occasionally confirmed by immunohistochemistry and microbiological cultivation. This retrospective study showed that despite being uncommon, fungal diseases are important causes of death in dogs and cats in the region, with cats being the most affected species and sporotrichosis being the most prevalent mycosis. Clinical signs were correlated with the portal of entry of the infection associated with localized or disseminated superficial and deep skin lesions or affecting parenchymal organs, especially in systemic infections. Microscopic lesions were mainly characterized by pyogranulomatous inflammation associated with fungal structures. In dogs, mycoses occurred mainly owing to an immunological impairment secondary to canine distemper virus co-infection.
Toxoplasma gondii in domiciled dogs and cats in urban areas of Brazil: risk factors and spatial distribution

Abstract:Toxoplasma gondii is a highly prevalent zoonotic parasite in Brazil capable of infecting mammals and birds. The increase in the urban populations of pets and the narrowing of the human–animal relationship can facilitate the transmission of important public health zoonoses, such as toxoplasmosis. This study aimed to evaluate the frequency and spatial distribution of T. gondii infection and its risk factors in domiciled dogs and cats attended at the Jorge Vaitsman Institute, Rio de Janeiro. Serum samples from 400 dogs and 272 cats were evaluated by an indirect fluorescent antibody test (IFAT) for IgG anti- T. gondii antibodies. Epidemiological questionnaires were used to interview the animals’ owners to identify risk factors for infection. Of the total, 34% (136/400) of dogs and 8.1% (22/272) of cats had anti- T. gondii antibodies. Breed (OR: 2.10–95%, CI 1.27–3.46) was a risk factor for dogs, while sex (OR: 3.40–95%, CI 1.10–10.52) and homemade food consumption (OR: 8.49–95%, CI 2.48–29.05) were risk factors for cats. Offal consumption was considered a risk factor for both species evaluated (OR: 2.74–95%, CI 1.38–5.43 for dogs; OR: 7.66–95%, CI 1.24–47.29 for cats). The spatial analysis showed that T. gondii seropositive animals were widely distributed in the metropolitan region of Rio de Janeiro state, with a concentration observed mainly in the west and north zones of Rio de Janeiro city. The results emphasize the importance of adopting prophylactic measures to control T. gondii transmission in domiciled dogs and cats in Rio de Janeiro, contributing positively to public health.

Abstract:Background: Sporotrichosis is a zoonotic disease caused by a dimorphic fungi of the Sporothrix schenckii complex. It is an emerging zoonosis with worldwide distribution, thus of great importance to public health. The infection occurs from traumatic inoculation of the fungus in the human skin from contaminated plants and soils and through bites or scratches of infected animals. The occurrence of sporotrichosis has been related to zoonotic transmission, especially by domestic felines. This work aims to report the successful use of itraconazole as monotherapy in a case of localized feline sporotrichosis and highlight the effectiveness of cytology in its diagnosis.Case: A 1-year-and-4-month-old spayed female cat undefined breed, weighing 3.1 kg, was referred to the Veterinary Medical Teaching Hospital of the Universidade Estadual de Maringá (UEM), city of Umuarama, presenting a serosanguinous ulcerative lesion located in the left periocular region The clinical picture of the patient progressed over 3 months. Treatments with antibiotic therapy, corticosteroids, and surgical procedures were conducted, without clinical improvement. A new lesion in the distal portion of the thoracic limb emerged, proving that the disease remained in progression. Hematological exams were performed, among them hemogram, renal and hepatic biochemical analyses, SNAPS to identify the feline immunodeficiency virus feline leukemia virus (FIV/FeLV), and cytology of the lesion through imprints. The hematological results were all within the normal standards for the species. Cytology showed a large amount of oval and fusiform structures phagocyted by polymorphonuclear cells, free at the bottom of the slide, compatible with Sporothrix spp. The treatment administered was itraconazole (100 mg/cat), orally administered every 24 h, cefovecin sodium 8 mg/kg, subcutaneous, single dose, topical use of antifungal ointment based on ketoconazole, twice a day. The animal underwent periodic physical and hematological evaluations throughout the treatment period, without significant changes. Complete remission of the lesion was observed after 25 weeks of treatment.Discussion: Cutaneous lesions caused by Sporothrix spp. are mainly located in the head, specifically in the nasal plane, pinna, and periocular regions, presenting ulcerative characteristics and exudate. The dissemination of the fungus through the animal's body may occur through autoinoculation while the feline scratches or licks itself. The cytology presents high sensitivity for diagnosing feline sporotrichosis due to the high fungal load found in the cutaneous lesions. In addition, it is a technique easy to perfom and presents immediate results, favoring an early beginning of the therapy. Itraconazole is considered the drug of choice for the treatment of sporotrichosis because of its efficacy and safety compared to that of other antifungal drugs. The average time of treatment is 4 to 9 months in cats, and it must be maintained for another month after complete remission of the clinical signs to prevent the reactivation of the lesions. FIV and FeLV are not predominant factors for the development of the disease. The use of itraconazole as monotherapy proved to be effective, with no side effects throughout the treatment. Cytology was satisfactory as a diagnostic method and allowed immediate initiation of therapy. Abstract:Haemolytic complement (C) in human sera is readily consumed by most of the common inhalant allergens. The reactivity of C among individual sera differs considerably and is a characteristic parameter for the condition of atopy. The phenomenon is not mediated by specific IgE or IgG4 antibodies. Whereas in groups of patilues (by RAST) are negative in about 50 percent of the cases, this is not so with regard to C-reactivity. In somes cases, especially in allergy to guinea pigs, correlation with the clinical history is better with IgG4 than with IgE antibodies. In other allergies, there is a close correlation between Ige and IgG4 antibodoes.

Abstract:The involvement of adenosine 3',5'-cyclic monophosphate (cAMP) in the stimulation of ventricular protein synthesis by aortic hypertension or adrenergic agonists in the adult rat heart was investigated. In either the retrogradely or anterogradely perfused heart, aortic hypertension increased protein synthesis rates by up to 19%. However, no changes in cAMP concentrations or in cAMP-dependent protein kinase activity ratios could be detected either at early (< 5 min) or late (90 min) time points. Although isoproterenol, 3-isobutyl-1-methylxanthine, or forskolin raised cAMP concentrations (by up to 4.5-fold) and cAMP-dependent protein kinase ratios (by up to 4-fold), protein synthesis rates were not increased; however, under some perfusion conditions, glucagon did stimulate protein synthesis by 25%. Epinephrine stimulated protein synthesis by up to 32%, an effect that was not prevented by propranolol. Phenylephrine also stimulated protein synthesis, an effect that was prevented by prazosin but was unaffected by yohimbine. These findings implicate the alpha 1-adrenoceptor in the regulation of cardiac protein synthesis. Because changes in adenine nucleotide concentrations were similar in hearts perfused with epinephrine or with the agents that raised cAMP, it is unlikely that adenine nucleotide depletion is responsible for the failure to observe effects of the latter group of agents on protein synthesis. Although isoproterenol or forskolin raised cAMP concentrations in isolated ventricular cardiomyocytes where ATP depletion was minimal, neither stimulated protein synthesis. alpha 1-Adrenergic agonists stimulate phosphoinositide hydrolysis in the heart (Brown, J. H., I. L. Buxton, and L. L. Brunton. Circ. Res. 57:532-537, 1985). Aortic hypertension doubled the rate of phosphoinositide hydrolysis in the perfused heart. We suggest that the phosphoinositide-linked signal transduction pathway is more likely to be involved in stimulation of cardiac protein synthesis by hypertension or adrenergic agonism than the adenylyl cyclase/cAMP-linked pathway.
