Abstract:Abstract Aims To understand the benefits and challenges of shift work, and the coping strategies used by nurses, midwives and paramedics to manage the impact of shift work on sleep and fatigue from shift work. Design A single case study with embedded units. Methods Twenty‐seven participants were interviewed exploring their shift work experiences, coping strategies used to improve sleep, and what their recommendations are for improving shift work management. Interviews were completed between November and December 2022. Results Participants enjoyed the lifestyle, flexibility and financial rewards offered by working shift work. However, fatigue and sleep deprivation undermined these benefits, as it impacted their ability to enjoy social and family events. There were also concerns of long‐term health consequences of shift work and delivery of care. Changes to rostering practices and sleep and shift work education were common recommendations. Conclusion This study provides insights on how healthcare professionals manage sleep and fatigue due to shift work and the inadequate support. There is absence of adequate policies, processes and training at an organizational, academic and personal level on how to best manage sleep and fatigue when working shift work. Future research is needed to explore how to equip healthcare shift workers with the skills to successfully manage their schedules to mitigate the negative impact that poor sleep and fatigue has on the health and safety of themselves and their patients. Implications for the Profession and/or Patient Care Understanding the specific challenges of shift work and how workers manage their shift work schedules is critical for improving the health and safety of themselves and their patients. This study identified that there is insufficient training regarding sleep and shift work management strategies, potentially leading to occupational health and safety concerns. Further education and training to equip staff with the necessary information, training and guidance to staff on how to reduce fatigue risk is required. Patient or Public Contribution This study involved healthcare shift workers in semi‐structured interviews. Data gathered from a previous survey that participants were involved in helped shape the interview topics and the study design.

Abstract:BACKGROUND & OBJECTIVES The polymeric immunoglobulin receptor (pIgR) is produced by mucosal epithelial cells and plays a crucial role in mucosal immunity. At the basolateral surface of mucosal cells, the pIgR binds predominantly polymeric immunoglobulins, such as dimeric IgA and polymeric IgA (pIgA) and mediates their transport across the polarized cells. This results in apical release of secretory component (SC), either free or bound covalently to IgA, forming secretory IgA (SIgA). The choline-binding protein (Cbp) SpsA, also called PspC and CbpA, has been shown to interact with the pIgR. A hexapeptide motif in SpsA was identified as the minimal binding motif required for binding specifically to pIgR and SC. The present study was carried out to show that the hexapeptide motif in SpsA is crucial for the interaction of pneumococci and pIgR-expressing cells. METHODS Streptococcus pneumoniae were cultured to mid-log phase. Calu-3 cells and MDCK epithelial cells, stably transfected with the hpIgR cDNA in pCB6 were used in in vitro infection experiments. Pneumococcal adherence to and invasion of epithelial cells were assayed. RESULTS By the use of the N-terminal domain of SpsA and SpsA(201), which exhibits a single amino acid substitution in the pIgR-binding motif, in vitro assays indicated the association of the identified hexapeptide motif, located between amino acid 198 and 203 in SpsA, with pneumococcal adherence to and invasion of hpIgR-expressing cells. INTERPRETATION & CONCLUSION The present findings demonstrated not only the crucial role of the hexapeptide of SpsA, not only for the SpsA-pIgR interaction, but also for adherence and invasion of hpIgR-expressing cells.
Free-Living Sleep, Food Intake, and Physical Activity in Night and Morning Shift Workers

Abstract:Objective: Shift work is associated with risk for adverse health outcomes including cardiovascular disease, type 2 diabetes, cancer, and obesity. Short sleep duration combined with disruptions to the circadian system may alter factors involved with the behavioral regulation of energy intake and expenditure. We aimed to determine how shift work affects sleep, food intake, and physical activity.Methods: This was a field-based observational study using objective assessments of sleep and physical activity and a 24-hour dietary recall in shift workers. Day (n = 12) and night (n = 12) hospital shift workers (nurses and technicians) who were women had their free-living sleep and physical activity tracked via accelerometry, and completed a computer-assisted 24-hour food recall, during a series of work shifts.Results: Compared to day workers, night workers had significantly shorter sleep duration and reported more premature awakenings and feeling less refreshed upon awakening. Daily self-reported energy and macronutrient intakes were not different between groups, although the night shift workers reported a significantly longer total daily eating duration window than day workers. Objectively recorded physical activity levels were not different between groups.Conclusions: The present findings confirm that sleep is disturbed in women night workers, while there are relatively less effects on objectively recorded physical activity and self-reported food intake. We also observed a prolonged daily eating duration in night vs. day workers. These observations can help inform the design of novel behavioral interventions, including, potentially, time restricted feeding approaches (e.g., by limiting daily eating episodes to within a 10–12 h window), to optimize weight management in shift workers.

Effects of Sleep Deprivation on the Cognitive Performance of Nurses Working in Shift

Abstract:INTRODUCTION: Sleep deprivation and altered circadian rhythm affects the cognitive performance of an individual. Quality of sleep is compromised in those who are frequently involved in extended working hours and shift work which is found to be more common among nurses. Cognitive impairment leads to fatigability, decline in attention and efficiency in their workplace which puts their health and patients' health at risk.AIM: To find out the prevalence of sleep deprivation and its impact on cognition among shift working nurses.MATERIALS AND METHODS: Sleep deprivation among 97 female and three male healthy nurses of age 20-50 years was assessed by Epworth sleepiness scale (ESS). Cognition was assessed by Montreal Cognitive Assessment (MoCA) questionnaire. Mobile applications were used to test their vigilance, reaction time, photographic memory and numerical cognition. The above said parameters were assessed during end of day shift and 3-4 days after start of night shift.RESULTS: Poor sleep quality was observed among 69% of shift working nurses according to ESS scores. The cognitive performance was analysed using Wilcoxon signed rank test. The MoCA score was found to be lesser among 66% of nurses during night (25.72) than day (26.81). During the night, 32% made more mathematical errors. It was also found that, 71%, 83% and 68% of the nurses scored lesser during night in the Stroop's colour test, vigilance test and memory tests respectively. Thus, impairment in cognitive performance was statistically significant (p<0.001) among shift working nurses.CONCLUSION: Cognitive performance was found to be impaired among shift working nurses, due to poor sleep quality and decreased alertness during wake state. Thus, shift work poses significant cognitive risks in work performance of nurses.
Shift work and hospital employees: A descriptive multi-site study

Abstract:Background: Hospitals staff with 12-hour and other shift work patterns to account for daily and seasonal workload requirements. Research in healthcare and industries requiring shift work suggests there may be negative health consequences to the workers related to fatigue and other factors. Due to the 24/7 nature of healthcare, it is important to understand the impact of shift work on employees. Objectives: The objectives of this study were to investigate how shift work impacts satisfaction with time for social and home activities, based on the Social and Domestic Survey, and chronic fatigue. In addition, an exploration of drowsy driving was undertaken. Design and setting: This study was a multi-site, descriptive survey study, conducted in 4 acute care hospitals in one metropolitan area in the central United States. Participants: Participants included healthcare workers in five 24/7 departments: nursing, respiratory therapy, laboratory, radiology and pharmacy. Methods: Measures included demographics, work characteristics, and scales including the: Social and Domestic Survey, Circadian Type Inventory, and Chronic Fatigue Scale along with data regarding drowsy driving and automobile accidents/near misses. Data were analyzed using descriptive statistics for sample characteristics. An exploratory factor analysis was conducted on the Social and Domestic Survey. Group differences were tested using Mann-Whitney U tests and regression analyses. Results: The sample included 1563 subjects (24% RR). The mean age was 40 years. Nursing comprised 77% of the sample, 88% were female, 85% were in a direct patient care role, 67% worked day shift, and 49% worked 12-hour shifts. Pay was important for working night shift, but home life was important for day shift. Night shift (p<0.001) and 12-hour shift workers (p<0.001) had higher chronic fatigue. However, after controlling for other factors, working night shift remained a significant predictor of chronic fatigue and lower satisfaction with time for daily tasks and family/social life, whereas working 12-hour shifts predicted higher satisfaction with daily tasks and periodic life activities. Thirty-five percent of workers reported altering their driving behavior at least half the time due to drowsy driving and 19% had an automobile accident or near miss due to drowsy driving. Conclusions: Findings suggest that night and long shifts can have negative effects on fatigue, family, and social life. Drowsy driving and accidents/near misses frequently occur due to drowsy driving. Leaders should continue to study the 24/7 work environment and test measures to improve the safety of shift workers.

Abstract:Objectives The aim of this observational study was to examine sleep obtained between consecutive night shifts from shift workers in their natural environment. The goal was to identify the various sleep strategies and the timing, duration, regularity, and quality of sleep associated with the strategies. Methods Participants (N = 33, 23 women, aged 40 ± 15 years) reported their sleep information in daily diaries over 2 weeks while working at least one series of consecutive night shifts. Sleep timing, duration, quality, and regularity were calculated for each sleep episode between consecutive night shifts. Results Based on the reported sleep behavior, shift workers were categorized as either morning, delayed, split- or mixed sleepers. We found significant differences between the groups in timing of sleep, feeling refreshed, and regularity of sleep between consecutive night shifts, whereas duration and subjective soundness of sleep did not show significant differences. Conclusions In this sample, four sleep strategies were observed between consecutive night shifts in actual shift workers. These observations may help design future interventions to improve sleep that are individualized to the worker.

Non-Pharmacological Interventions to Improve Chronic Disease Risk Factors and Sleep in Shift Workers: A Systematic Review and Meta-Analysis

Abstract:Shift work is associated with adverse chronic health outcomes. Addressing chronic disease risk factors including biomedical risk factors, behavioural risk factors, as well as sleep and perceived health status, affords an opportunity to improve health outcomes in shift workers. The present study aimed to conduct a systematic review, qualitative synthesis, and meta-analysis of non-pharmacological interventions targeting chronic disease risk factors, including sleep, in shift workers. A total of 8465 records were retrieved; 65 publications were eligible for inclusion in qualitative analysis. Random-effects meta-analysis were conducted for eight eligible health outcomes, including a total of thirty-nine studies. Interventions resulted in increased objective sleep duration (Hedges’ g = 0.73; CI: 0.36, 1.10, k = 16), improved objective sleep efficiency (Hedges’ g = 0.48; CI: 0.20, 0.76, k = 10) and a small increase in both subjective sleep duration (Hedges’ g = 0.11; CI: −0.04, 0.27, k = 19) and sleep quality (Hedges’ g = 0.11; CI: −0.11, 0.33, k = 21). Interventions also improved perceived health status (Hedges’ g = 0.20; CI: −0.05, 0.46, k = 8), decreased systolic (Hedges’ g = 0.26; CI: −0.54, 0.02, k = 7) and diastolic (Hedges’ g = 0.06; CI: −0.23, 0.36, k = 7) blood pressure, and reduced body mass index (Hedges’ g = −0.04; CI: −0.37, 0.29, k = 9). The current study suggests interventions may improve chronic disease risk factors and sleep in shift workers; however, this could only be objectively assessed for a limited number of risk factor endpoints. Future interventions could explore the impact of non-pharmacological interventions on a broader range of chronic disease risk factors to better characterise targets for improved health outcomes in shift workers.

Abstract:Night shift workers are often associated with circadian misalignment and physical discomfort, which may lead to burnout and decreased work performance. Moreover, the irregular work hours can lead to significant negative health outcomes such as poor eating habits, smoking, and being sedentary more often. This paper uses commercial wearable sensors to explore correlates and differences in the level of physical activity, sleep, and circadian misalignment indicators among day shift nurses and night shift nurses. We identify which self-reported assessments of affect, life satisfaction, and sleep quality, are associated with physiological and behavioral signals captured by wearable sensors. The results using data collected from 113 nurses in a large hospital setting, over a period of 10 weeks, indicate that night shift nurses are more sedentary, and report lower levels of life satisfaction than day-shift nurses. Moreover, night shift nurses report poorer sleep quality, which may be correlated with challenges in their attempts to fall asleep on off-days.

Abstract:Society and Mental Health, Ahead of Print. Shift work is an integral part of living in a 24-hour society. However, shift work can disrupt circadian rhythms, negatively impacting health. Guided by the Stress Process Model (SPM), this study examines the association between shift work and depressive symptoms and investigates whether sleep health (duration, quality, and latency) mediates this relationship among midlife adults. Utilizing data from the National Longitudinal Survey of Youth 1979 cohort (N = 6,372), findings show that working evening, night, and irregular shifts is associated with increased depressive symptoms. The results also show that part of the association between shift work and depressive symptoms among night and irregular shift workers, is indirect, operating through short sleep during the week and on the weekend. Although shift work can negatively affect mental health, getting more restorative sleep may mitigate part of the harmful mental health consequences of non-standard work schedules.

Abstract:Abstract Young adults are vulnerable to the consequences of disturbed sleep from insufficient sleep, shift work and sleep disorders. The impact of these causes of disturbed sleep on perceived work performance and quality of life (QOL) in young adults is unclear. This study sought to investigate these associations in a community sample of 22-year-old adults. Participants studied (n=565, 53% female) were a subset of the Raine Study. All were employed and provided information on sleep disorders (obstructive sleep apnoea by polysomnography and insomnia and restless legs syndrome symptoms by validated questionnaires), shift work status, habitual sleep duration, QOL (Short Form (SF) 12) and perceived work performance (categorical response: perform better, same or worse than most workers). Multinomial and linear regression models adjusted for sex, comorbidities and work hours were used to examine the associations between disturbed sleep with work performance and QOL, respectively. In total 25% of participants had a sleep disorder, 29% were shift workers, and 31% had insufficient sleep (<6 hours). Presence of a sleep disorder was associated with poorer perceived work performance (OR, 2.27; 95%CI, 1.3 – 4.1). Sleep disorders were also associated with significantly lower SF-12 mental (β, -4.19; 95%CI, -5.92 to -2.45, p<0.001) and physical (β, -1.56; 95%CI, -2.70 to -0.42, p<0.008) component scores. Shift work and insufficient sleep were not associated with either perceived work performance or QOL. Identifying and treating sleep disorders in young adults may be important given its negative association with perceived work performance and QOL.
Comparison of Sleep, Perceived Health, and Job Stress According to Symptoms of Shift Work Disorder in Shift Nurses

Abstract:Objectives: This study aimed to investigate the prevalence of symptoms of shift work disorder (SSWD) and its related factors in rapidly rotating three-shift nurses. Methods: We enrolled 344 nurses (mean age, 28.7 years) without prior history of sleep disturbance before starting shift work in a university-affiliated hospital. SSWD were defined using self-reported sleep questionnaires (insomnia severity index >14 and/or Epworth Sleepiness Scale ≥10) and without any sleep problem before shift work. Sleep pattern in each of the three shifts was measured using the Munich Chronotype Questionnaire for Shift-Workers. Mood (anxiety, depression) and job stress were also measured using self-reported questionnaires. Results: Our results showed that 62.2% of the participants presented SSWD. Compared to the non-SWD group, the SSWD group showed worse sleep hygiene, lower total sleep time (TST) during workdays, higher anxiety and depressive mood, and higher job stress. In the SSWD group, the evening chronotype individuals presented the shortest TST during day shifts, while the morning chronotype individuals presented the shortest TST during evening shifts. Conclusions: SSWD is highly prevalent in fast rotating three-shift nurses. This study suggests that intensive education on sleep hygiene and appropriate scheduling of shift work while considering individual chronotypes may improve sleep patterns and sleep quality of shift workers and minimize SSWD.

Abstract:Research has shown that shiftworkers experience poor sleep and high levels of fatigue. Although considerable research has been performed on fatigue within many shift-work occupations, very little has been done with emergency physicians (EPs). This qualitative study was conducted with the goal of gaining insight into EPs’ perceptions of fatigue at work. Twenty EPs from an academic medical center participated in virtual interviews, with nine open-ended questions asked in a semi-structured interview format. Twelve common topics with four main themes emerged from the interviews. Three of these common themes included sources of fatigue (including both work- and home-related sources), consequences of fatigue (including impacts on individuals and performance), and prevention and mitigation strategies to cope with fatigue. The fourth main theme was the belief in the inevitability of fatigue due to high cognitive load, emotionally taxing work experiences, work unpredictability, and the 24/7 shift-work nature of emergency medicine. EPs’ experiences with fatigue are consistent with but extend those of other types of shiftworkers. Our findings suggest that EPs tend to incorporate the inevitability of fatigue at work into their identity as EPs and experience a sense of learned helplessness as a result, suggesting areas for future interventions.
Napping to Reduce Sleepiness, Fatigue and Improve Reasoning for Nursing Shift Workers

Abstract:Shift work outside the normal working hours of 6 a.m. to 6 p.m. is a requirement in acute and chronic care facilities that provide nursing care on a 24-hour basis. Slightly more than 30% of full-time health care workers are shift workers. Shift work sleep disorder, defined as a primary complaint of insomnia and excessive sleepiness, has been diagnosed in almost ten % of shift workers. The excessive sleepiness and fatigue nursing shift workers experience often lead to performance impairment and diminished attentiveness that may affect job productivity and patient safety. Napping is the most natural countermeasure against sleepiness. This quasiexperimental study was performed to detemine whether a 20 minute nap during the niight shift could reduce sleepiness and fatigue and improve cognitive functioning among nursing shift workers in an acute care setting. Ten study participants were randomly assigned to participate in both the control (no nap) and the intervention (nap) within the two week study. Self-testing of fatigue and sleepiness, and a timed reaction test programmed into a personal digital assistant (PDA) were performed throughout the two shifts. Results showed a trend toward a reduction in sleepiness and fatigue were reduced when nursing staff took a 20 minute nap during shift work, that was sustained through the remainder of the shift. The Advanced Practice Nurse is critical to advancing napping for shift workers because she/he can interpret, evaluate, and participate in research, create evidence-based policy, and collaborate with key stakeholders.
The role of sleep hygiene in the risk of Shift Work Disorder in nurses

Abstract:Abstract A high proportion (20%–30%) of shift workers experience Shift Work Disorder (SWD), characterized by chronic sleepiness and/or insomnia associated with work schedules. The reasons for individual variation in shift work tolerance are not well understood, however. The aim of this study was to identify individual factors that contribute to the risk of SWD. Nurses (n = 202) were categorized as low or high risk of SWD based on the Shift Work Disorder Questionnaire. Participants provided demographic and lifestyle information and completed the Sleep Hygiene Index (SHI) and Morningness–Eveningness Questionnaire (MEQ). High risk of SWD was associated with poorer sleep hygiene (SHI, 35.41 ± 6.19 vs. 31.49 ± 7.08, p < .0001) and greater eveningness (MEQ, 34.73 ± 6.13 vs. 37.49 ± 6.45, p = .005) compared to low risk. No other factors, including body mass index, marital status, having children, or caffeine or alcohol intake were significant. Logistic regression showed that SHI was the most significant contributing factor to SWD risk (odds ratio [OR] = 1.09, 95% confidence interval [CI] = 1.04 to 1.14). Standardized odds ratio further revealed that with every unit increase on the SHI score, the odds of being at high risk of SWD increased by 80% (OR = 1.84). Most individuals at high risk of SWD reported “always” or “frequently” going to bed at different times (79%) and waking at different times (83%; compared to 58%, p = .017, and 61%, p = .002, respectively for the low-risk group), as well as going to bed stressed/angry (67% vs. 41%, p < .0001) and/or planning/worrying in bed (54% vs. 22%, p < .0001). Interventions aimed at improving sleep hygiene practices and psychological health of shift workers may help reduce the risk of SWD.

Abstract:Fatigue is a critical issue for nurses that may lead to medical errors, degradation in performance, decreased mental acuity, and social problems. Poor sleep quality is also a contributing factor in fatigue that nurses experience. Therefore, the purpose of this study was to examine the differences in perceptions of fatigue between night-shift and day-shift nurses, as measured by scores on the Brief Fatigue Inventory (BFI) and to examine differences in sleep quality between the two groups, as measured by responses on the Pittsburgh Sleep Quality Index (PSQI). Univariate analysis of variance showed significant differences between the two groups on the BFI, with the night-shift cohort reporting higher mean scores on the BFI. Significant differences were also found between the two groups on PSQI mean scores, with the night-shift cohort reporting higher mean scores. Findings from this study suggest that night-shift nurses, compared with day-shift nurses, perceived a much higher level of fatigue and had poorer sleep quality. Further research is needed to evaluate interventions that might decrease fatigue in nurses and improve their sleep quality.

Abstract:AIM To evaluate the effect of the application of plasma rich in growth factors (PRGF)-Endoret to the remaining intact bundle in partial anterior cruciate ligament (ACL) tears. METHODS A retrospective review of the rate of return to play in football players treated with the application of PRGF-Endoret in the remaining intact bundle in partial ACL injuries that underwent surgery for knee instability. Patients with knee instability requiring revision surgery for remnant ACL were selected. PRGF was applied in the wider part of posterolateral bundle and the time it took patients to return to their full sporting activities at the same level before the injury was evaluated. RESULTS A total of 19 patients were reviewed. Three had a Tegner activity level of 10 and the remaining 16 level 9. The time between the injury and the time of surgery was 5.78 wk (SD 1.57). In total, 81.75% (16/19) returned to the same pre-injury level of sport activity (Tegner 9-10). 17 males and 2 females were treated. The rate of associated injury was 68.42% meniscal lesions and 26.31% cartilage lesions. The KT-1000 values were normalized in all operated cases. One patient was not able to return to sport due to the extent of their cartilage lesions. The 15 patients with Tegner activity level 9 returned to play at an average of 16.20 wk (SD 1.44) while the 3 patients with Tegner activity level 10 did so in 12.33 wk (SD 1.11). CONCLUSION With one remaining intact bundle the application of PRGF-Endoret in instability cases due to partial ACL tear showed high return to sport rates at pre- injury level in professional football players.

Abstract:<h2 class="section-title u-h3 u-margin-l-top u-margin-xs-bottom">Summary</h2><p>In response to demand for the '24/7' service availability, shift work has become common. Given their non-traditional working hours, shift workers sleep at non-traditional times, with significant research undertaken to understand shift worker sleep. However, sleep hygiene in shift workers has been paid little research attention. To investigate shift worker engagement with sleep hygiene, a systematic review using the databases Sage, ScienceDirect, and Scopus was undertaken. The search terms utilised were: shift work, shiftwork, shift-work, sleep hygiene, sleep routine, and sleep habit. Sixteen studies were included for review. Findings show that shift workers frequently report caffeine consumption and daytime napping, in line with best-practice fatigue-management strategies, but contrary to existing sleep hygiene recommendations. Shift workers also altered their bedroom environment to optimise sleep. Diet, exercise, alcohol and nicotine consumption were investigated minimally from a sleep hygiene perspective. Given that shift workers are engaging in practices in-line with current fatigue-management strategies, but contrary to sleep hygiene recommendations, further research is required. Specifically, assessment of the applicability of current sleep hygiene guidelines to shift workers (particularly caffeine and napping recommendations) is required, in addition to the development of shift work-specific sleep hygiene guidelines and interventions for this sleep-vulnerable population.</p>

Abstract:Abstract Introduction Night shift work is associated with compromised cognitive function, and with chronic exposure, may place shift workers at elevated risk for dementia. Given that former night shift workers experienced prior long-term sleep and circadian rhythm disruption, their sleep in retirement—after returning to a daytime schedule—may be particularly salient for cognitive function. This study examined associations between multidimensional sleep health and neurocognitive function among retired night shift workers and retired day workers. Methods Participants (N = 61; mean age: 67.9 +/- 4.7 years; 61% females; 13% non-Hispanic White) were 31 RDW and 30 RNSW equated on age, sex, race/ethnicity, premorbid IQ, and years in retirement. Multidimensional sleep health was quantified using wrist actigraphy measures of sleep efficiency, timing, duration, and regularity, and diary measures of alertness and satisfaction. Each component was dichotomized and summed to create a composite score (0-6); higher values indicated better sleep health. Participants completed a neurocognitive battery assessing six cognitive domains (language, visuospatial ability, attention, immediate and delayed memory, executive function) and self-reported cognitive function. Linear regression models tested associations of multidimensional sleep health and its individual components with cognitive domains in each group, adjusting for age, sex, and education. Results Retired night shift workers and retired day workers did not differ on multidimensional sleep health or its individual components. Among retired night shift workers, poorer multidimensional sleep health was associated with poorer self-reported global cognitive function (beta = 0.51, p = .018), and more irregular sleep timing was associated with poorer self-reported memory (beta = 0.41, p = .054). Multidimensional sleep health and its individual components were unrelated to cognitive function in retired day workers. Conclusion Retired night shift workers may be cognitively vulnerable to poor sleep health in retirement, possibly due to a history of long-term sleep and circadian rhythm disruption while working. Given that self-reported cognitive difficulties often precede more objective cognitive changes, future research should determine whether associations of sleep health with cognitive function are found in more objective cognitive measures as retired night shift workers age. Abstract:Shift work has been associated with a number of health problems including cardiovascular disease, impaired glucose and lipid metabolism, gastrointestinal discomfort, reproductive difficulties, and breast cancer. The specific contributions of disturbed physiological rhythms, circadian misalignment, and sleep debt to the various medical problems encountered by shift workers remain to be clarified. Fatigue can be caused by extended on-duty and/or waking periods, inadequate sleep quantity, sleep disturbances, disruption of circadian rhythms, and difficult work and familial conditions. Fatigue-related accidents raise a safety concern for shift workers, especially at the end of the night when the circadian nadir of alertness interacts with increased time awake. Individuals vary greatly in their capacity to adjust to atypical work schedules and their tolerance to circadian misalignment. Predisposing individual and domestic factors have been identified, such as increasing age, being a single woman in charge of children, and split sleep patterns, all of which can affect the ability to adjust to atypical schedules. However, prior studies indicate that predisposing individual and social determinants are generally poor predictors of shift work tolerance in a given individual. In this manuscript, we review several countermeasures to improve adaptation to shift work.
