Abstract:Alzheimer's disease (AD) is a multifaceted neurodegenerative disorder with varied patient progression. We aim to test the hypothesis that AD patients can be categorized into subgroups based on differences in progression. We leveraged data from three randomized clinical trials (RCTs) to develop a knowledge-guided, deep temporal clustering (KG-DTC) framework for AD subtyping. This model combined autoencoders for contextual information capture, k-means clustering for representation formation, and clinical outcome classification for clinical knowledge integration. The derived representations, encompassing demographics, APOE genotype, cognitive assessments, brain volumes, and biomarkers, were clustered using the Gaussian Mixture Model to identify AD subtypes. Our novel KG-DTC framework was developed using placebo data from 2,087 AD patients across three solanezumab clinical trials (EXPEDITION, EXPEDITION2, and EXPEDITION3), achieving high performance in outcome prediction and clustering. The KG-DTC model demonstrated superior clustering structures, especially when combined with k-means clustering loss. External validation with independent clinical trial data showed consistent clustering results, with a 0.33 silhouette score for three clusters. The model's stability was confirmed through a leave-one-out approach, with an average adjusted Rand Index around 0.945. Three distinct AD subtypes were identified, each exhibiting unique patterns of cognitive function, neurodegeneration, and amyloid beta levels. Notably, Subtype 3 (S3) showed rapid cognitive decline across multiple clinical measures (e.g., 0.64 in S1 vs. -1.06 in S2 vs. 15.09 in S3 of average ADAS total change score, p<.001). This innovative approach offers promising insights for understanding variability in treatment outcomes and personalizing AD treatment strategies.

Abstract:Alzheimer’s disease (AD) is a complex heterogeneous neurodegenerative disease that requires an in-depth understanding of its progression pathways and contributing factors to develop effective risk stratification and prevention strategies. In this study, we proposed an outcome-oriented model to identify progression pathways from mild cognitive impairment (MCI) to AD using electronic health records (EHRs) from the OneFlorida+ Clinical Research Consortium. To achieve this, we employed the long short-term memory (LSTM) network to extract relevant information from the sequential records of each patient. The hierarchical agglomerative clustering was then applied to the learned representation to group patients based on their progression subtypes. Our approach identified multiple progression pathways, each of which represented distinct patterns of disease progression from MCI to AD. Abstract:High-throughput microarray and sequencing technology have been used to identify disease subtypes that could not be observed otherwise by using clinical variables alone. The classical unsupervised clustering strategy concerns primarily the identification of subpopulations that have similar patterns in gene features. However, as the features corresponding to irrelevant confounders (e.g. gender or age) may dominate the clustering process, the resulting clusters may or may not capture clinically meaningful disease subtypes. This gives rise to a fundamental problem: can we find a subtyping procedure guided by a pre-specified disease outcome? Existing methods, such as supervised clustering, apply a two-stage approach and depend on an arbitrary number of selected features associated with outcome. In this paper, we propose a unified latent generative model to perform outcome-guided disease subtyping constructed from omics data, which improves the resulting subtypes concerning the disease of interest. Feature selection is embedded in a regularization regression. A modified EM algorithm is applied for numerical computation and parameter estimation. The proposed method performs feature selection, latent subtype characterization and outcome prediction simultaneously. To account for possible outliers or violation of mixture Gaussian assumption, we incorporate robust estimation using adaptive Huber or median-truncated loss function. Extensive simulations and an application to complex lung diseases with transcriptomic and clinical data demonstrate the ability of the proposed method to identify clinically relevant disease subtypes and signature genes suitable to explore toward precision medicine.

Abstract:The discovery of disease subtypes is an essential step for developing precision medicine, and disease subtyping via omics data has become a popular approach. While promising, subtypes obtained from existing approaches are not necessarily associated with clinical outcomes. With the rich clinical data along with the omics data in modern epidemiology cohorts, it is urgent to develop an outcome-guided clustering algorithm to fully integrate the phenotypic data with the high-dimensional omics data. Hence, we extended a sparse K-means method to an outcome-guided sparse K-means (GuidedSparseKmeans) method. An unified objective function was proposed, which was comprised of (i) weighted K-means to perform sample clusterings; (ii) lasso regularizations to perform gene selection from the high-dimensional omics data; (iii) incorporation of a phenotypic variable from the clinical dataset to facilitate biologically meaningful clustering results. By iteratively optimizing the objective function, we will simultaneously obtain a phenotype-related sample clustering results and gene selection results. We demonstrated the superior performance of the GuidedSparseKmeans by comparing with existing clustering methods in simulations and applications of high-dimensional transcriptomic data of breast cancer and Alzheimer's disease. Abstract:Due to the tremendous heterogeneity of disease manifestations, many complex diseases that were once thought to be single diseases are now considered to have disease subtypes. Disease subtyping analysis, that is the identification of subgroups of patients with similar characteristics, is the first step to accomplish precision medicine. With the advancement of high-throughput technologies, omics data offers unprecedented opportunity to reveal disease subtypes. As a result, unsupervised clustering analysis has been widely used for this purpose. Though promising, the subtypes obtained from traditional quantitative approaches may not always be clinically meaningful (i.e. correlate with clinical outcomes). On the other hand, the collection of rich clinical data in modern epidemiology studies has the great potential to facilitate the disease subtyping process via omics data and to discovery clinically meaningful disease subtypes. Thus, we developed an outcome-guided Bayesian clustering (GuidedBayesianClustering) method to fully integrate the clinical data and the high-dimensional omics data. A Gaussian mixed model framework was applied to perform sample clustering; a spike-and-slab prior was utilized to perform gene selection; a mixture model prior was employed to incorporate the guidance from a clinical outcome variable; and a decision framework was adopted to infer the false discovery rate of the selected genes. We deployed conjugate priors to facilitate efficient Gibbs sampling. Our proposed full Bayesian method is capable of simultaneously (i) obtaining sample clustering (disease subtype discovery); (ii) performing feature selection (select genes related to the disease subtype); and (iii) utilizing clinical outcome variable to guide the disease subtype discovery. The superior performance of the GuidedBayesianClustering was demonstrated through simulations and applications of breast cancer expression data and Alzheimer's disease. An R package has been made publicly available on GitHub to improve the applicability of our method.

Abstract:Alzheimer's disease (AD) is a complex neurodegenerative disorder that has impacted millions of people worldwide. The neuroanatomical heterogeneity of AD has made it challenging to fully understand the disease mechanism. Identifying AD subtypes during the prodromal stage and determining their genetic basis would be immensely valuable for drug discovery and subsequent clinical treatment. Previous studies that clustered subgroups typically used unsupervised learning techniques, neglecting the survival information and potentially limiting the insights gained. To address this problem, we propose an interpretable survival analysis method called Deep Clustering Survival Machines (DCSM), which combines both discriminative and generative mechanisms. Similar to mixture models, we assume that the timing information of survival data can be generatively described by a mixture of parametric distributions, referred to as expert distributions. We learn the weights of these expert distributions for individual instances in a discriminative manner by leveraging their features. This allows us to characterize the survival information of each instance through a weighted combination of the learned expert distributions. We demonstrate the superiority of the DCSM method by applying this approach to cluster patients with mild cognitive impairment (MCI) into subgroups with different risks of converting to AD. Conventional clustering measurements for survival analysis along with genetic association studies successfully validate the effectiveness of the proposed method and characterize our clustering findings.
Temporal Phenotyping using Deep Predictive Clustering of Disease Progression

Abstract:Due to the wider availability of modern electronic health records, patient care data is often being stored in the form of time-series. Clustering such time-series data is crucial for patient phenotyping, anticipating patients' prognoses by identifying "similar" patients, and designing treatment guidelines that are tailored to homogeneous patient subgroups. In this paper, we develop a deep learning approach for clustering time-series data, where each cluster comprises patients who share similar future outcomes of interest (e.g., adverse events, the onset of comorbidities). To encourage each cluster to have homogeneous future outcomes, the clustering is carried out by learning discrete representations that best describe the future outcome distribution based on novel loss functions. Experiments on two real-world datasets show that our model achieves superior clustering performance over state-of-the-art benchmarks and identifies meaningful clusters that can be translated into actionable information for clinical decision-making.

Abstract:Parkinson's disease (PD) is a common neurodegenerative disease with a high degree of heterogeneity in its clinical features, rate of progression, and change of variables over time. In this work, we present a novel data-driven, network-based Trajectory Profile Clustering (TPC) algorithm for 1) identification of PD subtypes and 2) early prediction of disease progression in individual patients. Our subtype identification is based not only on PD variables, but also on their complex patterns of progression, providing a useful tool for the analysis of large heterogenous, longitudinal data. Specifically, we cluster patients based on the similarity of their trajectories through a time series of bipartite networks connecting patients to demographic, clinical, and genetic variables. We apply this approach to demographic and clinical data from the Parkinson's Progression Markers Initiative (PPMI) dataset and identify 3 patient clusters, consistent with 3 distinct PD subtypes, each with a characteristic variable progression profile. Additionally, TPC predicts an individual patient's subtype and future disease trajectory, based on baseline assessments. Application of our approach resulted in 74% accurate subtype prediction in year 5 in a test/validation cohort. Furthermore, we show that genetic variability can be integrated seamlessly in our TPC approach. In summary, using PD as a model for chronic progressive diseases, we show that TPC leverages high-dimensional longitudinal datasets for subtype identification and early prediction of individual disease subtype. We anticipate this approach will be broadly applicable to multidimensional longitudinal datasets in diverse chronic diseases.
Multi-layer Trajectory Clustering: a Network Algorithm for Disease Subtyping

Abstract:Abstract Many diseases display heterogeneity in clinical features and their progression, indicative of the existence of disease subtypes. Extracting patterns of disease variable progression for subtypes has tremendous application in medicine, for example, in early prognosis and personalized medical therapy. This work presents a novel, data-driven, network-based Trajectory Clustering (TC) algorithm for identifying Parkinson’s subtypes based on disease trajectory. Modeling patient-variable interactions as a bipartite network, TC first extracts communities of co-expressing disease variables at different stages of progression. Then, it identifies Parkinson’s subtypes by clustering similar patient trajectories that are characterized by severity of disease variables through a multi-layer network. Determination of trajectory similarity accounts for direct overlaps between trajectories as well as second-order similarities, i.e., common overlap with a third set of trajectories. This work clusters trajectories across two types of layers: (a) temporal, and (b) ranges of independent outcome variable (representative of disease severity), both of which yield four distinct subtypes. The former subtypes exhibit differences in progression of disease domains (Cognitive, Mental Health etc.), whereas the latter subtypes exhibit different degrees of progression, i.e., some remain mild, whereas others show significant deterioration after 5 years. The TC approach is validated through statistical analyses and consistency of the identified subtypes with medical literature. This generalizable and robust method can easily be extended to other progressive multi-variate disease datasets, and can effectively assist in targeted subtype-specific treatment in the field of personalized medicine.

Abstract:Increasing evidence suggests that Parkinson’s disease (PD) exhibits disparate spatial and temporal patterns of progression. Here we used a machine-learning technique—Subtype and Stage Inference (SuStaIn) — to uncover PD subtypes with distinct trajectories of clinical and neurodegeneration events. We enrolled 228 PD patients and 119 healthy controls with comprehensive assessments of olfactory, autonomic, cognitive, sleep, and emotional function. The integrity of substantia nigra (SN), locus coeruleus (LC), amygdala, hippocampus, entorhinal cortex, and basal forebrain were assessed using diffusion and neuromelanin-sensitive MRI. SuStaIn model with above clinical and neuroimaging variables as input was conducted to identify PD subtypes. An independent dataset consisting of 153 PD patients and 67 healthy controls was utilized to validate our findings. We identified two distinct PD subtypes: subtype 1 with rapid eye movement sleep behavior disorder (RBD), autonomic dysfunction, and degeneration of the SN and LC as early manifestations, and cognitive impairment and limbic degeneration as advanced manifestations, while subtype 2 with hyposmia, cognitive impairment, and limbic degeneration as early manifestations, followed later by RBD and degeneration of the LC in advanced disease. Similar subtypes were shown in the validation dataset. Moreover, we found that subtype 1 had weaker levodopa response, more GBA mutations, and poorer prognosis than subtype 2. These findings provide new insights into the underlying disease biology and might be useful for personalized treatment for patients based on their subtype.

Abstract:Diseases change over time, both phenotypically and in their underlying molecular processes. Though understanding disease progression dynamics is critical for diagnostics and treatment, capturing these dynamics is difficult due to their complexity and the high heterogeneity in disease development between individuals. We present TimeAx, an algorithm which builds a comparative framework for capturing disease dynamics using high-dimensional, short time-series data. We demonstrate the utility of TimeAx by studying disease progression dynamics for multiple diseases and data types. Notably, for urothelial bladder cancer tumorigenesis, we identify a stromal pro-invasion point on the disease progression axis, characterized by massive immune cell infiltration to the tumor microenvironment and increased mortality. Moreover, the continuous TimeAx model differentiates between early and late tumors within the same tumor subtype, uncovering molecular transitions and potential targetable pathways. Overall, we present a powerful approach for studying disease progression dynamics-providing improved molecular interpretability and clinical benefits for patient stratification and outcome prediction.

Abstract:Deciphering information hidden in the gene expression assays for identifying disease subtypes has significant importance in precision medicine. However, computational limitations thwart this process due to the intricacy of the biological networks and the curse of dimensionality of gene expression data. Therefore, clustering in such scenarios often becomes the first choice of exploratory data analysis to identify natural structures and intrinsic patterns in the data. However, sparse and high dimensional nature of omics data prevents conventional clustering algorithms to discover subtypes that are clinically relevant and statistically significant. Hence, non-linear dimensionality reduction coupled with clustering in such scenarios often becomes imperative to improve the clustering results. In this study, we present a robust pipeline to discover disease subtypes with clinical relevance. Specifically, we focus on discovering patient sub-groups that have a residual life patterns remarkably different from other sub-groups. This is significant because by refining prognosis, subtyping can reduce uncertainty in approximating patients expected outcome. The methodology present is based on robust correlation estimation , UMAP – a non-linear dimensionality reduction method and mapper – a tool from topology. Notably, we suggest a method for improving the robustness of the correlation matrix of gene expression data for improving the clustering results. The performance of the model is evaluated by applying to five cancer datasets obtained through TCGA and comparisons are performed with some state of the art methods of NEMO, RSC-OTRI and SNF with regard to log−rank test and Restricted Life Expectancy Difference . For example in GBM dataset, the minimum separation for any two discovered subtypes is 221 days which is significantly higher than the other methodologies. We also compared the results without using the robust correlation based estimate and observed that robust correlation improves separability between survival curves significantly. From the results we infer that our methodology performs better compared to other methodologies with regard to separating survival curves of patient sub-groups despite using single omics profiles of patients compared to multiple omics profiles of SNF and NEMO. Pathway over-representation analysis is performed on the final clustering results to investigate the biological underpinnings characterising each subtype.

Abstract:Abstract One of the visions of precision medicine has been to re-define disease taxonomies based on molecular characteristics rather than on phenotypic evidence. However, achieving this goal is highly challenging, specifically in neurology. Our contribution is a machine-learning based joint molecular subtyping of Alzheimer’s (AD) and Parkinson’s Disease (PD), based on the genetic burden of 15 molecular mechanisms comprising 27 proteins (e.g. APOE) that have been described in both diseases. We demonstrate that our joint AD/PD clustering using a combination of sparse autoencoders and sparse non-negative matrix factorization is reproducible and can be associated with significant differences of AD and PD patient subgroups on a clinical, pathophysiological and molecular level. Hence, clusters are disease-associated. To our knowledge this work is the first demonstration of a mechanism based stratification in the field of neurodegenerative diseases. Overall, we thus see this work as an important step towards a molecular mechanism-based taxonomy of neurological disorders, which could help in developing better targeted therapies in the future by going beyond classical phenotype based disease definitions.

Identifying common disease trajectories of Alzheimer's disease with electronic health records

Abstract:Backgrounds: Alzheimer's disease (AD), a leading cause of dementia, poses a growing global public health challenge. While recent studies have identified AD risk factors, they often focus on specific comorbidities, neglecting the complex interrelations and temporal dynamics. Our study addresses this by analyzing AD progression through longitudinal trajectories, utilizing clinical diagnoses over time. Using machine learning and network analysis, we created a computational framework to identify common AD progression patterns. Methods: We analyzed patient diagnoses from UC Health Data Warehouse's Electronic Health Records, coded with the International Classification of Diseases, version 10 (ICD-10). Using the Fine and Gray model to detect significant temporal risk factors between diagnoses, we examined associations between diagnosis pairs and refined the patients' diagnostic trajectories, delineating all possible trajectory combinations. These refined trajectories were compared using Dynamic Time Warping and grouped into clusters with hierarchical clustering. We investigated common AD trajectories through network analysis and compared patient demographics, symptoms, and AD manifestations across clusters. The Greedy Equivalence Search algorithm was used to infer causal relationships within these trajectories. We rigorously evaluated these trajectories through association tests and comparison to controls, Results: Our analysis included 24,473 eligible AD patients, which was filtered to include 5,762 patients with 6,794 unique AD progression trajectories. We identified four trajectory clusters: 1) a mental health cluster (e.g., anxiety disorder → depressive episode) (N_patient = 1,448); 2) an encephalopathy cluster (e.g., hypertension → other disorders of brain) (N_patient = 3,223); 3) a neurodegenerative disease cluster (e.g., transient cerebral ischemic attacks → other degenerative disease of nervous system) (N_patient = 1,502); and 4) a vascular disease cluster (e.g. hypertension → other cerebrovascular diseases) (N_patient = 1,446). Significant differences were observed in demographics, symptoms, and AD features across clusters. Causal analysis indicated that 26.2% of the identified trajectory connections were causal. We also observed patients with risk trajectories faced higher risks of AD compared to those without the trajectory or with only a single risk factor. Conclusion: We uncovered AD diagnosis trajectories, incorporating temporal aspects and causal relationships. These insights improve our understanding of AD development and AD subtypes, and can enhance risk assessment. Our findings can significantly benefit patient care and medical research by moving toward earlier and more accurate diagnoses, along with personalized treatment, such as medical risk factors management and lifestyle modifications.

Abstract:Subtyping of Alzheimer's disease (AD) can facilitate diagnosis, treatment, prognosis and disease management. It can also support the testing of new prevention and treatment strategies through clinical trials. In this study, we employed spectral clustering to cluster 29,922 AD patients in the OneFlorida Data Trust using their longitudinal EHR data of diagnosis and conditions into four subtypes. These subtypes exhibit different patterns of progression of other conditions prior to the first AD diagnosis. In addition, according to the results of various statistical tests, these subtypes are also significantly different with respect to demographics, mortality, and prescription medications after the AD diagnosis. This study could potentially facilitate early detection and personalized treatment of AD as well as data-driven generalizability assessment of clinical trials for AD.
Characterizing the clinical heterogeneity of early symptomatic Alzheimer's disease: a data-driven machine learning approach

Abstract:Introduction: Alzheimer's disease (AD) is highly heterogeneous, with substantial individual variabilities in clinical progression and neurobiology. Amyloid deposition has been thought to drive cognitive decline and thus a major contributor to the variations in cognitive deterioration in AD. However, the clinical heterogeneity of patients with early symptomatic AD (mild cognitive impairment or mild dementia due to AD) already with evidence of amyloid abnormality in the brain is still unknown. Methods: Participants with a baseline diagnosis of mild cognitive impairment or mild dementia, a positive amyloid-PET scan, and more than one follow-up Alzheimer's Disease Assessment Scale-Cognitive Subscale-13 (ADAS-Cog-13) administration within a period of 5-year follow-up were selected from the Alzheimer's Disease Neuroimaging Initiative database (n = 421; age = 73±7; years of education = 16 ± 3; percentage of female gender = 43%; distribution of APOE4 carriers = 68%). A non-parametric k-means longitudinal clustering analysis in the context of the ADAS-Cog-13 data was performed to identify cognitive subtypes. Results: We found a highly variable profile of cognitive decline among patients with early AD and identified 4 clusters characterized by distinct rates of cognitive progression. Among the groups there were significant differences in the magnitude of rates of changes in other cognitive and functional outcomes, clinical progression from mild cognitive impairment to dementia, and changes in markers presumed to reflect neurodegeneration and neuronal injury. A nomogram based on a simplified logistic regression model predicted steep cognitive trajectory with an AUC of 0.912 (95% CI: 0.88 - 0.94). Simulation of clinical trials suggested that the incorporation of the nomogram into enrichment strategies would reduce the required sample sizes from 926.8 (95% CI: 822.6 - 1057.5) to 400.9 (95% CI: 306.9 - 516.8). Discussion: Our findings show usefulness in the stratification of patients in early AD and may thus increase the chances of finding a treatment for future AD clinical trials.

Abstract:Due to the high heterogeneity and complexity of cancers, patients with different cancer subtypes often have distinct groups of genomic and clinical characteristics. Therefore, the discovery and identification of cancer subtypes are crucial to cancer diagnosis, prognosis and treatment. Recent technological advances have accelerated the increasing availability of multi-omics data for cancer subtyping. To take advantage of the complementary information from multi-omics data, it is necessary to develop computational models that can represent and integrate different layers of data into a single framework. Here, we propose a decoupled contrastive clustering method (Subtype-DCC) based on multi-omics data integration for clustering to identify cancer subtypes. The idea of contrastive learning is introduced into deep clustering based on deep neural networks to learn clustering-friendly representations. Experimental results demonstrate the superior performance of the proposed Subtype-DCC model in identifying cancer subtypes over the currently available state-of-the-art clustering methods. The strength of Subtype-DCC is also supported by the survival and clinical analysis.

Abstract:Precision medicine can be defined as providing the right treatment to the right patient at the right time, and it requires the ability to identify clinically relevant patient subgroups with high accuracy. The increasing availability of large-scale electronic health records (EHR) datasets has provided major opportunities for artificial intelligence and machine learning in mining such complex datasets for identifying novel disease subtypes. However, disease subtypes often exist in the context of certain disease-relevant risk events, and current efforts have been limited by analyzing clustering and event risk independently, resulting in subgroups that still display great heterogeneity in event risk and/or underlying molecular mechanisms. To address this problem, we developed TransVarSur (Transformer Variational Survival modeling). TransVarSur integrates a Transformer-based Gaussian mixture variational autoencoder with time-to-event modeling to capture complex relationships between cluster-specific EHR trajectories and survival times. We validated TransVarSur by showing superior performance relative to baseline methods, on both synthetic and real-world benchmark datasets with known ground-truth clustering. We then applied TransVarSur to 1908 Crohn’s disease patients from the UK Biobank and successfully identified four clusters displaying both divergent EHR trajectories and divergent progression towards the risk event intestinal obstruction. A further analysis of the clusters revealed known clinical and genetic factors relevant in Crohn’s disease and progression to intestinal obstruction. In conclusion, we demonstrated TransVarSur’s ability to accurately stratify a patient population into clinically and genetically relevant, risk-associated subgroups. Hence, it can be a powerful tool in the development of precision medicine approaches.

Abstract:Alzheimer's disease (AD) is a degenerative brain disease impairing a person's ability to perform day to day activities. The clinical manifestations of Alzheimer's disease are characterized by heterogeneity in age, disease span, progression rate, impairment of memory and cognitive abilities. Due to these variabilities, personalized care and treatment planning, as well as patient counseling about their individual progression is limited. Recent developments in machine learning to detect hidden patterns in complex, multi-dimensional datasets provides significant opportunities to address this critical need. In this work, we use unsupervised and supervised machine learning approaches for subtype identification and prediction. We apply machine learning methods to the extensive clinical observations available at the Alzheimer's Disease Neuroimaging Initiative (ADNI) data set to identify patient subtypes and to predict disease progression. Our analysis depicts the progression space for the Alzheimer's disease into low, moderate and high disease progression zones. The proposed work will enable early detection and characterization of distinct disease subtypes based on clinical heterogeneity. We anticipate that our models will enable patient counseling, clinical trial design, and ultimately individualized clinical care.
Machine learning on longitudinal multi-modal data enabling the understanding and prognosis of Alzheimer’s disease progression

Abstract:Abstract Background Alzheimer’s disease (AD) is a complex pathophysiological disease. Its progression is heterogenous and associated with clinical symptoms as well as prognosis. Hence, understanding the progression of AD is important in guiding patient management, evaluating treatment response and designing therapeutic trials. We developed and validated a novel machine learning model for revealing the underlying disease progression patterns from longitudinal multi-modal data of AD. Methods The prospective ADNI study data (1192 patients with cognitive impairment and 388 healthy controls) was used to train, validate and test a novel personalized hidden Markov model to identify disease-related states. The transition diagram of disease-related states exhibited two distinct disease progression patterns. We used the index of disease-related states as a predictor, and evaluated the prediction performance on time to conversion to AD dementia. External validation was performed on the AIBL cohort (87 patients with cognitive impairment and 179 healthy controls). Findings Our model identified ten biologically and clinically meaningful disease-related states from longitudinal multi-modal data, which constitute three non-overlapping stages (i.e., neocortical atrophy [NCA], medial temporal atrophy [MTA], and whole brain atrophy [WBA]) and two distinct disease progression patterns (i.e., NCA→WBA and MTA→WBA) hidden in the trajectories of AD patients. Disclosed patterns showed different atrophy trends of brain regions, and presented distinct genetic, clinical and biomarker characteristics, especially in the early course of the disease. The index of the identified disease-related states provided a remarkable performance on predicting the time to conversion to AD dementia (C-Index: 0.923±0.007), in comparison with the other commonly recognized AD risk factors, including APOE (0.607±0.012), MMSE score (0.808±0.006), Abeta (0.7445±0.013), pTau (0.689±0.006), and hippocampal volume (0.774±0.010). Interpretation A machine learning algorithm was proposed to capture the full spectrum of AD progression by identifying meaningful AD-related states as well as distinct disease progression patterns from longitudinal multi-modal data, showing its potential for promoting the understanding of heterogeneous disease progression, and early predicting the conversion time to AD dementia.
