Abstract:Purpose Deep learning-based denoising is promising for myocardial perfusion (MP) SPECT. However, conventional convolutional neural network (CNN)-based methods use fixed-sized convolutional kernels to convolute one region within the receptive field at a time, which would be ineffective for learning the feature dependencies across large regions. The attention mechanism (Att) is able to learn the relationships between the local receptive field and other voxels in the image. In this study, we propose a 3D attention-guided generative adversarial network (AttGAN) for denoising fast MP-SPECT images. Methods Fifty patients who underwent 1184 MBq 99m Tc-sestamibi stress SPECT/CT scan were retrospectively recruited. Sixty projections were acquired over 180° and the acquisition time was 10 s/view for the full time (FT) mode. Fast MP-SPECT projection images (1 s to 7 s) were generated from the FT list mode data. We further incorporated binary patient defect information (0 = without defect, 1 = with defect) into AttGAN (AttGAN-def). AttGAN, AttGAN-def, cGAN, and Unet were implemented using Tensorflow with the Adam optimizer running up to 400 epochs. FT and fast MP-SPECT projection pairs of 35 patients were used for training the networks for each acquisition time, while 5 and 10 patients were applied for validation and testing. Five-fold cross-validation was performed and data for all 50 patients were tested. Voxel-based error indices, joint histogram, linear regression, and perfusion defect size (PDS) were analyzed. Results All quantitative indices of AttGAN-based networks are superior to cGAN and Unet on all acquisition time images. AttGAN-def further improves AttGAN performance. The mean absolute error of PDS by AttcGAN-def was 1.60 on acquisition time of 1 s/prj, as compared to 2.36, 2.76, and 3.02 by AttGAN, cGAN, and Unet. Conclusion Denoising based on AttGAN is superior to conventional CNN-based networks for MP-SPECT.

Abstract:Deep-learning (DL)-based methods have shown significant promise in denoising myocardial perfusion SPECT images acquired at low dose. For clinical application of these methods, evaluation on clinical tasks is crucial. Typically, these methods are designed to minimize some fidelity-based criterion between the predicted denoised image and some reference normal-dose image. However, while promising, studies have shown that these methods may have limited impact on the performance of clinical tasks in SPECT. To address this issue, we use concepts from the literature on model observers and our understanding of the human visual system to propose a DL-based denoising approach designed to preserve observer-related information for detection tasks. The proposed method was objectively evaluated on the task of detecting perfusion defect in myocardial perfusion SPECT images using a retrospective study with anonymized clinical data. Our results demonstrate that the proposed method yields improved performance on this detection task compared to using low-dose images. The results show that by preserving task-specific information, DL may provide a mechanism to improve observer performance in low-dose myocardial perfusion SPECT.
Dual gating myocardial perfusion SPECT denoising using a conditional generative adversarial network

Abstract:Purpose: Dual respiratory-cardiac gating reduces respiratory and cardiac motion blur in myocardial perfusion single-photon emission computed tomography (MP-SPECT). However, image noise is increased as detected counts are reduced in each dual gate (DG). We aim to develop a denoising method for dual gating MP-SPECT images using a 3D conditional generative adversarial network (cGAN). Methods: Twenty extended cardiac-torso phantoms with various 99m Tc-sestamibi distributions, defect characteristics, and body and organ sizes were used in the simulation, modeling six respiratory and eight cardiac gates (CGs), that is, 48 DGs for ordered subset expectation maximization reconstruction. Twenty clinical 99m Tc-sestamibi SPECT/CT datasets were re-binned into 7 respiratory gates and 8 CGs, that is, 56 DGs for maximum likelihood expectation maximization reconstruction. We evaluated the use of (i) phantoms' own datasets (patient-specific denoising [PD]) or other phantoms' datasets (cross-patient denoising) for training; (ii) the CG or the static (non-gated [NG]) data as the training references for cGAN; and (iii) cGAN as compared to conventional 3D post-reconstruction filtering, cardiac gating methods, and convolutional neural network. Normalized mean squared error, noise as assessed by normalized standard deviation, spatial blurring measured as the full-width-at-half-maximum of left ventricular wall, ejection fraction, joint correlation histogram, and defect size were analyzed as metrics of image quality. Results: Training using patients' own dataset is superior to conventional training based on other patients' data. Using CG image as training reference provides a better trade-off in terms of noise and image blur as compared to the use of NG. cGAN-CG-PD provides superior performance as compared to other denoising methods for all physical and diagnostic indices evaluated in both simulation and clinical studies. Conclusions: cGAN denoising is promising for dual gating MP-SPECT based on the metrics mentioned earlier.

Abstract:Background: The aim of this research was to asses perfusion-defect detection-accuracy by human observers as a function of reduced-counts for 3D Gaussian post-reconstruction filtering vs deep learning (DL) denoising to determine if there was improved performance with DL. Methods: SPECT projection data of 156 normally interpreted patients were used for these studies. Half were altered to include hybrid perfusion defects with defect presence and location known. Ordered-subset expectation-maximization (OSEM) reconstruction was employed with the optional correction of attenuation (AC) and scatter (SC) in addition to distance-dependent resolution (RC). Count levels varied from full-counts (100%) to 6.25% of full-counts. The denoising strategies were previously optimized for defect detection using total perfusion deficit (TPD). Four medical physicist (PhD) and six physician (MD) observers rated the slices using a graphical user interface. Observer ratings were analyzed using the LABMRMC multi-reader, multi-case receiver-operating-characteristic (ROC) software to calculate and compare statistically the area-under-the-ROC-curves (AUCs). Results: For the same count-level no statistically significant increase in AUCs for DL over Gaussian denoising was determined when counts were reduced to either the 25% or 12.5% of full-counts. The average AUC for full-count OSEM with solely RC and Gaussian filtering was lower than for the strategies with AC and SC, except for a reduction to 6.25% of full-counts, thus verifying the utility of employing AC and SC with RC. Conclusion: We did not find any indication that at the dose levels investigated and with the DL network employed, that DL denoising was superior in AUC to optimized 3D post-reconstruction Gaussian filtering.
DEMIST: A deep-learning-based task-specific denoising approach for myocardial perfusion SPECT

Abstract:There is an important need for methods to process myocardial perfusion imaging (MPI) SPECT images acquired at lower radiation dose and/or acquisition time such that the processed images improve observer performance on the clinical task of detecting perfusion defects. To address this need, we build upon concepts from model-observer theory and our understanding of the human visual system to propose a Detection task-specific deep-learning-based approach for denoising MPI SPECT images (DEMIST). The approach, while performing denoising, is designed to preserve features that influence observer performance on detection tasks. We objectively evaluated DEMIST on the task of detecting perfusion defects using a retrospective study with anonymized clinical data in patients who underwent MPI studies across two scanners (N = 338). The evaluation was performed at low-dose levels of 6.25%, 12.5% and 25% and using an anthropomorphic channelized Hotelling observer. Performance was quantified using area under the receiver operating characteristics curve (AUC). Images denoised with DEMIST yielded significantly higher AUC compared to corresponding low-dose images and images denoised with a commonly used task-agnostic DL-based denoising method. Similar results were observed with stratified analysis based on patient sex and defect type. Additionally, DEMIST improved visual fidelity of the low-dose images as quantified using root mean squared error and structural similarity index metric. A mathematical analysis revealed that DEMIST preserved features that assist in detection tasks while improving the noise properties, resulting in improved observer performance. The results provide strong evidence for further clinical evaluation of DEMIST to denoise low-count images in MPI SPECT.

Need for objective task-based evaluation of deep learning-based denoising methods: A study in the context of myocardial perfusion SPECT

Abstract:Background: Artificial intelligence-based methods have generated substantial interest in nuclear medicine. An area of significant interest has been the use of deep-learning (DL)-based approaches for denoising images acquired with lower doses, shorter acquisition times, or both. Objective evaluation of these approaches is essential for clinical application. Purpose: DL-based approaches for denoising nuclear-medicine images have typically been evaluated using fidelity-based figures of merit (FoMs) such as root mean squared error (RMSE) and structural similarity index measure (SSIM). However, these images are acquired for clinical tasks and thus should be evaluated based on their performance in these tasks. Our objectives were to: (1) investigate whether evaluation with these FoMs is consistent with objective clinical-task-based evaluation; (2) provide a theoretical analysis for determining the impact of denoising on signal-detection tasks; and (3) demonstrate the utility of virtual imaging trials (VITs) to evaluate DL-based methods. Methods: A VIT to evaluate a DL-based method for denoising myocardial perfusion SPECT (MPS) images was conducted. To conduct this evaluation study, we followed the recently published best practices for the evaluation of AI algorithms for nuclear medicine (the RELAINCE guidelines). An anthropomorphic patient population modeling clinically relevant variability was simulated. Projection data for this patient population at normal and low-dose count levels (20%, 15%, 10%, 5%) were generated using well-validated Monte Carlo-based simulations. The images were reconstructed using a 3-D ordered-subsets expectation maximization-based approach. Next, the low-dose images were denoised using a commonly used convolutional neural network-based approach. The impact of DL-based denoising was evaluated using both fidelity-based FoMs and area under the receiver operating characteristic curve (AUC), which quantified performance on the clinical task of detecting perfusion defects in MPS images as obtained using a model observer with anthropomorphic channels. We then provide a mathematical treatment to probe the impact of post-processing operations on signal-detection tasks and use this treatment to analyze the findings of this study. Results: Based on fidelity-based FoMs, denoising using the considered DL-based method led to significantly superior performance. However, based on ROC analysis, denoising did not improve, and in fact, often degraded detection-task performance. This discordance between fidelity-based FoMs and task-based evaluation was observed at all the low-dose levels and for different cardiac-defect types. Our theoretical analysis revealed that the major reason for this degraded performance was that the denoising method reduced the difference in the means of the reconstructed images and of the channel operator-extracted feature vectors between the defect-absent and defect-present cases. Conclusions: The results show the discrepancy between the evaluation of DL-based methods with fidelity-based metrics versus the evaluation on clinical tasks. This motivates the need for objective task-based evaluation of DL-based denoising approaches. Further, this study shows how VITs provide a mechanism to conduct such evaluations computationally, in a time and resource-efficient setting, and avoid risks such as radiation dose to the patient. Finally, our theoretical treatment reveals insights into the reasons for the limited performance of the denoising approach and may be used to probe the effect of other post-processing operations on signal-detection tasks.
Dual-Domain Coarse-to-Fine Progressive Estimation Network for Simultaneous Denoising, Limited-View Reconstruction, and Attenuation Correction of Cardiac SPECT

Abstract:Single-Photon Emission Computed Tomography (SPECT) is widely applied for the diagnosis of coronary artery diseases. Low-dose (LD) SPECT aims to minimize radiation exposure but leads to increased image noise. Limited-view (LV) SPECT, such as the latest GE MyoSPECT ES system, enables accelerated scanning and reduces hardware expenses but degrades reconstruction accuracy. Additionally, Computed Tomography (CT) is commonly used to derive attenuation maps ($\mu$-maps) for attenuation correction (AC) of cardiac SPECT, but it will introduce additional radiation exposure and SPECT-CT misalignments. Although various methods have been developed to solely focus on LD denoising, LV reconstruction, or CT-free AC in SPECT, the solution for simultaneously addressing these tasks remains challenging and under-explored. Furthermore, it is essential to explore the potential of fusing cross-domain and cross-modality information across these interrelated tasks to further enhance the accuracy of each task. Thus, we propose a Dual-Domain Coarse-to-Fine Progressive Network (DuDoCFNet), a multi-task learning method for simultaneous LD denoising, LV reconstruction, and CT-free $\mu$-map generation of cardiac SPECT. Paired dual-domain networks in DuDoCFNet are cascaded using a multi-layer fusion mechanism for cross-domain and cross-modality feature fusion. Two-stage progressive learning strategies are applied in both projection and image domains to achieve coarse-to-fine estimations of SPECT projections and CT-derived $\mu$-maps. Our experiments demonstrate DuDoCFNet's superior accuracy in estimating projections, generating $\mu$-maps, and AC reconstructions compared to existing single- or multi-task learning methods, under various iterations and LD levels. PET Image Denoising Using Unsupervised Deep Learning.

Abstract:Image quality of positron emission tomography (PET) is limited by various physical degradation factors. Our study aims to perform PET image denoising by utilizing prior information from the same patient. The proposed method is based on unsupervised deep learning, where no training pairs are needed. In this method, the prior high-quality image from the patient was employed as the network input and the noisy PET image itself was treated as the training label. Constrained by the network structure and the prior image input, the network was trained to learn the intrinsic structure information from the noisy image and output a restored PET image. To validate the performance of the proposed method, a computer simulation study based on the BrainWeb phantom was first performed. A 68Ga-PRGD2 PET/CT dataset containing 10 patients and a 18F-FDG PET/MR dataset containing 30 patients were later on used for clinical data evaluation. The Gaussian, non-local mean (NLM) using CT/MR image as priors, BM4D, and Deep Decoder methods were included as reference methods. The contrast-to-noise ratio (CNR) improvements were used to rank different methods based on Wilcoxon signed-rank test. For the simulation study, contrast recovery coefficient (CRC) vs. standard deviation (STD) curves showed that the proposed method achieved the best performance regarding the bias-variance tradeoff. For the clinical PET/CT dataset, the proposed method achieved the highest CNR improvement ratio (53.35% ± 21.78%), compared with the Gaussian (12.64% ± 6.15%, P = 0.002), NLM guided by CT (24.35% ± 16.30%, P = 0.002), BM4D (38.31% ± 20.26%, P = 0.002), and Deep Decoder (41.67% ± 22.28%, P = 0.002) methods. For the clinical PET/MR dataset, the CNR improvement ratio of the proposed method achieved 46.80% ± 25.23%, higher than the Gaussian (18.16% ± 10.02%, P < 0.0001), NLM guided by MR (25.36% ± 19.48%, P < 0.0001), BM4D (37.02% ± 21.38%, P < 0.0001), and Deep Decoder (30.03% ± 20.64%, P < 0.0001) methods. Restored images for all the datasets demonstrate that the proposed method can effectively smooth out the noise while recovering image details. The proposed unsupervised deep learning framework provides excellent image restoration effects, outperforming the Gaussian, NLM methods, BM4D, and Deep Decoder methods.
Joint Denoising and Few-angle Reconstruction for Low-dose Cardiac SPECT Using a Dual-domain Iterative Network with Adaptive Data Consistency

Abstract:Myocardial perfusion imaging (MPI) by single-photon emission computed tomography (SPECT) is widely applied for the diagnosis of cardiovascular diseases. Reducing the dose of the injected tracer is essential for lowering the patient's radiation exposure, but it will lead to increased image noise. Additionally, the latest dedicated cardiac SPECT scanners typically acquire projections in fewer angles using fewer detectors to reduce hardware expenses, potentially resulting in lower reconstruction accuracy. To overcome these challenges, we propose a dual-domain iterative network for end-to-end joint denoising and reconstruction from low-dose and few-angle projections of cardiac SPECT. The image-domain network provides a prior estimate for the projection-domain networks. The projection-domain primary and auxiliary modules are interconnected for progressive denoising and few-angle reconstruction. Adaptive Data Consistency (ADC) modules improve prediction accuracy by efficiently fusing the outputs of the primary and auxiliary modules. Experiments using clinical MPI data show that our proposed method outperforms existing image-, projection-, and dual-domain techniques, producing more accurate projections and reconstructions. Ablation studies confirm the significance of the image-domain prior estimate and ADC modules in enhancing network performance.

Abstract:Background To improve myocardial delayed enhancement (MDE) CT, a deep learning (DL)-based post hoc denoising method supervised with averaged MDE CT data was developed. Purpose To assess the image quality of denoised MDE CT images and evaluate their diagnostic performance by using late gadolinium enhancement (LGE) MRI as a reference. Materials and methods MDE CT data obtained by averaging three acquisitions with a single breath hold 5 minutes after the contrast material injection in patients from July 2020 to October 2021 were retrospectively reviewed. Preaveraged images obtained in 100 patients as inputs and averaged images as ground truths were used to supervise a residual dense network (RDN). The original single-shot image, standard averaged image, RDN-denoised original (DL) image, and RDN-denoised averaged (DL) image of holdout cases were compared. In 40 patients, the CT value and image noise in the left ventricular cavity and myocardium were assessed. The segmental presence of MDE in the remaining 40 patients who underwent reference LGE MRI was evaluated. The sensitivity, specificity, and accuracy of each type of CT image and the improvement in accuracy achieved with the RDN were assessed using odds ratios (ORs) estimated with the generalized estimation equation. Results Overall, 180 patients (median age, 66 years [IQR, 53-74 years]; 107 men) were included. The RDN reduced image noise to 28% of the original level while maintaining equivalence in the CT values ( < .001 for all). The sensitivity, specificity, and accuracy of the original images were 77.9%, 84.4%, and 82.3%, of the averaged images were 89.7%, 87.9%, and 88.5%, of the DL images were 93.1%, 87.5%, and 89.3%, and of the DL images were 95.1%, 93.1%, and 93.8%, respectively. DL images showed improved accuracy compared with the original images (OR, 1.8 [95% CI: 1.2, 2.9]; = .011) and DL images showed improved accuracy compared with the averaged images (OR, 2.0 [95% CI: 1.2, 3.5]; = .009). Conclusion The proposed denoising network supervised with averaged CT images reduced image noise and improved the diagnostic performance for myocardial delayed enhancement CT. © RSNA, 2022 See also the editorial by Vannier and Wang in this issue.

Abstract:Positron emission tomography (PET) images still suffer from low signal-to-noise ratio (SNR) due to various physical degradation factors. Recently deep neural networks (DNNs) have been successfully applied to medical image denoising tasks when large number of training pairs are available. Previously the deep image prior framework1 shows that individual information can be enough to train a denoising network, with noisy image itself as the training label. In this work, we propose to improve PET image quality by jointly employing population and individual information based on DNN. The population information was utilized by pre-training the network using a group of patients. The individual information was introduced during testing phase by fine-tuning the population-information-trained network. Unlike traditional DNN denoising, in this framework fine-tuning during testing phase is available as the noisy PET image itself was treated as the training label. Quantification results based on clinical PET/MR datasets containing thirty patients demonstrate that the proposed framework outperforms Gaussian, non-local mean and deep image prior denoising methods.

Abstract:Background: Myocardial perfusion (MP) SPECT is a well-established method for diagnosing cardiac disease, yet its radiation risk poses safety concern. This study aims to apply and evaluate the use of Pix2Pix generative adversarial network (Pix2Pix GAN) in denoising low dose MP SPECT images.Methods: One hundred male and female patients with different 99mTc-sestamibi activity distributions, organ and body sizes were simulated by a population of digital 4D Extended Cardiac Torso (XCAT) phantoms. Realistic noisy SPECT projections of full dose of 987 MBq injection and 16 min acquisition, and low dose ranged from 1/20 to 1/2 of the full dose, were generated by an analytical projector from the right anterior oblique (RAO) to the left posterior oblique (LPO) positions. Additionally, twenty patients underwent ~1,184 MBq 99mTc-sestamibi stress SPECT/CT scan were also retrospectively recruited for the study. For each patient, low dose SPECT images (7/10 to 1/10 of full dose) were generated from the full dose list mode data. Our Pix2Pix GAN model was trained with full dose and low dose reconstructed SPECT image pairs. Normalized mean square error (NMSE), structural similarity index (SSIM), coefficient of variation (CV), full-width-at-half-maximum (FWHM) and relative defect size differences (RSD) of Pix2Pix GAN processed images were evaluated along with a reference convolutional auto encoder (CAE) network and post-reconstruction filters.Results: NMSE values of 0.0233±0.004 vs. 0.0249±0.004 and 0.0313±0.007 vs. 0.0579±0.016 were obtained on 1/2 and 1/20 dose level for Pix2Pix GAN and CAE in the simulation study, while they were 0.0376±0.010 vs. 0.0433±0.010 and 0.0907±0.020 vs. 0.1186±0.025 on 7/10 and 1/10 dose level in the clinical study. Similar results were also obtained from the SSIM, CV, FWHM and RSD values. Overall, the use of Pix2Pix GAN was superior to other denoising methods in all physical indices, particular in the lower dose levels in the simulation and clinical study.Conclusions: The Pix2Pix GAN method is effective to reduce the noise level of low dose MP SPECT. Further studies on clinical performance are warranted to demonstrate its full clinical effectiveness.

Cross-domain Iterative Network for Simultaneous Denoising, Limited-angle Reconstruction, and Attenuation Correction of Low-dose Cardiac SPECT

Abstract:Single-Photon Emission Computed Tomography (SPECT) is widely applied for the diagnosis of ischemic heart diseases. Low-dose (LD) SPECT aims to minimize radiation exposure but leads to increased image noise. Limited-angle (LA) SPECT enables faster scanning and reduced hardware costs but results in lower reconstruction accuracy. Additionally, computed tomography (CT)-derived attenuation maps ($\mu$-maps) are commonly used for SPECT attenuation correction (AC), but it will cause extra radiation exposure and SPECT-CT misalignments. In addition, the majority of SPECT scanners in the market are not hybrid SPECT/CT scanners. Although various deep learning methods have been introduced to separately address these limitations, the solution for simultaneously addressing these challenges still remains highly under-explored and challenging. To this end, we propose a Cross-domain Iterative Network (CDI-Net) for simultaneous denoising, LA reconstruction, and CT-free AC in cardiac SPECT. In CDI-Net, paired projection- and image-domain networks are end-to-end connected to fuse the emission and anatomical information across domains and iterations. Adaptive Weight Recalibrators (AWR) adjust the multi-channel input features to enhance prediction accuracy. Our experiments using clinical data showed that CDI-Net produced more accurate $\mu$-maps, projections, and reconstructions compared to existing approaches that addressed each task separately. Ablation studies demonstrated the significance of cross-domain and cross-iteration connections, as well as AWR, in improving the reconstruction performance.

Abstract:Purpose: The problem of image denoising in single-photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI) is a fundamental challenge. Although various image processing techniques have been presented, they may degrade the contrast of denoised images. The proposed idea in this study is to use a deep neural network as the denoising procedure during the iterative reconstruction process rather than the post-reconstruction phase. This method could decrease the background coefficient of variation (COV_bkg) of the final reconstructed image, which represents the amount of random noise, while improving the contrast-to-noise ratio (CNR). Methods: In this study, a generative adversarial network is used, where its generator is trained by a two-phase approach. In the first phase, the network is trained by a confined image region around the heart in transverse view. The second phase improves the network's generalization by tuning the network weights with the full image size as the input. The network was trained and tested by a dataset of 247 patients who underwent two immediate serially high- and low-noise SPECT-MPI. Results: Quantitative results show that compared to post-reconstruction low pass filtering and post-reconstruction deep denoising methods, our proposed method can decline the COV_bkg of the images by up to 10.28% and 12.52% and enhance the CNR by up to 54.54% and 45.82%, respectively. Conclusion: The iterative deep denoising method outperforms 2D low-pass Gaussian filtering with an 8.4-mm FWHM and post-reconstruction deep denoising approaches.

Abstract:Low-dose computed tomography (LDCT) has gained increasing attention owing to its crucial role in reducing radiation exposure in patients. However, LDCT-reconstructed images often suffer from significant noise and artifacts, negatively impacting the radiologists' ability to accurately diagnose. To address this issue, many studies have focused on denoising LDCT images using deep learning (DL) methods. However, these DL-based denoising methods have been hindered by the highly variable feature distribution of LDCT data from different imaging sources, which adversely affects the performance of current denoising models. In this study, we propose a parallel processing model, the multi-encoder deep feature transformation network (MDFTN), which is designed to enhance the performance of LDCT imaging for multisource data. Unlike traditional network structures, which rely on continual learning to process multitask data, the approach can simultaneously handle LDCT images within a unified framework from various imaging sources. The proposed MDFTN consists of multiple encoders and decoders along with a deep feature transformation module (DFTM). During forward propagation in network training, each encoder extracts diverse features from its respective data source in parallel and the DFTM compresses these features into a shared feature space. Subsequently, each decoder performs an inverse operation for multisource loss estimation. Through collaborative training, the proposed MDFTN leverages the complementary advantages of multisource data distribution to enhance its adaptability and generalization. Numerous experiments were conducted on two public datasets and one local dataset, which demonstrated that the proposed network model can simultaneously process multisource data while effectively suppressing noise and preserving fine structures. Low dose SPECT image denoising using a generative adversarial network

Abstract:The image noise level and resolution of SPECT images are relatively poor attributed to the limited number of detected counts and various physical degradation factors during acquisitions. This study aims to apply and evaluate the use of generative adversarial network (GAN) method in static SPECT image denoising. A population of 4D extended cardiac-torso (XCAT) phantoms were used to simulate 10 male and female patients with different organ sizes and activity uptakes. An analytical projector was applied to simulate 120 projections from right anterior oblique to left posterior oblique positions with two noise levels. The first noise level was based on a standard clinical count rate of 987 MBq injection and 16 min acquisition (low noise) while the other was 1/8 of the previous count rate (high noise). The high noise and low noise SPECT reconstructed images of 9 patients, i.e., 1026 images (9*114 axial slices) respectively, were paired for training. High noise SPECT images of 1 patient were tested using the trained GAN. The noise level is substantially reduced in high noise SPECT reconstructed images after using the GAN. Our method has the potential to decrease the noise level of SPECT images, which could be traded for a reduced injection dose or acquisition time while still maintaining the similar image quality as compared to the original low noise images for clinical diagnosis.

Clinical Feasibility of Deep Learning–Based Attenuation Correction Models for Tl-201 Myocardial Perfusion SPECT

Abstract:Purpose We aimed to develop deep learning (DL)–based attenuation correction models for Tl-201 myocardial perfusion SPECT (MPS) images and evaluate their clinical feasibility. Patients and Methods We conducted a retrospective study of patients with suspected or known coronary artery disease. We proposed a DL-based image-to-image translation technique to transform non–attenuation-corrected images into CT-based attenuation-corrected (CT AC ) images. The model was trained using a modified U-Net with structural similarity index (SSIM) loss and mean squared error (MSE) loss and compared with other models. Segment-wise analysis using a polar map and visual assessment for the generated attenuation-corrected (GEN AC ) images were also performed to evaluate clinical feasibility. Results This study comprised 657 men and 328 women (age, 65 ± 11 years). Among the various models, the modified U-Net achieved the highest performance with an average mean absolute error of 0.003, an SSIM of 0.990, and a peak signal-to-noise ratio of 33.658. The performance of the model was not different between the stress and rest datasets. In the segment-wise analysis, the myocardial perfusion of the inferior wall was significantly higher in GEN AC images than in the non–attenuation-corrected images in both the rest and stress test sets ( P < 0.05). In the visual assessment of patients with diaphragmatic attenuation, scores of 4 (similar to CT AC images) or 5 (indistinguishable from CT AC images) were assigned to most GEN AC images (65/68). Conclusions Our clinically feasible DL-based attenuation correction models can replace the CT-based method in Tl-201 MPS, and it would be useful in case SPECT/CT is unavailable for MPS.

Abstract:Although supervised convolutional neural networks (CNNs) often outperform conventional alternatives for denoising positron emission tomography (PET) images, they require many low- and high-quality reference PET image pairs. Herein, we propose an unsupervised 3D PET image denoising method based on an anatomical information-guided attention mechanism. The proposed magnetic resonance-guided deep decoder (MR-GDD) utilizes the spatial details and semantic features of MR-guidance image more effectively by introducing encoder-decoder and deep decoder subnetworks. Moreover, the specific shapes and patterns of the guidance image do not affect the denoised PET image, because the guidance image is input to the network through an attention gate. In a Monte Carlo simulation of [$^{18}$F]fluoro-2-deoxy-D-glucose (FDG), the proposed method achieved the highest peak signal-to-noise ratio and structural similarity (27.92 $\pm$ 0.44 dB/0.886 $\pm$ 0.007), as compared with Gaussian filtering (26.68 $\pm$ 0.10 dB/0.807 $\pm$ 0.004), image guided filtering (27.40 $\pm$ 0.11 dB/0.849 $\pm$ 0.003), deep image prior (DIP) (24.22 $\pm$ 0.43 dB/0.737 $\pm$ 0.017), and MR-DIP (27.65 $\pm$ 0.42 dB/0.879 $\pm$ 0.007). Furthermore, we experimentally visualized the behavior of the optimization process, which is often unknown in unsupervised CNN-based restoration problems. For preclinical (using [$^{18}$F]FDG and [$^{11}$C]raclopride) and clinical (using [$^{18}$F]florbetapir) studies, the proposed method demonstrates state-of-the-art denoising performance while retaining spatial resolution and quantitative accuracy, despite using a common network architecture for various noisy PET images with 1/10th of the full counts. These results suggest that the proposed MR-GDD can reduce PET scan times and PET tracer doses considerably without impacting patients.
