Abstract:The effect of temperature and various glycerides to modulate the ability of lysolecithin and fatty acid to promote high phospholipase A2 activity was studied using dipalmitoylphosphatidylcholine large unilamellar vesicles as substrate. The length of the lag phase prior to the accumulation of sufficient hydrolysis products (lysolecithin and fatty acid) to support high phospholipase activity was shortest at temperatures near the thermotropic phase transition of the phospholipid substrate. A reduction in the lag phase correlated with a reduction in the requirement for hydrolysis products at the phase transition temperature, where the bilayer exists in a state of fluctuating domains of gel and liquid crystal. Dipalmitoylglycerol and tripalmitoylglycerol also reduced the length of the lag phase. This reduction was both concentration-dependent and temperature-dependent relative to the phase transition in the presence of the glycerides. As with the effect of temperature, the ability of di- and triglycerides to decrease the lag time correlated with a decrease in the amount of reaction products necessary to promote high phospholipase activity. This effect coincided with the tendency of the glycerides to form domains in the bilayer. Glycerides that did not form domains either had no effect (monopalmitoylglycerol) or increased the length of the lag phase (dicaprylglycerol). These data suggest that the effect of the reaction products to increase phospholipase A2 activity is aided by the presence of fluctuations in lipid domains within the bilayer.

Abstract:Aims and background Immunoglobulin heavy chain gene rearrangement serves as a marker of clonality and cell lineage in B-cell lymphoproliferative disorders. In this study we used the polymerase chain reaction (PCR) to detect clonal rearrangements of the immunoglobulin heavy chain gene in a group of patients with B-cell lymphomas. Methods DNA was extracted from frozen tissue of 40 B-cell non-Hodgkin's lymphomas and subjected to PCR amplification using primers that recognize conserved sequences of the variable and joining regions of the immunoglobulin heavy chain gene. Results Monoclonal rearrangements were detected in 23 of 40 malignant B-cell lymphomas. No clonal rearrangements were detected in the 10 control cases. Conclusions We conclude that this PCR-based technique may provide a simplified and rapid approach for the detection of clonal immunoglobulin heavy chain gene rearrangements in B-cell lymphomas without recourse to Southern blotting, which can be reserved for cases in which PCR is negative.

Abstract:Objective: This article describes the statistical analysis plan for the Biomarker-guided intervention to prevent AKI after major surgery (BigpAK-2) trial. Design: Adaptive trial design with an interim analysis after enrolment of 618 evaluable patients. Setting: The BigpAK.-2 trial is an international, prospective, randomised controlled multicentre study. Participants: The BigpAK-2 study enrols patients after major surgery who are admitted to the intensive care or high dependency unit and are at high-risk for postoperative AKI as identified by urinary biomarkers (tissue inhibitor of metalloproteinases-2 and insulin-like growth factor binding protein 7 ([TIMP-2]∗[IGFBP7]) will be enrolled. Intervention: Patients are randomly and evenly allocated to standard of care (control) group or the implementation of a nephroprotective care bundle (intervention group), as recommended by the Kidney Disease: Improving Global Outcomes (KDIGO) guidelines. The KDIGO care bundle recommends discontinuation of nephrotoxic agents if possible, ensuring adequate volume status and perfusion pressure, considering functional haemodynamic monitoring, regular monitoring of serum creatinine and urine output, avoiding hyperglycemia, and considering alternatives to radiocontrast procedures when possible. Results: The BigpAK-2 study investigates whether the biomarker-gudied implementation of the KDIGO care bundle reduces the incidence of moderate or severe AKI (stage 2 or 3), according to the KDIGO 2012 criteria, within 72 h after surgery. Conclusion: AKI is a common and often severe complication after major surgery. As no specific treatments exist, prevention of AKI is of high importance. The BigpAK-2 study investigates a promising approach to prevent AKI after major surgery. Trial registration: The trial was registered prior to start at clinicaltrials.gov; NCT04647396.

Abstract:Wide variations are observed in the potency of allergen preparations available for use on humans. There are numerous methods of in vivo or in vitro testing: skin testing, passive anaphylaxis testing on animals, study of precipitins, histamine release tests on human leukocytes, radio-immunological assay test (RAST), blasticlymphocyte transformation test (TTB). The first investigations seemed to show that the allergenic potency of a large number of inducing substances was due to proteic activity; this gave rise to the theory that potency could be evaluated by protein nitrogen units (PNU). However, the protein content of an allergenic extract is not always active. In addition, natural specific proteins are altered during the extraction process or subsequently during preservation. Where the active antigen is known, comparison of different testing with various preparations may be made. The results show excellent correlation between the two biological results (cutaneous reaction and histamine release) and the content of E antigen. No relation with PNU has been observed. Certain authors propose the following: skin testing, PNU titration, dosage of certain elements (polysaccharides, lipids, phosphates, etc.), RAST. The great differences observed between the value of skin testing and the RAST may be due to the large number of active antigens in different 'dusts' and to the fact that the RAST method measures only IgE whereas allergic activity of these allergens is no doubt associated with other types of antibodies. The complexity of the problem shows that a great many studies are still necessary to provide a satisfactory solution.
