Abstract:A vibro-tactile stimulation and vibro-signature synchronization device was proposed in this paper. The device was used to elicit steady-state somatosensory evoked potential (SSSEP) and synchronize the actual vibraion information with the corresponding electroencephalography (EEG) signals in temporal domain. The device provided five independent stimulation channels and could generate vibro-tactile stimulation with arbitrary waveform, amplitude and frequency. Each channel used the intended stimulation waveform to drive a linear resonant actuator (LRA). The vibro-signature of each channel could be detected by a force sensing resistor (FSR) and fed back into the EEG recording system. Four stimulation patterns with a random sequence were applied to the index finger of three healthy subjects. Results showed that SSSEP features could be evoked with different stimulation patterns and the actual vibro-signature could be synchronized with the EEG signals. Comparing the actual stimulation with the intended stimulation, the errors of carrier frequency and modulation frequency were quite small. Results indicated that this novel device stood a good chance of serving in SSSEP-based studies.

Abstract:Sensory stimulation played a critical role in both motivating subject's anticipation in brain-computer interface but also enhancing the sensory-motor interaction and closing the sensory motor loop. In this paper, mechanical vibrotactile stimulation effect in motor imagery was evaluated on 10 healthy subjects, and preliminary results showed that 5 subjects would achieve a reliable control above 80% with sensory stimulation as comparable with motor imagery without any stimulation. Besides, 3 subjects reached a better control with approximately 70% as compared with a chance level of 50% in motor imagery without sensory stimulation. Further analysis showed subject who was poor in conventional motor imagery condition exhibited enhanced R(2) value distribution in motor imagery with sensory stimulation condition. Meanwhile there was sensorimotor rhythmic enhancement both at upper alpha band and upper beta band in some subjects. But these rhythmic changes resulted performance reduction as incongruence of training and testing sets effect from off-line analysis. This research provided some guidance in integration of the sensory stimulation channel with motor imagery based BCI system.
Effect of Weight Bearing Vibration Stimulation on Surface Myoelectric Activation of Lower Limb Muscle Groups

Abstract:This paper aims to find the optimal vibration frequency and weight bearing of vibration stimulation. Methods: (1) Half squat, no weight bearing, 30% body weight, and 50% body weight, vibration frequencies 0 Hz, 30 Hz, 40 Hz, and 50 Hz; (2) Seven muscles: rectus femoris, vastus medialis, vastus lateralis, biceps femoris, tibialis anterior, and medial and lateral gastrocnemius. The results show that no weight bearing vibration was a significant change in the activation of the tibialis anterior and medial gastrocnemius muscles. 30% weight bearing activated the vastus medialis, vastus lateralis, and gastrocnemius medialis more significantly. 50% weight bearing and 40 Hz vibration had the most significant activation of the medial peroneal muscle. 30% weight bearing and 40 Hz vibration stimulated vastus medialis, biceps femoris, and lateral activation was better. 50 Hz vibration stimulation and 30% weight bearing activated the vastus medialis, biceps femoris better. Conclusion: (1) the activation of lower limb muscle groups tends to increase under weight bearing vibration conditions, especially in the quadriceps femoris muscle, and the vibration frequency is better at about 40 Hz. (2) under the stimulation conditions of changing vibration frequency, small load weight bearing had better activation of lower limb muscles at low vibration frequency, and large load weight bearing had better activation of lower limb muscles with increasing vibration frequency

Abstract:Objective: This study introduces a novel system that can simulate diverse mechanical properties of the human chest to enhance the experience of CPR training by reflecting realistic chest conditions of patients. Methods: The proposed system consists of Variable stiffness mechanisms (VSMs) and Variable damper (VD) utilizing stretching silicone bands and dashpot dampers with controllable valves to modulate stiffness and damping, respectively. Cyclic loading was applied with a robot manipulator to the system. Compression force and displacement were measured and analyzed to evaluate the system's mechanical response. Long-term stability of the system was also validated. Results: A non-linear response of the human chest under compression is realized through this design. Test results indicated non-linear force-displacement curves with hysteresis, similar to those observed in the chest of patients. Controlling the VSM and VD allowed for intentional changes in the slope and area of curves that are related to stiffness and damping, respectively. Stiffness and damping of the system were computed using performance test results. The stiffness ranged from 5.34 N/mm to 13.59 N/mm and the damping ranges from 0.127 N[Formula: see text] s/mm to 0.511 N[Formula: see text] s/mm. These properties cover a significant portion of the reported mechanical properties of the human chests. The system demonstrated satisfactory stability even when it was subjected to maximum stiffness conditions of the long-term compression test. Conclusion: The system is capable of emulating the mechanical properties and behavior of the human chests, thereby enhancing the CPR training experience.
In vitro comparative study of wall portbased high-frequency chest wall oscillation device and internal air-pulse generator device

Abstract:Owing to environmental and disease issues, the use of high-frequency chest wall oscillation (HFCWO) devices in hospitals is consistently increasing. This study proposes a cost-effective actuator-less HFCWO device that utilizes an external wall port utility in hospitals to generate the positive and negative pneumatic pressures required for HFCWO treatment instead of an embedded mechanical actuator. The manufactured prototype with the no-amplification (NO-AMP) setting contained an electric pressure regulator to enable intensity level adjustment and two solenoid valves to enable vibration frequency adjustment, whereas the prototype with the pre-amplification (PRE-AMP) setting contained an additional air reservoir and an air-pressure booster. The prototype device was tuned to output average local maximum values in the pressure waveform similar to a commercial VEST-205 device at an 8-12 Hz frequency and 2-4 pressure intensity levels. In vitro comparative experiments demonstrated that the prototype device showed similar local maximum pressures to those of the VEST-205 (mean absolute pressure difference, <3 mmH2O); in contrast, the proposed device showed significantly higher local minimum pressures than those of the VEST-205 (mean absolute pressure difference, >8 mmH2O). Additionally, the driving sound of the proposed device was 17.0-17.8 dB higher than that of VEST-205. We conclude that the proposed device has the potential to substitute for conventional HFCWO devices under the limited but most frequently used operating conditions, although more detailed modifications are necessary in future studies to improve its performance and clinical usability.
The Predictive Value of Serial Changes in Diaphragm Function During the Spontaneous Breathing Trial for Weaning Outcome: a Study Protocol

Abstract:Introduction There is a variety of tools being used in clinical practice for the prediction of weaning success from mechanical ventilation. However, their diagnostic performances are less than satisfactory. The purpose of this study is to investigate the value of serial changes in diaphragm function measured by ultrasound during the spontaneous breathing trial (SBT) as a weaning predictor. Methods and analysis This is a prospective observational study conducted in a 10-bed medical emergency intensive care unit (EICU) in a university-affiliated hospital. The study will be performed from November 2016 to December 2017. All patients in the EICU who are expected to have mechanical ventilation for more than 48 hours through endotracheal tube are potentially eligible for this study. Patients will be included if they fulfil the criteria for SBT. All enrolled patients will be ventilated with an Evita-4 by using volume assist control mode prior to SBT. Positive end-expiratory pressure (PEEP) will be set to 5 cmH2O and fractional inspired oxygen (FiO2) will be set to a value below 0.5 that guarantees oxygen saturation by pulse oximetry (SpO2) greater than 90%. Enrolled patients will undergo SBT for 2 hours in semirecumbent position. During the SBT, the patients will breathe through the ventilator circuit by using flow triggering (2 L/min) with automatic tube compensation of 100% and 5 cmH2O PEEP. The FiO2 will be set to the same value as used before SBT. If the patients fail to tolerate the SBT, the trial will be discontinued immediately and the ventilation mode will be switched to that used before the trial. Patients who pass the 2-hour SBT will be extubated. Right diaphragm excursion and bilateral diaphragm thickening fraction will be measured by ultrasonography during spontaneous breathing. Images will be obtained immediately prior to the SBT, and at 5, 30, 60, 90 and 120 min after the initiation of SBT. Rapid shallow breathing index will be simultaneously calculated at the bedside by a respiratory nurse. Ethics and dissemination The study protocol is approved by the ethics committee of Sir Run Run Shaw Hospital, an affiliate of Zhejiang University, Medical College. The results will be published in a peer-reviewed journal and shared with the worldwide medical community. Trial registration number ISRCTN42917473; Pre-results.

Abstract:BACKGROUND: Aging affects respiratory strength that could cause reduction in functional capacity and quality of life, playing a fundamental role in healthy aging and survival. To prevent these declines, the whole body vibration (WBV) has been proposed to increase strength and functional capacity. The aim of the study was to evaluate the effects of WBV on respiratory muscle strength, thoracoabdominal ventilation, and quality of life in the elderly adults.METHODS: This study was a controlled, randomized double-blind clinical trial. The study included 28 elderly adults randomized into three groups: Resistance (n = 9), WBV (n = 9), or WBV + resistance exercises (n = 10), performing training, sham, or double training for 3 months, twice per week. The variables of the study were as follows: maximal inspiratory and expiratory pressures (MIP and MEP), distribution of thoracoabdominal volumes variation in optoelectronic plethysmography (pulmonary rib cage-VRCp, abdominal rib cage-VRCa, and abdomen-VAB), and quality of life.RESULTS: After training, WBV and WBV + resistance groups increased MIP and MEP (p < .001). During inspiratory capacity maneuver, WBV groups had incremental increases in chest wall total volume (p < .001), showing a rise in pulmonary rib cage (p = .03) and abdominal rib cage (p = .04). Furthermore, WBV groups improved SF-36 scores in functional capacity, physical aspects, energy, pain, and general heath domains.CONCLUSIONS: The WBV is a training that could improve respiratory muscle strength and quality of life and promote different ventilatory strategies in chest wall and thoracoabdominal compartments in healthy elderly adults.
Non-Invasive Assessment of Abdominal/Diaphragmatic and Thoracic/Intercostal Spontaneous Breathing Contributions

Abstract:(1) Background: Technically, a simple, inexpensive, and non-invasive method of ascertaining volume changes in thoracic and abdominal cavities are required to expedite the development and validation of pulmonary mechanics models. Clinically, this measure enables the real-time monitoring of muscular recruitment patterns and breathing effort. Thus, it has the potential, for example, to help differentiate between respiratory disease and dysfunctional breathing, which otherwise can present with similar symptoms such as breath rate. Current automatic methods of measuring chest expansion are invasive, intrusive, and/or difficult to conduct in conjunction with pulmonary function testing (spontaneous breathing pressure and flow measurements). (2) Methods: A tape measure and rotary encoder band system developed by the authors was used to directly measure changes in thoracic and abdominal circumferences without the calibration required for analogous strain-gauge-based or image processing solutions. (3) Results: Using scaling factors from the literature allowed for the conversion of thoracic and abdominal motion to lung volume, combining motion measurements correlated to flow-based measured tidal volume (normalised by subject weight) with R2 = 0.79 in data from 29 healthy adult subjects during panting, normal, and deep breathing at 0 cmH2O (ZEEP), 4 cmH2O, and 8 cmH2O PEEP (positive end-expiratory pressure). However, the correlation for individual subjects is substantially higher, indicating size and other physiological differences should be accounted for in scaling. The pattern of abdominal and chest expansion was captured, allowing for the analysis of muscular recruitment patterns over different breathing modes and the differentiation of active and passive modes. (4) Conclusions: The method and measuring device(s) enable the validation of patient-specific lung mechanics models and accurately elucidate diaphragmatic-driven volume changes due to intercostal/chest-wall muscular recruitment and elastic recoil.

Abstract:BACKGROUND: The digital Auto-Trak™ system is a technology capable of automatically adjusting the triggering and cycling mechanisms during pressure support ventilation (PSV).OBJECTIVE: To compare Auto-Trak with conventional settings in terms of patient-ventilator synchrony and discomfort.METHODS: Twelve healthy volunteers underwent PSV via the mouth by breathing through an endotracheal tube. In the conventional setting, a pressure support of 8 cm H2O with flow cycling (25% peak inspiratory flow) and a sensitivity of 1 cm H2O was adjusted. In Auto-Trak the triggering and cycling were automatically set. Discomfort, effort of breathing, and the asynchrony index (AI) were assessed. In a complementary bench study, the inspiratory and expiratory time delays were quantified for both settings in three mechanical models: 'normal', obstructive (COPD), and restrictive (ARDS), using the ASL 5000 simulator.RESULTS: In the volunteer study the AI and the discomfort scores did not differ statistically between the two settings. In the bench investigation the use of Auto-Trak was associated with a greater triggering delay in the COPD model and earlier expiratory cycling in the ARDS model but with no asynchronic events.CONCLUSIONS: Use of the Auto-Trak system during PSV showed similar results in comparison to the conventional adjustments with respect to patient-ventilator synchrony and discomfort in simulated conditions of invasive mechanical ventilation.

Effects of Vibration Therapy on the Physical Function of Critically Ill Adults Trial: A Randomized Controlled Trial

Abstract:Objectives: Vibration therapy uses vibration to rehabilitate physical functions. Recently, it has been demonstrated to be safe for critically ill patients. However, its effects on physical functions are unclear. Design: Randomized controlled trial. Setting: A single-center, ICU. Patients: Patients were randomly assigned to either vibration therapy coupled with protocolized mobilization or protocolized mobilization alone. We included patients who could sit at the edge of the bed or in a wheelchair during their ICU stay. The exclusion criteria were based on the early mobilization inhibition criteria. Interventions: The primary outcome was the Functional Status Score for the ICU (FSS-ICU) at ICU discharge. Secondary outcomes were the Medical Research Council score, ICU-acquired weakness, delirium, ICU Mobility Scale (IMS), and ventilator- and ICU-free days. For safety assessment, vital signs were monitored during the intervention. Measurements and Main Results: Among 180 patients, 86 and 90 patients remained in the vibration therapy and control groups, respectively. The mean age was 69 ± 13 vs. 67 ± 16 years in the vibration therapy and control groups, and the Acute Physiology and Chronic Health Evaluation (APACHE) II score was 19 (14–25) vs. 18 (13–23). The total FSS-ICU at ICU discharge was 24 (18–27) and 21 (17–26) in the intervention and control groups, respectively ( p = 0.09), and the supine-to-sit ability significantly improved in the intervention group ( p < 0.01). The secondary outcomes were not significantly different. Vital signs remained stable during vibration therapy. In the predefined subgroup analysis, FSS-ICU improved in the population with a higher body mass index (≥ 23 kg/m 2 ), lower APACHE II scores (< 19), and higher IMS scores (≥ 6). Conclusions: Vibration therapy did not improve the total FSS-ICU. However, the supine-to-sit ability in the FSS-ICU improved without any adverse event.

Abstract:Background: Patient-ventilator asynchrony is common during pressure support ventilation (PSV) because of the constant cycling-off criteria and variation of respiratory system mechanical properties in individual patients. Automatic adjustment of inspiratory triggers and cycling-off criteria based on waveforms might be a useful tool to improve patient-ventilator asynchrony during PSV. Method: Twenty-four patients were enrolled and were ventilated using PSV with different cycling-off criteria of 10% (PS 10 ), 30% (PS 30 ), 50% (PS 50 ), and automatic adjustment PSV (PS AUTO ). Patient-ventilator interactions were measured. Results: The total asynchrony index (AI) and NeuroSync index were consistently lower in PS AUTO when compared with PS 10 , PS 30 , and PS 50 , ( P < 0.05). The benefit of PS AUTO in reducing the total AI was mainly because of the reduction of the micro-AI but not the macro-AI. PS AUTO significantly improved the relative cycling-off error when compared with prefixed controlled PSV ( P < 0.05). PS AUTO significantly reduced the trigger error and inspiratory effort for the trigger when compared with a prefixed trigger. However, total inspiratory effort, breathing patterns, and respiratory drive were not different among modes. Conclusions: When compared with fixed cycling-off criteria, an automatic adjustment system improved patient-ventilator asynchrony without changes in breathing patterns during PSV. The automatic adjustment system could be a useful tool to titrate more personalized mechanical ventilation.

Abstract:An educational augmented reality auscultation system (EARS) is proposed to enhance the reality of auscultation training using a simulated patient. The conventional EARS cannot accurately reproduce breath sounds according to the breathing of a simulated patient because the system instructs the breathing rhythm. In this study, we propose breath measurement methods that can be integrated into the chest piece of a stethoscope. We investigate methods using the thoracic variations and frequency characteristics of breath sounds. An accelerometer, a magnetic sensor, a gyro sensor, a pressure sensor, and a microphone were selected as the sensors. For measurement with the magnetic sensor, we proposed a method by detecting the breathing waveform in terms of changes in the magnetic field accompanying the surface deformation of the stethoscope based on thoracic variations using a magnet. During breath sound measurement, the frequency spectra of the breath sounds acquired by the built-in microphone were calculated. The breathing waveforms were obtained from the difference in characteristics between the breath sounds during exhalation and inhalation. The result showed the average value of the correlation coefficient with the reference value reached 0.45, indicating the effectiveness of this method as a breath measurement method. And the evaluations suggest more accurate breathing waveforms can be obtained by selecting the measurement method according to breathing method and measurement point.

Abstract:Some evidence has demonstrated that focal vibration (FV) plays an important role in the mitigation of spasticity. However, the research on developing the FV system to mitigate the spasticity effectively has been seldom reported. To relieve post-stroke spasticity, a new pneumatic FV system has been proposed in this paper. An image processing approach, in which the edge of vibration actuator was identified by the Canny edge detector, was utilized to quantify this system's parameters: the frequency ranging from 44 Hz to 128 Hz and the corresponding amplitude. Taking one FV protocol with the frequency of 87 Hz and the amplitude 0.28 mm of this system as an example, a clinical experiment was carried out. In the clinical experiment, FV was applied over the muscle belly of the antagonist of spastic muscle for twelve chronic spastic stroke patients. Spasticity was quantified by the muscle compliance and area under the curve for muscle (AUC_muscle). The result has demonstrated that, in the state of flexion of spastic muscle, the AUC_muscle and muscle compliance of the spastic muscle significantly increased immediately after FV compared with before-FV, illustrating the mitigation of the spasticity. This study will not only provide a potential tool to relieve post-stroke spasticity, but also contribute to improving the sensory and motor function of patients with other neurological diseases, e.g. spinal cord injury, multiple sclerosis, Parkinson and dystonia, etc.

Preliminary Study of an Adjustable, Wearable, Noninvasive Vest Providing Chest Compression to Assist with Breathing

Abstract:Respiratory muscle paralysis caused by acute cervical spinal cord injury usually leads to pulmonary ventilation dysfunction and even death from respiratory failure. In addition to invasive treatments such as mechanical ventilation, the utilization of noninvasive respiratory support equipment plays an important role in long-term assisted breathing. In this study, we describes a wearable, noninvasive vest with adjustable pressure that enables assisted breathing and with an automatic alarm, and we aims to explore its safety and effectiveness on healthy adult participants. The vest monitors the human heart rate and the blood oxygen index data in real time, the alarm is automatically activated when the data is abnormal. Eight healthy participants had no obvious discomfort during the test while wearing the vest. Lung volumes, antero-posterior diameters, and left-right diameters at the second, fourth, and sixth ribs levels were acquired before and after inflation of the vest airbag, the data acquired by the imaging analysis using chest computed tomography showed significant differences before and after the inflation (p < 0.05). Thus, The vest designed for this study can achieve uniform and effective compression of the thorax, significantly changed the size of the thorax and lungs. It is expected to be applied as noninvasive support for patients with respiratory dysfunction.
Effects Of Vibratory Sputum Ejection As An Assisted Therapy For Severe Pneumonia Patients On Mechanical Ventilation And The Influence On Respiratory Function

Abstract:Objective: The current study aimed to investigate the effects of vibratory sputum ejection (VSE), as an assisted therapy for patients with severe pneumonia, on mechanical ventilation and its influence on respiratory function. Methods: Seventy patients with severe pneumonia on mechanical ventilation were randomly divided into the experimental group (n=35) and control group (n=35). Patients in the control group received fiberoptic bronchoalveolar lavage (FBL), while patients in the experimental group underwent combined treatment of FBL with VSE. Vital signs, blood-gas parameters, respiratory function, inflammatory response, and prognosis of the two groups were compared. Results: There were no significant differences in respiratory rates, heart rates, and mean arterial pressure between the two groups 15 minutes before and 15 minutes after treatment. Levels of PaO2, PaO2/FiO(2), FEV1, and FVC in both groups were significantly increased after 7 days of treatment, compared with levels before treatment (all P=0.001). Levels in the experimental group were significantly higher (all P<0.01). Levels of PaCO2 and airway resistance decreased in both groups (all P=0.001) and the experimental group enjoyed significantly lower levels than the control group (both P<0.01). Levels of C-reactive protein and procalcitonin in both groups were significantly decreased after 24 hours of treatment (all P=0.001). The experimental group enjoyed significantly lower levels than the control group (both P<0.01). In the experimental group, the effective rate was significantly higher than that of the control group and 28-day mortality was significantly lower than that of the control group (both P<0.05). Conclusion: FBL combined with VSE for patients with severe pneumonia on mechanical ventilation can significantly reduce inflammation, contribute to the recovery of respiratory function, and improve prognosis. It is worthy of clinical promotion.
Acute Effects of Handheld Vibration Massage on Posterior Shoulder Soft Tissues

Abstract:Background: Interventions using vibration stimulation have been recognized for their potential for increasing range of motion (ROM) without compromising muscle strength. Handheld vibration massagers can efficiently deliver vibration therapy to the shoulder joint and may be a potential treatment. Purpose: To evaluate the effects of vibration massage using a handheld device on the soft tissues of the posterior shoulder joint, particularly on internal rotation (IR) passive ROM and external rotation (ER) muscle strength. Study design: Crossover study design. Methods: A crossover study with a 5-min vibration massage and passive control condition was conducted in healthy male volunteers (mean age 20.5 ± 1.7 years). Vibration massage was applied to the posterior shoulder soft tissues of the dominant arm, with no intervention under control conditions. IR-ROM (vertebral level and in abduction) and strength of the external rotators (isometric and isokinetic) were measured before and immediately after the intervention. Vertebral levels were calculated as a ratio of lengths (ratio decreases with increased mobility). IR-ROM in abduction, the angle was measured. Statistical analysis was performed with two-way repeated measures ANOVA and paired t-test (Bonferroni correction). Results: Vibration application decreased (improved) vertebral level IR ROM by -4.1% (p < 0.01, d = 0.445) and increased abduction position IR ROM by 11.4° (p < 0.01, d = 0.694). These changes exceeded the 95% confidence interval for the minimum detectable change. By contrast, the control condition produced no changes. IR-ROM (vertebral level and abduction) immediately after the intervention showed significant differences between the control and vibration conditions (p = 0.036, d = 0.273; p = 0.048, d = 0.483, respectively). Muscle strength did not show any interaction, time, or between-condition effects. Conclusions: A massage using a handheld vibration massager applied to the posterior shoulder soft tissues increased IR-ROM without negatively affecting muscle strength, suggesting its potential use as a means of warming up. Sensor Systems for Automatic Control of Abdominal Stimulation for Respiratory Support in Tetraplegia

Abstract:This thesis describes the evaluation of using an inertial measurement unit (IMU) device to measure the movement of the abdomen with the aim to detect different breathing activities and to demonstrate the feasibility to use this technique for automatic control of Functional Electrical Stimulation of Abdominal Muscles (FESAM). People with high-level spinal cord injury (SCI) have difficulties on voluntary breathings, as well as forced respiration, such as cough. The method of FESAM can improve respiratory function. Respiratory activity can be obtained directly by measuring the airflow at the mouth and nose, using a face mask connected to a spirometer. While this approach is suitable in a laboratory environment, the face mask is inconvenient for long-term, every day use. An alternative way to detect respiratory activity is to measure the movement of the abdomen, which is less intrusive and more comfortable. Plethysmography is typically used to measure such movement in sleep studies. In this work, the suitability of an IMU sensor device attached to the abdomen is investigated. Experiments were conducted with 5 neurologically intact subjects with both an IMU and spirometer device. Signals recorded from both sensors during different breathing tasks such as quiet breathing, cough, deep breathing and talking are compared. The phase shifts between the signals from the two sensors are analysed and found to be within +/-U. Analysis of the magnitude of the IMU signals and their power spectrum confirm that it is possible to represent different breathing activities with these sensors. A control system which can detect breathing activity in real-time, and controls a stimulator to generate appropriate electrical stimulations to the abdominal muscles, is also presented in this thesis. A multi characteristic-analysis algorithm has been developed. This enhanced control system can analyse multiple characteristics of the breathing signal in real-time, and uses a flowchart structure to detect breathing activities. The results are used to control the stimulator which delivers suitable electrical stimulations during quiet breathing and coughing. A graphical user interface (GUI) was implemented to interface with the sensor system, control system and stimulator system. This GUI was designed to graphically control the parameters of the entire system, and to show the system results visually. By using the GUI, the entire control system is more accessible to non-technical people. In addition, the control of the three systems becomes easier and is simplified. The possibility to save and load profiles for different patients also makes the configuration of the system more convenient.
Inspiratory response and side-effects to rapid bilateral magnetic phrenic nerve stimulation using differently shaped coils: implications for stimulation-assisted mechanical ventilation

Abstract:Background: Rapid magnetic stimulation (RMS) of the phrenic nerves may serve to attenuate diaphragm atrophy during mechanical ventilation. With different coil shapes and stimulation location, inspiratory responses and side-effects may differ. This study aimed to compare the inspiratory and sensory responses of three different RMS-coils either used bilaterally on the neck or on the chest, and to determine if ventilation over 10 min can be achieved without muscle fatigue and coils overheating. Methods: Healthy participants underwent bilateral anterior 1-s RMS on the neck (RMSBAMPS) (N = 14) with three different pairs of magnetic coils (parabolic, D-shape, butterfly) at 15, 20, 25 and 30 Hz stimulator-frequency and 20% stimulator-output with + 10% increments. The D-shape coil with individual optimal stimulation settings was then used to ventilate participants (N = 11) for up to 10 min. Anterior RMS on the chest (RMSaMS) (N = 8) was conducted on an optional visit. Airflow was assessed via pneumotach and transdiaphragmatic pressure via oesophageal and gastric balloon catheters. Perception of air hunger, pain, discomfort and paresthesia were measured with a numerical scale. Results: Inspiration was induced via RMSBAMPS in 86% of participants with all coils and via RMSaMS in only one participant with the parabolic coil. All coils produced similar inspiratory and sensory responses during RMSBAMPS with the butterfly coil needing higher stimulator-output, which resulted in significantly larger discomfort ratings at maximal inspiratory responses. Ten of 11 participants achieved 10 min of ventilation without decreases in minute ventilation (15.7 ± 4.6 L/min). Conclusions: RMSBAMPS was more effective than RMSaMS, and could temporarily ventilate humans seemingly without development of muscular fatigue. Paced breathing and respiratory movement responses evoked by bidirectional constant current stimulation in anesthetized rabbits

Abstract:Objective: Diaphragm pacing (DP) is a long-term and effective respiratory assist therapy for patients with central alveolar hypoventilation and high cervical spinal cord injury. The existing DP system has some limitations, especially high price, inconvenience preoperative evaluation methods and diaphragm fatigue easily. Our objective was to develop a DP system and evaluated reliability through hardware testing and animal experiments. Methods: A DP system with bidirectional constant current was designed, manufactured and tested. Effects of a wide range of stimulus amplitudes (range: .5-2.5 mA) and frequencies (range: 10-250 Hz) on airflow and corresponding inspired volume were investigated during DP. Differences in airflow characteristics under various stimulation parameters were evaluated using power function. ECG interference in diaphragm electromyography (EMGdi) was filtered out using stationary wavelet transform to obtain pure EMGdi (EMGdip). 80-min period with a tendency for diaphragm fatigue by root mean square (RMS) and centroid frequency (f c ) of EMGdip was studied. Results: The increase of stimulus frequency and amplitude in animals resulted in different degrees of increase in envoked volume. Significant difference in Airflow Index (b) between anesthesia and DP provided a simple, non-invasive and feasible solution for phrenic nerve conduction function test. Increased stimulation duration with the developed DP system caused less diaphragm fatigue. Conclusion: A modular, inexpensive and reliable DP was successfully developed. Its effectiveness was confirmed in animal experiments. Significance: This study is useful for design of future implantable diaphragmatic pacemakers for improving diaphragm fatigue and convenient assessment of respiratory activity in experiments.
