Abstract:Objectives: This study aimed to assess indoor air quality (IAQ) in long-term care facilities (LTCFs) in California during the COVID-19 pandemic and evaluate their implementation of IAQ best practices described by public health authorities to control respiratory pathogen transmission via inhalation. Design: This observational study conducted IAQ assessments in a convenience sample of LTCFs to gather qualitative data on the implementation of IAQ best practices. The design included 5 pilot visits to develop a standardized method of data collection and then systematic data collection at 10 facilities. Setting and participants: The study focused on 10 LTCFs across California, chosen from facilities that responded to flyers advertising free IAQ assessments. Some of the facilities had previously experienced COVID-19 outbreaks affecting residents and staff. Methods: State health department industrial hygienists performed site visits to collect data on each facility's heating, ventilation, and air-conditioning (HVAC) system operation, outdoor air introduction, recirculated air filtration, use of portable air cleaners, and directional airflow in isolation areas to evaluate implementation of IAQ best practices in each of these areas. Qualitative data were obtained through visual inspections and interviews with maintenance personnel. Results: Findings indicated suboptimal implementation of IAQ best practices across the assessed facilities: no facility operated HVAC systems continuously, 40% had all outdoor air dampers open, 20% used MERV-13 or higher rated filters, 20% used portable air cleaners, and 20% performed directional airflow assessment and management for isolating COVID-19 cases. Conclusions and implications: Most LTCFs assessed were not adhering to IAQ best practices, highlighting a significant opportunity for improvement. IAQ best practices described in this study are achievable with existing systems and are critical for reducing virus transmission through the air in LTCFs. The findings underscore the need for more systematic assessments and improvements in IAQ within LTCFs to protect staff and residents.

Abstract:Abstract Background: We are a tertiary care cancer center and have approximately 1000–1500 emergency visits by cancer patients undergoing treatment under the adult medical oncology unit each month. However, due to the lack of a systematic audit, we are unable to plan steps toward the improvement in quality of emergency services, and hence the audit was planned. Methods: All emergency visits under the adult medical oncology department in the month of July 2015 were audited. The cause of visit, the demographic details, cancer details, and chemotherapy status were obtained from the electronic medical records. The emergency visits were classified as avoidable or unavoidable. Descriptive statistics were performed. Reasons for avoidable emergency visits were sought. Results: Out of 1199 visits, 1168 visits were classifiable. Six hundred and ninety-six visits were classified as unavoidable (59.6%, 95% CI: 56.7–62.4), 386 visits were classified as probably avoidable visit (33.0%, 95% CI: 30.4–35.8) whereas the remaining 86 (7.4%, 95% CI: 6.0–9.01) were classified as absolutely avoidable. Two hundred and ninety-seven visits happened on weekends (25.6%) and 138 visits converted into an inpatient admission (11.9%). The factors associated with avoidable visits were curative intention of treatment (odds ratio - 2.49), discontinued chemotherapy status (risk ratio [RR] - 8.28), and private category file status (RR – 1.89). Conclusion: A proportion of visits to emergency services can be curtailed. Approximately one-fourth of patients are seen on weekends, and only about one-tenth of patients get admitted.

Abstract:The rapid global spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) at the beginning of 2020 presented the world with its greatest health challenge in decades. It soon became clear that governments were unprepared to respond appropriately to this crisis. National and international public health authorities were confused about the transmission routes of the virus and the control measures required to protect against it. In particular, the need to reduce the risk of infection through sufficient and effective ventilation of indoor spaces was given little attention. In this review, we discuss insights and key lessons learned from the COVID-19 pandemic regarding the role of ventilation as an effective means against airborne transmission of pathogens and, more broadly, for supporting good indoor air quality.

Abstract:Maintaining indoor environmental (IEQ) quality is a key priority in educational buildings. However, most studies rely on outdoor measurements or evaluate limited spatial coverage and time periods that focus on standard occupancy and environmental conditions which makes it hard to establish causality and resilience limits. To address this, a fine-grained, low-cost, multi-parameter IOT sensor network was deployed to fully depict the spatial heterogeneity and temporal variability of environmental quality in an educational building in Sydney. The building was particularly selected as it represents a multi-use university facility that relies on passive ventilation strategies, and therefore suitable for establishing a living lab for integrating innovative IoT sensing technologies. IEQ analyses focused on 15 months of measurements, spanning standard occupancy of the building as well as the Black Summer bushfires in 2019, and the COVID-19 lockdown. The role of room characteristics, room use, season, weather extremes, and occupancy levels were disclosed via statistical analysis including mutual information analysis of linear and non-linear correlations and used to generate site-specific re-design guidelines. Overall, we found that 1) passive ventilation systems based on manual interventions are most likely associated with sub-optimum environmental quality and extreme variability linked to occupancy patterns, 2) normally closed environments tend to get very unhealthy under periods of extreme pollution and intermittent/protracted disuse, 3) the elevation and floor level in addition to room use were found to be significant conditional variables in determining heat and pollutants accumulation, presumably due to the synergy between local sources and vertical transport mechanisms. Most IEQ inefficiencies and health threats could be likely mitigated by implementing automated controls and smart logics to maintain adequate cross ventilation, prioritizing building airtightness improvement, and appropriate filtration techniques. This study supports the need for continuous and capillary monitoring of different occupied spaces in educational buildings to compensate for less perceivable threats, identify the room for improvement, and move towards healthy and future-proof learning environments.

Abstract:Building Services Engineering Research and Technology, Ahead of Print. To assess risk factors for COVID-19 transmission and address the closure of mass gathering events since March 2020, the UK Government ran the Events Research Programme (ERP), following which it reopened live events in sports, music, and culture in July 2021. We report the rapid post-occupancy evaluation of Indoor Air Quality (IAQ) and associated long-range airborne transmission risk conducted in the Environmental Study of the ERP. Ten large venues around the UK were monitored with CO2 sensors at a high spatial and temporal resolution during 90 events. An IAQ Index based on CO2 concentration was developed, and all monitored spaces were classified in bands from A to G based on their average and maximum CO2 concentrations from all events. High resolution monitoring and the IAQ Index depicted the overall state of ventilation at live events, and allowed identification of issues with ventilation effectiveness and distribution, and of spaces with poor ventilation and the settings in which long-range airborne transmission risk may be increased. In numerous settings, CO2 concentrations were found to follow patterns relating to event management and specific occupancy of spaces around the venues. Good ventilation was observed in 90% of spaces monitored for given occupancies.Practical applications: High-resolution monitoring of indoor CO2 concentrations is necessary to detect the spatial variation of indoor air quality (IAQ) in large mass gathering event venues. The paper summarises COVID-19 ventilation guidance for buildings and defines a methodology for measurement and rapid assessment of IAQ during occupancy at live events that can be implemented by venue managers. Comparisons of the CO2 concentrations measured during the events identified the spaces at high risk of long-range transmission of airborne pathogens. Building operators should be mindful of the ventilation strategies used relative to the total occupancy in different spaces and the occupant's activities.

Abstract:The COVID-19 pandemic drew attention to the critical role of building ventilation as a measure for controlling infection transmission. With the substantial number of COVID-19 outbreaks in care homes worldwide, the effectiveness of ventilation is an important consideration for infection control and wider exposure to indoor air pollutants. In this study, we used IoT-based sensors in two residential care homes to evaluate ventilation in various areas, including bedrooms, corridors, and communal spaces. Our monitoring focused on carbon dioxide (CO 2 ) levels as a proxy for ventilation, as well as temperature and humidity, during the spring of 2022. We also developed a ventilation model using the software CONTAM and coupled it with an infection risk model to assess airborne transmission risks under different weather and occupancy conditions. Our results suggest that ventilation is generally adequate based on UK COVID-19 guidelines at the time, with CO 2 below 800 ppm for the majority of the time, and opening windows in communal spaces in elderly care environments can help preserve indoor ventilation during periods of high occupancy. However, modelling data suggests that low CO 2 values may be indicative of low occupancy in many spaces and therefore ventilation rates may not be sufficient to mitigate infection transmission. Encouraging positive ventilation behaviours in staff and residents, potentially supported by visible CO 2 monitors, and taking additional precautions such as using air cleaners, enabling additional window openings or staff wearing masks during outbreaks and periods of high disease prevalence is likely to be beneficial for resident and staff health.

Abstract:Classrooms are often under-ventilated, posing risks for airborne disease transmission as schools have reopened amidst the COVID-19 pandemic. While technical solutions to ensure adequate air exchange are crucial, this research focuses on teachers' perceptions and practices that may also have important implications for achieving a safe classroom environment. We report on a (pre-pandemic) survey of 84 teachers across 11 California schools, exploring their perceptions of environmental quality in relation to monitored indoor environmental quality (IEQ) data from their classrooms. Teachers were not educated regarding mechanical ventilation. Errors in HVAC system installation and programming contributed to misunderstandings (because mechanical ventilation was often not performing as it should) and even occasionally made it possible for teachers to turn off the HVAC fan (to reduce noise). Teachers did not accurately perceive (in)sufficient ventilation; in fact, those in classrooms with poorer ventilation were more satisfied with IEQ, likely due to more temperature fluctuations when ventilation rates were higher combined with occupants' tendency to conflate perceptions of air quality and temperature. We conclude that classroom CO<sub>2</sub> monitoring and teacher education are vital to ensure that teachers feel safe in the classroom and empowered to protect the health of themselves and their students.

Abstract:Building Services Engineering Research and Technology, Ahead of Print. During a disruption, actionable insights generated from real-time data of the disrupted system can be used to dynamically recalibrate mitigation and recovery responses but there is currently a paucity of investigation on such assessment and management of resilience in real time. In this study, we propose the concept of real-time resilience to encapsulate the capacity of a disrupted system to continuously recalibrate its responses and minimize its damage. Quantitative metrics to assess the real-time resilience are also established. Subsequently, a digital-twin-based control for mechanical ventilation systems was developed as a tool to enable real-time resilience against airborne infection in indoor spaces. For demonstration, numerical simulations were performed with the adoption of the new tool in an indoor food court. Results showed that the gross resilience of the diners was enhanced in terms of improvements to the metrics of disruption duration (26%–61%), loss of resilience (2%–39%), and average rate of recovery (26%–74%). At the same time, the tempo-spatial variations suggested that increasing the ventilation rate increased the dilution and dispersion of infectious aerosols simultaneously, which can have opposing effects on individual resilience depending on the diner's location. The trade-off between real-time resilience and energy use was discussed based on the results. Practical applications: This study proposed a new tool based on the concept of real-time resilience to control ventilation to mitigate the indoor transmission of airborne infectious disease. The tool utilized numerical simulations to assess the tempo-spatial variation of infection risks and determine the adaptive changes needed for risk mitigation based on the predictive assessment. The evaluation of the tool using the proposed metrics of real-time resilience was demonstrated and the results showed that adoption of the tool can lead to improvements in disruption duration, loss of resilience, and average rate of recovery for diners in a food court.

Abstract:Over the past four years, five major variants of the SARS-CoV-2 virus have circulated globally, causing seven million deaths. Meanwhile, specific preventative ventilation measures were recommended to minimize exposure to the aerosol infection risk, such as opening doors, and disabling ventilation systems. Given more attention to high-rise buildings, where quite a few outbreaks have been reported, there is a notable lack of reported studies assessing preventive ventilation measures. Therefore, this study focused on practicing several ventilation measures in a 16-story high-rise building located in Montreal, Canada. A variety of inter-zonal tracer measurements, including room-to-floor and inter-floor tests, were carried out considering controlled airflow/pressure across zones. An automated data acquisition system was developed for real-time monitoring of tracer/CO 2 spatial concentrations. Findings show that enabled ventilation with opened doors reduces CO 2 concentrations by up to 82 % in nearby source locations and positively pressurized zones; however, negatively pressurized zones experience a 40 % increase in tracer exposure. With ventilation active, a corridor source leads to a higher risk of tracer exposure (up to 86 %) than the in-room source. Disabled ventilation with closed doors is a recommended ventilation measure, for corridors with infection sources, to minimize tracer transport across zones. Zones with infection sources follow the latter ventilation measure; however, ventilation with open doors offers a conditional ventilation measure if careful airflow is designed particularly at return and exhaust points within the ventilation system. This study has implications for practicing preventative ventilation measures for post-COVID-19 building practices and design considerations.

The Cost-effectiveness of Improving Indoor Air Ventilation for the Prevention of Covid-19 in Commercial Spaces

Abstract:Abstract Objective : Airborne infection from aerosolized SARS-CoV-2 poses an economic challenge for the large number of small businesses without existing heating, ventilation, and air conditioning (HVAC) systems. Standalone units have been recommended, but are costly and may not achieve adequate air exchange to prevent infection. Study design : Cost-effectiveness analysis with Monte Carlo simulation. Methods : We built a probabilistic decision-analytic model in a Monte Carlo simulation that examines aerosol transmission of SARS-CoV-2 in an indoor space. As a case study, we modeled a poorly ventilated indoor restaurant. We evaluated the cost-effectiveness of improving ventilation rate to 12 air changes per hour (ACH), the equivalent of hospital-grade filtration systems used in emergency departments. Our base-case scenario assumes that the mean year-round prevalence of actively infectious cases in the community surrounding the restaurant is 1%, and that 50% of patrons were fully vaccinated. Our best-case scenario assumes a community prevalence of 0.1% and 70% of patrons vaccinated, and our worst-case scenario assumes a community prevalence of 2% and 0% of patrons vaccinated. We also provide a customizable online tool. Results : All 3 scenarios resulted in a net cost-savings and infections averted. For the base-case scenario, improving ventilation to 12 ACH was associated with 33 (95% Credible Interval [CrI]: 18-50) aerosol infections averted over one year, producing an estimated cost savings of -$97,467 (95% CrI: -$156,760, -$50,877) and 0.84 (95% CrI: 0.46, 1.40) quality-adjusted life years (QALYs) gained. Conclusions : It is cost-effective to improve indoor ventilation in small businesses in older buildings that lack HVAC systems during the pandemic.
Evaluating a peer-to-peer health education program in Australian public housing communities during the COVID-19 pandemic

Abstract:Background: The cohealth Health Concierge program operated in Melbourne, Australia from July 2020 to 30 June 2022. It provided peer-to-peer support to culturally and linguistically diverse residents of high-rise public housing. During this time, the COVID-19 public health response changed frequently and included movement restriction, testing and vaccination. We conducted a mixed-methods evaluation to determine the Health Concierge program's impact on residents' engagement with health services and public health activities. Methods: The evaluation, informed by a Project Reference Group, used the RE-AIM framework. We analysed data from 20,901 routinely collected forms describing interactions between Concierges and residents from August 2021 to May 2022. Additional evaluation-specific data were collected between March and May 2022 in four housing estates; we surveyed 301 residents and conducted 32 interviews with residents, Concierges and program stakeholders. Results: Concierges promoted COVID-safe behaviours; linked residents with support, testing and vaccination services; and disseminated up-to-date information. Of the 20,901 recorded interactions, 8,872 (42%) included Concierges providing support around COVID-19 vaccination. Most surveyed residents (191/301, 63%) reported speaking with a Concierge in the previous six months. The self-reported two-dose COVID-19 vaccine uptake was 94% (283/301). Some residents described having meaningful, appreciated conversations with Concierges, and some described superficial interactions. While residents initially welcomed the program, many felt it failed to evolve. Poorly defined management and hiring criteria led to variable program implementation. A need for bicultural workers to continue linking residents with services was discussed. Conclusions: Concierges' impact on residents may have contributed to high community uptake of COVID-19 testing and vaccination, and had benefits beyond the COVID-19 remit. We recommend the program be revised and continued to inform further preparedness planning and support service access generally. Program models such as this have potential to inform and reassure high-risk communities during a pandemic. In addition, such programs can help overcome vaccine hesitancy and promote protective health behaviours, regardless of whether a pandemic is currently occurring. Ensuring these programs remain responsive to the changing needs of end-users needs over time is imperative.
A review of facilities management interventions to mitigate respiratory infections in existing buildings

Abstract:The Covid-19 pandemic reveals that the hazard of the respiratory virus was a secondary consideration in the design, development, construction, and management of public and commercial buildings. Retrofitting such buildings poses a significant challenge for building owners and facilities managers. This article reviews current research and practices in building operations interventions for indoor respiratory infection control from the perspective of facilities managers to assess the effectiveness of available solutions. This review systematically selects and synthesises eighty-six articles identified through the PRISMA process plus supplementary articles identified as part of the review process, that deal with facilities' operations and maintenance (O&M) interventions. The paper reviewed the context, interventions, mechanisms, and outcomes discussed in these articles, concluding that interventions for respiratory virus transmission in existing buildings fall into three categories under the Facilities Management (FM) discipline: Hard services (HVAC and drainage system controls) to prevent aerosol transmissions, Soft Services (cleaning and disinfection) to prevent fomite transmissions, and space management (space planning and occupancy controls) to eliminate droplet transmissions. Additionally, the research emphasised the need for FM intervention studies that examine occupant behaviours with integrated intervention results and guide FM intervention decision-making. This review expands the knowledge of FM for infection control and highlights future research opportunities.

Ventilation and Public Health: A Fraught History

Abstract:Since the beginning of the COVID-19 pandemic, a vigorous public health discussion has arisen over indoor air quality and ventilation. In popular press articles, bestselling books, and the US Environmental Protection Agency's recently announced Clean Air in Buildings Challenge, scholars and policy experts have claimed that improved ventilation systems can lead to better productivity and performance. By reevaluating those claims in light of the history of public health in Great Britain and the United States, we found that better ventilation has frequently been proposed as a cost-effective and nonintrusive means of improving health in institutions experiencing structural and environmental public health problems. Furthermore, our examination of efforts to provide ventilation for enslaved people, incarcerated people, and the urban poor revealed a consistent lack of government regulation and a disassociation of air quality concerns from broader environmental, social, and economic realities. Abstract:COVID-19 has already claimed over 7 million lives and has infected over 775 million people globally [1]. SARS-CoV-2, the virus that causes Covid-19, spreads primarily through droplets from infected people's airways, rendering Heating, Ventilation, and Air Conditioning (HVAC) systems critical in controlling infection risk levels in the indoor environment. To understand the dynamics of exhaled droplets and aerosols and the percentage of particles that are inhaled, escaped, recirculated, or trapped on different surfaces for a variety of environmental settings, we have presented our findings from the Computational Fluid Dynamics (CFD) modeling to investigate the impact of changing HVAC parameters in this paper. When combined with the spatial and temporal distribution of droplets, this method can be used to assess the potential risk and strengthen resilience. This finding demonstrates the viability and usefulness of CFD for modeling the distribution and dynamics of droplets and aerosols in confined environments. Our study demonstrates that raising the Air Change per Hour (ACH) from 2 to 8 reduces the risk of particle inhalation by nearly 70 %. Additionally, limiting the amount of air recirculation or increasing the amount of fresh air helps to reduce the number of airborne particles in an indoor space. To reduce the potential for respiratory droplet-related transmission and to provide relevant recommendations to the appropriate authority, the same computational approach could be applied to a wide range of ventilated indoor environments such as public buses, restaurants, exhibitions, and theaters.
Real-time Monitoring and Controlling Strategies of Indoor Environment in the Frontline Hospitals During COVID-19 Pandemic

Abstract:Many emergent measures on mitigating and controlling the COVID-19 pandemic have been unprecedented in China, one of which is the urgent construction of isolation wards in Huoshenshan Hospital and Leishenshan Hospital, as well as in sixteen Fangcang shelter hospitals. These hospitals, built or reconstructed within a remarkably short period, have treated a large number of COVID-19 patients in time and successfully mitigated disease spread in population. One of the many challenges during the hospitals' functioning period is the precaution of potential airborne transmission. Airborne transmission could occur during medical procedures including tracheotomy, intubation, extubation, etc., and several surveillance studies have detected SARS-CoV-2 virus in air by polymerase chain reaction (PCR), but direct methods on monitoring airborne SARS-CoV-2 in real-time have not been developed so far. Though it is yet to know whether these airborne viruses are viable and able to cause person-to-person infection, airborne precautions such as negative-pressure wards and N95 respirators have already been recommended and implemented in China. In order to protect the healthcare workers (HCWs) and reduce the demand of high-level protection gear, this study proposed an indirect method to identify and alert the airborne infection risk. We deployed numerous sensors to monitor CO2 concentration, PM2.5 mass concentration, air temperature and relative humidity, and illumination at ICUs (intensive care units), general wards, nurse stations, buffer rooms, etc. in real-time. Online CO2 concentration reflects the dilution of occupants' exhalation substances by the ventilation and air distribution system. High CO2 concentrations indicate either the fresh air supply fails or the room is overcrowded at the moment. The readings of CO2 concentration across adjacent zones with known functions can also help identify whether the airflow direction is consistent with the designed requirement. PM2.5 mass concentration indicates the air cleanness of the measured zones, which could be crucial for patient recovery and reduction of symptom complication. Air temperature and relative humidity are key parameters that influence the evaporation of expiratory droplets and the thermal comfort of patients, while illumination indicates the intake of ultraviolet from outdoor. The sensors could keep quiet during the sampling process, so they would not disturb the HCWs and patients. Moreover, they are integrated with 3G SIM cards and powered just by a DC 5V/3A power adapter with a USB Type-C line, so they could be easily installed at target locations, and adapt for multi-scene, multi-demand, and long-term online and offline monitoring. When integrated with online meta-analysis and computer fluid dynamics (CFD) simulation, this method can send alarms to the HCWs about the locations where the ventilation against potential pathogen-laden aerosols fails. This study introduces the principle of the method in detail as well as its application at Huoshenshan Hospital, Leishenshan Hospital and Jiangxia Dahuashan Fangcang shelter hospital, etc., in Wuhan, China, during February and March of 2020. As the society gradually steps into post-pandemic phase, the proposed method could help optimize the ventilation and filtration strategies in public buildings and transportations.
Emergency Department Environmental Responses to COVID-19 Pandemic: A Qualitative Study

Abstract:Objective: This study aimed to document and empirically evaluate the physical environment strategies used by emergency departments (EDs) to address the challenges posed by the COVID-19 pandemic; and to develop recommendations for managing future crises. Background: Emergency departments made significant environmental modifications in responding to the COVID-19 pandemic but these modifications and the decision-making processes were seldomly studied. Methods: In this in-depth qualitative case study, a multidisciplinary research team conducted semistructured interviews with 11 professionals of various roles in environmental responses to the pandemic at a large urban ED in the U.S. Qualitative content analysis generated codes and code categories from the data as well as a conceptual framework. Design documents and photographic documentation were used to cross-check the interview data. Results: The ED faced challenges in making rapid changes with limited information and resources. Physical barriers separating patients, air filtration, airflow control, and alternative care spaces were key physical environmental strategies implemented. Among them, the physical separation of patients was perceived to be most effective, followed by air quality control measures. Interviewees recommended flexibility in building design (self-contained zones, negative pressure and air filtration in all patient rooms, pandemic mode of air ventilation system), and an all-inclusive bottom-up decision-making process. Concerns included ventilation, security, communication strategies, and workplace ergonomics. Conclusion: The physical environment constitutes an important part of ED pandemic response and the proactive preparation for future crises. Hospitals should consider the ED environment's role in pandemic response, including ventilation capability, security visibility, and functionality for staff.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aims</h3><p>To explore the status quo and the influencing factors of residents' knowledge, attitude and practice in the prevention and control of coronavirus disease 2019 (COVID‐19), as well as the difficulties or challenges perceived by residents in their preventive practice.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Design</h3><p>An online questionnaire survey.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>The self‐designed questionnaire was distributed among residents online in February 2020. Descriptive statistics, two independent samples t‐tests, one‐way analysis of variance (ANOVA), Pearson's correlation analysis, multivariate linear regression and content analysis were performed.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>A total of 919 valid questionnaires were collected. The scoring rates of residents' knowledge, attitude and practice were 85.2%, 92.9% and 84.4%, respectively. Main factors influencing residents' knowledge included gender and occupation; while those influencing attitude were occupation, family economic level and knowledge; and those influencing practice included place of residence, occupation, with or without chronic disease, knowledge and attitude. Mass media was the primary approach for people to learn the knowledge and information of COVID‐19. Difficulties or challenges faced were mainly lack of protective equipments, concerns about the risk of prevention and control, impact on daily life, work and study, lack of knowledge and consensus, psychological problems and information problems.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The attitude of residents towards COVID‐19 prevention and control is generally positive. The knowledge and practice have been popularized to a certain extent, but there are still deviations or deficiencies in residents' understanding of certain important knowledge and the adoption of relevant preventive measures. Evidence‐based tailored public education initiatives are indicated.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Impact</h3><p>Findings of this study add important knowledge about residents' understanding, attitude, practice and the influencing factors on COVID‐19 prevention and control, which serves as a scientific foundation for optimizing the pandemic public education and decision‐making.</p></section>

Abstract:Background Clinical management of ventilator-assisted individuals (VAIs) was challenged by social distancing rules during the COVID-19 pandemic. In May 2020, the Long-Term In-Home Ventilator Engagement (LIVE) Program was launched in Ontario, Canada to provide intensive digital care case management to VAIs. The purpose of this qualitative study was to explore the acceptability of the LIVE Program hosted via a digital platform during the COVID-19 pandemic from diverse perspectives. Methods We conducted a qualitative descriptive study (May 2020–April 2021) comprising semi-structured interviews with participants from eight home ventilation specialty centers in Ontario, Canada. We purposively recruited patients, family caregivers, and providers enrolled in LIVE. Content analysis and the theoretical concepts of acceptability, feasibility, and appropriateness were used to interpret findings. Results A total of 40 individuals (2 VAIs, 18 family caregivers, 20 healthcare providers) participated. Participants described LIVE as acceptable as it addressed a longstanding imperative to improve care access, ease of use, and training provided; feasible for triaging problems and sharing information; and appropriate for timeliness of provider responses, workflows, and perceived value. Negative perceptions of acceptability among healthcare providers concerned digital workload and fit with existing clinical workflows. Perceived benefits accorded to LIVE included enhanced physical and psychological safety in the home, patient–provider relations, and VAI engagement in their own care. Conclusions Study findings identify factors influencing the LIVE Program's acceptability by patients, family caregivers, and healthcare providers during pandemic conditions including enhanced access to care, ease of case management triage, and VAI safety. Findings may inform the implementation of digital health services to VAIs in non-pandemic circumstances.
