Abstract:Nutcracker syndrome (NCS), which is caused by compression of the left renal vein between the abdominal aorta and the superior mesenteric artery, leads to a series of clinical symptoms including hematuria, proteinuria, flank pain, and varicocele. The diagnosis of NCS is difficult due to variations in normal anatomy. Treatment, which ranges from observation to nephrectomy, remains controversial. We conducted a review based on the related literature and our experience with hundreds of cases. We summarize the characteristics of NCS, the different measurements used in diagnosis, and the current treatment options. We present our diagnostic criteria and recommend endovascular stenting as the primary option for NCS.
Treatment of Six Cases of Left Renal Nutcracker Phenomenon: Surgery and Endografting

Abstract:Objective A retrospective study was undertaken to examine therapeutic strategies used in 6 patients with left renal venal nutcracker phenomenon.Methods Three patients underwent surgical transposition of the superior mesenteric artery. Three patients underwent endograft stent implantations with intervention.Results All 6 cases were completely relieved of left renal vein compression after the operations, leading to improved clinical symptoms and normal urine tests.Conclusions Transposition of the superior mesenteric artery is a feasible method, but leads to complications and great trauma. Treating left renal venal nutcracker phenomenon with endografting techniques is minimally invasive in certain cases, and seems to have a prospective future.
Endovascular Stenting for Treatment of Nutcracker Syndrome: Report of 61 Cases with Long-Term Followup.

Abstract:PURPOSE:We report the efficacy and safety of endovascular stenting for nutcracker syndrome at long-term followup. MATERIALS AND METHODS:We retrospectively evaluated the endovascular stenting experience with 61 patients with a median age of 26 years. Symptoms were hematuria, proteinuria or flank pain. Median followup was 66 months. RESULTS:Peak velocity in the aortomesenteric portion, and the anteroposterior diameter ratio of the renal hilum and the aortomesenteric portion of the left renal vein on Duplex ultrasound after stenting was significantly decreased compared to that on Duplex ultrasound before stenting (p<0.05). However, peak velocity in the hilar portion did not statistically differ (p>0.05). Symptoms resolved or improved in 15, 24 and 20 of the 61 patients within 1 week, and 1 and 6 months, respectively, after endovascular stenting. Symptoms remained unchanged in 2 patients and recurred in 1. A perioperative complication was noted in 1 patient, that is a stent that was mistakenly moved and poorly deployed in a left renal vein collateral required operative intervention. Postoperative complications included stent migration into the right atrium, stent protrusion into the inferior vena cava and stent migration into the hilar left renal vein in 1 case each. CONCLUSIONS:Based on our long-term followup endovascular stenting is a safe, effective procedure in select adults. We recommend endovascular stenting as primary option for nutcracker syndrome.
Clinical Effect Of Microscopic Subinguinal Varicocelectomy Or Bypass Surgery On Nutcracker Phenomenon-Associated Varicocele

Abstract:Objective: The goal of this study was to assess the effectiveness and safety of microscopic subinguinal varicocelectomy or bypass surgery on nutcracker phenomenon-associated varicocele. Methods: A total of 96 patients with varicocele were retrospectively analyzed in our institution between August 2015 and August 2017. In group 1, 35 patients were included with nutcracker phenomenon-associated varicocele underwent microscopic subinguinal varicocelectomy. In group 2, 30 patients were included with nutcracker phenomenon-associated varicocele underwent bypass surgery. In group 3, 31 patients were included with varicocele underwent microscopic subinguinal varicocelectomy. Results: There was no significant difference among the three groups in term of left testicular volume, left internal spermatic vein diameter, and semen parameters. After 6 months postoperative, the mean semen parameters were significantly improved substantially in all groups. However, no significant difference was observed in left testicular volume, left internal spermatic vein diameter, and reflux time after surgery among group 1, group 2, and group 3. The improvement of semen parameters showed no significant difference in patients with nutcracker phenomenon-associated varicocele that underwent microscopic subinguinal varicocelectomy compared with those that underwent bypass surgery. None of the patients reported serious adverse events. Conclusion: Outcomes of microscopic subinguinal varicocelectomy and bypass surgery are similar in term of surgical effect and safety for the management of nutcracker phenomenon-associated varicocele.
Extravascular Stent Management for Migration of Left Renal Vein Endovascular Stent in Nutcracker Syndrome

Abstract:Background: Nutcracker syndrome is an entity resulting from left renal vein compression by the aorta and the superior mesenteric artery, which leads to symptoms of hematuria or left flank pain. The alternative option of endovascular or extravascular stenting is very appealing because of the minimal invasive procedures. Stents in the renal vein can cause fibromuscular hyperplasia, proximal migration or embolization. Case presentation: A 30-year-old female was diagnosed with nutcracker syndrome for severe left flank pain. After failed conservative approach, she underwent endovascular stenting and subsequently developed recurrent symptom for stent migration one month postoperatively. She underwent successful extravascular stenting with complete symptom resolution. Conclusion: The extravascular stenting is an alternative option after migration of left renal vein endovascular stenting. The computed tomographic imaging was closely correlated to therapeutic interventions and stent migration.
Low Socioeconomic Status Is Associated with Cardiovascular Risk Factors and Outcomes in Systemic Lupus Erythematosus

Abstract:Objective. Accelerated atherosclerosis is a major cause of death in systemic lupus erythematosus (SLE), yet little is known about the effect of socioeconomic status. We investigated whether education or income levels are associated with cardiovascular risk factors and outcomes in SLE. Methods. Our study involved a longitudinal cohort of all patients with SLE enrolled in the Hopkins Lupus Cohort from 1987 through September 2011. Socioeconomic status was measured by education level (≥ 12 years or < 12) and income tertiles (> $60,000, $25,000–$60,000, or < $25,000). Results. A total of 1752 patients with SLE were followed prospectively every 3 months. There were 1052 whites and 700 African Americans. Current smoking, obesity, hypertension, and diabetes mellitus were more common in African Americans (p < 0.01 for all), but there was no statistical difference in the frequency of myocardial infarction or stroke. In multivariate analyses stratified by ethnicity, low income was strongly associated with most traditional cardiovascular risk factors in whites, but only with smoking and diabetes in African Americans. In whites, low income increased the risk of both myocardial infarction (OR 3.24, 95% CI 1.41–7.45, p = 0.006) and stroke (OR 2.85, 95% CI 1.56–5.21, p = 0.001); in African Americans, these relationships were not seen. Low education, in contrast, was associated with smoking in both ethnic groups. Conclusion. Low income, not low education, is the socioeconomic status variable associated with cardiovascular risk factors and events. This association is most clearly demonstrable in whites.

Abstract:Nutcracker syndrome results from compression of the left renal vein between the superior mesenteric artery and the aorta, leading to symptoms of hematuria and left flank pain. Management options include endovascular or laparoscopic extravascular stent placement, which is very appealing given the minimally invasive nature of these procedures. Abstract:Objective: To present a new approach using a shunt operation for the management of nutcracker phenomenon presenting as left varicocele in adolescent patients. Materials and methods: 12 adolescent patients with the nutcracker phenomenon presenting as left varicocele underwent a shunt operation consisting of anastomosis of the proximal part of the spermatic vein and inferior epigastric vein to lower the left renal vein (LRV) pressure. A simple ligation of the left spermatic vein was then used to repair the varicocele. Results: 12 patients underwent surgery, and symptoms of hematuria, proteinuria, scrotum discomfort, and flank pain disappeared post surgery in all patients. Patients were followed for 24-72 months (mean 48 months). The diameters of the proximal LRV and the peak velocities in the aortomesenteric portion of the LRV were significantly decreased after surgery (p < 0.001). Left testicular volume significantly increased after surgery. One patient had recurrence of the left varicocele and one adolescent had minimal hydrocele requiring no intervention. No major complications were observed during and after surgery. Conclusion: Anastomosis of the proximal part of the spermatic vein and inferior epigastric vein is an efficacious and safe surgical approach for the management of nutcracker phenomenon presenting as left varicocele in adolescents. Abstract:Background Nutcracker syndrome (NCS) is a rare cause of varicocele and its treatment is still controversial. Objective To summarize the surgical strategy and outcomes of microvascular Doppler (MVD)-assisted microsurgical left spermatic-inferior epigastric vein anastomosis (MLSIEVA) with microsurgical varicocelectomy (MV) at the same incision for treating NCS-associated varicocele. Design, setting, and participants A retrospective analysis of 13 cases of NCS-associated varicocele between July 2018 and January 2022 was performed. Surgical procedure A small incision in the body projection corresponding to the deep inguinal ring was chosen as the surgical incision. All patients underwent MLSIEVA and MV with the assistance of MVD. Measurements Patients received real-time Doppler ultrasound (DUS) before and after surgery; urine red blood cells and protein were tested, with a follow-up time of 12–53 mo. Results and limitations All patients had no intraoperative complications, and all postoperative symptoms of hematuria or proteinuria, scrotal swelling, and low back pain disappeared. Comparing pre- and postoperative DUS, two patients did not show any improvement in their postoperative measurements. However, in the remaining patients, the internal diameter of the renal vein at the hilum portion and at the aortomesenteric angle portion, as well as their ratio, improved significantly compared with preoperative measurements. No complications or recurrence of varicocele was observed during postoperative follow-up. Conclusions Our study suggests that MVD-assisted MLSIEVA with MV is feasible with no major short-term complication and effective regarding the treatment of varicocele and NCS. Patient summary We investigated microsurgery mediated by microultrasound for the treatment of varicocele associated with nutcracker syndrome. We found this procedure to be safe and effective with good long-term results.

Abstract:Objective:To improve the recognition of Nutcracker phenomenon(left renal vein entrapment syndrome)and enhance the operation technique of this syndrome.Methods:One female patient and two male patients of Nutcracker phenomenon,whose age is 12,13 and 15 separately.One case's main clinical manifestation is recurrent blood urine under naked eyes,and the other has severe left varicocele.Three patients receive the reconstruction of left renal vein without block of artery,and the blood urine and varicocele disappear after the operation.Besides,the volume of urine and the creatinine remain unchanged and the patients are all completely cured and discharged from hospital.Results:During the six months' average follow up time(ranging from 2 to 12),no one has relapsed.Conclusions:Reconstruction of left renal vein is an effective way to treat blood urine.And the surgery-reconstruction of renal vein without block of artery,which refrains kidney from the cold and warm ischemia and accordingly reduces the damage of kidney function,is possible under the condition of an expect technique of vessel anastomosis.

Abstract:The development of a general and practical zinc-catalyzed enantioselective alkyne addition methodology is reported. The commercially available ProPhenol ligand (1) has facilitated the addition of a wide range of zinc alkynylides to aryl, aliphatic, and α,β-unsaturated aldehydes in high yield and enantioselectivity. New insights into the mechanism of this reaction have resulted in a significant reduction in reagent stoichiometry, enabling the use of precious alkynes and avoiding the use of excess dimethylzinc. The enantioenriched propargylic alcohols from this reaction serve as versatile synthetic intermediates and have enabled efficient syntheses of several complex natural products.

Abstract:Objective To investigate the therapeutic effect of microsurgical varicocelectomy for nutcracker syndrolne ( NCS ) combined with left varicocele ( VC ) . Methods The clinical data of 8 patients with NCS combined with left VC from June 2013 to February 2016 in our hospital were retrospectively analyzed. All patients were followed up for 6-12 months. The semen quality, spermatic vein diameter and sex hormone levels were recorded and compared before and after surgery. Results The postoperative semen quality of 8 patients was significantly improved ( P<0. 05 ) , and the levels of follicle stimulating hormone and luteinizing hormone were significantly decreased ( P<0. 05 ) . There was no significant difference in the level of testosterone before and after surgery ( P>0. 05). Postoperative spermatic vein diameter was significantly narrowed ( P<0. 05 ) . Conclusions Microscope ligation of varicose veins can improve the clinical syndrome, protect the testicular function and improve the quality of semen in the patients with NCS combined with left VC.

Abstract:BackgroundHematuria is one of the common complaints of patients with kidney disease and often occurs concurrently with proteinuria. Hematuria caused by nutcracker syndrome (NCS) is relatively common, while hematuria caused by type I von Willebrand disease (VWD) is less common.Case PresentationA 12-year-old boy was admitted to our ward due to sudden gross hematuria that had lasted for 4 days. After admission, his gross hematuria lasted for 24 days without remission and was combined with nephrotic-range proteinuria. A series of blood biochemical and urine tests, as well as renal ultrasound, lower abdominal CT angiography, cystoscopy, kidney biopsy, and genetic testing, were completed, and he was eventually diagnosed with NCS combined with type 1 VWD. After nutritional support, oral angiotensin-converting enzyme inhibitor (ACEI), and plasma infusion treatment, gross hematuria and microhematuria disappeared, proteinuria turned negative, and there was no recurrence during the 10 months of follow-up.ConclusionsNCS and/or VWD may exist in patients with hematuria, and bleeding disorders such as VWD should generally be on the list of suspected diagnoses in patients with hematuria.

Abstract:A retrospective analysis was performed to review 20 patients with left renal vein (LRV) entrapment syndrome. All cases were diagnosed based on ultrasonography, magnetic resonance angiography, and renal venography. Technical success was achieved in all patients. Transposition of superior mesenteric artery was performed in three cases, transposition of LRV in two, and stent implantation in the LRV in 15. Stent migration occurred in one case, and stent reimplantation was performed. One case suffered from hematoma after transposition of the superior mesenteric artery, and reoperation was performed. Follow-up was made from 6 months to 6 years after the operation. Abnormalities were not found in the urine test of 18 patients and their symptoms disappeared. Two cases still had microscopic hematuria after exercise. All surgical operations are effective for nutcracker syndrome except excessive invasiveness. Stent may be one of the mainstream therapies because of its minimal invasiveness.

Abstract:Objective: To describe the feasibility of retroperitoneal laparoscopic reimplantation of the left renal vein (LRV) for nutcracker syndrome (NCS). Patients and Methods: Two patients with NCS underwent the surgery. Both patients complained of gross hematuria and flank discomfort that could not be relieved by resting. They were placed in a supine position and 5 ports were placed in the right abdominal wall. The procedures were performed with a retroperitoneal approach. The LRV was transected and then reimplanted into the distal inferior vena cava. Results: The procedures were performed successfully without any major complications. The total operation time was 105 and 120 min, respectively. Hematuria and flank discomfort were resolved after the surgery. Ultrasonography revealed a patent lumen without compression. Conclusions: Retroperitoneal laparoscopic reimplantation of the LRV appears to be a feasible procedure with satisfactory short-term outcomes. (C) 2014 S. Karger AG, Basel

Abstract:OBJECTIVE:To investigate the clinical effect of microscopic spermatic vein ligation in the treatment of nutcracker phenomenon (NCP) complicated with left varicocele (VC). METHODS:This retrospective study included 31 cases of NCP complicated with left VC treated in our hospital by subinguinal microscopic ligation of the left spermatic vein (group A, n = 11), open retroperitoneal high ligation of the left spermatic vein (group B, n = 11), or conservative therapy (group C, n = 9). The patients were followed up for 6－24 (15.3 ± 5.4) months. We compared the semen parameters, spermatic vein diameter, left testis volume, and recurrence rate among the three groups of patients before and after treatment. RESULTS:Compared with the baseline, the semen quality parameters were significantly improved in both groups A and B at 6 months after treatment (P<0.05) but reduced in group C (P<0.05); the spermatic vein diameter at rest and that at Valsalva maneuver were markedly decreased in groups A (［2.53 ± 0.27］ vs ［1.84 ± 0.22］ and ［3.53 ± 0.19］ vs ［2.16 ± 0.25］ mm, P<0.05) and B (［2.62 ± 0.33］ vs ［2.15 ± 0.43］ and ［3.36 ± 0.25］ vs ［2.44 ± 0.27］ mm, P<0.05) but increased in group C (［2.56 ± 0.28］ vs ［2.94 ± 0.24］ and ［3.33 ± 0.21］ vs ［3.77 ± 0.26］ mm, P<0.05). No statistically significant differences were found in the left testis volume at 6 months after treatment in group A (［9.85 ± 1.86］ vs ［10.27 ± 1.18］ ml, P>0.05), B (［9.77 ± 2.03］ vs ［9.96 ± 1.72］ ml, P>0.05), or C (［9.83 ± 1.59］ vs ［10.48 ± 2.05］ ml, P>0.05), nor in the recurrence rate between groups A and B (P>0.05). CONCLUSIONS:Hematuria, proteinuria and other mild symptoms of nutcracker phenomenon complicated with left VC can be treated palliatively by microscopic ligation of the spermatic vein, which can relieve the clinical symptoms, improve the semen quality, and protect the testicular function of the patient.

Abstract:OBJECTIVE:To report our initial experience in treating renal nutcracker syndrome by retroperitoneal laparoscopic extravascular stenting. PATIENTS AND METHODS:Two male patients, aged 13 and 16 years, were diagnosed with nutcracker syndrome and received retroperitoneal laparoscopic extravascular stent placement. The perioperative data were collected and evaluated. The follow-up was 10 and 18 months. RESULTS:Both procedures were successful without obvious complications. Total operative time was 65 and 50 min, estimated blood loss was 110 and 70 ml, and postoperative hospital stay was 4 and 6 days. The symptom of gross hematuria ceased 3 and 6 days after surgery. Both patients had normal findings during follow-up. CONCLUSIONS:Treatment of nutcracker syndrome by retroperitoneal laparoscopic extravascular stent placement is a safe and feasible procedure, especially for youngsters in the period of physical development. Longer follow-up and further experience are needed to evaluate the efficacy of this procedure.

Abstract:Hematuria is frequently encountered in clinical practice. Its diagnostic spectrum is wide: urinary tract infection, lithiasis, malignant tumor and nephropathy. In the absence of one of these causes, the nutcracker syndrome must be evoked. It results from compression of the left renal vein between the abdominal aorta and the superior mesenteric artery. Knowing how to diagnose it can avoid morbid consequences (chronic renal disease, renal vein thrombosis). In addition to hematuria, its main symptoms are left lumbago, varicoceles, and orthostatic proteinuria. The clinical picture and complementary examinations (ultrasound-doppler, computed tomography angiography, magnetic resonance angiography, and phlebography) generally allow the diagnosis to be made. Treatment varies according to age and severity of symptoms.

Abstract:OBJECTIVE:To study the diagnosis of adult nutcracker phenomenon (NCP) and assess the therapeutic value of endovascular stenting (ES) and superior mesentery artery transposition (SMAT) for the treatment of NCP in long-term follow-up.METHODS:Six patients (6 men) aged 16 and 34 years old (mean age, 22.7 +/- 18.0 years) were diagnosed as having NCP using the examination of the doppler ultrasound and/or magnetic resonance of artery/digital radiography (MRA/DSA). Three patients underwent ES and 3 patients received SMAT for the treatment of the NCP patients associated with recurrent gross hematuria and left flank pain. Doppler ultrasound and urine examination were used at pre- and post-operation.RESULTS:In 3 patients who underwent SMAT, the postoperative complications comprised retroperitoneal hematoma necessitating surgical revision (n = 1). Functional disorder of intestine (n = 1) and paralytic ileus (n = 1) that were resolved by conservative management. In 3 patients who experienced ES, 1 patient received surgical revision because the position of stent was not suitable in left renal vein but no other complication took place. During the follow-up of 6 approximately 50 months (mean 24.7 +/- 18.0 months), except that 1 patient's hematuria disappears at rest and reappears after motion while the other 5 patients remain asymptomatic and free of hematuria. The dopplar ultrasound showed the left renal vein diameters of the angel segment between superior mesentery artery and aorta were (0.18 +/- 0.05) cm preoperation and (0.65 +/- 0.17) cm postoperation, P < 0.001; the left renal vein diameters of the portal segment were (0.89 +/- 0.22) cm preoperation and (0.79 +/- 0.20) cm postoperation, P = 0.003; the left renal vein diameter's ratio between portal and angel segment were (4.99 +/- 0.79) preoperation and (1.23 +/- 0.16) postoperation, P < 0.001.CONCLUSIONS:Dopplar ultrasound plays a very important role in the diagnosis of adult NCP. SMAT is an efficient surgical approach to the treatment of the nutcracker phenomenon and is associated with an acceptable risk of complications. But ES is safer and more efficient than SMAT and may represent a valuable approach to lessening the morbidity of surgical procedures. ES is a new therapeutic method for adult NCP.
