Abstract:Background : The association between gaseous pollutants and cerebrovascular morbidity has been widely studied; however, the association in patients with type 2 diabetes has remained unknown in developing countries. Methods : A time-stratified case-crossover design combined with a distributed lag nonlinear model was adopted to estimate the short-term effects of gaseous pollutants (nitrogen dioxide [NO 2 ], sulfur dioxide [SO 2 ], ozone [O 3 ], and carbon monoxide [CO]) on cerebrovascular cases among patients with type 2 diabetes in Beijing. In addition, our study explored the variability across sex and age groups. Results : A total of 223,216 (male 57.6% and elderly 61.7%) cerebrovascular cases were reported from 2014 to 2018. The cumulative exposure-response curves were U-shaped for NO 2 , J-shaped for SO 2 , and V-shaped for O 3 and CO. Extreme low-O 3 , low-CO and high-CO increased the risk of cerebrovascular morbidity, with the maximum relative risk (RR) of 1.14 (95% CI: 1.01-1.28) (1st vs median), 1.02 (95% CI: 1.01-1.03) (1st vs median) and 1.25 (95% CI: 1.09-1.45) (99th vs median), respectively, appearing at lag0-13. Elderly individuals aged over 65 years were susceptible to extremely low-O 3 (maximum RR: 1.27, 95% CI: 1.08-1.48). Men and elderly individuals were more susceptible to extremely high-CO, and the maximum RR was 10% [1.31 (1.11-1.54) vs. 1.19 (1.00, 1.41)] and 26% higher [1.41(1.17-1.69) vs. 1.12 (0.97, 1.30)] than women and adults, respectively. Conclusions : NO 2 , SO 2 , O 3 and CO presented nonlinear and lagged effects on cerebrovascular disease among patients with type 2 diabetes, which may vary by sex and age. Our study added to the limited evidence for the risk of cerebrovascular disease due to gaseous pollutants among patients with comorbid type 2 diabetes.
Residential Greenness, Air Pollution, and Incident Ischemic Heart Disease: A Prospective Cohort Study in China

Abstract:Background and aim: Green living environments are associated with beneficial health outcomes, whereas higher air pollution exposure might increase the risk of chronic diseases. Moreover, few studies have explored the interaction between residential greenness and air pollution on the risk of ischemic heart disease (IHD). Therefore we aimed to estimate the associations of residential greenness and air pollution with IHD and their interaction. Methods: We performed a prospective cohort study that included 29,141 adult participants recruited from Yinzhou District, Ningbo, China. Normalized Difference Vegetation Index (NDVI) around each participant's residence was calculated to measure residential greenness exposure. Land-use regression models were conducted to estimate long-term individual exposure to air pollutants, including nitrogen dioxide (NO2) and particulate matter with aerodynamic diameters ≤ 2.5 μm (PM2.5) and ≤10 μm (PM10). Cox proportional hazard models were used to calculate the hazard ratios (HRs) and 95% confidence intervals (95% CIs) for the association of residential greenness and air pollutants with the risk of incident IHD. Results: During 101,172.5 person-years of follow-up, 1,392 incident IHD cases were reported in the study population. Residential greenness, expressed as an interquartile range (IQR) increase in NDVI within 300m, was inversely associated with incident IHD. However, long-term air pollution exposures were associated with higher IHD incidence. Mediation analyses suggested that the beneficial effect of residential greenness on incident IHD could be partly mediated by reducing the exposure to PM2.5. Conclusion: Higher greenness was associated with decreased risk of IHD, while air pollutants were positively associated with incident IHD. Long-term Exposure to Residential Surrounding Greenness and Incidence of Diabetes: A Prospective Cohort Study

Abstract:Exposure to residential greenness might affect population health through increasing physical activity and social engagement, improving mental health, and reducing harmful environmental exposure. However, evidence on the association of greenness with risk of diabetes is still controversial. In this study, we recruited a total of 22,535 participants aged ≥18 years from Yinzhou District, Ningbo, Zhejiang Province, China to investigate the associations between residential greenness and risk of diabetes incidence. Residential greenness was estimated using Normalized Difference Vegetation Index (NDVI), Enhanced Vegetation Index (EVI), and Vegetation Continuous Field (VCF). We also calculated cumulative average NDVI, EVI and VCF values, and changes in NDVI, EVI and VCF during the follow-up period. We used Cox proportional hazards models controlling for demographic characteristics, lifestyles, individual socioeconomic status, history of diseases and particulate matter with an aerodynamic diameter ≤2.5 μm (PM2.5) to examine hazard ratios (HRs) and 95% confidence intervals (95% CIs) and assessed physical activity, body mass index (BMI) or PM2.5 as potential mediators. During 84,992.64 person-years of follow-up, a total of 1,154 incident cases of diabetes occurred. In multivariable models, living in the highest quartile of cumulative average NDVI, EVI and VCF within 250-m buffer was associated with 57% (HR = 0.43, 95% CI: 0.36, 0.52), 62% (HR = 0.38, 95% CI: 0.32, 0.45), and 55% (HR = 0.45, 95% CI: 0.38, 0.54) reduction in diabetes risk compared with the lowest quartile, respectively. Results remained similar for NDVI, EVI, and VCF within 500-m and 1000-m buffers. Stratified analyses showed stronger association for residential greenness and diabetes among older people. The association between greenness and diabetes did not appear to be mediated by physical activity, PM2.5 or BMI. Our findings suggested that higher residential greenness was significantly associated with lower risk of diabetes.

Little evidence has been observed for the associations between particulate matter (PM), SO2 and O3 exposure and diastolic blood pressure in present study.Conclusions: In our study, diabetes patients were at increased risk for higher blood pressure when exposed to higher residential PM and SO2. Individuals with higher BMI, younger age, and FBG≥7.0mmol/L were more susceptible to ambient air pollution.

Abstract:Air pollution exposure has been associated with hypertension, but limited studies have examined the associations in diabetic patients. We aimed to examine the associations of exposure to air pollution with blood pressure in diabetic patients. We identified patients who were previously diagnosed as type-2 diabetes mellitus from hospital records from January 2013 to May 2017. We used generalized linear models to estimate adverse effects of air pollution on systolic/diastolic blood pressure in diabetic patients. A total of 40,125 patients were enrolled in this study. An IQR increase in PM2.5, PM10, NO2, SO2, and O3 exposure was associated with an increased risk of higher systolic blood pressure level (β = 0.628 for PM2.5, β = 0.758 for PM10, β = 0.766 for NO2, β = 0.516 for SO2, β = 0.482 for O3, all P value < 0.05). Significant associations were observed for some air pollutants and SBP among patients with fasting blood glucose (FBG) ≥ 7.0 mmol/L, normal weight, and younger age. Little evidence was available for the associations between particulate matter (PM) or O3 exposure and diastolic blood pressure in present study. In our study, diabetic patients were at increased risk of higher blood pressure when exposed to higher residential PM, NO2 and SO2. Individuals with lower BMI, younger age, and FBG ≥ 7.0 mmol/L were more susceptible to the adverse effects of ambient air pollution.

Abstract:Background: Ambient air pollution has recently been related to type 2 diabetes mellitus (T2DM), a disease that has caused an economic and health burden worldwide. Evidence of an association between air pollution and T2DM was reported in the United States and Europe. However, few studies have focused on the association with high levels of air pollutants in a developing country. Objectives: We conducted a 12-year cohort study to assess the incidence and mortality of T2DM associated with long-term exposure to PM10, SO2, and NO2. Methods: A retrospective cohort with participants from four cities in northern China was conducted to assess mortality and incidence of T2DM from 1998 to 2009. Incidence of T2DM was self-reported, and incident intake of an antidiabetic drug or injection of insulin simultaneously and mortality of T2DM was obtained from a family member and double checked against death certificates provided from the local center for disease control and prevention. Individual pollution exposures were the mean concentrations of pollutants estimated from the local environmental monitoring centers over the survival years. Hazard ratios (HRs) were estimated using Cox regression models after adjusting for potential confounding factors. Results: A total of 39 054 participants were recruited into the mortality cohort, among which 59 subjects died from T2DM; 38 529 participants were analyzed in the incidence cohort, and 1213 developed new cases of T2DM. For each 10 mu g/m(3) increase in PM10, SO2, and NO2 , the adjusted HRs and 95% confidence interval (CI) for diabetic incidence were 1.831 (1.778, 1.886), 1.287 (1.256, 1.318), and 1.472 (1.419, 1.528), respectively. Similar results can be observed in the analysis of diabetic mortality with HRs (95% CI) up to 2.260 (1.732, 2.950), 1.130 (1.042, 1.225), and 1.525 (1.280, 1.816), respectively. Conclusions: Our results suggested that long-term exposure to high levels of PM10, SO2, and NO2 increase risk of incident and mortality of T2DM in China.
Associations of Residential Green Space with Incident Type 2 Diabetes and the Role of Air Pollution: A Prospective Analysis in UK Biobank.

Abstract:Background: Green space is a newly modifiable environmental factor which would bring health benefits, and identify-ing potential pathways is important to better promote public health. Nowadays, limited evidence is available on resi-dential green space and risk of incident type 2 diabetes (T2D). To evaluate the longitudinal association between residential green space and incident T2D, and further illustrate the role of air pollution, we conducted a prospective analysis in UK Biobank.Methods: Incident cases of T2D were ascertained through medical linkage of hospital admissions. Residential green space indicated by percentage of green space at 300 m buffer was estimated using land use data. Annual average air pollution was modelled using Land Use Regression model. Cox proportional hazard regression models were used to determine the association between the exposure and incident T2D. Effect modification of air pollution was assessed using log-likelihood tests. Casual mediation analysis including interaction of green space and air pollution was used to quantity pure natural indirect effect of air pollution.Results: Of 379,238 participants at baseline, 15564 incident T2D cases were identified with 12.4 years of follow-up. In main models, individuals who exposed to residential green space at 300 m buffer in high level (>= 75 %) had 14.4 % (95 % CI: 8.0 %, 20.3 %) lower risk of incident T2D compared with those in low level (<25 %). This association was modified by NO2, with green space indicating higher protective effect in low NO2 level (P interaction = 0.098). PM2.5 had a high mediation effect of 37.0 % in the association.Conclusion: Elevated residential green space level was associated with lower risk of incident T2D, and air pollution played an important role in this association. These findings would contribute to policy making and healthy city construction to take air pollution into consideration while planning green space, which would maximize public health benefits.
The Association of Daily Diabetes Mortality and Outdoor Air Pollution in Shanghai, China

Abstract:The objective of the study reported here was to assess the association between air pollution and daily diabetes mortality in Shanghai. Death records were collected for all individuals who lived in the Zhabei district of Shanghai and died from diabetes from January 1, 2001, to December 31, 2002. The authors used a time-series approach to study the acute effects of air pollution on diabetes mortality after controlling for long-term trends, weather variables, and day of the week. Each increase of 10 microg/m3 in PM10, SO2, or NO2 was found to correspond, respectively, to a 1.006 (95 percent CI: 1.000-1.012), 1.011 (95 percent CI: 0.990-1.032), or 1.013 (95 percent CI: 1.000-1.026) relative risk of diabetes mortality in Shanghai. The air pollutants also were observed to have a greater effect on diabetics than on nondiabetics. These findings provide new evidence for the association between air pollution and diabetes mortality risk, and suggest that diabetics are more susceptible to air pollution than are nondiabetics.

Abstract:BackgroundBoth air pollution and green space have been shown to independently affect mortality. We aimed to study whether greenness might interact with air pollution-related mortality among Chinese adults aged 65 years and older.MethodsWe used the 2008 wave of the Chinese Longitudinal Healthy Longevity Survey, with participants surveyed in 2012 and 2014. We used satellite remote sensing around participants' residential addresses to calculate our exposure variables. Normalized Difference Vegetation Index (NDVI) within a 500 m radius around their residence was used to measure contemporaneous NDVI (value at the time closest to the outcome event). Concentrations of particulate matter (PM2·5) were calculated using the 3-year average in a 1 km × 1 km grid. Cox proportional hazards models were used to estimate the effects of NDVI, PM2·5, and their interaction on all-cause mortality. We controlled for age, sex, ethnicity, marital status, geographic region, urban or rural residence, education, occupation, financial support, social and leisure activity, smoking status, alcohol consumption, and physical activity. We also stratified analysis by sex, urban or rural regions, smoking status, and exercise status.FindingsWe followed up 12 873 participants, totalling 47 884 person-years. There were 7426 mortality events from 2008 to 2014. At baseline, the mean age was 87 years (SD 11·34) and 84·62% lived in a rural area. The mean contemporaneous NDVIs were 0·42 (SD 0·21); the mean PM2·5 concentration was 49·63 μg/m3 (SD 13·72). In the fully adjusted model, the mortality hazard ratios (HRs) for each 0·1-unit decrease in contemporaneous NDVI was 1·08 (95% CI 1·03–1·13), each 10 μg/m3 increase in PM2·5 was 1·13 (1·09–1·18), interaction term 1·01 (1·00–1·02) with statistical significance (p=0·027). In our stratified analysis, people in urban areas were more likely to benefit from greenness, and people in rural areas were more likely to be harmed by air pollution. There was no effect modification by sex.InterpretationOur study showed that there is a synergistic effect of greenness and air pollution, which has important implications for urban planning and air pollution control to improve public health.FundingNone.

Abstract:Background: The effect of fine particulate matter (PM2.5) components on prediabetes and diabetes is of concern, but the evidence is limited and the specific role of different green space types remains unclear. This study aims to investigate the relationship of PM2.5 and its components with prediabetes and diabetes as well as the potential health benefits of different types and combinations of green spaces. Methods: A multicenter cross-sectional study was conducted in eastern China by using a multi-stage random sampling method. Health screening and questionnaires for 98,091 participants were performed during 2017-2020. PM2.5 and its five components were estimated by the inverse distance weighted method, and green space was reflected by the Normalized Difference Vegetation Index (NDVI), percentages of tree or grass cover. Multivariate logistic regression and quantile g-computing were used to explore the associations of PM2.5 and five components with prediabetes and diabetes and to elucidate the potential moderating role of green space and corresponding type combinations in these associations. Results: Each interquartile range (IQR) increment of PM2.5 was associated with both prediabetes (odds ratio [OR]: 1.15, 95%CI [confidence interval]: 1.10-1.20) and diabetes (OR: 1.18, 95% CI: 1.11-1.25), respectively. All five components of PM2.5 were related to prediabetes and diabetes. The ORs of PM2.5 on diabetes were 1.49 (1.35-1.63) in the low tree group and 0.90 (0.82-0.98) in the high tree group, respectively. In the high tree-high grass group, the harmful impacts of PM2.5 and five components were significantly lower than in the other groups. Conclusion: Our study suggested that PM2.5 and its components were associated with the increased risk of prediabetes and diabetes, which could be diminished by green space. Furthermore, the coexistence of high levels of tree and grass cover provided greater benefits. These findings had critical implications for diabetes prevention and green space-based planning for healthy city.
Urban overall and visible greenness and diabetes among older adults in China

Abstract:Neighborhood greenness has been shown to reduce diabetes risk, however, no studies have compared the effects of overall greenness with visible greenness, which is crucial for understanding how greenness influences diabetes risk. Our study aims to explore the associations between greenness matrix and diabetes, as well as the potential effect modifications and mediating factors. We used logistic regressions to examine the cross-sectional associations of the satellite-based Normalized Difference Vegetation Index (NDVI) and street view-based Green View Index (GVI) with diabetes in 3,924 urban older adults enrolled in the 2017–2018 wave of Chinese Longitudinal Healthy Longevity Survey (CLHLS). We conducted the stratified analyses by age, sex, household income and education. Mediation analyses were also performed to see whether physical activity, BMI, air pollution, and social interaction mediate the associations. Significant associations with diabetes were only observed for NDVI but not for GVI. Participants in the highest quartile of NDVI and GVI had 52% (95 % CI: 48%, 63%) and 14% (-10%, 44%) lower odds of reporting having diabetes diagnosed by a doctor. The protective effects of NDVI were more pronounced in the young old (≥75–95 years) and high-education or high-income groups. No difference between males and females were observed. Air pollution (e.g., PM 2.5 , NO 2 and O 3 ) partially mediated the associations, but physical activity, BMI, and social interaction may not mediate the associations. Our findings indicate beneficial associations between overall greenness but not visible greenness surrounding residences with diabetes in older urban residents in China, especially for old adults with higher education or household income levels. Environmental factors (e.g., air pollution) but not individual behavioural characteristics are the potential underlying mechanisms.

Abstract:Background: The evidence for an association between particulate air pollution and type 2 diabetes mellitus (T2DM) in developing countries was very scarce.Objective: To investigate the associations of long-term exposure to fine particulate matter (PM2.5) with T2DM prevalence and with fasting glucose and glycosylated hemoglobin (HbA1c) levels in China.Methods: This is a cross-sectional study based on a nation-wide baseline survey of 11,847 adults who participated in the China Health and Retirement Longitudinal Study from June 2011 to March 2012. The average residential exposure to PM2.5 for each participant in the same period was estimated using a satellite-based spatial statistical model. We determined the association between PM2.5 and T2DM prevalence by multivariable logistic regression models. We also evaluated the association between PM2.5 and fasting glucose and HbA1c levels using multivariable linear regression models. Stratification analyses were conducted to explore potential effect modification.Results: We identified 1760 cases of T2DM, corresponding to 14.9% of the study population. The average PM2.5 exposure for all participants was 72.6 mu g/m(3) during the study period. An interquartile range increase in PM2.5 (41.1 mu g/m(3)) was significantly associated with increased T2DM prevalence (prevalence ratio, PR = 1.14), and elevated levels of fasting glucose (0.26 mmol/L) and HbA1c (0.08%). The associations of PM2.5 with T2DM prevalence and with fasting glucose and HbA1c were stronger in several subgroups.Conclusions: This nationwide cross-sectional study suggested that long-term exposure to PM2.5 might increase the risk of T2DM in China. Abstract:The associations between air pollution and diabetes mortality of different subtypes and complications were largely unclear. We performed an individual-level, time-stratified case-crossover study among over 0.9 million diabetes deaths from all administrative regions of Chinese mainland during 2013-2019. Daily concentrations of fine particles (PM2.5), coarse particles (PM2.5-10), nitrogen dioxide (NO2) and ozone (O3) were obtained for each decedent using high-resolution prediction models. Conditional logistic regression models were utilized to analyze the data. Each interquartile range increment in PM2.5, PM2.5-10, NO2 and O3 concentrations on lag 0-2 d increased the risks of overall diabetes mortality by 2.81 %, 1.92 %, 3.96 % and 2.15 %, respectively. Type 2 diabetes had stronger associations with air pollution than type 1 diabetes. Air pollutants were associated with diabetic ketoacidosis and diabetic nephropathy, but not other complications. The exposure-response curves were approximately linear with a plateau at higher concentrations of PM2.5, PM2.5-10, and NO2, while the associations for O3 appear to be statistically significant beyond 60 μg/m3. This nationwide study reinforces the evidence of higher risks of acute diabetic events following short-term air pollution exposure. We identified differential effects of air pollutants on various subtypes and complications of diabetes, which require further mechanistic investigations.

Abstract:OBJECTIVE:Exposure to high levels of air pollutants may be linked to diabetes-associated mortality, but the associations remain unclear. To assess the associations between main air pollutants and diabetes-associated mortality, a systematic review and meta-analysis was performed. METHODS:PubMed, Embase and Web of Science were searched for studies investigating the associations between increments in gaseous (nitrogen dioxide (NO2), sulphur dioxide, ozone (O3) and carbon monoxide) and particulate matter (PM; diameter<2.5 μm (PM2.5) or <10 μm (PM10)) air pollutants and diabetes-associated mortality. Using a random-effects model, relative risks (RRs) and 95% CIs were calculated per interquartile range (IQR) increment or per 10 μg/m3 increment in pollutant concentrations. RESULTS:Out of 925 identified articles, 36 were reviewed in depth and 12 studies from 13 articles satisfying the inclusion criteria (five time-series, five case-crossovers and two cohorts) were finally included. Increased risk of diabetes-associated mortality was associated with higher levels of PM2.5 (per 10 μg/m3: RR=1.123, 95% CI 1.036-1.217, P=0.005, I2=96.1%), PM10 (per 10 μg/m3: RR=1.008, 95% CI 1.004-1.013, P<0.001, I2=0%), NO2 (per 10 μg/m3: RR=1.024, 95% CI 1.007-1.041, P=0.006, I2=49.7%) and O3 (per IQR increment: RR=1.065, 95% CI 1.017-1.115, P=0.007, I2=0.0%). No obvious risk of publication bias was observed. CONCLUSIONS:Exposure to high levels of air pollutants is significantly associated with an increased risk of diabetes-associated mortality.
The Effects of Ambient Air Pollution and Residential Greenness on Metabolic Disease Biomarkers in China

Abstract:BACKGROUND AND AIM: We aim to study the association between air pollution, greenness, and metabolic syndrome (MetS) biomarkers, as mechanisms for cardiovascular diseases (CVD) and the mortality among the elderly in China. METHODS: We used the 2012 wave of the Chinese Longitudinal Healthy Longevity Survey (CLHLS) Biomarker Cohort. The cohort sampled participants from urban and rural areas of eight longevity counties. We matched the annual average fine particulate matter (PM2.5) concentrations in a 1 km × 1 km grid and calculated Normalized Difference Vegetation Index (NDVI) with a 500-meter radius. The medical technicians measured blood cholesterol, fasting glucose, blood pressure, height, weight, and waist circumference. We defined metabolic syndrome using modified Adult Treatment Panel III criteria. RESULTS:We had 1801 participants. Three-year average NDVI was slightly higher in the rural area than urban area (mean: 0.495 vs. 0.443) while the rural area had a similar three-year average PM2.5 level with the urban area (mean: 49.0 vs. 49.1). We found higher PM2.5 was associated with higher odds of abdominal obesity (AO) [OR (95%CI): 1.254 (1.159, 1.361)] and MetS [OR (95%CI): 1.165 (1.067, 1.278)]. Higher NDVI was associated with lower risk of AO [0.811 (0.711, 0.925)] and elevated fasting glucose [OR (95%CI): 0.84 (0.721, 0.982)], but was not significantly associated with MetS [OR (95% CI): 0.95 (0.821, 1.104)]. PM2.5 and NDVI had significant interaction on the risk of AO but not on the risk of MetS [p value of interaction term: 0.005 and 0.630 respectively]. CONCLUSIONS:We saw convergence of air pollution and greenness levels between urban and rural area among the eight counties studied in our cohort, with higher harmful effect of air pollution in rural areas and higher beneficial effect of greenness in urban areas. Abstract:BACKGROUND: Both air pollution and green space have been shown to affect health. We aimed to assess whether greenness protects against air pollution-related mortality.METHODS: We used data from the 2008 wave of the Chinese Longitudinal Healthy Longevity Survey. We calculated contemporaneous normalised difference vegetation index (NDVI) in the 500 m radius around each participant's residence. Fine particulate matter (PM2·5) concentration was calculated using 3-year average concentrations in 1 km × 1 km grid resolution. We used Cox proportional hazards models to estimate the effects of NDVI, PM2·5, and their interaction on all-cause mortality, adjusted for a range of covariates.FINDINGS: The cohort contained 12 873 participants, totalling 47 884 person-years. There were 7426 deaths between 2008 and 2014. The mean contemporaneous NDVI was 0·42 (SD 0·21), and the mean 3-year average PM2·5 was 49·63 μg/m3 (13·72). In the fully adjusted model, the mortality hazard ratio for each 0·1-unit decrease in contemporaneous NDVI was 1·08 (95% CI 1·03-1·13), each 10 μg/m3 increase in PM2·5 was 1·13 (1·09-1·18), and the interaction term was 1·01 (1·00-1·02) with a p value of 0·027. We observed non-linear associations in our stratified analyses: people living in urban areas were more likely to benefit from greenness, and people living in rural areas were more likely to be harmed by air pollution.INTERPRETATION: Our study showed some indication of a synergistic effect of greenness and air pollution, suggesting that green space planning and air pollution control can jointly improve public health.FUNDING: Bill & Melinda Gates Foundation, National Institutes of Health, National Key R&D Program of China, National Natural Science Foundation of China.

Abstract:Background Although emerging studies have illuminated the protective association between greenness and respiratory mortality, efforts to quantify the potentially complex role of air pollution in the causal pathway are still limited. We aimed to examine the potential roles of air pollution in the causal pathway between greenness and respiratory mortality in China. Methods We used data from a community-based prospective cohort of 654,115 participants in southern China (Jan 2009–Dec 2020). We evaluated the greenness exposure as a three-year moving average Normalized Difference Vegetation Index (NDVI) within the 500 m buffer around the residence. Cox proportional hazards model was applied to estimate the association between greenness and respiratory mortality. Causal mediation analysis combined with a four-way dimensional decomposition method was utilized to simultaneously quantify the interaction and mediation role of air pollution including PM 2.5 , PM 10 , or NO 2 on the greenness-respiratory mortality relationship. Findings We observed 6954 respiratory deaths during 12 years of follow-up. Increasing NDVI level from the lowest to the highest quartile is associated with a 19 % (95%CI: 13–25 %) reduction in the respiratory mortality risk. For the total protective effect, the proportion attributable to the overall negative interaction between greenness and air pollution (PM 2.5 , PM 10, or NO 2 ) was 2.2 % (1.7–3.2 %), 3.5 % (0.4–3.7 %), or 25.0 % (22.8–27.1 %), respectively. Simultaneously, we estimated 25.5 % (20.1–32.0 %), 49.5 % (32.5–71.9 %), or 1.0 % (0.8–1.2 %) of the total protective association was mediated through a reduction in PM 2.5 , PM 10 , or NO 2 , respectively. Interpretation Increased greenness exposure mitigated respiratory mortality through both the antagonistic interaction and mediation pathway of air pollution (PM 2.5 , PM 10 , or NO 2 ).

Abstract:Abstract Background Evidence remains limited and inconsistent for the associations between sustained air pollution exposures and diabetes development. This study aimed to determine the potential effects of particulate matter with a diameter of ≤10 micrometres (PM10), particulate matter with a diameter of ≤2.5 micrometres (PM2.5) and nitrogen dioxide (NO2) on alterations of fasting plasma glucose (FPG), glycated haemoglobin (HbA1c), in particular, on prevalence and incidence of diabetes. Methods Cross-sectional analyses were conducted based on 9628 participants aged ≥45 years from the baseline survey (2011) of the China Health and Retirement Longitudinal Study (CHARLS), whereas cohort analyses were based on 3510 individuals without diabetes at baseline in the third survey (2015). Residences of participants were geocoded and the air pollution exposures were estimated using a satellite-based spatiotemporal model. Linear, logistic and modified Poisson regression models, adjusting for multiple confounders, were applied to assess the associations between air pollution and FPG, HbA1c, prevalence and incidence of diabetes, respectively. Results Associations between PM10, PM2.5 and increased levels of FPG and HbA1c were identified. The levels of FPG and HbA1c increased by 0.025 mmol/L (95% CI: 0.007, 0.044) and 0.011 mmol/L (95% CI: 0.002, 0.019), respectively, for a 10-μg/m3 increase in PM10, and the levels of FPG and HbA1c increased by 0.061 mmol/L (95% CI: 0.028, 0.096) and 0.016 mmol/L (95% CI: 0.000, 0.031), respectively, for a 10-μg/m3 increase in PM2.5. There were also positive associations between diabetes prevalence and PM2.5 and PM10. In the cohort analyses, PM10, PM2.5 and NO2 were associated with a higher incidence of diabetes. Conclusion Air pollution was allied to diabetes development in elderly Chinese populations. Considering the impact of the dramatic increase in the incidence and prevalence of diabetes in China, interventions to improve air quality are urgently needed.

Abstract:Abstract Background We hypothesize higher air pollution and fewer greenness exposures jointly contribute to metabolic syndrome (MetS), as mechanisms on cardiometabolic mortality. Methods We studied the samples in the Chinese Longitudinal Healthy Longevity Survey. We included 1755 participants in 2012, among which 1073 were followed up in 2014 and 561 in 2017. We used cross-sectional analysis for baseline data and the generalized estimating equations (GEE) model in a longitudinal analysis. We examined the independent and interactive effects of fine particulate matter (PM 2.5 ) and Normalized Difference Vegetation Index (NDVI) on MetS. Adjustment covariates included biomarker measurement year, baseline age, sex, ethnicity, education, marriage, residence, exercise, smoking, alcohol drinking, and GDP per capita. Results At baseline, the average age of participants was 85.6 (SD: 12.2; range: 65–112). Greenness was slightly higher in rural areas than urban areas (NDVI mean: 0.496 vs. 0.444; range: 0.151–0.698 vs. 0.133–0.644). Ambient air pollution was similar between rural and urban areas (PM 2.5 mean: 49.0 vs. 49.1; range: 16.2–65.3 vs. 18.3–64.2). Both the cross-sectional and longitudinal analysis showed positive associations of PM 2.5 with prevalent abdominal obesity (AO) and MetS, and a negative association of NDVI with prevalent AO. In the longitudinal data, the odds ratio (OR, 95% confidence interval-CI) of PM 2.5 (per 10 μg/m 3 increase) were 1.19 (1.12, 1.27), 1.16 (1.08, 1.24), and 1.14 (1.07, 1.21) for AO, MetS and reduced high-density lipoprotein cholesterol (HDL-C), respectively. NDVI (per 0.1 unit increase) was associated with lower AO prevalence [OR (95% CI): 0.79 (0.71, 0.88)], but not significantly associated with MetS [OR (95% CI): 0.93 (0.84, 1.04)]. PM 2.5 and NDVI had a statistically significant interaction on AO prevalence ( p interaction : 0.025). The association between PM 2.5 and MetS, AO, elevated fasting glucose and reduced HDL-C were only significant in rural areas, not in urban areas. The association between NDVI and AO was only significant in areas with low PM 2.5 , not under high PM 2.5 . Conclusions We found air pollution and greenness had independent and interactive effect on MetS components, which may ultimately manifest in pre-mature mortality. These study findings call for green space planning in urban areas and air pollution mitigation in rural areas.
