Meningeal Myofibroblastoma in the Frontal Lobe: A Case Report

Abstract:Myofibroblastoma is a benign tumor composed of spindle cells in clusters and fascicles. To date, only three cases of intracranial myofibroblastoma have been reported. The present study reports the case of a 47-year-old female with meningeal myofibroblastoma. The patient had a history of ovarian cyst resection and presented with paroxysmal mild headaches that had been apparent for 4 years. Magnetic resonance imaging disclosed a well-circumscribed mass in the left frontal lobe. A resection of the mass was performed. Abundant fascicular clusters of spindle- and oval-shaped cells were found by conventional histopathology. Immunohistochemical staining demonstrated that these cells were strongly positive for smooth muscle actin, weakly positive for epithelial membrane antigen and negative for cluster of differentiation (CD)117, CD34, S-100 or desmin, with a Ki-67 index of >10%. These results supported the diagnosis of myofibroblastoma. No recurrence of the mass was found during the 24-month follow-up period. Overall, the patient exhibited a rare type of meningeal neoplasm. Resection of the tumor proved to be successful and no recurrence were found. Histopathological and immunohistochemical staining is crucial to form a diagnosis. To the best of our knowledge, the present study is the first to show the presence of myofibroblastoma in the left frontal lobe.

Abstract:ABSTRACT Patients in psychiatric settings may present with medical conditions, such as brain tumors, which may or may not be associated with neurological symptoms. In some cases, patients may only have psychiatric symptoms, such as mood changes (depression or mania), psychotic symptoms, panic attacks, changes in personality, or memory difficulties. Brain tumors may be detected in patients at their first presentation to mental health services or sometimes in patients with well-established psychiatric diagnoses.This article presents the case of a 29-year-old woman who was treated for >4 years for posttraumatic stress disorder and borderline personality traits, who developed depressive symptoms and memory difficulties. However, she did not develop any major neurological signs or symptoms. Brain imaging showed the presence of a left thalamic tumor, later confirmed as glioblastoma multiforme. She underwent surgical treatment and radiation therapy. With this we show that in some cases, brain tumors can be neurologically silent and only present atypical psychiatric symptoms.

Presenting Psychiatric and Neurological Symptoms and Signs of Brain Tumors before Diagnosis: A Systematic Review

Abstract:Brain tumors can present with various psychiatric symptoms, with or without neurological symptoms, an aspect that complicates the clinical picture. However, no systematic description of symptoms that should prompt a neurological investigation has been provided. This review aims to summarize available case reports describing patients with brain tumors showing psychiatric symptoms before brain tumor diagnosis, in order to provide a comprehensive description of these symptoms as well as their potential relationship with delay in the diagnosis. A systematic literature review on case reports of brain tumors and psychiatric symptoms from 1970 to 2020 was conducted on PubMed, Ovid, Psych Info, and MEDLINE. Exclusion criteria comprised tumors not included in the World Health Organization (WHO) Classification 4th edition and cases in which psychiatric symptoms were absent or followed the diagnosis. A total of 165 case reports were analyzed. In a subset of patients with brain tumors, psychiatric symptoms can be the only manifestation or precede focal neurological signs by months or even years. The appearance of focal or generalized neurological symptoms after, rather than along with, psychiatric symptoms was associated with a significant delay in the diagnosis in adults. A timely assessment of psychiatric symptoms might help to improve early diagnosis of brain tumors.

Isolated Neurosarcoidosis Mimicking Multifocal Meningiomas: A Diagnosis Pitfall A Case Report

Abstract:Introduction: Neurosarcoidosis accounts for approximately 5% of the sarcoidosis, which develops exclusively in the nervous system and is always difficult to diagnose. We describe a rare case of isolated neurosarcoidosis mimicking as multifocal meningiomas. A 27-year-old male was admitted to our hospital with a history of unconsciousness and convulsion 1 month ago, which was suspected as a seizure. The results showed no abnormalities in complete blood count; serum electrolytes; erythrocyte sedimentation rate and ultrasonography of the liver, pancreas, spleen, kidney and parotid gland, and so on. Chest radiograph and electroencephalogram were also normal. Serum-angiotensin-converting enzyme slightly increased. Normal opening pressure was shown in cerebrospinal fluid sampling, which includes 8/mL white blood cells, 0.93 g/L protein, and 3.03mmol/L glucose. Enhanced magnetic resonance imaging revealed multifocal enhancement lesions, including left sphenoid wing region, left temporal and bilateral occipitoparietal region, which were suspected as multiple "meningioma". A left frontotemporal craniotomy was further performed. Both necrotizing and non-necrotizing granulomas were revealed in the pathological specimen, most of which were associated with multinucleated giant cells and macrophages. We could also see the fibrosis and inflammatory reaction in the sample composed of lymphocytes, histiocytes, and plasma cells. Histopathological examination showed that the cells were positive for human CD68 (KP1), CD68 (PGM1), and CD163; however, they were negative for the AF, epithelial membrane antigen, and glial fibrillary acidic protein. Tuberculosis-deoxyribonucleic acid test and special stains for acid-fast bacilli and fungi were negative. The diagnosis was finally made as isolated neurosarcoidosis. Then the patient was treated with additional corticosteroid therapy. Serial imaging examination 4 months later revealed that the lesions extremely decreased.Conclusion: The diagnosis of isolated central nervous system sarcoidosis was still difficult because of limitations of available diagnostic tests. So neurosarcoidosis in the clinical work should never be neglected.
Neurological emergencies masquerading as psychiatric disturbances detected by neuroimaging in individuals with pre-existing mental illness: A case series

Abstract:Certain clinical features, like changes in presentation, the presence of neurological signs, and a poor response to treatment, mandate an organic evaluation of psychiatric illnesses. In this case series, four cases with established mental illnesses who presented with psychiatric symptoms, on evaluation by neuroimaging, were found to have neurological disorders like neurocysticercosis, sub-arachnoid hemorrhage, sub-dural hemorrhage, and lacunar infarction. They were managed with a combination of medical and surgical treatment and psychotropics, and no relapse of symptoms was noted at follow-up. The formulation of clinical practice guidelines can be a way forward in holistic management for such patients.
A Case Report on a Common Tumour With an Uncommon Presentation: Glioblastoma

Abstract:The most common type of primary brain tumour in adults is gliomas although rare. Glioblastomas are a subtype of gliomas with the worst prognosis having the ability to rapidly increase in size, even doubling within days to weeks. Patients can present with varied presentations depending on the site of the involvement, thus misleading in diagnosis due to vagueness. The most common clinical presentations include headaches, seizures, and focal neurological signs. However, there can be atypical presentations like personality changes and back pain due to meningeal irritation which may be the only presenting complaint in the early stages. Magnetic resonance imaging (MRI) is usually considered the only investigation required for the diagnosis of the illness. However, it can mislead in the early stages. Therefore, brain biopsy remains the gold standard in the diagnosis of glioblastoma multiforme. It is important to identify the subtype to decide on the prognosis and plan the management thereafter. Here, we present a 49-year-old woman with prominent personality changes, depressive symptoms, and atypical brain imaging findings. The definitive diagnosis was made with the brain biopsy as two MRI findings were contradictory. This article highlights the importance of suspicion of primary brain tumours in adults presenting with atypical neuropsychiatric manifestations.

Abstract:BACKGROUND We described a case of a patient with a meningioma in the posterior fossa presenting atypically with an isolated unilateral vocal cord palsy causing severe respiratory distress. This is of interest as the patient had no other symptomatology, especially given the size of the mass, which would typically cause a pressure effect leading to neurological and auditory symptoms. CASE SUMMARY This case report described a 48-year-old male who was married with two children and employed as a car guard. He had a medical history of asthma for the past 10 years controlled with an as-needed beta 2 agonist metered dose inhaler. He initially presented to our facility with severe respiratory distress. He reported a 1-wk history of shortness of breath and wheezing that was not relieved by his bronchodilator. He had no constitutional symptoms or impairment of hearing. On clinical examination, the patient’s chest was “silent.” Our initial assessment was status asthmaticus with type 2 respiratory failure, based on the history of asthma, a “silent chest,” and the arterial blood gas results. CONCLUSION A posterior fossa meningioma of such a large size and with extensive infiltration rarely presents with an isolated unilateral vocal cord palsy. The patient’s chief presenting feature was severe respiratory distress, which combined with his background medical history of asthma, was misleading. Clinicians should thus consider meningioma as a differential diagnosis for a unilateral vocal cord palsy even without audiology involvement.

Abstract:Introduction We present the case of a 70-year-old man who, after presenting atypical depressive symptoms, was diagnosed with incipient frontotemporal dementia. Objectives Through the presentation of the case, a brief review is made of the affective prodromes of frontotemporal dementia Methods The patient, who had no personal history of interest, suddenly began to present depressive symptoms consisting of marked irritability, dysphoric mood, anxious semiology with a subjective feeling of anguish, maintenance insomnia and a feeling of lack of self-control, with a tendency towards verbal heteroaggressiveness. The patient reported all these symptoms with great suffering. After one year of treatment with venlafaxine 300g DMD and quetiapine 400g DMD, with one admission to the short-stay inpatient unit for self-harm threats, the patient had not experienced any improvement. In addition, during this year, the patient's family began to observe small memory lapses that affected his daily functioning, making the patient progressively more dependent. Results In view of this clinical picture, it was decided to request an MRI and a brain PET scan, where deficits in the frontal and temporal regions were observed, and a diagnosis of incipient frontotemporal dementia was made. Conclusions Frontotemporal dementia is the third most common dementia in people over 65 years of age. About half of the patients debut with psychiatric symptoms, one of them being depressive symptoms. Treatment is focused on the use of psychotropic drugs with the aim of symptom management. Olanzapine or aripiprazole are effective for psychotic symptoms or acute agitation. For more subacute conditions, SSRIs or trazodone are recommended. The iACOs are not recommended, because they are ineffective and worsen neuropsychiatric symptoms. Disclosure of Interest None Declared

Abstract:In this report, we present the case of a 71-year-old African-American woman experiencing 2 months of intermittent headaches and episodes of blurred vision. Despite a comprehensive medical history that revealed chronic conditions and previous unrelated surgeries, the initial evaluation appeared to be unremarkable. Following the discovery of a mass on an imaging and a subsequent biopsy, the diagnosis of craniopharyngioma (WHO grade I) was confirmed. However, a brain mass was identified after additional ophthalmologic examination and MRI. This case explores the significance of recognizing atypical presentations of a brain injury that required a specific approach for diagnosis, surgical intervention and treatment, and postoperative care. This case contributes to the constantly evolving understanding of atypical manifestations of tumor characteristics and their complexities, along with the need to develop appropriate patient management strategies and provide optimal outcomes.

Abstract:Introduction Tuberous sclerosis complex (TSC) is a disorder that affects multiple systems and was first described in 1880. Its symptoms include seizures, intellectual disability, and adenoma sebaceum. TSC is caused by mutations in the TSC1 and TSC2 genes and is inherited in an autosomal dominant manner. Objectives This report highlights a case of a patient with an unusual psychological presentation evaluated in a psychiatric hospital. Methods The patient presented with psychotic features and abnormal behavior. A physical examination showed neurocutaneous lesions. After assessment a diagnosis of Tuberous sclerosis complex was confirmed through MRI Brain and genetic testing. Some of his relatives also showed similar neuropsychiatric symptoms. Results Tuberous sclerosis complex is diagnosed based on TSC Clinical Consensus Group guidelines of 2012. Our patient fulfilled 4 of the major criteria and genetic testing also yielded a pathogenic variant. A TAND checklist (TSC-Associated Neuropsychiatric Disorders) is used to guide clinicians on areas to prioritize when managing TSC patients. Image: Image 2: Image 3: Conclusions Given that psychiatry may be the first contact for TSC patients, especially in low-resource settings. Patients referred to psychiatry, therefore, need to be thoroughly examined to exclude neuropsychiatric disorders, and a multidisciplinary team approach is vital in investigating and managing these cases. Abstract:Differential diagnosis of metastatic brain tumor, carcinomatous meningitis, and paraneoplastic neurological syndrome (PNS) can be challenging in atypical cases. When examining patient with increased T2 fluid-attenuated inversion recovery (FLAIR) hyperintensities in the temporal polar white matter, autoimmune encephalitis, including PNS, should be considered. Herein, we report the case of an 85-year-old man with carcinomatous meningitis due to lung large cell carcinoma. He showed disturbance of consciousness, abnormal behavior, incomprehensible speech, and apathy, which suggested brain dysfunction. Magnetic resonance imaging revealed high intensities on the whole cerebellum on a diffusion-weighted image and bilateral T2 FLAIR hyperintensities in the temporal polar white matter. Cerebrospinal fluid analysis and cytology showed elevated total protein levels, pleocytosis, and atypical cells with nuclear enlargement, hyperchromasia, and irregular shape. Autopsy revealed lung large cell carcinoma and its brain metastasis. Tumor cells were disseminated to the central nervous system along the subarachnoid space. Furthermore, plenty of carcinoma cells and peritumoral enlarged perivascular space were observed in the temporal poles. To our knowledge, this is the first report of bilateral T2 FLAIR hyperintensities in the temporal polar white matter caused by carcinomatous meningitis with pathological confirmation. In patient with carcinomatous meningitis, abnormal T2 FLAIR hyperintensities may not be derived from ischemia or tumor invasion to parenchyma.

Case 317: Adult-Onset Leukoencephalopathy with Axonal Spheroids and Pigmented Glia

Abstract:A 44-year-old previously healthy man with a 9-month history of progressive cognitive decline, depression, urinary incontinence, and inability to perform tasks of daily living presented to the emergency department with worsening cognitive and neuropsychiatric symptoms. He had become more distressed, and his family noticed him departing the house without closing doors, leaving water faucets running, and sending his children to school on Sundays. History taken from the patient's wife revealed that his brother had passed away in his late 30s after a slowly progressing functional and cognitive decline over the course of 5 years. No further detailed family history could be obtained. The review of systems was negative; he had no prior medical, psychiatric, or surgical history; and he denied any history of recent travel, camping, hiking, or vaccination. The patient was not taking any dietary supplements, nor was he taking any over-the-counter or prescription medication. Examination revealed vital signs were within normal limits. Neurocognitive assessment revealed a conscious, coherent, and alert patient with impaired memory and concentration. He showed poor attention, depressed mood, and restricted affect. He was unable to spell the word world forward, nor was he able to understand a request to spell it backward. The rest of the physical and neurologic examination revealed no abnormalities. Extensive laboratory work-up was conducted and included the following: toxicology screening; screening for HIV-1, HIV-2, and syphilis treponemal antibodies; COVID-19 polymerase chain reaction; and measurement of B1 and B12 levels. The results of screening were negative. Cerebrospinal fluid (CSF) assays, including CSF oligoclonal bands and CSF flow cytometry, revealed values within normal limits. CT of the brain without intravenous contrast material was performed in the emergency department to rule out acute intracranial abnormality. Multiplanar multisequence MRI of the brain without and with intravenous contrast material was ordered for further assessment. CT images of chest, abdomen, and pelvis were unremarkable (images not shown).
Giant Meningioma Diagnosis and Clinical Treatment: A Case Report

Abstract:This case report shows the importance of semiology during a clinical examination not only to diagnose spine clinical symptoms but also to review the central nervous system tumor as a foot drop cause. We report a unique case of a patient who consulted for constant progressive numbness and motor symptoms in the right lower extremity. Lumbar CT and MRI were negative for acute or chronic lumbar pathology. This is a 41-year-old female patient with a history of eight-month progressive right leg weakness. The physical examination did not reveal neurological alterations besides foot drop. MRI and lumbar X-rays showed no significant findings. Electromyography (EMG) indicated right peroneal neuropathy. Based on these findings, surgical treatment was not indicated; therefore, physical therapy and a referral to neurology were indicated. However, symptoms increased, resulting in a right lower extremity hemiparesis. A brain MRI showed a left frontoparietal giant meningioma, which was surgically resected after embolization. The patient evolved with a full recovery of the right-sided hemiparesis after surgery. Our case highlights the foot drop's multiple etiologies. Initially, a lumbar disc hernia was suspected, but it was ruled out by imaging studies. Later, the EMG revealed peroneal neuropathy, leading to a neurology consult. Unexpectedly, a giant intracranial meningioma was found, a rare case of foot drop. A consideration of upper motor neuron (UMN) and lower motor neuron (LMN) syndromes aided diagnosis. Tumoral resection with embolization resulted in significant improvement, showcasing the complexities of such cases. Foot drop is a peculiar clinical manifestation that must have an integral assessment to rule out peripheral and central causes. Even rare, giant meningiomas can cause focalized symptoms such as foot drop. Therefore, the assessment of foot drop should include the CT and MRI of the central nervous system.
Venus banded terrain: Tectonic models for band formation and their relationship to lithospheric thermal structure

Abstract:Recent radar images of the mountains of Ishtar Terra, Venus, at approximately 3-km radar resolution show a series of linear bands of alternatively greater and lesser backscatter; the bands are generally aligned with the topographic strike of the mountain ranges. We test the hypothesis that these band features are tectonic in origin, the product either of folding during lithospheric compression or of block faulting during lithospheric extension. Tested models for folding include uniform elastic and viscous plates overlying inviscid substrates, layered plates, and a viscous half space in which viscosity decreases exponentially with depth. The uniform plate models can produce folds at a dominant wavelength equal to the characteristic spacing between bands on Venus, about 15 to 20 km, if the elastic or high-viscosity layer is, at most, a few kilometers in thickness, though the required compressive stresses are several kilobars in magnitude. For the case of an exponentially decreasing viscosity the skin depth plays the role of layer thickness, and similar results hold. Layered elastic or viscous plates, however, can fold at the required wavelength with subkilobar stresses and, therefore, are favored over uniform plate models. Tested extensional models include graben and horst formation in a surface elastic-brittle layer, imbricate normal faulting, and necking of a surficial layer that is everywhere in a state of extensional failure. The spacing between adjacent extensional features can match the spacing between bands on Venus if the surficial brittle layer is no more than a few kilometers in thickness. Both compressional and extensional models for band formation are therefore consistent with radar images of Venus mountain ranges at the image resolution presently available. Additional evidence, however, including the linearity and continuity of individual bands, the relationship between band trends and topographic contours in some regions near the ends of mountain ranges, and the very different manifestations of lithospheric extension in other highland areas of Venus, leads us to favor the hypothesis that banded terrain formed contemporaneously with the mountain ranges of Ishtar Terra as a result of horizontal compression of the Venus lithosphere. Independent arguments on the thermal structure of the Venus crust and the mechanical behavior of crustal rocks as a function of temperature indicate that the elastic lithosphere of Venus is approximately 1-10 km thick. We therefore suggest that the surficial layer of elastic-brittle or high-viscosity behavior required for either the compressional or extensional models may in fact be the elastic lithosphere of the Venus highlands at the time of band formation.
Case report of a patient with an intraosseous meningioma presenting as possible metastasis from prostate cancer: Diagnostic dilemma and review of literature

Abstract:Intraosseous meningiomas are a rare subtype of meningiomas representing approximately 2% of all cases. They can confound a diagnosis of other bone lesions including metastatic tumors. We present a case of a patient with prostate cancer who on staging workup was suspected to have a skull metastasis. Both bone scan and CT Head demonstrated a lesion in the right frontal calvarium. Surgical resection and pathology revealed an intraosseous meningioma. The patient was restaged as having localized prostate cancer and the was offered curative treatment for his malignancy. The case highlights the importance of obtaining tissue diagnosis in cases of radiographic isolated oligometastatic disease in patients with a known primary malignancy.

Abstract:The simultaneous occurrence of two distinct intracranial neoplasms within a single patient presents a unique and exceedingly rare clinical scenario. Such cases pose significant diagnostic and therapeutic challenges, complicating clinical decision-making and treatment planning. Understanding the intricacies of these cases is crucial for improving patient outcomes and advancing clinical knowledge. This report aims to provide valuable insights into the complexities surrounding the management of patients with coexisting different intracranial neoplasms. This case report seeks to enhance the understanding of healthcare professionals dealing with similar cases and contribute to the existing literature on this rare phenomenon. This case report details a patient harboring two different but coexisting intracranial neoplasms. The patient, a 73-year-old woman, presented with behavioral changes and bilateral leg weakness. Initial imaging studies, including CT and MRI scans, revealed the presence of two distinct intracranial masses located in the frontal lobes and the falx cerebri. Neurosurgical, chemotherapy, and radiotherapy were offered; however, the patient's family opted for palliative care. The concurrent presence of two distinct intracranial neoplasms in a single patient underscores the complexity of such cases. This report highlights the diagnostic and therapeutic challenges encountered and the importance of a multidisciplinary approach to managing these patients. By sharing this case, we hope to contribute to a broader understanding of such rare clinical scenarios and aid in developing more effective management strategies in the future.
Drug-resistant schizophrenia-like psychosis associated with temporal non-anaplastic pleomorphic xanthoastrocytoma: unusual revealing symptom of a rare pathology

Abstract:Introduction and importance: Pleomorphic xanthoastrocytoma (PXA) was first described by Kepes et al. in 1979. Fewer than 200 cases have been reported in the literature. It generally involves the temporoparietal lobe. PXA has a favorable prognosis. The most reported clinical manifestation is epileptic seizures. Revealing psychiatric symptoms have an incidence varying from 50 to 78%. The most common symptoms encountered are anxiety disorders, depression, schizophrenia-like psychosis, cognitive dysfunction or even anorexia nervosa. Case presentation: Here, the authors report a new case of non-anaplastic pleomorphic xanthoastrocytoma revealed by a drug-resistant schizophrenia-like psychosis in a 26-year-old male patient known with epileptic seizures in whom these two pathologies were intertwined and had been evolving for 5 years. The postoperative course was uneventful, and positive symptoms of schizophrenia were relatively stabilized at discharge. Clinical discussion: Given the superficial hemispheric location of PXA, the most common clinical presentation is seizures. Psychiatric symptoms revealing brain tumors have an incidence varying from 50 to 78%. Most of these symptoms concern frontal and limbal tumors. In their case, the tumor was located in the right temporal lobe. Surgery was performed and postoperative course was uneventful even though there are conflicting reports regarding the importance of the surgical excision quality. Conclusion: PXA remains a rare and benign primary CNS tumor. Psychiatric disorders represent a rare revealing mode of this pathology, which must lead to neuroimaging in any patient carrying this type of symptoms.
Challenging Resection of Bilateral Parasagittal and Falcine Meningioma Involving Both Anterior Third and Middle Third of the Superior Sagittal Sinus: A Case Report and Literature Review

Abstract:Meningiomas typically manifest as benign, slow-growing, and well-defined tumors on a macroscopic level and are usually asymptomatic. However, the mass effect caused by large meningiomas may lead to various neurological symptoms, commonly headaches and visual problems. Radiological imaging can establish the diagnosis, and a biopsy can provide a definitive diagnosis. Our case report describes the surgical intervention for bilateral parasagittal-falcine meningioma in a 57-year-old male who presented to the emergency department with a tonic-clonic seizure. On examination, he had a bifrontal longitudinal mass. Magnetic resonance imaging (MRI) revealed a large anterior superior falcine extra-axial mass, measuring about 5.7 x 5.3 x 3.1 cm, with surrounding vasogenic edema and superior sagittal sinus invasion. He underwent surgery for tumor resection involving the anterior third and middle third of the superior sagittal sinus without radiotherapy. He did not develop any intraoperative complications, and during the post-operative evaluation, he was symptom-free. A follow-up MRI with contrast performed three months later showed no neurological complications or recurrent tumor. To achieve better outcomes, surgical intervention for parasagittal and falcine meningiomas involving the superior sagittal sinus should aim to eliminate clinical signs, control tumor growth, and prevent neurological deterioration post-operatively.
Neuropsychiatric symptoms in Multiple Sclerosis (MS): Case Report of a First Manic Episode in a Patient with Suspected MS

Abstract:Introduction Multiple Sclerosis (MS) is an inflammatory disease affecting primarily the central nervous system, characterized by focal lesions of white-matter demyelination. It can present with a variety of neurological symptoms, including monocular vision loss, sensory loss, paresthesias, limb weakness, ataxia and bladder dysfunction, and has a typically chronic and progressive course. Neuropsychiatric manifestations including depressive or manic symptoms, anxiety disorders and psychosis, are also frequently observed, and are of particular importance to mental health practitioners. Objectives To describe a case of a 45-year-old female patient with a history of suspected MS presenting with manic symptoms, and to discuss the possible neuropsychiatric manifestations of Multiple Sclerosis. Methods Clinical case report and literature review. Results A 45-year-old woman was brought to the emergency department presenting with severe acute agitation, irritable mood, rapid speech and persecutory delusions. She had no prior history of neuropsychiatric symptoms, but her medical history was notable for a suspected diagnosis of MS, having suffered an episode of optic neuritis 16 years before the present episode. Magnetic Ressonance Imaging performed 3 months before emergency admission documented non-specific white-matter lesions presenting as hyper-intense in long TR sequences, as well as a cervical lesion of atypical characteristics, representing possible spondylotic myelopathy or demyelination. A head CT performed at emergency admission did not reveal relevant acute findings. The patient was hospitalized and initiated risperidone and valproic acid therapy. She responded favorably to medication, with progressive stabilization of mood and remission of delusional ideas over three weeks. Conclusions Neuropsychiatric symptoms are a common and concerning manifestation of Multiple Sclerosis. The present case illustrates that clinicians should be on alert for signs of mood and psychotic symptoms in patients with suspected or confirmed MS, as these can manifest at any point during the disease course.
