Abstract:Dr. Anandibai Joshi was the first Indian female physician to complete her studies in Western medicine at the Women's Medical College of Pennsylvania. A pioneer in medicine, she was a trailblazing Indian woman, who had faced many hurdles during her quest for education. In her short span of life, she faced a lot of emotional, mental, physical, and social turbulence and had to prove herself at every step to the orthodox patriarchal Indian society the need to study medicine. Studying medicine in India was impossible for females during the 19th century. During those days, the females in India were uneducated due to gender differences prevalent in society, so the only option was to pursue medicine in a foreign land. Her life was full of hardships and challenges; she got married at the age of nine and, at the young age of 14, delivered a baby boy who could not survive beyond 10 days due to lack of medical care. This was the turning point in her life, and she was determined to become a physician so that the other women would not suffer the way she had. Thus began her educational journey, to pursue a career in Western medicine. Before Anandibai went to the United States, she gave a motivating speech in a public gathering, expressing the need for female doctors in India and her keenness to pursue medicine in the United States. She challenged all the social norms in those days, whether it was fighting to break gender stereotypes or convincing the male-dominated society to study medicine in the United States. This article is a reflection of Anandibai's accomplishments in her life and medical career and her dedication, determination, and strong commitment to improving the lives of Indian women. Her short life is a reflection of hope, perseverance, determination, and success and an inspiration to many Indian women to pursue education and medicine.

Abstract:A study of the coexistence of sparrows in grassland and woodland habitats of the Research Ranch in southeastern Arizona supports the predictability of current community theory. For each of the habitats studied, the production of seeds and the abundance of seed-eating, winter-resident sparrows were measured. The theory correctly predicts the number of species supported in each of the habitats. In both cases for which the prediction is that only one species could be supported, the theory correctly predicts which species should be present.

Throughout her illustrious career, Bagshaw demonstrated exceptional perseverance, navigating familial obligations following her father's untimely passing during her academic pursuits. Establishing her practice in Hamilton, Ontario, Bagshaw delivered over 3,000 neonates, frequently offering pro bono care to immigrant populations. Notably in 1932, Bagshaw assumed the role of Medical Director of Canada's first birth control clinic, challenging restrictive legislation and paving the way for the legalization of contraception in 1969. Abstract:Disaster management (DM) is a continuous, highly collaborative process involving governments, DM organisations, responders, the construction sector, and the general public. Most research approaches to DM include the development of information and communication technologies (ICT) to support the collaboration process rather than the creation of a collaboration process to provide information flows and patterns. An Intelligent Disaster Collaboration System (IDCS) is introduced in this paper as a conceptual model to integrate ICT into DM and the mitigation process and to enhance collaboration. The framework is applicable to the collaboration process at the local, regional and national levels. Within this context, the deployment of ICT tools in DM is explored and scenario-based case studies on flooding and terrorism--examples of natural and human-induced disasters, respectively--are presented. Conclusions are drawn regarding the differences found in collaboration patterns and ICT used during natural and human-induced disasters and the differences between currently available ICT and proposed ICT.

Abstract:A certain number of hairdressing parlour employers have been taken to court because their employees allegedly suffered from lung allergies caused by the vapours from PPD (paraphenylendiamine) which allegedly developed when applying hair dyes. These conclusions may be reached if the specific hazards of the rubber and particularly leather and fur dyeing industry are extrapolated as is widely reported in literature, to hairdressing employees. The purpose of this study is to assess the actual exposure rate to PPD vapours of hairdressing employees during a working day, on the basis of the type and characteristics of the salon (small, medium and large), the number of dye applications per day and the chemical‐physical characteristics of the oxidation dyes. The results prove that, even under the hypothesis of experimental conditions pushed to the extreme, it is not reasonable to speak of exposure to PPD through the lungs of hairdressing employees.

Abstract:Dr. Nitya Anand was a leading figure in Indian pharmaceutical research. His innovation of Centchroman (Saheli), the world's first non-steroidal oral contraceptive pill, changes the course of contraception use in India. Abstract:This is an analysis of important aspects of health equity in caring for children and adolescents written by a multidisciplinary team from different medical centers. In this discussion for clinicians, we look at definitions of pediatric health equity and the enormous impact of social determinants of health in this area. Factors involved with pediatric healthcare disparities that are considered include race, ethnicity, gender, age, poverty, socioeconomic status, LGBT status, living in rural communities, housing instability, food insecurity, access to transportation, availability of healthcare professionals, the status of education, and employment as well as immigration. Additional issues involved with health equity in pediatrics that are reviewed will include the impact of the COVID-19 pandemic, behavioral health concepts, and the negative health effects of climate change. Recommendations that are presented include reflection of one's own attitudes on as well as an understanding of these topics, consideration of the role of various healthcare providers (i.e., community health workers, peer health navigators, others), the impact of behavioral health integration, and the need for well-conceived curricula as well as multi-faceted training programs in pediatric health equity at the undergraduate and postgraduate medical education levels. Furthermore, ongoing research in pediatric health equity is needed to scrutinize current concepts and stimulate the development of ideas with an ever-greater positive influence on the health of our beloved children. Clinicians caring for children can serve as champions for the optimal health of children and their families; in addition, these healthcare professionals are uniquely positioned in their daily work to understand the drivers of health inequities and to be advocates for optimal health equity in the 21st century for all children and adolescents.

Abstract:Journal of Transcultural Nursing, Ahead of Print. Introduction:We used an implementation science framework to investigate perceived health inequities in preventive health care initiatives in rural communities in the Philippines. We also identified health equity–focused quality improvement strategies.Methods:Three health care providers and 12 barangay clinic patients were interviewed.Results:Patient interviews showed that social determinants of health, respect, and attitudes toward health care providers affected patient empowerment to engage in self-management for noncommunicable diseases (NCDs). Health care providers identified six challenges in managing NCDs: (a) inefficient work processes; (b) staffing shortages; (c) insufficient access to low-cost medications; (d) inadequate primary preventive health education from the schools; (e) health care not considered a priority; and (f) lack of local government support.Discussion:Inadequate preventive health care and education intensify health access and resource inequities in rural communities in the Philippines. A multi-sectoral plan may improve rural health infrastructure and education toward improving health care access and decreasing care disparities.

[Life and Works of Heo Yeng-suk, the First Female Medical Practitioner in Modern Korea]

Abstract:Heo Yongsuk (1897-1975) was the second female medical doctor to study Western medicine in a foreign country, the second female journalist, and the one of the representative 'new modern woman' in Korea. She is unfamiliar, however, to Korean people. Few historians of medicine and few researchers of the history of literature recall her for her own achievements, instead remembering her as a wife who saved her husband, Yi Gwangsu (1892~1950), the great novelist, from his dreadful tuberculosis. Removing her from the shadow of Yi Gwangsu, this paper tries to uncover her life and her contribution to Korean society during the Japanese colonial period. As a pioneer, she went to Japan to study medicine in 1914 for the purpose of breaking down the long-established custom of female patients, who abhorred showing their bodies to male doctors. After acquiring her license, she opened in Korea for women and children, though this clinic had a brief span of only two years owing to her devotion to caring for her husband as his disease worsened. She became a reporter in place of her husband for about two years. However, with her efforts, she gave women a considerable amount of useful medical information. She wrote many enlightening articles to awaken Korean women's 'nationalistic spirit' against Japanese colonial oppression. She is worthy of a favorable evaluation as the second female reporter and the first who specialized in medicine in the history of newspapers in Korea. As a 'new modern woman,' she presented her own thinking about the best role model for married females, by saying, "Be good mother and good wife in the family household, it is the best way to strengthen Korean race." When she became pregnant, she resigned her job as a reporter. She exerted herself by bringing up her children and nursing her sick husband, gaining fame as the representative of the conservative women's movement. Medical knowledge was always behind her various activities. She can be evaluated successfully as a medical doctor; after studying newly developed medical theory and skills for about two years in Japan, she established first the specialized hospital for delivery in 1937 and had great success. As a successful female doctor, she projected a positive image of a new modern woman who was loyal to her family household, unlike many famous modern females who pursued women's liberation during the Japanese colonial period.

Abstract:Education and career choices are two major life decisions.1 One who considers a degree in Medicine may ask: “am I up to the challenge,” “am I curious about how medicine improves life,” and “do I care profusely about human life?” Practicing clinical medicine is the most common path; but, not all those with a career in Medicine are physicians. There are researchers in the basic sciences, and there are inventors and innovators of biomedical devices. All those working in Medicine start with valuing and pursuing the improvement of life. The ideal medical school integrates disciplines for a complete understanding of medicine.2 Two of the articles in this edition describe challenges facing Philippine medical schools. The first article re-evaluates the adjustment factor used in the admission process. The second article highlights flexibility and innovation in the sudden shift to online learning. The MD-PhD Program connects scientific research with clinical practice. In the US, this program started in the 1950s and expanded through the Medical Scientist Training Program to more than 90 active MD-PhD programs in 2016.3 The country’s first MD-PhD Program started in 2010 at the University of the Philippines College of Medicine (UPCM). One of our physician-scientist graduates authored an article on the pharmacologic potential of locally harvested edible mushrooms. Many other UPCM graduates do research at the Philippine General Hospital. As an academic medical center, it houses “a large and diverse sample of patients, experienced and interested investigators, with organizational support.”4 In this edition, physician trainees and faculty authored or mentored seven studies. They study various disease and systems processes with the goal of health promotion. Three articles studied hospital processes related to health service delivery. Radiation oncologists explored delays in repeated CT simulation for the radiation of head and neck cancer patients. Ophthalmologists formed a time quality management team to reduce the total consultation time at their outpatient clinic. OB-GYN sonologists looked at methods of manual disinfection of transvaginal probes to keep patients safe while undergoing such procedures. Two articles studied the epidemiology of cerebrovascular diseases and neoplasms (periampullary malignancies), two of the top causes of mortality in the country. Two other studies looked at diseases that affect the quality of life: visual impairment in the young, and the use of a modified approach to hip arthroplasty. Case reports remain valuable sources of unusual information and new ideas.5 This edition also features a case report on a rare childhood malignancy found in an elderly patient who was successfully treated. The road from discovery to healing can take many forms as exemplified by the 11 articles in this edition of Acta Medica Philippina. Publication is a step on the road from discovery to healing. To complete the journey is to implement their findings in clinical practice. Many implementation challenges exist due to differing approaches.6 The road from discovery to healing is not an easy one. We hope to close the gap between evidence and practice; we contribute to an ever-evolving and growing body of knowledge; and we offer the best treatment to our patients, the lives we value the most. Abstract:Sofia Quintero Romero graduated as a medical doctor at Universidad del Rosario, Bogotà, Colombia, in 1977. She spent a compulsory rural year working in a remote indigenous community in the Sierra Nevada de Santa Marta in Columbia. In 1979 she worked in Bolivia for Terre des Hommes and Oxfam, evaluating their health projects with the Aymara Indians and in the tin mines. She had to leave Colombia for political reasons and went to England, where she obtained, in 1981, an MSc in Community Health at the London School of Hygiene & Tropical Medicine (LSHTM). That's where she met her husband, Adriano Cattaneo. She then worked in Mozambique and Nicaragua where she was in charge of maternal and child health services at the regional level. Since 1990, she devoted her time to the protection, promotion, and support of breastfeeding. Sofia obtained a PhD in Maternal and Child Health at the University of Bologna, Italy, and a Diploma in Breastfeeding Theory and Practice at the Child Health Institute in London UK. She taught hundreds of breastfeeding courses for health professionals and peer counsellors in Italy and in dozens of countries abroad, using the World Health Organization/United Nations Children's Fund (WHO/UNICEF) manuals. In the past 15 years, she changed her approach for breastfeeding education to biological nurturing. Sofia has been a member of the International Baby Food Action Network (IBFAN) since 1992 and coordinated the Nestlé Boycott in Italy. She retired in 2018.
Little Doctors: Agents of change in Indian rural communities

Abstract:The child-to-child approach to health advocacy is one that draws on the strengths and agency of children to make a positive impact within their communities. The approach has been popularly used for health education in low- and middle-income countries. This article describes the 'Little Doctors' program that implemented the child-to-child approach in the towns of KC Patty and Oddanchatram, located in remote hilly regions of Tamil Nadu, India starting in 1986 to train middle- and high school children to respond to diseases prevalent in their communities along with practices for preventative measures. The program involved sessions that used a combination of creative instructional methods to engage students and provided take-home messages for them to act on with their families and community. The program was successful in creating a creative learning environment for children, offering a shift from the traditional methods of classroom instruction. Students who successfully completed the program were awarded certificates as 'Little Doctors' in their communities. Although the program did not conduct formal evaluations of the program effectiveness, students reported successfully recalling complex topics such as early signs of diseases like tuberculosis and leprosy that were prevalent in the community during the time. The program experienced several challenges and had to be discontinued despite its continued benefits to the communities.

Abstract:P450 RhF from Rhodococcus sp. NCIMB 9784 is the first example of a new class of cytochrome P450 in which electrons are supplied by a novel, FMN- and Fe/S-containing, reductase partner in a fused arrangement. We have previously cloned the gene encoding the enzyme and shown it to comprise an N-terminal P450 domain fused to a reductase domain that displays similarity to the phthalate family of oxygenase reductase proteins. A reductase of this type had never previously been reported to interact with a cytochrome P450. In this report we describe the purification and partial characterization of P450 RhF. We show that the enzyme is self-sufficient in catalyzing the O-dealkylation of 7-ethoxycoumarin. The P450 RhF catalyzed O-dealkylation of 7-ethoxycoumarin is inhibited by several compounds that are known inhibitors of cytochrome P450.
