Abstract:The objectives of this study were to investigate the pattern of unintentional drowning deaths and to identify the factors associated with drowning deaths in Thailand from 2000 to 2009. Reported death data were obtained from the Bureau of Health Policy and Strategy, Ministry of Public Health. During the period from 2000 to 2009, 41 620 deaths were reported and 1016 deaths were excluded from this study because of unknown place of death. The overall drowning death rate per 100 000 population was 6.3 (9.6 in males and 3.0 in females). The highest death rate was in males aged 0 to 4 years and in year 2006. Males in all age-groups (except in those aged 15-29 years) and females aged 0 to 4 years had higher drowning death rates than the average rate. Death rates in some public health areas in the central, the northeast, and the northern regions and in year 2005 and 2006 were higher than the average rate.

Abstract:AIMS:To investigate the relationship between serum phospholipid omega-3 polyunsaturated fatty acids (ω-3 PUFAs) and insulin resistance (IR) in patients with type 2 diabetes mellitus (T2DM) and non-alcoholic fatty liver disease (NAFLD). METHODS:51 patients with T2DM and NAFLD (T2DM+NAFLD group), 50 with T2DM alone (T2DM group), 45 with NAFLD alone (NAFLD group), and 42 healthy control subjects (NC group) were studied. Serum ω-3 PUFA profiles were analyzed by gas chromatography, and alanine aminotransferase (ALT), aspartate aminotransferase (AST), γ-glutamyltransferase (GGT), and serum lipid concentrations were measured. Insulin resistance was assessed by the homeostasis model assessment method (HOMA-IR). RESULTS:HOMA-IR levels were higher in the T2DM+NAFLD group than in the T2DM, NAFLD and NC groups (p<0.05), as were ALT, AST, GGT, total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and triglyceride (TG) concentrations (p<0.05). Conversely, serum ω-3 PUFA levels were significantly lower in the T2DM+NAFLD group than in the other groups (p<0.05). The ω-3 PUFA level was negatively correlated with HOMA-IR, TC, LDL-C and TG. CONCLUSIONS:Serum phospholipid ω-3 PUFA levels were significantly decreased in patients with T2DM and NAFLD, and were negatively related with insulin resistance. Thus, reduced ω-3 PUFAs may play an important role in the development of T2DM and NAFLD.
Relationship between Plasma Phospholipid Fatty Acid Profile and Insulin Resistance in Type 2 Diabetics

Abstract:Objective To investigate the relationship between plasma phospholipid fatty acid profile and insulin resistance(IR) in patients with type 2 diabetes(T2DM).Methods Totally 74 patients with T2DM from coastal areas of Zhejiang province were enrolled.Plasma phospholipid fatty acids were analyzed with capillary gas chromatography.Homeostasis model assessment(HOMA-IR) was applied to assess the status of insulin resistance(IR).Plasma lipids were measured by enzymatic assay.Results(1) Bivariate correlation analysis showed fasting plasma glucose(FPG) was negatively correlated with C20:2n-6,HbA1c was negatively correlated with C20:2n-6 and C20:3n-6;C-peptide was positively correlated with C14:0,C16:0,saturated fatty acid(SFA),waist hip ratio(WHR),triglyceride(TG),and negatively correlated with C23:0,polyunsaturated fatty acid(PUFA),high-density lipoprotein cholesterol(HDL-C).Fasting insulin(FINS) was positively correlated with C16:0,n-6/n-3,Body mass index(BMI),WHR and TG,and negatively correlated with C22:6n-3,n-3PUFA and HDL-C.HOMA-IR was positively correlated with C16:0,BMI,total cholesterol(TC),TG and Low-density lipoprotein cholesterol(LDL-C),and negatively correlated with HDL-C.(2) In the multiple regression analysis(Stepwise regression),the HOMA-IR was predicated significantly only by C16:0,BMI and TC;and the general correlation coefficient was 0.716.Conclusion In patients with T2DM the insulin sensitivity is modulated by C16:0,BMI and TC;Increasing plasma phospholipid n-3 PUFA proportion and reducing C16:0 proportion may improve the insulin sensitivity.

Abstract:BACKGROUND: Associations of either insulin receptor substrate 1 (IRS1) variants or circulating 25- hydroxyvitamin D [25(OH) D] with type 2 diabetes (T2D) and insulin resistance (IR) are inconsistent. This study sought to determine whether circulating 25(OH) D modulates the association of a potentially functional variant at IRS1 (rs2943641) with insulin resistance.METHOD: Interaction between IRS1 rs2943641 and circulating 25(OH) D on homeostasis model assessment for IR (HOMA- IR) was examined in the Boston Puerto Rican Health Study (BPRHS) (n = 1144). Replication was performed in the African- American (n = 1126), non- Hispanic white (n = 1967), and Hispanic (n = 1241) populations of the Multi- Ethnic Study of Atherosclerosis (MESA) with genotypes of 3 IRS1 variants, rs2972144, rs1515104, and rs2673142, which are tag single nucleotide polymorphisms (SNPs) and in strong linkage disequilibrium with rs2943641.RESULTS: Higher circulating 25(OH) D was associated with lower risk ofT2DandIR inBPRHSwomenhomozygous for minor allele rs2943641T. Consistently, in each of 3 MESA populations, HOMA- IR and insulin decreased more evidently with higher circulating 25(OH) D in women of the rs2943641TT genotype than in carriers of the major allele (rs2943641C). Metaanalysis indicated significant and consistent interactions between circulating 25(OH) D and IRS1 variants on HOMA- IR (log transformed) [pooled beta = -0.008, 95% CI: - 0.016 to -0.001, P interaction =0.004] and insulin (log transformed) (pooled beta = -0.006, 95% CI: -0.011 to -0.002, P interaction -0.023) in 3065 women of the 4 populations.CONCLUSIONS: Participants with different genotypes of IRS1 rs2943641 exhibit differential benefit from high circulating 25(OH) D for the reduction of insulin resistance and T2D risk. This gene- nutrient interaction, which appears to be limited to women, warrants further examination in randomized controlled trials of vitamin D supplementation. Abstract:OBJECTIVE: Toassess the relationship of circulating fatty acids (FAs) with risk of type 2diabetes (T2D) and potential interactions with genetic risk.RESEARCH DESIGN AND METHODS: 95,854participants with complete data on plasma FAs from the UK Biobank, wereenrolled between 2006 and 2010, and were followed up to the end of 2020. Plasmaconcentrations of saturated fatty acids (SFAs), monounsaturated fatty acids(MUFAs), and polyunsaturated fatty acids (PUFAs) were analyzed by ahigh-throughput NMR-based biomarker profiling platform. The genetic risk score(GRS) was calculated on the basis of 424 variants associated with T2D.Pathway-specific GRSs were calculated based on robust clusters of T2D loci.RESULTS: 3,052 T2D cases were documentedafter an average follow-up of 11.6 years. Plasma concentrations of SFAs andMUFAs were positively associated with T2D risk while plasma PUFAs wereinversely associated. After adjustment for major risk factors, HRs (95% CIs) ofT2D for 1 SD increment were 1.03 (1.02-1.04) for SFAs, 1.03 (1.02-1.05) forMUFAs, 0.62 (0.56-0.68) for PUFAs, 0.67 (0.61-0.73) for n-6 PUFAs, 0.90(0.85-0.95) for n-3 PUFAs, and 1.01 (0.98-1.04) for n-6/n-3 ratio. Plasma MUFAshad significant interactions with the overall GRS and GRSs for Proinsulin andLiver/Lipid clusters on T2D risk. The protective associations of n-3 PUFAs withT2D risk were weaker among individuals with higher Obesity GRS (Pinteraction=0.040) and Liver/Lipid GRS (P interaction=0.012). Additionally,increased plasma n-3 PUFA concentration was associated with more reductions inT2D risk among participants carrying more docosapentaenoic acid-associatedalleles (P interaction=0.007).CONCLUSIONS: Plasma concentrations of SFAs and MUFAs wereassociated with a higher T2D risk whereas plasma PUFAs and n-6 and n-3 PUFAswere related to a lower risk. Circulating MUFAs andn-3 PUFAs had significant interactions with genetic predisposition to T2D and FA-associated variants.

Abstract:OBJECTIVE To investigate the relationship between serum omega-3 polyunsaturated fatty acid (omega-3PUFA) and insulin resistance (IR) in patients with type 2 diabetes mellitus and non-alcoholic fatty liver disease (NAFLD). METHODS This trial involved 51 patients of type 2 diabetes mellitus with NAFLD (G4 group), 50 patients of type 2 diabetes alone (G3 group), 45 patients of NAFLD alone (G2 group) and 42 healthy control subjects (G1 group). Serum omega-3PUFA profile was analyzed with capillary gas chromatography. Insulin resistance was assessed by homeostasis model assessment (HOMA-IR). ALT, AST, gamma-glutamyltransferase (GGT) and serum lipids were measured. RESULTS The levels of HOMA-IR were higher in G4 group than those in G3, G2 and G1 group (4.90 + or - 2.54 vs 2.38 + or - 1.23, 2.20 + or - 1.15, 1.13 + or - 0.42; P < 0.05). The level of ALT, AST, GGT, TC, TG, LDL-C were higher in G4 group than those in G3, G2 and G1 group (P < 0.05). The level of omega-3PUFA was significantly lower in G4 group than those in G3, G2 and G1 group (5.68 + or - 2.02 vs 7.17 + or - 2.38, 6.97 + or - 2.32, 10.08 + or - 2.76; P < 0.05). omega-3PUFA concentration was negatively correlated with HOMA-IR, TC, TG and LDL-C (r = -0.491, -0.376, -0.462, -0.408, P < 0.05). CONCLUSIONS Serum omega-3PUFA is significantly decreased in patients with type 2 diabetes mellitus and NAFLD. Serum omega-3PUFA is negatively correlated with insulin resistance. omega-3PUFA plays a very important role in the development of diabetes mellitus and NAFLD.

Abstract:OBJECTIVE Insulin receptor substrate 1 (IRS1) is central to insulin signaling pathways. This study aimed to examine the association of IRS1 variants with insulin resistance (IR) and related phenotypes, as well as potential modification by diet.RESEARCH DESIGN AND METHODS Two IRS1 variants (rs7578326 and rs2943641) identified by genome-wide association studies as related to type 2 diabetes were tested for their associations with IR and related traits and interaction with diet in the Genetics of Lipid Lowering Drugs and Diet Network (GOLDN) study (n = 820) and the Boston Puerto Rican Health Study (BPRHS) (n = 844).RESULTS Meta-analysis indicated that rs7578326 G-allele carriers and rs2943641 T-allele carriers had a lower risk of IR, type 2 diabetes, and metabolic syndrome (MetS). Significant interactions on IR and MetS were found for these two variants and their haplotypes with diet. In GOLDN, rs7578326 G-allele carriers and rs2943641 T-allele carriers and their haplotype G-T carriers had a significantly lower risk of IR and MetS than noncarriers only when the dietary saturated fatty acid-to-carbohydrate ratio was low (0.24). In both GOLDN (P = 0.0008) and BPRHS (P = 0.011), rs7578326 G-allele carriers had a lower risk of MetS than noncarriers only when dietary monounsaturated fatty acids were lower than the median intake of each population.CONCLUSIONS IRS1 variants are associated with IR and related traits and are modulated by diet in two populations of different ancestries. These findings suggest that IRS1 variants have important functions in various metabolic disorders and that dietary factors could modify these associations.
Iron status correlates strongly to Insulin Resistance among U.S. adults: A nationwide population-base study

Abstract:Background: Evidence on the link between iron status markers and insulin resistance (IR) is limited. We aimed to explore the relationship between iron status and IR among U.S. adults. Methods: This study involved 2993 participants from the National Health and Nutrition Examination Survey (NHANES) 2003-2006, 2017-2020. IR is characterized by a HOMA-IR value of ≥2.5. Weighted linear and multivariable logistic regression analyses were used to examine the linear relationships between iron status and IR. Furthermore, restricted cubic splines (RCS) were used to identify the non-linear dose-response associations. Stratified analyses by age, sex, BMI and PA were also performed. Last, ROC curve was used to evaluate the predictive value of iron status in IR. Results: In weighted linear analyses, serum iron (SI) exhibited a negative correlation with HOMA-IR (β (95% CI) = -0.03(-0.05, -0.01), P = 0.01). In weighted multivariate logistic analyses, iron intake and serum transferrin receptor (sTfR) were positively correlated with IR (OR =1.02; 95% CI=1.00-1.04, P = 0.04; OR =1.07; 95% CI=1.02-1.13, P = 0.01). Also, SI and transferrin saturation (TSAT) were negatively correlated with IR (OR =0.96; 95% CI=0.94-0.98, P <0.0001; OR =0.98; 95% CI=0.97-0.99, P <0.001) after adjusting for confounding factors. RCS depicted a nonlinear dose-response relationship between sTfR and TSAT and IR. This correlation remained consistent across various population subgroups. ROC curve showed that TSAT performed better than iron intake, SI, sTfR and TSAT in ROC analyses for IR prediction. Conclusion: All biomarkers demonstrated significantly lower risk of IR with increasing iron levels, which will contribute to a more comprehensive and in-depth understanding of the relationship between the two and provide a solid foundation for future exploration of the mechanisms underlying their relationship.
Higher dietary insulinaemic potential is associated with increased risk of liver steatosis and fibrosis

Abstract:Background and AimsHyperinsulinemia and insulin resistance play a central role in the progression of hepatic steatosis and fibrosis, and diet can modulate insulin response. We thus hypothesized that diet with higher insulinemic potential is associated with increased risk of these conditions. MethodsTwo empirically dietary indices for hyperinsulinemia (EDIH) and insulin resistance (EDIR) were derived to identify food groups most predictive of fasting concentrations of C-peptide and insulin and homeostatic model assessment for insulin resistance, respectively. Hepatic steatosis and fibrosis were defined by controlled attenuation parameter and liver stiffness measurement using transient elastography (TE). Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated by logistic regression. ResultsOf the 4,171 participants with TE examination, 1,436 (age-standardized prevalence, 33.8%) were diagnosed with steatosis, 255 (5.6%) with advanced fibrosis, and 101 (2.2%) with cirrhosis. The multivariable-adjusted ORs for participants comparing the highest to the lowest EDIH tertile were 1.17 (95% CI: 0.99-1.39, Ptrend= 0.005) for steatosis, 1.74 (95% CI: 1.24-2.44, Ptrend= 0.001) for advanced fibrosis, and 2.05 (95% CI: 1.21-3.46, Ptrend= 0.004) for cirrhosis. Similar associations were observed for EDIR with ORs of 1.32 (95% CI: 1.11-1.55, Ptrend< 0.001) for steatosis and 1.43 (95% CI: 1.03-1.99, Ptrend= 0.006) for advance fibrosis. These positive associations remained among never drinkers and individuals who were free of hepatitis B and/or C. ConclusionsOur findings suggest that hyperinsulinemia and insulin resistance may partially underlie the influence of diet on hepatic steatosis and fibrosis, and highlight the importance of reducing or avoiding insulinemic dietary pattern.

Abstract:Introduction Previous studies have demonstrated a correlation between the ratio of alanine aminotransferase to high-density lipoprotein cholesterol (ALT/HDL-C) in the serum and the risk of diabetes. However, no existing study has investigated the association between insulin resistance (IR) and ALT/HDL-C. Therefore, this study aims to explore the association between ALT/HDL-C and IR in American adults. Methods A total of 7,599 adults selected from the National Health and Nutrition Examination Survey (NHANES) in 2013 to 2020 were studied. IR was assessed based on the homeostatic model assessment of insulin resistance (HOMA-IR). And the association between IR and ALT/HDL-C was assessed through multiple logistic regression, generalized smooth curve fitting and subgroup analyses. Results Multiple logistic regression analysis indicated a significant correlation between IR and ALT/HDL-C, with odds ratios (OR) of 1.04 (95% CI = 1.02–1.05) in males and 1.04 (95% CI = 1.02–1.07) in females. A non-linear association and saturation effect between ALT/HDL-C and IR risk were identified, with an inverted L shaped curve and an inflection point at 33.62. The area under the ROC curve (AUC) of ALT/HDL-C was significantly larger (AUC = 0.725 for males and 0.696 for females, all p < 0.01) compared with the use of ALT, HDL-C, AST and AST/ALT. Subgroup analysis showed a significantly higher independent association in obese individuals and individuals aged ≥50 years (All P interaction <0.05). Conclusion Elevated ALT/HDL-C demonstrates a significant correlation with IR, which can be used as a potential indicator of IR in American adults.

Association of insulin resistance with bone mineral density in a nationwide health check-up population in China

Abstract:Introduction Insulin resistance (IR) is closely associated with cardio-metabolic diseases. However, the impact of IR on bone mass remains obscure. The present study is to evaluate the association between the triglyceride-glucose (TyG) indicated IR and bone mass in a nationwide health check-up population in China. Methods We conducted a retrospective cross-sectional study including 788,247 participants and a longitudinal cohort study in 8770 participants who had repeated measurements of TyG index and bone mass in at least a 2-year follow-up period. The restricted cubic splines and logistic models were used to analyze the association between IR and bone mass in the cross-sectional study. The Cox model was applied to evaluate the relationship between baseline IR and the subsequent incidence of low bone mass and osteoporosis in the longitudinal study. Results In the cross-sectional study, the TyG index had positive correlations with low bone mass, osteoporosis, or both after adjusting for confounding factors (all P < 0.001). In the longitudinal cohort study, the baseline TyG index was significantly associated with the incidence of low bone mass, osteoporosis, or both during the follow-up period, with hazard ratios (HRs) of 1.56 (95 % confidence interval [CI]: 1.25, 1.93, P < 0.05), 1.66 (95%CI: 1.06, 2.59, P < 0.05), and 1.55 (95%CI: 1.27, 1.88, P < 0.05) after adjusting for confounding factors, respectively. Conclusions These results suggest that IR indicated by TyG is significantly associated with an increased risk of low bone mass and osteoporosis. Therefore, bone mass monitoring and early prevention strategies may be needed in individuals with IR to prevent the occurrence of low bone mass and osteoporosis.

Abstract:Objective: Although insulin resistance (IR) has been recognized to be a causal component in various diseases, current information on the relationship between IR and long-term mortality in the general population is limited and conclusions varied among different IR indicators and different populations. We aimed to assess associations between different measurements of IR with long-term all-cause mortality and cardiovascular mortality risk for the general population. Research design and methods: We included 13,909 individuals from the Third National Health and Nutrition Examination Survey. Mortality was identified via National Death Index information until December 31, 2019. IR was measured using fasting insulin, homeostasis model assessment of IR (HOMA-IR), homeostasis model assessment of β-cell function, quantitative insulin sensitivity check index (QUICKI), insulin-to-glucose ratio (IGR), triglyceride glucose (TyG) index, TyG-body mass index (TyG-BMI), and hypertriglyceridemic-waist phenotype. Results: During median 25-year follow-up, 5,306 all-cause mortality events occurred. After multivariate adjustment, variables significantly associated with elevated all-cause mortality risk were (hazard ratio [95 % confidence interval]): higher insulin (1.07 [1.02;1.13]); HOMA-IR (1.08 [1.03;1.13]); IGR (1.05 [1.00;1.11]); TyG (1.07 [1.00;1.14]); TyG-BMI (1.24 [1.02;1.51]); lower QUICKI (0.91 [0.86-0.96]). After stratification by diabetes status, higher insulin, HOMA-IR, TyG-BMI and lower QUICKI were significantly associated with increased risk of all-cause mortality in both diabetes and non-diabetes populations (all P for interaction > 0.05). Higher TyG (adjusted HR 1.17 [1.09;1.26], P for interaction = 0.018) and hypertriglyceridemic-waist phenotype (adjusted HR 1.26 [1.08;1.46], P for interaction = 0.047) were significantly associated with increased risk of all-cause mortality in patients with diabetes, however, these associations could not be seen in people without diabetes. Similar results were observed between the above-mentioned IR indicators and cardiovascular death. Conclusions: Fasting insulin, HOMA-IR, TyG-BMI, and QUICKI may indicate mortality risk in diabetes and non-diabetes populations, with TyG and the hypertriglyceridemic-waist phenotype showing particular relevance for individuals with diabetes. Further studies are needed to validate these findings and determine their broader applicability.

Abstract:Introduction: Genome-wide association studies have identified a group of genetic variants associated with insulin resistance (IR). Hypothesis: Participants with genetic risk of IR may respond differently to diet interventions on weight loss and improvement of insulin sensitivity. Methods: The Preventing Overweight Using Novel Dietary Strategies (POUNDS Lost) trial is a randomized, controlled 2-year weight-loss trial. We assessed the genetic risk of IR by a genetic risk score (GRS) based on 17 IR variants, and examined the genetic effect on changes in body weight and insulin sensitivity according to diet intervention groups in 692 overweight or obese adults. All models were adjusted for age, sex, ethnicity, and the baseline value for the respective outcome trait. Results: We observed significant interactions between the GRS and dietary protein on 6-month changes in weight, insulin, homeostatic model assessment-insulin resistance (HOMA-IR) and HOMA-insulin sensitivity (HOMA-S) (P for interaction=0.03, 0.02, 0.02, and 0.03, respectively). In the low-protein diet group, participants with higher GRS showed less reduction in insulin (P=0.03) and HOMA-IR (P=0.03); whereas in the high-protein group, changes in insulin and HOMA-IR were not statistically different across the GRS tertiles (Figure). Fasting insulin levels rebounded in parallel with weight regain after 6 months, and the interactions between the GRS and diet interventions on weight change and improvement in insulin sensitivity were attenuated. Conclusions: Individuals with higher genetic risk of IR may benefit less in improvement of insulin sensitivity than those with lower genetic risk when choosing an energy-restricted low-protein weight-loss diet.

Abstract:Antagonism of antibiotic activity by peritoneal dialysate has been postulated to be a cause of failure of treatment of peritonitis complicating continuous ambulatory peritoneal dialysis. We evaluated by a case-control study whether unexpected treatment failure could be attributed to such antagonism. Bacteria isolated from 34 patient episodes of peritonitis treated with the same regimen of ciprofloxacin monotherapy were studied. Ciprofloxacin was significantly less active in dialysate than in Iso-Sensitest broth (IB). The median MIC in IB was 0.5 microgram/ml, increasing to 2.0 micrograms/ml for both fresh dialysate (FD) (P = 0.003) and pooled dialysis effluent (PDE) (P = 0.03); the median MBC in IB was 8.0 micrograms/ml, increasing to 128.0 micrograms/ml in FD (P = 0.0002) and 64.0 micrograms/ml in PDE (P = 0.02). However, no significant differences were found in the results for patients suffering unexpected treatment failure (relapse of peritonitis) compared with the results for patients whose infection resolved without sequel. In IB the median MICs for relapsers and nonrelapsers were 1.0 and 0.5 microgram/ml, respectively (P = 0.88); median MBCs were 32.0 and 4.0 micrograms/ml (P = 0.19). In FD median MICs for relapsers and nonrelapsers were 2.0 and 1.0 micrograms/ml (P = 0.06); median MBCs were 128.0 micrograms/ml for both groups (P = 0.84). In PDE the median MICs were 2.0 micrograms/ml for both groups (P = 0.78); median MBCs were 256.0 and 64.0 micrograms/ml (P = 0.17). We therefore found no evidence to suggest that antagonism of antibiotic activity by dialysate is a cause of treatment failure or that conventional methods for laboratory susceptibility testing in peritonitis complicating continuous ambulatory peritoneal dialysis should be abandoned in favor of testing in media containing dialysate.

Abstract:Anthropometric parameters are widely used in the clinical assessment of hypertension, type 2 diabetes, and cardiovascular disease. However, few studies have compared the association between different anthropometric parameters and insulin resistance (IR). This study was aimed at investigating the relationship between 6 indicators, including body mass index (BMI), calf circumference (CC), arm circumference (AC), thigh circumference (TC), waist circumference (WC), waist-height ratio (WHtR), and IR. Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) was used to measure IR. Weighted linear regression was used to assess the relationship between different parameters and IR. The receiver operating characteristic curve (ROC) was employed to compare the strength of the relationship between different anthropometric parameters and IR. A total of 8069 participants were enrolled in our study, including 4873 without IR and 3196 with IR. The weighted linear regression results showed that BMI, CC, AC, TC and WC were significantly correlated with IR, except WHtR. After adjusting for multiple confounding factors, we found that BMI, AC and WC were significantly positively correlated with IR, while TC was significantly negatively correlated with IR. Logistic regression results showed that a larger TC was associated with a decreased risk of IR. In addition, BMI and WC had similar areas under the curve (AUC: 0.780, 95% CI 0.770–0.790; AUC: 0.774, 95% CI 0.763–0.784, respectively), which were higher than TC and AC (AUC: 0.698, 95% CI 0.687–0.710, AUC: 0.746, 95% CI 0.735–0.757, respectively). To our knowledge, this is the first study to report a negative correlation between TC and IR among patients without diabetes mellitus. Therefore, TC may be a new tool to guide public health and a clinical predictor of IR in non-diabetic patients.

Abstract:The aim of this study is to assess the relationships among the apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio), low-density lipoprotein cholesterol (LDL-C) and insulin resistance (IR) in a Chinese population with abdominal obesity. This is a population-based, cross-sectional study of 3,945 men and 2,141 women with abdominal obesity. Individuals were referred to a primary health service and recruited for analysis. IR was measured using a homeostasis model assessment of insulin resistance (HOMA2-IR) with a HOMA2 calculator. Metabolic syndrome (MetS) was diagnosed using International Diabetes Federation (IDF) criteria. Comparing the apoB/apoA-I ratio and lipid indices using the HOMA2-IR showed that the ratio, LDL-C, total cholesterol level (TC) and triglyceride level (TG) were higher; and the high-density lipoprotein cholesterol level (HDL-C) was lower in the fourth than in the first quartile in both sexes ( p ≤ 0.001). After adjustment for age, HOMA2-IR was positively correlated with the apoB/apoA-I ratio, LDL-C, TC and TG; and negatively correlated with HDL-C in men (all p < 0.0001). HOMA2-IR was also positively correlated with the apoB/apoA-I ratio, LDL-C, TC and TG; and negatively correlated with HDL-C in women (all p < 0.01). After adjustment for age and LDL-C, HOMA2-IR was found to be correlated with the apoB/apoA-I ratio in both men and women ( r = 0.066 and 0.116, p < 0.0001). After adjustment for age and the apoB/apoA-I ratio, HOMA2-IR was correlated with LDL-C in men and women ( r = 0.063 and 0.044, p < 0.0001 and p = 0.0431, respectively). Gender, age, LDL-C, BMI, HOMA2-IR and apoB/apoA-I were the covariates independently associated with presence of the MetS (Odds ratio, OR: 2.183, 1.034, 1.013, 1.157, 1.726 and 1.570, respectively; all p < 0.05). In conclusion, the study showed that the apoB/apoA-I ratio and LDL-C were positively correlated with IR. Excluding reciprocal interactions, the apoB/apoA-I ratio and LDL-C were still significantly correlated with IR, but the apoB/apoA-I ratio showed a greater correlation with IR than LDL-C in women with abdominal obesity, compared with men with abdominal obesity. Both LDL-C and apoB/apoA-I were independent risk factors of MetS, and the apoB/apoA-I ratio was stronger in this regard than LDL-C for this obese population.
Serum Irisin Level and Its Correlation with Insulin Resistance in Patients with Type 2 Diabetes

Abstract:Objective To investigate the association between serum irisin and insulin resistance.Methods Eighty two subjects with type 2 diabetes mellitus(T2DM) were selected from in-patients in Department of Endocrinology,the First Affiliated Hospital,Zhejiang University School of Medicine (including 48 men and 34 women) as type 2 diabetes group,and 62 control subjects (NC) were selected from healthy people in medical centre (including 33 men and 29 women) as normal control group.Anthropometric indexes,metabolic parameters and serum irisin levels were measured in all subjects.Fasting insulin (FINS),postprandials blood glucose was measured in T2DM group.The data were analyzed using student-t test,Pearson correlation analysis and multiple linear regression analysis.Results Serum irisin levels were significantly decreased in T2D group compared with NC group((1.5 ±0.6)vs (2.6±0.6) ng/L,t=-10.233,P＜0.05).Pearson correlation analysis showed serum irisin levels in T2DM group were inversely associated with waist hip ratio (WHR),2 hour-postprandial blood glucose and homeostasis model assessment of insulin resistance (HOMA-IR) (r=-0.603,-0.295,-0.22,P＜0.05).Other clinical parameters,including disease duration,gender,age,smoking habit,diabetic complications,other complications,body mass index (BMI),systolic blood pressure,diastolic blood pressure,fasting blood glucose (FBG),glycated hemoglobin A1c (HbA1c),FINS,high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),serum total cholesterol (TC),total triglycerides (TG) and atherosclerosis index (AI),were not correlated with irisin levels (P＞0.05 all above).Multiple linear regression analysis showed that after adjusting for gender,age,blood pressure,diabetic complications and other complications,WHR was significantly negatively associated with serum irisin levels,and WHR was the negative independent determinant of serum irisin (standardized regression coefficient =-3.995,P＜0.05).Conclusion Serum irisin levels are decreased in patients with T2DM and associated with the development of insulin resistance.
Associations between serum total bilirubin, obesity and type 2 diabetes

Abstract:Background: This study aims to examine the cross-sectional association between serum total bilirubin (STB) and type 2 diabetes (T2D) risk in the general population, and whether obesity could moderate this association. Methods: We used data from the 1999-2018 National Health and Nutrition Examination Surveys (NHANES), including a total of 38,641 US adult participants who were 18 years or older. The STB was classified as the low, moderate, and high groups according to tertiles. Results: We found that participants with lower STB had a significantly higher risk of T2D than those with moderate (OR = 0.81; 95% CI 0.74, 0.89; P < 0.001) and high (OR = 0.65; 95% CI 0.59, 0.73; P < 0.001) STB. Also, a significant interaction between body mass index (BMI) and STB on T2D was observed (P < 0.001). Stratified analysis showed that low STB was associated with a 20% and 27% decrease of T2D risk for moderate and high STB groups in obese patients, however, these effect estimates were smaller in the population with lower BMI (< 30 kg/m2). Similar associations of STB with glycohemoglobin and insulin resistance were observed. Conclusion: This study suggests that STB is associated with an elevated risk of T2D. More importantly, we reported for the first time that BMI may moderate the association between bilirubin and T2D.

Abstract:The paper presents a method for simplifying the complex managerial phenomena in the life-cycle software production by means of systems analysis. The reason for this is that software products are becoming increasingly complex and that project teams are, and will continue to be, the dominant way of organising software development. There is a fundamental principle; the complexity of the field in question is an interpretation of 'reality' according to a coarse systematisation. The interpretation is about how we think, what we do and how we cope. Based on this philosophical framework, the method consists of the following: describing the complex phenomena in terms of interactions; the interactions between a team and the tasks (e.g. modelling), the interactions between the teams (e.g. co-operating) and the interactions between tasks (e.g. configuring); defining a pattern to categorise the events of the interactions and interpreting the pattern to rationalise the concepts; system context and model contexts, communication and information context. The benefits of the practical use of the method are to provide unified objectives for supplying management with technical measures, and thus to enhance the efficiency and quality of the operational processes.
Potential mediation effect of insulin resistance on the association between iron metabolism indicators and non-alcoholic fatty liver disease

Abstract:Objectives: Iron metabolism and insulin resistance (IR) are closely related to non-alcoholic fatty liver disease (NAFLD). However, the interplay between them on the occurrence and progression of NAFLD is not fully understood. We aimed to disentangle the crosstalk between iron metabolism and IR and explore its impact on NAFLD. Methods: We analyzed data from the National Health and Nutritional Examination Survey (NHANES) 2017-2018 to evaluate the association between serum iron metabolism indicators (ferritin, serum iron, unsaturated iron-binding capacity [UIBC], total iron-binding capacity [TIBC], transferrin saturation, and transferrin receptor) and NAFLD/non-alcoholic steatohepatitis (NASH). Mediation analysis was conducted to explore the role of IR played in these relationship. Results: A total of 4812 participants were included, among whom 43.7% were diagnosed with NAFLD and 13.2% were further diagnosed with NASH. After adjusting the covariates, the risk of NAFLD increases with increasing serum ferritin (adjusted odds ratio [aOR] 1.71, 95% confidence interval [CI] 1.37-2.14), UIBC (aOR 1.45, 95% CI 1.17-1.79), and TIBC (aOR 1.36, 95% CI 1.11-1.68). Higher levels of serum ferritin (aOR 3.70, 95% CI 2.25-6.19) and TIBC (aOR 1.69, 95% CI 1.13-2.56) were also positively associated with NASH. Participants with IR were more likely to have NAFLD/NASH. Moreover, IR-mediated efficacy accounted for 85.85% and 64.51% between ferritin and NAFLD and NASH, respectively. Conclusion: Higher levels of serum ferritin and TIBC are closely associated with the occurrence of NAFLD and NASH. IR may be considered a possible link between NAFLD or NASH and increased serum ferritin levels.
