Computed Tomography-Assisted Auricular Cartilage Graft for Depression of the Alar Base in Secondary Unilateral Cleft Lip Repair: A Preliminary Report

Abstract:Objectives: To evaluate the feasibility, effectiveness, and safety of computed tomography-assisted auricular cartilage grafting for treating alar base depression secondary to unilateral cleft lip. Design and Setting: For patients with obvious depression of the alar base, the difference in heights of the alar base and the piriform margin between the cleft side and the noncleft side were measured with computed tomography. If both were >3.0 mm, the cartilage was harvested postauricularly and subdivided into 2 to 4 pieces. A multiple layer graft was inserted under the depressed alar base. The procedure was performed from 2006 to 2013, and the follow-up period was 3 to 15 months. Participants: Chinese patients with alar base depression secondary to unilateral cleft lip were selected. Intervention: Suture and cartilage graft techniques. Main Outcomes Measures: Differences in bilateral alar base heights and piriform apertures. Results: There was no wound dehiscence, exposure of bone, or donor site morbidity. The difference in heights in the bilateral alar bases and piriform apertures decreased. There were no obvious scars in any of the cases. Conclusions: This technique has several advantages including ease of operation, minimal trauma, satisfactory outcomes, and useful references for operation provided by computed tomography. It is a superior alternative for reconstruction of secondary alar depression.
The Application of Nasolabial Fold Flap and Auricle Compound Tissue in Repairing Nasal Alar Defect

Abstract:Objective To explore the application of nasolabial fold flap and auricle compound tissue in repairing nasal alar defect. Methods From October 2011 to March 2016, 32 patients with nasal alar defect were treated with nasolabial fold flap graft and auricle compound tissue free graft. The defect state of nasal alar, repair methods, incision scar, symmetry and postoperative appearance were all observed and analyzed. Results Nineteen patients received the treatment of nasolabial fold flap graft and 13 patients received the treatment of auricle compound tissue free graft. Nasal alar defect was well-repaired, and the outcome appearance was satisfactory without obvious scars around wound surface and donor tissue's incision. Conclusion Both nasolabial fold flap graft and auricle compound tissue free graft are favorable treatments for repairing nasal alar defect. Auricle compound tissue free graft should first be considered for patients without severe nasal alar defect, while nasolabial fold flap free graft should be employed for patients with severe nasal alar defect; Both methods have its advantages and disadvantages, the choice should be made base on the comprehensive consideration of the actual conditions of each patient.
Sudden Onset, Rapidly Expansile, Cervical Cystic Hygroma in an Adult: A Rare Case with Unusual Presentation and Extensive Review of the Literature

Abstract:Cystic hygroma (CH) is a benign infiltrative malformation of the lymphatic channels. We report a case of a 28-year-old Indian female who presented with rapidly enlarging right sided neck swelling over the posterior triangle since 5 days. Complete resection of CH is sometimes not amenable because of its infiltrative nature and involvement of surrounding vital structures. However, in our patient successful complete surgical resection was undertaken. The MRI findings of our patient were consistent with brachial cleft cyst; this posed a challenge in the diagnosis of CH. The histopathological analysis of the resected mass confirmed CH. CH is rare in adults and such an acute presentation is exceptionally atypical. History of prior trauma and infection are known etiological factors for adult CH; these were conspicuously absent in our patient. CH should be considered in the differentials of rapidly enlarging cystic swelling of posterior region of neck in adults. Optimal and timely management is necessary to achieve a favorable prognosis. Therefore, we report a case of rapidly enlarging cervical CH in an adult along with extensive literature review to have a better understanding regarding epidemiology, etiopathogenesis, clinical presentation, optimal management, and prognosis of such a rare entity in adults.
Tailored Nasal Reconstruction after Dog-Bite Injury in a Young Woman

Abstract:Due to the prominence in the facial framework, nose is commonly susceptible to accidental injury, with functional and psychological related damage. The aim of nasal reconstruction is the full restoration of the complex nasal architecture, made on concave and convex osteocartilaginous surfaces with cutaneous and mucosal linings.Optimal treatment is still debated. According to literature, standard criterion for nasal reconstruction is the frontal flap.[<a href="#JR_1">1</a>] Despite its safety and reliability, due to a robust pedicle and large amount of tissue, this option implies a central-facial esthetic defect. Auricular composite grafts can be a good option, taking into account the minor donor site comorbidity and esthetic defect. However, viability remains a concern and affects the maximal graft width, as recommended by Ahuja et al.[<a href="#JR_2">2</a>] Most failures occur in tobacco users or in cardiovascular and diabetic patients where microvascular circulation can be compromised.[<a href="#JR_3">3</a>] A tailored therapy has to be planned based on age, sex, and comorbidities.Herein, we present a 28-year-old female patient, victim of dog bite resulting in avulsion of the right nasal ala with irregular wound (1.8 cm × 2.0 cm). The defect included several nasal subunits (partial nasal tip, medial third of right alar cartilage, and soft triangle) besides the total avulsion of skin, subcutaneous fat, cartilage, and mucosa, falling in IIIB category of modified Lackmann's classification for facial wound.[<a href="#JR_4">4</a>] The avulsed portion was not retrieved. The patient immediately refused the forehead flap reconstruction, scared by predictable poor esthetic results. Considering young age and absence of comorbidities, we opted for an immediate nasal reconstruction with auricular composite graft.[<a href="#JR_5">5</a>] This choice would not compromise an eventually second reconstruction with the gold-standard flap. Furthermore, we fully informed the patient about the possibility of a secondary surgical procedure with frontal flap in case of composite graft failure.The homolateral helix was chosen as donor site and the graft was demarcated and sculpted based on the size and shape of the nasal defect, to harvest a cutaneous and cartilaginous flap. Donor site's defect was remodeled at the end of the procedure using a superior-based preauricular rotation flap ([<a href="#FI00430-1">Fig. 1</a>]).At 1-year follow-up, nasal ala shape was stable, color was consistent with surrounding tissue, and satisfactory symmetry was achieved without aesthetic defects in size and framework of the ear ([<a href="#FI00430-2">Fig. 2</a>]). Auricular composite graft demonstrated to be a safe option to reconstruct nasal ala injuries in young healthy patients when no conditions of reimplantation exist.

Abstract:Objective:To investigate the strategy of the plastic repair of the nasal trauma involving the nasal tip, nasal alar, and columella.Methods:From January 2013 to December 2019, medical records of the patients suffering from nasal subunits trauma and postoperative nasal deformities were collected for retrospective analysis in the Department of the Burns and Plastic Surgery, the Nanjing Drum Tower Hospital. According to the traumatic condition, orthotopic suture and skin grafting were performed in the emergency department. At secondary stage, modified auricle composite tissue flap was used in the patients with full-thickness defect of nasal alar. Local flap, expanded skin of nasal dorsum combined with cartilage, skin grafting were respectively chosen in the repair of the defect involving multiple nasal subunits(the diameter of defect was less 3 cm).Results:The study included 53 patients(44 males, 9 females), the mean age was (42.1±17.0) years. 33 cases underwent in situ suturing in primary operative treatment and they were satisfied with the appearance during one-year follow-up. However, tissue necrosis occurred in 12 cases with total resection of nasal tissues, secondary reconstruction of defect was performed. 8 cases with secondary nasal deformities after extramural hospital treatment were sent to our hospital. Of 20 cases, 6 patients suffered from full-thickness defect of nasal alar( the size of defect was 1.0-7.5 cm 2) and they were treated with modified auricle composite tissue flap, the size ranged (1.0-3.0) cm×(1.0-2.5) cm. All flaps survived and the deformity of auricle was acceptable during one-two years follow-up. The defect of nasal alar and columella occurred in 14 patients, 4 of them were treated with local flap transfer (the size was 1.5-3.0 cm 2). Two flaps was trimmed six months after surgery and all flaps were satisfactory during one-two years follow-up. Skin expansion and cartilage graft were applied to practice in 4 patient. Skin grafting(the size of skin was 2-4 cm 2) was used in 6 patients and all patients were satisfied with final cosmetic result. Conclusions:When the nasal subuints was incomplete or the wound was not contaminated, cosmetic suturesat the emergency procedure can lead to good outcome. When the patient was subjected to complete tissue loss with cartilage defect or contaminated wound, staged surgical procedures were required to achieve better clinical efficacy.
Three-Dimensional M-Shaped Resection for Nasal Alar Hypertrophy in Asian Patients

Abstract:Background Nasal alarplasty is an important component of esthetic rhinoplasty in Asians. The two main surgical techniques that correct alar hypertrophy by reducing the height or length often leave external scars and are associated with a high relapse rate. Methods We developed a new technique, called three-dimensional（3 D） M-shaped resection, which corrects both the nasal alar height and length and simultaneously minimizes external scarring. We performed this procedure from January 2013 to September 2016 in 49 consecutive female patients diagnosed with saddle nose and nasal alar hypertrophy. Their mean age was 28.6（range, 18–40） years. All patients had previously undergone simple rhinoplasty. Nasal alar length and height, nostril length and width, and maximal nose width were analyzed preoperatively and postoperatively from photographs. Results After a mean of 9（range, 3–24） months of follow-up, surgery was considered successful in 46 women（94%） with good cosmetic effects. In three patients, nasal alar hypertrophy recurred（6 months postoperatively）. There were no early complications such as hematomas, infections, skin or mucosal necrosis, or wound dehiscence. The mean reductions postoperatively were 1.7 mm and 0.9 mm for nasal alar length and height, respectively, 1.6 mm for both nostril length and width, and 3.5 mm for nose width.Conclusion The 3 D M-shaped resection for nasal alar hypertrophy effectively reduced hypertrophy in 94% of patients for up to 24 months, producing minimal external scars and good cosmetic effects. It is a simple and convenient technique that is an effective and safe option for nasal alarplasty.

Abstract:Background: The repair of nasal alar defects is challenging for plastic surgeons, and there is currently no standard operation. Herein, the authors reported the clinical outcomes of a nasofacial groove pedicled flap for the reconstruction of alar defect. Methods: This retrospective study included patients who underwent the nasofacial groove pedicled flap for the reconstruction of alar defect between January 2018 and June 2020. Photographs of standard facial postures were taken before and after surgery to record the surgical results of the patients. The patient’s medical history was reviewed retrospectively. Self-reported satisfaction of patients on scar morphology and reconstructive effect were evaluated with a questionnaire survey. Results: There were 26 eligible patients enrolled, and all patients were followed up for more than 1 year after surgery. All flaps were free of ischemia and necrosis and healed well. No patient experienced restricted nostril ventilation. Eight patients underwent reoperation to trim the flap pedicle and the scar. Eight patients (8/26) reported “very satisfied,” and 17 patients (17/26) reported “satisfied” with the repair effect and scar morphology. One patient went through multiple laser treatments to improve her scars but still remained visible hyperpigmentation. She was dissatisfied with postoperative flap pigmentation but was satisfied with the correction effect. Conclusions: The clinical results indicated that the nasal groove flap was safe for the treatment of the lateral alar defect, and the patients were satisfied with the clinical results. The authors believe that this flap can be used as an alternative method for repairing the lateral alar defect. Level of Evidence: Level —IV, therapeutic study.

Ear Reconstruction for Auricle Defects Using an Expanded Neck Flap: A Case Report

Abstract:Ear, Nose &Throat Journal, Ahead of Print. This case report describes a patient with a left ear deformity resulting from a flame burn sustained 20 years ago. The patient underwent an ear reconstruction procedure utilizing an expanded neck flap. The autologous rib cartilage was used as the framework, while an expanded neck flap served as the covering for the framework. The surgery was completed in 3 stages. Initially, tissue expanders were implanted and gradually inflated with water. After sufficient expansion, the expanders were removed, and the scar tissue was excised. Subsequently, the expanded flap was used to cover the defects, and the expanded neck flap was rotated to cover the autogenous costal cartilage framework obtained intraoperatively. Finally, the reconstructed ear was repaired by constructing the cephaloauricular sulcus, removing postauricular scars, and trimming the neck-flap pedicle. After a 1-year follow-up, the wound had healed satisfactorily with only minor complications. The shape of the reconstructed ear appeared realistic, and its function was maintained. Most of the scars were repaired, and the scarred alopecia was significantly improved. In patients with limited availability of the postauricular flap, especially burn patients, using an expanded neck flap can lead to superior outcomes.

Abstract:Abstract Defects of the nasal ala are challenging to reconstruct, given its complex three-dimensional structure. Successful repair of these defects needs to provide aesthetic symmetry and preserve nasal function. A wide variety of reconstructive options have been described for nasal ala defects, ranging from skin grafts to locoregional flaps, and also includes the auricular composite graft. However, there are currently no comprehensive guidelines for nasal ala repair, and the versatile role of the auricular composite graft has not been well defined. In this review, we aim to provide a comprehensive algorithm to guide repair of nasal ala defects. Additionally, we compare our experience using the auricular composite graft with the available literature to better define its utility in nasal ala repair.
Natural history of intestinal neoplasms induced in rats by a single injection of methyl(acetoxymethyl)nitrosamine.

Abstract:Summary Male Sprague-Dawley-derived rats (Charles River CD) were injected once i.p. at 5 weeks of age with methyl-(acetoxymethyl)nitrosamine at a dose level of 0.1 mmole/kg (one-half of the 50% lethal dose). Groups of 27 to 30 rats were sacrificed 20, 25, 30, 40, and 50 weeks later to study the development and natural history of induced tumors of the intestines. An additional 30 rats were allowed to die or were killed in poor condition. From 70 to 90% of the rats developed tumors from 20 to 50 weeks after injection, and 70 to 86% of the rats had intestinal tumors, most commonly in the small intestine. Most tumors of the small intestine were tubular adenocarcinomas that progressively invaded the gut wall. Tumor size correlated with degree of invasion of extramucosal tissues. As they grew large, these tumors developed cystic spaces and foci of stromal osteoid metaplasia. Such cystadenocarcinomas rarely metastasized despite their large size. Stromal lymphoid reaction was frequently observed. Four of 14 mucinous (signet ring) adenocarcinomas orginating in the small intestine metastasized to the peritoneal cavity. Colon tumors occurred more variably in 6 to 42% of rats in different groups and were usually polypoid adenocarcinomas that never invaded the tunica muscularis. Only a few invasive tubular adenocarcinomas were seen. The carcinogen also induced other tumors in the peritoneal cavity and abdominal viscera in low incidence, including sarcomas, mesotheliomas, lymphangiomas, renal tumors, carcinomas of the seminal vesicles, and, occasionally, tumors of other tissues.
Nasal Reconstruction for Chinese Patients: A Retrospective Study

Abstract:Abstract This article reviewed our experience of Chinese nasal reconstruction over 12 years and evaluated the effect of expanded forehead flap both aesthetically and functionally. The special skin type and other anatomic features of Chinese patients was understood thoroughly during the treatment. This article thus catered for the need of multiracial nasal reconstruction. We analyzed existing clinical data and demonstrated a typical case in detail. The postoperative result supported our strategy which advocated the extensive application of expanded forehead flap, together with flip scar flap as the internal lining. The features of Chinese patients also prompted the use of costal and auricular cartilage. Emerging technology like 3D-printing would benefit nasal reconstruction from more aspects.
Nasal deformity corrected by auricular cartilage in conjuction with silicone prosthesis

Abstract:Objective To explore the application of auricular cartilage in conjuction with silicone prosthesis to correct the nasal deformity. Metheds Augmentation rhinoplaslty was perfommnd on patients with a short nose with a low flat, blunt break point; at the same time the upper lateral part of the nasal ala cartilage was cut to reduce the width and uhgthen the nasal columella ,and the auricular cantilage in cinjuction with silicone prosthesis was used to raise and extend the break point. Results 32 cases were involved, the effects were satisfactory and there is no complication by follow-up from 3 to 18 months. In addition, the appearance of the pinna in the donor site had no change. Conclusion Auricular cartilage in conjuction with silicone prosthesis can accomplish the supportine function of nasal cartilage struction and a simple augmentation rhinoplasly with this methed is effective to correce the nasal deformity cause by cleft lip.
Microvascular Nasal Repair Using a Reversed Superficial Temporal Artery Auricular Flap.

Abstract:Objective A reversed superficial temporal artery auricular flap was presented to explore a new method for reconstructing the defects of the distal nose by microsurgical techniques. Methods A reversed superficial temporal artery auricular flap had been used on fifteen patients with nasal defects, including thirteen patients with alar defects and two patients with nasal tip defects. The reversed superficial temporal vessels of the flap were anastomosed with the recipient facial vessels. The size of the flap was 2.5 cm × 2.0 cm - 4. 0 cm × 2.5 cm, the length of the vascular pedicle was 5 - 8 cm, average 6. 5 cm Results The flap survived uneventfully in all fifteen patients. Results demonstrated satisfactory symmetry between the reconstructed ala and the contralateral side as well as an excellent tip projection. The donor-site defect was minimal. Conclusions The reversed superficial temporal artery attricular flap offers an adequate length of vascular pedicle of the flap, it delivers a good solution to the problem of the vascular pedicle shortage of the proximal superficial artery auricular flap. Effects of Caffeine on the Bones of Aged, Ovariectomized Rats

Abstract:Caffeine is a substance which many people consume in their daily life. Caffeine’s effects on bone are still controversial. Using ovariectomized rats, the present study was conducted to determine to what extent caffeine intake affects the mechanical properties, bone minerals and histology. Aged rats were divided into 2 groups after ovariectomy. Group 1 was fed a 20% protein diet as a control, and group 2 was fed a 20% protein diet supplemented with caffeine (2 mg/100 g body weight). The respective diets were fed to the rats of each group for 90 days. Rats were then killed by heart puncture, blood was collected, and femurs were removed. In 1 group of femurs paraffin cross-sections were made at the midshaft of each bone. Total width, cortical width, total cross-sectional bone area of the midshaft, and the number of osteocytes in randomly selected areas were measured. Another group of bones was subjected to three-point bending testing until failure. Bones were then pulverized and Ca, P, Mg, Zn, Sr, Si, hydroxyproline and hexosamine contents and crystallite size were measured. Various mechanical properties, except modulus of elasticity, in the caffeine group were consistently 7–23% lower than the noncaffeine controls. Yield strain in the caffeine group was significantly less than in the noncaffeine controls. Zinc, Sr, and crystallite size of bone showed a significant decrease in the caffeine group, whereas Si contents significantly increased. Our current results indicate that routine intake of caffeine in the elderly should be regarded with some caution.
Choice of Single-Staged Reconstruction of Nasal Defect

Abstract:INTRODUCTION:The nasal defect is still a challenge for plastic surgeon now. Many surgical options are reported in previous literatures. There are few studies reported about the 1-staged repair of large nasal defect. In this article, the authors describe the experience about the nasal reconstruction in the last 8 years.PATIENTS AND METHODS:Fifty-two patients diagnosed with nasal defect were retrospectively examined between 2011 and 2018. All of them performed under local anesthesia and received 1-staged surgical method.RESULTS:In the authors' patients, the mean size of the defect was 2.16 cm. Nine patients were treated by nasolabial flap, and 30 patients were treated by local flap. The modified auricular composite tissue flap was used in 5 patients. There were no major postoperative complications. All patients were satisfied with aesthetically nasal contours as well as inconspicuous scars when follow-up.CONCLUSION:Single-staged reconstruction is still considered the first choice for nasal defect. The design of flap depends on the surrounding condition and the size of defect. Locoregional flaps are still considered as an ideal choice for nasal reconstruction in most patients. O-Z flap and modified auricular free flap could be an option for large-sized defect of nasal alar and nasal tip.
Effect of Y-shaped Nasal Columella Made of Autologous Cartilage in Secondary Nasal Deformity after Cleft Lip Surgery

Abstract:Aim: Due to the diversity and complexity of tissues involved in secondary nasal deformities following unilateral cleft lip, secondary nasal deformity correction surgeries are challenging and often yield unsatisfactory results, posing a difficult problem for plastic surgeons. Autologous cartilage, with its low sculpting difficulty, minimal absorption, and stable tissue compatibility, is considered the optimal material for reconstructing the columella, nasal tip, and alar. This study analyzed the clinical outcomes of using autologous cartilage to create a Y-shaped columella to correct secondary nasal deformities after cleft lip surgery. Methods: In this retrospective study, 75 patients with secondary nasal deformity after unilateral cleft lip surgery were treated from January 2018 to December 2023. Appropriate costal cartilage, auricular cartilage, and iliac crest cartilage were fashioned into Y-shaped stents and implanted into the nasal columella of the patients to strengthen the tip cartilage. The free alar cartilage was sutured and fixed with a Y-shaped bracket. The wound healing rate, subjective satisfaction evaluation, and complications were analyzed. The nasal appearance, nasal deformity classification, and objective indices of healthy and affected nasal sides, function, and quality of life were compared before and after the operation. Results: All patients healed well during the first stage without related complications, and the average incision healing time was 5.16 ± 1.37 days. The scores of nasal tip position, nasal tip shape, nostril shape, nasal dorsum shape, and nasal floor shape at 6 months post-operation were significantly lower compared to pre-operation (p < 0.001). The grade of nasal deformity at 6 months post-operation was lower than the pre-operation (p < 0.001). Pre-operation, the nasal base and nasal width of the affected side were higher than those of the healthy side, while the nasal columellar and nostril height were lower (p < 0.001). At 6 months post-operation, the nasal base and nasal width decreased, and the nasal columellar and nostril height increased (p < 0.001), with no significant difference between the two sides (p > 0.05). The nasal obstruction symptom evaluation (NOSE) and Functional Rhinoplasty Outcome Inventory-17 (FROI-17) scores at 6 months post-operation were significantly lower than pre-operation, while the rhinoplasty outcome evaluation (ROE) score was higher (p < 0.001). The total patient satisfaction was 97.33% (73/75). Conclusions: Y-shaped nasal columella made of autologous cartilage can effectively correct secondary nasal deformity after cleft lip surgery, improve nasal alar collapse, lengthen nasal columella, elevate nasal end, enhance the quality of life, nasal appearance, and nasal ventilation function of patients, with high satisfaction and few complications.
Designing a Nasal Lining-Framework Complex for Reconstructing Total Nasal Defects

Abstract:Objective: To validate the feasibility of an innovative nasal lining-framework complex (NLFC) for reconstructing total nasal defects. Study design: Retrospective cohort study. Methods: This NLFC is composed of forearm flap and support framework. Twenty-four patients were followed up for a minimum of 17 months in 5 centers. Patients' medical history data were retrospectively analyzed. Visual Analog Scale (VAS) of surgeons and patients was used to evaluate the aesthetic effects and self-satisfaction. The Nasal Obstruction Symptom Evaluation (NOSE) questionnaire and Rhinoplasty Outcome Evaluation (ROE) questionnaire were used for functional assessment. Results: Reconstruction surgeries were all successfully completed. The flaps healed well in all patients, and there were no signs of ischemic necrosis. The healing time of the wound was 10 to 14 days, except for 1 case with infection. The nasal lining was reconstructed and no significant contracture was observed. The average VAS of surgeons was 4.29 ± 0.69 (range 3-5). The mean VAS score of patients was 3.75 ± 0.79 (range 2-5). There was a significant positive correlation between patients' and surgeons' VAS scores (P = .007, r = .5355). The results of the NOSE questionnaire showed that all patients had no obvious ventilation restriction, and only 3 patients mildly felt that the nasal inspirations were slightly insufficient during exercise or exertion. The mean ROE of the patients was 21 ± 1.96 (range 18-25). Conclusion: This NLFC is suitable for total nasal reconstruction, which can provide effective support to prevent flap collapse and retraction and ensure good nasal ventilation. Abstract:Background: The combined use of various flap techniques has rapidly improved the reconstruction quality of auricle defects that are complicated by a scarcity of periauricular skin after severe burns. Nevertheless, there is still no preferable method when the optimal alternative skin to cover the auricular framework is unavailable and the periauricular vascular network is devastated.Case Description: Copious scars were observed in the periauricular region, neck, forearm, and supraclavicular region of a 19-year-old man. He had been burned by high-voltage electricity and exhibited a right auricular defect. We innovatively created a prefabricated expanded island flap constructed with an anastomosed vascular pedicle buried in the anterior thoracic chest, followed by flap transfer, tissue re-expansion, and sculpted autologous costal cartilage implantation. The remnant ear was successfully reconstructed in a three-stage surgical procedure.Conclusions: All the flaps survived well without any complications. The reconstructed right ear had a natural shape and a clear structure without apparent displacement and deformation during follow-up. The patient was satisfied with the final appearance, and his neck mobility markedly improved. Advantages and disadvantages were discussed. This procedure explored a novel solution to construct an auricular framework covering for patients who do not have high-quality donor skin and lack anastomotic vessels in the recipient area.

[Correction of severe alar retraction with alar rotation flap]

Abstract:Objective: To investigate the therapeutic effect of alar rotation flap for severe alar retraction. Methods: Patients with severely retracted alar underwent ala reconstruction using alar rotation flaps and autogenous cartilage batten grafts. First, costal cartilage was used to reshape the nasal tip and nasal dorsum. Then cartilage patch was used to extend and thicken the retracted alar. Then the alar rotation flap was transferred to correct retracted alar. Results: Fourteen patients with severe alar retraction underwent alar reconstruction with alar rotation flap and alar batten grafts. The alar retraction was corrected in all cases, with improvements functionally and aesthetically. No recurrence of alar retraction was noted. The incision healed with acceptable cosmetic results, with obvious scar in only one patient (one side). Conclusions: The alar rotation flap is an effective and reliable surgical option to correct severe alar retraction. Scar can be kept inconspicuous by precise placement of the incision within the junction of the ala and the nasal dorsum, following principles of the aesthetic nasal subunits.

Abstract:To accomplish total ear reconstruction with aesthetic appearance is a great challenge for plastic surgeons worldwide due to insufficient skin coverage. A retroauricular fascial flap and skin graft technique are commonly used, but the results obtained are not satisfactory due to color mismatch, severe edema, and donor-site morbidity. Here, we describe a novel strategy for total ear reconstruction, utilizing an extended retroauricular flap prelaminated with tissue expansion to obtain enough skin for ear reconstruction.About 2 months before ear reconstruction, a kidney-shaped tissue expander was inserted at the mastoid region subcutaneously. The retroauricular skin became enlarged and thinner in 2 months after expansion. Next, the expander was removed and the retroauricular flap was extended by dissecting the surrounding scalp subcutaneously. By mobilizing the scalp-extended retroauricular flap, we could encapsulate the entire framework with thin and non-hair-bearing skin.From August 2014 to September 2015, 36 microtia patients had undergone ear reconstruction using the novel strategy. Satisfactory aesthetics along with fine structure, symmetry, and maintenance of the auriculocephalic angle was achieved in all patients.Using our novel strategy, we can obtain satisfactory aesthetic outcomes by fully mobilizing the expanded skin without additional morbidities of the donor sites. Patients are satisfied with the vivid ear contour and lack of groin scar.
