Abstract:Background: It remains unclear whether the degree of white matter tract damage or cerebral hypoperfusion can better predict global cognitive impairment in CADASIL. We sought to determine the independent effects of cerebral perfusion status and white matter integrity on the cognition. Methods: We reviewed prospectively collected clinical and imaging data from genetically-confirmed CADASIL patients who underwent both arterial spin labeling (ASL) perfusion MRI and diffusion tensor imaging (DTI). We analyzed the cerebral blood flow (CBF), mean diffusion (MD), and fractional anisotropy (FA) by dividing the brain tissue into white matter hyperintensity (WMH) and normal-appearing white matter (NAWM). Global cognitive function was evaluated by using Mini-Mental State Examination (MMSE) and the Montreal Cognitive Assessment (MoCA). Results: Of the included 29 CADASIL patients, the mean age was 48.4 ± 7.9 years, and 17 (58.6%) were women. MD was significantly correlated with CBF in both WMH (r = -0.407, P = 0.035) and NAWM (r = -0.437, P = 0.023) after adjusting for age and WMH volume. A MoCA score was obtained in 13 patients and was significantly correlated with CBF in both WMH (r = 0.742, P = 0.004) and NAWM (r = 0.659, P = 0.014). Both CBF in WMH (area under the curve, 0.767; 95% CI, 0.586-0.947, P = 0.015) and MD in WMH (area under the curve, 0.740; 95% CI, 0.557-0.924, P = 0.028) were good predictors for cognitive impairment (MMSE score < 27). However, multiple linear regression analysis revealed that global cognitive function was independently associated with CBF in WMH only (standardized β = 0.485, P = 0.015), after adjusting for age, gender, WMH volume, the presence of subcortical infarcts and DTI metrics. Conclusions: Our findings suggested that cerebral hypoperfusion was more strongly associated with global cognitive dysfunction than the severity of brain microstructural damage, supporting that CBF assessed by ASL could serve as a candidate imaging indicator for monitoring alterations of global cognitive function in CADASIL.

Abstract:Background and Objectives Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is the most common monogenic form of stroke and early-onset dementia. We determined the prevalence of vascular cognitive impairment (VCI) in a group of patients with CADASIL and investigated which factors were associated with VCI risk, including clinical, genetic, and MRI parameters. Methods Cognition was assessed in patients with genetically confirmed CADASIL (n = 176) and healthy controls (n = 265) (mean [SD] age 50.95 [11.35] vs 52.37 [7.93] years) using the Brief Memory and Executive Test (BMET) and the Montreal Cognitive Assessment (MoCA). VCI was defined according to previously validated cutoffs. We determined the prevalence of VCI and its associations with clinical risk factors, mutation location (epidermal growth factor–like repeats [EGFr] 1–6 vs EGFr 7–34), and MRI markers of small vessel disease. Results VCI was more common in patients with CADASIL than in controls; 39.8 vs 10.2% on the BMET and 47.7% vs 19.6% on the MOCA. Patients with CADASIL had worse performance across all cognitive domains. A history of stroke was associated with VCI on the BMET (OR 2.12, 95% CI [1.05, 4.27] p = 0.04) and MoCA (OR 2.55 [1.21, 5.41] p = 0.01), after controlling for age and sex. There was no association of VCI with mutation site. Lacune count was the only MRI parameter independently associated with VCI on the BMET (OR: 1.63, 95% CI [1.10, 2.41], p = 0.014), after controlling for other MRI parameters. These associations persisted after controlling for education in the sensitivity analyses. Discussion VCI is present in almost half of the patients with CADASIL with a mean age of 50 years. Stroke and lacune count on MRI were both independent predictors of VCI on the BMET.

Abstract:Objective To assess retinal arteriole stenosis in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) and to evaluate the relationship between retinal arteriole stenosis with cranial MRI changes and clinical features. Methods Sixteen CADASIL patients (mean age was (43.4±8.1)years, mean duration was(4.7±3.4)years)and sixteen age matched healthy individuals were enrolled. Mini-Mental State Examination (MMSE) and modified Rankin scale scoring were performed in 16 patients. Cranial MRl with Coulthard scores was assessed on CADASIL patients as well. Retinal examinations for arteriole stenosis were done for each subject. Bivariate correlations (Kendall's tau-b) were used to assess the relationship among the grade of retinal stenosis, MMSE, modified Rankin seale, cranial MRI with Coulthard scores and age. Results Retinal arteriole stenosis presented in 15 cases. in whom 4 cases showed mild arterio-venous nicking. while it only presented in 2 controls. Grade 0.Ⅰ, and Ⅲ of retinal arteriole stenosis are noted in 1/16,4/16,7/16 and 4/16cases respectively. Coulthard scores of cerebral MRl were 6.0.19.0.25.1 and 29.8 respectively from Grade 0 toⅢ of retinal arteriole stenosis. The correlation coefficient of retinal arteriole stenosis and cranial MRI scores was 0.743(P＜0.001＝,MMSE-0.429(P＜0.05＝,modified Rankin scale 0.437(P＞0.05).and age 0.299(P＞0.05).Conclusion Retinal arteriole stenosis is significantly correlated with lesion in cranial MRI and dementia.

Abstract:Aims: The study aimed to compare the frequency of neuropsychiatric symptoms (NPS) across the declining memory continuum, from normal aging, subjective cognitive impairment (SCI), and mild cognitive impairment (MCI) to Alzheimer’s disease (AD), and to explore the clinical correlates of NPS. Method: In a memory clinic, 157 subjects (46 mild AD patients, 38 MCI individuals, 24 SCI subjects, and 49 normal controls) completed the neurobehavioral assessments with the Neuropsychiatric Inventory (NPI). The clinical significance of each NPI domain was defined as an item score ≥4. Result: Clinically significant depression was more common in the SCI than in the normal control group (p < 0.05). The frequency of NPS was significantly greater in the mild AD group compared to other groups. Clinically significant apathy and aberrant motor behavior were more common among the AD group than the MCI group (p < 0.05). The MMSE score (OR 4.84, 95% CI 1.92–12.16, p = 0.001) and apathy (OR 12.73, 95% CI 1.48–109.68, p = 0.021) significantly determined the diagnostic status as MCI or mild AD. Conclusion: Across the declining memory continuum, the frequency of NPS was highest among mild AD patients. Depression, apathy, and aberrant motor behavior deserve more attention. Presence of apathy might be an independent determinant for mild AD.
Comparison of the Clinical and Magnetic Resonance Imaging Characteristics of Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarct and Leucoencephalopathy in Mainland China and Other Countries

Abstract:Objectives To analyse the clinical and MRI features of cerebral autosomal dominant arteriopathy with subcortical infarct and leucoencephalopathy (CADASIL) in mainland China and compare these features with those reported in other countries.Methods All 26 CADASIL families were collected in First Hospital of Peking University from January 2003 to October 2009, and the diagnosis was confirmed by ultrastructural examination or Notch3 gene analysis.The age of onset, initial symptoms, main symptoms in 102 patients were described and the features with those reported in Germany, Japan, Arab and France were compared using x2 test.The cranial MRI changes in 35 patients were analysed and compared with the British and French patients.Results 102 patients had their initial symptoms between 22-80 years with the mean age of onset at (43.9 ± 11.0) years.There was no significant difference in the frequency of ischemic stroke or TIA (79.41% ) between our patients and the patients in other countries.Dementia rate (50.00% ) was significantly greater than that of Arab (21.05% ,x2 =5.513, P =0.020) and French patients(31.11%,x2 =4.517, P =0.034).The frequency of mood disturbances ( 14.71% ) was significantly lower than that of German patients (30.39% ,x2 =7.185, P =0.007).The frequency of migraine ( 13.73% ) was similar to that of France but lower than that of Japan (40%, x2 = 12.658, P = 0.000), Germany (38.24%, x2 =15.932, P=0.000) and Arab (42.11% ,x2 =6.869, P=0.009).Cranial MRIs were abnormal in all 35patients, but no lesion was seen in the medulla oblongata.Lacunar infarcts in the basal ganglia (82.86% )were significantly more frequent than that of French patients (60% , x2 = 5.663, P = 0.017).The frequency of leukoaraiosis in the cerebrum was similar to that of French patients.Anterior temporal involvement (68.57%) was significantly lower than of British patients ( 95%, x2 = 5.211, P = 0.022 ).Conclusion The clinical symptoms and MRI changes of CADASIL in different countries were not identical.Abnormal anterior temporal pole signal on MRI is not a sensitive marker for diagnosis of CADASIL in Chinese patients.
Association of Cerebral Small Vessel Disease Burden with Neuropsychiatric Symptoms in Non-Demented Elderly: A Longitudinal Study

Abstract:Background: The links between cerebral small vessel disease (CSVD) burden and neuropsychiatric symptoms (NPS) have not been fully studied. Objective: We aimed to explore the associations of the CSVD burden with Neuropsychiatric Inventory (NPI) total scores and its subsyndromes in the elderly without dementia. Methods: We investigated 630 non-demented participants from the Alzheimer’s Disease Neuroimaging Initiative. All of them had NPI assessments and 3 Tesla MRI scans at baseline and 616 had longitudinal NPI assessments during the follow-up. Linear mixed-effects models were used to investigate the cross-sectional and longitudinal associations of CSVD burden with NPI total scores and its subsyndromes. Results: Higher CSVD burden longitudinally predicted more serious neuropsychiatric symptoms, including NPS (p < 0.0001), hyperactivity (p = 0.0006), affective symptoms (p = 0.0091), and apathy (p < 0.0001) in the total participants. Lacunar infarcts (LIs), white matter hyperactivities (WMHs), and cerebral microbleeds (CMBs) might play important roles in the occurrence of NPS, since they were longitudinally associated with specific neuropsychiatric subsyndromes. LIs contributed to hyperactivity (p = 0.0092), psychosis (p = 0.0402), affective symptoms (p = 0.0156), and apathy (p < 0.0001). WMHs were associated with hyperactivity (p = 0.0377) and apathy (p = 0.0343). However, CMBs were only related to apathy (p = 0.0141). Conclusion: CSVD burden was associated with multiple neuropsychiatric symptoms, suggesting the importance of monitoring and controlling vascular risk factors. Different markers of CSVD were associated with specific subsyndromes of NPS, suggesting that different markers tended to occur in different encephalic regions.

Abstract:OBJECTIVE:Cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy (CADASIL) is an inherited cerebral arteriolar disease in adulthood, which is caused by NOTCH3 gene mutation. The main symptoms were migraine, cerebral stroke, later with mood disorders and dementia in Caucasian patients. Recently, the disease was also recognized in Asian patients, in whom the migraine is rarely reported. In order to give the clinical features of Chinese patients, we described the clinical symptoms in 4 CADASIL families.METHODS:CADASIL was diagnosed by the investigation of ultra-structure changes of arteriole in sural nerve and NOTCH3 gene mutation in the 4 index cases. Detailed clinical and routine laboratory examinations were performed in these 4 patients, including electrocardiography, nerve conduction velocity, serum glycogen, and serum homocysteine. Additionally, we also collected the clinical data of the other 83 family members through interviews and the available medical records.RESULTS:Of the 83 persons, 29 were classified as clinical suspected patients, who presented one or more of the disease-related neurological symptoms, such as cerebral ischemic events and the cognitive impairment. All of them showed no common risk factors for stroke, such as diabetic mellitus, hypertension, and heart disease. The clinical suspected patients distributed in every consecutive generations and involved both sexes, which was according to the autosomal dominant inherited pattern. The onset age of the disease ranged from 28 to 70-year-old and mainly between the 4th and the 5th decades. The main symptoms were recurrent episodic vertigo, with or without hemiplegia. At the same time or a little bit later, the cognitive impairment was developed in some patients. Compared with the typical presentations of the disease in European patients, none of our 29 patients showed migraine,one index case showed mild sensory disturbance in extremities. Elevated serum homocysteine level and abnormal of nerve conduction study in two index cases (3 and 4) were noticed.CONCLUSION:The onset age of the disease of our patients is similar to that of Caucasian patients. The main symptoms were stroke and dementia. Involvement of post circulation system was the main clinical feature for ischemic events in our patients. Dementia could be found in the early stage of disease. Migraine should not be regarded as a common clinical feature in our patients. The involvement of the peripheral nerves expanded the disease expression outside the central nervous system.
The 7.0 Tesla Magnetic Resonance Imaging Change and Its Relationship with Cognitive Impairment in CADASIL Patients

Abstract:Objective To explore the relationship between the cognitive impairment and cerebral lesions using 7.0 Tesla magnetic resonance imaging (MRI) in CADASIL patients. Methods Thirty five CADASIL patients confirmed by serum NOTCH3 gene detection in Peking University First Hospital from June 2015 to November 2018 were enrolled, including 19 males and 16 females, of which the age of onset was (39.28±8.31) years, the age of admission was (44.61±8.42) years, and the course of disease was (5.29 ± 3.65) years. 7.0 Tesla MRI was performed in all the patients. The numbers of lacunar infarcts and microbleeds were counted and the white matter changes were evaluated with age‐related white matter rating scale (ARWMrs). Neuropsychological tests were used to evaluate the global cognition, memory, attention, executive function, visuo‐spatial function and language function separately. The z score was calculated to evaluate the impairment extent in different scales. The correlation analysis was performed between image changes and neuropsychological tests. Thirty nine normal controls including 20 males and 19 females with age of (42.54±8.92) years were also enrolled, and the same neuropsychological tests were performed in these subjects. Results The numbers of microbleeds and lacunar infarcts were 13.71±10.29 and 5.89 (8.74). The ARWMrs score was 11.26 ± 5.31. There were 21 patients (60%) presented with cognitive impairment. In comparison with the controls, the patients presented with global cognitive impairment (MMSE score 26.87± 3.95 vs 29.08 ± 0.95), including executive (finishing time of Stroops‐c: 80.00 (103.75) s vs 67.79 (16.00) s, correct number of Stroops‐c: 48.00 (44.26) vs 50.00 (2.00), time of trail making A test: 55.5 (81.5) s vs 39.0 (5.0) s, time of trail making B test: 171.0 (159.5) s vs 103.0 (54.0) s, false number of trail making B test: 0(2) vs 0(0)), memory (number of register memory: 16.13±5.41 vs 21.1±15.21, number of long term recall: 4.78± 2.83 vs 7.41±2.24, number of cue recall memory: 4(6) vs 8(4), number of recognition memory: 10.00 (2.25) vs 11.00 (2.00)), attention (number of digital span: 4.42±1.46 vs 7.89±1.65, correct number of symbol digitalis modality test: 38.47 ± 17.29 vs 51.41 ± 13.00), visuo‐spatial (Rey‐osterrich: 34 (5) vs 36 (2)) and language function (number of semantic fluency: 14.70 ± 5.54 vs 17.46 ± 5.63) (P<0.05). The z score demonstrated impaired executive function, followed by visuo‐spatial dysfunction. The number of lacunar infarcts and microbleeds significantly correlated with short term recall memory (r=-0.404, -0.393), long term recall memory (r=-0.375,-0.395), cue memory (r=-0.395,-0.437), Stroops‐c time (r=0.412, 0.503), trails making A test time (r=0.400, 0.434) (P<0.05). The number of lacunar infarcts significantly correlated with symbol digitalis modality test (r=-0.475) (P<0.05). The number of microbleeds significantly correlated with digital span test (r=-0.390), Boston naming test (r=-0.382) and semantic fluency (r=-0.449) (P<0.05). ARWMrs score significantly correlated with MMSE score (r=-0.357), rigister memory (r=-0.342), trails making A finishing time (r=0.425), trails making B finishing time (r=0.463) and correct numbers of trails making B (r=0.392) (P<0.05). Conclusions CADASIL presented with global cognitive impairment, especially executive function and visuo‐spatial function. The white matter changes, lacunar infarcts and microbleeds affected different cognitive function.

Abstract:Community-based studies have reported depression and anxiety are more prevalent among individuals with mild cognitive impairment (MCI). However, the neuropsychiatric features of MCI in memory clinic have not been fully defined. This study aims to investigate the neuropsychiatric features of Alzheimer's disease (AD) and MCI in memory clinic setting. From Feburary, 2006-August, 2007, fifty-seven AD patients, 28 MCI subjects and 46 normal elderly controls were recruited in the memory clinic. Neuropsychiatric features were assessed with Neuropsychiatric Inventory (NPI), and general cognitive status was evaluated with MMSE. After controlling the effect of age and education and adjusting multiple comparison effect, the total score of NPI was significantly increased in AD group (28.60±20.70) than MCI (10.96±13.73) and normal control (2.83±3.52) groups (P<0.05), while there was no significant difference between MCI and normal controls. Specifically, the item scores of hallucination, agitation/aggression, apathy, irritability, and abnormal motor behaviors were significantly higher in AD group than MCI and normal controls groups (P<0.05). The item scores of delusion and anxiety were significantly different only between AD and control group (P<0.05). Other items scores were not significantly different among three groups. There was no significant relationship between total scores of NPI and MMSE (P>0.05). In AD group, the total score of caregiving distress was significantly associated with that of NPI (r=0.92, P<0.05). The results suggested that in memory clinic, the problem associated with hallucination, agitation, apathy, irritability, and abnormal motor behaviors were more pronounced in AD than MCI individuals. However, the findings that individuals with MCI did not present significant neuropsychiatric features in memory clinic were not consistent with the results of community-based studies. The study indicate that neuropsychiatric symptoms should be attended to more closely in memory clinic services.
Characters of diffusion tensor imaging and its relationship to clinical symptoms in CADASIL

Abstract:Objective Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a systemic small vessel disease. Here we studied the change of fractional anisotropy(FA) of diffusion tensor imaging (DTI) and the relationship between FA values and MMSE and NIHSS scores in CADASIL patients. Methods Cranial DTI and conventional MRI scans were obtained from 14 CADASIL patients and 14 age-matched controls. MMSE and NIHSS scores were measured and calculated for all patients. Using DTI,FA values were calculated for the white matter in each cerebral lobe,the posterior limbs of the internal capsule,external capsule and corpus callosum. After adjusting for age,correlations between MMSE,NIHSS scores and the FA values were assessed using Partial correlation coefficients. Results Patients' FA values were significantly lower than the controls' (P<0.05). Whole FA value and FA values of the white matter of the right frontal lobe,right temporal lobe,left parietal lobes,left occipital lobe,posterior limbs of internal capsule,external capsule were significantly related to MMSE (P<0.05). Whole FA value and FA values of the white matter of each measured regions were not related to NIHSS scores. Conclusions FA values decrease in the different area of white matter in CADASIL. The white matter lesions in some regions result in cognitive impairment.

Abstract:Introduction. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is common variant of small vessel disease. Determination of neuroimaging markers have already contributed at an early stage to the more timely diagnosis of the CADASIL. Objective. The aim of the study was to determine probable CADASIL magnetic resonance imaging (MRI) biomarkers for patients from the Russian population. Material and methods. The study included 16 patients. Diagnosis of CADASIL was suspected based on clinical data and an MRI and confi rmed by a molecular genetic study of the NOTCH3 gene. Brain MRI data from 16 patients and spinal cord MRI data from 5 patients were collected and analyzed. Cerebrospinal fl uid (CSF) analysis was performed for diff erential diagnosis in 2 subjects. Results. White matter hyperintensity was observed in 100% of cases. The involvement of the anterior temporal lobes and external capsules was revealed in 62 and 68% of subjects respectively. Multiple lacunar infarcts are characteristic for CADASIL. Microbleeds were detected in 27% of patients. Brain atrophy was observed in 56% of cases. Lesions in corpus callosum and cervical spinal cord were found in 2 patients. Conclusion. Detection of white matters hyperintensities in frontal and parietal lobes, multiple lacunar infarcts with suspected CADASIL makes it possible to select the group of patients for further molecular genetic testing in order to detect mutations in NOTCH3 gene.

Abstract:Objective To summarize the phenotypic spectrum of cerebral autosomal-dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) in Fujian population,evaluate the efficiency of the scale and try to adjust it.Methods Thirty-eight CADASIL patients and 64 CADASIL-like patients were recruited based on the CADASIL scale and gene tests,who visited the First Affiliated Hospital of Fujian Medical University and Fujian Neurology Research Institute from May 2011 to November 2017.Their clinical and neuroimaging characteristics were analyzed.Results The migraine,migraine with aura,transient ischemic attack / stroke,early onset age,psychiatric disturbances,cognitive decline,leukoencephalopathy,subcortical infarcts showed no statistically significant differences between the two groups.Instead,compared with CADASIL-like patients (10/64,15.6％;47/64,73.4％;10/64,15.6％),CADASIL patients demonstrated higher percentages of temporal pole involvements (13/38,34.2％;x5=4.716,P=0.030),external capsule involvements (36/38,94.7％;P=0.008) and family history in at least two generations (13/38,34.2％;x2=4.716,P=0.030).According to the scale,the scores showed statistically significant difference between CADASIL (14.84 ± 3.03) and CADASIL-like patients (13.34 ± 3.31;t=2.282,P=0.025) with an area under receiver operating characteristic curve of 0.622.Conclusions CADASIL showed no specific symptoms in Fujian population.The neuroimaging features were proposed to be focused on,especially the external capsule involvements.CADASIL scale could improve diagnostic efficiency,but still needs to be adjusted for Fujian population.The weight value of migraine,migraine with aura and cognitive decline was suggested to be decreased.
Exploration of the Mechanism Underlying the Association of Incident Microinfarct and Motor Deficit: A Preliminary Functional MRI Study

Abstract:Background: Cerebral microinfarcts (CMIs) might cause measurable disruption to brain connections and are associated with cognitive decline, but the association between CMIs and motor impairment is still unclear. Objective: To assess the CMIs effect on motor function in vivo and explore the potential neuropathological mechanism based on graph-based network method. Methods: We identified 133 non-demented middle-aged and elderly participants who underwent MRI scanning, cognitive, and motor assessment. The short physical performance battery (SPPB) assessed motor function, including balance, walking speed, and chair stand. We grouped participants into 34 incident CMIs carriers and 99 non-CMIs carriers as controls, depending on diffusion-weighted imaging. Then we assessed the independent CMIs effects on motor function and explored neural mechanisms of CMIs on motor impairment via mapping of degree centrality (DC) and eigenvector centrality (EC). Results: CMIs carriers had worse motor function than non-carriers. Linear regression analyses showed that CMIs independently contributed to motor function. CMIs carriers had decreased EC in the precuneus, while increased DC and EC in the middle temporal gyrus and increased DC in the inferior frontal gyrus compared to controls (p < 0.05, corrected). Correlation analyses showed that EC of precuneus was related to SPPB (r = 0.25) and balance (r = 0.27); however, DC (r = -0.25) and EC (r = -0.25) of middle temporal gyrus was related with SPPB in all participants (p < 0.05, corrected). Conclusion: CMIs represent an independent risk factor for motor dysfunction. The relationship between CMIs and motor function may be attributed to suppression of functional hub region and compensatory activation of motor-related regions.

Abstract:Background and Purpose— Accumulating evidence has demonstrated hemodynamic abnormalities and cerebral hypoperfusion in patients with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). Increased venous susceptibility assessed by susceptibility weighted imaging and mapping has been shown to indicate a decrease in venous oxygen saturation. This study aimed to investigate whether altered venous oxygen saturation is related to clinical phenotypes of CADASIL patients. Methods— Using 7.0-T susceptibility weighted imaging and mapping, we compared venous susceptibility of cortical veins between 41 CADASIL patients and 43 age- and sex-matched healthy controls. The magnetic resonance imaging lesion load, mini-mental state examination score, Barthel Index, and modified Rankin Scale were examined in the patient group, and the correlations between venous susceptibility and clinical characteristics were analyzed. Results— Venous susceptibility increased with age ( r =0.508, P =0.001) and was higher in CADASIL patients than in healthy controls ( t =−4.673; P <0.001). We found a positive association between venous susceptibility and the age-related white matter change scores ( r =0.364; P =0.019), number of lacunar infarctions ( r =0.520; P <0.001), number of cerebral microbleeds ( ρ =0.445; P =0.004), and small-vessel disease scores ( ρ =0.465; P =0.002) in CADASIL patients. Moreover, increased venous susceptibility was associated with higher modified Rankin Scale scores in CADASIL patients after adjustment for age- and small-vessel disease scores (odds ratio=3.178; 95% CI, 1.101–9.179; P =0.033). Conclusions— Our findings indicate that extensive cerebral hypoperfusion may induce central nervous system impairment in CADASIL, and susceptibility weighted imaging and mapping could be used clinically to assess the condition of CADASIL patients.

Abstract:Abstract Objectives Neuropsychiatric symptoms (NPS) increase risk of developing dementia and are linked to various neurodegenerative conditions, including mild cognitive impairment (MCI due to Alzheimer's disease [AD]), cerebrovascular disease (CVD), and Parkinson's disease (PD). We explored the structural neural correlates of NPS cross‐sectionally and longitudinally across various neurodegenerative diagnoses. Methods The study included individuals with MCI due to AD, ( n = 74), CVD ( n = 143), and PD ( n = 137) at baseline, and at 2‐years follow‐up (MCI due to AD, n = 37, CVD n = 103, and PD n = 84). We assessed the severity of NPS using the Neuropsychiatric Inventory Questionnaire. For brain structure we included cortical thickness and subcortical volume of predefined regions of interest associated with corticolimbic and frontal‐executive circuits. Results Cross‐sectional analysis revealed significant negative correlations between appetite with both circuits in the MCI and CVD groups, while apathy was associated with these circuits in both the MCI and PD groups. Longitudinally, changes in apathy scores in the MCI group were negatively linked to the changes of the frontal‐executive circuit. In the CVD group, changes in agitation and nighttime behavior were negatively associated with the corticolimbic and frontal‐executive circuits, respectively. In the PD group, changes in disinhibition and apathy were positively associated with the corticolimbic and frontal‐executive circuits, respectively. Conclusions The observed correlations suggest that underlying pathological changes in the brain may contribute to alterations in neural activity associated with MBI. Notably, the difference between cross‐sectional and longitudinal results indicates the necessity of conducting longitudinal studies for reproducible findings and drawing robust inferences.

Abstract:Objective To analyze white matter hyperintensities(WMH)and brain volumes quantitatively in patients with Cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy(CADASIL)on conventional MRI and correlate them with clinical parameters.Methods Fifteen patients with CADASIL confirmed by pathological investigation underwent conventional MRI examinations.Involvement of centrum semiovale,posterior limbs of the internal capsule,corpus callosum,external capsules and white matter of the temporal poles was determined by an experienced radiologist.Normalized brain volume(NBV)and the percentage of WMH to brain volume were calculated with softwares and they were correlated with NIHSS and MMSE statistically.Results Involvement of white matter in the brains of patients with CADASIL included:semiovale centrum in 13 cases,white matter of the temporal poles in 10 cases,external capsules in 8 cases,posterior limbs of the internal capsule in 5 cases,corpus callosum in 4 cases.The percentage of WMH to brain volume was(5.7±1.4)%,and the NBV was(1602±58)×103mm3.Spearman test showed a significant relationship between age and NBV(r=-0.555,P＜0.05).There was a significant relationship between NBV and NIHSS(r=-0.624.P＜0.05).The percentage of WMH to brain volume correlated significantly with NIHSS and MMSE(r=0.522,P＜0.05;r=-0.679,P＜0.01).Conclusions The white matter hvperintensities and brain volume in patients with CADASIL can be assessed quantitatively,which showed correlation with severity of the clinical scale.The development of WMH may refleet the degree of cognitive impairment in CADASIL.

Abstract:Objectives To describe MRI features in 5 Chinese CADASIL families.Methods All the 8 patients came from 5 different CADASIL families,which were diagnosed on the vascular pathological results and Notch3 genetic findings.All the patients developed symptoms in early adults with recurrent ischemic attack and/or progressive dementia.MRI examinations were performed in the index case 1 and his mother,the index case 2,as well as her sister and elder brother,the index 3,4 and 5.Results Lacunar infarcts were noted in thalamus,basal ganglia and periventricular white matter in all 8 patients,external capsule in 6 cases,corpis callosum in 4 cases and brain stem in 3 cases.Bilateral,multifocal or diffusive leukoaraiosis were found in cerebral white matter in all 8 patients,of whom hyperdensity in bilateral temporal poles were detected on T 2 weighted image in 5 cases.MRA were unremarkable in 4 examined patients.During a one-year followup in index case 3,it was noted that leukoaraiosis progressed from a multifocal to more diffusive pattern.Conclusions Lacunar infarcts were commonly found in thalamus,basal ganglia and periventricular white matter in CADASIL.Our result could confirm that the infarcts in external capsule and corpis callosum as well as hyperintensity of temporal poles are MRI markers for diagnosis of CADASIL,but not appeared in all cases.Brain stem may be affected in early stage of the disease.The change of Leukoaraiosis is in keeping with the development of the disease.
Decreased Retinal Vascular Density is Associated with Cognitive Impairment in CADASIL: an Optical Coherence Tomography Angiography Study.

Abstract:This study aimed to assess alterations in retinal vascular density in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) patients using optical coherence tomography angiography (OCTA) and investigate their association with MRI and cognitive features. Twenty-five patients with CADASIL and forty healthy controls were evaluated by Cirrus HD-OCT 5000 with AngioPlex OCTA to determine changes in macular retinal vasculature. Retinal vasculature parameters between two groups were compared. The MRI lesion burden and neuropsychological scales were also examined in patients. The association between OCTA parameters and MRI/cognitive features was evaluated using partial Spearman rank correlation. The vessel density and perfusion density of whole image in macular region (vessel density: t = − 2.834, p = 0.005; perfusion density: t = − 2.691, p = 0.007) were significantly decreased in patients with CADASIL. Moreover, vessel density of whole image in macular region was negatively associated with Fazekas scores (ρ = − 0.457; p = 0.025) and the number of lacunar infractions (ρ = − 0.425, p = 0.038) after adjustment for age. Decreased macular vessel density and perfusion density of whole image were also associated with MoCA scores (vessel density: ρ = 0.542, p = 0.006; perfusion density: ρ = 0.478, p = 0.018) and other domain-specific neuropsychological tests (p < 0.05) after adjustment for age. Decreased retinal vascular density was associated with increased MRI lesion burden and cognitive impairment in patients with CADASIL. Our findings suggest that the degree of retinal vascular involvement, as demonstrated by OCTA, may be consistent with the severity of MRI lesions and the degree of cognitive impairment in patients.
