Comparison of Low-Value Care Among Commercial and Medicaid Enrollees

Abstract:Background: Low-value healthcare is costly and inefficient and may adversely affect patient outcomes. Despite increases in low-value service use, little is known about how the receipt of low-value care differs across payers. Objective: To evaluate differences in the use of low-value care between patients with commercial versus Medicaid coverage. Design: Retrospective observational analysis of the 2017 Rhode Island All-payer Claims Database, estimating the probability of receiving each of 14 low-value services between commercial and Medicaid enrollees, adjusting for patient sociodemographic and clinical characteristics. Ensemble machine learning minimized the possibility of model misspecification. Participants: Medicaid and commercial enrollees aged 18-64 with continuous coverage and an encounter at which they were at risk of receiving a low-value service. Intervention: Enrollment in Medicaid or Commercial insurance. Main measures: Use of one of 14 validated measures of low-value care. Key results: Among 110,609 patients, Medicaid enrollees were younger, had more comorbidities, and were more likely to be female than commercial enrollees. Medicaid enrollees had higher rates of use for 7 low-value care measures, and those with commercial coverage had higher rates for 5 measures. Across all measures of low-value care, commercial enrollees received more (risk difference [RD] 6.8 percentage points; CI: 6.6 to 7.0) low-value services than their counterparts with Medicaid. Commercial enrollees were also more likely to receive low-value services typically performed in the emergency room (RD 11.4 percentage points; CI: 10.7 to 12.2) and services that were less expensive (RD 15.3 percentage points; CI 14.6 to 16.0). Conclusion: Differences in the provision of low-value care varied across measures, though average use was slightly higher among commercial than Medicaid enrollees. This difference was more pronounced for less expensive services indicating that financial incentives may not be the sole driver of low-value care.

Abstract:An additional dose of 500 mg of rapidly absorbed quinidine increased the digoxin concentration in serum after 3-5 hours by up to 46% and prolonged digoxin half life from 50 to 100 hours in six probands who were on chronic quinidine-digoxin medication. These effects were not elicited regularly in persons pretreated with digoxin only. The results show that quinidine both diminishes elimination and produces transient redistribution of digoxin. Chronic quinidine medication leads to protracted digoxin elimination resulting in marked prolongation of digoxin half life. This is the reason for persisting increase of digoxin concentration in serum. Estimation of serum digoxin levels should thus be done 8 hours after the last quinidine (and digoxin) medication at the earliest. On cessation of digoxin, as is done preparing for electric cardioversion, one should remember that digoxin elimination is clearly prolonged should quinidine treatment be continued.

Abstract:BACKGROUND:There are plenty of studies investigating the disparity of payer status in accessing to care. However, most studies are either disease-specific or cohort-specific. Quantifying the disparity from the level of facility through a large controlled study are rare. This study aims to examine how the payer status affects patient hospitalization from the perspective of a facility.METHODS:We extracted all patients with visiting record in a medical center between 5/1/2009-4/30/2014, and then linked the outpatient and inpatient records three year before target admission time to patients. We conduct a retrospective observational study using a conditional logistic regression methodology. To control the illness of patients with different diseases in training the model, we construct a three-dimension variable with data stratification technology. The model is validated on a dataset distinct from the one used for training.RESULTS:Patients covered by private insurance or uninsured are less likely to be hospitalized than patients insured by government. For uninsured patients, inequity in access to hospitalization is observed. The value of standardized coefficients indicates that government-sponsored insurance has the greatest impact on improving patients' hospitalization.CONCLUSION:Attention is needed on improving the access to care for uninsured patients. Also, basic preventive care services should be enhanced, especially for people insured by government. The findings can serve as a baseline from which to measure the anticipated effect of measures to reduce disparity of payer status in hospitalization.

Abstract:Hepatitis C virus (HCV) virions have been shown to be bound to antibodies in patients with chronic HCV infection. The sera from patients infected with genotype 1b HCV contained more antibody‐free virions than those from patients with genotype 2a/2b HCV. When compared at the same levels of serum HCV RNA, free virion titers of genotype 2a/2b‐infected patients were much lower than those of genotype 1b‐infected patients, indicating that a larger fraction of HCV virions are bound to antibodies in the former than in the latter. The gene segments encoding the hypervariable region (HVR) 1, a principal neutralization epitope, of HCV were amplified from the patients' sera by reverse transcription‐polymerase chain reaction. The majority of genotype 2a/2b‐infected patients had very similar HVR 1 sequences to one another, whereas patients infected with genotype 1b HCV had highly heterogeneous sequences. Commercial Versus Medicaid Insurance and Use of High-Priced Anticancer Treatments

Abstract:Abstract Background Because the markups on cancer drugs vary by payor, providers’ financial incentive to use high-price drugs is differential according to each patient’s insurance type. We evaluated the association between patient insurer (commercial vs Medicaid) and the use of high-priced cancer treatments. Materials and Methods We linked cancer registry, administrative claims, and demographic data for individuals diagnosed with cancer in North Carolina from 2004 to 2011, with either commercial or Medicaid insurance. We selected cancers with multiple FDA-approved, guideline-recommended chemotherapy options and large price differences between treatment options: advanced colorectal, lung, and head and neck cancer. The outcome was a receipt of a higher-priced option, and the exposure was insurer: commercial versus Medicaid. We estimated risk ratios (RRs) for the association between insurer and higher-priced treatment using log-binomial models with inverse probability of exposure weights. Results Of 812 patients, 209 (26%) had Medicaid. The unadjusted risk of receiving higher-priced treatment was 36% (215/603) for commercially insured and 27% (57/209) for Medicaid insured (RR: 1.31, 95% CI: 1.02-1.67). After adjustment for confounders the association was attenuated (RR: 1.15, 95% CI: 0.81-1.65). Exploratory subgroup analysis suggested that commercial insurance was associated with increased receipt of higher-priced treatment among patients treated by non-NCI-designated providers (RR: 1.53, 95% CI: 1.14-2.04). Conclusions Individuals with Medicaid and commercial insurance received high-priced treatments in similar proportion, after accounting for differences in case mix. However, modification by provider characteristics suggests that insurance type may influence treatment selection for some patient groups. Further work is needed to determine the relationship between insurance status and newer, high-price drugs such as immune-oncology agents.

Aortofemoral Bypass to Bridge End-Stage Renal Disease Patients with Severe Iliac Calcification to Kidney Transplantation

Abstract:Patients undergoing hemodialysis have a lower survival rate than those who receive a kidney transplant. Mortality among hemodialysis patients is approximately 14.5% compared with 1.5% for transplant recipients. One of the exclusion criteria for renal transplant is severe iliac artery calcification. We performed an aortofemoral bypass in these patients to make them eligible for renal transplantation. Eleven patients were selected to receive an aortofemoral bypass. All had severe calcification of iliac arteries. Eight patients required a bypass from the thoracic aorta and two from the infrarenal level. Revascularization was successful in 10 patients. Patency was 100%. Surgery could not be performed in one owing to severe calcification of the femoral artery. One patient died owing to gastrointestinal bleeding. Two patients developed complications; one needed a splenectomy, and the other developed meningitis and paralytic ileus. To date, four patients have received transplants, and the viability of the transplanted kidney is good in all cases. Renal transplantation is the only method known to improve survival and quality of life for hemodialysis patients. We consider that if patients with severe iliac calcification are well informed of the morbidity and mortality risk of an aortic bypass, this intervention can be justified in this setting.
Multimode monopulse tracking feed with dual-band potential for land-mobile satellite communications in Ka-band

Abstract:The growing interest in bi-directional mobile satellite communications in Ka-band necessitates the development of dedicated antenna systems and feeds. In this paper we describe an operational feeding structure for a Cassegrain satellite reflector antenna which facilitates a multimode monopulse tracking system for circular polarisation at the satellite downlink frequency of 20 GHz. The basic principle can be extended to dual-band functionality, to cover the uplink at 30 GHz as well. This work contributes to the development of a satellite terminal for mobile communications in disaster scenarios [1], [2].

Abstract:Importance: Medicare's Bundled Payments for Care Improvement (BPCI) is a voluntary pilot program evaluating bundled payments for several common cardiovascular conditions. Evaluating the external validity of this program is important for understanding the effects of bundled payments on cardiovascular care.Objective: To determine whether participants in BPCI cardiovascular bundles are representative of US acute care hospitals and identify factors associated with participation.Design, Setting, and Participants: Retrospective cross-sectional study of hospitals participating in BPCI model 2 bundles for acute myocardial infarction (AMI), congestive heart failure (CHF), coronary artery bypass graft, and percutaneous coronary intervention and nonparticipating control hospitals (October 2013 to January 2017). The BPCI participants were identified using data from the Centers for Medicare and Medicaid Services, and controls were identified using the 2013 American Hospital Association's Survey of US Hospitals. Hospital structural characteristics and clinical performance data were obtained from the American Heart Association survey and Centers for Medicare and Medicaid Services. One hundred fifty-nine hospitals participating in BPCI model 2 cardiac bundles and 1240 nonparticipating control hospitals were compared, and a multivariable logistic regression was estimated to identify predictors of BPCI participation.Exposures: Bundled payments.Main Outcomes and Measures: Hospital-level structural characteristics and 30-day risk-adjusted readmission and mortality rates for AMI and CHF.Results: Compared with nonparticipants, BPCI participants were larger, more likely to be privately owned or teaching hospitals, had lower Medicaid bed day ratios (ratio of Medicaid inpatient days to total inpatient days: 17.0 vs 19.3; P < .001), and were less likely to be safety net hospitals (2.5% vs 12.3%; P < .001). The BPCI participants had higher AMI and CHF discharge volumes, were more likely to have cardiac intensive care units and catheterization laboratories, and had lower risk-standardized 30-day mortality rates for AMI (13.7% vs 16.6%; P = .001) and CHF (11.3 vs 12.4; P = .005). In multivariable analysis, larger hospital size and access to a cardiac catheterization laboratory were positively associated with participation. Being a safety net hospital was negatively associated with participation (odds ratio, 0.3; 95% CI, 0.1-0.7; P = .001).Conclusions and Relevance: Hospitals participating in BPCI model 2 cardiac bundles differed in significant ways from nonparticipating hospitals. The BPCI outcomes may therefore have limited external validity, particularly among small and safety net hospitals with limited clinical cardiac services.

The use of unenhanced Doppler sonography in the evaluation of solid breast lesions.

Abstract:OBJECTIVE The objectives of our study were to investigate differences in Doppler sonography features between benign and malignant breast lesions and between malignant lesions with different prognostic factors and to propose diagnostic criteria for Doppler sonography of breast lesions. SUBJECTS AND METHODS We performed power and duplex Doppler sonography examinations in 826 breast lesions scheduled for sonographically guided core needle biopsy. Lesion vascularity, pulsatility index (PI), and resistive index (RI) of the vessels detected were analyzed and correlated with histologic results. RESULTS Color flow was more frequently seen in malignant (237/348 lesions, 68%) than in benign (171/478, 36%) lesions (p < 0.001). However, sensitivity, specificity, and positive and negative predictive values for this sign were low (68%, 64%, 58%, and 73%, respectively). The RI and PI values were significantly higher (p < 0.001) in cancers. Although an overlap in these values between benign and malignant lesions was observed, all but one nodule with an RI of greater than 0.99 (those with null or inverted diastolic flow) or a PI of greater than 4 were malignant. No significant relationship was found between PI, RI, or flow visualization on power Doppler sonography and tumor grade or lymph node involvement in cancers. CONCLUSION Flow visualization on power Doppler sonography indicates a higher possibility of malignancy but is not useful as the main sign for malignancy. However, any lesion with a vessel that has an RI value greater than 0.99 or a PI value greater than 4 within it must be considered as probably malignant regardless of any other sonography sign present. Doppler findings are not useful to predict tumor grade or lymph node involvement.

Abstract:148 Background: Systemic therapies targeting breast cancers associated with germline BRCA1/2 mutations (gBRCAm) have been available since 2018, approved first for locally advanced or metastatic HER-2 negative disease and more recently for use in the adjuvant setting. Prior studies have found substantial underuse of BRCA testing in breast cancer patients and disparities by socioeconomic factors. We compared rates of BRCA testing in non-elderly breast cancer patients with Medicaid vs. commercial insurance. Methods: We used the Clarivate Real-World Data Product (Clarivate RWD), a database of open claims from clearinghouses. The database links medical and pharmacy claims from ≥98% of commercial and government health insurance plans and covers 300 million lives. We identified adults ages 18-64 with commercial insurance or Medicaid with claims for breast cancer diagnosis and treatment in 2016-2021. The primary outcome was receipt of a BRCA test within 6 months after the first breast cancer (index) claim. We compared the proportion who received a BRCA test in Medicaid vs. commercial insurance. In multivariable logistic regression we estimated the impact of insurance type on receipt of a BRCA test, controlling for demographic and health characteristics. We also examined trends over time. Results: We identified 147,050 patients with commercial insurance and 41,259 with Medicaid who met inclusion criteria. The cohort was 99% female, 65% were age 50-64, 35% lived in the South, 11% had diagnosis codes for metastatic disease, and 61% had an Elixhauser comorbidity score of 0. Compared to those with commercial insurance, patients with Medicaid were younger, less likely to live in the South, more likely to have metastatic disease and have comorbid conditions. Medicaid beneficiaries were more likely to receive a BRCA test within 6 months of index claim than commercially insured patients (16.8% vs. 11.7%), with an adjusted odds ratio of 1.41 (95% CI 1.36-1.46). Receipt of a BRCA test was also more likely in patients who were male; younger; lived in the Northeast, Midwest or West; had metastatic disease or greater comorbidity. In analysis stratified by presumed stage, the advantage of Medicaid coverage was statistically significant only in patients with metastatic disease. While the proportion of all patients who received a BRCA test increased substantially in both groups beginning in 2019, it grew at a much greater rate among those with Medicaid, exceeding 20% in those patients by late 2021. Conclusions: In this analysis of health insurance claims, breast cancer patients with Medicaid were more likely to receive a BRCA test than their peers with commercial insurance, and this difference was driven by testing rates in those with presumed metastatic disease. Expanding testing in both groups could facilitate access to effective therapies for both metastatic and non-metastatic breast cancers associated with gBRCAm, thereby improving health outcomes.

Diversity of Stink Bug (Hemiptera: Pentatomidae) Egg Parasitoids in Woodland and Crop Habitats in Southwest Georgia, USA

Abstract:Abstract Euschistus servus (Say), Chinavia hilaris (Say), and Nezara viridula (L.) (Hemiptera: Pentatomidae) are economic pests of row crops. They move within and between closely associated crop and non-crop habitats throughout the growing season in response to deteriorating suitability of their current host plants. This study was conducted to investigate parasitism of naturally occurring C. hilaris and E. servus eggs in woodland habitats and crops alongside these habitats in southwest Georgia, USA. Also, parasitism of sentinel eggs of N. viridula was examined in a woodland habitat. Ten species of parasitoids, including 7 scelionids, 2 eupelmids, and 1 encyrtid, parasitized E. servus eggs. Telenomus podisi Ashmead (Hymenoptera: Scelionidae) was the most prevalent parasitoid of E. servus eggs in each of 3 habitats: woodlands, an early-season crop, and late-season crops. In woodlands, 27.9% of E. servus eggs were parasitized by Anastatus reduvii (Howard) and A. mirabilis (Walsh & Riley) (Hymenoptera: Eupelmidae). Four species of parasitoids, including 1 scelionid, 2 eupelmids, and 1 encyrtid, parasitized C. hilaris eggs. Trissolcus edessae Fouts (Hymenoptera: Scelionidae) was the most prevalent parasitoid of C. hilaris eggs in woodlands and the only parasitoid of C. hilaris eggs in late-season crops. In woodlands, 40.7% of C. hilaris eggs were parasitized by A. reduvii and A. mirabilis. In a woodland habitat, 6.6% of N. viridula sentinel eggs were parasitized by A. reduvii females, Anastatus males, and 1 encyrtid. Anastatus species were the only parasitoids that existed primarily in woodland habitats. In conclusion, a diversity of parasitoid species parasitizes native stink bug eggs in southwest Georgia, and species of parasitoids emerging from stink bug eggs can vary by habitat.
How Overall Health Status and Insurance Influence Utilization of the Primary Prevention Implantable Defibrillator in the Current Era

Abstract:Introduction: The benefit of the implantable cardioverter defibrillator (ICD) for primary prevention of sudden cardiac death in selected patients with left ventricular dysfunction is well established. However, several studies have reported that not all eligible patients receive this therapy. Hypothesis: We hypothesized that co-morbid health conditions and nature of health insurance play a role in the utilization patterns of the primary prevention ICD. Methods: Patients eligible for PPICD based on current guidelines were analyzed from the electronic medical records of a large US tertiary care center. Demographic and clinical data as well as insurance status were reviewed in detail for ICD recipients before implantation compared to ICD non-recipients. The Charlson Comorbidity Index (CCI) was calculated and categorized as low(0-1), medium(2-6) and high(7-12). Results: Over an 8-month period, comparisons were made between 110 recipients and 110 non-recipients. ICD recipients were younger (62.1±15.0 vs. 68.0±18.2; P=0.009), with higher proportion of males (80% vs. 65.5%; P=0.01), and higher BMI (27.8±7.7 vs. 24.3±6.9; P=0.001). Compared to ICD non-recipients, ICD recipients were more likely to have lower comorbidities (high CCI 5.5 % vs. 11.8%; P=0.05) and lower LVEF (23.8±6.4 vs. 25.3±7.2; P=0.04). ICD recipients were less likely to be current alcohol users (5.5% vs. 8.2%; P=0.05), smokers (5.5% vs. 13.6%, P=0.03) or have history of drug abuse (0.9% vs. 5.5%; P=0.02). ICD recipients were more likely to have combined public and private insurance (29.1% vs. 18.2%, P=0.03), and less likely to have public insurance alone (28.2% vs. 32.7%, P=0.05). In multivariable model, independent predictors of implantation included male sex (OR 2.23[1.15-4.33]; P=0.02), age Conclusions: Our findings suggest that age, sex and co-morbid conditions continue to determine ICD utilization in real world clinical practice, with insurance status also playing a role. This important area warrants significant further investigation in diverse US communities.
Center Variation in Medicare Spending for Durable Left Ventricular Assist Device Implant Hospitalizations

Abstract:Importance: Hospitalizations for durable left ventricular assist device (LVAD) implants are expensive and increasingly common. Insights into center-level variation in Medicare spending for these hospitalizations are needed to inform value improvement efforts. Objective: To examine center-level variation in Medicare spending for durable LVAD implant hospitalizations and its association with clinical outcomes. Design, setting, and participants: Retrospective cohort study of linked Medicare administrative claims and Interagency Registry for Mechanically Assisted Circulatory Support (INTERMACS) clinical data comprising 106 centers in the United States providing durable LVAD implant. Centers were grouped into quartiles based on the mean price-standardized Medicare spending of their patients. The study included Medicare beneficiaries receiving primary durable LVAD implant between January 2008 and December 2014. Data were analyzed between November 2017 and October 2018. Main outcomes and measures: Price-standardized Medicare payments and clinical outcomes. Overall and component (facility diagnosis-related group payments, outlier payments, physician services) payments and clinical outcomes (postimplant length of stay and adverse events) were compared across payment quartiles. Results: The study sample included 4442 hospitalized patients, with mean (SD) age of 63.0 (10.8) years, 18.7% female, 27.2% nonwhite, and 6.1% Hispanic ethnicity. Among 4442 hospitalizations, the mean (SD) price-standardized Medicare payment was $176 825 ($60 286) and ranged from $122 953 to $271 472 across 106 centers. The difference in price-standardized payments between lowest and highest spending quartiles was $55 446 ($152 714 vs $208 160; 36%; P < .001), with outlier payments making up most of the difference ($42 742; 77%), followed by DRG ($6929; 13%) and physician services ($5774; 10%). After risk standardization, there was a modest decline in the difference in payments between quartiles ($53 221; 35%), with outlier payments accounting for a larger proportion of the difference (84%). After adjusting for patient characteristics, higher price-standardized payment quartiles were associated with longer postimplant length of stay but were not associated with any adverse events. Conclusions and relevance: Medicare payments for durable LVAD implant hospitalizations vary widely across centers; this was not well explained by prices or case mix. While associated with longer postimplant length of stay, increased spending was not associated with adverse events. As the supply and demand for durable LVAD therapy continues to rise, identifying opportunities to reduce variation in spending from both explained and unexplained sources will ensure high-value use.

Abstract:Background: Although pay for performance (P4P) has become common, many worry that P4P will lead providers to avoid offering surgical procedures to the sickest patients out of concern that poor outcomes will lead to financial penalties. Methods and results: We used Medicare data to compare change in rates of coronary artery bypass graft surgery between 2002 to 2003 and 2008 to 2009 among patients with acute myocardial infarction (AMI) admitted to 126 hospitals participating in Medicare's Premier Hospital Quality Incentive Demonstration P4P program with patients in 848 control hospitals participating in public reporting through the Health Quality Alliance. We examined rates for all patients with AMI and those in the top decile of predicted mortality based on demographics, medical comorbidities, and AMI characteristics. We identified 91 393 patients admitted for AMI in Premier hospitals and 502 536 Medicare patients admitted for AMI in control hospitals. Coronary artery bypass graft surgery rates for patients with AMI in Premier decreased from 13.6% in 2002 to 2003 to 10.4% in 2008 to 2009; there was a comparable decrease in non-Premier hospitals (13.6%-10.6%; P value for comparison of changes between Premier and non-Premier, 0.67). Coronary artery bypass graft surgery rates for high-risk patients in Premier decreased from 8.4% in FY 2002 to 203 to 8.2% in 2008 to 2009. Patterns were similar in non-Premier hospitals (8.4%-8.3%; P value for comparison of changes between Premier and non-Premier, 0.82). Conclusions: Our results show no evidence of a deleterious effect of P4P on access to coronary artery bypass graft surgery for high-risk patients with AMI. These results should be reassuring to those concerned about the potential negative effect of P4P on high-risk patients.
Variation in and Hospital Characteristics Associated With the Value of Care for Medicare Beneficiaries With Acute Myocardial Infarction, Heart Failure, and Pneumonia

Abstract:Importance: Payers and policy makers have advocated for transitioning toward value-based payment models. However, little is known about what is the extent of hospital variation in the value of care and whether there are any hospital characteristics associated with high-value care. Objectives: To investigate the association between hospital-level 30-day risk-standardized mortality rates (RSMRs) and 30-day risk-standardized payments (RSPs) for acute myocardial infarction (AMI), heart failure (HF), and pneumonia (PNA); to characterize patterns of value in care; and to identify hospital characteristics associated with high-value care (defined by having lower than median RSMRs and RSPs). Design, setting, and participants: This national cross-sectional study applied weighted linear correlation to investigate the association between hospital RSMRs and RSPs for AMI, HF, and PNA between July 1, 2011, and June 30, 2014, among all hospitals; examined correlations in subgroups of hospitals based on key characteristics; and assessed the proportion and characteristics of hospitals delivering high-value care. The data analysis was completed in October 2017. The setting was acute care hospitals. Participants were Medicare fee-for-service beneficiaries discharged with AMI, HF, or PNA. Main outcomes and measures: Hospital-level 30-day RSMRs and RSPs for AMI, HF, and PNA. Results: The AMI sample consisted of 4339 hospitals with 487 141 hospitalizations for mortality and 462 905 hospitalizations for payment. The HF sample included 4641 hospitals with 960 960 hospitalizations for mortality and 903 721 hospitalizations for payment. The PNA sample contained 4685 hospitals with 952 022 hospitalizations for mortality and 901 764 hospitalizations for payment. The median (interquartile range [IQR]) RSMRs and RSPs, respectively, was 14.3% (IQR, 13.8%-14.8%) and $21 620 (IQR, $20 966-$22 567) for AMI, 11.7% (IQR, 11.0%-12.5%) and $15 139 (IQR, $14 310-$16 118) for HF, and 11.5% (IQR, 10.6%-12.6%) and $14 220 (IQR, $13 342-$15 097) for PNA. There were statistically significant but weak inverse correlations between the RSMRs and RSPs of -0.08 (95% CI, -0.11 to -0.05) for AMI, -0.21 (95% CI, -0.24 to -0.18) for HF, and -0.07 (95% CI, -0.09 to -0.04) for PNA. The largest shared variance between the RSMRs and RSPs was only 4.4% (for HF). The correlations between the RSMRs and RSPs did not differ significantly across teaching status, safety-net status, urban/rural status, or the proportion of patients with low socioeconomic status. Approximately 1 in 4 hospitals (20.9% for AMI, 23.0% for HF, and 23.9% for PNA) had both lower than median RSMRs and RSPs. Conclusions and relevance: These findings suggest that there is significant potential for improvement in the value of AMI, HF, and PNA care and also suggest that high-value care for these conditions is attainable across most hospital types.

Abstract:Importance: In 2014, Maryland implemented the all-payer model, a distinct hospital funding policy that applied caps on annual hospital expenditures and mandated reductions in avoidable complications. Expansion of this model to other states is currently being considered; therefore, it is important to evaluate whether Maryland's all-payer model is achieving the desired goals among surgical patients, who are an at-risk population for most potentially preventable complications. Objective: To examine the association between the implementation of Maryland's all-payer model and the incidence of avoidable complications and resource use among adult surgical patients. Design, setting, and participants: This comparative effectiveness study used hospital discharge records from the Healthcare Cost and Utilization Project state inpatient databases to conduct a difference-in-differences analysis comparing the incidence of avoidable complications and the intensity of health resource use before and after implementation of the all-payer model in Maryland. The analytical sample included 2 983 411 adult patients who received coronary artery bypass grafting (CABG), carotid endarterectomy (CEA), spinal fusion, hip or knee arthroplasty, hysterectomy, or cesarean delivery between January 1, 2008, and December 31, 2016, at acute care hospitals in Maryland (intervention state) and New York, New Jersey, and Rhode Island (control states). Data analysis was conducted from July 2019 to July 2021. Exposures: All-payer model. Main outcomes and measures: Complications (infectious, cardiovascular, respiratory, kidney, coagulation, and wound) and health resource use (ie, hospital charges). Results: Of 2 983 411 total patients in the analytical sample, 525 262 patients were from Maryland and 2 458 149 were from control states. Across Maryland and the control states, there were statistically significant but not clinically relevant differences in the preintervention period with regard to patient age (mean [SD], 49.7 [19.0] years vs 48.9 [19.3] years, respectively; P < .001), sex (22.7% male vs 21.4% male; P < .001), and race (0.3% vs 0.4% American Indian, 2.8% vs 4.5% Asian or Pacific Islander, 25.9% vs 12.7% Black, 4.7% vs 11.9% Hispanic, and 63.5% vs 63.4% White; P < .001). After implementation of the all-payer model in Maryland, significantly lower rates of avoidable complications were found among patients who underwent CABG (-11.3%; 95% CI, -13.8% to -8.7%; P < .001), CEA (-1.6%; 95% CI, -2.9% to -0.3%; P = .02), hip arthroplasty (-0.8%; 95% CI, -1.0% to -0.5%; P < .001), knee arthroplasty (-0.4%; 95% CI, -0.7% to -0.1%; P = .01), and cesarean delivery (-1.0%; 95% CI, -1.3% to -0.7%; P < .001). In addition, there were significantly lower increases in index hospital costs in Maryland among patients who underwent CABG (-$6236; 95% CI, -$7320 to -$5151; P < .001), CEA (-$730; 95% CI, -$1367 to -$94; P = .03), spinal fusion (-$3253; 95% CI, -$3879 to -$2627; P < .001), hip arthroplasty (-$328; 95% CI, -$634 to -$21; P = .04), knee arthroplasty (-$415; 95% CI, -$643 to -$187; P < .001), cesarean delivery (-$300; 95% CI, -$380 to -$220; P < .001), and hysterectomy (-$745; 95% CI, -$974 to -$517; P < .001). Significant changes in patient mix consistent with a younger population (eg, a shift toward private/commercial insurance for orthopedic procedures, such as spinal fusion [4.3%; 95% CI, 3.4%-5.2%; P < .001] and knee arthroplasty [1.6%; 95% CI, 1.0%-2.3%; P < .001]) and a lower comorbidity burden across surgical procedures (eg, CABG: -0.7% [95% CI, -0.1% to -0.5%; P < .001]; hip arthroplasty: -3.0% [95% CI, -3.6% to -2.3%; P < .001]) were also observed. Conclusions and relevance: In this study, patients who underwent common surgical procedures had significantly fewer avoidable complications and lower hospital costs, as measured against the rate of increase throughout the study, after implementation of the all-payer model in Maryland. These findings may be associated with changes in the patient mix.
Second-Trimester Ultrasound Receipt Mediates the Relationship Between Public Insurance and Prenatal Diagnosis of a Congenital Heart Defect

Abstract:Objective: To delineate the mechanism behind insurance-related disparities in the prenatal diagnosis of a congenital heart defect (CHD). Methods: This was a retrospective analysis of electronic health records of pregnant individuals whose infants received CHD surgery between 2019 and 2020 in the third-largest United States metropolitan area. The outcome was whether a prenatal diagnosis was received. The exposure was the pregnant individual's insurance status. The mediator was second-trimester ultrasound receipt. Control variables included sociodemographic and clinical characteristics of the pregnant individual and infant. The relationships between exposure, mediator, and outcome were quantified using mediation analysis with multivariable fixed-effects regression. Results: In total, 496 pregnant individuals met inclusion criteria; 215 (43.3%) were publicly insured and 305 (61.5%) had prenatal diagnosis. In bivariate regressions, public insurance was associated with a 12.6% lower probability (CI 3%-21%) of prenatal diagnosis. In multivariable models, public insurance was associated with 13.2% lower probability (CI 2%-25%) of second-trimester ultrasound receipt but was no longer associated with prenatal diagnosis after adjusting for second-trimester ultrasound receipt, suggesting a possible mediation effect. Mediation analysis confirmed that second-trimester ultrasound receipt mediated 39% of the relationship between public insurance and prenatal diagnosis. Conclusion: An appreciable portion of insurance-related differences in prenatal CHD diagnosis is due to the lower frequency of second-trimester ultrasound receipt among those with public insurance.

Abstract:Introduction: Safety-net hospitals disproportionately care for medically complex, socially vulnerable, and racial/ethnic minority populations. Prior to the Affordable Care Act, safety-net hospitals had higher mortality and readmission rates for common cardiovascular conditions than their non-safety-net counterparts, but it is unclear whether these disparities remain. Methods: We conducted a serial, cross-sectional study using data from 3265 acute-care hospitals in the US. Performance on 30-day mortality, readmission, and excess days in acute care (EDAC) measures for acute myocardial infarction (AMI), heart failure (HF), stroke, and coronary artery bypass graft surgery (CABG) was compared at safety-net hospitals versus non-safety-net hospitals in fiscal year (FY) 2020 (July 2016 to June 2019) using linear regression models that adjusted for hospital-level characteristics. Interaction terms for safety-net status and year were used to assess for differential changes between FY 2010 (July 2006 to June 2009) and FY 2020. Results: Thirty-day mortality rates at safety-net hospitals compared with non-safety-net hospitals were lower for HF (10.9% vs 11.5%; adjusted difference: -0.49%, 95% CI: -0.65% to -0.33%), higher for AMI (12.8% vs 12.6%; adjusted difference: 0.15%, 0.04% to 0.26%), and similar for CABG and stroke. Thirty-day readmission rates were modestly higher at safety-net sites for AMI (16.4% vs 16.0%; adjusted difference: 0.37%, 0.26% to 0.48%) and HF (22.4% vs 21.8%; 0.58%, 0.44% to 0.72%), and comparable for CABG. However, safety-net hospitals performed markedly worse on the 30-day EDAC measures for AMI (15.2 vs 3.4 excess days; adjusted difference: 10.8, 95% CI: 8.3 to 13.4) and HF (14.1 vs 3.9 excess days; adjusted difference: 10.2, 6.4 to 10.4). From FY 2010 to FY 2020, differences in 30-day mortality and readmission rates differentially improved at safety-net sites for HF, but not AMI. Conclusions: Thirty-day mortality rates for common cardiovascular conditions are now similar at safety-net and non-safety-net hospitals. However, large gaps in 30-day post-discharge acute care utilization indicate that policy initiatives are still needed to support under-resourced safety-net systems and the patients they serve.

Abstract:Background: The goal of the Patient Protection and Affordable Care Act (PPACA) was to expand patient access to health care. Since the rollout of the PPACA, Medicaid patients have demonstrated difficulty obtaining appointments in some specialty care settings. Purpose: To assess the effect of insurance type (Medicaid and private) on patient access to orthopaedic surgery sports medicine specialists for a semiurgent evaluation of a likely operative bucket-handle meniscus tear. The study was designed to determine whether disparities in access exist since the PPACA rollout. Study Design: Cohort study; Level of evidence, 2. Methods: The design was to call 180 orthopaedic surgery sports medicine specialists in 6 representative states (California, Ohio, New York, Florida, Texas, and North Carolina) between June 2015 and December 2015. An appointment was requested for the caller’s fictitious 25-year-old-brother who had suffered a bucket-handle meniscus tear. Each office was called twice to assess the ease of obtaining an appointment: once for patients with Medicaid and once for patients with private insurance. For each call, data pertaining to whether an appointment was given, wait times, and barriers to receiving an appointment were recorded. Results: A total of 177 surgeons were called within the study period. Overall, 27.1% of offices scheduled an appointment for a patient with Medicaid, compared with 91.2% ( P < .0001) for a patient with private insurance. Medicaid patients were significantly more likely to be denied an appointment due to lack of referral compared with private patients (40.2% vs 3.7%, P < .0001), and Medicaid patients were more likely to experience longer wait times for an appointment (15 vs 12 days, P < .029). No significant differences were found in patients’ access to orthopaedic surgery sports medicine specialists between Medicaid-expanded and -nonexpanded states. Medicaid reimbursement for knee arthroscopy with meniscus repair was not significantly correlated with appointment success rate or patient waiting periods. Conclusion: Despite the passage of the PPACA, patients with Medicaid have reduced access to care. In addition, patients with Medicaid confront more barriers to receiving appointments than patients with private insurance and wait longer for an appointment.

Abstract:Importance: Overuse of health care services exposes patients to unnecessary risk of harm and costs. Distinguishing patterns of overuse among hospitals requires hospital-level measures across multiple services. Objective: To describe characteristics of hospitals associated with overuse of health care services in the US. Design, setting, and participants: This retrospective cross-sectional analysis used Medicare fee-for-service claims data for beneficiaries older than 65 years from January 1, 2015, to December 31, 2017, with a lookback of 1 year. Inpatient and outpatient services were included, and services offered at specialty and federal hospitals were excluded. Patients were from hospitals with the capacity (based on a claims filter developed for this study) to perform at least 7 of 12 investigated services. Statistical analyses were performed from July 1, 2020, to December 20, 2020. Main outcomes and measures: Outcomes of interest were a composite overuse score ranging from 0 (no overuse of services) to 1 (relatively high overuse of services) and characteristics of hospitals clustered by overuse rates. Twelve published low-value service algorithms were applied to the data to find overuse rates for each hospital, normalized and aggregated to a composite score and then compared across 6 hospital characteristics using multivariable regression. A k-means cluster analysis was used on normalized overuse rates to identify hospital clusters. Results: The primary analysis was performed on 2415 cohort A hospitals (ie, hospitals with capacity for 7 or more services), which included 1 263 592 patients (mean [SD] age, 72.4 [14] years; 678 549 women [53.7%]; 101 017 191 White patients [80.5%]). Head imaging for syncope was the highest-volume low-value service (377 745 patients [29.9%]), followed by coronary artery stenting for stable coronary disease (199 579 [15.8%]). The mean (SD) composite overuse score was 0.40 (0.10) points. Southern hospitals had a higher mean score than midwestern (difference in means: 0.06 [95% CI, 0.05-0.07] points; P < .001), northeast (0.08 [95% CI, 0.06-0.09] points; P < .001), and western hospitals (0.08 [95% CI, 0.07-0.10] points; P < .001). Nonprofit hospitals had a lower adjusted mean score than for-profit hospitals (-0.03 [95% CI, -0.04 to -0.02] points; P < .001). Major teaching hospitals had significantly lower adjusted mean overuse scores vs minor teaching hospitals (difference in means, -0.07 [95% CI, -0.08 to -0.06] points; P < .001) and nonteaching hospitals (-0.10 [95% CI, -0.12 to -0.09] points; P < .001). Of the 4 clusters identified, 1 was characterized by its low counts of overuse in all services except for spinal fusion; the majority of major teaching hospitals were in this cluster (164 of 223 major teaching hospitals [73.5%]). Conclusions and relevance: This cross-sectional study used a novel measurement of hospital-associated overuse; results showed that the highest scores in this Medicare population were associated with nonteaching and for-profit hospitals, particularly in the South.
