Abstract:Laboratory medicine is a digital science. Every large hospital produces a wealth of data each day—from simple numerical results from, e.g., sodium measurements to highly complex output of "-omics" analyses, as well as quality control results and metadata. Processing, connecting, storing, and ordering extensive parts of these individual data requires Big Data techniques. Whereas novel technologies such as artificial intelligence and machine learning have exciting application for the augmentation of laboratory medicine, the Big Data concept remains fundamental for any sophisticated data analysis in large databases. To make laboratory medicine data optimally usable for clinical and research purposes, they need to be FAIR: findable, accessible, interoperable, and reusable. This can be achieved, for example, by automated recording, connection of devices, efficient ETL (Extract, Transform, Load) processes, careful data governance, and modern data security solutions. Enriched with clinical data, laboratory medicine data allow a gain in pathophysiological insights, can improve patient care, or can be used to develop reference intervals for diagnostic purposes. Nevertheless, Big Data in laboratory medicine do not come without challenges: the growing number of analyses and data derived from them is a demanding task to be taken care of. Laboratory medicine experts are and will be needed to drive this development, take an active role in the ongoing digitalization, and provide guidance for their clinical colleagues engaging with the laboratory data in research.

Abstract:The primary aim of laboratory medicine is to make an accurate diagnosis or risk stratification. A diagnostic tool is said to be useful if its diagnostic performance is good, but it cannot be routinely recommended for clinical use unless it also provides clinical benefits in patient-important outcomes. The latter condition is the effectiveness of the diagnostic tool. While the gold standard of effectiveness relies on randomized controlled trials (RCTs), big-data clinical study employing electronic healthcare records (EHR) provides an alternative to provide evidence to guide decision-making. RCTs are limited by their strict inclusion/exclusion criteria, high cost and ethical constraint. EHR contains data on patient-level granularity and can help to disentangle complex research questions. The Achilles’ heel of observational studies (e.g., big-data study is a kind of observational study in nature) is uncontrolled confounding. Although it is still impossible to control for unmeasured confounding factors, the re-use of EHR helps to control as much confounding as possible. As compared with registry and administrative databases, EHR contains more detailed information on demographics, drugs, procedures and operations. Recent decades have witnessed exponential increase in the application of machine learning techniques in laboratory medicine and these novel techniques continue to provide advanced tool for better prediction of diseases.

Abstract:This paper surveys big data with highlighting the big data analytics in medicine and healthcare. Big data characteristics: value, volume, velocity, variety, veracity and variability are described. Big data analytics in medicine and healthcare covers integration and analysis of large amount of complex heterogeneous data such as various - omics data (genomics, epigenomics, transcriptomics, proteomics, metabolomics, interactomics, pharmacogenomics, diseasomics), biomedical data and electronic health records data. We underline the challenging issues about big data privacy and security. Regarding big data characteristics, some directions of using suitable and promising open-source distributed data processing software platform are given.

Abstract:Background: Healthcare 4.0. refers to the integration of advanced technologies, such as artificial intelligence (AI) and big data analysis, into the healthcare sector. Recognizing the impact of Healthcare 4.0 technologies in laboratory medicine (LM), we seek to assess the overall awareness and implementation of Healthcare 4.0 among members of the Korean Society for Laboratory Medicine (KSLM). Methods: A web-based survey was conducted using an anonymous questionnaire. The survey comprised 36 questions covering demographic information (seven questions), big data (10 questions), and AI (19 questions). Results: In total, 182 (17.9%) of 1,017 KSLM members participated in the survey. Thirty-two percent of respondents considered AI to be the most important technology in LM in the era of Healthcare 4.0, closely followed by 31% who favored big data. Approximately 80% of respondents were familiar with big data but had not conducted research using it, and 71% were willing to participate in future big data research conducted by the KSLM. Respondents viewed AI as the most valuable tool in molecular genetics within various divisions. More than half of the respondents were open to the notion of using AI as assistance rather than a complete replacement for their roles. Conclusions: This survey highlighted KSLM members' awareness of the potential applications and implications of big data and AI. We emphasize the complexity of AI integration in healthcare, citing technical and ethical challenges leading to diverse opinions on its impact on employment and training. This highlights the need for a holistic approach to adopting new technologies.

Abstract:The concept of big data, commonly characterized by volume, variety, velocity, and veracity, goes far beyond the data type and includes the aspects of data analysis, such as hypothesis-generating, rather than hypothesis-testing. Big data focuses on temporal stability of the association, rather than on causal relationship and underlying probability distribution assumptions are frequently not required. Medical big data as material to be analyzed has various features that are not only distinct from big data of other disciplines, but also distinct from traditional clinical epidemiology. Big data technology has many areas of application in healthcare, such as predictive modeling and clinical decision support, disease or safety surveillance, public health, and research. Big data analytics frequently exploits analytic methods developed in data mining, including classification, clustering, and regression. Medical big data analyses are complicated by many technical issues, such as missing values, curse of dimensionality, and bias control, and share the inherent limitations of observation study, namely the inability to test causality resulting from residual confounding and reverse causation. Recently, propensity score analysis and instrumental variable analysis have been introduced to overcome these limitations, and they have accomplished a great deal. Many challenges, such as the absence of evidence of practical benefits of big data, methodological issues including legal and ethical issues, and clinical integration and utility issues, must be overcome to realize the promise of medical big data as the fuel of a continuous learning healthcare system that will improve patient outcome and reduce waste in areas including nephrology.

Laboratory Data as a Potential Source of Bias in Healthcare Artificial Intelligence and Machine Learning Models

Abstract:Artificial intelligence (AI) and machine learning (ML) are anticipated to transform the practice of medicine. As one of the largest sources of digital data in healthcare, laboratory results can strongly influence AI and ML algorithms that require large sets of healthcare data for training. Embedded bias introduced into AI and ML models not only has disastrous consequences for quality of care but also may perpetuate and exacerbate health disparities. The lack of test harmonization, which is defined as the ability to produce comparable results and the same interpretation irrespective of the method or instrument platform used to produce the result, may introduce aggregation bias into algorithms with potential adverse outcomes for patients. Limited interoperability of laboratory results at the technical, syntactic, semantic, and organizational levels is a source of embedded bias that limits the accuracy and generalizability of algorithmic models. Population-specific issues, such as inadequate representation in clinical trials and inaccurate race attribution, not only affect the interpretation of laboratory results but also may perpetuate erroneous conclusions based on AI and ML models in the healthcare literature.

Abstract:Background: To ensure valid results of big data research in the medical field, the input laboratory results need to be of high quality. We aimed to establish a strategy for evaluating the quality of laboratory results suitable for big data research.Methods: We used Korean Association of External Quality Assessment Service (KEQAS) data to retrospectively review multicenter data. Seven measurands were analyzed using commutable materials: HbA1c, creatinine (Cr), total cholesterol (TC), triglyceride (TG), alpha-fetoprotein (AFP), prostate-specific antigen (PSA), and cardiac troponin I (cTnI). These were classified into three groups based on their standardization or harmonization status. HbA1c, Cr, TC, TG, and AFP were analyzed with respect to peer group values. PSA and cTnI were analyzed in separate peer groups according to the calibrator type and manufacturer, respectively. The acceptance rate and absolute percentage bias at the medical decision level were calculated based on biological variation criteria.Results: The acceptance rate (22.5%-100%) varied greatly among the test items, and the mean percentage biases were 0.6%-5.6%, 1.0%-9.6%, and 1.6%-11.3% for all items that satisfied optimum, desirable, and minimum criteria, respectively.Conclusions: The acceptance rate of participants and their external quality assessment (EQA) results exhibited statistically significant differences according to the quality grade for each criterion. Even when they passed the EQA standards, the test results did not guarantee the quality requirements for big data. We suggest that the KEQAS classification can serve as a guide for building big data.

Abstract:Objective: To synthesize data quality (DQ) dimensions and assessment methods of real-world data, especially electronic health records, through a systematic scoping review and to assess the practice of DQ assessment in the national Patient-centered Clinical Research Network (PCORnet).Materials and Methods: We started with 3 widely cited DQ literature-2 reviews from Chan et al (2010) and Weiskopf et al (2013a) and 1 DQ framework from Kahn et al (2016)-and expanded our review systematically to cover relevant articles published up to February 2020. We extracted DQ dimensions and assessment methods from these studies, mapped their relationships, and organized a synthesized summarization of existing DQ dimensions and assessment methods. We reviewed the data checks employed by the PCORnet and mapped them to the synthesized DQ dimensions and methods.Results: We analyzed a total of 3 reviews, 20 DQ frameworks, and 226 DQ studies and extracted 14 DQ dimensions and 10 assessment methods. We found that completeness, concordance, and correctness/accuracy were commonly assessed. Element presence, validity check, and conformance were commonly used DQ assessment methods and were the main focuses of the PCORnet data checks.Discussion: Definitions of DQ dimensions and methods were not consistent in the literature, and the DQ assessment practice was not evenly distributed (eg, usability and ease-of-use were rarely discussed). Challenges in DQ assessments, given the complex and heterogeneous nature of real-world data, exist.Conclusion: The practice of DQ assessment is still limited in scope. Future work is warranted to generate understandable, executable, and reusable DQ measures.

Abstract:Prognostics and health management (PHM) is critical for enhancing equipment reliability and reducing maintenance costs, and research on intelligent PHM has made significant progress driven by big data and deep learning techniques in recent years. However, complex working conditions and high-cost data collection inherent in real-world scenarios pose small-data challenges for the application of these methods. Given the urgent need for data-efficient PHM techniques in academia and industry, this paper aims to explore the fundamental concepts, ongoing research, and future trajectories of small data challenges in the PHM domain. This survey first elucidates the definition, causes, and impacts of small data on PHM tasks, and then analyzes the current mainstream approaches to solving small data problems, including data augmentation, transfer learning, and few-shot learning techniques, each of which has its advantages and disadvantages. In addition, this survey summarizes benchmark datasets and experimental paradigms to facilitate fair evaluations of diverse methodologies under small data conditions. Finally, some promising directions are pointed out to inspire future research.

Abstract:Lecture As most people expect, the COVID-19 pandemic will change our life radically. One of the directions is expected to minimize off-line contacts in daily lives and healthcare is surely not the exception. Big hospitals, which are centralized systems evolved to deliver healthcare most effectively, can become the media of the spread of diseases in some cases and the adoption of a decentralized healthcare system will be a helpful strategy to prevent it. This strategy can be implemented through a smart healthcare system, and it will be one more necessity of a smart healthcare system in our society. Smart healthcare, which will be delivered mainly through online contact, is a very comprehensive concept enabling otherwise not possible healthcare such as health monitoring using life-log data from the wearable device and far beyond the online consultation with a doctor. Although there will be many social, ethical, and technical challenges for smart healthcare, the discussion today will be limited to two challenges, interoperability and protecting the data subjects' rights. Smart healthcare needs to collect and process comprehensive health information from a vast number of data subjects during its development and delivery and the data subjects' rights are highly vulnerable during these processes. Collecting and exchanging health information from various sources such as hospitals, clinics, smartwatches, and so on, inevitably raises the issue of interoperability of the data from different sources. The implementation of smart healthcare requires technical solutions to both challenges. General Data Protection Regulation, recently issued by the EU, deals the data subjects' rights very seriously, such as the right to be informed, the right of access, the right to rectification, the right to erasure, the right to restrict processing, the right to data portability, the right to object, and the rights in relation to automated decision making and profiling. Technical innovations required to protect these data subjects' rights will be discussed here. Compared to traditional healthcare which mostly dealt with medical data generated within the healthcare facility itself, smart healthcare requires health data beyond the healthcare facilities. Not to mention the life-log data from different wearable devices, even the medical data from different hospitals are not interoperable. Although many health information standards have been proposed, such as Systematized Nomenclature of Medicine Clinical Terms (SMOMED CT), Logical Observation Identifier Name Code (LOINC), Health Level 7 Fast Healthcare Interoperability Resources (HL7 FHIR), and so on, the adoption of these standards in hospital information system in Korea has been very slow. This low adoption of health information standards in our hospital information system obstructs the so-called, meaningful use of electronic medical records. The necessity of adopting health information standards will be refreshed today. Smart healthcare requires the active participation of health consumers and active participation of health consumers requires their health literacy. Health literacy of health consumers will be fostered by innovative technical aids such as data visualization, the decision support system for consumers, and more. Health literacy will the most powerful driving force of smart healthcare implementation.

Abstract:Laboratory medicine is integral to public health and healthcare provision, and it relies on numerous analytical techniques to provide timely, objective data to healthcare professionals to guide disease prevention, diagnosis, treatment, and monitoring. Driven and defined by a culture of innovation, recent technological advances have revolutionized modern laboratory medicine and added significant value and visibility to its role in healthcare and clinical decision-making. Noteworthy innovations in laboratory automation, genomics, nuclear magnetic resonance spectroscopy, mass spectrometry, microfluidics, and electronic tools have changed the face of omics research. The growing application of these technologies, as well as their integration with microtechnology and point-of-care testing, has contributed to improved patient outcomes and narrowing of the clinical-laboratory interface to facilitate a patient-centered approach to healthcare. However, to adequately capitalize on these advancements, new tools such as artificial intelligence and data mining are needed to harness the exciting potential of medical big data derived from these novel techniques. The current review provides an overview of the recent technological advancements in laboratory medicine, with a critical discussion on their clinical utility and future perspectives. The promise and potential for precision and personalized medicine is also discussed and appraised, with specific attention paid to the contingency of its success on advanced information technology capabilities.

Abstract:The amount of data generated in the field of laboratory medicine has grown to an extent that conventional laboratory information systems (LISs) are struggling to manage and analyze this complex, entangled information ("Big Data"). Statistical learning, a generalized framework from machine learning (ML) and artificial intelligence (AI) is predestined for processing "Big Data" and holds the potential to revolutionize the field of laboratory medicine. Personalized medicine may in particular benefit from AI-based systems, especially when coupled with readily available wearables and smartphones which can collect health data from individual patients and offer new, cost-effective access routes to healthcare for patients worldwide. The amount of personal data collected, however, also raises concerns about patient-privacy and calls for clear ethical guidelines for "Big Data" research, including rigorous quality checks of data and algorithms to eliminate underlying bias and enable transparency. Likewise, novel federated privacy-preserving data processing approaches may reduce the need for centralized data storage. Generative AI-systems including large language models such as ChatGPT currently enter the stage to reshape clinical research, clinical decision-support systems, and healthcare delivery. In our opinion, AI-based systems have a tremendous potential to transform laboratory medicine, however, their opportunities should be weighed against the risks carefully. Despite all enthusiasm, we advocate for stringent added-value assessments, just as for any new drug or treatment. Human experts should carefully validate AI-based systems, including patient-privacy protection, to ensure quality, transparency, and public acceptance. In this opinion paper, data prerequisites, recent developments, chances, and limitations of statistical learning approaches are highlighted.

Abstract:In the last decades, clinical laboratories have significantly advanced their technological capabilities, through the use of interconnected systems and advanced software. Laboratory Information Systems (LIS), introduced in the 1970s, have transformed into sophisticated information technology (IT) components that integrate with various digital tools, enhancing data retrieval and exchange. However, the current capabilities of LIS are not sufficient to rapidly save the extensive data, generated during the total testing process (TTP), beyond just test results. This opinion paper discusses qualitative types of TTP data, proposing how to divide laboratory-generated information into two categories, namely metadata and peridata. Being both metadata and peridata information derived from the testing process, it is proposed that the first is useful to describe the characteristics of data, while the second is for interpretation of test results. Together with standardizing preanalytical coding, the subdivision of laboratory-generated information into metadata or peridata might enhance ML studies, also by facilitating the adherence of laboratory-derived data to the Findability, Accessibility, Interoperability, and Reusability (FAIR) principles. Finally, integrating metadata and peridata into LIS can improve data usability, support clinical utility, and advance AI model development in healthcare, emphasizing the need for standardized data management practices.
Beyond the hype of big data and artificial intelligence: building foundations for knowledge and wisdom

Abstract:Big data, coupled with the use of advanced analytical approaches, such as artificial intelligence (AI), have the potential to improve medical outcomes and population health. Data that are routinely generated from, for example, electronic medical records and smart devices have become progressively easier and cheaper to collect, process, and analyze. In recent decades, this has prompted a substantial increase in biomedical research efforts outside traditional clinical trial settings. Despite the apparent enthusiasm of researchers, funders, and the media, evidence is scarce for successful implementation of products, algorithms, and services arising that make a real difference to clinical care. This article collection provides concrete examples of how "big data" can be used to advance healthcare and discusses some of the limitations and challenges encountered with this type of research. It primarily focuses on real-world data, such as electronic medical records and genomic medicine, considers new developments in AI and digital health, and discusses ethical considerations and issues related to data sharing. Overall, we remain positive that big data studies and associated new technologies will continue to guide novel, exciting research that will ultimately improve healthcare and medicine-but we are also realistic that concerns remain about privacy, equity, security, and benefit to all.

Abstract:OBJECTIVES: The main purpose of this study was to explore whether the use of big data can effectively reduce healthcare concerns, such as the selection of appropriate treatment paths, improvement of healthcare systems, and so on.METHODS: By providing an overview of the current state of big data applications in the healthcare environment, this study has explored the current challenges that governments and healthcare stakeholders are facing as well as the opportunities presented by big data.RESULTS: Insightful consideration of the current state of big data applications could help follower countries or healthcare stakeholders in their plans for deploying big data to resolve healthcare issues. The advantage for such follower countries and healthcare stakeholders is that they can possibly leapfrog the leaders' big data applications by conducting a careful analysis of the leaders' successes and failures and exploiting the expected future opportunities in mobile services.CONCLUSIONS: First, all big data projects undertaken by leading countries' governments and healthcare industries have similar general common goals. Second, for medical data that cuts across departmental boundaries, a top-down approach is needed to effectively manage and integrate big data. Third, real-time analysis of in-motion big data should be carried out, while protecting privacy and security.

Abstract:During recent years, the digital revolution has changed the face of societies including industrial production, economies and peoples' social lives. From these changes we may extrapolate the developments that digitization of health care will bring to medicine in general and laboratory medicine in particular. Disruptive technologies will fundamentally change the way laboratory tests are going to be ordered, carried out and interpreted in the future, and test results from various sources need to be curated to be of added value for the patient's condition. Wearables and implantables will quantify the concentrations for an unknown number of laboratory parameters, and the data will be stored in cloud services at the fingertips of the patient as the sovereign of his/her health care data. A 24/7 online availability of health services will strengthen predictive medicine and may enable a vastly improved preventive health care that is supported by deep-learning algorithms for clinical decision-making not only on behalf of the physician, but also the empowered patient (e.g. health bots). This will likely shift the current role of Laboratory Medicine as a central provider of diagnostic information from a "hidden champion" towards a higher visibility redefining the patient-physician-laboratory relationship. For example, accessing digital health data will allow Laboratory Medicine to more efficiently contribute to the medical dialog than is often the case today. From this perspective, this will require major readjustments in the way we execute our profession, and it will also need new concepts of education and continuous professional development.

Abstract:Clinicians decisions are becoming more and more evidence-based meaning in no other field the big data analytics so promising as in healthcare. Due to the sheer size and availability of healthcare data, big data analytics has revolutionized this industry and promises us a world of opportunities. It promises us the power of early detection, prediction, prevention and helps us to improve the quality of life. Researchers and clinicians are working to inhibit big data from having a positive impact on health in the future. Different tools and techniques are being used to analyze, process, accumulate, assimilate and manage large amount of healthcare data either in structured or unstructured form. In this paper, we would like to address the need of big data analytics in healthcare: why and how can it help to improve life?. We present the emerging landscape of big data and analytical techniques in the five sub-disciplines of healthcare <a class="link-external link-http" href="http://i.e.medical" rel="external noopener nofollow">this http URL</a> image analysis and imaging informatics, bioinformatics, clinical informatics, public health informatics and medical signal analytics. We presents different architectures, advantages and repositories of each discipline that draws an integrated depiction of how distinct healthcare activities are accomplished in the pipeline to facilitate individual patients from multiple perspectives. Finally the paper ends with the notable applications and challenges in adoption of big data analytics in healthcare.

Abstract:Big data technologies are increasingly used for biomedical and health-care informatics research. Large amounts of biological and clinical data have been generated and collected at an unprecedented speed and scale. For example, the new generation of sequencing technologies enables the processing of billions of DNA sequence data per day, and the application of electronic health records (EHRs) is documenting large amounts of patient data. The cost of acquiring and analyzing biomedical data is expected to decrease dramatically with the help of technology upgrades, such as the emergence of new sequencing machines, the development of novel hardware and software for parallel computing, and the extensive expansion of EHRs. Big data applications present new opportunities to discover new knowledge and create novel methods to improve the quality of health care. The application of big data in health care is a fast-growing field, with many new discoveries and methodologies published in the last five years. In this paper, we review and discuss big data application in four major biomedical subdisciplines: (1) bioinformatics, (2) clinical informatics, (3) imaging informatics, and (4) public health informatics. Specifically, in bioinformatics, high-throughput experiments facilitate the research of new genome-wide association studies of diseases, and with clinical informatics, the clinical field benefits from the vast amount of collected patient data for making intelligent decisions. Imaging informatics is now more rapidly integrated with cloud platforms to share medical image data and workflows, and public health informatics leverages big data techniques for predicting and monitoring infectious disease outbreaks, such as Ebola. In this paper, we review the recent progress and breakthroughs of big data applications in these health-care domains and summarize the challenges, gaps, and opportunities to improve and advance big data applications in health care.
