Abstract:Background: Although the use of expanded-criteria donors (ECDs) alleviates the problem of organ shortage, it significantly increases the incidence of delayed graft function (DGF). DGF is a common complication after kidney transplantation; however, the effect of DGF on graft loss is uncertain based on the published literature. Hence, the aim of this study was to determine the relationship between DGF and allograft survival. Methods: We conducted a retrospective, multicenter, observation cohort study. A total of 284 deceased donors and 541 recipients between February 2012 and March 2017 were included. We used logistic regression analysis to verify the association between clinical parameters and DGF, and Cox proportional hazards models were applied to quantify the hazard ratios of DGF for kidney graft loss. Results: Among the 284 deceased donors, 65 (22.8%) donors were ECD. Of the 541 recipients, 107 (19.8%) recipients developed DGF, and this rate was higher with ECD kidneys than with standard-criteria donor (SCD) kidneys (29.2% vs. 17.1%; P = 0.003). The 5-year graft survival rate was not significantly different between SCD kidney recipients with and without DGF (95.8% vs. 95.4%; P = 0.580). However, there was a significant difference between ECD kidney recipients with and without DGF (71.4% vs. 97.6%; P = 0.001), and the adjusted hazard ratio (HR) for graft loss for recipients with DGF was 1.885 (95% confidence interval [CI] = 1.305-7.630; P = 0.024). Results showed that induction therapy with anti-thymocyte globulin was protective against DGF (odds ratio = 0.359; 95% CI = 0.197-0.652; P = 0.001) with all donor kidneys and a protective factor for graft survival (HR = 0.308; 95% CI = 0.130-0.728; P = 0.007) with ECD kidneys. Conclusion: DGF is an independent risk factor for graft survival in recipients with ECD kidneys, but not SCD kidneys.
Effect of delayed graft function on longer‐term outcomes after kidney transplantation from donation after circulatory death donors in the United Kingdom: A national cohort study

Abstract:<p>Kidneys from donation after circulatory death (DCD) donors are utilized variably worldwide, in part due to high rates of delayed graft function (DGF) and putative associations with adverse longer‐term outcomes. We aimed to determine whether the presence of DGF, and its duration, were associated with poor longer‐term outcomes after kidney transplantation from DCD donors. Using the UK transplant registry, we identified 4714 kidney‐only transplants from controlled DCD donors to adult recipients between 2006‐2016; 2832 recipients (60·1%) had immediate graft function and 1882 (39.9%) had DGF. Of the 1847 recipients with DGF duration recorded, 926 (50·1%) had DGF <7 days, 576 (31·2%) had DGF 7‐14 days, and 345 (18·7%) had DGF >14 days. After risk adjustment, the presence of DGF was not associated with inferior long‐term graft or patient survivals. However, DGF duration of >14 days was associated with an increased risk of death‐censored graft failure (hazard ratio 1·7, p=0·001) and recipient death (hazard ratio 1·8, p<0·001) compared to grafts with immediate function. This study suggests that shorter periods of DGF have no adverse influence on graft or patient survival after DCD donor kidney transplantation and that DGF >14 days is a novel early biomarker for significantly worse longer‐term outcomes.</p>

Abstract:BACKGROUND:Donation after cardiac death (DCD) and living-donor (LD) kidney transplantation are the main kidney transplantation types in China. But the outcome of DCD kidney transplantation compared with LD kidney transplantation remains unclear. METHODS:In this study, 325 DCD and 409 LD kidney transplantations were included. We retrospectively compared 3-year graft survival, death-censored graft survival, recipient survival, and graft function. All kidneys of the DCD group were procured from voluntary donation after the citizens' death by the Organ Procurement Organization (OPO) in the presence of the Red Cross, and the transplantation application was approved by the Organ Transplant Ethics Committee. RESULTS:The graft function at year 3 in the DCD group was superior to that of the LD group (eGFR: 71.14±22.28 vs 64.29±16.76 mL/min/1.73 m2; P < .001). After matching donor age, there was no significant difference between the paired DCD and LD group (eGFR: 62.22±18.50 vs 66.99±17.81 mL/min/1.73 m2; P = .068). The 3-year graft survival (94.7% vs 97.4%; P = .041) and recipient survival (97.2% vs 99.5%; P = .011) were a little worse in the total DCD group. However, once the DCD kidney transplantation recipients survived more than 2 months, graft and recipient survival rates were similar between the DCD and LD groups (97.7% vs 97.4%, P = .866 and 99.5% vs 99.0%, P = .466). These results were confirmed in an age-paired groups study. Severe infection was the main cause of graft loss and recipient death in the early stage of DCD transplantation. CONCLUSIONS:Medium- and long-term graft function of DCD kidney transplantation were comparable to LD kidney transplantation. Our results supported the continued use of DCD kidneys.

Abstract:Abstract This meta-analysis examined the relationship between self-efficacy and performance in sport. Based on 45 studies (102 correlations), the average correlation between self-efficacy and sport performance was .38. Given the heterogeneity of findings, follow-up univariate and multivariate moderator analyses were conducted. Results indicated that the most important moderator was concordance, thereby highlighting the importance of matching the self-efficacy and performance measures. Additional moderators we examined included the types of self-efficacy measures, the types of performance measures, the nature of the task, and the time of assessments. These variables accounted for approximately 44% of the variance in the self-efficacy-performance relationship. Practical implications of the findings are discussed.
The Effect of Warm Ischemic Related Factors on the Function of Renal Allograft after Donation after Cardiac Death Renal Transplantation

Abstract:Objective The effect of renal ischemia-reperfusion injury on the function of renal allograft was investigated after donation after cardiac death (DCD) renal transplantion. Methods We retrospectively analyzed the clinical data of renal transplant donors and recipients at the Kidney Disease Center of the First Affiliated Hospital of Zhejiang University from May 2011 to June 2015. One year after transplantation, the recipients were classified into high renal function group [estimated glomerular filtration rate (eGFR) ] ≥60 mL·min-1·(1.73 m2)-1) and low renal function group (eGFR <60 mL·min-1·(1.73 m2)-1. According to the DCD donor surgery record, the systolic blood pressure (SBP) and blood oxygen saturation (SpO2) of the donors during different periods of warm ischemia in two groups were analyzed. The normal distribution measurement data were showed as the mean±standard deviation (±s), and the t-test was used to compare the basic characteristic data, donor SBP and SpO2 in the different warm ischemia periods between the two groups, except for gender. The Wilcoxon Signed Rank test was used to compare the pathological scores of glomerular and glomerular of donor kidneys between two groups when organ procurement. Count data were expressed as a percentage. Chi-square test was used to compare the donor gender and DGF incidence in the two groups. P<0.05 was considered statistically significant. Results By June 2016, all patients were followed up (28.4±2.8) months, ranged from 13.1 to 62.5 months. 340 recipients were eventually included, including 259 recipients in the high renal function group and 81 recipients in the low renal function group. The incidence of DGF was 14.7% (38/259) and 22.2% (18/81) in high and low renal function groups within 1 year after transplantation, with no significant difference (χ2=2.557, P>0.05). The average age and BMI of donors in the high renal function group were lower than those in the low renal function group, and the percentage of males and eGFR at the time of donation was higher than that in the low renal function group (t=-6.363, -2.049, 4.190, χ2=4.863, P all <0.05). The renal pathology scores of donors with high renal function were lower than those with low renal function, and the difference was statistically significant (Z=-2.606, P<0.05). The recipients of the high renal function group were younger than the low renal function group, and the proportion of males was higher than the low renal function group, with significant difference (t=-2.790, χ2=9.658, P<0.05). In the high and low renal function groups, the initial SpO2 decreased by 40%, the average time from life support device withdrawal, different initial SpO2 with 90%, 80%, 70%, 60% of to SpO2 to SpO2 could not be measured, were (5.9±4.3) and (4.8±3.3), (8.0±5.2) and (6.1±4.4), (4.5±3.6) and (3.5±2.8), (4.0±3.7) and (2.9±2.4), (4.0±3.6) and (2.8±2.7), (3.6±3.5) and (2.4±2.5) minutes, with significant difference (t=2.088, 2.983, 2.328, 2.622, 2.557, 2.759, all P<0.05). In the high and low renal function groups, the average change rate of initial SpO2 declined by 10%, 40%, life support device withdrawal to SpO2 not detected, initial 60% SpO2 to SpO2 not detected were (2.40±1.78) and (2.90±1.70), (8.71±6.96) and (15.01±12.97), (19.60±17.49) and (25.80±22.85), (22.41±15.94) and (29.93±19.36) percentage per minute, with significant difference (t=-2.230, -5.647, -2.577, -3.514, P all <0.05). Conclusions The prognosis of DCD renal transplantation was influenced by the general factors such as donor and recipient age and BMI. The high and low renal function recipients had different high and low SpO2 time intervals and different rates of change derived from the DCD donors. During DCD renal transplantation, optimized surgical procedures should be taken to minimize the low SpO2 duration, so that the warm ischemia damage of the donor would be alleviated to acheive a better prognosis of tranplant. Abstract:Expanded criteria donor (ECD) kidneys experience suboptimal outcomes compared with standard criteria donor kidneys. To examine the additional impact of deceased organ category, donation after circulatory death (DCD), and neurologic determination of death (NDD) on ECD outcomes, we examined 1- and 3-year patient and graft survival in all ECD kidney recipients in our institution between January 2008 and December 2017. Of 166 ECD recipients, 49 (29.5%) were DCD and 117 (70.5%) were NDD. Delayed graft function was higher in the DCD/ECD group 61.2 % vs 32.0 % among NDD/ECD recipients. Graft loss was significantly increased among DCD/ECD (hazard ratio for graft loss 4.81 [95% CI1.78-13.01], P = .002 at 1 year and 2.03 [95% CI 1.03-4.0], P = .042 at 3 years). Death-censored graft loss was higher among DCD/ECD (hazard ratio was 10.12 [95% CI, 2.14, 47.92], P = .004 at 1 year and 2.83 [95% CI, 1.24, 6.46], P = .014 at 3 years). There was no statistically significant difference in all-cause mortality. Our study demonstrated that DCD/ECD kidneys have lower graft survival compared with NDD/ECD kidneys. Time on dialysis, waiting time, and panel reactive antibody should be taken into account when offering these organs to patients.

Abstract:In this section, we provide a multi authority CP-ABE scheme for secure data retrieval in decentralized DTNs. Each local authority issues partial personalized and attribute key components to a user by performing secure 2PC protocol with the central authority. Each attribute key of a user can be updated individually and immediately. Thus, the scalability and security can be enhanced in the proposed scheme. Since the first CP-ABE scheme proposed by Bettencourt et al , dozens of CP-ABE schemes have been proposed. The subsequent CP-ABE schemes are mostly motivated by more rigorous security proof in the standard model. However, most of the schemes failed to achieve the expressiveness of the Bettencourt et al.’s scheme, which described an efficient system that was expressive in that it allowed an encrypt or to express an access predicate in terms of any monotonic formula over attributes. Therefore, in this section, we develop a variation of the CP-ABE algorithm partially based on (but not limited to) Bettencourt et al.’s construction in order to enhance the expressiveness of the access control policy instead of building a new CP-ABE scheme from scratch

Abstract:Objce tiev To compare the short -term effects of kidney transplantation between donation after cardiac death (DCD) and standard criteria donors (SCD).Methods From January 2011 to December 2014, the clinical data of 74 cases of kidney transplantation from DCD (DCD group) and 143 cases from SCD (SCD group) were retrospectively analyzed.All patients were performed with antibody induction and triple immunosuppressive regimens ( tacrolimus and mycophenolate mofetil plus prednisone ) .The function of transplant kidney ,survival rate of transplants and recipients during the short term after transplantation were evaluated .The incidence of delayed graft function ( DGF) , acute rejection ( AR) and pulmonary infection were monitored as well .Twos-ample t test was used to compare between the two groups .Chi-square test was used to evaluate survival rate of recipients , incidence of DGF, AR and pulmonary infection .P<0.05 was considered to have statistical significance .Results Up to August 2015 , DCD group and SCD group were followed up with ( 23 ±13 ) months and ( 25 ± 17) months, respectively;and the 6-month survival rate of transplant kidney was 90.5% (67/74), 93.0% (133/143), respectively.The 6-month survival rate of recipients was 93.2% (69/74), 94.4%(135/143), respectively.There was no significant difference in the survival rate of transplants and recipients between the two groups (χ2 =0.41, 0.12, P>0.05).In DCD group, the incidence of DGF was 28.4%(21/74), significantly higher than 7.0% (10/143) in SCD group (χ2 =18, 21, P<0.05).The 6-month incidence of pulmonary infection and AR rate of DCD and SCD were 14.9%(11/74), 21.0% (30/143) and 13.5% (10/74), 7.0% (10/143), respectively.The serum creatinine of 1, 3 months after operation in DCD group was significantly higher than SCD group ( t=4.74, 2.95, P<0.05).There was no significant difference for the seurm creatinine of 6 months after operation (t=1.22, P>0.05).Conclusions Kidney transplantation from DCD has higher risk of DGF and AR incidence .Renal function in the SCD group was better than the DCD group during the short term.Further following up is needed to evaluate the distal survival effects .

Abstract:Background: The quality of deceased donation seriously increased the incidence of delayed graft function (DGF) after renal transplantation. However, there is still no recognized donor evaluation system for DGF prediction in China. This study investigated the impact of donor risk factor on DGF and developed a donor risk score system for DGF prediction. Methods: A total of 1807 deceased donors and 3599 their donated renal transplant performed in 22 center of China were included this retrospective study. Associations of DGF with the clinical characteristics of donors were quantified by univariate analysis. A donor risk scoring system was developed by multivariate logistic regression and validated using an independent sample. Results: In this retrospective multi-center study, the incidence of DGF was 18.7% from donor aspect. Six donor characteristics and different layings including age, primary disease, hypertension history, terminal serum creatininer, and hypotension persistent and CPR tine were risk factors of DGF. A 49-point score system of donor risk was established for DGF prediction which showed a good degree of discrimination. The prediction model was achieved an area under the curve of 0.7423 (95% CI 0.69-0.79) in predicting DGF in internal validation. The AUROC was 0.7552 (95% CI0.67-0.84) in external validation by a independent one center sample compared to Irish model which demonstrated an AUROC of 0.6181 (95% CI 0.53-0.70). Conclusion: The study determined deceased donor risk factors related to DGF after renal transplantation in China and firstly develop a scoring system in line with the characteristics of the Chinese population for DGF prediction more effectively. The scoring system had good diagnostic significance and consistency and can be used by clinicians to make evidence-based decisions on the use of renal from deceased donor and guiding renal transplantation therapy.References: 1. Abstract:Background. The number of donors from donation after circulatory determination of death (DCDD) has increased by at least 4-fold over the past decade. This study evaluated the association between the antecedent cardiac arrest status of controlled DCDD donors and the risk of delayed graft function (DGF). Methods. Using data from the Australia and New Zealand Dialysis and Transplant, the associations between antecedent cardiac arrest status of DCDD donors before withdrawal of cardiorespiratory support, DGF, posttransplant estimated glomerular filtration rate (eGFR), and allograft loss were examined using adjusted logistic, linear mixed modeling, and cox regression, respectively. Among donors who experienced cardiac arrest, we evaluated the association between duration and unwitnessed status of arrest and DGF. Results. A total of 1173 kidney transplant recipients received DCDD kidneys from 646 donors in Australia between 2014 and 2019. Of these, 335 DCDD had antecedent cardiac arrest. Compared with recipients of kidneys from donors without antecedent cardiac arrest, the adjusted odds ratio (95% confidence interval) for DGF was 0.85 (0.65-1.11) among those with kidneys from donors with cardiac arrest. There was no association between antecedent cardiac arrest and posttransplant eGFR or allograft loss. The duration of cardiac arrest and unwitnessed status were not associated with DGF. Conclusions. This focused analysis in an Australian population showed that the allograft outcomes were similar whether DCDD donors had experienced a prior cardiac arrest, with no associations between duration or unwitnessed status of arrest and risk of DGF. This study thus provides important reassurance to transplant programs and the patients they counsel, to accept kidneys from donors through the DCDD pathway irrespective of a prior cardiac arrest.

Abstract:Introduction Delayed graft function (DGF) is often defined as the need for dialysis treatment in the first week after a kidney transplantation. This definition, though readily applicable, is generic and unable to distinguish between “types” of DGF or time needed to recover function that may also significantly affect longer-term outcomes. We aimed to profile biological pathways in DCD kidney donors that correlate with DGF and different DGF durations. Methods N=30 DCD kidney biopsies were selected from the UK Quality in Organ Donation biobank and stratified according to DGF duration (immediate function, IF n=10; “short-DGF” (1-6 days), SDGF n=10; “long-DGF” (7-22 days), LDGF n=10). Samples were matched for donor and recipient demographics and analyzed by label-free quantitative proteomics, yielding identification of N=3,378 proteins. Results Ingenuity Pathway Analysis on differentially abundant proteins showed that SDGF kidneys presented upregulation of stress response pathways, while LDGF presented impaired response to stress, compared to IF. LDGF showed extensive metabolic deficits compared to IF and SDGF. Conclusion DCD kidneys requiring dialysis only in the first week post-transplant present acute cellular injury at donation, alongside repair pathways upregulation. In contrast, DCD kidneys requiring prolonged dialysis beyond 7 days present minimal metabolic and antioxidant responses, suggesting that current DGF definitions might not be adequate in distinguishing different patterns of injury in donor kidneys contributing to DGF.

Vasopressor selection during critical care management of brain dead organ donors and the effects on kidney graft function

Abstract:Background: Delayed graft function (DGF), the need for dialysis in the first week following kidney transplant, affects approximately one quarter of deceased-donor kidney transplant recipients. Donor demographics, donor serum creatinine, and graft cold ischemia time are associated with DGF. However, there is no consensus on the optimal management of hemodynamic instability in organ donors after brain death (DBDs). Our objective was to determine the relationship between vasopressor selection during donor management and the development of DGF. Methods: Prospective observational data, including demographic and critical care parameters, were collected for all DBDs managed by 17 organ procurement organizations from nine Organ Procurement and Transplantation Network Regions between 2012 and 2018. Recipient outcome data were linked with donor data through donor identification numbers. Donor critical care parameters, including type of vasopressor and doses, were recorded at three standardized time points during donor management. The analysis included only donors who received at least one vasopressor at all three time points. Vasopressor doses were converted to norepinephrine equivalent doses and analyzed as continuous variables. Univariate analyses were conducted to determine the association between donor variables and DGF. Results were adjusted for known predictors of DGF using binary logistic regression. Results: Complete data were available for 5,554 kidney transplant recipients and 2,985 DBDs. On univariate analysis, donor serum creatinine, donor age, donor subtype, kidney donor profile index, graft cold ischemia time, phenylephrine dose, and dopamine dose were associated with DGF. After multivariable analysis, increased donor serum creatinine, donor age, kidney donor profile index, graft cold ischemia time, and phenylephrine dose remained independent predictors of DGF. Conclusion: Higher doses of phenylephrine were an independent predictor of DGF. With the exception of phenylephrine, the selection and dose of vasopressor during donor management did not predict the development of DGF. Level of evidence: Prognostic study, Level III.

Abstract:This paper describes a novel BSN-based integrated system for detecting, monitoring, and securely recording human physical activities using wearable sensors, a personal mobile device, and a Cloud-computing infrastructure supported by the BodyCloud platform. An integration with a smart-wheelchair system is also presented. BSNs are a key enabling technology for the revolution of personal-health services and their integration with Cloud infrastructure can effectively supports the diffusion of such services in our daily life. Many of these personal-health systems - regardless of their final aim - are based, use or are supported by contextual information on user's physical activity (body posture, movement or action) being performed. This work, hence, aims at providing a basic physical activity service that is capable of supporting personal, mobile-Health applications with real-time activity recognition and labeling both on the personal mobile device and on the Cloud.
Severe Primary Graft Dysfunction in Heart Transplant Recipients Using Donor Hearts After Circulatory Death: United States Experience

Abstract:Objective This study compares the incidence of severe Primary Graft Dysfunction (PGD) in a contemporaneous cohort of donors after circulatory death (DCD) and brain death (DBD) heart transplant recipients. Method The United Network for Organ Sharing database was queried for isolated adult heart transplant recipients from 9/2023 to 6/2024. Heart recipients were stratified based on the organ donation type (DCD vs DBD). DCD heart recipients were further categorized based on the procurement method: time between circulatory death to cross-clamp: ≤ 30 minutes (Direct Procurement and Preservation, DPP), >30 minutes (Normothermic Regional Perfusion, NRP). Outcomes of interest included: severe PGD (Left/Bi-Ventricular; LV/BiV) at 24 hours and Severe Graft Dysfunction at 72 hours (patients with severe PGD at 24 hours that remain on mechanical support at 72 hours). Results A total of 2,590 adult heart transplant recipients were identified, of which 17.1% underwent DCD heart transplantation. DCD heart recipients were less likely to be on inotrope (36.7% vs 41.6%, p=0.046) and ECMO (4.1% vs 9.9%, p<0.001) prior to transplant than DBD heart recipients. DCD heart recipients were more likely than DBD heart recipients to develop severe PGD (LV/BiV) at 24 hours (9.5% vs 5.1%, p<0.001). The Severe Graft Dysfunction at 72 hours (2.3% vs 2.9%, p=0.67) and 30-day mortality were similar between the 2 groups. Recipients of DCD heart procured with DPP or NRP had similar severe PGD (LV/BiV) at 24 hours (9.4% vs 9.7%, p=0.93). Conclusion Severe PGD at 24 hours is higher among the DCD than DBD heart recipients, but Graft Dysfunction improves by 72 hours.

Abstract:BACKGROUND:Kidney transplantation is limited by the shortage of donor kidneys. Donation after cardiac death (DCD) has been explored to alleviate this problem. To better understand the outcome of DCD kidney transplantation, we reanalyzed the Mycophenolic Renal Transplant (MORE) Registry.METHODS:We compared delayed graft function (DGF), biopsy-proved acute rejection (BPAR), graft loss, and patient death between DCD and donation after brain death (DBD) kidney transplantations. Recipients were further stratified into depleting and nondepleting induction groups for exploratory analysis.RESULTS:There were 548 patients who received kidney transplants from deceased donor in the MORE Registry. Among them, 133 received grafts from DCD donors and 415 received from DBD donors. The incidence of DGF was 29.4% and 23.5% in the DCD group and the DBD group, respectively (P = .1812), and the incidence of BPAR at 12 months was 9.0% and 9.9% respectively (P = .7713). The 1-year graft loss rate in the DCD group was higher than that in the DBD group (7.5% vs 3.1%, P = .0283), and the 4-year graft loss rate and patient death rate were not significantly different between the 2 groups.CONCLUSIONS:The DCD kidney transplant group had acceptable short-term outcomes and good long-term outcomes as compared with the DBD kidney transplant group.

Abstract:The effect of delayed graft function (DGF) recovery on long-term graft outcome is unclear. The aim of this study was to examine the association of DGF recovery status with long-term outcome. We analyzed 385 recipients who underwent single kidney transplantation from brain-dead donors between 2004 and 2015. Patients were grouped according to renal function at 1 month post-transplantation: control (without DGF); recovered DGF (glomerular filtration rate [GFR] ≥ 30 mL/min/1.73 m2); and incompletely recovered DGF group (GFR < 30 mL/min/1.73 m2). DGF occurred in 104 of 385 (27%) recipients. Of the DGF patients, 70 recovered from DGF and 34 incompletely recovered from DGF. Death-censored graft survival rates for control, recovered DGF, and incompletely recovered DGF groups were 95.3%, 94.7%, and 80.7%, respectively, at 5 years post-transplantation (P = 0.003). Incompletely recovered DGF was an independent risk factor for death-censored graft loss (HR = 3.410, 95%CI, 1.114-10.437). DGF was associated with increased risk for patient death regardless of DGF recovery status. Mean GFRs at 5 years were 65.5 ± 20.8, 62.2 ± 27.0, and 45.8 ± 15.4 mL/min/1.73 m2 for control, recovered, and incompletely recovered DGF groups, respectively (P < 0.001). Control group and recovered DGF patients had similar renal outcomes. However, DGF was associated with increased risk for patient death regardless of DGF recovery status.

Abstract:Background: This study examined 1071 adult primary kidney transplants from the French-controlled donation after the circulatory determination of death (cDCD) program, which uses normothermic regional perfusion (NRP), and involves short cold ischemia times (CIT) and constrained asystole times differing by donor age. Methods: Logistic regression identified risk factors for primary nonfunction (PNF), delayed graft function (DGF), and graft failure. Results: Risk factors for PNF included donor hypertension, admission for ischemic vascular stroke, and HLA DR mismatches. Risk factors for DGF included functional warm ischemia time >40 min, dialysis >2 y, recipient body mass index of 30 kg/m2 or higher, recipient diabetes, and CIT >10 h. Risk factors for 1-y graft failure included donor hypertension, donor lung recovery, ostial calcification, recipient cardiovascular comorbidities, and HLA DR mismatches. A high donor estimated glomerular filtration rate protected against DGF and graft failure at 1-y. After adjustment restricted to recipient and graft factors and donor age, the risks of PNF, DGF, and graft failure increased with donor age up to 65 y and then remained stable. Conclusions: The study suggests that cDCD kidney transplants are highly successful, but also that its outcomes are influenced by lung recovery, poor HLA DR matching, and warm ischemia times differing with donor age. Our study identified several risk factors for kidney transplantation failure after cDCD with systematic use of NRP and some of them seem as modifiable variables associated with cDCD transplant outcome.

Abstract:Introduction: The impact of deceased donor characteristics on kidney transplant outcomes is controversial. Correspondingly, the predictive performance of deceased donor scores remains moderate, and many transplant centers lack validated criteria for graft acceptance decisions. To better dissect donor-related risk from recipient and periprocedural variables, we analyzed outcomes of kidney donor pairs transplanted in different individuals. Methods: This study explored (a)symmetry of early outcomes of 328 cadaveric kidney transplant recipients from 164 donor pairs transplanted at three Eurotransplant centers. The primary discriminatory factor was (a)symmetry of partner graft function, defined as early graft loss or impaired graft function [estimated glomerular filtration rate (eGFR) <30 mL/min] 3 months after transplantation. We reasoned that a relevant impact of donor factors would result in a high concordance rate of limited graft function or failure. Results: The observed number of symmetric graft failure after transplantation was less than statistically expected (3 months: 1 versus 2, p = 0.89; and 12 months: 3 versus 5, p = 0.26). However, we found a trend toward an impaired 5-year graft survival of grafts with good function 3 months after transplantation but a failed or impaired partner graft compared to symmetrically well-functioning grafts (p = 0.09). Subsequently, we explored the impact of individual donor and recipient variables on early transplant outcomes. Generalized estimating equations after feature selection with LassoGEE bootstrap selected donor age, donor body mass index, and donor eGFR as the relevant risk factors. Discussion: Our findings indicate that donor factors impact early outcomes in kidney transplantation but may have a limited role in long-term graft survival, once a graft has been accepted for transplantation. Utilizing donor-based clinical scores has the potential to aid clinicians in acceptance decisions, giving them an estimate of individual posttransplant outcomes. However, the ultimate decision for acceptance should rest with clinicians, who must consider the complex interplay of donor factors, as well as recipient and periprocedural characteristics.
