Abstract:Introduction: Actinomycosis is an uncommon subacute or chronic suppurative bacterial granulomatous infectious disease with clinical heterogeneity. The majority of actinomycosis cases were of extra-abdominal origin, with oro-cervico-facial cases representing 55%, abdominopelvic representing 20%, and thoracic representing 15% of total reports. Currently, abdominal actinomycosis incidence is approximately 1 case per 119,000 people, being found three times more frequently among males. We report two rare clinical presentations of abdominal actinomycosis affecting the mesentery and the retroperitoneum, respectively. Case report 1: A 58-year-old Caucasian male presented to our clinic with abdominal pain in the right upper quadrant. Pre-operative evaluation, although inconclusive, showed a mesocolic mass infiltrating the right and transverse colon. The patient underwent exploratory laparotomy. After partial resection of the mass, the histopathology report demonstrated mesenteric actinomycosis. Case report 2: A 40-year-old Caucasian male presented to our clinic complaining about a mucopurulent material from an orifice at the right inguinal region. After appropriate work-up, a large abdominopelvic, stellate mass (75 x 22.8 mm) in the retroperitoneum was revealed. Surgery along with the appropriate antibiotics was used to treat the patient. Conclusion: Preoperative suspicion and diagnosis of actinomycosis are very challenging, with a high rate of misdiagnosis often resulting in delayed treatment. Our case reports highlight that abdominal actinomycosis should always be part of differential diagnosis, especially when there is involvement of multiple organs. The gold standard treatment of actinomycosis is surgical excision with prolonged antibiotic treatment.
Abdominopelvic Actinomycosis-The Diagnostic and Therapeutic Challenge of the Most Misdiagnosed Disease

Abstract:Abdominopelvic actinomycosis is a rare chronic or subacute bacterial infection caused by Actinomyces israelii, a Gram-positive anaerobic bacterium that normally colonizes the digestive and genital tracts, clinically presented as an inflammatory mass or abscess formation. Methods: We reviewed the medical records of the patients from our clinic with abdominopelvic actinomycosis who underwent surgery between 2002 and 2022. In this period, 28 cases (9 men and 19 women) were treated. The mean age was 43.36 years and they were hospitalized for abdominopelvic tumors or inflammatory tumors in 15 cases and inflammatory disease in 13 cases. Results: Causes of actinomycosis in the studied group were an intra-uterine contraceptive device in 17 cases, foreign bodies in 2 cases, diabetes in 4 cases, stenting of the bile duct in 1 case, and immunodepression. For 6 patients, we performed surgery by open approach and for 21 patients by a laparoscopic approach. For nine patients, abdominopelvic actinomycosis had been mimicking a colon malignancy (cecum and ascending colon, four cases; transverse colon, two cases; and on the greater omentum, three cases) and for six patients, a pelvic tumor (advanced ovarian cancer). After surgery the patients underwent specific treatment with antibiotics, with good results. In two cases we discovered and treated hepatic actinomycosis, one case by a laparoscopic approach and one case by a percutaneous approach. In our lot we noticed three recurrences that required reintervention in patients who had had short-term antibiotics due to non-compliance with treatment out of four such cases. Conclusions: For abdominopelvic malignancies, actinomycosis should be included in the differential diagnosis, as well as for inflammatory bowel diseases and bowel obstructions. We have a wide range of patients considering the rarity of this condition. Long-term antibiotics are necessary to prevent recurrence.
Rheumatoid arthritis complicated with cervical actinomycosis and ureteral obstruction: A case report and literature review

Abstract:Actinomycosis is a rare chronic granulomatous disease characterized by granuloma formation and tissue fibrosis with sinus tracts, often misdiagnosed due to its similarity to many infectious and non-infectious diseases. This report presents a case of a 60-year-old female with more than 10 years history of rheumatoid arthritis who developed actinomycosis infection after long-term treatment with immunosuppressants and biologics, including methotrexate, leflunomide, and infliximab, leading to recurrent joint pain, poorly controlled rheumatoid arthritis activity, and persistent elevation of white blood cell counts. Abdominal CT revealed a pelvic mass and right ureteral dilation. Pathological examination of cervical tissue showed significant neutrophil infiltration and sulfur granules, indicating actinomycosis. The patient received 18 months of doxycycline treatment for the infection and continued rheumatoid arthritis therapy with leflunomide, hydroxychloroquine sulfate, and tofacitinib, resulting in improved joint symptoms and normalized white blood cell counts. After 2 years of follow-up, the patient remained stable with no recurrence. This case highlights the importance of clinicians being vigilant for infections, particularly chronic, occult infections from rare pathogens, in rheumatoid arthritis patients on potent immunosuppressants and biologics, advocating for early screening and diagnosis.
Actinomycosis as a rare cause of acute appendicitis: Case report and comprehensive literature review

Abstract:Introduction and importance: Actinomycosis is a rare, chronic bacterial infection caused by Actinomyces species. While it commonly affects the cervicofacial region, thorax, and abdomen, appendicular involvement is extremely uncommon. This case report details the presentation, diagnosis, and management of a 45-year-old female patient with acute appendicitis secondary to actinomycosis. Case presentation: A 45-year-old woman, with an 8-year intrauterine device history, presented with three-day right iliac fossa pain, vomiting, and stable vital signs. Laboratory tests revealed an inflammatory syndrome. Suspecting acute appendicitis, a CT scan confirmed a swollen 10 mm appendix. Laparoscopic surgery revealed a phlegmonous appendix, leading to an uncomplicated appendectomy. Pathological examination confirmed actinomycotic granules, supporting the diagnosis of actinomycosis appendicitis. The patient received 18 million units of intravenous penicillin G daily for 6 weeks followed by a 6-month course of oral amoxicillin (1 g three times daily) thereafter, showing favorable progression with no symptoms. Normal clinical and ultrasound follow-ups were observed at one year. Clinical discussion: Appendiceal actinomycosis is a rare condition. Women, especially those with intrauterine contraceptives, experience an increase in cases due to chronic inflammation. Typically underdiagnosed, actinomycosis mimics other conditions, presenting with nonspecific symptoms. Laboratory results offer limited assistance, and histological confirmation is crucial. Histopathological examination is mandatory for diagnosis confirmation. Management involves surgical resection and prolonged penicillin-based antibiotics, providing a favorable prognosis with low mortality. Conclusion: This case underscores the importance of considering rare etiologies, such as actinomycosis, in the differential diagnosis of appendicitis. Timely recognition and management are crucial for optimal patient outcomes.

Abstract:Pulmonary actinomycosis is a rare infection caused by the bacterial species actinomyces. This paper aims to provide a comprehensive review of pulmonary actinomycosis to improve awareness and knowledge. The literature was analysed using databases including Pubmed, Medline and Embase from 1974 to 2021. After inclusion and exclusion, a total of 142 papers were reviewed. Pulmonary actinomycosis is a rare disease occurring in approximately 1 per 3,000,000 people annually. Historically, pulmonary actinomycosis was a common infection with high mortality; however, the infection has become rarer since the widespread use of penicillins. Actinomycosis is known as "the great masquerade"; however, it can be differentiated from other diseases with acid-fast negative ray-like bacilli and sulphur granules being pathognomonic. Complications of the infection include empyema, endocarditis, pericarditis, pericardial effusion, and sepsis. The mainstay of treatment is prolonged antibiotic therapy, with adjuvant surgery in severe cases. Future research should focus on multiple areas, including the potential risk secondary to immunosuppression from newer immunotherapies, the utility of newer diagnostic techniques and ongoing surveillance post-therapy.
Case Series of Abdominal Actinomycosis: An Old Diagnostic Conundrum

Abstract:Abdominal actinomycosis is a rare disease caused by the Actinomyces bacteria. We report a case series of two similar cases with a mismatch between the initial differential diagnosis, the radiological findings, the surgical findings, and the actual histological result. The first case is a 25-year-old woman with a month's history of right-sided abdominal pain. The diagnostic workup including computed tomography (CT) showed possible acute appendicitis, in addition to ascending colon looking abnormal with inflammatory changes and adjacent peritoneal nodules. A diagnostic laparoscopy revealed a normal-looking appendix and a hard mass involving the ascending colon potentially of inflammatory or malignant origin. A laparoscopic right hemicolectomy was performed. Surprisingly, the histology concluded actinomycosis. The second case is a 38-year-old woman with a six-week history of lower abdominal pain and right iliac fossa (RIF) swelling. The diagnostic workup including a CT scan showed a complex mass suggesting complicated appendicitis, pelvic inflammatory disease, or neoplastic process. Furthermore, magnetic resonance imaging (MRI) of the pelvis showed complex bilateral cystic lesions in both iliac fossas with an abscess formation within the subcutaneous fat overlying the lower pelvis. The patient underwent an ultrasound (US)-guided drainage of the subcutaneous pelvic collection and a diagnostic laparoscopy that showed an inflamed mass with dense adhesions to the anterior abdominal wall. Intraoperative biopsies were taken to confirm the diagnosis, which ultimately confirmed severe pelvic actinomycosis. Both patients were discharged home and received extended antibiotic treatment according to microbiology guidelines with good outcomes. Both cases are interesting because there was a mismatch between the initial differential diagnosis, the radiological findings, the intraoperative findings, and the final histology. Reported cases in the literature highlight the diagnostic challenges of abdominal actinomycosis, as it often presents with atypical symptoms and imaging findings sometimes mimic malignancy leading to surgical intervention.
CA 125 and carcinoembryonic antigen levels in cyst fluid, ascites and serum of patients with ovarian neoplasms.

Abstract:The levels of CA 125 and carcinoembryonic antigen (CEA) in tumour cyst fluid, ascites and serum of patients with epithelial ovarian neoplasms were estimated. In addition, the serum concentration of both antigens was evaluated 10 days after operation. Independently of histological type of ovarian tumour, CA 125 and CEA values were significantly higher in cyst and ascitic fluid than in corresponding patients' serum. The higher values of both markers were also found in malignant than benign ovarian cysts. CEA occurred at higher concentrations in mucinous carcinomas whereas CA 125 values were comparable both in mucinous and serous cyst fluid. The levels of these markers in cyst fluid did not correlate with the serum concentrations. The significant individual differences existed in the ability of tumour cells to produce and release both studied antigens. Post-operative CEA levels returned to normal more slow than CA 125 values.

Abstract:Phenomenology is one of several qualitative research traditions. Undergraduate and graduate nursing students have sought to understand the differences between Husserl’s descriptive and Heidegger’s interpretive phenomenology. This article is a basic resource for nursing students that describes and interprets the differences between the two philosophical phenomenological schools of thought. The origin of phenomenology is presented. A descriptive and an interpretive article from two peer reviewed nursing journals are compared and contrasted based on their purpose, data collection and data analysis. The selected articles were chosen based on their topic of relevance related to nursing students in educational settings.
Invasive Ureteral Aspergillosis Causing an Abscess: A Report of a Rare Case

Abstract:Aspergillosis is a vicious fungal infection that can develop in immunosuppressed patients. The presence of aspergillosis in the ureter or elsewhere in the genito-urinary tract is highly uncommon and rarely reported in the literature. Here, we present a 54-year-old gentleman, with uncontrolled diabetes, who presented with urosepsis. Right hydronephrosis and ureteric stricture with urinary extravasation were seen on imaging. Right percutaneous nephrostomy was done, with drainage and analysis of the pus revealing the growth of Aspergillus fumigatus species. On open exploration, a ureteric abscess cavity, which was adherent to the duodenum, was drained and uretero-ureterostomy along with feeding jejunostomy was performed. Histopathological examination and special staining confirmed the growth of aspergillosis. The patient was treated with antifungal agents and responded well with an uneventful post-operative recovery.

Abstract:Background/Objectives: The aim of this study is the presentation of an endoscopic therapeutic approach for three patients with a solitary kidney who were diagnosed with urothelial cancer of the upper tract. Methods: This retrospective analysis included patients with solitary kidneys who suffered from high-grade UTUC (urothelial cancer of the upper urinary tract) and underwent conservative treatment. Results: The first patient was a 67-year-old male who had a prior history of a nephroureterectomy due to UTUC six years ago. The patient was diagnosed with high-grade UTUC in the contralateral kidney. The tumor has been managed with endoscopic ablation. The second patient was a 74-year-old male with a non-functional kidney and high-grade UTUC diagnosed in the contralateral side. The patient underwent endoscopic ablation for the tumor. The third case was a 68-year-old female patient who had a history of a nephroureterectomy due to UTUC. Afterward, she was diagnosed with high-grade UTUC in the contralateral kidney. The patient was treated with percutaneous tumor resection and the placement of a nephrostomy tube. The first patient was included in an immunotherapy program based on an oncologist consultation after laser ablation treatment for Ta high-grade UTUC, followed by the endoscopic management of two recurrences. Afterward, no recurrence was detected. The remaining two patients followed up without the detection of a new recurrence. Conclusions: The kidney-sparing approach (tumor laser ablation or resection) for high-risk UTUC treatment in selected patients with solitary kidneys seems to provide adequate early outcomes in relation to preserving renal function and effective disease management. It is important to personalize the way of treatment in every case after a thorough examination of the patient's data.

Nontuberculous Mycobacterium Infection in Renal Transplant Recipients: a Systematic Review.

Abstract:BACKGROUND:Nontuberculous mycobacterium (NTM) infections are important complications in renal transplant recipients.METHODS:Cases of NTM diseases post-renal transplantation published in English were identified in the PubMed database. The clinical and laboratory characteristics of these cases were reviewed.RESULTS:One-hundred and fifteen cases of NTM infection in renal transplant recipients in 100 articles were identified. The median time of NTM infection from renal transplantation was 37 months (range, 3 d to 252 months). Disseminated disease (40.0%) was the most common manifestation of NTM infection in renal transplant recipients. The main diagnostic procedure was culture combined with histopathological examination (38.3%). Thirteen NTM species were identified; the most common pathogen was Mycobacterium chelonae (19.1%). The anti-Mycobacterium treatment lasted more than 12 months in 43.3% of the recorded patients. During the treatment, 30.2% of the recipients lost kidney graft function and 20.9% died from NTM infection and/or other complications, including cardiac death and respiratory failure.CONCLUSIONS:NTM infection is easily neglected. Physicians must keep a high suspicion for NTM infections in renal transplant recipients.
Colonization of Soybean Cyst Nematode Females, Cysts, and Gelatinous Matrices by the Fungus Verticillium lecanii.

Abstract:Heterodera glycines was grown in monoxenic culture on soybean roots and then inoculated with the antagonistic fungus Verticillium lecanii. Use of root explant cultures allowed evaluation of the fungus-nematode interaction with the nematode attached to roots or removed from the host, and avoided contamination with other fungi. From 16 hours to 14 days following inoculation, female and cyst samples were examined with the light microscope, or prepared for either conventional or low-temperature scanning electron microscopy. Within 16 hours, hyphae had begun colonizing the gelatinous matrices (GM). The fungus proliferated in the GM of some specimens within a week, but was rarely seen in unhatched eggs. Fungus penetration holes in female and cyst walls were observed 3 days after inoculation; penetration through nematode orifices was not seen at that time. More cysts than females were colonized at the earliest sampling dates. Specimens associated with external hyphae exhibited variable internal colonization, ranging from no fungal penetration to extensive mycelial growth.
A Rare Case and Literature Review of Pyelo-Hepatic Abscess in an Immunocompetent Patient: When Effective Source Control and Targeted Antimicrobial Therapy Might Not Be Enough

Abstract:Pyelo-hepatic abscess is a rare complication of upper urinary tract infections (UTIs). We describe a case of polymicrobial pyelo-hepatic abscess in an immunocompetent patient. A 71-year-old male patient with a double-J stent for right ureteral lithiasis was admitted in our Infectious Diseases Department for a pyelo-hepatic abscess. Despite a targeted antibiotic therapy against an extended spectrum betalactamase-negative Escherichia coli, the patient did not improve. Further examinations revealed a possible polymicrobial aetiology, including Candida spp. and E. coli resistant to piperacillin/tazobactam but sensitive to third-generation cephalosporins. To date, a paucity of articles regarding pyelo-hepatic abscess exist, consisting mostly of case reports. Urinary stones and a ureteral stent indwelling time exceeding 90 days are known risk factors for upper UTIs and for bacterial dissemination in contiguous organs. Pyelo-hepatic abscesses usually involve Gram-negative bacilli, but they can be polymicrobial, including fungi. As a range of factors could limit the efficacy of antibiotics inside an encapsulated lesion and might contribute to the selection of resistant species during treatment, clinicians should be aware of this complication and try to prevent this event by acting on the main modifiable risk factor.

Abstract:Abdominal actinomycosis, a rare and often misdiagnosed condition caused by Actinomyces israelii, typically a commensal organism in the oral cavity and gastrointestinal tract, can become pathogenic, leading to chronic granulomatous infections that mimic various abdominal pathologies, including malignancies. We present a case of a 59-year-old male with coronary artery disease and type 2 diabetes who presented with severe abdominal pain, initially diagnosed as acute appendicitis. During exploratory laparotomy, an ileocecal band mimicking a congenital anomaly was discovered. Histopathological examination confirmed abdominal actinomycosis, revealing clusters of Actinomyces bacteria surrounded by acute inflammatory cells. The patient was successfully treated with surgical intervention and prolonged penicillin therapy, with no recurrence during a four-month follow-up. This case highlights the diagnostic challenges posed by abdominal actinomycosis and emphasizes the importance of considering it in the differential diagnosis of abdominal masses and appendicitis-like symptoms.
[A clinical analysis of 9 cases of actinomycosis]

Abstract:Objective: To study the clinical features of actinomycosis and to increase the understanding of it. Methods: To review the clinical data of 9 cases of actinomycosis retrospectively in Peking Union Medical College Hospital from January 1990 to June 2006. Results: (1) Nine patients including 7 female and 2 male, with age ranging from 30 to 50 years (mean age 39.8 years) were studied and they suffered from the disease for a mean period of 13.1 months (2 - 36 months). (2) Three of the 9 patients had cervicofacial actinomycosis, 2 pelvic actinomycosis, 2 thoracic actinomycosis, 1 breast and 1 intestinal actinomycosis. In 3 cases the lesion spread to many other organs from the primary site. (3) The diagnosis was made with biopsy in 6 cases, sputum or sulphur granules in 2 and clinical as well as laboratory examination in one. (4) All the 9 patients accepted appropriate therapy of antibiotics and the treatment was effective in 8. The one failed to respond to antibiotics died. Conclusion: Actinomycosis is infrequent clinically. It usually presents as chronic abscess and may be misdiagnosed as malignant tumor. Clinical doctors should be vigilant of it, in order to diagnose and treat it in good season and to greatly improve the prognosis.
Ultrastructural morphometry and the control of lymphocyte nuclear size in non-Hodgkin lymphoma: a review.

Abstract:Morphologic features, particularly nuclear characteristics, continue to be a major factor in the classification of the subtypes of non-Hodgkin lymphoma (NHL). Information is limited or nonexistent, however, concerning the mechanisms responsible for nuclear appearance. This is particularly true for nuclear size. In determining which of the nuclear compartments--condensed chromatin or the interchromatinic (euchromatin or nuclear matrix) region--is responsible for nuclear size, ultrastructural morphometric analysis has been a useful tool. Linear regression analysis has been used to compare, in turn, condensed chromatin area (CCA) and interchromatinic area (ICA) against nuclear area (NA). Whether in normal or neoplastic lymphocytes, correlation coefficients are moderately good when CCA is matched against NA, but CCA is only a weak predictor of NA. In contrast, correlation coefficients are remarkably high for ICA compared to NA, and ICA predicts NA to the degree of 94% to 97%. This holds true for all NHLs tested and for reactive hyperplasias. Despite abnormalities in lymphocyte nuclear size in NHL, the size of any nucleus is controlled by the amount of interchromatinic material to an exceptional degree. Even so, the mechanisms responsible for major changes in nuclear volume during transformation of normal lymphocytes are apparently altered in NHL. Morphometric analysis, ultrastructurally, can assist in understanding, testing, and defining diagnostic morphologic criteria in this disease.
Pediatric appendicular actinomycosis: a case report and literature review

Abstract:Background: Actinomycosis (ACM) is a rare infectious granulomatous disease caused by Actinomyces, a Grampositive, filamentous, saprophytic bacteria. There are several types of pediatric ACM, such as orocervicofacial (55%) and other less common forms: abdominopelvic and thoracic. We report a case of a 16-year-old who presented with abdominal ACM in the setting of acute appendicitis. After the case report, we provide a short literature review of pediatric appendicular ACM cases published. Case: A 16-year-old boy presented with nausea, vomiting, pain in the upper part of the abdomen and fever (37.5°C) lasting for 24 hours. On physical examination, the patient`s epigastrium and lower right abdominal quadrant were tender. White cell count and C-reactive protein (CRP) were elevated at 16,300/μL and 48.6mg/L respectively. Ultrasonography (US) showed appendicolith and edema of the appendiceal wall, focally with stratification as well as periappendiceal inflammation. The patient underwent a classic appendectomy, and the postoperative course was without complications. Histopathological analysis showed diffuse transmural neutrophilic infiltration of the appendix, focally with areas of necrosis and abscesses. There were numerous brightly eosinophilic colonies made of filamentous bacteria, located predominantly in submucosa. Special stains Grocott-Gomori`s Methenamine Silver and Gram were positive and a diagnosis of ACM was made. Conclusions: Although appendicitis is very common in the general population, appendicitis associated with ACM is very rare, accounting for 0.02% - 0.06%, especially in the pediatric population. Diagnosis can be very challenging because they usually present with non-specific symptoms, and can form masses that mimic malignancies. Although rare, clinicians and pathologists should be aware of this entity. Satisfactory results and complete cure are achieved with adequate antibiotic therapy and surgery. In most cases, if there are no associated diseases, early and accurate diagnosis ensure an excellent prognosis.

Abstract:Various infectious and inflammatory diseases affect the genitourinary system. This paper provides a review of multiple common and uncommon infectious and inflammatory conditions affecting the genitourinary system and some associated complications. These include acute infectious cystitis, emphysematous cystitis, acute pyelonephritis, emphysematous pyelonephritis, renal and perinephric abscesses, pyonephrosis, xanthogranulomatous pyelonephritis, epididymo-orchitis, vasitis, prostatitis, pelvic inflammatory disease, renal hydatid infection, renal tuberculosis, actinomycosis, Erdheim-Chester Disease, IgG4-Related Kidney Disease, urethritis and urethral strictures, ureteritis cystica, and genitourinary fistulas. Radiologists should be aware of these diseases' complications and management. Uncommon conditions must be considered when evaluating the genitourinary system.
