Abstract:Abstract Exposure to patients who have experienced major traumatic events places burn therapists at risk of developing high levels of anxiety and stress, directly impacting their quality of life. Poor professional quality of life has been associated with negative physical and emotional characteristics in healthcare professionals and may subsequently influence patient safety and satisfaction, job retention, and productivity. The purpose of this study was to understand and describe the experiences of occupational and physical therapists who provide acute therapy to people who have sustained burn injuries. Semistructured interviews were completed with 10 participants (8 females and 7 physical therapists) for this qualitative descriptive study. Conventional content analysis was used to analyze the data collected from semistructured interviews. Six themes emerged after content analysis: (1) the importance of therapeutic relationships to patient successes and therapists’ retention; (2) the autonomy and flexibility of burn therapists; (3) the impact of career longevity on compassion; (4) the uniqueness of burn team relationships and camaraderie; (5) the challenges of operating within the business of a healthcare system; and (6) the physical, mental, and emotional challenges to the burn therapists’ resiliency. This research brings to light the physical, mental, and emotional challenges that impact burn therapists’ resiliency, and which should be considered when creating programs to increase retention and professional quality of life in this professional population.

Abstract:Background: Although burnout is not an F‑Diagnosis in the ICD-11, many people are suffering from it. This has many economic and health-related consequences, because burnout is closely tied to an inability to work and significant secondary illnesses. This article provides an umbrella review of therapy approaches for burnout that have already been used and evaluated and shows in which areas there are promising, but still little researched, therapeutic support options for those affected. Methods: A systematic literature search of studies was carried out in PubMed and Google Scholar between 2010 and 2024. The search terms were "burnout," "therapy," and "intervention" combined with "systematic review" and "meta-analysis." Overall, 10 systematic reviews or meta-analyses met the inclusion criteria. Results: In five systematic reviews or meta-analyses, the therapy approaches focused on a specific professional group (nurses, doctors, and teachers); the others related to students or various groups of people. In seven studies, the effects of mindfulness-based stress reduction on burnout were shown. The effects of cognitive behavioral therapy (CBT) were shown in four studies. Acceptance and commitment-therapy (ACT) as well as rational emotive therapy (RET) also proved to be effective. Conclusion: It still needs to be researched whether established thinking and behavioral patterns can be changed through the therapies discussed. The professional context should be given more attention. Some resource-oriented approaches like optimism, a sense of control, self-efficacy, and self-acceptance seem promising in the treatment of burnout.

Abstract:Abstract Introduction Recovery from a severe burn injury requires early and aggressive therapy, which is often painful and distressing to the patient. Burn therapists who guide these interventions may be prone to experiencing secondary trauma and compassion fatigue through repeated exposure to difficult situations. At the same time, therapists may gain a sense of purpose and altruism from their work, fostering compassion satisfaction. Despite being well documented in other health professions, compassion fatigue and compassion satisfaction have not been sufficiently explored among burn therapists. The purpose of this study was to quantify and characterize burn therapist’s compassion fatigue and satisfaction using the Professional Quality of Life (ProQOL) scale and to answer the question of what job-related factors may impact these outcomes. Methods This cross-sectional study surveyed active burn therapists using Research Electronic Data Capture (REDCap). ProQOL scores were calculated for individual respondents and described for the sample using mean, standard deviation, and range. Multiple regression analyses were conducted to assess the impact of predictor variables of years in burn care, practice setting, and population on the subscales of the ProQOL for compassion satisfaction and compassion fatigue (burnout and secondary traumatic stress). Results A total of 143 burn therapists were included, which exceeded the n needed to achieve statistical power. The included participants presented with low to moderate burnout scores (mean=23.5, range=11-38), secondary traumatic stress (22.5, 11-40), and moderate to high compassion satisfaction (40.4, 25-50). Regression analyses found years of practice significantly predicted (p ≤ .01) each of the compassion constructs. Conclusions This study presents new knowledge for the field of burn care in its quantification of professional quality of life in burn therapists. While burn therapists experience moderate-to-high levels of compassion satisfaction in their work, burnout and stress also approached moderate levels. Years of practice was a significant predictor of compassion and may be related to increased competence or confidence in practice. Applicability of Research to Practice This study is the first to study compassion fatigue and satisfaction in burn therapists. Understanding and addressing these issues are essential to recruiting and retaining a healthy and productive burn care workforce. Regular assessment of professional quality of life and career stage-specific supportive measures could provide a protective measure against compassion fatigue.

Clinician Burnout and Effectiveness of Guideline-Recommended Psychotherapies

Abstract:Importance Clinician burnout has been associated with clinician outcomes, but the association with patient outcomes remains unclear. Objective To evaluate the association between clinician burnout and the outcomes of patients receiving of guideline-recommended trauma-focused psychotherapies for posttraumatic stress disorder (PTSD). Design, Setting, and Participants This cohort study was set at the US Veterans Affairs Health Care System and included licensed therapists who provided trauma-focused psychotherapies and responded to an online survey between May 2 and October 8, 2019, and their patients who initiated a trauma-focused therapy during the following year. Patient data were collected through December 31, 2020. Data were analyzed from May to September 2023. Exposures Therapists completing the survey reported burnout with a 5-point validated measure taken from the Physician Worklife Study. Burnout was defined as scores of 3 or more. Main Outcomes and Measures The primary outcome was patients' clinically meaningful improvement in PTSD symptoms according to the PTSD Checklist for the Diagnostic and Statistical Manual of Mental Disorders ( Fifth Edition ). Patient dropout, therapist adherence, and session spacing was assessed through electronic health records. Multivariable random-effects logistic regression examined the association of therapist burnout and clinically meaningful improvement, adjusted for case-mix. Results In this study, 165 of 180 (91.7%) therapists (89 [53.9%] female) completed the burnout measure and provided trauma-focused psychotherapies to 1268 patients (961 [75.8%] male) with outcome data. Fifty-eight (35.2%) therapists endorsed burnout. One third of patients (431 [34.0%]) met criterion for clinically meaningful improvement. Clinically meaningful improvement in PTSD symptoms was experienced by 120 (28.3%) of the 424 patients seen by therapists who reported burnout and 311 (36.8%) of the 844 patients seen by therapists without burnout. Burnout was associated with lower odds of clinically meaningful improvement (adjusted odds ratio [OR],0.63; 95% CI, 0.48-0.85). The odds of clinically meaningful improvement were reduced for patients who dropped out (OR, 0.15; 95% CI, 0.11-0.20) and had greater session spacing (OR, 0.80; 95% CI, 0.70-0.92). Therapist adherence was not associated with therapy effectiveness. Adjusting for dropout or session spacing did not meaningfully alter the magnitude of the association between burnout and clinically meaningful improvement. Conclusions and Relevance In this prospective cohort study, therapist burnout was associated with reduced effectiveness of trauma-focused psychotherapies. Studying when and how burnout affects patient outcomes may inform workplace interventions.

A Qualitative Analysis of Job Burnout in Eating Disorder Treatment Providers

Abstract:Although job burnout is common in mental health care settings, almost no research has examined burnout in eating disorder treatment providers. Using qualitative methodology, this study examined a) perceived contributors of burnout, b) efforts to manage burnout, and c) recommendations for avoiding burnout in a sample of professional eating disorder treatment providers. Recruited via professional organizations, 298 participants completed an online questionnaire designed by the authors. Qualitative responses were coded and grouped into themes. Results indicated that almost all participants worried about their patients' health, which frequently resulted in negative affect (e.g., anxiety, sadness). The most frequently cited contributors to burnout were common characteristics of eating pathology (e.g., chronicity, relapse, symptom severity); patient characteristics (e.g., personality conflict); work-related factors (e.g., time demands); and, financial issues (e.g., inadequate compensation). To avoid burnout, over 90% of participants engaged in self-care behaviors (e.g., exercise, social support). Early-career practitioners were encouraged to utilize supervision, create a work/life balance, engage in self-care, and limit caseloads. These results suggest that supervision and training of eating disorder treatment providers should include burnout management.

Abstract:Objective: Clinical psychologists, as professionals, come across various people and their problems frequently. Regular exposure to information which are distressing, stressful, and sometimes traumatic can create health issues and burnout, which is a psychological syndrome of emotional exhaustion, depersonalization, and reduced personal accomplishment, in them. A number of psychological variables can contribute to the burnout of clinical psychologists. The ones taken in this study are personality, empathy, countertransference (CT) and compassion fatigue. The study had been conceptualized to find the relation and contribution of the mentioned variables on burnout of clinical psychologists. Methods: Fifty one clinical psychologists participated in the study. Maslach Burnout Inventory, NEO Five-Factor Inventory, Questionnaire of Cognitive and Affective Empathy, Assessment of CT Scale and Compassion fatigue (CF), and satisfaction self-test for helpers were used for assessing the constructs. Relevant statistical analysis were performed. Results: Findings show that there are significant correlations between personality variables, i.e., neuroticism, agreeableness, and conscientiousness with other variables, i.e., compassion fatigue, empathy, and CT. Some of the personality variables are correlated to burnout. Empathy, CT, and CF contribute significantly to burnout. Conclusion: The study shows the probable indicators which can cause burnout in clinical psychologists, and which, if correctly addressed, may help in the maintenance of sound mental health among professionals.
A crisis of exhaustion in the game of escalation: a qualitative exploration of the consequences of occupational burnout among addiction therapists

Abstract:Purpose: Research shows that occupational burnout can affect health, the quality of personal relationships, and levels of job satisfaction and engagement. At the same time, the impact of individual burnout at the group and organisational levels has tended to be neglected. We aim to provide theoretical insights into the multidimensionality of burnout consequences at the individual, interpersonal and societal levels. Methods: A theory-driven, computer-assisted qualitative data analysis was conducted, comprising a thematic analysis of 40 semi- structured telephone interviews with therapists working in alcohol treatment facilities in Poland. Maximum variation sampling was used to ensure the representation of participants with different characteristics. Results: To theorise the implications of the collected data, the different viewpoints of addiction therapists on burnout and its consequences were interpreted through the lens of Rosa's resonance theory. Four interrelated sets of consequences were identified in the data: they related to (a) the therapists themselves, (b) their patients and the therapeutic process, and - in a broader sense - (c) the therapeutic team and (d) the treatment facility. Conclusions: Occupational burnout in individual therapists has serious implications for their patients and colleagues. It can also lead to a reduction in the quality and ultimately the effectiveness of the treatment of alcohol use disorders leading to a negative social image of the treatment facility and thus creating a further barrier to treatment for people with alcohol-related problems. Furthermore, the complexity of the individual experience of occupational burnout and a cause-and-effect chain forms a loop, deepening the severity of its consequences.
"Anxiety, COVID, Burnout and Now Depression": a Qualitative Study of Primary Care Clinicians' Perceptions of Burnout

Abstract:Background: Clinician burnout has become a major issue in the USA, contributing to increased mental health challenges and problems with quality of care, productivity, and retention. Objective: The objective of this study was to understand primary care clinicians' perspectives on burnout during the COVID-19 pandemic as well as their perspectives on the causes of burnout and strategies to improve clinician well-being. Approach: This qualitative research involved in-depth interviews with 27 primary care clinicians practicing in a range of settings across the USA. Semi-structured interviews lasted between 60 and 90 min and were conducted using Zoom video conferencing software between July 2021 and February 2023. Transcripts were analyzed in NVivo software using multiple cycles of coding. Key results: Clinicians shared their experiences with burnout and mental health challenges during the COVID-19 pandemic. Contributors to burnout included high levels of documentation, inefficiencies of electronic health record (EHR) systems, high patient volume, staffing shortages, and expectations for responding to patient emails and telephone calls. The majority of participants described the need to work after clinic hours to complete documentation. Many clinicians also discussed the need for health system leaders to make sincere efforts to enhance work-life balance and create a culture of health and well-being for health professionals. Suggested strategies to address these issues included supportive leadership, accessible mental health services, and additional administrative time to complete documentation. Conclusions: The results of this study provide an in-depth view of participating primary care clinicians' experiences and perceptions of burnout and other mental health challenges. These viewpoints can improve awareness of the issues and strategies to improve the health and well-being of our clinician workforce. Strategies include aligning payment models with the best approaches for delivering quality patient care, reducing administrative burden related to documentation, and redesigning EHR systems with a human factors approach.
Burnout among trauma social workers: The contribution of personal and environmental resources

Abstract:Summary In recent years, there has been increasing research interest in the negative consequences for therapists of working with trauma victims. In that context, burnout is a major concept used in the literature. One of the main questions examined in the literature relates to the factors that contribute to burnout. This question is particularly relevant to therapists working with trauma victims, but has hardly been examined among that population. To fill this gap, the present study sought to examine burnout among therapists working with trauma victims. In addition, we examined the contribution of the therapists’ background characteristics as well as the contribution of their personal resources (mastery, self-esteem, and role competence), and environmental resources (social and colleagues support). The study was conducted among a sample of 214 social workers who work with trauma victims at social services in Israel. Findings The research findings indicate that the participants’ levels of burnout were average. In addition, the therapist’s age, past exposure to trauma, self-esteem, and mastery, as well as influence (as a component of role competence) contributed significantly to burnout. Applications The study sheds light on the negative consequences of therapeutic work for therapists, and on the factors that lead to those consequences. The findings indicate that the impact of the therapists’ personal resources was more significant than that of environmental resources.

Abstract:Background Physiatry is a specialty with high rates of burnout. Although organizational strategies to combat burnout are key, it is also important to understand strategies that individual physiatrists can use to address burnout. Objective To identify changes that resulted in improvement of occupational well‐being of physiatrists over a 6‐ to 9‐month period. Design We employed two quantitative surveys spaced 6 to 9 months apart to identify physiatrists who experienced meaningful improvement in occupational burnout and/or professional fulfillment between the two survey timepoints. These physiatrists were subsequently recruited to participate in a qualitative study using semi‐structured interviews to identify changes that respondents felt contributed to improvements in burnout and professional fulfillment. Setting Online surveys and interviews. Participants Physiatrists in the American Academy of Physical Medicine and Rehabilitation (AAPM&R) Membership Masterfile. Main Outcome Measure Burnout and professional fulfillment were assessed using the Stanford Professional Fulfillment Index. Results One hundred twelve physiatrists responded to the baseline and follow‐up surveys. Of these, 35 were eligible for interviews based on improvements in the Stanford Professional Fulfillment Index, and 23 (64%) agreed to participate. Themes from the qualitative interviews highlighted the importance of personal lifestyle choices, approaches to improve professional satisfaction, and strategies to foster work‐life harmony. Personal lifestyle strategies included investing in wellness and mental health. Efforts to improve professional satisfaction included decreasing work intensity, prioritizing meaningful aspects of work, and building relationships with colleagues. Fostering work‐life harmony also included making trade‐offs in both domains, setting boundaries at work, setting expectations at home, and overcoming personal challenges. Conclusion Our findings illustrate that, in addition to organizational strategies demonstrated to be effective, there are actions that individual physiatrists can take to recover from burnout and foster professional fulfillment.

Abstract:The experience of burnout has been the focus of much research during the past few decades. Measures have been developed, as have various theoretical models, and research studies from many countries have contributed to a better understanding of the causes and consequences of this occupationally-specific dysphoria. The majority of this work has focused on human service occupations, and particularly health care. Research on the burnout experience for psychiatrists mirrors much of the broader literature, in terms of both sources and outcomes of burnout. But it has also identified some of the unique stressors that mental health professionals face when they are dealing with especially difficult or violent clients. Current issues of particular relevance for psychiatry include the links between burnout and mental illness, the attempts to redefine burnout as simply exhaustion, and the relative dearth of evaluative research on potential interventions to treat and/or prevent burnout. Given that the treatment goal for burnout is usually to enable people to return to their job, and to be successful in their work, psychiatry could make an important contribution by identifying the treatment strategies that would be most effective in achieving that goal.

Burnout amongst clinical and counselling psychologist: The role of early maladaptive schemas and coping modes as vulnerability factors

Abstract:Psychologists are subject to multiple competing emotional demands that increase the risk of burnout. Research has demonstrated that burnout arises from both organizational and personal factors, including psychologists' personal beliefs and coping. Preliminary research indicates that early maladaptive schemas (EMS) are associated with high burnout, yet, to date, the role of EMS and associated coping responses (maladaptive coping modes [MCM]) in predicting high burnout amongst psychologists has not been investigated. Four hundred forty-three psychologists completed a self-report online questionnaire comprising the Maslach Burnout Inventory-emotional exhaustion scale (EE), Young Schema Questionnaire, and Schema Mode Inventory. The two most common EMS amongst psychologists were unrelenting standards and self-sacrifice. There was substantial indication of burnout, with 18.3% in the high range and 29.6% in the moderate range of EE. The most common MCM were detached protector and detached self-soother. Controlling for demographics and job demands, EMS accounted for an additional 18% variance in EE. MCM accounted for an additional 6% beyond the variance explained by demographics, job demands, and EMS. Practical recommendations are suggested to reduce psychologist burnout.
A Scoping Review of Compassion Fatigue and Compassion Satisfaction in Burn Therapists

Abstract:Abstract Occupational and physical therapists who routinely provide potentially painful and distressing rehabilitation to burn survivors are at risk for developing compassion fatigue. Burn therapists may also experience compassion satisfaction based on the successes their patients accomplish. The purpose of this review was to synthesize the literature related to compassion fatigue and satisfaction in therapists who work with burn survivors, explore previously reported predictors and mediators of (and responses to) these constructs, and to identify gaps in the literature. Original, peer-reviewed papers published from journal inception to May 2023 were obtained from five electronic bibliographic databases: CINAHL, Embase, PsycINFO, PubMed, and Web of Science. Studies were eligible if they a) reported compassion fatigue or its related concepts and b) focused on occupational or physical therapists who work with burn survivors directly or as a member of a multidisciplinary burn treatment team. The initial search revealed 284 articles, two of which met inclusion criteria and are summarized in this scoping review. Neither article focused solely on burn therapists but examined the experiences of multidisciplinary burn team members working in single burn centers in North America. Both studies used a cross-sectional survey-based design to assess concepts related to compassion fatigue and compassion satisfaction. This scoping review uncovered important gaps in the literature related to compassion fatigue or satisfaction in burn therapists. More research is needed to better understand the interplay between concepts as well as their relationship to burn therapists’ outcomes.

Sources of Clinician Burnout in Providing Care for Underserved Patients in a Safety-Net Healthcare System

Abstract:Background: Burnout among clinicians is common and can undermine quality of care, patient outcomes, and workforce preservation, but sources of burnout or protective factors unique to clinicians working in safety-net settings are less well understood. Understanding these clinician experiences may inform interventions to reduce burnout. Objective: To describe clinician perspectives on sources of burnout in a safety-net healthcare system. Design: Semi-structured interviews were conducted with clinicians from multiple disciplines who provided care at a safety-net healthcare system from October 2018 to January 2019. Transcripts were analyzed using thematic analysis. Participants: Forty clinicians (25 female and 15 male; mean [SD] age, 41 [9.1]) participated, including physicians, social workers, advanced practice providers, nurses, psychologists, physical and occupational therapists, and other healthcare professionals. Main outcomes and measures: Themes and subthemes that reflected clinician experiences, burnout, and workload expectations. Key results: Five themes emerged: limited resources (entrenched social injustices, brokenness of the US healthcare system, precarious discharge options, and "revolving door" readmissions), barriers to building trust with patients (chasm of communication, addressing fear and mistrust, and being exposed to threats), administrative requirements (criticism hampering meaningful care, assuming extra workloads, bureaucratic burden), compassion fatigue (confronting traumatic situations, persistent worry about patient safety and welfare, witnessing mistreatment and stigmatization, overextending and overinvesting, withdrawing and shutting down, blaming self and career crisis), and advocacy as a counterbalance to burnout (solidarity when helping underserved communities, fervent advocacy, and patient gratitude). Conclusions: Among clinicians who provide care in a safety-net healthcare system, sources of burnout included limited resources, barriers to building trust with patients, administrative requirements, and compassion fatigue, but clinicians remained driven by their desire to advocate for patients. Policy-level interventions which increase funding to safety-net healthcare systems to bolster existing resources and staffing, create peer-support and wellness programs, and support advocacy efforts may mitigate burnout.

Abstract:The healthcare industry is the largest and fastest growing industry in the world; however, being a part of the healthcare system involves being at an increased risk of experiencing mental health problems, loneliness, stress, and increased susceptibility to experiencing compassion fatigue related to the emotional wear associated with providing patient-centered care. Healthcare workers include, but are not limited, to physicians, nurses, allied health professionals, and psychologists. Often, healthcare workers place the needs and wellbeing of patients before their own. This article aims to highlight the occupational hazards of working in the healthcare field, the physical and emotional isolation associated with clinical practice, managing distressing behaviors by patients, and reviewing the systemic barriers influencing the development and management of moral distress. We further aim to bring attention to the need for healthcare professionals to place self-care at the forefront of their therapeutic repertoire through various individualized strategies, through the importance of building moral resilience, and the shift towards improving workplace spirituality. Practicing self-care can address the consequences of neglecting one’s own wellbeing, positively impact the ability to provide better quality patient care, and benefits relationships with patients, loved ones, and of most importantly with oneself.

Abstract:OBJECTIVE To explore the way for testing of multivariate in therapeutical equivalence. METHODS By improved Hotelling T(2) test, the mean vector of two groups was tested. The information of each variable could then be obtained by deducting the their correlations. "Clinical Equivalence of Drug A and Drug B for Postmenopausal Bone Looseness" was used in this research. The primary measure of efficacy was regarded as the improvement rate of four variables with predefined limit of equivalence (2%) with the results of univariate test and multivariate test compared. RESULTS Under P > alpha, all the variables were nonequivalent. Under P </= alpha, it was necessary to filtrate the variables of unequivalent until the residuals were equivalent. The method of multivariate test was a remedy to the shortage of univariate test. CONCLUSION With the combination of the filtration of nonequivalent variables and the improved Hotelling T(2) test, the test of multivariate could be done. The method is reasonable and practical.

Abstract:Background Professional burnout is a widespread phenomenon in health care. The health of patients and organizations begins with the well-being of health care professionals. Identifying and understanding self-care strategies that professionals perceive to be helpful is crucial to combat burnout. Objective To determine perceptions of self-care strategies to combat professional burnout among nurses and physicians in pediatric critical care settings. Methods This was a qualitative descriptive study with a phenomenological overtone. The study was conducted in a 20-bed pediatric intensive care unit and an 8-bed intermediate care unit of a children’s hospital in the United States. Information flyers and emails were used to introduce the study. A combination of convenience and purposive sampling methods was used to recruit participants who were full-time nurses and physicians in the 2 units. Information saturation was used to regulate sample sizes, resulting in 20 participants. Data were collected through a onetime face-to-face interview with each participant. A qualitative descriptive approach was used to analyze the data. The first author was the primary coder and discussed the codes with the coauthors throughout the coding process. Results Six major self-care strategies were identified: finding meaning in work, connecting with an energy source, nurturing interpersonal connections, developing an attitude of positivity, performing emotional hygiene, and recognizing one’s uniqueness and contributions at work. Conclusions Developing effective self-care strategies helps promote health care professionals’ physical and psychological well-being and reduce burnout. It is vital for health care professionals to care for themselves so that they can best care for others.

Abstract:Background: Primary care physicians are particularly prone to high levels of burnout and poor well-being. Despite this, no qualitative studies have specifically investigated the best ways to improve well-being and prevent burnout in primary care physicians. Previous interventions within primary care have been person-oriented and mainly focused on mindfulness, but there has been no prior research on whether general practitioners (GPs) deem this to be the best approach.Objectives: To explore strategies that could improve GP well-being and reduce or prevent burnout, based on GP perceptions of the workplace factors that affect their levels of well-being and burnout.Methods: Five focus groups were conducted, with 25 GPs (locums, salaried, trainees, and partners) in the UK, between September 2015 and February 2016. Focus groups took place in GP practices and private meeting rooms. Discussions were centered on the workplace factors that they perceived to influence their well-being, along with strategies that they use either personally, or as a practice, to try and prevent burnout. Furthermore, strategies that could feasibly be implemented by individuals and practices to improve well-being, as well as changes that are needed by groups or organizations that are external to their practice (e.g., the government) to improve the working conditions, were explored. Thematic analysis was conducted on the transcripts.Results: Based on the contributors to burnout and workplace well-being that the participants identified, the following feasible strategies were suggested: compulsory daily coffee breaks, increasing self- and organizational awareness of the risks of burnout and mentoring or buddy systems. System-level organizational changes were voiced as vital, however, to improve the well-being of all primary care physicians. Increasing resources seemed to be the ideal solution, to allow for more administrative staff and GPs.Conclusion: These strategies merit further consideration by researchers, physicians, healthcare organizations and policy makers both in the UK and beyond. Failure to do so may result in healthcare staff becoming even more burntout, potentially leading to a loss of doctors from the workforce.

Abstract:Background: This study aimed to determine what, how, and under what circumstances individual-focused interventions improve well-being and decrease burnout for critical care healthcare professionals. Method: This realist approach, expert opinion interview, was guided by the Realist And Meta-narrative Evidence Synthesis: Evolving Standards II (RAMESES II) guidelines. Semi-structured interviews with critical care experts were conducted to ascertain current and nuanced information on a set of pre-defined individual interventions summarized from a previous umbrella review. The data were appraised, and relationships between context, mechanisms, and outcomes were extracted, which created theory prepositions that refined the initial program theory. Results: A total of 21 critical care experts were individually interviewed. By understanding the complex interplay between organizational and personal factors that influenced intervention uptake, it was possible to decipher the most likely implementable intervention for critical care healthcare professionals. The expert recommendation suggested that interventions should be evidence-based, accessible, inclusive, and collaborative, and promote knowledge and skill development. Unique mechanisms were also required to achieve the positive effects of the intervention due to the presence of contextual factors within critical care settings. Mechanisms identified in this study included the facilitation of self-awareness, self-regulation, autonomy, collaboration, acceptance, and inclusion (to enable a larger reach to different social groups). Conclusion: This validation of a theoretical understanding of intervention that addressed well-being and burnout in critical care healthcare professionals by expert opinion demonstrated essential mechanisms and contextual factors to consider when designing and implementing interventions. Future research would benefit by piloting individual interventions and integrating these new theoretical findings to understand better their effectiveness for future translation into the "real-world" setting.
