Abstract:Down syndrome is one of the most common genetic diseases, generally associated with an increased probability of congenital heart diseases. This increased risk contributes to escalated levels of morbidity and mortality. In this study, we sought to analyze nationwide data of pediatric and adult patients with Down syndrome and congenital heart disease over a 15-year period. Data obtained from the hospital discharge form between 2001 and 2016 of patients diagnosed with Down syndrome in Italy and at least one congenital heart disease were included. Information on 12362 admissions of 6527 patients were included. Age at first admission was 6.2 ± 12.8 years and was a predictor of mortality (HR = 1.51, 95% CI 1.13–2.03, p = 0.006). 3923 (60.1%) patients underwent only one admission, while 2604 (39.9%) underwent multiple (> 1) admissions. There were 5846 (47.3%) admissions for cardiac related symptoms. Multiple admissions (SHR: 3.13; 95% CI: 2.99, 3.27; P < 0.01) and cardiac admissions (SHR: 2.00; 95% CI: 1.92, 2.09; P < 0.01) were associated with an increased risk of additional potential readmissions. There was an increased risk of mortality for patients who had cardiac admissions (HR = 1.45, 95% CI: 1.08–1.94, p = 0.012), and for those who underwent at least 1 cardiac surgical procedure (HR = 1.51, 95% CI 1.13–2.03, p = 0.006).

Abstract:Down Syndrome (DS) is the most common chromosomal abnormality compatible with life. The life of patients suffering from DS can be strongly impacted by Recurrent Respiratory tract Infections (RRIs), leading to an increased rate of hospitalisation, a higher need for intensive care and fatality. With a literature review, we summarise here the main etiological factors for RRI in this category of patients, particularly focusing on airway malformations such as tracheomalacia, tracheal bronchus and bronchomalacia, comorbidities associated with the syndrome, like congenital heart diseases, dysphagia, gastroesophageal reflux, musculoskeletal involvement and obesity, and immunologic impairments, involving both innate and adaptive immunity. For these patients, a multidisciplinary approach is imperative as well as some preventive strategies, in particular vaccinations in accordance with their national schedule for immunization.

Hospitalizations among children involved in the child protection system: A long-term birth cohort study from infancy to adulthood using administrative data

Abstract:Background: Despite considerable health consequences of child abuse and neglect, there is limited evidence on hospitalizations in this population. Objectives: To describe frequency and reasons for hospitalization by lifetime child protection system (CPS) involvement. Participants: 608,540 children born from January 1, 1986 to June 30, 2017 in South Australia, Australia METHODS: Using linked administrative data on CPS involvement and hospitalizations, we descriptively examined cumulative incidence, cumulative count and reasons for hospitalization from infancy to early adulthood by CPS involvement. Results: By 16.5 years of age, cumulative incidence of ever-being hospitalized was 58% (95% CI 58-58) for children with no CPS involvement and significantly higher (72% (95% CI 71-73) to 88% (95% CI 86-90), P < 0.001) among those with different levels of CPS involvement. Cumulative mean counts of hospitalizations were highest at every age for those placed in out-of-home-care (reaching 7.7 by 16.5 years), almost four times higher than for children with no CPS involvement (2.0 by 16.5 years). Most frequent reasons for hospitalizations were similar across CPS involvement in the early years. From adolescence through early adulthood, mental health, and 'injury, poisoning or toxic effects of drugs' were frequent reasons for hospitalization among individuals with CPS involvement. Conclusions: This study highlights the vulnerability of children who have been maltreated, or are at risk of maltreatment, and the urgency of implementing effective preventive strategies early in life including consideration of adequate responses of child protection services. Frequent hospitalizations for mental health and injury confirms the potentially preventable nature of these hospitalizations.

Abstract:Down syndrome (DS) is the most common genetic syndrome associated with immune defects. The extent of immune dysregulation in DS is substantial, spanning the innate and adaptive systems and including anomalies in: T and B cells, monocytes, neutrophil chemotaxis, circulating cytokines, and suboptimal antibody responses which all contribute to an increased risk of infections, poorer clinical outcomes and chronic inflammation in this vulnerable cohort. Other aspects of innate immunity may also be abnormal and contribute to the increased morbidity and warrant further interrogation such as: gamma delta T cell function, the inflammasome, Toll-like receptors and their pathways. Pharmacotherapies such as pavilizumab, pneumococcal and influenza immunizations, as well as potential immunoprophylactic agents such as pidotimod, azithromycin and Broncho-Vaxom may help alleviate the infectious consequences. Children with DS need to be managed with a heightened sense of awareness and urgency in the setting of sepsis and signs of chronic inflammation need regular screening and appropriate follow up.

Abstract:The present study is based on a systematic review of cases of patients with Down syndrome and COVID‐19. Introduction Down syndrome (DS) is associated with multiple comorbid conditions and chronic immune dysfunction. Persons with DS who contract COVID‐19 are at high risk for complications and have a poor prognosis. We aimed to study the clinical symptoms, laboratory and biochemical profiles, radiologic findings, treatment, and outcomes of patients with DS and COVID‐19. Method We systematically searched PubMed, MEDLINE, Web of Science, Scopus, and the Cochrane Library using the keywords COVID‐19 or coronavirus or SARS‐CoV‐2 and DS or trisomy 21. Seventeen articles were identified: eight case reports and nine case series published from December 2019 through March 2022, with a total of 55 cases. Results Patients averaged 24.8 years (26 days to 60 years); 29 of the patients were male. The most common symptoms were fever, dyspnea, and cough. Gastrointestinal and upper respiratory tract symptoms were commonly reported for pediatric patients. The most common comorbidities present in patients with DS were obesity (49.0%), hypothyroidism (21.6%) and obstructive sleep apnea (15.6%). The patients were hospitalized for a mean of 14.8 days. When the patients were compared with the general COVID‐19 population, the mean number of hospitalized days was higher. Most patients had leukopenia, lymphopenia, and elevated inflammatory markers (d‐dimer and C‐reactive protein). Bilateral infiltrations and bilateral ground‐glass opacifications were frequently seen in chest radiographs and chest computed tomographic imaging. Most of the patients were treated with methylprednisolone, macrolides, and hydroxychloroquine. Of the 55 patients, 22 died. The mean age of the patients who died was 42.8 years. Mortality rate was higher in individuals with DS over 40 years of age. Conclusion More studies are needed to better understand COVID‐19 infections among persons with DS. In addition, the study was limited by a lack of statistical analyses and a specific comparison group.

Abstract:Objectives Our objective was to investigate the prevalence of small intestinal atresia and Hirschsprung's disease (HD) in infants with Down syndrome (DS) and its impact on outcomes. Study Design We analyzed the National Inpatient Sample dataset. We included infants with DS, small intestinal atresia, HD, and the concomitant occurrence of both conditions. Regression analysis was used to control clinical and demographic variables. Results A total of 66,213,034 infants were included, of whom, 99,861 (0.15%) had DS. The concomitant occurrence of small intestinal atresia and HD was more frequent in infants with DS compared with the general population, adjusted odds ratio (aOR): 122, 95% confidence interval (CI): 96–154, ( p < 0.001). Infants with DS and concomitant small intestinal atresia and HD had higher mortality compared with those without these conditions, aOR: 8.59, 95% CI: 1.95–37.8. Conclusion Infants with DS are at increased risk of concomitant small intestinal atresia and HD, and this condition is associated with increased mortality. Key Points Infants with Down syndrome are at increased risk of congenital GI anomalies. Infants with Down syndrome are at increased risk of necrotizing enterocolitis. Increased mortality in Down syndrome infants with concomitant small intestinal atresia and Hirschsprung's disease. Death of children with Down syndrome by gestational age and cause

Abstract:Background: We often encounter preterm infants with Down syndrome (DS) who die in the neonatal intensive care unit (NICU). In this study, we examined survival until NICU discharge and assessed the developmental prognosis of preterm infants with DS. Methods: We retrospectively reviewed 416 infants with DS hospitalized during the past 27 years at our NICU. Results: Death occurred in 8/20 (40%) infants at <32 weeks' gestation, 11/23 (48%) at 32-33 weeks, 9/99 (9%) at 34-36 weeks, and 9/274 (3%) at >36 weeks. In total, 84% of infants who died and 25% of those who survived had a non-reassuring fetal status (p < 0.001). Sex, small-for-gestational-age status, and postnatal transport were not associated with death. The main causes of death were bronchopulmonary dysplasia in 4/8 (50%) infants at <32 weeks' gestation, transient abnormal myelopoiesis in 11/20 (55%) and lymphatic dysplasia in 6/20 (30%) at 32-36 weeks, and varied causes at >36 weeks. Among survivors born at <34 weeks' gestation, 6/19 (32%) aged >2 years had moderate or severe cerebral palsy. Conclusions: These data on the high mortality and morbidity of preterm infants with DS may be useful for patient treatment and parent counseling in NICUs treating critically ill infants. Impact: Most infants with Down syndrome born at <34 weeks' gestation are born by cesarean section because of the non-reassuring fetal status. The mortality rate before discharge for infants with Down syndrome born at <34 weeks' gestation was 40%, and 30% of survivors developed moderate or severe cerebral palsy. The risk of death due to bronchopulmonary dysplasia and pulmonary hypertension was high in very preterm infants with Down syndrome despite the absence of chorioamnionitis. Infants with Down syndrome were born 1-2 weeks earlier than unaffected controls.
The burden of respiratory syncytial virus (RSV) associated acute lower respiratory infections in children with Down syndrome: A systematic review and meta-analysis

Abstract:Background: Acute lower respiratory tract infections (ALRIs) caused by respiratory syncytial virus (RSV) are a leading cause of hospitalization in infants. Numerous risk factors have been identified in the aetiology of severe RSV-associated ALRI necessitating hospitalisation, including prematurity and congenital heart disease. Down syndrome (DS), a common genetic disorder associated with congenital and dysmorphic features, has recently been identified as an independent risk factor for RSV-associated ALRI requiring hospitalisation; however, the disease burden of RSV-associated ALRI in this population has not yet been established. Similarly, the impact of DS as an independent risk factor has not yet been quantified. We aimed therefore to estimate the incidence of admissions in children with DS, and by comparing this with unaffected children, to quantify the risk of DS independent of other risk factors. Methods: A systematic review of the existing literature published between 1995 and March 1, 2017 was performed to quantify the incidence of hospitalisation due to RSV-associated ALRI in children with DS. Meta-analyses were performed on extracted data using STATA statistical software, and hospitalisation rates for children with and without DS under the age of 2 were calculated. Findings: 5 articles were ultimately deemed eligible for analyses. Analyses were limited to children under the age of 2 years. We calculated the hospitalisation rate for children with DS in this age group to be 117.6 per 1000 child-years (95% CI 67.4-205.2), vs a rate of 15.2 per 1000 child-years (95% CI 8.3-27.6) in unaffected children. This indicates DS contributes to a 6.8 (95% CI 5.5-8.4) fold increase in the relative risk of hospitalisation for RSV-associated ALRI. Interpretation: Though limited by a small number of articles, this review found sufficient evidence to conclude DS was a significant independent risk factor for the development of severe RSV-associated ALRI requiring hospitalisation. Further studies are needed to define the impact of DS in conjunction with other comorbidities on the risk of severe RSV infection. Determining benefits of immunoprophylaxis or future vaccines against RSV in this at-risk population is warranted.
Enhancing understanding of SARS-CoV-2 infection among individuals with Down syndrome: An integrative review

Abstract:BACKGROUND: Down syndrome (DS) is a non-rare genetic condition that affects approximately 1 in every 800 live births worldwide. Further, it is associated with comorbidities, anatomical alterations of the respiratory tract, and immunological dysfunctions that make individuals more susceptible to respiratory infections.OBJECTIVE: To systematize the current scientific knowledge about the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection among individuals with DS.DESIGN AND SETTING: This integrative review was conducted at the Universidade Federal de São Carlos, São Paulo, Brazil.METHODS: This review was conducted in the following databases: the Virtual Health Library (Biblioteca Virtual em Saúde, BVS), PubMed, and Web of Science, using MeSH descriptors. The search included English or Portuguese studies published between January 1, 2020, and October 14, 2022.RESULTS: A total of 55 articles from 24 countries were selected, comprising 21 case-control or cohort studies, 23 case reports or series, and 11 narrative reviews or opinion studies. The articles were grouped into five categories: previous comorbidities, coronavirus disease 2019 (COVID-19) clinical features and evolution, cytokine storm and interleukins, living in institutions as a risk factor, and behavioral actions as a protective factor against SARS-CoV-2 infection.CONCLUSION: Individuals with DS are more susceptible to COVID-19 infection due to variables such as previous comorbidities, immunological factors, and their habitable environments. These aspects confer a higher risk of infection and an unfavorable clinical course. The precise pathways involved in the pathophysiology of COVID-19 in individuals with DS are not clear, thus requiring further studies.SYSTEMATIC REVIEW REGISTRATION: The Open Science Framework registered the research protocol (https://osf.io/jyb97/).

Abstract:Although Down syndrome (DS) is considered a risk factor for hemodynamic instabilities (mainly pulmonary hypertension–PH) following surgery for congenital cardiac communications, many DS patients do surprising well postoperatively. We prospectively analyzed perioperative factors for a possible correlation with post-cardiopulmonary bypass (CPB) inflammatory reaction and postoperative PH in pediatric subjects. Sixty patients were enrolled (age 3 to 35 months), 39 of them with DS. Clinical and echocardiographic parameters (anatomical and hemodynamic) were computed preoperatively. Pulmonary and systemic mean arterial pressures (PAP and SAP) were assessed invasively intra and postoperatively. Immediate postoperative PAP/SAP ratio (PAP/SAP IPO ) and the behavior of pressure curves were selected as primary outcome. Serum levels of 36 inflammatory proteins were measured by chemiluminescence preoperatively and 4 h post CPB. Of all factors analyzed, peripheral oxygen saturation (O 2 Sat, bedside assessment) was the only preoperative predictor of PAP/SAP IPO at multivariate analysis (p = 0.007). Respective values in non-DS, DS/O 2 Sat ≥ 95% and DS/O 2 Sat < 95% subgroups were 0.34 (0.017), 0.40 (0.027) and 0.45 (0.026), mean (SE), p = 0.004. The difference between non-DS and DS groups regarding postoperative PAP curves (upward shift in DS patients, p = 0.015) became nonsignificant (p = 0.114) after adjustment for preoperative O 2 Sat. Post-CPB levels of at least 5 cytokines were higher in patients with O 2 Sat < 95% versus those at or above this level, even within the DS group (p < 0.05). Thus, a baseline O 2 Sat < 95% representing pathophysiological phenomena in the airways and the distal lung, rather than DS in a broad sense, seems to be associated with post-CPB inflammation and postoperative PH in these patients.

Abstract:We conducted a cross-sectional study to describe the health care problems of children with Down syndrome in Central Java, Indonesia. A total of 162 children (81 boys, 81 girls) with Down syndrome were included. Congenital heart defects and hypothyroidism were found in about 50%, followed by vision and hearing problems in 27.7% and 17.3%, respectively. Almost half of cases were diagnosed after the first month of age. Advanced maternal age was identified in more than 50%, and less than 10% was based on karyotype analysis. This study describes the essential issues such as critical co-morbidities, delayed diagnosis, advanced maternal age, and lack of (accessibility to) genetic testing facilities; thus, better health care and management is needed.

Abstract:Background Studies investigating the quality of the diet and dietary intake of children with Down syndrome (DS) are required because the features attributed to the syndrome can affect growth, development and quality of life. Methods This cross‐sectional study was conducted with 77 Brazilian children with DS between 5 and 36 months of age receiving care at the multidisciplinary outpatient clinic of the University Hospital. Participants' sociodemographic, dietary and anthropometric data were collected from the care protocols. Dietary data were collected from 24‐h recalls and dietary practices were assessed according to the WHO dietary guidelines. Associations between inadequate feeding practices and demographic variables were assessed using logistic regression models. Results Fruits, milk or infant formula, vegetables, beans and meat were among the five most consumed foods by the children investigated. Overall, we observed a high number of cases of early weaning (50.6%), low minimum dietary diversity (MDD; 40.3%), inadequate consistency for age (64.9%), early presence of ultra‐processed foods (76.6%), sugars and sweets (33.8%) in the diet of the children with DS. In the associations of inadequate feeding practices by age group, low MDD [odds ratio (OR): 18.6; 95% confidence interval (CI): 3.4; 57.1] and inadequate consistency (OR: 6.65; 95% CI: 1.8; 24.7) were more frequent among children aged below 12 months while this relationship was inverse for early introduction of sugar and sweets (OR: 0.04; 95% CI: 0.01; 0.29). Conclusion Our findings showed a high number of cases of inadequate dietary practices in children with DS investigated, which could adversely affect the long‐term health of this population.

Abstract:Down syndrome (DS) is the most common chromosomal disorder worldwide. Along with intellectual disability, endocrine disorders represent a remarkable share of the morbidities experienced by children, adolescents and young adults with DS. Auxological parameters are plotted on syndrome-specific charts, as growth rates are reduced compared to healthy age- and gender-matched peers. Furthermore, children with DS are at increased risk for thyroid dysfunctions, diabetes mellitus, osteopenia and obesity compared to general population. Additionally, male individuals with DS often show infertility, while women tend to experience menopause at an overall younger age than healthy controls. Given the recent outstanding improvements in the care of severe DS-related comorbidities, infant mortality has dramatically decreased, with a current average life expectancy exceeding 60 years. Accordingly, the awareness of the specificities of DS in this field is pivotal to timely detect endocrine dysfunctions and to undertake a prompt dedicated treatment. Notably, best practices for the screening and monitoring of pediatric endocrine disorders in DS are still controversial. In addition, specific guidelines for the management of metabolic issues along the challenging period of transitioning from pediatric to adult health care are lacking. By performing a review of published literature, we highlighted the issues specifically involving children and adolescent with DS, aiming at providing clinicians with a detailed up-to-date overview of the endocrine, metabolic and auxological disorders in this selected population, with an additional focus on the management of patients in the critical phase of the transitioning from childhood to adult care.

Abstract:Biniam Endale Geleta, Girma Seyoum Department of Anatomy, Addis Ababa University, Addis Ababa, Ethiopia Correspondence: Biniam Endale Geleta, Email Background: Children with DS are at higher risk of developing congenital anomalies, particularly cardiac anomalies. Methods: Medical records of 502 DS patients were reviewed. The logistic regression analyses were performed to determine independent predictors. Results: Of the total 502 study subjects, 53.4% were males. Only 1.4% of the DS case diagnosis were confirmed by karyotyping. All cases were diagnosed postnatally. The median age at DS diagnosis was 5 months. About 13% were born preterm; 50.2% of the subjects maternal age at conception were thirty-five years and above. Over three-quarters (75.1%) had at least one structural congenital anomaly. Multiple anomalies were diagnosed in 12.8% of the subjects. At least one cardiac congenital anomaly was diagnosed in 67.3% of the study subjects, and 32.8% of them were diagnosed with multiple cardiac anomalies. Patent ductus arteriosus (28.5%), Ventricular septal defect (23.2%), and AVSD (21.9%) were the three common lesions. At least one genitourinary system anomaly was identified in 32 (6.4%) of them. Roughly, 8% of study participants exhibited congenital anomaly of the head, eye, nose, and throat. Anorectal malformation was found as the most common gastrointestinal anomaly. Maternal age at conception was found as independent predictor for presence of structural congenital anomaly (AOR 2.59; 95% CI 1.58– 4.23, p-value < 0.01). Advanced maternal age is also found increasing the risk of developing congenital heart defect (AOR 2.37; 95% CI 1.52– 3.7, p-value < 0.01). Conclusion: High prevalence of congenital anomalies has been noted in the current study compared to previous studies. Predictive factors increasing risk of congenital anomalies in DS patients have been identified. The current findings may help in developing strategies and more targeted preventive and therapeutic interventions. Keywords: Down syndrome, congenital anomaly, cardiac anomaly, maternal age, chromosome 21 Down syndrome (DS) is a chromosomal abnormality characterized by the presence of an extra copy of human chromosome 21. 1 It is the most prevalent chromosomal abnormality that allows for survival beyond the embryonic stage; affecting 1 per 700 to 1000 live births globally. 2–4 The incidence of Down syndrome (DS) is decreasing in developed countries due to widespread prenatal testing and elective termination of affected pregnancies. 5 In contrast, limited access to antenatal care in Africa and other developing countries poses a significant challenge for prenatal DS diagnosis, leading to late diagnoses after birth. 1,6 While karyotyping remains the gold standard for DS diagnosis, its accessibility and affordability are significant barriers in developing countries like Ethiopia. Therefore, clinical diagnosis based on physical features and behavior is commonly employed in these settings, including the one in which this study was conducted. 6–8 However, these clinical diagnostic standards were primarily developed in high-income countries and may not be as accurate in LMICs due to potential phenotypic variations across regions and ethnicities. 1,8,9 Additionally, studies have shown that the accuracy of clinical diagnosis varies significantly between DS subtypes. One study reported overdiagnosis in up to one-third of cases, while another found accuracy rates of 90% for free trisomy, 100% for translocation, and only 37.5% for mosaicism. 9,10 Congenital anomalies are more common among DS patients than the general population. This is attributed to a combination of gene dosage imbalance and epigenetic factors. However, the pattern and prevalence of these anomalies vary from one region to the other. 11 Stoll et al reported 64% prevalence of congenital anomalies in children with DS. While the Egyptian study reported 59.9% prevalence among infants. 12 Chance of development of these anomalies was found increased by factors like advanced maternal age, paternal age, gestational age, and birth weight. 13 Cardiac lesions are the most common congenital anomalies among children with DS. About half of patients with DS have at least one form of cardiac anomaly. 12,14 Variations are noted among regions. Accordingly, a study of twenty European countries reported prevalence of 43.6%. Abstract:The thermodynamic functions Gibbs energy, enthalpy and entropy of solution in cyclohexane, were evaluated from solubility data for a group of sulfonamides over the temperature range from 20.0 to 40.0 °C. The excess Gibbs energy and the activity coefficients of the solutes were also determined. The results are discussed in terms of solute-solvent interactions.

Abstract:<p>Down syndrome is the most common human chromosomal disorder. Among clinical findings, one constant concern is the high prevalence of gastrointestinal system alterations. The aim of this study was to determine the prevalence of gastrointestinal disorders at a Down syndrome outpatient clinic during a 10‐year follow‐up period. Data from medical files were retrospectively reviewed from 1,207 patients. Gastrointestinal changes occurred in 612 (50.7%). The most prevalent disorder was chronic intestinal constipation. Intestinal parasite occurred in 22% (mainly giardiasis), gastroesophageal reflux disease in 14%, digestive tract malformations occurred in 5%: 13 cases of duodenal atresia, 8 of imperforate anus, 4 annular pancreases, 2 congenital megacolon, 2 esophageal atresias, 2 esophageal compression by anomalous subclavian and 1 case of duodenal membrane. We had 38/1,207 (3.1%) patients with difficulty in sucking and only three with dysphagia that resolved before the second year of life. Peptic ulcer disease, celiac disease, and biliary lithiasis were less prevalent with 3% each. Awareness of the high prevalence of gastrointestinal disorders promotes outstanding clinical follow‐up as well as adequate development and greater quality of life for patients with Down syndrome and their families.</p>

Abstract:The aim of this study was to assess the prevalence of congenital heart defects (CHDs) and persistent pulmonary hypertension of the neonate (PPHN) in children with Down syndrome (DS) and to assess its impact on neonatal factors. It was a prospective study of a birth cohort of children with DS born between 2003 and 2006 registered by the Dutch Paediatric Surveillance Unit (DPSU). A CHD occurred in 43% of 482 children with trisomy 21. Atrioventricular septal defect was found in 54%, ventricular septal defect in 33.3% and patent ductus arteriosus in 5.8%. The incidence of PPHN in DS was 5.2%, which is significantly higher than the general population (p < 0.001). The reported mortality in newborns with DS was overall 3.3% and was still significant higher in children with a CHD versus no CHD (5.8% versus 1.5%) (p = 0.008). The presence of CHD in children with DS had no influence on their birth weight, mean gestational age and Apgar score. In neonates with DS, we found not only a 43% prevalence of CHD, but also a high incidence of PPHN at 5.2%. Early recognition of the cardiac condition of neonates with DS seems justified.

Abstract:Summary The levels of seven enzymes involved in the metabolism of 1-carbon compounds and their precursors were investigated in the cytoplasmic fractions of the livers of rats receiving the hepatocarcinogens diethylnitrosamine, 2-acetylaminofluorene, and N,N -dimethyl-4-aminoazobenzene for 1 to 5 weeks. The enzymes studied included serine hydroxymethylase, dihydrofolate reductase (H 2 -folate reductase), formylase, methylene-H 4 -folate dehydrogenase, formiminoglutamic acid transferase, urocanase, and histidase. The levels of formiminoglutamic acid transferase and urocanase were found to be significantly lower in the livers of rats treated with each of the hepatocarcinogens for 3 to 5 weeks than in the livers of corresponding control rats. The hepatic activities of formylase, methylene-H 4 -folate dehydrogenase, and histidase were decreased during the chronic administration of diethylnitrosamine and 2-acetylaminofluorene but not by the chronic feeding of N,N -dimethyl-4-aminoazobenzene; however, the low-protein, low-riboflavin diet used as a control diet in the aminoazo dye experiments also led to low hepatic levels of these 3 enzymes. Administration of diethylnitrosamine and N,N -dimethyl-4-aminoazobenzene to rats for 3 weeks led to a slight but significant increase in the hepatic levels of H 2 -folate reductase, in comparison to the corresponding levels in livers of control rats; the feeding of 2-acetylaminofluorene appeared to have no significant effect on hepatic H 2 -folate reductase activity. None of the carcinogens investigated appeared to have a significant effect on the hepatic levels of serine hydroxymethylase. The administration of a choline-deficient diet to weanling rats for 5 weeks produced significant decreases in the hepatic levels of formylase, methylene-H 4 -folate dehydrogenase, histidase, urocanase, and formimino-glutamic acid transferase; the choline deficiency produced no effect on the hepatic levels of serine hydroxymethylase and H 2 -folate reductase. The results indicate that chronic administration to rats of either a hepatocarcinogenic or methyl-deficient regimen decreased the hepatic levels of five enzymes involved in the synthesis of the formyl derivatives of H 4 -folate.

Abstract:Abstract Background Since the beginning of the pandemic, the American Academy of Pediatrics and the Children′s Hospital Association have reported over 15.4 million cases of COVID-19 in children in the United States (18% of all cases).1 UNICEF on the other hand, has reported more than 288 million cases all over the world and 4.1 million deaths related to COVID-19 in children.2 Healthcare-Associated Infections (HAI) represent a big burden on the health care system, with a subsequent increase in morbidity, hospital stay, medication/procedures, intensive care unit admissions, mortality, etc. Globally there is not enough data of HAI for SARS-CoV-2 in children, specially from low and lower-middle income countries. Methods Prognosis study through the description and analysis of an ambispective cohort in children at a tertiary care pediatric hospital [Hospital Infantil de Mexico Federico Gomez: (HIMFG)] in Mexico City from April 2020 to December 2022. We identified patients <18 years old with probable and definite healthcare-associated (HA) COVID-19 according to the CDC definition:3 symptom onset (or positive test) on day 8 or on, after admission. Independent variables included socio-demographic information, clinical symptoms, laboratory results, among others. Outcome variables measured included severity (severe and critical), and death.4 Chi square and Wilcoxon tests were performed to identify independent risk factors for severity and death. After the bivariate analysis, logistic regression models were used to identify factors associated with patient severity and mortality. Results 102 patients were included; mean age was 6.4 years (min 0.02, max 17.9) and 57 were male. 32 were asymptomatic, 27 had mild disease, 6 were moderate, 31 severe and 6 critical; of this total, 6 patients died. 98 patients had at least one comorbidity, and 54 had at least 2: 35 with immunosuppression, 28 with cancer, 18 with heart disease, 14 with pulmonary disease and 14 with neurologic disease. 14 patients required admission to a Pediatric Intensive Care Unit. Mean hospital stay at diagnosis was 35.5 days (min 8, max 143): 24 patients were identified with probable HA-COVID-19 (8-14 days after admission) and 78 with definite HA-COVID-19 (>14 days after admission). Conclusions Since the beginning of the COVID-19 pandemic, multiple strategies in infection prevention and control have been implemented at our hospital: universal screening before admission, active surveillance for patients/caretakers/health-care workers with respiratory symptoms and for contacts, universal use of facemasks, isolation of patients with respiratory symptoms, among others. Despite all these measures, a free Hospital from HA-COVID-19 remains a challenge. Our study found that the sudden onset of symptoms, the presence of fever, irritability and polypnea increases significantly the risk for severity in patients that acquired the disease in the hospital. On the other hand, patients with an underlying heart disease have 30 times more risk of dying than those who doesn’t. The presence of O2 desaturation and admission to a PICU increases significantly the mortality in patients with HA-COVID-19. Overall, mortality in children with HA-COVID-19 was 5.9% (up to 8.6% if strictly considering only the symptomatic), much more that the one reported globally (1.4%).2 References 1. Cull B HMBLRG.
