Abstract:Retroperitoneal hemorrhage in the form of spontaneous adrenal hemorrhage or bleeding into a renal tumor can have varied and non-specific presentations in pregnancy. In the absence of risk factors, these life-threatening conditions are rarely suspected. We present our experience with three patients who presented to us in the third trimester of pregnancy with hemorrhage in retroperitoneal organs. One of the patients had a spontaneous adrenal hemorrhage and the other two had a hemorrhage in the renal tumor. None of the patients was known to have pre-existing tumors, coagulopathy, or trauma. Both the patients with hemorrhage in the renal tumor had intrauterine fetal demise at the time of presentation. Immediate resuscitation and recruitment of a multidisciplinary team resulted in optimal maternal outcomes in all cases and a healthy fetal outcome in the patient with adrenal hemorrhage.
Primary Aldosteronism and Pregnancy: A Case Report

Abstract:A 34-year-old Chinese woman was referred to our hospital at 24 weeks of gestation for persistent hypokalaemia and new onset of hypertension. Prior to pregnancy, she had a three-year history of recurrent episodes of muscle weakness and found to have low potassium levels (the lowest level was 1.8 mmol/L, normal 3.5–5.3 mmol/L). During the clinical visits, her blood pressure was normal, consistently 110–120/70–80 mmHg, until the twenty-third week of pregnancy, when it was found to be 160/100 mmHg. During her hospitalisation, blood pressure remained elevated despite treatment, with a peak level of 210/130 mmHg. Diagnostic evaluation included potassium levels, which remained low at 2.94–3.65 mmol/L despite supplementation; urinary protein was 1.98 g/24 h; urinary potassium excretion was 48 mmol/24 h, suggesting renal loss of potassium. The plasma renin activity (PRA) was 0.76 ng/mL/h (normal 0.2–1.9 ng/mL/h in non-pregnancy); plasma aldosterone concentration (PAC) was 830 pg/mL (normal 21–136 pg/mL in non-pregnancy), with a PAC-to-PRA ratio of 109. Urine catecholamines, plasma corticotrophin, cortisol and calcium concentrations were almost normal. Ultrasound scan identified a parenchymatous mass in the left adrenal gland. Primary aldosteronism was suspected, but surgery during pregnancy was not considered due to lack of expertise in this setting, though laparoscopic adrenalectomy during pregnancy might be a possible choice for treatment. Conservative treatment was adopted to prolong the pregnancy as much as possible, including nitroglycerin, nifedipine, labetabol, furosemide, magnesium sulfate, potassium supplement and other supportive therapies. Spironolactone was not adopted considering its uncertain effect to the fetus, although there was a report of it being used safely at high dose in a mother with Batter’s syndrome for two pregnancies without evidence of harm to male infants. Emergency caesarean section was performed at the twenty-fifth week of gestation because of HELLP syndrome (type III), uncontrolled hypertension and mild placental abruption. Her blood pressure decreased after delivery but not to normal. Abdominal computed tomography six months later confirmed a tumour in the left adrenal gland (Fig. 1). Laparoscopic adrenalectomy was then performed and pathological analysis confirmed a cortical adenoma of the left adrenal gland. After surgery, her serum potassium level and blood pressure gradually returned to normal. PRA, PAC and PAC-to-PRA ratio all returned to normal after three months. On follow up, serum potassium levels and blood pressure have remained normal without medication. This patient was not hypertensive in early pregnancy, most probably due to the physiological vasodilation that occurs in pregnancy. Hypertension due to Conn’s may not become apparent until midto late pregnancy (as occurred for this patient) or until post-partum. Bilateral adrenal hemorrhage in a postpartum woman with multiple thromboemboli: A case report

Abstract:Background: Bilateral adrenal hemorrhage is a rare but often a fatal cause of primary adrenal insufficiency that can result in adrenal crisis if not identified and managed appropriately. Case presentation: We present a case of a 27-year-old Caucasian female who was admitted to the hospital 17 days postpartum with pleuritic chest and flank pain, shortness of breath and nausea. Computed tomography imaging confirmed multiple thromboemboli including pulmonary emboli and noted bilateral bulky adrenal glands. She was managed for infection and pulmonary emboli; however, she complained of persistent headaches, nausea, and vomiting despite appropriate management. Radiology re-review found the computed tomography imaging was consistent with bilateral adrenal hemorrhage in hindsight. Subsequent endocrine evaluation with hypothalamic-pituitary-adrenal axis interrogation and adrenocorticotropic hormone (Synacthen) stimulation testing confirmed resultant primary adrenal insufficiency. She required urgent intravenous hydrocortisone and was subsequently discharged on oral adrenal replacement therapy and anticoagulation. Conclusions: Delay in identification and treatment of adrenal insufficiency can lead to catastrophic outcomes. This case highlights the challenge of diagnosing bilateral adrenal hemorrhage and resultant adrenal insufficiency as patients may not present with the classic risk factors, signs, symptoms, and electrolyte derangements.

Abstract:Ovarian arterial haemorrhage (OAH), a rare cause of spontaneous retroperitoneal hematoma (SRH), usually occurs in women during pregnancy or in those with a history of repeated pregnancies. The clinical manifestations of OAH are non-specific; hence, proper and timely imaging examinations are extremely important. Contrast Enhanced CT scan is the first choice for clarifying the cause of haemorrhage in patients with SRH. Trans-arterial embolisation (TAE) has become the most common treatment option for OAH. Here, we report a case of SRH in the left ovarian artery region in a patient with a congenital solitary kidney. The patient was initially misdiagnosed as left kidney haemorrhage which was successfully embolised with tissue glue. There are no previous reports of OAH in patients with a congenital solitary kidney.

Abstract:BACKGROUND Heterotopic cesarean scar pregnancy（HCSP） is very rare and has a high risk of massive uterine bleeding. Preservation of concurrent intrauterine pregnancy（IUP） is one of the great challenges in the management of HCSP. No universal treatment protocol has been established when IUP is desired to be preserved.CASE SUMMARY We report a case of HCSP at 8+ wk gestation in a 34-year-old woman with stable hemodynamics. A two-step intervention was applied. Selective embryo aspiration was performed first, and surgical removal of ectopic gestational tissue by suction and curettage was performed 2 d later. Both steps were performed under ultrasound guidance. The patient had an uneventful course, and a healthy baby was delivered at 34+6 wk gestation.CONCLUSION Selective embryo aspiration followed by suction and curettage was successful in the preservation of IUP in the management of HCSP. This approach is an alternative option for HCSP in the first trimester when the IUP is desired to be preserved.
Conservatively managed spontaneous splenic rupture in pregnancy with severe preeclampsia: an interesting case report

Abstract:Spontaneous splenic rupture during pregnancy can be catastrophic with most of the cases needing splenectomy. We presence an interesting case of conservatively managed spontaneous splenic rupture in pregnancy. A 24-year-old G2P1L1 with 35 weeks gestation with previous LSCS severe preeclampsia and severe anaemia was referred from a peripheral centre. On examination, she was vitally stable with marked pallor and pedal oedema. Abdominal wall oedema and ascites was present, uterus was 28 weeks with FHS localised by Doppler. After 2 hours, patient complained of continuous pain abdomen. It was tense, tender with FHS non-localised, uterine fundus could not be made out. USG revealed a retroplacental hematoma of 3.5×2.2 cm with free fluid in the abdomen. Differential diagnosis of rupture uterus and abruptio placenta were made. Emergency laparotomy was done. Intraoperatively, 800 cc fresh hemoperitoneum was present, previous scar was intact and a live baby delivered by LSCS. Uterus, bilateral tubes and ovaries were normal. After the uterus was closed, fresh intraabdominal bleeding was still present. With the help of general surgeon, exploration was done and two long superficial splenic tears (grade 2 splenic injury) with continuous oozing were identified. Contact pressure was applied by gauze. Bleeding stopped and omental wrapping around spleen was done. Abdomen was closed after putting drain and patient was shifted to ICU for monitoring. After 27 days, patient was discharged in a stable condition. Thus, spontaneous splenic rupture should be considered in the diagnostic differential of hemodynamic instability in a case of severe preeclampsia. Emergency laparotomy before the setting of collapse and DIC are vital steps to save the spleen and improve feto-maternal survival.

Abstract:Background: Functioning adrenal adenoma during pregnancy is rare, and the diagnosis is challenging owing to unspecific symptoms and restricted investigations. The obstetric outcomes of patients who undergo surgery during pregnancy or who receive only medical treatment are poorly described. Objective: The aim was to investigate the associations between functioning adrenal adenomas and obstetric outcomes. Methods: A retrospective study was performed in a tertiary center over 20 years. The clinical characteristics, management and obstetric outcomes of the diagnosed pregnant women were reviewed. Results: A total of 12 women were diagnosed with functioning adrenal adenomas during pregnancy from January 2002 to September 2022. Eight women had cortisol-secreting adrenal adenomas, two had excessive catecholamine secretion, and two had primary aldosteronism. The initial symptoms of adrenal adenoma during pregnancy included hypertension or preeclampsia, gestational diabetes mellitus or prepregnancy diabetes mellitus, hypokalemia and ecchymosis. Four women underwent adrenalectomy during pregnancy, while 8 women received only medical therapy. Preterm birth occurred in all patients who received medicine, whereas 1 patient who underwent surgery experienced preterm birth. Among the 8 women in the medical treatment group, 3 had neonates who died. Conclusions: Once hypertension, hyperglycemia and hypokalemia occur during the 1st or 2nd trimester, pregnant women with adrenal adenomas should be evaluated via laboratory and imaging examinations. The maternal and fetal outcomes were unpredictable owing to the severity of adrenal adenoma, particularly in patients who received only medical treatment. Adrenalectomy should be recommended during pregnancy.

Abstract:Secondary kidney involvement by disseminated non-Hodgkin’s lymphoma (NHL) is quite common and is estimated to approach 30 – 60% in NHL patients. However, primary renal lymphoma is exceedingly rare and estimated to make up less than 1% of all kidney masses. We report a case of primary renal NHL presenting with profound hypercalcemia and renal failure recalcitrant to medical management, ultimately treated with urgent radical nephrectomy. To our knowledge, this is the first report of primary renal lymphoma presenting in this acute fashion.

Abstract:Neuromyelitis optica (NMO) is an inflammatory demyelinating disease of the central nervous system. Circulating autoantibodies (NMO‐immunoglobulin [Ig]G) against astrocyte water channel aquaporin‐4 (AQP4) cause complement‐ and cell‐mediated astrocyte damage with consequent neuroinflammation and demyelination. Current NMO therapies, which have limited efficacy, include immunosuppression and plasma exchange. The objective of this study was to develop a potential new NMO therapy based on blocking of pathogenic NMO‐IgG binding to its target, AQP4.

Abstract:Background Bilateral adrenal hemorrhage (BAH) is a rare but potentially catastrophic condition. Its clinical manifestation is often non-specific and sometimes difficult to be diagnosed in time. Case summary A 57-year-old woman, who presented with severe fatigue, nausea and vomiting after left hip arthroplasty due to her femoral neck fracture in a local hospital, was transferred to our medical center. Laboratory results revealed significant hyponatremia, low serum cortisol and elevated serum ACTH. Computed tomography (CT) showed a bilateral adrenal mass, measured 3.6 × 2.7 cm on the left and 3.4 × 2.3 cm on the right. Further magnetic resonance imaging (MRI) confirmed the diagnosis of BAH. The patient was prescribed with oral prednisolone acetate, 5 mg, tid, and her condition improved gradually. Nine months after, the patient was in good condition with 5 mg prednisolone acetate per day. CT revealed a clearly shrunken adrenal mass compared with 9 months ago. Conclusions This case illustrates the difficulty in making the diagnosis of BAH with atypical presentation. Such cases necessitate greater alertness on the part of the clinician and require rapid diagnosis and prompt glucocorticoid replacement for better clinical outcomes.
Spontaneous Spinal Epidural Hematoma Complicated by Pulmonary Embolism During Pregnancy: A Case Report

Abstract:Background Spontaneous spinal epidural hematoma, without discernable underlying conditions, is considered a neurological emergency, and is rare during pregnancy. Case Presentation We report the case of a 24-year-old patient at 37 weeks of gestation. She had back pain that progressed to paraplegia of both lower limbs within 2 days. Thoracic magnetic resonance imaging revealed a lesion behind the spinal cord at the T5–T6 level, suggestive of spontaneous spinal epidural hematoma. Due to the rapid recovery of muscle strength in her lower limbs after an emergency cesarean section, we used methylprednisolone therapy to reduce spinal edema rather than decompression of the spinal canal. We incidentally found that the patient’s left pulmonary artery was occluded. In consideration of spontaneous spinal epidural hematoma as relative contraindication to anticoagulation, and in the absence of pulmonary embolism symptoms, including good partial oxygen pressure, we did not administer anticoagulant therapy. The patient’s condition improved rapidly in the following week. Conclusion Spontaneous spinal epidural hematoma concomitant with pulmonary artery embolism is an extremely rare manifestation during pregnancy. As exemplified by our case, desirable treatment outcomes are possible under such cases.

Abstract:Introduction: Aneurysmal subarachnoid hemorrhage (aSAH) during pregnancy is a rare but serious complication associated with significant maternal and fetal morbidity and mortality. Due to a limited number of published cases, development of guidelines for the management of aSAH in pregnant women has proven difficult. In the present article, we present a case and review the available literature on patients with aSAH during pregnancy. Research question: What is the optimal management of aSAH during pregnancy? Material and methods: We describe the case of a pregnant woman with aSAH. In addition, a search of the PUBMED database was conducted to collect all pertinent case reports of aSAH in pregnant women published since 1990. Results: A 36 years old Caucasian primigravid woman in the 37th GW presented to our department with aSAH, due to rupture of a saccular basilar tip aneurysm. After multidisciplinary discussion, a Caesarian section (CS) and subsequent aneurysm treatment by endovascular coiling were performed without complications. On day four after ictus endovascular spasmolysis were initiated as the patient developed angiographic cerebral vasospasm and delayed cerebral ischemia (DCI). Two days later, brain tissue hypoperfusion was further aggravated by cardiopulmonary failure under induced hypertension, so that the patient died on day seven from severe cerebral infarction. Discussion and conclusion: While there are still no formal studies that could guide the optimal management of aSAH during pregnancy, primary CS prior to definitive management of ruptured aneurysms during the third trimester seems to be the safest treatment approach for both mother and child.
Hypertensive crisis in pregnancy due to a metamorphosing pheochromocytoma with postdelivery Cushing's syndrome

Abstract:Pheochromocytomas in pregnancy are rare but potentially lethal. Even rarer is the combination of pheochromocytoma in pregnancy with subsequent development of ectopic Cushing's syndrome. We report a 36-year-old woman, previously diagnosed with essential hypertension, who developed severe hypertension in pregnancy complicated by insulin-dependent gestational diabetes. A cesarean section was performed at 32 weeks following a hypertensive crisis after routine administration of betamethasone. Postnatal persistence of signs and symptoms of catecholamine excess led to the diagnosis of a left adrenal pheochromocytoma. Between diagnosis and planned tumor removal, the patient developed signs and symptoms of Cushing's syndrome (facial edema and hirsutism, myopathy and fatigue). Biochemical testing confirmed hypercortisolism with extremely elevated levels of plasma adrenocorticotropin, urinary cortisol and multiple steroids of a plasma panel that were all normal at previous testing. The previously noradrenergic tumor also started producing epinephrine. Histopathological examination confirmed the pheochromocytoma, which was also immunohistochemically positive for adrenocorticotropin. Full post-surgical recovery was sustained with normal blood pressure and biochemical findings after one year. This report not only underlines the chameleon behavior of pheochromocytoma but also illustrates its potential for a metamorphosing presentation. Corticosteroid administration in pregnancy requires a cautious approach in patients with hypertension.

Abstract:Background and objectives Neonatal adrenal hemorrhage (NAH) is relatively uncommon in neonates and it is often noted accidently by abdominal ultrasonogram. Few studies discussed risk factors for and impacts of NAH. This study aimed to assess incidence, perinatal characteristics and follow-up outcomes in neonates with adrenal hemorrhage. Methods This was a retrospective cohort study in a single institute from April 2008 to August 2018. All neonates who received abdominal ultrasonogram within seven days-of-life were recruited and divided in to 2 groups according to the presence of NAH. The perinatal characteristics and anthropometric measurements, the follow-up course and the clinical impact of NAH were reviewed in detail. Results 7217 neonates had received abdominal ultrasonogram within seven days-of-life and 29 of them (0.4%) were diagnosed with NAH. Mean gestation age was 38 ± 1.2 weeks and mean birth weight was 3406 ± 403 g. Most infants (96.6%) had unilateral hemorrhage over the right adrenal gland. Compared with the control group, infants with NAH were significantly heavier (3406 vs. 3094 gm, p < 0.001), longer in body length (50.1 vs. 48.8 cm, p < 0.001) and wider in chest girth (33.2 vs. 32.4 cm, p = 0.006). They also tended to be delivered via vaginal delivery with vacuum-extraction rather than cesarean section. The prevalence of nuchal cord, neonatal jaundice and subgaleal hemorrhage was higher in the NAH group. The hemorrhage area of adrenal gland had a positive correlation with the peak bilirubin level (r = 0.422, p < 0.001) and the days to resolution (r = 0.198, p = 0.033). All infants had resolution of AH before 7 months of age. Conclusions NAH occurred more frequently in heavier neonates that were delivered via vaginal delivery with vacuum extraction. The hemorrhage involved mostly over the right adrenal gland. Neonatal jaundice was the major comorbidity. All infants had spontaneous resolution of AH before 7 months of age.

Abstract:Spontaneous haemoperitoneum in pregnancy (SHiP) is a rare condition that can seriously endanger the life of both the mother and child. It can occur at any time during pregnancy but is most common in the last trimester. The etiology of SHiP is unknown. Endometriosis is one of the main risk factors for spontaneous haemoperitoneum due to the rupture of the utero-ovarian vasculature or bleeding from endometrial foci in the abdomen, but so is adenomyosis. We present an infrequent clinical case of a patient with uterine adenomyosis rupture and bleeding from endometrial foci in the third trimester of pregnancy.

Abstract:Objective To investigate the etiology, presentation, management, and outcomes of patients with adrenal hemorrhage (AH). Patients and Methods Longitudinal study of consecutive adult patients with radiologically confirmed AH (January 1, 2017, through December 31, 2021). Results Of the 363 patients with AH (median age, 62 years [interquartile range (IQR, 52-70 years]; 128 women [35%]), 338 (93%) had unilateral AH and 25 (7%) had bilateral AH. It was discovered incidentally in 152 patients (42%) and during the evaluation of trauma in 103 (28%), abdominal/back pain in 90 (25%), critical illness in 13 (4%), and symptoms of adrenal insufficiency in 5 (1%). Etiologies included postoperative complications in 150 patients (41%), trauma in 107 (30%), coagulopathy in 22 (6%), anticoagulant/antiplatelet therapy in 39 (11%), adrenal neoplasm in 22 (6%), and sepsis in 11, (3%). Overall, 165 patients (46%) were hospitalized, and no deaths occurred due to AH. Median (IQR) baseline AH size was 34 mm (24-40 mm) on the right and 29 mm (22-37 mm) on the left. Among 246 patients with follow-up imaging, AH resolution was complete in 155 (63%) and incomplete in 74 (30%) at a median of 15 months (IQR, 6-31 months). Patients with bilateral AH were more likely to have underlying coagulopathy (44% vs 3%) and to develop primary adrenal insufficiency (72% vs 0%) than those with unilateral AH (P<.001). Conclusion Often, AH presents as an incidental unilateral lesion with normal adrenal function, commonly attributed to postoperative complications or trauma. In contrast, bilateral AH is rare and typically linked to underlying coagulopathy, with primary adrenal insufficiency developing in most patients.

Abstract:Background Although uncommon, adrenal hemorrhage has multiple etiologies. Because clinical characteristics, management, and outcomes of patients with adrenal hemorrhage are inadequately described, we examined the underlying etiology, need for intervention, evolution of imaging characteristics, and adequacy of subsequent evaluation. Methods We performed a retrospective review of patients diagnosed with adrenal hemorrhage (radiologist-confirmed density consistent with hemorrhage on computed tomography) from 2005 to 2021 at a university-based institution. Demographic characteristics, hemorrhage etiology, and subsequent follow-up were analyzed. Results Of 193 adrenal hemorrhage patients, the mean age was 49.2 ± 18.3 years, and 35% were female. Clinical presentations included trauma (47%), abdominal or flank pain (28%), incidental findings on imaging acquired for other reasons (12%), postoperative complication (8%), or shock (3%). Hemorrhage outside of the gland was present in 62% of patients. Unilateral hemorrhage was more frequent (93%) than bilateral (7%). A total of 12% of patients had nodules, but only 70% of these were identified on initial imaging, and only 43% had hormonal evaluation. Of 7 patients who had adrenalectomy or biopsy, pathology was either benign (57%) or nonadrenal malignancy (43%). No adrenocortical carcinomas were identified. Follow-up imaging was performed in 56% of patients and revealed decreased, stable, resolved, or increased adrenal hemorrhage size in 39%, 19%, 30%, and 12% of patients, respectively. Conclusion Adrenal hemorrhage is secondary to multiple etiologies, most commonly trauma. In the setting of adrenal hemorrhage, many adrenal nodules were not identified on initial imaging. Only a minority of patients with nodules underwent "complete" biochemical evaluation. Follow-up imaging may improve the identification of underlying nodules needing hormonal evaluation.

Abstract:A spontaneous adrenal hemorrhage is a rare complication occurring in the setting of interventional radiology procedures. Here, we present the case of a 73-year-old male who underwent CT-guided drainage of a periduodenal collection. During the procedure, he developed sudden onset left back pain and hypertension, which revealed a left adrenal gland hemorrhage on CT imaging. Potential mechanisms for this complication include a physiological stress response, procedure-associated hypercoagulability, and direct trauma to the adrenal gland. Moreover, an adrenal hemorrhage should be monitored closely as it can lead to severe clinical consequences requiring treatment with IV analgesics and antihypertensives.
