Abstract:Fatigue bezeichnet einen andauernden Erschöpfungszustand, der auf eine Infektionskrankheit folgen kann. Sie zählt zu den häufigsten Symptomen beim Post-Covid-Syndrom (PCS). Auch bei psychischen Erkrankungen kann Fatigue auftreten, allerdings ist Fatigue, wie andere chronische Erkrankungen, auch selbst ein Risikofaktor für Depressivität und Ängstlichkeit. Ziel der Analyse ist es, zu untersuchen, inwieweit sich Fatigue und Depressivität/Ängstlichkeit gegenseitig bedingen und ob es Unterschiede zwischen PCS-Betroffenen und vollständig Genesenen gibt.

Impact of fatigue as the primary determinant of functional limitations among patients with post-COVID-19 syndrome: a cross-sectional observational study

Abstract:Objectives To describe self-reported characteristics and symptoms of treatment-seeking patients with post-COVID-19 syndrome (PCS). To assess the impact of symptoms on health-related quality of life (HRQoL) and patients' ability to work and undertake activities of daily living. Design Cross-sectional single-arm service evaluation of real-time user data. Setting 31 post-COVID-19 clinics in the UK. Participants 3754 adults diagnosed with PCS in primary or secondary care deemed suitable for rehabilitation. Intervention Patients using the Living With Covid Recovery digital health intervention registered between 30 November 2020 and 23 March 2022. Primary and secondary outcome measures The primary outcome was the baseline Work and Social Adjustment Scale (WSAS). WSAS measures the functional limitations of the patient; scores of ≥20 indicate moderately severe limitations. Other symptoms explored included fatigue (Functional Assessment of Chronic Illness Therapy–Fatigue), depression (Patient Health Questionnaire–Eight Item Depression Scale), anxiety (Generalised Anxiety Disorder Scale, Seven-Item), breathlessness (Medical Research Council Dyspnoea Scale and Dyspnoea-12), cognitive impairment (Perceived Deficits Questionnaire, Five-Item Version) and HRQoL (EQ-5D). Symptoms and demographic characteristics associated with more severe functional limitations were identified using logistic regression analysis. Results 3541 (94%) patients were of working age (18-65); mean age (SD) 48 (12) years; 1282 (71%) were female and 89% were white. 51% reported losing ≥1 days from work in the previous 4 weeks; 20% reported being unable to work at all. Mean WSAS score at baseline was 21 (SD 10) with 53% scoring ≥20. Factors associated with WSAS scores of ≥20 were high levels of fatigue, depression and cognitive impairment. Fatigue was found to be the main symptom contributing to a high WSAS score. Conclusion A high proportion of this PCS treatment-seeking population was of working age with over half reporting moderately severe or worse functional limitation. There were substantial impacts on ability to work and activities of daily living in people with PCS. Clinical care and rehabilitation should address the management of fatigue as the dominant symptom explaining variation in functionality.

Abstract:Considering the relevance for patients, economics and public health data about the course of the neurological Post-COVID Syndrome (PCS) are urgently needed. In this study 94 PCS patients (73% female, age in median 49 years) were examined in median 9.4 (T1) and for a second time 14 months (T2) after mild to moderate SARS-CoV-2 infection. Mood, sleep quality and health related quality of life (QoL) were evaluated via structured anamnesis and self-report questionnaires; attention, concentration and memory via psychometric tests. 47% of the patients reported an improvement of their symptoms over time, but only 12% full recovery. 4% noticed deterioration and 49% no change. Main disturbances at both time points were fatigue, deficits in concentration and memory. In patients with perceived improvement QoL significantly increased between T1 and T2, although their test performance as well as the fatigue score remained unchanged. In patients with persisting impairment QoL, fatigue scores and psychometric test results did not change significantly. Abnormal psychometric tests were more frequent at both time points in the group without improvement. But, significant fatigue and cognitive impairment persisted for more than 1 year after SARS-CoV-2 infection in both groups.

Abstract:Adrenal dysfunction in sepsis, described for over a century and previously termed relative adrenal insufficiency, now is labeled critical illness-related corticosteroid insufficiency (CIRCI) [1]. The history of the investigation, data, and practice patterns revolving around the adrenal gland and corticosteroid supplementation in septic shock is a tour de force in evidence-based uncertainty. Corticosteroid replacement in sepsis has seen an evolution in dose (high vs. low), dosing strategies (bolus dosing vs. infusion, taper vs. no taper), duration of therapy, and guideline recommendations [2]. High dose steroids were commonly given until the mid-1990s when data revealed not only a lack of benefit, but harm associated with this strategy [3]. On the heels of smaller studies showing earlier shock reversal and potential mortality benefit with low-dose steroids, a multicenter randomized controlled trial (RCT) showed a 10% absolute risk reduction in mortality for septic shock patients with relative adrenal insufficiency [4]. These results were not duplicated by another multicenter RCT, which differed significantly from the Annane trial [5]. Accordingly, as these data have evolved, the “official” recommendations have changed from “….are recommended….” (2004) to “We suggest not using…..” (2012) [6, 7]. Data suggests that CIRCI increases mortality, yet it is difficult to say who even has it, given the testing limitations to diagnose it [8]. So at least two questions still remain: 1) Does this patient have CIRCI?”; and 2) “Should I dose this particular patient with steroids?”. The present study is an attempt to tackle the first question [9]. Using a post-hoc analysis, the authors aimed to use computer-assisted modeling to examine free cortisol appearance and elimination rates [maximal cortisol secretion rate (CSRmax)] and free cortisol half-life]. Subjects included septic shock (n=45), sepsis (n= 25), and healthy controls (n= 10), and were given a cosyntropin stimulation test within 24 hours of sepsis diagnosis. Survivors also received cosyntropin testing at hospital discharge and as outpatients. Contrary to the primary hypothesis, CSRmax was higher in septic shock than in sepsis and healthy controls, and this decreased between the first and second cosyntropin test. The free cortisol half-life was also longer in septic shock and sepsis vs. controls. The strengths of the study are its accuracy when compared to isotope dilution, as well as its attempt at better refining this topic by assessing the kinetics of cortisol parameters. It also has weaknesses. The small numbers and lack of patient-level details make extrapolation of this data beyond this manuscript impossible. We do know that the original study from which this post-hoc data was obtained excluded patients with hepatic and liver disease, a fact that further limits generalizability. The use of healthy subjects as a comparator group can also be debated as well. Furthermore, there was a significant age difference between the healthy controls and the sepsis patients, raising further questions about an accurate comparator. There was also no difference in the CSRmax between sepsis patients and healthy volunteers. Given the abundance of data regarding sepsis, inflammation, and the adrenal axis, this makes little intuitive sense. Finally, while this study is an attempt at better diagnosing CIRCI, the variance in CSRmax in the septic shock group, shows some rather high between-subject variability. This suggests that this approach will either be inaccurate in a significant percentage of patients, and/or needs further refinement. What do we do with these results? While they add to our knowledge base regarding the possible diagnosis of CIRCI, practice cannot be changed based on these data. At the end of the day, even if we can accurately diagnoses CIRCI, it still doesn’t mean that patients will benefit from exogenous steroid replacement. Anemia is easily diagnosed and associated with worse outcome in critically ill patients, yet transfusion does not improve outcome. I believe the decision to treat septic shock patients for possible CIRCI should revolve around the question: “Is my patient more similar to the Annane trial (early septic shock with ≥ 60% mortality risk) or the CORTICUS trial?” If the former (an absolute minority of sepsis patients), then dosing with steroids can be considered. Until our specialty is better at tailoring therapy to an individual, it will be difficult to answer these questions. In the meantime, I thank the authors of this work for advancing the science and inching us closer.

Abstract:Fatigue is a pervasive symptom experienced by many individuals after COVID-19. Despite its widespread occurrence, fatigue remains a poorly understood and complex phenomenon. Our aim is to evaluate the subjective experience of mental fatigue after COVID-19 and to assess its significance for daily life functioning. In this online questionnaire study ( N = 220), the Fatigue Severity Scale (FSS), World Health Organization Quality-of-Life assessment (WHOQoL) and a subjective severity rating of the COVID-19 disease progression were used. For our statistical analyses we utilized independent samples t-tests, one-way ANOVA with post-hoc analyses, and a multiple regression. As expected our findings revealed the COVID group reported significantly higher levels of subjective fatigue compared to the control group. Moreover, there was a significant difference between experienced fatigue across the four severity groups. Participants who had a milder course of disease also experienced severe subjective fatigue. Subjective fatigue explained 40% variance in quality-of-life. In conclusion, severe subjective fatigue appears to be associated with increased self-reported COVID-19 symptom severity and lower quality-of-life but is already observable in milder cases. This underscores, firstly, the importance of considering also less severe cases and, secondly, the need to develop rehabilitation and psychological interventions for fatigue.

Abstract:Objective Fatigue has been identified as the core symptom of long-Covid, however, putative pandemic-related influences remain largely unclear. We investigated trajectories of total, physical and mental fatigue and the factors associated with it in previously infected and non-infected individuals up to one year post- infection. Methods We used data from a longitudinal cohort study of German adults with two samples: A representative probability sample and a sample of individuals with proven SARS-CoV-2 infection. Surveys were conducted in spring 2020(T1), autumn 2020(T2) and summer 2021(T3). Fatigue was assessed using the FAS, distinguishes between physical and mental fatigue. Depression, anxiety and stress were assessed using PHQ-4 and PSQ. Results 1990 participants [mean age 47.2 (SD = 17.0), 30.5% previously infected] were included in the survey at T1 ( n = 1118 at T2, n = 692 at T3). Total and physical fatigue, but not mental fatigue were significantly higher in the previously infected compared to the non-infected sample at T2, but this group difference disappeared at T3. We identified Covid-infection as a factor associated with transient total and physical fatigue at T2. Depression, anxiety and stress at T1 were associated with total, physical and mental fatigue at both follow-ups. Conclusions Our results highlight the importance of considering physical and mental fatigue as separate entities, while suggesting a greater relevance of the physical signs of fatigue in understanding long-Covid. The results further showed that baseline mental health symptoms were the most strongly associated with fatigue trajectories.

Abstract:Background/Objectives: A significant number of patients experience prolonged symptoms following COVID-19 in particular cases of fatigue. Yet, pathomechanisms of COVID-19-related fatigue remain unclear. Methods: This study investigated patients after confirmed SARS-CoV-2 infection (PC) with relevant fatigue according to the Fatigue Assessment Scale (≥22 points) to rule out malnutrition as a driving factor for fatigue and to evaluate daily activity and sleep characteristics. Dietary behavior was recorded through food diaries and physical activity by self-reported (questionnaires) and objective (activity tracker) outcomes. Data were collected over a 7-day period and compared with a healthy control group (HC). A subgroup analysis of patients with fatigue and severe fatigue, as well as a sex-specific analysis, were included. Results: No significant differences in dietary intake were observed, but an indication toward a healthier Mediterranean diet in PC patients with a median Mediterranean Diet Score of 4 (IQR 3, 5) in HC vs. 5 (IQR 3, 6) in PC (p = 0.24). There were also no differences in physical activity, either by objective or subjective measures. However, the median sleep duration was 49 min longer in PC patients (p = 0.003). Conclusions: In conclusion, malnutrition did not significantly contribute to fatigue, yet patients with COVID-19-related fatigue showed increased sleep duration. As sleep characteristics play a crucial role in mental and physical wellbeing, the association of sleep, physical activity, and fatigue should be evaluated in further studies.

Abstract:Introduction With the prolongation of the COVID‐19 pandemic, more individuals are experiencing sequelae after COVID‐19 infection, also known as post‐acute COVID‐19 syndrome (PCS). The aims of this study were to describe the prevalence and characteristics of PCS symptoms such as fatigue, anxiety, and depression and to compare these symptoms according to participant characteristics in patients who had been previously hospitalized due to COVID‐19. Design A descriptive cross‐sectional study design was used. Methods We included 114 individuals who had been hospitalized for COVID‐19 and were discharged from the hospital at least 4 weeks before. Symptoms were assessed using the Fatigue Severity Scale, the Hospital Anxiety‐Depression Scale, and the PCS symptom questionnaire developed by the authors. We used descriptive statistics, the Student's t‐test, the Wilcoxon rank‐sum test, and the Kruskal–Wallis test for statistical analyses. Results The most prevalent symptoms were anxiety (66.7%), fatigue (64.0%), headache (57.9%), and concentration or memory difficulties (57.9%). Concentration or memory difficulties and sleep disturbances had the highest mean frequency. Concentration or memory difficulties were rated with the highest mean severity, and cough, loss of taste, and muscle and joint pain had the highest mean distress scores. Female participants, individuals hospitalized for more than 2 weeks, individuals discharged more than 9 months ago, unvaccinated patients, and those who tried at least one symptom relief method reported higher symptom distress. Conclusion The findings of this investigation into the frequency, severity, and distress of symptoms shed light on the identification of post‐COVID symptoms in detail. To objectively evaluate and comprehend the symptom trajectories of PCS, prospective studies about the development of symptom assessment tools and studies with a longitudinal design should be conducted. Clinical Relevance A substantial number of respondents reported numerous symptoms and expressed symptom distress; therefore, the development of nursing interventions and treatments to alleviate PCS symptoms is crucial.

Abstract:Ziel Die Post-COVID-Sprechstunde (PCS) wird im Rahmen eines umfassenden Behandlungsangebotes der gesetzlichen Unfallversicherung für Post-COVID-Patient*innen durchgeführt, um individuelle Empfehlungen für die weitere Versorgung festzulegen. Ziel der Untersuchung war die Erfassung der Hauptsymptome sowie die damit verbundenen Einschränkungen der sozialen und beruflichen Teilhabe, um Konsequenzen für die ambulante Rehabilitation abzuleiten. Methodik Neben einer ärztlichen Untersuchung sowie einem psychologischen Konsil wurden zahlreiche Assessments zur Evaluation des biopsychosozialen Gesundheitszustands durchgeführt. 373 weibliche (82,2%) und 81 männliche Patient*innen im Alter zwischen 40 und 60 Jahren aus den Berufen der Gesundheits- und Pflegedienste, der Erziehung und Pädagogik nahmen seit April 2021 an der PCS teil. Ergebnisse Nahezu alle untersuchten Patient*innen (98,2%) gaben Fatigue als Kardinalsymptom ihrer Post-COVID-Beschwerden an, in über 73% der Fälle in Kombination mit subjektiv erlebten Einschränkungen der Hirnleistungsfähigkeit. Die Dauer der Symptomatik persistierte dabei sowohl bei den weiblichen als auch männlichen Versicherten durchschnittlich seit 14–15 Monaten. Somit sind über 85% der Gesamtstichprobe als Fälle chronischer Erschöpfung (Fatigue Skala) einzustufen. Die Schwere der Fatigue beeinflusst zudem proportional die Lebensqualität (SF-36), Gefühle von Angst und Depression (HADS), die psychische Widerstandskraft (RS-13) sowie motorische Parameter wie die maximale Greifkraft und die Ausdauerleistungsfähigkeit. 54,3% der Versicherten erhielten zudem eine psychische Verdachtsdiagnose und 38,1% die Empfehlung zu einer weiterführenden neuropsychologischen Diagnostik. Schlussfolgerung Zur weiteren Behandlung des Leitsymptoms der chronischen Fatigue ist eine multimodale und interdisziplinäre ambulante Rehabilitation zu empfehlen, die sich an der Behandlung der Diagnose des Chronischen Fatiguesyndroms (ME/CFS) und somit insbesondere an einem psychoedukativen und weniger an einem kurativen Therapieansatz orientieren und Nachsorgestrategien berücksichtigen sollte. Gesicherte psychische Störungen und neuropsychologische Defizite sind begleitend zu behandeln. Objective The post-COVID consultation (PCC) is offered as part of a comprehensive range of treatment services provided by the statutory accident insurance for post-COVID patients to determine individual recommendations for further care. The aim of the study was to record the main symptoms and the associated restrictions on social and occupational participation in order to derive consequences for outpatient rehabilitation. Method In addition to a medical examination and a psychological consil, numerous assessments were carried out to evaluate the biopsychosocial state of health. 373 female (82.2%) and 81 male patients aged between 40 and 60 years from the professions of health and care services, education and pedagogy participated in the PCC since April 2021. Results Nearly all patients (98.2%) reported fatigue as a cardinal symptom of their post-COVID complaints, in combination with subjectively experienced limitations in brain functioning in over 73% of cases. The duration of the symptomatology persisted for an average of 14–15 months in both female and male insured persons. Thus, over 85% of the total sample can be classified as cases of chronic fatigue (Fatigue Scale). The severity of fatigue also proportionally affects quality of life (SF-36), feelings of anxiety and depression (HADS), psychological resilience (RS-13), and motor parameters such as maximum grip strength and endurance capacity. 54.3% of the patients also received a suspected mental diagnosis and 38.1% a recommendation for further neuropsychological diagnostics. Impact of pre-existing conditions on the severity of post-COVID syndrome among workers in healthcare and social services in Germany

Abstract:Background: The underlying mechanisms of post-COVID syndrome (PCS) are still not fully understood. While pre-existing conditions have been described as a risk factor for severe PCS in the general population, data specific to different occupational groups in this context is lacking. This study aimed to investigate the impact of pre-existing conditions on severe post-COVID syndrome among the occupational group of healthcare and social services employees. Methods: Baseline data from a longitudinal, observational study were analysed. In February 2021, health workers who had a COVID-19 infection in 2020 were surveyed about sequelae of the infection. Factors influencing severe PCS with at least one persistent symptom categorised as severe were subjected to a multivariate logistic regression analysis. Results: Of a total of 2,053 participants, 21.5% had severe PCS. Underlying respiratory (OR 1.94; CI 1.44-2.61), cardiovascular (OR 1.35; CI 1.04-1.77) and urogenital (OR 1.79; CI 1.10-2.91) disease were risk factors for severe PCS overall. Respiratory and mental illnesses had a statistically significant impact on persistent fatigue/exhaustion, concentration/memory difficulties and shortness of breath categorised as severe. Urogenital disease was associated with severe fatigue/exhaustion. Other significant risk factors for severe PCS were female sex, smoking, physical exercise and hospitalisation due to COVID-19 infection. Conclusion: Workers in healthcare and social services with pre-existing conditions may face a higher risk of developing severe PCS. Additional analyses performed as part of the longitudinal study will show if and how this result changes over time.
Bentall operation in 375 patients: long-term results and predictors of death.

Abstract:BACKGROUND AND AIM OF THE STUDY The Bentall operation is a 40-year-old standardized procedure for treating aortic valve diseases and aneurysms involving the aortic root. The study aim was to analyze the results and predictors of long-term outcome after the Bentall procedure for aortic root diseases. METHODS Between January 1990 and December 2007, a total of 375 patients (296 males, 79 females) underwent the Bentall operation at the authors' institution. Bicuspid aortic valve (BAV) was present in 91 patients, and Marfan syndrome in 13. Thirty-six patients were treated as emergencies, and 30 for acute dissection. A concomitant surgical procedure was performed in 78 patients. The operative procedure included both classic Bentall and button techniques. Follow up data were obtained from hospital and office records and from telephone contacts. Kaplan-Meier survival analysis and Cox regression analysis were performed to investigate the predictors of long-term outcome. RESULTS The overall in-hospital mortality was 4.5%, and after elective operations was 2.3%. A 20-year long-term follow up included 32 late deaths, of which 14 were cardiac-related. Freedom from late all-cause mortality at 5, 10, and 15 years was 97.1%, 81.9%, and 53.9%, respectively. At univariate analysis, long-term mortality was associated with age, diabetes, BAV, NYHA class III/IV, emergency treatment, cardiopulmonary bypass time, and coronary artery bypass grafting. Independent predictors of long-term mortality were age (OR 1.16; CI: 1.08-1.23), emergency surgery (OR 28; CI: 4-192) and BAV (OR 3; CI: 1.3-6.9). CONCLUSION The Bentall procedure is a safe and durable operation, with a very good early and long-term results and a low rate of reoperation. In the present series, age, BAV and emergency surgery were important independent predictors of mortality.

Abstract:Objectives The objectives of this study were to examine post COVID-19 syndrome (PCS) among healthcare workers (HCWs) in England and explore risk factors for the condition. Methods Data were collected by National Health Service (NHS) CHECK, a longitudinal study exploring HCWs' mental and physical well-being during and after the COVID-19 pandemic. NHS CHECK collected data at four timepoints: the baseline survey between April 2020 and January 2021, and then three follow-up surveys at approximately 6, 12 and 32 months post baseline. PCS data were collected at 12 and 32 months, while risk factor data were from baseline. HCWs were asked what COVID-19 symptoms they experienced and for how long and were classified as having PCS if they had any symptom for ≥12 weeks. Multilevel regressions were used to examine risk factors for PCS. Results This study included 5248 HCWs. While 33.6% (n=1730) reported prolonged COVID-19 symptoms consistent with PCS, only 7.4% (n=385) reported a formal diagnosis of PCS. Fatigue, difficult concentrating, insomnia and anxiety or depression were the most common PCS symptoms. Baseline risk factors for reporting PCS included screening for common mental disorders, direct contact with COVID-19 patients, pre-existing respiratory illnesses, female sex and older age. Conclusions While a third of HCWs reported prolonged COVID-19 symptoms consistent with PCS, a smaller percentage reported a formal diagnosis of the condition. We replicate findings that direct contact with COVID-19 patients, older age, female sex, pre-existing respiratory illness and symptoms of common mental disorders are associated with increased risk of PCS.

Abstract:Abstract Fatigue in its many forms of physical, mental, and psychosocial exhaustion is a common symptom of post-COVID-19 condition, also known as “Long COVID.” Persistent fatigue in COVID-19 patients is frequently accompanied by cognitive dysfunction and neuropsychiatric symptoms; however, less is known about the relationships between these components of post-COVID-19 condition and fatigue itself. Consequently, the present study sought to (1) distinguish the types of fatigue experienced by participants, and (2) investigate whether cognitive deficits across various domains and neuropsychiatric conditions predicted these different types of fatigue. The study included 136 COVID-19 patients referred for neuropsychological evaluation due to cognitive complaints 8 months on average after SARS-CoV-2 infection. Measures included self-reported fatigue (physical, cognitive, and psychosocial), neuropsychiatric questionnaires (assessing symptoms of depression, anxiety, apathy, and executive functioning), a comprehensive neuropsychological assessment, and self-reported quality of life and everyday functioning. Results showed that reports of clinical significant fatigue were pervasive in our sample (82.3% of participants), with physical fatigue rated highest on average relative to the subscale maximum. Elevated levels of apathy, anxiety, and executive dysfunction in neuropsychiatric measures along with executive and attentional difficulties on cognitive tests were found to be consistently important predictors among different types of fatigue. This implicates both cognitive and neuropsychiatric symptoms as predictors of fatigue in post-COVID-19 condition, and stresses the importance of a holistic approach in assessing and considering potential treatment for COVID-19 patients experiencing fatigue.

Abstract:Background: Post-COVID syndrome (PCS) is defined by symptom persistence accompanied by daily life impairment (DLI). The association of somatic symptom disorder (SSD) and symptoms with DLI after SARS-CoV-2 infection in the general population is unclear to date. The main objective of the study was to investigate the association of possible SSD, depression, anxiety, and participant-reported symptoms with DLI in a local population sample. Methods: Anonymised cross-sectional study. A symptom questionnaire, including the scales Patient Health Questionnaire PHQ-15 (somatisation module), SSD-12 (psychological distress in SSD), PHQ-2 (depression), GAD-2 (anxiety), and FAS (fatigue assessment scale) was sent in 02/2022 to all adult residents of the district Bad Tölz-Wolfratshausen, Germany, who were registered for SARS-CoV-2-infection between 03/2020 and 11/2021 (8925 delivered). Associations between DLI, symptoms and scales were estimated using binary logistic regression models and network analysis. Results: 2828 questionnaires (31.7%) were complete. 1486 (52.5%) reported persistent symptoms, and 509 (18.0%) perceived DLI. DLI was strongest associated with self-reported fatigue (OR 7.86; 95%CI 5.63-10.97), dyspnea (3.93; 2.73-5.67), impaired concentration (3.05; 2.17-4.30), SSD-12 (4.36; 2.57-7.41), and PHQ-2 (2.48; 1.57-3.92). Self-reported fatigue showed the strongest correlation (rp = 0.248) and closest proximity to DLI in network analysis. Conclusion: PCS appears as a complex clinical picture in which SSD might play an important role when DLI is present. The pychological burden might partly be explained by the persistent symptoms, which are difficult to treat up to now. Screening for SSD could help in differential diagnostic decision-making to ensure that patients receive appropriate psychosocial interventions for disease coping.
Burden of post-COVID-19 syndrome and implications for healthcare service planning: A population-based cohort study

Abstract:Background Longer-term consequences after SARS-CoV-2 infection are becoming an important burden to societies and healthcare systems. Data on post-COVID-19 syndrome in the general population are required for the timely planning of healthcare services and resources. The objective of this study was to assess the prevalence of impaired health status and physical and mental health symptoms among individuals at least six months after SARS-CoV-2 infection, and to characterize their healthcare utilization. Methods This population-based prospective cohort study (Zurich SARS-CoV-2 Cohort) enrolled 431 adults from the general population with polymerase chain reaction-confirmed SARS-CoV-2 infection reported to health authorities between 27 February 2020 and 05 August 2020 in the Canton of Zurich, Switzerland. We evaluated the proportion of individuals reporting not to have fully recovered since SARS-CoV-2 infection, and the proportion reporting fatigue (Fatigue Assessment Scale), dyspnea (mMRC dyspnea scale) or depression (DASS-21) at six to eight months after diagnosis. Furthermore, the proportion of individuals with at least one healthcare contact after their acute illness was evaluated. Multivariable logistic regression models were used to assess factors associated with these main outcomes. Results Symptoms were present in 385 (89%) participants at diagnosis and 81 (19%) were initially hospitalized. At six to eight months, 111 (26%) reported not having fully recovered. 233 (55%) participants reported symptoms of fatigue, 96 (25%) had at least grade 1 dyspnea, and 111 (26%) had DASS-21 scores indicating symptoms of depression. 170 (40%) participants reported at least one general practitioner visit related to COVID-19 after acute illness, and 10% (8/81) of initially hospitalized individuals were rehospitalized. Individuals that have not fully recovered or suffer from fatigue, dyspnea or depression were more likely to have further healthcare contacts. However, a third of individuals (37/111) that have not fully recovered did not seek further care. Conclusions In this population-based study, a relevant proportion of participants suffered from longer-term consequences after SARS-CoV-2 infection. With millions infected across the world, our findings emphasize the need for the timely planning of resources and patient-centered services for post-COVID-19 care.

Abstract:Post-COVID syndrome (PCS) is characterized by a variety of non-specific symptoms. One of the leading symptoms is fatigue. So far, there is no evidence-based causal therapy established and treatment of PCS is primarily symptom-oriented. The Clinic for Internal and Integrative Medicine in Bamberg, Germany, offers a comprehensive multimodal integrative inpatient therapy for PCS patients. Within a prospective uncontrolled observational study, the results of N = 79 patients were analysed. Post-COVID fatigue patients were hospitalized for up to 14 days. The treatment consists of individual modules depending on the patient's needs. It includes a wide range of integrative non-pharmacological treatment modalities. Outcomes were assessed before and after the inpatient treatment as well as 6 months after discharge from the hospital. Results show that fatigue of post-COVID patients in this study ( M = 76.30, SD = 10.18, N = 64) was initially significantly higher than in the subsample "women aged 60–92 years" of the general German population ( M = 51.5, Schwarz et al. [Schwarz et al. in Onkologie 26:140–144, 2003]; T(63) = 19.50, p < .001). Fatigue was significantly and clinically relevant reduced directly after discharge (M T1 = 76.21, SD = 11.38, N = 42; M T2 = 66.57, SD = 15.55, N = 42), F(1, 41) = 19.80, p < .001, partial eta squared = .326, as well as six months after discharge (M T3 = 65.31, SD = 17.20, N = 42), F(1, 41), p < .001, partial eta squared = .371. Additionally, self-reported ability to work (NRS, 0–10) improved significantly from admission (M T1 = 2.54, SD = 2.23, N = 39) to discharge (M T2 = 4.26, SD = 2.60, N = 39), F(1, 38) = 26.37, p < .001, partial eta squared = .410), as well as to six months later (M T3 = 4.41, SD = 3.23, N = 39), F(1, 38) = 15.00, p < .001, partial eta squared = .283. The study showed that patients suffering from chronic post-COVID syndrome for several months can achieve a significant improvement in their leading fatigue symptoms and a significant improvement in the subjective assessment of their ability to work through a comprehensive two-week multimodal integrative inpatient program.

Abstract:Introduction Cognitive changes are very frequently reported by people with post‐COVID‐19 syndrome (PCS), but there is limited understanding of the underpinning mechanisms leading to these difficulties. It is possible that cognitive difficulties are related to immune status and/or low mood. The aim of the present study was to examine the relationship between immune status and cognitive functioning in PCS, while considering whether depression symptoms also influence this association. Methods Participants were recruited in an online study of cognitive and psychological consequences of PCS, involving individuals attending a post‐COVID clinic in an acute general hospital in Ireland, and a comparison sample of age‐ and sex‐matched community controls who had also been infected with COVID‐19 but had not experienced PCS. Participants with PCS (n = 71) and community controls (n = 50) completed the immune status questionnaire, Cognitive Failures Questionnaire (CFQ), Hospital Anxiety and Depression Scale (HADS), and the Functional Assessment of Chronic Illness Therapy‐Fatigue Scale (FACIT‐F). Results Significant differences were observed between groups in terms of perceived immune status, perceived cognitive function, depression scores, and fatigue, with the "PCS" group reporting lower immune status, more cognitive difficulties, and higher levels of depression and fatigue. Regression analysis in the PCS group indicated that immune status and depression significantly contributed to variance in subjective cognitive functioning, with immune status remaining a significant predictor of cognitive functioning scores even when accounting for depression, fatigue, and other covariates related to PCS, such as Body Mass Index (BMI). Conclusion Our findings suggest that subjective cognitive functioning is influenced by self‐reported immune status in PCS, emphasising the importance of immune status, cognitive, and mood screening as part of routine clinical care in PCS.
