Abstract:Chiari malformation type III is a rare congenital anomaly. It consists of the posterior fossa contents herniation through an occipital or high cervical encephalocele sac. Although it has traditionally been associated with a high mortality rate, the absence of certain poor prognostic factors and appropriate medical and surgical treatment allow these children to survive and have a remarkable initial functional improvement. Surgical goals are defect repair, preservation of viable brain tissue, adequate skin coverage and hydrocephalus management. Despite all of this, the tendency of these newborns is to maintain a significant disability and if they present poor prognostic criteria, they usually demise within a short period of time. We report the case of a newborn with Chiari malformation type III diagnosed during pregnancy. After characterizing the anomaly with a postnatal MRI, the encephalocele was excised and multi-layer closure was performed. The patient progressively developed hydrocephalus during the postoperative period and required ventriculoperitoneal shunt placement. After an initial uneventful course, our patient suffered several episodes of respiratory disturbances. The child became ventilator dependent and palliative care was established in agreement with the parents after ruling out shunt malfunction.
Meckel-Gruber syndrome together with Dandy-Walker malformation: an atypical case report of a 2nd recurrence in a consanguine marriage

Abstract:Meckel-Gruber syndrome is a lethal disorder characterized by occipital encephalocele, polycystic kidneys, and polydactyly. In most cases, it is identified and terminated antenatally. In this report, the authors present a case of Meckel-Gruber syndrome together with Dandy-Walker malformation. A pregnant woman referred at the 28th week of gestation with an abnormal ultrasound scan showing posterior encephalocele and bilaterally enlarged kidneys. Further imaging also indicated communication between the 4th ventricle and posterior cerebellar cerebrospinal fluid space, after which the fetus was diagnosed with Meckel-Gruber syndrome and Dandy-Walker malformation. Pregnancy termination was refused by the parents and the offspring was prematurely born to be the 2nd recurrence of Meckel-Gruber syndrome in this consanguine family. Remarkably, at the 3 different pregnancies, ultrasound was inconclusive before the 7th month of gestation. Though up to date Meckel-Gruber syndrome is ultimately lethal, the lifespan of affected newborns varied greatly. We suggest developing a severity classification to estimate life expectancy in unterminated cases.
Neurosurgical management of Myelomeningocele in premature infants: a case series

Abstract:Introduction: Myelomeningocele (MMC) is the most common neural tube defect, but rarely seen in premature infants. Most centers advocate for closure of MMC within 24 h of birth. However, this is not always possible in severely premature infants. Given the rarity of this patient population, we aimed to share our institutional experience and outcomes of severely premature infants with MMC. Methods: We performed a retrospective, observational review of premature infants (≤ 32 weeks gestational age) identified through our multidisciplinary spina bifida clinic (1995-2021) and surgical logs. Descriptive statistics were compiled about this sample including timing of MMC closure and incidence of adverse events such as sepsis, CSF diversion, meningitis, and death. Results: Eight patients were identified (50% male) with MMC who were born ≤ 32 weeks gestational age. Mean gestational age of the population was 27.3 weeks (SD 3.5). Median time to MMC closure was 1.5 days (IQR = 1-80.8). Five patients were taken for surgery within the recommended 48 h of birth; 2 patients underwent significantly delayed closure (107 and 139 days); and one patient's defect epithelized without surgical intervention. Six of eight patients required permanent cerebrospinal fluid (CSF) diversion (2 patients were treated with ventriculoperitoneal shunting (VPS), three were treated with endoscopic third ventriculostomy (ETV) with choroid plexus cauterization (CPC) and 1 patient treated with ETV; mean of 3 years after birth, ranging from 1 day to 16 years). Two patients required more than one permanent CSF diversion procedure. Two patients developed sepsis (defined as meeting at least 2/4 SIRS criteria). In both cases of sepsis, patients developed signs and symptoms more than 72 h after birth. Notably, both instances of sepsis occurred unrelated to operative intervention as they occurred before permanent MMC closure. Two patients had intraventricular hemorrhage (both grade III). No patients developed meningitis (defined as positive CSF cultures) prior to MMC closure. Median follow up duration was 9.7 years. During this time epoch, 3 patients died: Two before 2 years of age of causes unrelated to surgical intervention. One of the two patients with grade III IVH died within 24 h of MMC closure. Conclusions: In our institutional experience with premature infants with MMC, some patients underwent delayed MMC closure. The overall rate of meningitis, sepsis, and mortality for preterm children with MMC was similar to MMC patients born at term.

Abstract:Spina bifida is a neural tube defect that occurs in about 1 in 1,500 pregnancies. Open spinal defects are associated with paralysis, incontinence and hydrocephalus requiring postnatal shunting of the cerebrospinal fluid. Neural tube defects are preventable through preconceptional folic acid supplementation. Occasionally, the diagnosis is made during routine anomaly scan at 18-20 weeks of gestation, as the earliest signs of the anomaly can be seen at the end of first trimester. The information provided by ultrasound plays a crucial role in patient counseling and pregnancy management. The authors report a case of prenatal diagnosis of spina bifida aperta with focus on detailed ultrasound presentation and difficulties in consulting in relation to the prognosis for the newborns.

Abstract:Background: To summarize the clinical experience of intracranial aneurysm clipping in the treatment of ruptured intracranial aneurysms in the second trimester of pregnancy. Methods: A case of ruptured middle cerebral aneurysm in the second trimester of pregnancy was reported. Craniotomy and aneurysm clipping were performed at 24 weeks of pregnancy, and fetal preservation was continued after the operation. Results: The prognosis of the parturient was good and the skull was missing on the operative side. A healthy baby boy was delivered by cesarean section 2 months after the operation, and skull repair was performed 4 months after the operation. During the follow-up for 1 year, the mother and son were healthy and no obvious sequelae were found. Conclusion: Ruptured intracranial aneurysm hemorrhage in mid-pregnancy is a rare and critical case. Summarizing the corresponding clinical experience will help to have a reference plan for the next time when facing a similar situation, and it will help to treat critically ill patients. The treatment of ruptured intracranial aneurysm in mid-pregnancy requires multidisciplinary collaboration, and cranial aneurysm clamping + fertility preservation can reduce the impact of radiation on the fetus and improve the prognosis for both the mother and the fetus.

[Congenital anomalies of the central nervous system--from the pediatric standpoint].

Abstract:Congenital anomalies of the central nervous system frequently cause severe emotional, economic, and physical hardship for both the child and the family. Craniofacial and developmental abnormalities may cause social unacceptability. Congenital hydrocephalus may result in neonatal death or mental and motor disabilities. Frequent hospitalization of prolonged institutionalization may be necessary for the many surviving patients. With recent advances in the diagnostic techniques of amniocentesis, fetoscopy, ultrasound, computerized tomography (CT) and magnetic resonance imaging (MRI), an improved accuracy in the diagnosis of fetal abnormalities and in the determination of prognosis is possible in many instances. When confronted with the diagnosis of congenital anomalies of the central nervous system during the intra-uterine period, there are five options for management: 1) no intervention; 2) termination of the pregnancy; 3) allowing pregnancy to proceed to term; 4) induction of preterm delivery followed by postnatal intervention; and 5) intrauterine surgical intervention. However, in practice it may be very difficult to choose the best opinion. In order to help make such decisions, this paper reviewed the current understanding of the main congenital anomalies of the central nervous system. According to a 17 year survey of major congenital neurologic malformations in the USA, the incidence of major central nervous system anomalies (anencephaly, spina bifida and encephalocele) has been declining. This has also been found in Japan. In the USA an organization was established--The Medical Task Force on Anencephaly--for the purpose of offering guidelines on the ethical issues relating to anencephaly, especially in regard to the use of fetal organs in transplantation.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:(1) Background: Fetal surgery for meningomyelocele (MMC) should reduce the occurrence of Arnold–Chiari malformations, hydrocephalus, the associated need for craniocervical decompression, and the need for cerebrospinal fluid shunt insertion. Fetal surgery should improve ambulatory status. (2) Methods: We used retrospective analysis of the documentation and descriptive statistics to summarize the clinical data and measured MRI parameters. The neurosurgical results are presented as the frequency of findings in percentages and compared with the results of the Management of Myelomeningocele Study (MOMS). (3) Results: A total of eight patients who underwent prenatal correction of MMC between 2016 and 2020 participated. MRI detected Chiari II malformation in all patients during prenatal imaging and in 87.5% of the patients at the age of 12 months. Craniocervical decompression was used in 25% of the patients. Shunt-dependent hydrocephalus occurred in 50% of the cases. In 87.5% of the cases, the functional level exceeded the expected ambulatory status. (4) Conclusions: We present the clinical status of our patients. Analysis of the complete cohort confirmed that prenatal surgery is associated with a reduced occurrence of Chiari malformations and reduced associated occurrence of hydrocephalus. Specific lesion levels are not associated with the need for craniocervical decompression. The results of our study are valuable in prenatal counseling and important for treatment planning.

Abstract:Objective To discuss the clinical significance of fetal large cavum septum pellucidum and cavum vergae by prenatal ultrasonography. Methods Prenatal ultrasonography was performed on women at our hospital for prenatal diagnosis during January 2006 to December 2008. The changes of fetal large cavum septum pellucidum and cavum vergae were observed regularly until delivery.The live infants were followed up regularly in one year. Results Totally 58 women with fetal large cavum septum pellucidum and cavum vergae were included in the study,with the incidence of 0.19%.But 7 cases losed in the observating progress. They were diagnosed between 24~37 gestational weeks,with a median of (30±4) weeks. In the pregnancy period,24 cases (24/51,47.0%) of the large cavum septum pellucidum and cavum vergae were decreased,and 19 cases were seen decreased to nomal degree in gestation. There was significant difference between the two groups,which the degree of cavum septum pellucidum and cavum vergae expansion decreased and the other didn’t,also with statistical significance (P<0.05). And between cases which were combined with other abnormalities with large cavum septum pellucidum and cavum vergae and the others not,perinatal outcomes also have significant difference (P<0.05).Conclusion Fetus with large cavum septum pellucidum and cavum vergae occur mainly in the second and third trimesters of pregnancy. When we found that the degree of the expansion degree of fetal large cavum septum pellucidum and cavum vergae decreased,it may be possible good signs of perinatal outcomes. Either it is combined with other intracranial and extracranial structures anomalies can affect the perinatal outcomes. So when the ultrasound examination diagnosis of large cavum septum pellucidum and cavum vergae,we should be close ultrasound follow-up of the size of cavum septum pellucidum and cavum vergae and other intracranial,extracranial structures anomalies.

Abstract:Objective Prenatal myelomeningocele (MMC) repair offers improved motor function and decreased rates of cerebrospinal fluid (CSF) diversion compared to than postnatal repair. However, comparative analysis of other associated neuroanatomical findings is lacking. The purpose of this study is to use magnetic resonance imaging (MRI) imaging to compare characteristic Chiari II malformation stigmata in patients who underwent fetal MMC repair vs. postnatal repair. Methods A retrospective review was performed of neonates who underwent prenatal or postnatal MMC repair at our institution and had postnatal MRIs. We analyzed anatomical findings characteristically seen with Chiari II malformation on brain MRI in patients who underwent prenatal MMC repair vs. postnatal repair. Results CSF diversion was required in 24% of prenatal cohort vs. 67% of postnatal cohort (p = 0.002), and syrinx was present in 12% of prenatal cohort compared to 42% in postnatal cohort (p = 0.03). Corpus callosum (CC) morphology was abnormal in 52% of prenatal cohort vs. 53% of postnatal cohort (p = 0.92), while falx morphology was normal in 92% of the prenatal cohort vs. 34% of the postnatal cohort (p = <0.001). Prenatal cohort patients had shorter tentorium to foramen magnum distance than postnatal cohort patients (18.4mm vs. 22.4mm, p = 0.01), overall larger foramen magnum diameter (22.9mm vs. 18.9mm, p

Abstract:Introduction and importance: Neural tube defect occurs as a result of failure of spontaneous closure of the neural tube between the third and fourth weeks of foetal life. Exencephaly is a rare malformation of the neural tube characterized by a large amount of protruding brain tissue in the absence of the calvarium. Case presentation: The authors report a 29-year-old female, non-compliant to iron, calcium and folic acid tablets due to nauseating and itchy sensation after intake for 2 weeks, was admitted in ward Obstetrics ward in view of twin pregnancy. After proper counselling, she was advised for caesarean section, which revealed gross malformation in the form of cleft lip, cleft palate and exposed brain tissue covered by thin layer of membrane with incompletely formed cranial vault and multiple-haematoma and ulcerations in the exposed brain tissue suggestive of Exencephaly. The deformed baby survived for 2 days after birth while the other baby was grossly healthy. Clinical discussion: Exencephaly is said to be the embryological precursor anomaly of anencephaly. Exencephaly is a type of cranial malformation that characteristically involves a large disorganized mass of brain tissue. The flat bones of calvaria are absent and the brain mass is left uncovered. This condition is incompatible with life. Conclusion: Each and every pregnant lady must be advised to undergo ultrasonography in every trimester, especially second trimester scan (anomaly scan) to diagnose any gross congenital malformations. Each pregnant lady is suggested to take the necessary vitamins (like folic acid) to avoid any Neural tube defects.
Current Selection Criteria and Perioperative Therapy Used for Fetal Myelomeningocele Surgery

Abstract:Objective: To determine the current maternal and fetal selection criteria and operative approaches used at centers performing fetal myelomeningocele surgery. Methods: The 17 principal investigators participating in the Fetal Myelomeningocele Consortium were asked to participate in an anonymous online survey regarding the current practice of maternal-fetal surgery for neural tube defect repair and results were tabulated. The 35-question survey related to diagnostic testing, inclusion and exclusion criteria, and clinical management. Results: Sixty-five percent (11/17) of principal investigators responded to the survey and not all centers responded to all 35 questions. All centers continue to use magnetic resonance imaging in their preoperative evaluation. Diagnostic testing from amniocentesis is varied: 5 of 11 (45%) require amniotic fluid α-fetoprotein, 4 of 10 (40%) amniotic fluid acetylcholinesterase, and 8 of 11 (73%) DNA microarray. There is also variation from the Management of Myelomeningocele Study with regard to body mass index (BMI) (1/11; 9% would offer surgery with BMIs higher than 35), maternal medical risk factors (surgery would be offered for controlled pregestational diabetes [3/10 (30%)]), hepatitis C with negative viral load (4/11 [36%]), and human immunodeficiency virus with an undetectable viral load (1/10 [10%] or an obstetric history [3/11 (27%)] would offer surgery with a history of preterm delivery on progesterone). Ten of 11 (91%) centers did not consider ventriculomegaly of 18 mm and 9 of 11 (82%) centers did not consider lack of leg movement as an exclusion criteria. Nuances in the perioperative and intraoperative management were also reported, including 5 of 11 (45%) use intraoperative echocardiography and alterations in postoperative tocolytics. Conclusion: Variation in practice patterns for offering and performing maternal-fetal surgery for myelomeningocele repair exists among centers. Ongoing evaluation of inclusion and exclusion criteria as well as operative techniques is warranted to ensure continued safety, effectiveness, and beneficence.

Abstract:We fabricate a nonlinear optical device based on graphene-silicon microring resonator (GSMR). Using such graphene-assisted nonlinear optical device, we experimentally demonstrate up and down wavelength conversion of a 10-Gbaud quadrature phase-shift keying (QPSK) signal by exploiting degenerate four-wave mixing (FWM) progress in the fabricated GSMR. We study the conversion efficiency as a function of the pump power. In addition, the resonant wavelength of GSMR is tuned by changing the temperature from 20°C to 40°C. We evaluate the bit-error rate (BER) performance for up and down wavelength conversion. The observed optical signal-to-noise ratio (OSNR) penalties for QPSK up and down wavelength conversion are less than 1.4 dB at a BER of 1 × 10-3. The BER performance as a function of the pump power for up wavelength conversion is also assessed. The minimum OSNR penalty is less than 0.8 dB when the pump power is 13.3 dBm.

Abstract:IP-based networks such as the Internet traverse paths of widely ranging bandwidths and load factors. Several investigators have proposed the use of hierarchical video as a solution for dissemination of video across heterogeneous networks of this sort. In particular, such approaches avoid some the "least common denominator" problems inherent in single-layer heterogeneous multicast. We describe an experimental video delivery system that makes use of several new Internet technologies such as the resource reservation protocol (RSVP), integrated services (IntServ), the realtime transport protocol (RTF) and the IP multicast to control the dissemination of a hierarchical media streams across heterogeneous IP multicast networks.

Abstract:Anencephaly is a congenital malformation caused by neural tube failure, which can be managed through folic acid intake during pregnancy. Diagnosis is typically prenatal, and termination of pregnancy is often recommended. A newborn born at Kolwezi paediatric clinic was born with a visible congenital malformation. The mother had a low socio-economic background and was 41 years old. An ultrasound scan revealed a progressing pregnancy at 30 weeks gestation with polyhydramnios. No antenatal care monitoring or folic acid prophylaxis was implemented during the periconceptional period and throughout the pregnancy. Anencephaly diagnosis was confirmed, and the newborn died five minutes post-birth. Preventive folic acid treatment has shown a decrease in neural tube defects, and first-trimester ultrasound scans can help identify life-incompatible congenital malformations. Advocating for legislative measures and genetic counselling is crucial to educate parents on recurrence risks and the advantages of folic acid supplementation before subsequent pregnancies.

Abstract:Background: Myelomeningocele (MMC) is the most serious form of spina bifida, a congenital defect in neural tube development. Defect closure in a patient with an extremely low birth weight presents unique challenges and risks; lower birth weight is associated with multiple organ system concerns, homeostasis is difficult, and local tissue is underdeveloped. To the authors' knowledge, the present case is the lowest reported weight (490 g) for a neonate with postnatal MMC repair. Observations: A preterm male with a prenatally diagnosed lumbosacral MMC and associated Chiari malformation type II was born at 23 weeks 1 day to a 29-year-old mother, gravidity 6 parity 4. The patient was medically stabilized and underwent MMC closure on day of life 5. His weight was 490 g at the time of this repair, and he did not have any surgical complications. At age 16 months, he underwent endoscopic third ventriculostomy with choroid plexus cauterization; he has not required any further hydrocephalus treatments since the last follow-up at 30 months of age. Lessons: To the authors' knowledge, this case is the lowest birth weight ex utero MMC closure reported in the literature. Challenges of prematurity and size required appropriate preoperative stabilization, careful hemostasis and temperature regulation, and meticulous surgical technique.

Abstract:Objectives This study aims to follow up on low-lying conus medullaris (CM) cases and explore the correlation between the CM location and the final prognosis. Methods We retrospectively collected 37 cases diagnosed with low-lying CM during pregnancy in the Peking University First Hospital from January 2019 to December 2020. The location of CM was confirmed by 3D ultrasonography, and clinical data, including postnatal outcomes, were recorded. When the conus medullaris was below L3 (excluding L3), it was diagnosed as low-lying conus medullaris, regardless of gestational age. The short-term postnatal outcome included assessment of symptoms and signs of motor and sensory neuron dysfunction. Results The average gestational weeks of low-lying diagnosis was between 23 and 24 weeks. Among 37 cases, nine (24.3%) were complicated with spine dysraphism (3 cases of open spina bifida, 6 cases of tethered cord syndrome). Apart from 7 cases of pregnancy termination, the remaining 30 live births had a good prognosis in the short term, though 5 out of 6 cases of tethered cord syndrome underwent surgical release. The mean location of cases of open spinal dysraphism ( n = 3) and those of closed dysraphism/tethered cord syndrome ( n = 6) was at Lumber vertebra 5 (L5) and between L5 and Sacral vertebra 1 (S1), respectively, which showed statistical significance compared with the postnatally normal group. When we set Lumber 4.25 as the cut-off value to predict the diagnosis of spine dysraphism (mainly involving open spinal dysraphism and closed spinal dysraphism/tethered cord syndrome), the sensitivity was 66.7. At the same time, the specificity was 96%, along with the area under the curve (AUC) at 0.877. Conclusion The second trimester finding of low CM is associated with spinal defects, mainly open spinal dysraphism and closed spinal dysraphism/tethered cord syndrome. Careful assessment of the fetal spine should be considered especially when the location of CM is lower than L4.
