Abstract:Although several self-rating scales for depression have been proposed in the past two decades, very few have been validated on non-English-speaking populations. The present study was carried out in northern Italy in order to examine the validity of the CES-D self-rating scale for depression in another language and culture. The scale was used with 40 depressives and 40 normals who were matched. The Hamilton Rating Scale for Depression (HRSD) was used as an additional test of concurrent validity. The results show that the CES-D is a valid measure in that it sensitively discriminates between depressed patients and normals and presents satisfactory correlations with the observer rating scale (HRSD) in both groups. The scale in its Italian translation is likely to be helpful in the assessment of depression of Italian immigrants in North America and Australia, especially in those whose English is poor.

Abstract:Visual evoked potential (VEP) is an evoked response recorded from the visual cortex. It is sometimes known as visual evoked cortical potential (VECP), evoked by a changing visual stimulus like flash or pattern when retina was stimulated. VEP can objectively reflect the func- tion of visual system and brain system and is specially suitable for visual assessment of children since it is seldom influenced by acknowledge or coorperation. A Study of the Correlation between VEP and Clinical Severity in Children with Autism Spectrum Disorder

Abstract:Visual evoked potential (VEP) is a technique used to assess the brain’s electrical response to visual stimuli. The aims of this study were to examine neural transmission within the visual pathway through VEP testing in preschool children with autism spectrum disorder (ASD) and compare it to age-matched controls, as well as search for a correlation between the VEP parameters and the symptoms of ASD. Participants were composed of ASD children (9 males) and typically developing children (8 males and 4 females), aged between 3 and 5 years. Checkerboards were chosen as the pattern-reversal VEP. The clinical severity of ASD was assessed using the Autism Treatment Evaluation Checklist (ATEC) and the Vineland Adaptive Behavior Scales 2nd edition (VABS-II). Our findings demonstrated that children with ASD had significantly longer N145 latency compared to the controls. A longer N145 latency correlated with a higher score of ATEC within the sensory/cognitive awareness subdomain. In addition, a slower N145 response was also associated with a lower VABS-II score within the socialization domain. The correlation between longer VEP latency and abnormal behaviors in children with ASD suggests a delayed neural communication within other neural circuits, apart from the visual pathway. These lines of evidence support the possibility of using VEP, along with clinical parameters, for the assessment of ASD severity.

VEP analysis methods in children with optic nerve hypoplasia: relationship to visual acuity and optic disc diameter

Abstract:Purpose: Assessing vision in young children with optic nerve hypoplasia (ONH) is challenging due to multi-directional infantile nystagmus, the range of optic nerve loss, and cognitive delay. This study examined visual evoked potential (VEP) responses and averaging techniques in children with ONH. The assumption is that EEG epochs with inconsistent temporal phase would be associated with nystagmus, signal reduction due to axon loss, and visual inattention. Methods: A retrospective chart review was performed on 44 children (average age 2.2 years; SD 1.9). Optic disc diameter was estimated by ophthalmoscopy. Visual function was measured under binocular viewing and then compared to the eye with the larger optic disc to exclude secondary amblyopia. Visual acuity was measured by Teller cards or by recognition optotypes, and both measures were converted into log minimum angle of resolution (logMAR). VEPs were recorded to onset/offset of horizontal gratings and to reversing checkerboards. Signal-to-noise ratios (SNRs) were estimated from phase consistency across epochs in the Fourier domain. VEPs were also averaged after (1) correction of epochs for phase shifts across a limited bandwidth, or (2) selection of only epochs showing phase consistency. Results: Optic disc diameter, logMAR, VEP amplitudes, and VEP SNR were all significantly inter-correlated. Optic disc diameter correlated best with VEP SNR (Spearman rho = 0.82; p < 0.001). Age-corrected logMAR correlated with optic disc diameter and VEP SNR (Spearman rho = -0.695 and 0.70, respectively; p < 0.001). VEP latency poorly correlated with optic disc diameter or logMAR. Correction of phase shifts or selection of epochs based on phase consistency significantly increased VEP amplitude and SNR for children with optic disc diameters <1000 microns. Correction of phase inconsistency did not improve the correlation of VEP parameters with optic disc diameter or with logMAR. Conclusions: In ONH, the size of the optic nerve is correlated with VEP SNR and logMAR. The results imply a direct relationship between the reduction in optic nerve axons and generalized reduction in visual function. Our calculation of VEP SNR provides objective assessment of optic nerve function that is independent of subjective scoring of VEP peaks.

Abstract:One of the promising ways to perform single-photon counting of terahertz radiation consists in sensitive probing of plasma excitation in the electron gas upon photon absorption. We demonstrate the ultimate sensor operating on this principle. It is assembled from a GaAs/AlGaAs quantum dot, electron reservoir and superconducting single-electron transistor. The quantum dot is isolated from the surrounding electron reservoir in such a way that when the excited plasma wave decays, an electron could tunnel off the dot to the reservoir. The resulting charge polarization of the dot is detected with the single-electron transistor. Such a system forms an easy-to-use sensor enabling single-photon counting in a very obscure wavelength region.
A comparison of visual electrophysiology measures in optic neuropathy

Abstract:Aims/Purpose: Our aim is to explore the inter‐relationship of clinical PVEP and PERG measurements with each other and the optic nerve structure in children investigated for possible optic neuropathies. Methods: ISCEV Standard PVEPs and PERGs to 50′ check widths to large 30 and standard 15 degree fields were reviewed from children suspected to have optic neuropathy of various pathology. Amplitudes and peak times of PERG P50, N95, the N95:P50 amplitude ratio, the amplitude of the N95 from baseline and at 30 ms from the P50‐N95 slope were measured. Spectralis OCT RNFL sector measures and macular ganglion cell volumes were collated. Results: PERG, PVEP with OCT measurements were available for 42 patients, (83 eyes) aged 6‐17 yrs. Those with abnormal PVEPs (33 eyes) had the most severe macular RGC volume loss (mean 0.26 mm 3 in 3.45 mm) with mean LogMAR VA 0.52 (range − 0.01 to 1.00). These patients had inherited optic neuropathies, optic neuritis associated with MOGAD and RMMS. In contrast, patients with papilledema, such as associated with IIH, or presenting with headache and reduced vision and full discs had normal PVEPs. Mean VA LogMAR 0.23 (range − 1.0 to 0.9), and mean macular RGC volume 0.45 mm 3 in 3.45 mm circle. N95:P50 amplitude ratios ranged from 1.83 to 0.44 in 30 degree fields and 2.16 to 0.36 in 15 degree fields. The N95:P50 amplitude ratios, slope at 30 ms and N95 from baseline correlated with macular RGC volume, but the best PERG discriminator (AUC 0.83) in the large field was the N95 slope measured at 30 ms. RNFL values were confounded by disc oedema in many cases. Conclusions: Pattern reversal VEPs were abnormal in all patients with severe RGC volume loss. The PERG measures correlate with macular RGC volume. RNFL was confounded by disc oedema, but was helpful in highlighting temporal sector thinning in optic neuritis and inherited optic atrophy such as LHOA. References 1. The Visual Evoked Potential Test in Children

Abstract:Objectives Two methods of evaluating data from the visual evoked potential acuity test,amplitude difference,and latency difference-were used and compared for detecting monocular acuity impairment in pediatric patients.Methods To compare the accuracy of each method,normative data were obtained from 330 infants and children by pattern reversal visually evoked potentials.Two groups of pediatric patients(One group of these patients with low vision in the affected eye;the other group had clinically determined normal vision in the affected eye) were also studied by pattern reversal visually evoked potentials.The sensitivity and specifity of each method were evaluated in both groups.Results Specifity was high for all methods,ranging from 0.83 to 0.94.The sensitivity of the amplitude difference method was significantly higher than the latency difference of analysis in all age groups.Conclusions The conclusion of this study is that of the two methods used and compared,amplitude difference is the most useful for detecting monocular acuity impairment in the clinical setting.

Abstract:Objective assessment of the visual system can be performed electrophysiologically using the visual evoked potential (VEP). In many clinical circumstances, this is performed using high contrast achromatic patterns or diffuse flash stimuli. These methods are clinically valuable but they may only assess a subset of possible physiological circuitries within the visual system, particularly those involved in achromatic (luminance) processing. The use of chromatic VEPs (cVEPs) in addition to standard VEPs can inform us of the function or dysfunction of chromatic pathways. The chromatic VEP has been well studied in human health and disease. Yet, to date our knowledge of their underlying mechanisms and applications remains limited. This likely reflects a heterogeneity in the methodology, analysis and conclusions of different works, which leads to ambiguity in their clinical use. This review sought to identify the primary methodologies employed for recording cVEPs. Furthermore cVEP maturation and application in understanding the function of the chromatic system under healthy and diseased conditions are reviewed. We first briefly describe the physiology of normal colour vision, before describing the methodologies and historical developments which have led to our understanding of cVEPs. We thereafter describe the expected maturation of the cVEP, followed by reviewing their application in several disorders: congenital colour vision deficiencies, retinal disease, glaucoma, optic nerve and neurological disorders, diabetes, amblyopia and dyslexia. We finalise the review with recommendations for testing and future directions.

Abstract:Visual electrophysiology is uniquely valuable in paediatric practice because it provides objective measures of visual pathway function. The combination of electroretinograms and visual evoked potentials to flashes and patterns enables a functional assessment of the entire pathway, from photoreceptors to striate cortex. This information can be impossible to obtain from children by other means. Electrophysiology complements structural information, often highlighting discrepant structure function relationships. Objective functional electrophysiology facilitates early diagnosis and can delineate potential treatment windows from the youngest ages. Thus visual electrophysiology findings in children contribute to natural history and deep phenotyping studies that inform clinical trials of new treatments not only for ocular disease, but also systemic diseases that involve the eye. This talk will describe ways in which ISCEV tests can be adapted and optimized for young children in clinical practice without loss of diagnostic accuracy, and how electrophysiology can distinguish phenotypes and inform genetic analysis. Examples of the value and limitations of electrophysiology in potential gene trials and other ameliorating treatments will be presented and discussed; including inherited retinal disease such as RPE65, neurodegenerations that affecting the eye such as CLN2 and iron brain disorders such as PKAN and ciliopathies such as BBS. Abstract:OBJECTIVE:To evaluate the practicality of transient PVEP (pattern visual evoked potential, PVEP) acuity assessment for 2 to 5 years old normal children from September 2007 to January 2008.METHODS:Pattern acuity and transient PVEP-A of 179 (355 eyes) normal children between 2 to 5 years were detected and analyzed. Pattern acuity data were got from test for single uncorrected eye with pattern acuity table. PVEP-A was measured by the horizontal gratings evoked potential, voyich were cognizable response. chi2 and rank tests methods were used to analysis. The value of PVEP-A was converted to international standard acuity, and then comparison to pattern acuity with Logistic regression method.RESULTS:With the growth of age, pattern acuity was gradually increased and the value was greater than 0.4. The positive correlation also existed among PVEP-A and age, PVEP-A from test children was all greater than 3.4 c/d. The analysis result of rank regression was P<0.01, that weaned PVEP-A difference among four groups had statistics significance. The correlation coefficient of PVEP-A with pattern acuity was good (r=0.673). The difference of PVEP-A and pattern acuity between two eyes was correlated(r = 0.664). The result also pointed out PVEP-A growth was earlier than pattern acuity among 179 children.CONCLUSION:Stabilization and reliability PVEP is a good objective visual assessment for younger and handicapped children.
Portable multi-focal visual evoked potential diagnostics for multiple sclerosis/optic neuritis patients

Abstract:Purpose: Multiple sclerosis (MS) is a neuro-inflammatory disease affecting the central nervous system (CNS), where the immune system targets and damages the protective myelin sheath surrounding nerve fibers, inhibiting axonal signal transmission. Demyelinating optic neuritis (ON), a common MS symptom, involves optic nerve damage. We've developed NeuroVEP, a portable, wireless diagnostic system that delivers visual stimuli through a smartphone in a headset and measures evoked potentials at the visual cortex from the scalp using custom electroencephalography electrodes. Methods: Subject vision is evaluated using a short 2.5-min full-field visual evoked potentials (ffVEP) test, followed by a 12.5-min multifocal VEP (mfVEP) test. The ffVEP evaluates the integrity of the visual pathway by analyzing the P100 component from each eye, while the mfVEP evaluates 36 individual regions of the visual field for abnormalities. Extensive signal processing, feature extraction methods, and machine learning algorithms were explored for analyzing the mfVEPs. Key metrics from patients' ffVEP results were statistically evaluated against data collected from a group of subjects with normal vision. Custom visual stimuli with simulated defects were used to validate the mfVEP results which yielded 91% accuracy of classification. Results: 20 subjects, 10 controls and 10 with MS and/or ON were tested with the NeuroVEP device and a standard-of-care (SOC) VEP testing device which delivers only ffVEP stimuli. In 91% of the cases, the ffVEP results agreed between NeuroVEP and SOC device. Where available, the NeuroVEP mfVEP results were in good agreement with Humphrey Automated Perimetry visual field analysis. The lesion locations deduced from the mfVEP data were consistent with Magnetic Resonance Imaging and Optical Coherence Tomography findings. Conclusion: This pilot study indicates that NeuroVEP has the potential to be a reliable, portable, and objective diagnostic device for electrophysiology and visual field analysis for neuro-visual disorders.
A study of pattern visual evoked potential acuity (PVEP -A) and the international standard for visual acuity in amblyopic children

Abstract:Objective To investigate the relevance of pattern visual evoked potential (PVEP) acuity and the international standard for visual acuity; to explore the feasibility and practicality of the objective assessment of visual acuity in amblyopic children using transient pattern visual evoked potentials. Methods Thirty-two amblyopic children aged 3～11 years were examined with both the international standard visual acuity chart and transient pattern visual evoked potentials (PVEP acuity). PVEPs were recorded with horizontal gratings with 5 different spatial frequencies ranging from 0.85 to 13.60 cpd (LKC UTAS-E3000,LKC company). The responses were recorded for different spatial frequencies,then the highest spatial frequency that evoked the specific PVEP waveform was determined as the objective visual acuity of the subject. The international standard for visual acuity was compared to the PVEP acuity. Results PVEP acuity was highly correlated with the international standard for visual acuity (r=0.712,P<0.05). The subjects were divided into four groups. There were significant differences in PVEP acuity among the four groups (P<0.05). There was a regular correspondence between PVEP-A and the international standard for visual acuity (VA). In the high amblyopia group,all PVEP VAs were 0.85 cpd (the lowest cpd). In the moderate group,PVEP VA distribution scattered between 3.40 cpd and 6.80 cpd. No eyes achieved 13.60 cpd. In the mild group,PVEP VA was distributed between 3.40 cpd and 6.80 cpd. Among them,71.2% of the eyes achieved 6.80 cpd. And in the group with a visual acuity of ≥0.9,PVEP vision was ≥6.80 cpd. Within the 4 groups,PVEP vision showed a centralized distribution. As the international standard visual acuity increased,PVEP VA gradually increased. Conclusion This study involving a small sample showed that an objective assessment of visual acuity based on PVEP is highly correlated with the international standard visual acuity chart. Objective assessment of visual acuity with transient pattern visual evoked potentials in young children shows that the international standard for visual acuity is unsuitable.

Abstract:Visual evoked potentials (VEP) are a significant visual electrophysiological diagnostic exam, which can be used as a suitable objective measure of optic nerve function. The topic was greatly debated and many correlations between the magnitude of the VEP latency parameters and parameters of Humphrey static perimetry suggested that the abnormal cortex responses in patients with glaucomatous changes could be tested by both electrophysiological and physical methods. Moreover, the optic nerve glaucomatous damage observed by reduction in ganglion cell layer and retinal fibre layer thickness through SD-OCT tests, which are known to precede Humphrey visual field defects, correlates with VEP latency parameters, thus consolidating the position of the VEP testing in glaucoma progression.

Abstract:Electrophysiological studies of congenitally deaf children and adults have reported atypical visual evoked potentials (VEPs) which have been associated with both behavioral enhancements of visual attention as well as poorer performance and outcomes in tests of spoken language speech processing. This pattern has often been interpreted as a maladaptive consequence of early auditory deprivation, whereby a remapping of auditory cortex by the visual system ultimately reduces resources necessary for optimal rehabilitative outcomes of spoken language acquisition and use. Making use of a novel electrophysiological paradigm, we compare VEPs in children with severe to profound congenital deafness who received a cochlear implant(s) prior to 31 months (n = 28) and typically developing age matched controls (n = 28). We observe amplitude enhancements and in some cases latency differences in occipitally expressed P1 and N1 VEP components in CI-using children as well as an early frontal negativity, N1a. We relate these findings to developmental factors such as chronological age and spoken language understanding. We further evaluate whether VEPs are additionally modulated by auditory stimulation. Collectively, these data provide a means to examine the extent to which atypical VEPs are consistent with prior accounts of maladaptive cross-modal plasticity. Our results support a view that VEP changes reflect alterations to visual-sensory attention and saliency mechanisms rather than a re-mapping of auditory cortex. The present data suggests that early auditory deprivation may have temporally prolonged effects on visual system processing even after activation and use of cochlear implant.

Abstract:BACKGROUND: Virtual reality field testing may provide an alternative to standard automated perimetry. This study evaluates a virtual reality game-based automated perimetry in a healthy pediatric population.METHODS: A prospective series of pediatric patients at one institution who performed VisuALL perimetry (Olleyes Inc, Summit, NJ) using a game-based algorithm. Participants were examined by an experienced pediatric optometrist or ophthalmologist, who confirmed that there was no evidence of ocular disease expected to affect visual fields. Testing was performed binocularly, with the child wearing their spectacle correction in place. Age, refractive error, test duration, false positives, and stereoacuity were evaluated for associations with performance on VisuALL, as defined by mean deviation (MD) and pattern standard deviation (PSD).RESULTS: A total of 191 eyes of 97 patients (54% female) were included, with a mean age of 11.9 ± 3.1 years. The average MD was -1.82 ± 3.5 dB, with a mean foveal sensitivity of 32.0 ± 4.7 dB. Fifty-nine eyes (30.9%) had MD < -2 dB. Better performance, as assessed by MD and PSD, was associated with shorter test duration (P < 0.001) and older age (P < 0.001). False positives (P = 0.442), wearing spectacles (P = 0.092), Titmus stereoacuity (P = 0.197), and refractive error (P = 0.120) did not appear to be associated with improved performance, adjusting for age as a covariate.CONCLUSIONS: VisuALL virtual reality field testing was well tolerated in this pediatric study cohort. Older age and shorter test duration were associated with better performance on field testing.

Abstract:Purpose: To assess the utility of electroretinogram (ERG) as a screening tool for vigabatrin-induced retinal toxicity in children with infantile spasms. Methods: This was an observational cohort study including children with infantile spasms receiving treatment with vigabatrin. A 30-Hz flicker potential ERG, using the RETeval system (LKC Technologies), was done at baseline before starting vigabatrin at 6 months and 1 year. The amplitudes were recorded. Results: Eleven children were included in the study. The most common etiologic factor for infantile spasms was tuberous sclerosis (36.4%) followed by West syndrome (27.3%). The mean age of the children was 7.14 ± 2.9 months, with a range of 3 to 16 months. The mean difference in amplitude was 3.21 ± 2.45 and 5.72 ± 4.18 μV at 6 and 12 months follow-up, respectively ( P < .001). Eight of the 11 children (72.7%) showed vigabatrin-induced retinal toxicity, and all 8 children were receiving vigabatrin for more than 6 months. Conclusions: ERG can be used for vigabatrin-induced retinal toxicity monitoring in children with infantile spasms. Abstract:This article describes the main visual electrodiagnostic tests relevant to neuro-ophthalmology practice, including the visual evoked potential (VEP), and the full-field, pattern and multifocal electroretinograms (ffERG; PERG; mfERG). The principles of electrophysiological interpretation are illustrated with reference to acquired and inherited optic neuropathies, and retinal disorders that may masquerade as optic neuropathy, including ffERG and PERG findings in cone and macular dystrophies, paraneoplastic and vascular retinopathies. Complementary VEP and PERG recordings are illustrated in demyelinating, ischaemic, nutritional (B12), and toxic (mercury, cobalt, and ethambutol-related) optic neuropathies and inherited disorders affecting mitochondrial function such as Leber hereditary optic neuropathy and dominant optic atrophy. The value of comprehensive electrophysiological phenotyping in syndromic diseases is highlighted in cases of SSBP1- related disease and ROSAH (Retinal dystrophy, Optic nerve oedema, Splenomegaly, Anhidrosis and Headache). The review highlights the value of different electrophysiological techniques, for the purposes of differential diagnosis and objective functional phenotyping.

Abstract:The first major development in the visually evoked potentials (VEP) was the move from flash to pattern stimulation in the 1970's. The neurophysiological basis of the responses to flash and to pattern stimuli will be reviewed to explain the superiority of the pattern VEP. The following questions will be considered: Why were the early findings in multiple sclerosis so influential? In what situations does the pVEP continue to be useful clinically, with particular emphasis on the classification of optic neuritis? Should VEP and ERG always be carried out in combination? When is flash VEP useful? Is there a role for a chromatic VEP?
