Abstract:The purpose of the present study was to increase the knowledge of angiomyofibroblastoma (AMF)-like tumors in males by describing the second case of this rare lesion in the Chinese population with a long period of follow-up and by reviewing the literature. AMF-like tumor is a rare, circumscribed, slow-growing mesenchymal tumor that occurs predominantly in the vulva, perineum and pelvis of females. The present report presents a case of left scrotal AMF-like tumor in a 37-year-old male. Complete surgical excision was performed. The tumor was composed of spindle-shaped cells and small vessels proliferating in the edematous stroma. Immunohistologically, the tumor cells stained positive for smooth muscle actin and negative for S-100, CD34 and actin. Following seven years of follow-up the patient was asymptomatic and no evidence of tumor was found. In addition, the current literature was reviewed and the characteristics of this tumor were summarized. AMF-like tumors must also be distinguished from spindle cell lipoma, solitary fibrous tumor and aggressive angiomyxoma.
Rare Cardiac Myxoma with A Hard Texture: A Case Report

Abstract:Background: Primary tumors of the heart are uncommon but not rare. Myxoma comprises 50% of all benign cardiac tumors in adults and 15% of such tumors in children. Cardiac myxoma is usually soft in texture and has a friable or villous surface, which tends to be associated with embolic events. It is also rare for myxoma to be full of calcium and metastatic bone deposits.Case presentation: We herein present a 58-year-old female patient with cardiac space-occupying lesion within the left atrium, and the lesion is lubricous and hard. Her only symptoms were hypertension, dizziness, and chest stuffiness. After surgical resection, we confirmed that the lesion was atrial myxoma by histopathology examination. The patient recovered uneventfully and was discharged. A 2-year follow-up exam showed no evidence of tumor recurrence.Conclusions: Cardiac myxoma is rare and has various clinical manifestations. It is usually friable and has an irregular surface, making embolic manifestations one of the most common complications. Consequently, immediate surgical excision is suggested upon diagnosis. In our case, the patient had a rare type of cardiac myxoma, which had a hard texture and lubricous surface. In our opinion, this case has a low risk of embolism. We prefer elective surgery rather than emergency surgery to perfect the preoperative examination for this kind of patient.

Abstract:Superficial angiomyxoma (SA) is a rare benign tumor that occurs either in the superficial dermis or subcutaneously. It often occurs in the trunk, neck, or limbs, and grows slowly. The diameter of the tumor is usually less than 5 cm. A giant SA of the perineum in men is very rare. We detailed the diagnosis and treatment of male patients with perineal SA and performed a literature review. We report a case of a 42-year-old male patient. He was admitted to hospital with a perineal mass found more than 1 year previously. A pelvic contrast-enhanced computed tomography scan in our hospital suggests that a round slightly hypointense foci of about 6.0 cm × 8.6 cm × 4.5 cm in size with still clear borders was seen below the penile corpus cavernosum in the perineum. We performed a perineal mass excision under continuous epidural anesthesia. A postoperative pathology report diagnosed perineal SA. There was no recurrence at follow-up for 27 months up to May 2022. Perineal SA is rare and should be combined with patient history and imaging to ensure complete excision of the mass margins. Adherence to long-term postoperative follow-up is the key to curing this case.

Abstract:In contrast-enhanced MR angiography (CE-MRA), vascular signal is produced by the acquisition of a T1-weighted MR imaging scan while the presence of a contrast agent induces a low T1 in blood. In this thesis, CE-MRA of the aortoiliac arteries was evaluated. Different contrast agents and techniques for synchronisation of the scan with the contrast bolus passage were assessed. In 30 patients with clinically suspected iliac artery stenoses, CE-MRA was compared with duplex ultrasound scanning (DUS) and digital subtraction X-ray angiography (DSA), with intra-arterial pressure measurements as reference. No statistically significant differences in sensitivity or specificity were observed between the techniques regarding detection of haemodynamically significant iliac stenoses. The use of multiplanar reformat and source images in the MRA examinations was of value for differentiation between highgrade stenoses and occlusions. With DSA as reference method, MRA had significantly higher sensitivity and specificity than DUS for detection of > or = 50% stenoses. In 14 patients who underwent iliac artery MRA, differences in contrast arrival time of up to 7 s were observed between the aorta and the common femoral artery. A dual-station timing technique adjusting for this difference was found feasible. Compared with a fluoroscopically triggered technique (n = 13), which is used in clinical routine, the dual-station technique was more reliable for the visualisation of distal vessels. In a clinical phase II study comparing doses of the contrast agent gadobenate dimeglumine from 0.0125 to 0.2 mmol/kg b.w. for enhancement of iliac artery MRA, significant improvement in subjective diagnostic quality compared with time-of-flight-MRA was found at all doses from 0.025 mmol/kg b.w. An increasing trend with dose was observed up to a dose of 0.05-0.1 mmol/kg b.w. In a phase I clinical study on the intravascular iron oxide contrast agent NC100150 Injection, a positive dose response was observed for abdominal vascular enhancement, with the highest contrast-to-noise ratio observed at 4.0 mg Fe/kg b.w. at 1.5 T and at 2.5-4 mg Fe/kg b.w. at 0.5 T. At 1.5 T higher calculated R2* values were found for the aorta than for the inferior vena cava.

Abstract:Mesenchymal neoplasms of the scrotum are extremely rare in the clinical practices, in which lipoma is the most common benign neoplasm of them. Primary scrotal lipoma refers to the lipoma originated from scrotal wall. We reported a rare case of a 47-year-old man who suffered from bilateral giant primary scrotal lipoma along with lipomas in multiple sites of the body. Scrotal ultrasound indicated that huge hypoechogenic masses were observed in the bilateral scrotums. Contrast-enhanced computed tomography (CT) indicated increased fat density in the scrotal areas. Surgical resection was performed on both scrotal neoplasms. Diagnosis of lipoma was confirmed by the pathological examination through the morphological observation as well as the absence of murine double minute2 (MDM2) gene amplification in the fluorescence in situ hybridization (FISH) test. After five months of follow-up, the wound of the patient recovered well and no sign of local recurrence was observed. Based on the literature review, very few cases of primary scrotal lipoma were reported in the scientific literature up to date, and this is the first report of bilateral primary scrotal lipoma along with multiple lipomas of the body. We presented this case as a rare phenomenon. Although primary scrotal lipoma is rare, clinicians should take it into account when encountering similar scrotal lesions and know the methods for diagnosis and how to make differential diagnosis with other diseases, which is associated with the patient's treatment strategy and prognosis.

Abstract:Factor H, a secretory glycoprotein composed of 20 short consensus repeat modules, is an inhibitor of the complement system. Previous studies of inherited factor H deficiency revealed single amino acid substitutions at conserved cysteine residues, on one allele arginine for cysteine 518 (C518R) and on the other tyrosine for cysteine 941 (C941Y) (Ault, B. H., Schmidt, B. Z., Fowler, N. L., Kashtan, C. E., Ahmed, A. E., Vogt, B. A., and Colten, H. R. (1997) J. Biol. Chem. 272, 25168–25175). To ascertain if the phenotype, impaired secretion of factor H, is due to the C518R substitution or the C941Y substitution and to ascertain the mechanism by which secretion is impaired, we studied COS-1 and HepG2 cells transfected with wild type and several mutant factor H molecules. The results showed markedly impaired secretion of both C518R and C941Y factor H as well as that of factor H molecules bearing alanine or arginine substitutions at the Cys518–Cys546 disulfide bond (C518A, C546A, C546R, C518A-C546A). In each case, mutant factor H was retained in the endoplasmic reticulum and degraded relatively slowly as compared with most other mutant secretory and membrane proteins that are retained in the endoplasmic reticulum. These data indicate that impaired secretion of the naturally occurring C518R and C941Y mutant factor H proteins is due to disruption of framework-specific disulfide bonds in factor H short consensus repeat modules.

Abstract:Abstract We develop the theory of partial satisfaction relations for structures that may be proper classes and define a satisfaction predicate (⊨*) appropriate to such structures. We indicate the utility of this theory as a framework for the development of the metatheory of first-order predicate logic and set theory, and we use it to prove that for any recursively enumerable extension Θ of ZF there is a finitely axiomatizable extension *Θ′ of GB that is a conservative extension of Θ. We also prove a conservative extension result that justifies the use of ⊨* to characterize ground models for forcing constructions.

Abstract:Superficial angiomyxomas (SA) of the skin are rare benign cutaneous tumors of soft tissue composed of prominent myxoid matrix and numerous blood vessels. SA are more common in males [1] and they are usually located on the trunk but can also appear on the lower limbs, head, neck and genitalia [2,3]. Treatment is surgical, the total excision is curative, but local recurrence is possible [4]. Herein we present a 72-year-old patient with a history of melanoma in situ, with a new lesion on the lower back.

Abstract:The effects of ivermectin and ivermectin aglycone on pharyngeal uptake of a carbohydrate substrate (3H-inulin) were measured in larvae of a macrocyclic lactone (ML)-susceptible isolate and 2 ML-resistant isolates of Haemonchus contortus. The resistant isolates showed a tolerance (in terms of the concentration of compound required to reduce feeding to 50%) toward ivermectin of approximately 4.5- and 9-fold and toward ivermectin aglycone of approximately 14-fold, compared to the susceptible isolate. This indicates that susceptible and resistant isolates can be readily distinguished on the basis of the sensitivity of pharyngeal uptake to MLs. The use of various metabolic inhibitors in this assay system did not reveal the nature of the resistance mechanism. Pretreatment of resistant larvae with inhibitors of multidrug resistance mechanisms (P-glycoprotein and multidrug resistance protein) and detoxification enzymes (monooxygenases, esterases, and glutathione transferases) did not reduce their level of tolerance to the ivermectin aglycone.

Abstract:Introduction: Mycetoma is a form of chronic granulomatous disease which involves subcutaneous tissues and causes bone destruction in advanced stages. The characteristic features are sinus formation, granules formation, and mass in subcutaneous region. Case report: A 19-year-old male presented to our out-patient clinic with complaint of a painless swelling around the medial aspect of the right knee joint for 8 months with no sinus or discharge of granules. Pes anserinus bursitis was thought of as differential diagnosis for the present condition. "Staging-classification of mycetoma" is commonly used for classifying mycetoma and as per classification, the present case had Stage-A. Conclusion: Single-stage local excision was performed and supplemented with anti-fungal agent for 6 months, which showed good outcome at the final follow-up of 13 months.

Abstract:Glomus tumors arise from a neuromyoarterial plexus origin, typically demonstrating branching vascular channels and aggregates of specialized glomus cells. They are characteristically identified in the subungual region of the hand, with the presentation of firm, red or blue nodules that are painful and sensitive to temperature. However, very few cases of malignant glomus tumors of the skin have been reported in the medical literature, making this case quite unique. This case report aims to identify an additional case of this rare condition, discuss the treatment plan pursued, and bring awareness to this distinct oncological entity. A 41-year-old female patient initially presented to her primary care physician with an enlarging and painful nodule on her left temple that had been present for the last two months. The patient was then referred to plastic surgery for excision and pathological workup of the mass. The pathology report of the excised specimen revealed a malignant glomus tumor of the skin. The patient was subsequently scheduled for an oncologic workup with medical oncology and radiation oncology, and a wide excision was performed after the pathologic diagnosis. With negative margins from the wide excision and no evidence of disease spread, it was determined that there was no role for systemic therapy or radiation therapy at that time. Biannual dermatologic examination and monitoring are indicated for follow-up. With very few reports of similar cases and little information in the medical literature on the treatment of this type of neoplasm, this is a unique and rare case that warrants discussion and exposure. Making the correct diagnosis, performing a complete workup, and removing the malignant glomus tumor are all essential parts of medical management in this case, and this type of neoplasm should not be excluded when evaluating the presentation of unusual cutaneous lesions.

Abstract:Abstract Angiomyxoma is a rare, slow growing mesenchymal neoplasm that occurs almost exclusively in the genital, perineal and pelvic regions of adult women of reproductive age. We present the case of a 52-year-old female with asymptomatic lesion over the vulva since 6 months which gradually increased in size. On examination single, well circumscribed, erythematous to skin coloured, pedunculated mass was seen over the right side of the vulva measuring approximately 10 x 5cms. On palpation it was mobile, soft to firm in consistency with a smooth surface. Complete Excision under aseptic precaution was done and sent for histopathological examination which showed features suggestive of Angiomyxoma, a very rare gynaecologic tumour and classified as “tumors of uncertain differentiation “with estrogen and progesterone receptors. We present this case to highlight the importance of Histopathology in dermatology to diagnose rare cases and improve patient care.

Abstract:Intramuscular myxoma is a rare benign tumor that presents as a slow-growing, deeply seated mass confined within a skeletal muscle. Histologically, these lesions most resemble umbilical cord tissue. They mostly occur in people between 40 and 70 years old, with a 57% female predilection. These tumors are very rare in children. Only one pediatric intramuscular myxoma case is reported in the literature. The goal of this study is to report the case of a 13-year-old girl who presented to our hospital emergency department in 2018 with right hip pain, elevated inflammatory markers, and fever; her initial differential diagnosis was hip septic arthritis, pelvic osteomyelitis, and pelvic abscess. A pelvic MRI revealed a well-defined enhancing round lesion in the right obturator internus muscle. The diagnosis was conformed with a CT-guided core biopsy. The patient's symptoms improved with conservative management, and she continues to be doing well 2 years later. Pediatric pelvic intramuscular myxomas are extremely rare; however, they can have a presentation that mimic a more serious condition such as hip septic arthritis, pelvic osteomyelitis, and pelvic abscess and should be considered in the differential diagnosis in a pediatric patient presenting with hip pain.

Abstract:Background Basal cell carcinoma (BCC) located on the scrotum is rare. Objective To analyze clinical and pathologic features, discuss therapeutic strategies, and identify prognostic factors of scrotal BCC in Chinese patients. Materials and Methods Between 2000 and 2010, 10 patients with scrotal BCC were diagnosed and treated at our institution. A review was performed using the clinical records and dermatopathologic slides of these patients. Results The median patient age was 70. Skin lesions presented as red nodules and brownish plaques. All patients were treated using wide excision without adjuvant therapy. After an average follow-up of 47 months, eight patients were in good health without any relapse. One patient developed left inguinal lymph node metastasis at 21 months that was successfully treated using bilateral inguinal lymphadenectomy. One patient developed bilateral pulmonary metastasis at 48 months and was palliatively treated with chemotherapy. The clinical and histopathologic risk factors predisposing to metastasis were large primary neoplasms; a long period of misdiagnosis; and infiltrating, morpheaform, spiky, irregular outline pathologic patterns. Conclusions BCC of the scrotum is rare. It can metastasize after a long period of initial therapy. Long-term surveillance including a complete metastatic examination is recommended for these patients.

Abstract:In this paper, we exploited the minimum mean square error (MMSE) estimator to define an iterative synthetic aperture radar (SAR) speckle filtering process. Hence, by optimally choosing the window size and the number of iterations, the proposed iterative filter outperforms classical MMSE speckle filtering techniques such as the improved Lee filter in terms of speckle reduction and spatial detail preservation.

Abstract:Myxoma is a rare tumor that can be challenging to diagnose, with imaging findings that can be nonspecific. We present a case of a 21-year-old man who presented with a subacute history of right visual deterioration and proptosis. Imaging showed a large right superomedial orbital mass of 43 × 31 × 24 mm, which enhanced heterogeneously and was eroding the adjacent orbital roof and medial wall. An excisional biopsy was performed via a lid crease approach under transorbital endoscopic guidance. We review the clinical, radiological, and histological characteristics of myxoma. In addition, we highlight important associations with genetic syndromes including Carney complex and Mazabraud syndrome.

Abstract:In this study, we discuss the clinical, radiological, and histopathological characteristics of intramuscular myxomas (IMM), a rare form of benign soft tissue tumors. We report the case of a 47-year-old female patient presenting with a painless, non-inflammatory mass in the right thigh, which was mobile relative to both superficial and deep planes. Imaging, biopsy, and subsequent histopathological study established the diagnosis of intramuscular myxoma. The patient underwent surgical excision of the mass, with a straightforward postoperative course. It is important to distinguish IMM from malignant tumors, such as soft tissue sarcomas, through comprehensive examinations including imaging and biopsy. The recommended treatment is surgery for complete excision of the mass, with an exceptionally low recurrence rate.

Abstract:A myxoma is a rare benign tumor derived from primitive mesenchyme that can manifest in various locations including the heart, bone, skin, skeletal muscle, and nasal sinuses, as well as the genitourinary and gastrointestinal systems. [ 1 , 2 ] Ocular myxomas are extremely rare, but have been documented in the orbit, eyelid, cornea, conjunctiva, and lacrimal gland. [ 3–10 ] The tumor may be isolated or constitute a component of the Carney complex, that includes cutaneous or cardiac myxomas with pigmented skin or ocular lesions, and endocrine overactivity.
