Abstract:Poor mental health in youth has been consistently shown to be rising over the past 20 years. While it is well established that mental health status is associated with social conditions, population-level perspectives make it difficult to identify the complex ways social and structural conditions impact on mental health. Based on longitudinal (mixed method) data, this exploratory longitudinal study aims to study how the life circumstances of education, work and financial situation are related to mental health in young Australians (aged 20–22). Findings show that the combination of study, work and financial hardship can be regarded as a stressor contributing to poor mental health, particularly if experienced over several years, and that those in the middle socio-economic bracket have the worst mental health outcomes. This research has implications for welfare policies and the responsibilities of educational institutions for the welfare of young people.

Abstract:Background Help‐seeking provides opportunities for early prevention and intervention of mental health problems. However, little is known about factors that impact help‐seeking from a life‐course and socioecological perspective. This study aimed to examine factors that impact adolescents' formal and informal help‐seeking in three population groups: the whole population, adolescents with depressive symptoms and adolescents with anxiety symptoms. Methods We drew on data from the Longitudinal Study of Australian Children birth cohort. Participants were followed from birth to 14–15 years. Based on previous literature and life‐course and socioecological models, we measured a range of factors at individual and family, interpersonal and community levels at three time points (0–1 year, 4–5 years and 12–13 years). Outcomes at 14–15 years were help‐seeking behaviours divided into three categories (formal help from health professionals, close informal help from friends and family members and broad informal help from other sources). Generalised linear models with logit‐binomial links were used. Results There were varying and common patterns of influencing factors across the three population groups and sources of help‐seeking. In the whole population, five common factors (female, previous depressive or anxiety symptoms, discrimination and bully victimisation) were associated with all three sources of help‐seeking. Among adolescents with depressive or anxiety symptoms, four common barriers to help‐seeking were male adolescents, speaking a language other than English, having poor relationships with peers and less social support. Conclusion Both intrinsic and extrinsic barriers exist for adolescents' mental health help‐seeking. There are priority groups of adolescents with mental health problems who are unlikely to seek support and warrant attention. There is a need for multisector collaborations to address barriers to mental health care and promote help‐seeking among adolescents.

Abstract:Australian &New Zealand Journal of Psychiatry, Ahead of Print. The objective of this paper is to summarise the policy implications of key findings from the 2020–22 Australian National Study of Mental Health and Wellbeing (NSMHWB). We provide an analysis of policy implications of four papers in this issue of the journal from the 2020–22 NSMHWB (N = 15,893) and the 2007 NSMHWB (N = 8841). The 2020–2022 NSMHWB reported a lifetime prevalence rate of common mental disorders of 40.2% (95% confidence interval [CI] = 39.2–41.3) and 12-month prevalence rate of 20.2% (95% CI 19.5–21.0). Overall, adult Australians were significantly more likely to experience a 12-month mental disorder in 2020–22 compared with 2007, with the change most striking in among those aged 16–24 years (odds ratio [OR] 1.2, 95% CI 1.1–1.3). Individuals aged 16–24 years in 2020–22 were significantly more likely to experience a 12-month anxiety disorder (OR 2.9, 95% CI = 2.3–3.7, depressive disorder (OR 2.8 95% CI = 2.1–3.9) or comorbidity (relative risk [RR] = 1.4, 95% CI = 1.2–1.7) compared with those aged 16–24 years in 2007. In 2020–22, the proportion of Australians who had experienced suicidal ideation, suicide plans and suicide attempts in the past 12 months was 3.3%, 1.1% and 0.3%. Under half (46.5% 95% CI 44.1–48.8) of adults with a 12-month mental disorder sought treatment. Mental disorders remain an endemic feature of Australia's overall health landscape and appear to be increasing, especially in younger cohorts. While service use rates have improved over time, there is still some way to go. Epidemiological surveys such as the 2020–22 NSMHWB are important for understanding changing prevalence and the population not accessing services. Innovative prevention and treatment strategies will be needed to address the increasing rates of disorders in younger Australian adults. Equally innovative and bold policy responses will be essential.

Abstract:BACKGROUND: Adolescence is a critical period for social and emotional development. We sought to examine the impacts of Covid-19 and related social restrictions and school closures on adolescent mental health, particularly among disadvantaged, marginalised, and vulnerable groups.METHODS: We analysed four waves of data - 3 pre-Covid-19 (2016-2019) and 1 mid-Covid-19 (May-Aug 2020; n, 1074; 12-18 years old, >80% minority ethnic groups, 25% free school meals) from REACH (Resilience, Ethnicity, and AdolesCent Mental Health), an adolescent cohort based in inner-London, United Kingdom. Mental health was assessed using validated measures at each time point. We estimated temporal trends in mental distress and examined variations in changes in distress, pre- to mid-Covid-19, by social group, and by pre- and mid-pandemic risks.RESULTS: We found no evidence of an overall increase in mental distress midpandemic (15.9%, 95% CI: 13.0, 19.4) compared with prepandemic (around 18%). However, there were variations in changes in mental distress by subgroups. There were modest variations by social group and by pre-Covid risks (e.g., a small increase in distress among girls (b [unstandardised beta coefficient] 0.42 [-0.19, 1.03]); a small decrease among boys (b - 0.59 [-1.37, 0.19]); p for interaction .007). The most notable variations were by midpandemic risks: that is, broadly, increases in distress among those reporting negative circumstances and impacts (e.g., in finances, housing, social support and relationships, and daily routines) and decreases in distress among those reporting positive impacts.CONCLUSIONS: We found strong evidence that mental distress increased among young people who were most negatively impacted by Covid-19 and by related social restrictions during the first lockdown in the United Kingdom.

Abstract:The COVID-19 pandemic disproportionately affected young people aged 16–25 years and has brought about a major increase in mental health problems. Although there is persisting evidence regarding SES differences in mental health status, there is still little knowledge of the processes linking SES to young people's mental health, in particular during the current pandemic. Guided by a stress process model this study examines the role of different psychosocial resource factors in mitigating the vulnerability to mental distress among disadvantaged young people and specifies a range of possible mediating pathways. The research draws on a nationally representative longitudinal sample of 16–25 year-olds who participated in the Youth Economic Activity and Health (YEAH) online survey conducted in the UK between February and October 2021. Mental health was measured using the Hopkins Symptom Checklist for anxiety and depression. Socio-economic disadvantage was indicated by parental education and receipt of free school meals. Experience of stress was indicated by financial strain. Psychosocial resource factors included indicators of optimism, self-efficacy, and social support. We controlled for age, gender, living arrangements, and economic activity of the young person (being in education, employment or NEET). The findings suggest sequential mediating processes where SES influences are partially mediated via financial strain and the psychosocial resource factors. In addition, the psychosocial resource factors showed independent effects supporting mental health in the face of socio-economic adversity. Moreover, social support played a significant role in boosting self-efficacy and optimism, which in turn reduce mental distress. The findings highlighting the importance of specifying the assumed mediating processes, and to consider multiple resource factors instead of single aspects to gain a more comprehensive understanding of the processes linking SES to young people's mental health.

Abstract:Australian &New Zealand Journal of Psychiatry, Ahead of Print. Objective:Evidence suggests that young adults (aged 18–34) were disproportionately impacted by the COVID-19 pandemic, but little is known about their longer-term mental health changes beyond the early pandemic period. This article investigates heterogeneous trajectories of mental health among Australian young adults across 2 years of the pandemic and identifies a broad range of associated risk and protective factors.Method:Young adults (N = 653, Mage = 27.8 years) from the longitudinal Alone Together Study were surveyed biannually between July 2020 and June 2022. Measures assessed anxiety (7-item Generalised Anxiety Disorder scale) and depression (9-item Patient Health Questionnaire) symptoms at Waves 1–4, as well as demographic, psychological, adversity and COVID-19 factors at baseline.Results:Four and three distinct trajectories of anxiety and depressive symptoms, respectively, were identified through growth mixture modelling. The proportion of participants in each anxiety trajectory were Asymptomatic (45.9%), Mild Stable (17.9%), Moderate–Severe Stable (31.1%) and Initially Severe/Recovering (5.1%). For depression, Mild Stable (58.3%), Moderate–Severe Stable (30.5%) and Reactive/Recovering (11.2%). Baseline factors associated with severe symptom trajectories included a lifetime mental health disorder, pre-pandemic stressful events, identifying as LGBTQIA+ and/or female, and experiencing one or more infection-control measures. Higher household income was protective.Conclusion:Most young adults demonstrated stable trajectories of low or high symptoms during the pandemic, with smaller groups showing initially severe or reactive symptoms followed by marked improvements over time. Vulnerable subgroups (gender- or sexuality-diverse, those with prior adversity or pre-existing mental ill-health) may face ongoing impacts and require targeted psychosocial supports to assist their mental health recovery post-COVID-19 and in the event of future crises.

Abstract:Objective: Mental and substance use disorders are the leading causes of disability worldwide. Contemporary estimates of prevalence, severity and impairment are essential for service planning. This study provides estimates of prevalence, severity, impairment and demographic correlates of mental and substance use disorders in 2020-22 and changes in prevalence since 2007. Methods: Data were from the two Australian National Surveys of Mental Health and Wellbeing conducted in 2020-22 (N = 15,893) and 2007 (N = 8841). Descriptive statistics report prevalence of lifetime and 12-month mental and substance use disorder by sex and age, proportion of people with each mental disorder by levels of severity (mild, moderate and severe) and mean days out of role by mental disorder class (mood, anxiety, substance use). Logistic regression analyses examined demographic correlates of mental disorder class and assessed changes over time. Results: The lifetime prevalence of any mental or substance use disorder in 2020-22 was 40.2%. The 12-month prevalence was 20.2% (mood disorder - 7.4%, anxiety disorder - 15.7% and substance use disorder - 3.1%). Mood disorders were associated with significant impairment. The prevalence of mental disorders has changed over time, with mood and anxiety disorders increasing and substance use disorders decreasing. These changes were most evident among young adults. Conclusion: Mental disorders are common in Australia. Impairment associated with mental disorders remains significant. Particular focus should be paid to young adults aged 16-24 years who have shown the largest increases in anxiety and mood disorder prevalence over the past 13 years.

Abstract:The impact of social determinants on mental disorders of adolescents and young adults has been amplified by the Covid-19 pandemic and its consequences. To raise clinicians' awareness of these effects in practice, this article examines two particularly well-studied areas: the effects of socioeconomic disadvantages on emerging psychotic disorders and the impact of gender on depression. In clinical practice it is crucial to consider social stress factors, confronting young people in an unequal manner, as they contribute to the emergence or relapse of mental disorders. Two illustrations of the potential impacts of social determinants on the mental health of adolescents and young adults are presented, as well as intervention strategies for practitioners.

Abstract:The COVID-19 pandemic has created many challenges for adolescents across the world. The current study aimed to investigate the prevalence of depressive and anxiety symptoms and suicidal ideation in adolescents living in Melbourne, Australia, during the state of Victoria’s second lockdown. The study also sought to identify pre-existing and current psychosocial stressors associated with adolescent’s depressive and anxiety symptoms, and to identify the extent to which adolescents experiencing mental health difficulties sought professional help during the pandemic. A COVID-19 sub-study of the Mothers’ and Young People’s Study—an 18-year longitudinal cohort study—was conducted between July and September 2020, an online survey completed by 257 adolescents aged 14–17 years and their mothers, which asked about pandemic-related stressors, remote learning, family life, and mental health. Descriptive statistics and hierarchical multiple regression analysis were conducted. Mental health difficulties were common with 38% and 20% of adolescents reporting clinically significant depressive and anxiety symptoms, respectively, and 21% reporting frequent suicidal or self-harm ideation. Factors associated with depressive and anxiety symptoms included being female, exposure to current maternal depressive symptoms, lower levels of resilience, experiences of loneliness, stressful life events, and school- and family-related stressors. Two-thirds of adolescents who were experiencing clinically significant depressive or anxiety symptoms had not sought professional help. Our findings highlight the urgent mental health need among adolescents and the importance of reducing barriers to accessing support.

Abstract:The relationship between mental well-being and distress in young people is unclear but important for both theoretical and practical reasons. This study tests these relationships using both dimensional and categorical measures of mental well-being and distress. Two thousand and eighty-two Australians' (16–25 years) completed an online survey. A subsample (n = 389) completed diagnostic telephone interviews to identify 12-month DSM-IV mood, anxiety and substance use disorders. Five competing models of the relationship between mental well-being and distress were compared. Only the bifactor model fit the data, indicating both mental well-being and distress are important subcomponents of a young person's overall level of mental health. Over 90% of young people had flourishing or moderate well-being and only 6% were languishing. Those with past year mental disorders were only 15% less likely to be flourishing and 4% more likely to be languishing. These findings highlight the well-being potential of young people living with and without mental disorders.
Diverging trends in alcohol use and mental health in Australian adolescents: A cross-cohort comparison of trends in co-occurrence

Abstract:Background: In recent years, psychological distress in Western countries has rapidly increased among older adolescents while alcohol use has declined, though little is known about younger adolescents. It is also unclear if and how these trends relate to co-occurring alcohol use and distress. This study sought to examine temporal changes in the prevalence of distress, alcohol use, and their co-occurrence among young Australians. Methods: This study used data from 13,388 youth in their early teens (aged 12-14). Differences in the prevalence of high psychological distress (Kessler-6 ≥ 13), any alcohol use (standard drink in past 3/6 months), and their co-occurrence across cohorts (2007, 2012, 2014, 2019) were tested through log-binomial regression models. Changes in co-occurrence across cohorts were tested with cohort-by-alcohol interactions predicting distress, and vice-versa. Differential trends by sex were evaluated. Results: From 2007 to 2019, the prevalence of high distress more than doubled (4.6%-13.5%) while alcohol use decreased by ∼90% (11.8%-3.1%). Distress increased more-so among females, while alcohol use decreased more-so among males. The prevalence of high distress was significantly greater among adolescents who used alcohol compared to those who had not (>2 times higher), with this co-occurrence remaining consistent across cohorts. Conclusions: Psychological distress appears to be increasing similarly among youth in their early teens who do and do not use alcohol. At the same time, alcohol use is decreasing similarly among youth with and without distress. While alcohol use does not appear to be a driver of increases in distress, rates of co-occurring alcohol use and distress remain high. Addressing co-occurrence and distress-specific mechanisms remain necessary.

Abstract:Backgroundheadspace centres provide enhanced primary mental healthcare for young people. A priority is to provide services for all young people irrespective of a range of social disadvantages or social exclusion. The aims of this study were to: (i) delineate extent of social inclusion across domains of housing, studying/employment, functioning, alcohol, and other drug use; and (ii) map profiles of young people deemed vulnerable to experiencing additional barriers to accessing services based on their social inclusion domains (e.g., those living in unstable housing, not in employment/education, and/or experiencing intersecting or multiple forms of disadvantage or difficulties), including detailing their clinical characteristics.MethodsYoung people were recruited from five headspace centres. Data relevant to social inclusion were examined. Multivariate logistic regression models were used to determine overlap between vulnerable groups, functional, social, clinical, and behavioural factors.Results1107 young people participated, aged 12–25 years (M = 18.1 years, SD = 3.3), most living in stable housing (96.5%) and engaged in studying/employment (84.8%). Specific vulnerabilities were evident in young people with NEET status (15.2%); in unstable accommodation (3.5%); of culturally diverse backgrounds (CALD) (12.2%); living in regional areas (36.1%); and identifying as lesbian, gay, bisexual, transgender, intersex, queer/questioning, and asexual plus (LGBTIQA+; 28.2%). Higher levels of distress, substance use, functional impairment, and lower social support were reported by those who were NEET and/or in unstable housing. LGBTIQA+ status was associated with high distress, depressive symptoms, and suicidal ideation.ConclusionsMost participants reported good social support, stable housing, and engagement in work or education. Those deemed vulnerable were likely to experience social exclusion across multiple domains and reported more mental health problems. The co-occurrence of mental ill-health and social exclusion highlights the importance of integrated mental healthcare.

Abstract:Objective COVID-19 significantly contributed to an increase in psychological distress and may have disproportionally impacted certain sociodemographic groups. This study aimed to assess the prevalence of psychological distress among Australian adults aged 18–64 years and identify sociodemographic factors associated with psychological distress during the COVID-19 pandemic. Methods Secondary analyses were conducted using data from the National Health Survey 2020–21 to calculate weighted national estimates of the prevalence of psychological distress using the Kessler Psychological Distress scale (K10). The association between sociodemographic factors and psychological distress was analysed using multivariable logistic regression, with adjustments for complex survey design. Results The average of K10 score was 16.94, with a prevalence of combined high/very high distress reported at 21.13 % among Australian adults. Young women aged 18–25 reported the highest average scores ( M = 20.44) across all sex and age groups. Risk factors associated with psychological distress included lower personal income, marital status (never married; widowed/divorced/separated) and being 'born in Australia'. Protective factors included homeownership and having children in the household. Additionally, among women, being aged ≥56 was significantly associated with lower odds of psychological distress. Limitations Data collection relied on self-completed online form, and a cross-sectional design limits the inference of a causal relationship. Discussion During COVID-19 pandemic, lower personal income and being female have consistently been identified as risk factors for psychological distress. Targeted and tailored interventions based on age, sex and disadvantage are warranted, particularly focusing on younger women, to alleviate the adverse effect of the pandemic on the population's mental health.

Abstract:The restrictions put in place to contain the COVID-19 virus have led to widespread social isolation, impacting mental health worldwide. These restrictions may be particularly difficult for adolescents, who rely heavily on their peer connections for emotional support. However, there has been no longitudinal research examining the psychological impact of the COVID-19 pandemic among adolescents. This study addresses this gap by investigating the impact of the COVID-19 pandemic on adolescents’ mental health, and moderators of change, as well as assessing the factors perceived as causing the most distress. Two hundred and forty eight adolescents (Mage = 14.4; 51% girls; 81.8% Caucasian) were surveyed over two time points; in the 12 months leading up to the COVID-19 outbreak (T1), and again two months following the implementation of government restrictions and online learning (T2). Online surveys assessed depressive symptoms, anxiety, and life satisfaction at T1 and T2, and participants’ schooling, peer and family relationships, social connection, media exposure, COVID-19 related stress, and adherence to government stay-at-home directives at T2 only. In line with predictions, adolescents experienced significant increases in depressive symptoms and anxiety, and a significant decrease in life satisfaction from T1 to T2, which was particularly pronounced among girls. Moderation analyses revealed that COVID-19 related worries, online learning difficulties, and increased conflict with parents predicted increases in mental health problems from T1 to T2, whereas adherence to stay-at-home orders and feeling socially connected during the COVID-19 lockdown protected against poor mental health. This study provides initial longitudinal evidence for the decline of adolescent’s mental health during the COVID-19 pandemic. The results suggest that adolescents are more concerned about the government restrictions designed to contain the spread of the virus, than the virus itself, and that those concerns are associated with increased anxiety and depressive symptoms, and decreased life satisfaction.

Abstract:Objectives To understand the extent to which various demographic and social determinants predict mental health status and their relative hierarchy of predictive power in order to prioritise and develop population-based preventative approaches. Design Cross-sectional analysis of survey data. Setting Internet-based survey from 32 countries across North America, Europe, Latin America, Middle East and North Africa, Sub-Saharan Africa, South Asia and Australia, collected between April 2020 and December 2021. Participants 270 000 adults aged 18–85+ years who participated in the Global Mind Project. Outcome measures We used 120+ demographic and social determinants to predict aggregate mental health status and scores of individuals (mental health quotient (MHQ)) and determine their relative predictive influence using various machine learning models including gradient boosting and random forest classification for various demographic stratifications by age, gender, geographical region and language. Outcomes reported include model performance metrics of accuracy, precision, recall, F1 scores and importance of individual factors determined by reduction in the squared error attributable to that factor. Results Across all demographic classification models, 80% of those with negative MHQs were correctly identified, while regression models predicted specific MHQ scores within ±15% of the position on the scale. Predictions were higher for older ages (0.9+ accuracy, 0.9+ F1 Score; 65+ years) and poorer for younger ages (0.68 accuracy, 0.68 F1 Score; 18–24 years). Across all age groups, genders, regions and language groups, lack of social interaction and sufficient sleep were several times more important than all other factors. For younger ages (18–24 years), other highly predictive factors included cyberbullying and sexual abuse while not being able to work was high for ages 45–54 years. Conclusion Social determinants of traumas, adversities and lifestyle can account for 60%–90% of mental health challenges. However, additional factors are at play, particularly for younger ages, that are not included in these data and need further investigation.

Abstract:Children and youth experienced marked impacts on day-to-day life in the COVID-19 pandemic that were associated with poorer familial and friend relationships, and greater mental health challenges. Few studies provide self-report data on mental health symptoms from children and youth themselves. We sought to examine the associations between social factors and child and youth self-reported symptoms of worsened mood, anxiety, and irritability during the COVID-19 pandemic.
