Abstract:Background: The choice between unicondylar knee arthroplasty (UKA) and total knee arthroplasty (TKA) is likely to have long-term implications for patient-reported health outcomes. However, high-quality studies that compare the outcomes of TKA and UKA and their effects are still lacking in the literature. Thus, the aim of the present study was to compare the UKA and TKA techniques with regard to functional outcomes and perioperative complications in patients who had isolated medial osteoarthritis. Methods: This was a retrospective, single-center, matched-controlled study performed with approval of our hospital (Kunshan hospital of Traditional Chinese Medicine affiliated to Nanjing University of Traditional Chinese Medicine), with the ethics number KZY2020-37. To reduce the effect of selection bias and potential confounding in this observational study, a 1:1 matching algorithm was applied. The groups were split by sex, age to within 6 years, and body mass index within 5 kg/m(2). Thus, we retrospectively reviewed the records of 240 consecutively enrolled patients who underwent UKA and 240 patients who underwent TKA from January 2013 to June 2015 from the database of our institution. Written informed consent was obtained from all subjects participating in the trial. Clinical outcomes included range of motion, Short Form 12 score, new Knee Society Score, Western Ontario and McMaster Universities Arthritis Index, and the complications. The outcome measures were evaluated by a physiotherapist and were assessed preoperatively and postoperatively at 6 months and 2 years. The mean follow-up time was 3 years. Conclusion: We hypothesized that there was no significant difference between the 2 groups in terms of postoperative outcomes.

Abstract:The purpose of this study is to evaluate the gait characteristics of bilateral limbs after unilateral total knee arthroplasty (TKA) using three-dimensional (3D) dynamic capture technology.Forty-two patients who underwent TKA were selected from the Orthopedic Medical Center of The Second Hospital of Jilin University from November 2018 to May 2019. We used a 3D dynamic capture system to measure the gait characteristics of patients at 3 months after TKA. The data, including relative position and direction of different body parts, the force between feet and ground, spatial and temporal relationship of the lower limb muscles, were measured. Besides, the surface electromyogram signal and the force plate analog signal were also collected. The walking ability, knee 3D kinematic, and kinetic characteristics were analyzed by the Cortex software.Spatial and temporal parameters, including stride frequency, double support phase, single support phase, step length, step time, step width, stride length, gait cycle, velocity, were no significant difference in bilateral lower extremities (P > .05). The reaction force of hip, knee, and ankle joint in the operation side were less than that of the healthy side, but the difference was not statistically significant (P > .05). However, when compared with the healthy side, the hip joint in operation side had a larger maximum extension angle (P < .001), the knee joint in operation side had a larger maximum valgus angle and valgus activity (P < .05), and had a smaller tibial maximum internal rotation angle (P < .05). Besides, the surface electromyogram signals of tibialis anterior muscles were reduced (P < .05).3D gait analysis, as an objective and quantitative evaluation method, is a safe, effective, and reliable method for evaluating postoperative knee function. The data of gait analysis prove that TKA is a vital treatment to improve the function of patients with knee arthritis. Besides, gait analysis also showed that there were various kinematic and biomechanical abnormalities in the knee after TKA, which may be the reason why the surgical knee could not immediately return to normal level.

Abstract:To acquire a better understanding of how individuals adapt to knee osteoarthritis (KOA) and assess the effect of TKA, 3D gait analysis was conducted 17 pre-TKA KOA patients, 10 post-TKA KOA patients and 12 age-matched control subjects. Principle component analysis was used to extract waveform characteristics. Improved function was observed after TKA, but the overall lower limb capacity of post-TKA patients at 4 month post-surgery was still far below that of the control subjects. Post-TKA patients were characterized by reduced knee flexion, delayed joint motion and more gentle moments and ground reaction curves compared to controls.

Abstract:Background: Gait ability can be objectively assessed using gait analysis. Three-dimensional gait analysis, the most commonly used analytical method, has limitations, such as a prolonged examination, high system costs, and inconsistently reported gait symmetry in patients with knee osteoarthritis (OA). Therefore, we aimed to evaluate the gait symmetry and changes before and after unilateral total knee arthroplasty (TKA) using the Walkway analyzer, a sheet-type gait analyzer. Methods: The healthy group included 38 participants from the Locomotor Frailty and Sarcopenia Registry study with lower limb pain or Kellgren-Lawrence classification grade 3 or 4 OA. The OA group included 34 participants from the registry study who underwent unilateral TKA. The walking speed, step length, step width, cadence, stride time, stance time, swing phase time, double-limb support phase time, stride, step length, and step width were analyzed per side using the Walkway gait analyzer. Results: No significant differences between the right and left sides were observed in the healthy group. In the OA group, the time indices and stance phase (p = 0.011) and the double-limb support phase time (p = 0.039) were longer on the contralateral side and the swing phase was longer on the affected side (p = 0.004) pre-operatively. However, these differences disappeared post-operatively. There were no significant differences in the spatial indices. Thus, this study revealed that patients undergoing unilateral TKA had an asymmetric gait pre-operatively, with a time index compensating for the painful side, and an improved symmetric gait post-operatively. Conclusions: The Walkway analyzer employs a simple test that requires only walking; hence, it is expected to be used for objective evaluation in actual clinical practice.

Abstract:A 24-hour-old cria was admitted for evaluation of stranguria evident shortly after birth. On physical examination, the prepuce appeared to be normally formed and small drops of fluid dripped from it as the cria strained to urinate. Testes could not be palpated. Ultrasonographic examination revealed that the bladder was distended. The only abnormality detected on serum biochemical analysis was high creatinine concentration. Tube cystostomy was performed, and the cria was treated with saline (0.9% NaCl) solution administered intravenously and antibiotics. Four days after admission, the cria could not urinate normally when the cystostomy tube was occluded. Five days after admission, normograde cystography and retrograde urethrography were performed. Radiographic findings indicated the cria was an intersex llama with urethral duplication. The llama was euthanatized at the owner's request. Necropsy revealed bilateral ovotestes with a phenotypically female reproductive tract and urethral duplication. Urethral duplication should be considered as a cause of stranguria in male neonatal crias, and hermaphroditism should be included as a cause of urethropathies in llamas.
Modified kinematic alignment better restores plantar pressure distribution than mechanical alignment in total knee arthroplasty: a randomized controlled trial

Abstract:Kinematic alignment (KA) in the short to medium term clinical outcomes is superior to the mechanical alignment (MA), but whether it will improve patients' postoperative gait is still controversial. Understanding whether and how KA influences postoperative gait mechanics could provide insights into optimizing alignment philosophy to improve functional outcomes. To investigate the impact of KA versus MA in total knee arthroplasty (TKA) on the operated and contralateral native lower limbs by analyzing plantar pressure distribution during walking gait. This study was designed as a secondary analysis from a randomized controlled trial. Thirty-seven patients were included, nineteen underwent KA-TKA and eighteen underwent MA-TKA, each with a native knee in the contralateral limb. Pressure-sensitive insoles were used to collect plantar pressure distribution of both limbs simultaneously during walking defined as medial-lateral load ratio (MLR). Perioperative characteristics including radiographic metrics (Hip-Knee-Ankle angle (HKA), mechanical lateral distal femoral angle (mLDFA), and mechanical medial proximal tibial angle (mMPTA) and clinical outcomes (Oxford Knee Scores (OKS)) were compared between the two groups pre-operatively and 2-year postoperatively. Significant differences were found in postoperative radiographic metrics, with KA showing better OKS 1 year postoperatively ( p = 0.021), lower mean HKA ( p = 0.009) and mMPTA ( p < 0.001). Other perioperative characteristics were similar between groups. In the pedobarographic analysis, the MA group demonstrated greater medial pressure distribution in forefoot compared to both the KA group ( p < 0.001) and the contralateral native knee ( p = 0.002). Besides, the MA group revealed a more lateral pressure distribution in rearfoot compared to the KA group ( p = 0.007) and the contralateral native knee ( p = 0.001). While there was no significant difference between KA and native group ( p = 0.064 and p = 0.802, respectively). KA offered advantages over MA in restoring a more physiologic plantar pressure distribution at two years postoperatively. These results underscore the potential clinical benefits of adopting KA techniques in TKA procedures.

Correlation Between Preoperative and Postoperative Alignment and Kinematic Gait in Total Knee Arthroplasty

Abstract:Objective: Maintaining the lower limb in a neutral posture following total knee arthroplasty (TKA) has long been a concept maintained by operators. This study aimed to investigate the relationship between changes in lower limb alignment and the dynamics of knee gait before and after TKA to understand the impact of alignment on gait better and offer a theoretical foundation for correcting lower limb alignment in TKA. Methods: Our study included a group of 20 participants. The cohort consisted of 1 male and 19 females, 11 left and 9 right knees, ages 58 to 81. Using the Opti_Knee® Knee Motion Test System with infrared reflective markers and a high-speed camera, the step length and 6 degrees of freedom of the knee were recorded. Following that, we investigated the association between HKA angle and knee kinematic gait before and after surgery. Results: For preoperative HKA angles ranging from -28° to -3°, we observed increased step length, flexion-extension rotation, and varus-valgus rotation with an increase in HKA angle. Conversely, an increase in HKA angle from -3 ° to 15° corresponded with decreased step length, flexion-extension rotation, and varus-valgus rotation. An increase in HKA angle from 1° to 3° postoperatively increased step length, flexion-extension rotation, and varus-valgus rotation. In contrast, increasing the HKA angle from 3° to 5° resulted in less flexion-extension rotation. The flexion-extension rotation was at its maximum when the HKA angle was 3°. A 3° postoperative varus resulted in improved kinematic gait. Step length, varus-valgus rotation, and flexion-extension rotation increased with increasing HKA angle in the neutral alignment group. In contrast, the non-neutral alignment group exhibited decreased flexion-extension rotation as the HKA angle increased, while step length and varus-valgus rotation increased as the HKA angle increased. The varus-valgus rotation was statistically significant (P < .05) in the preoperative versus early postoperative period in the 6 degrees of freedom. Conclusions: A 3° varus alignment was found to have a superior postoperative knee kinematic gait, implying that a 3° varus alignment may be more suitable as a new gold standard for TKA than the traditional "0°" alignment. The neutral alignment group demonstrated a better knee kinematic gait than the non-neutral alignment group. During early postoperative walking, significant improvements in varus-valgus rotation were found in the 6 degrees of knee freedom.

Abstract:Objective To study the radiographic changes of knee alignment in nonoperative contralateral knee after unilateral knee arthroplasty due to knee osteoarthritis and analyze the effect of K-L grade (Kellgren-Lawrence grade) on the radiographic changes of knee alignment in nonoperative contralateral knee after unilateral knee arthroplasty.Methods A total of 157 patients undergoing primary unilateral knee arthroplasty between January 2017 and April 2018 in the Department of Sports Medicine and Adult Reconstructive Surgery of Drum Tower Hospital were retrospectively analyzed, including 62 patients with K-L grade 1 to 2 (group A) and 95 patients with K-L grade 3 to 4 (group B).Postoperative change of hip-knee-angle (HKA), mechanical axis deviation (MAD), joint space width (JSW), lateral distal femur angle (LDFA), medial proximal tibial angle (MPTA), joint line convergence angle (JLCA) in contralateral knee and postoperative change of leg-length discrepancy (LLD) were compared between the two groups and analyze the relation between postoperative change of knee alignment in operative knee and contralateral knee.Postoperative changes of knee alignment in contralateral knee was compared between patients under total knee arthroplasty (TKA) and patients under unicompartmental knee arthroplasty (UKA).Results There was no statistically difference between the two groups regarding gender, age, body mass index (BMI), surgical methods and surgeon.Postoperative change of contralateral knee MAD, HKA in group A was (-2.3±7.6) mm, (-0.6±1.0) °, compared to that of group B (-8.0±14.3) mm, (-1.0±1.3) °, showing statistical significance (P＜0.05).Postoperative change ofcontralateral knee JSW, LDFA, MPTA, JLCA in group A was (0.6±1.2) mm, (0.4±1.6) °, (0.2±2.1) °, (-0.2±2.0) °, compared to that of group B (0.6±1.4) mm, (0.4±1.7) °, (-0.1±2.2) °, (-0.4±2.3) °, showing no statistical significance (P＞0.05).Postoperative change of LLD in group A was (-3.5±6.5) mm, compared to that of group B (-5.6±12.2) mm, showing no statistical significance (P＞0.05).The relation between postoperative change of MAD in operative knee and postoperative change of HKA in contralateral knee were in positive correlation (P＜0.05), and postoperative change of MAD in operative knee and postoperative change of MAD in contralateral knee were in positive correlation (P＜0.05).Postoperative changes of knee alignment between patients under total knee arthroplasty (TKA) and patients under unicompartmental knee arthroplasty (UKA) showed no statistical significance (P＞0.05).Conclusion In patients with knee osteoarthritis who undergo unilateral knee arthroplasty, improvement of contralateral mechanical axis can be measured radiographically, with more change in K-L grade 3 to 4 than K-L grade 1 to 2, which will not be influenced by surgical methods.Postoperative change of HKA, MAD in operative knee and postoperative change of MAD in contralateral knee are in positive correlation respectively.Further research is necessary to determine whether this improvement may delay the progression of contralateral knee osteoarthritis.
Total Knee Replacement for Posttraumatic Degenerative Arthritis of the Knee.

Abstract:OBJECTIVE:To evaluate the results of total knee arthroplasty (TKA) in patients with posttraumatic degenerative arthritis due to a previous fracture around the knee.METHODS:We analyzed the results of 15 TKAs, performed from 1997 to 2003, in 15 patients with post-traumatic degenerative arthritis due to a previous fracture around knee. There were 3 women and 12 men with an average age of 58 years (range, 31-76 years). The time from fracture to arthroplasty averaged 8.2 years (range, 2-27 years). Internal fixation had previously been performed in 8 patients resulting in retained hardware. At the time of arthroplasty a femoral fracture malunion was present in two knees. Lateral retinacular release (4 knees), extensor mechanism realignment (1 knee) or medial collateral ligament reconstruction (1 knee) were needed at the time of arthroplasty.RESULTS:Follow-up averaged 35 months (range, 12-73 months). No patient was lost for follow-up. According to the Knee Society Score scale, the mean preoperative knee score was 37 (range, 10-70) and functional score was 41 (range, 0-60). They were improved significantly to a mean of 84 (range, 10-100) and 76 (range, 20-100) points, respectively at the latest follow-up. The mean knee arc of motion were improved from 84 degree preoperation to 94 degree at the latest follow-up. Postoperative manipulation under anesthesia for poor motion was carried out in 4 knees. No knee had aseptic loosening that required subsequent revision. Two knees developed superficial infection and were treated with debridement. It subsequently recovered with the retention of components.CONCLUSIONS:Significant improvement in function and relief of pain has been achieved in patients with previous fractures undergoing subsequent TKA. However, this procedure is technically demanding and patients are at increased risk for restricted motion and need more care following TKA. This study suggests that the outcome of TKA may be improved further by making special efforts to restore limb alignment, to ensure correct component positioning, and to manage soft tissue balance.
Analysis of angular spread characteristics of mobile radio wave dispersion through foliage

Abstract:In this paper, the effect of the foliage on the radio wave propagation has been analyzed by using the SAGE algorithm. It has been shown that the estimates given by SAGE algorithm agree well with the geometry of the measurement site. The analysis results show that the azimuth direction spread for V polarization is always more than that for H polarization, and the opposite is true in the case of co-elevation direction spread. The average azimuth direction spread was found to be about 25° and the average co-elevation direction spread was about 6°.
Tibio‐Femoral Contact Force Distribution of Knee Before and After Total Knee Arthroplasty: Combined Finite Element and Gait Analysis

Abstract:Tibio‐femoral contact forces calculated by FEA models agree with those of the inverse dynamic analysis method. TKA could effectively restore tibio‐femoral contact force distribution. The gait changes correspond to changes of contact force distribution. Objective To assess the tibio‐femoral contact forces before and after total knee arthroplasty (TKA) in patients with knee osteoarthritis (KOA) by three‐dimensional (3D) finite element analysis (FEA) models and gait analysis. Methods Two hospitalized patients with Kellgren–Lawrence grade IV varus KOA and two healthy subjects were enrolled in this study. Both patients underwent unilateral TKA. FEA models were established based on CT and MR images of the knees of the patients with KOA and healthy subjects. Gait analysis was performed using a three‐dimensional motion capture system with a force plate. Three direction forces at the ankle joints were calculated by inverse dynamic analysis, which provided the load for the FEA models. The total contact forces of the knee joints were also calculated by inverse dynamic analysis to enable comparisons with the results from the FEA models. The total knee contact forces, maximum von Mises stress, and stress distribution of the medial plateau were compared between the patients and healthy subjects. The distributions of the medial plateau force at 2 and 6 months postoperatively were compared with the distributions of the forces preoperatively and those in the healthy subjects. Results During static standing, the medial plateau bore the most of the total contact forces in the knees with varus KOA (90.78% for patient 1 and 93.53% for patient 2) compared with 64.75 ± 3.34% of the total force in the healthy knees. At the first and second peaks of the ground reaction force during the stance phase of a gait cycle, the medial plateau bore a much higher percentage of contact forces in patients with KOA (74.78% and 86.48%, respectively, for patient 1; 70.68% and 83.56%, respectively, for patient 2) than healthy subjects (61.06% ± 3.43% at the first peak and 72.09% ± 1.83% at the second peak). Two months after TKA, the percentages of contact forces on the medial tibial plateau were 79.65%–85.19% at the first and second peaks of ground reaction forces during the stance phase of a gait cycle, and the percentages decreased to 53.99% – 68.13% 6 months after TKA. Conclusion FEA showed that TKA effectively restored the distribution of tibio‐femoral contact forces during static standing and walking, especially 6 months after the surgery. The changes in the gait were consistent with the changes in the contact force distribution calculated by the FEA model.

[Clinical affect of contralateral knee pain on patients undergoing total knee arthroplasty]

Abstract:Objective: To evaluate the effect of contralateral knee pain on joint dysfunction and treatment satisfaction on the operative side 1 year after total knee arthroplasty（TKA） in patients with osteoarthritis. Methods: From March 2019 to January 2021, 635 patients underwent TKA including 296 males and 339 females, with an average age of （69.33±9.38） years old, the duration of symptoms was （1.15±0.44） years. According to the degree of pain visual analogue scale（VAS） in the contralateral knee joint 12 months after operation, the patients were divided into three groups, 423 patients with no or mild pain（VAS 0 to 3）, 105 patients with moderate pain（VAS 4 to 6）, 107 patients with severe pain（VAS 7 to 10）. The related factors of knee function and satisfaction score 12 months after operation were analyzed and compared on different contralateral knee pain levels. Results: The contralateral knee pain VAS was significantly reduced after TKA. Old age, high body mass index, high WOMAC scores of postoperative knee joint, moderate and severe pain of contralateral knee joint were the risk factors of dissatisfaction （P<0.05）, OR were 1.285, 1.665, 2.319, 1.863 respectively. The high degree of knee pain on the operation side and the ladder room in the home environment were the risk factors for the high WOMAC scores 1 year after discharge（P<0.05）. Adherence to exercise and functional training after discharge were the protective factors for patients with high WOMAC scores 1 year after discharge（P<0.05）, OR were 3.016, 1.738, 0.619, 0.574 respectively. Conclusion: TKA can alleviate the pain of contralateral knee joint. Contralateral knee pain does not affect the WOMAC scores after TKA, but it will reduce the treatment satisfaction of patients.
Preoperative Abnormal Posture Improves After Total Knee Arthroplasty: Knee-Hip-Spine Syndrome

Abstract:Background: Spinal, pelvic, and lower extremity alignment is crucial for maintaining a healthy body posture. However, with aging, this posture becomes challenging to maintain due to muscle weakness and skeletal degeneration. Osteoarthritis (OA) of the hip and knee can also lead to abnormal posture, known as hip-spine and knee-spine syndrome. Total knee arthroplasty (TKA) can help relieve pain and improve lower extremity alignment. This may also improve abnormal posture, such as in knee-hip-spine syndrome. However, the condition of the contralateral knee may affect this improvement. This study evaluated the effects of TKA on clinical outcomes and radiographic body posture. Methods: Patients scheduled for primary one-sided TKA were divided into two groups: the unilateral group comprised patients who had contralateral knee OA conditions. The bilateral group included patients who had prior contralateral TKA. Knee range of motion (ROM), the Knee Injury and Osteoarthritis Outcome Score (KOOS), radiographic lateral femorotibial angle (FTA), hip and knee flexion angles, and sagittal vertical axis (SVA) while standing were measured 24 months after TKA. Results: Both groups showed improved knee extension, KOOS, and radiographical FTA and hip and knee flexion angles 24 months after TKA. The SVA also improved 6 months after TKA in both groups and was maintained in the bilateral group, while it deteriorated again in the unilateral group 24 months after TKA. Conclusion: A TKA can improve abnormal body posture due to knee OA as in knee-hip-spine syndrome. However, the condition of a contralateral knee can affect TKA outcomes.

Abstract:Abstract Purpose Patients with varus knee osteoarthritis usually compensate at the ankle and typically walk with hindfoot valgus alignment. As the neutral weight-bearing axis of the lower limbs is restored with Total Knee Arthroplasty (TKA), ankle and hindfoot biomechanics also acutely change. This study aims to investigate whether any ankle clinical-radiographical changes occur as a result of bilateral mechanical TKA in patients with bilateral Osteoarthritis knee at a minimum follow-up of 6 months. Methods The prospective observational study included 61 patients (122 knees) undergoing simultaneous bilateral TKA (mechanical alignment). Tibio-talar angle(TTA), tibial Anterior Surface angle (TAS), lateral distal tibial angle (LDTA), talar-tilt angle (TT), anatomical talocrural angle (aTC), ground surface and distal tibial plafond angle (GP), ground surface and an upper surface of talus angle (GT)and tibial plateau and tibial plafond angle (PP) were measured on long-film radiographs to look for changes in the ankle, whereas functional assessment was done using American Foot and Ankle Society (AOFAS), Foot and Ankle Disability Index (FADI), and Forgotten Joint (FJS-12) scores. Patients were sub-grouped based on the Hip-Knee-Ankle (HKA) axis, and the effect of the severity of knee varus on the ankles after TKA was also analyzed. The minimum follow-up was 6 months. Results A significant decrease in the tibial plateau-tibial plafond (PP), ground-tibial plafond (GP), and ground-talar dome (GT) angles was noted after TKA ( p -value < 0.05). Postoperative functional parameters were comparable to the preoperative status except for FADI, which significantly improved ( p -value-0.03). Sub-group analysis based on the severity of knee varus (HKA) revealed GT to be most significantly reduced ( p -value-0.036), while the talar tilt (TT) increased ( p -value-0.044). Functional outcomes of the ankles clinically improved with the correction of severe knee varus after TKA. At a mean follow-up of 13.2 months post-TKA, 7 out of 61 (11.4%) patients complained of post-TKA ipsilateral ankle pain. Conclusion Mechanically aligned bilateral TKA in severe varus deformity of the knee significantly decreases the GT angle but increases the varus tilt of the talus with lateral talar incongruency and under-coverage. Although the acute correction of severe knee varus deformity aligns the tibia more neutrally, resulting in an overall clinically evident improvement in ankle functional outcome, the increased varus talar tilt remains a deep concern. Abstract:Background: Fixed-bearing posterior-stabilized (PS) total knee arthroplasty (TKA) has been used in Asian countries for several years, but few studies have investigated differences in the kinematic properties of the patellar tendon after standard PS TKA as compared to high-flex PS TKA. Purpose: To quantify the in vitro three-dimensional (3D) kinematics of the patellar tendon during passive high flexion and full extension before and after two different types of PS TKAs. Methods: Six fresh-frozen cadaveric knees were tested under the following conditions: the unaltered state, status-post traditional PS prostheses (Simth-nephew GENESIS II) replacement, and status-post high-flexion PS prostheses replacement. The soft tissue around the knee and the quadriceps muscle were preserved, then tested under the load of a specific weight in an Oxford knee rig. We designed a specialized rigid body with four active markers fixed to each bone to track the 3D passive motion of the cadaveric knees. Flexion and extension was controlled by the knee rig and captured by an Optotrak Certus™ high precision optical tracking system. The attachment sites of the patellar tendon were registered as virtual markers to calculate the 3D kinematics. Results: The patellar tendon of the unaltered knee and both TKA knees showed similar deformation. We found the length of the patellar tendon changed significantly during a motion from full extension to 30°, but there was no significant change in length while undergoing a motion from 30° to full flexion. Both the sagittal plane and coronal plane angles of the patellar tendon decreased after PS TKAs. There was no significant difference in patellar tendon kinematics between the two types of PS TKAs. Conclusion: We believe the changes observed in the sagittal plane and coronal plane angles of the patellar tendon after PS TKAs may influence the extensor mechanism and be an important cause of patella-femoral complications. These data may- be used to assess patella-femoral complications after surgery so as to improve the design of high-flexion TKAs for Asians and achieve long-term stability.
Five-Year Changes in Gait Biomechanics After Concomitant High Tibial Osteotomy and ACL Reconstruction in Patients With Medial Knee Osteoarthritis

Abstract:Background: Concomitant high tibial osteotomy (HTO) and anterior cruciate ligament (ACL) reconstruction is a combined surgical procedure intended to improve kinematics and kinetics in the unstable ACL-deficient knee with varus malalignment and medial compartment knee osteoarthritis (OA). Purpose: To investigate 5-year changes in gait biomechanics as well as radiographic and patient-reported outcomes bilaterally after unilateral, concomitant medial opening wedge HTO and ACL reconstruction. Study Design: Controlled laboratory study. Methods: A total of 33 patients (mean ± SD age, 40 ± 9 years) with varus malalignment (mean mechanical axis angle, −5.9° ± 2.9°), medial compartment knee OA, and ACL deficiency completed 3-dimensional gait analysis preoperatively and 2 and 5 years postoperatively. Primary outcomes were the peak external knee adduction (first peak) and flexion moments. Secondary outcomes were the peak external knee extension and transverse plane moments, peak knee angles in all 3 planes, radiographic static knee alignment measures (mechanical axis angle and posterior tibial slope), and the Knee injury and Osteoarthritis Outcome Score (KOOS). Results: There was a substantial decrease in the knee adduction moment in the surgical limb (%BW × H, −1.49; 95% CI, −1.75 to −1.22) and a slight increase in the nonsurgical limb (%BW × H, 0.16; 95% CI, 0.03 to 0.30) from preoperatively to 5 years postoperatively. There was also a decrease in the knee flexion moment for both the surgical (%BW × H, −0.67; 95% CI, −1.19 to −0.15) and nonsurgical limbs (%BW × H, –1.06; 95% CI, –1.49 to −0.64). Secondary outcomes suggested that substantial improvements were maintained at 5 years, although smaller declines were observed in several measures and in both limbs from 2 to 5 years. Conclusion: Changes in the peak external moments about the knee in all 3 planes during walking were observed 5 years after concomitant medial opening wedge HTO and ACL reconstruction. These findings are consistent with an intended, sustained shift in the mediolateral distribution of knee loads. Clinical Relevance: These findings suggest that concomitant HTO and ACL reconstruction results in substantial changes in gait biomechanics. Future clinical research comparing treatment strategies is both warranted and required for this relatively uncommon but seemingly biomechanically efficacious procedure.

Abstract:Total knee arthroplasty (TKA) is a commonly performed surgery aimed at alleviating pain and improving functionality. However, patients often face uncertainties in selecting the timing, location, and type of TKA implant that best meets their needs. This study aims to comprehensively compare various variables, explore trends, and identify factors potentially influencing TKA outcomes. A cohort of 40 TKA subjects received either unilateral posterior stabilized (Persona) TKA or bi-cruciate stabilized (Journey II) TKA. Additionally, 20 healthy controls matched for age, gender, and BMI were included. Participants underwent patient-reported outcome assessments, range of motion evaluations, balance assessments, proprioception tests, and biomechanical analyses. These analyses covered motion, loading, and electromyography during five daily activities and two clinical tests. Multifactor ANOVA was utilized to compare 283 variables and assess their impact on TKA outcomes. A knee biomechanics index was formulated to evaluate deviations from healthy norms. Significant differences were observed in EMG varus/valgus rotation during both ramp-up and ramp-down phases between the two implant groups. Although significant improvements were noted post-TKA for both implants, the results remained below those of the control group. Gender, age, and BMI exhibited noticeable effects on TKA outcomes across several biomechanical variables and demonstrated significant disparities compared to the controls.
