Abstract:Background: Health is a matter of human rights, and dental caries is the most common noncommunicable disease globally. Consequently, dental caries is a matter of human rights and its control, particularly prevention, must be a priority. Although largely preventable, this is too often neglected, both in the literature of human rights and health law, and in dental research. The right to oral health has recently been acknowledged by the World Health Organization (WHO), but it is insufficiently clear what this right entails. Summary: This article introduces a right to health-based narrative in the context of dental caries. The right to health is stipulated in human rights treaties, including the International Covenant on Economic, Social and Cultural Rights (ICESCR) and the Convention on the Rights of the Child (CRC). States that are parties to these treaties, which are virtually all States globally, are mandated to ensure the enjoyment of individuals’ right to the highest attainable standard of health, including oral health. Key Messages: Dental caries is a matter of human rights. States have binding obligations to address dental caries: they require the regulation of the healthcare system, i.e., the traditional focus on operative care, but also put the regulation of other risk factors on an equal footing, such as the regulation of the living environment and access to fluoride. A right to health-based approach to dental caries thus offers a comprehensive approach to dental caries control, particularly prevention.

Abstract:We report the direct observation of a precursor state for the cycloaddition reaction (the di-sigma bond formation) of ethylene on Si(100)c(4 x 2) using high-resolution electron energy loss spectroscopy at low temperature, and the meta-stable precursor state is identified as a weakly bonded pi-complex type. The activation energy from the pi-complex precursor to the di-sigma bonded species is experimentally estimated to be 0.2 eV. First-principles calculations support the pi-complex precursor mediated cycloaddition reaction of ethylene on Si(100)c(4 x 2).

Abstract:Dental care is the most common unmet health care need of children. Those at increased risk for problems with oral health and access to care are from poor or minority families, lack health insurance, or have special health care needs. These factors place more than 52 percent of children at risk for untreated oral disease. Measures of access and parental report indicate unmet oral health needs, but do not provide guidance as to the nature of children's oral health needs. Children's oral health needs can be predicted from their developmental changes and position in the life span. their dependency and environmental context, and current demographic changes. Specific gaps in education include training of general dentists to care for infants and young children and those with special health care needs, as well as training of pediatric providers and other professionals caring for children in oral health promotion and disease prevention. Educational focus on the technical aspects of dentistry leaves little time for important interdisciplinary health and/or social issues. It will not be possible to address these training gaps without further integration of dentistry with medicine and other health professions. Children's oral health care is the shared moral responsibility of dental and other professionals working with children, parents, and society. Academic dental centers hold in trust the training of oral health professionals for society and have a special responsibility to train future professionals to meet children's needs. Leadership in this area is urgently needed.

Abstract:Much research on children’s oral health has focused on proximal determinants at the expense of distal (upstream) factors. Yet, such upstream factors—the so-called structural determinants of health—play a crucial role. Children’s lives, and in turn their health, are shaped by politics, economic forces, and social and public policies. The aim of this study was to examine the relationship between children’s clinical (number of decayed, missing, and filled teeth) and self-reported oral health (oral health–related quality of life) and 4 key structural determinants (governance, macroeconomic policy, public policy, and social policy) as outlined in the World Health Organization’s Commission for Social Determinants of Health framework. Secondary data analyses were carried out using subnational epidemiological samples of 8- to 15-y-olds in 11 countries ( N = 6,648): Australia (372), New Zealand (three samples; 352, 202, 429), Brunei (423), Cambodia (423), Hong Kong (542), Malaysia (439), Thailand (261, 506), United Kingdom (88, 374), Germany (1498), Mexico (335), and Brazil (404). The results indicated that the type of political regime, amount of governance (e.g., rule of law, accountability), gross domestic product per capita, employment ratio, income inequality, type of welfare regime, human development index, government expenditure on health, and out-of-pocket (private) health expenditure by citizens were all associated with children’s oral health. The structural determinants accounted for between 5% and 21% of the variance in children’s oral health quality-of-life scores. These findings bring attention to the upstream or structural determinants as an understudied area but one that could reap huge rewards for public health dentistry research and the oral health inequalities policy agenda.

Abstract:Aotearoa New Zealand (NZ) has a long tradition of providing publicly funded oral health care for children and young people; however, substantial inequities in child oral health remain. Dental caries is the most prevalent non-communicable childhood disease in NZ, with Māori and Pasifika, those from low socio-economic backgrounds, and those without access to community water fluoridation most affected. Children and whānau with dental caries suffer consequences that seriously affect their day-to-day lives; it is critical not to underestimate the disease or fail to include it when considering children's overall health. Dental caries is a complex disease, as is its prevention. This is particularly so in the current social context of child poverty, our food environment, the exploitation of children in advertising of non-healthy foods and drinks, and the immense challenges of meeting demand for oral health care in primary, secondary and tertiary care within current constraints. We review children's oral health in NZ and make recommendations for change among oral health professionals, all health professionals, health services and society. Abstract:Considerable progress has occurred in the US over the past quarter-century in terms of improving children's oral health. Federal and state policies, programs, and partnerships have contributed to improvements in oral health status. Medicaid and Children's Health Insurance Program coverage expansions helped increase the use of dental services, and investments in safety-net facilities and training programs helped expand service delivery and the oral health workforce. Nevertheless, dental caries remains the most common chronic disease of childhood, notable oral health disparities persist, and the adoption of evidence-based innovations remains slow and uneven. This article highlights improvements during the past twenty-five years in US children's oral health and oral health care that stem from major federal and state initiatives, as well as persistent disparities. We offer promising strategies for reducing gaps and suggestions for overcoming challenges to future progress, including renewed emphasis on oral health during early childhood; greater integration in education and clinical service delivery programs; development of standardized quality measures; and data collection systems that support more robust surveillance, program monitoring, and system improvements.

Abstract:The concept of childhood has evolved over the years, inspired by the United Nations Convention on the Rights of the Child in 1989, shifting from developmental models to a conception of childhood that recognizes children as moral agents. This evolution highlights the importance of respecting children's agency and their right to be heard in matters that are related to them. In conventional health research, however, children's voices are often inadequately accessed. In this commentary, we discuss the imperative to recognize children's agency in dental research and a shift from research on children to research with and by children. Moreover, we underscore the importance of actively seeking and listening to children's voices and recognizing their agency in shaping research and healthcare practices in the field of dentistry. Further, we explore the application of participatory research approaches in dental research and provide examples of studies that have involved children in various capacities. We conclude this commentary by emphasizing the potential benefits of participatory research in both qualitative and quantitative dental studies to promote deeper understanding, clearer communication, and stronger advocacy regarding children's interests. Primarily, we call for greater recognition of children's agency in dental research and advocate for more inclusive and child‐centred research methodologies.

Abstract:Dental caries remains the most common disease in man and presents a tremendous health-affecting challenge and fiscal burden to both developed and underdeveloped countries. Changing demographics including increased number of ethnic minorities, cultural practices and diet, the number of children living in poverty or near poverty, and the special needs of medically compromised children have made solutions more complex and evasive. Systemic and topical fluoride contacts remain the most cost-effective public health response to preventing caries among children. The time-honored impact of reducing sugars and carbohydrates in the diet and improving oral hygiene practices also remain essential. New technology has the potential of offering remineralization strategies. The dental profession is challenged to be proactive in identifying alternatives and implementing new and creative ways to embrace underserved children and improve their access to care including trauma prevention. The impact on families and society, including financial and general well-being, due to poor oral health is significant. Lower income families absorb disproportionately the effect of dental diseases due to lack of education, food availability and selection, and access to early preventive care.
Serum beta 2-microglobulin and neopterin levels in children with febrile illness: their relation to influenza and febrile seizures

Abstract:Abstract The aim of this study is to evaluate serum beta 2-microglobulin and neopterin levels in children with febrile illness and to clarify their relation to influenza and febrile seizures (FS). We studied 34 patients with febrile illness whose age was between 1 and 6 years. Influenza A antigen in the throat swab was positive in 17 patients (Flu group) and was negative in 17 (control group). Serum levels of beta 2-microglobulin and neopterin were evaluated on admission. FS were observed in 13 patients (6 in Flu group and 7 in control group). Serum beta 2-microglobulin and neopterin levels were higher in Flu group than in control group. On the other hand, there were no significant differences in serum beta 2-microglobulin or neopterin levels between those with and without FS. Higher serum beta 2-microglobulin and neopterin levels in Flu group suggested that stronger activation of immune system was caused by influenza than by infection with other agents. Abstract:Journal of Dental Research, Ahead of Print. The GLOBICS—Global Consortium of Oral Health Birth Cohort Studies (OHBCS)—is a long-lasting international collaborative research group among existing OHBCS, established in response to the Lancet Series on Oral Health, which pointed out the need to shift the global oral health agenda. This study aimed to identify priority research questions from OHBCS that can be leveraged to improve children's global oral health. A 2-phase online Delphi process sought consensus over research priorities for OHBCS. Participants were identified by mapping existing OHBCS. Fourteen research questions were grouped into 4 thematic areas. A 9-point Likert-type scale was applied. Consensus was achieved if the question had (1) at least 51% of participants scoring a research question as "high priority," (2) an interquartile range of <2.5, and (3) a standard deviation of <1.7 or 75% of responses falling between 2 points above and below the mean score. Forty-four of 114 participants completed the Delphi survey. The 8 research priorities identified included questions about the effect of childhood dental visits on oral health throughout life (n = 2), questions related to sugar consumption and dental caries in childhood and adolescence (n = 4), infant growth and dental diseases (n = 1), and whether socioeconomic inequalities in dental caries are mitigated by fluoride exposure (n = 1). Guided by expert insight, the priorities identified for future OHBCS research should leverage a life course perspective to address knowledge gaps in children's oral health and oral health equity.

Abstract:Background: The multidisciplinary and comprehensive nature of children`s oral health with mutual interactions among various determinants makes the area a window of more discussion among oral health policymakers, stakeholders, providers, and other interested parties. This commentary presents a triangle framework of the children`s oral health, including all the above groups, for new discussions in oral health policymaking. Main body: Three leading influencers could be recognised in children`s oral health as a triangle despite the contextual differences among the countries. The first angle, Families and community, determine the individual background, demographic, biological, genetic, and psychological factors, as well as community-based and social background, including cultural and socioeconomic factors. The second angle, Oral health providers, includes a variety of determinants from the provider`s perception toward oral health provision of services to availability of dental services, teledentistry and digital technology, surveillance, and monitoring systems for children`s oral health. And finally, as the third angle, Oral health policymakers affect the mechanism for funding dental care and supporting schemes, affordability of oral health services, regulations and standards and public education. Macro environmental policies related to the children`s ecosystem, community water fluoridation, and social marketing for promoting probiotics products` consumption are categorized in this category. Conclusion: The triangle framework of children`s oral health presents a big picture of the oral health concept at the multilevel. Although these determinant factors interact with each other, each can have a cumulative effect on children`s oral health; policymakers could try to consider them as a big picture with a systematic approach for better achievement of oral health among children considering the local and national contextual factors of the community.
Effects of cytokine application on glucocorticoid secretion in an animal model for systemic scleroderma.

Abstract:We previously reported on an altered immune-endocrine feedback loop via the hypothalamo-pituitary-adrenal (HPA) axis in Obese strain (OS) chickens afflicted with spontaneous autoimmune thyroiditis. These animals are deficient in plasma corticosterone increase after antigenic challenge or application of cytokine-containing conditioned medium of mitogen-stimulated spleen cells (CM). To investigate whether the impaired ability to respond to cytokines with glucocorticoid-increasing factor (GIF) activity, e.g. interleukin 1 (IL 1), is restricted to OS chickens as a model for an organ-specific autoimmune disease, we extended our experiments to another autoimmune-prone animal strain, the chickens of the University of California at Davis line 200 (UCD-200). These animals develop an inherited inflammatory fibrotic disease that closely resembles human progressive systemic sclerosis (scleroderma). Application of GIF-containing CM to UCD-200 chickens leads to a transient increase in glucocorticoid serum levels within 1-2 hours comparable to that of controls. But, while corticosterone levels in the latter returned to normal baseline levels after 4 hours, they were still elevated in autoimmune chickens. Although the peak of the glucocorticoid hormone serum concentrations was equal to that of controls, UCD-200 had to secrete twice as much adrenocorticotropic hormone to achieve this corticosterone serum level due to an apparent hyporesponsiveness of the adrenal gland to this secretagogue. The altered cytokine-induced glucocorticoid secretion is found in early as well as in chronic, sclerotic stages of the disease. Cellular alterations in the peripheral blood of UCD-200 chickens during the prolonged elevated corticosterone section, i.e. between 2-4 hours after CM application, are characterized by a significant decrease in the percentage of CD4+ and CD8+ cells. Furthermore, a significant increase in B cells up to 24 hours with a maximum after 1 hour was found. The proliferative response to the mitogen concanavalin A of peripheral mononuclear cells was inversely correlated to the serum corticosterone level, showing a permanent decrease of 80-90% after 1-4 hours in autoimmune animals. This functional alteration in UCD-200 was accompanied by an 80% decrease in serum interleukin 2 (sIL 2) activity 4 hours after CM application. Twenty-four hours later an eight-fold increase in sIL 2 rebound activity was found, indicating that the inhibitory effect of corticosterone in UCD-200 chickens is not long-lasting.

Abstract:Abstract Background Families’ understanding towards oral health problems among young children is poorly studied. More insight into parents’ experiences, especially of those living in disadvantaged neighbourhoods, is needed to address persistent oral health inequalities. This qualitative study aims to explore parental perspectives on children’s oral health (≤ 4 years) and the opportunities they see to improve children’s oral health. Methods Forty-seven mothers and five fathers with different migration backgrounds from a disadvantaged neighbourhood in Amsterdam, the Netherlands, participated in our study. Semi-structured interviews ( n = 27), participant observations ( n = 7) and one focus group discussion were conducted. A thematic data analysis was used. Results Parents describe their daily life with young children as busy, hectic and unpredictable. Parents seem to be most concerned about parenting. Mothers, in particular, feel fully responsible for raising their children and managing daily complexities. While most parents value their children’s oral health, they all experience challenges. Parents find it hard to limit daily candy intake and to handle unwilling children during tooth brushing. They feel limited support for these issues from their household, social network and professionals. Conclusion Parental struggles in children’s oral health are complex and interrelated as they occur across family, societal, community and professional levels. Given the complex daily reality of families with young children, establishing and maintaining healthy oral health habits seems not at the top of parents’ minds. They ask for advice in the upbringing of their children backed up by social support, increased attention to children’s oral health within the community and professional assistance. Collaborating with parents as knowledgeable partners might be the first step in acting upon the endeavour to address oral health inequality among young children.

Abstract:Children's health outcomes result from the complex interaction of biological determinants with sociocultural, family, and community variables. Dental professionals' efforts to reduce oral health disparities often focus on improving access to dental care. However, this strategy alone cannot eliminate health disparities. Rising rates of early childhood caries create an urgent need to study family and community factors in oral health. Using Los Angeles as a multicultural laboratory for understanding health disparities, the Santa Fe Group convened an experiential conference to consider models of ensuring child and family health within communities. This article summarizes key conference themes and insights regarding 1) children's needs and societal priorities; 2) the science of child health determinants; 3) the rapidly changing demographics of the United States; and 4) the importance of communities that support children and families. Conference participants concluded that to eliminate children's oral health disparities we must change paradigms to promote health, integrate oral health into other health and social programs, and empower communities. Oral health advocates have a key role in ensuring oral health is integrated into policy for children. Dental schools have a leadership role to play in expanding community partnerships and providing education in health determinants. Participants recommended replicating this experiential conference in other venues.

Abstract:The United Nations Convention on the Rights of the Child asserts that all children have the right to protection from abuse and exploitation. In Aotearoa New Zealand, the prevalence of child maltreatment requires vigilant action from oral health practitioners (OHPs), who are often among the first in the position to recognise signs of abuse and neglect due to their frequent interactions with children in dental settings. The Dental Council of New Zealand sets professional requirements, highlighting practitioners' roles in responding to potential signs of maltreatment. Multiple statutes, including the Oranga Tamariki Act 1989 and the Family Violence Act 2018, govern OHPs' responses. Despite this, there is a notable gap in OHPs' confidence and understanding of their child protection roles, underlining the need for clear guidance on the legal and professional requirements that inform their responsibilities. This paper critically examines the professional responsibilities and policies governing OHPs in Aotearoa New Zealand regarding child protection responses. It also proposes the development of equity-focused guidelines, that are informed by Te Tiriti o Waitangi, emphasising culturally responsive practices, collaborations with Māori health providers and clear protocols for child protection responses.

Abstract:Oral health is embedded in overall health and contributes to physical, social and mental wellbeing. Most diseases are preventable, and yet, oral diseases pose a significant public health problem and an economic burden globally. Poor oral health is a risk factor for certain systemic diseases, such as cardiovascular disease, diabetes and lung pathologies. Rural populations are disproportionately affected by oral disease, with higher levels of periodontal disease, caries and the loss of teeth. These issues are worsened by barriers in access to oral healthcare services and minimal promotion of healthy behaviours in rural communities. Certain interventions, including mobile dental clinics, teledentistry, dental outreach camps and educational initiatives, have been successful in addressing rural challenges. Policies and action plans should be considered by public health officials to reduce the disparities in oral health among rural communities, reduce the overall burden of oral health and promote health equity.
A Qualitative Study of Preschool Children’s Perspectives on an Oral Health Promotion Program in New South Wales, Australia

Abstract:In Australia, dental caries are observed in almost half of children starting school. Oral health promotion programs are being implemented in early childhood education and care (ECEC) settings to promote oral health. This study examined children’s perceptions of one such program, the Bright Smiles Bright Futures (BSBF) program in ECEC settings in New South Wales, Australia. Data were collected using focus group discussions from 15 children aged 3–5 years, transcribed verbatim, and analysed through inductive thematic analysis. Three themes were identified as follows: (i) oral health knowledge of children, (ii) oral hygiene practices routine and skills development, and (iii) evaluation of the oral health promotion kit and opportunities for improvement. Children’s perspectives highlight the BSBF program’s success in communicating key messages to promote oral health. The integration of family-centric approaches, acknowledgement of children’s preferences, and the use of interactive tools collectively enhance the overall effectiveness of the oral health promotion program.

Oral Health of Women and Children: Progress, Challenges, and Priorities

Abstract:Introduction: Tooth decay remains the most prevalent chronic disease in children and adults, even though it is largely preventable. Studies show that mothers' oral and overall health is linked to children's oral health and pregnancy outcomes. This paper examines achievements during the last 20 years, assesses current challenges, and discusses future priorities. Oral health status: Data show a modest improvement in children's oral health during the last 20 years; however, tooth decay still affects more than half of adolescents. According to national survey data, about 26% of working-age adults had untreated tooth decay. Overall, significant oral health disparities by race/ethnicity and income persist. Dental service utilization: The annual dental visit rate for children in the 2015 Medical Expenditure Panel Survey was 48%. Among children enrolled in Medicaid, dental visit rates increased from 18% in 1993 to nearly 50% in 2018. About 46% of women are estimated to receive teeth cleaning during pregnancy. Over the years, race or ethnicity and income-level differences in dental visits observed in the early 2000s have narrowed substantially in children but not among pregnant women. Discussion: Many effective interventions are available at the community and individual levels but are underutilized. Lack of integration of oral health into the overall health care system and programs, community conditions, poverty, and limited health literacy make it difficult for families to engage in healthy habits, use preventive interventions, and access treatment promptly. Conclusion: To further improve oral health, policy and system reforms are needed to address the factors mentioned above. Therefore, we urge the federal Maternal and Child Health Bureau to take steps to convene a workshop to develop a framework for future actions.
