Abstract:The objective of the study was to determine the short-term success rate of short and wide single-tooth implants, immediately placed into extraction sockets of the posterior area. A retrospective cohort study design was used. A total of 145 subjects received 162 short and wide-diameter single-tooth implants between 2006 and 2009. A minimal 7-mm residual height and 9-mm ridge width was available in all the implant sites, and the attached gingivae were at least 2 mm wide. All implants were placed and restored with the single crown by one experienced operator. The data were analyzed with descriptive statistics. All implants were placed in molar areas. There were 20 Ankylos implants with a diameter 5.5 or 7 mm and a length of 8 mm and 142 hydroxyapatite-coated implants with a diameter 5 or 6 mm and a length of 5.7 to 8 mm. One of the 162 implants failed before prosthetic restoration, resulting in a survival rate of 99.4% after loading. Patients were followed for up to 56 months (mean = 24 months) after loading of implants. The radiographic and clinical data revealed well-maintained hard and soft tissues with acceptable short-term results. For residual ridges with minimal height but adequate width, the immediate placement of short and wide-diameter implants in fresh extraction sockets may offer a simple and predictable treatment alternative if implants are positioned appropriately after a thorough preoperative analysis.

Abstract:Christopher, my son, has been lost in many places. One of his local adventures was in Sainsbury's. On losing sight of my wife he went to a member of staff and stated that he was lost. He was asked what his mummy looked like. “She's the one with a scarf on her head because she's got nits.” We frequently face the problem of head lice in practice, but our practice team felt uncertain about the most effective treatment. The first step was to design an answerable question. This is the three step question described by Scott Richardson.1 It should contain the problem, the intervention, and the outcome. Our final question was “In children attending primary care with nits what is the most effective treatment for eradication?” I gathered evidence from several sources hoping to gain a consensus. I was looking mainly for well designed clinical trials. A good source of evidence of a therapeutic effect is a meta-analysis of randomised controlled trials. The Cochrane Library is one source of high quality meta-analyses. However, I do not have this in my surgery so I looked up head lice in the Oxford Textbook of Medicine .2 The section on head lice began by giving me quotes from Robert Burns. There was a description of the biology and clinical features of head lice. It said that head lice are resistant to chlorinated hydrocarbons and suggested the use of malathion. “More recently Permethrin has been shown to be equally effective without the potentially toxic side effects (CNS) of Malathion.” It suggested that all these treatments achieve 100% cure rates but gave …
Immediate transmucosal implant placement in molar extraction sites: a 12-month prospective multicenter cohort study

Abstract:Aim: To assess the clinical and radiographic outcomes of immediate transmucosal placement of implants into molar extraction sockets. Study design: Twelve-month multicenter prospective cohort study. Material and methods: Following molar extraction, tapered implants with an endosseous diameter of 4.8 mm and a shoulder diameter of 6.5 mm were immediately placed into the sockets. Molars with evidence of acute periapical pathology were excluded. After implant placement and achievement of primary stability, flaps were repositioned and sutured allowing a non-submerged, transmucosal healing. Peri-implant marginal defects were treated according to the principles of guided bone regeneration (GBR) by means of deproteinized bovine bone mineral particles in conjunction with a bioresrobable collagen membrane. Standardized radiographs were obtained at baseline and 12 months thereafter. Changes in depth and width of the distance from the implant shoulder (IS) and from the alveolar crest (AC) to the bottom of the defect (BD) were assessed. Results: Eighty-two patients (42 males and 40 females) were enrolled and followed for 12 months. They contributed with 82 tapered implants. Extraction sites displayed sufficient residual bone volume to allow primary stability of all implants. Sixty-four percent of the implants were placed in the areas of 36 and 46. GBR was used in conjunction with the placement of all implants. No post-surgical complications were observed. All implants healed uneventfully yielding a survival rate of 100% and healthy soft tissue conditions after 12 months. Radiographically, statistically significant changes (P<0.0001) in mesial and distal crestal bone levels were observed from baseline to the 12-month follow-up. Conclusions: The findings of this 12-month prospective cohort study showed that immediate transmucosal implant placement represented a predictable treatment option for the replacement of mandibular and maxillary molars lost due to reasons other than periodontitis including vertical root fractures, endodontic failures and caries.

Abstract:Background: The aim of this prospective study was to report the outcome of treatment with implants inserted after tooth extraction and immediately loaded. Material and methods: Fifty-six patients with single tooth loss were treated with 116 IPX Galimplant® implants with internal connections and a sandblasted, acid-etched surface. All implants were placed after tooth extraction using a flapless approach without bone regeneration, and they were then immediately loaded with cemented acrylic prostheses. After a period of three months, definitive cemented ceramic prostheses were placed. Patients were examined throughout a total of 4 years of follow-up. Marginal bone loss and survival rates were evaluated using digital periapical radiographs, taking into account clinical variables such as age, gender, smoking, history of periodontitis, etiology of extraction, placement site, diameter, and implant length. The Mann-Whitney U and Kruskal-Wallis non-parametric tests were used to compare differences between subgroups created based on the different clinical variables identified. Results: Clinical results indicate an implant survival and success rate of 97.4%. Three implants were lost. Of the 116 immediate acrylic single crowns initially placed, 113 were replaced with definitive ceramic crowns after 3 months. A total of 77.8% of implants were inserted in the maxilla, while 22.2% were inserted in the mandible. No further complications were reported after the follow-up period (4 years). The mean marginal bone loss was 0.67 mm ± 0.40 mm. No differences were found among the subgroups of study patients. Conclusions: This study indicates that dental implants that are inserted after tooth extraction and immediately loaded may constitute a successful and predictable alternative implant treatment.

Abstract:Abstract Background Immediate implants are frequently employed in the anterior maxillary area. However, the installation of dental implants simultaneously with tooth extraction can also provide with benefits in the posterior areas with a reduction in time prior the recovery of the masticatory function. Results previously reported in the literature show high-survival and success rates for implants placed in extraction sockets in molar areas; however, this topic has received limited systematic analysis. Material and methods Electronic and manual literature searches were performed by two independent reviewers in several data-bases, including MEDLINE, EMBASE, and Cochrane Oral Health Group Trials Register, for articles up to January 2019 reporting outcomes of immediate implants placed in molar areas. Primary outcomes included survival and success rates, as well as marginal bone loss. Secondary outcomes included the influence of implant position, type of implant connection, grafting protocol, flap or flapless approach, implant diameter, surgical phase, presence of buccal plate, and loading protocol. Results Twenty studies provided information on the survival rate, with a total sample of 1.106 implants. The weighted mean survival rate of immediate implants after 1 year of follow-up was 96.6%, and the success rate was 93.3%. On the other hand, marginal bone loss was 1.29 ± 0.24 mm. Secondary outcomes demonstrated that grafting the gap and the loading protocol have an effect on survival and success rates. Similarly, the presence or absence of the buccal bone affect crestal bone levels. Meta-analysis of 4 investigations showed a weighted mean difference of 0.31 mm ± 0.8 IC 95% (0.15–0.46) more marginal bone loss at immediate implant placement versus implants in healed sites ( p < 0.001) I 2 = 15.2%. Conclusion In selected scenarios, immediate implant placement in molar extraction socket might be considered a predictable technique as demonstrated by a high survival and success rates, with minimal marginal bone loss.

Minimally Invasive Extraction and Immediate Implant Placement with Single-Stage Surgical Procedure:A Clinical Observation of 21 Cases

Abstract:OBJECTIVE:To evaluate the clinical results of minimally invasive extraction without flap reflection and immediate implant placement with single-stage surgical procedure.METHODS:Twenty-one patients (23 single-tooth implants) were included in the study. Eight fractured teeth and 15 residual roots in the maxillary anterior region were extracted without flap reflection and implants placed immediately. Special gingival former was connected at the same time of implant placement with single-stage surgical procedure, final prostheses were restored after 6 months. The follow-up time was from 13 months to 51 months (mean: 27.6 months) after final restoration. Clinical and radiographic study were conducted, the interproximal papillae were analyzed according to Jemt's classification and the level of soft-tissue margin and buccal soft tissue color were evaluated according to Fürhauser's method.RESULTS:All 23 implants acquired osseointegration, and the level of marginal bone at the mesial and distal aspects of all implants were stable. The mesial and distal papilla index score of 3 were observed in 18 implant sites, the papilla index score of 2 in 5 implant sites. Harmonious relationship of gingival margin between peri-implant restorations and adjacent natural teeth was observed in 15 implant sites. The color of buccal soft tissue matched that of the adjacent natural teeth in 18 implant sites, 5 sites showed minor discrepancies. All the patients were satisfied with the final restoration.CONCLUSIONS:Minimally invasive extraction without flap reflection and immediate implant placement with single-stage surgical procedure is a sensitive technique. Many factors may affect the final results and the strict control of indication is the key factor.

Abstract:The therapeutic mainstay in the treatment of acral lentiginous melanoma is excision with suffi cient surgical margins in order to prevent possible local tumor recurrence [ 1 ] . At the transition between the sole and the toes in particular, primary closure is rarely feasible. In addition to full-thickness and split-thickness grafts, options for defect closure include free fl ap repair. Secondary intention healing may also be considered but is relatively time-consuming. Given the tissue loss involved, the excision defects are frequently too deep to allow for skin grafting to be performed during the same procedure. In this context, vacuum sealing can be used to induce granulation [ 2–4 ] . However, this technique may not be practical if the plantar excision defects extend as far as the metatarsophalangeal joints. Here, achieving adequate airtightness, which is a sine qua non for vacuum therapy, is challenging. Herein, we like to introduce the ‘fl ipper’ vacuum seal as a suitable technique for the hermetic sealing of plantar excision defects.
Immediate Loading of Conical Connection Implants: Up-to-2-Year Retrospective Clinical and Radiologic Study

Abstract:Purpose: To report the up-to-2-year outcomes of the immediately loaded novel variable-thread tapered implant with an internal conical connection and built-in platform shifting. Materials and methods: Sixty-four partially and completely edentulous patients were referred to two private centers for implant treatments between December 2010 and June 2012. Clinical and radiographic parameters were assessed retrospectively; the outcomes were implant and prosthetic failure, marginal bone remodeling, and complications. Results: One hundred forty-eight NobelReplace Conical Connection implants were placed in both postextraction sockets (n = 67) and healed sites (n = 81). One maxillary postextractive implant failed before the definitive prosthesis delivery, accounting for a cumulative success rate of 99.3%. No definitive prostheses failed. A mean marginal bone loss (MBL) of 0.42 ± 1.16 mm and 0.71 ± 1.53 mm was observed between implant placement and the 1-year (n = 147) and 2-year (n = 74) follow-up periods, respectively. In the postextraction sites, the mean MBL was 0.28 ± 0.39 mm and 0.69 ± 0.75 mm at the 1-year (n = 66) and 2-year (n = 50) follow-up periods; in the healed sites, the mean MBL was 0.51 ± 0.88 mm (n = 81) and 0.62 ± 0.80 mm (n = 24), respectively. At the 2-year follow-up session (n = 74), bleeding on probing was 8.3%, and the plaque score was 15.7%. No technical complications were observed. One biologic complication (0.8%) was reported. Conclusion: Within the limitations of this study and the relatively short follow-up, the immediately loaded NobelReplace Conical Connection implants demonstrated good treatment outcomes with regard to implant and prosthetic survival rate, MBL, and soft tissue conditions, in both postextraction and healed sites.
Influence of Immediate Implant Placement and Provisionalization with or without Soft Tissue Augmentation on Hard and Soft Tissues in the Esthetic Zone: A One-Year Retrospective Study

Abstract:The purpose of this clinical research was to evaluate peri-implant marginal changes around immediate implants placed either with the application of SCTG or XCM or without soft tissue grafting. A total of 48 patients requiring a single implant-supported restoration in the anterior jaw were selected for inclusion. Three surgical procedures were performed, as follows: type 1 implant with subepithelial connective tissue graft (SCTG), type 1 implant with xenogenic collagen matrix (XCM), and type 1 implant without soft tissue augmentation (NG) (control group). The marginal change of peri-implant soft tissue, facial soft tissue thickness (FSTT), peri-implant health status, esthetics, and patient satisfaction were assessed at one year after surgery. All of the placed implants showed a survival rate of 100%. No significant differences in FSTT were recorded between the SCTG group and the XCM group after treatment ( P > 0.05 ), while the NG group presented a significant difference ( P < 0.05 ). Patients in the NG group lost significantly more in the buccal marginal level than did patients in the SCTG group and those in the XCM group ( P < 0.05 ). The favourable success rate recorded in all groups confirmed immediate tooth replacement as a choice of treatment for a missing anterior single tooth. The NG group presented significant changes of FSTT and buccal marginal level, while XCM constituted a viable alternative to SCTG.

Clinical Outcomes of Maxillary Anterior Postextraction Socket Implants with Immediate Provisional Restorations Using a Novel Macro-Hybrid Implant Design: An 18- to 24-Month Single-Cohort Prospective Study

Abstract:Extraction and immediate implant placement/restoration in the esthetic zone is clinically challenging; benefits include fewer surgical appointments and maintenance of peri-implant soft tissues throughout the treatment period, and limitations include gingival recession and bone dehiscence during surgery. Macro-hybrid implants (large-diameter apical/narrow-diameter occlusal) were placed in 19 patients immediately following the extraction of hopeless maxillary anterior teeth. Immediate restorations were fabricated without occlusal contacts. Pre- and postplacement cone beam computed tomography (CBCT) scans were taken. Nineteen implants were available for recall 13 to 25 months postoperatively. The overall implant cumulative survival rate was 100% (range: 13 to 25 months, mean: 19 months), and mean insertion torque value was 65 Ncm. Mean Pink Esthetic Score was 12.63 at 6 months, and was 13 at the 18- to 24-month follow-up. Mean mesial and distal tooth-to-implant distances immediately after implant placement were 2.55 ± 1.29 mm and 2.29 ± 0.82 mm, respectively. Interproximal bone crest width, distance, and height were maintained at implant platforms, mesially and distally, 18 to 24 months postoperative. The results of this study indicated that the macro-hybrid implant geometry for this immediate surgical/restorative protocol provided excellent and stable 2-year results relative to implant survival (100%), labial plate thickness via CBCT evaluations, tooth-to-implant distances immediately post-implant placement, PES, and interproximal bone crest width, distance, and heights, which were maintained at the implant platforms.
Immediate Loading Using the Digitalized Customized Restoration of Single-tooth Implants Placed in Fresh Extraction Sockets in the Aesthetic Anterior Maxilla: A 10-Year Prospective Study of Marginal Bone Level

Abstract:Aim: The objective of this study was to assess marginal bone level around single implants inserted in fresh extraction sockets in the anterior maxillary region and instantly restored with computer-aided design/computer-aided manufacturing customized temporary crowns cemented on the final abutment. Materials and methods: A total of 20 patients (15 females and 5 males, with a mean age of 30 years), where 20 were placed in fresh extraction sockets. After raising a full-thickness flap, atraumatic extraction was performed the implant site was prepared and fixtures were stabilized on the palatal bone wall. The implant location was immediately transmitted to the prepared master model using the pick-up impression coping seated in the surgical guide template. Prefabricated abutments were used as the final abutment on the master model, scanned and the crown was planned using computer-aided manufacturing customized software. Later on 8th weeks, abutments were torqued as per the manufacturer's recommendation, and the final crowns were cemented. Using personalized intraoral radiographs marginal bone level was evaluated mesially and distally to the implant shoulder as a reference at implant placement, 8 weeks, 1, 3, 5, and 10 years after loading. Results: Wholly implants were osteo-integrated positively after 10 years of practical loading, but only 18 were available for clinical and radiological follow-up, and 2 patients with two implants were excluded from the study due to relocation abroad without any implant failure. The average marginal bone loss (MBL) in the current report was 0.16 ± 0.167 mm at crown cementation, 0.275 ± 0.171 mm after 1 year, 0.265 ± 0.171 mm after 3 years, 0.213 ± 0.185 mm after 5 years, and 0.217 ± 0.194 mm at 10 years. Conclusion: The strategy of inserting and not removing the final abutment at the time of implant placement facilitates the establishment of adequate attachment of both soft and hard tissues to the abutment surface, ensuring uninterrupted organization of tissue architecture and offers advantages in helping maintain soft tissue maturation and preventing marginal bone level. Clinical significance: Immediately loaded implants in freshly extracted sockets lead to a significant reduction in marginal ridge resorption. The use of a temporary crown on a prefabricated abutment, exclusive of successive abutment manipulation, proved effective in preserving the primarily founding blood clot and served as a prototype for shaping the soft tissue around the previously wounded gum. Immediate implant and socket preservation using sticky bone and leukocyte-platelet-rich fibrin in the anterior maxilla: A 3-year case report

Abstract:Introduction: Immediate implant in the esthetic area is a challenging procedure. Chronic infection often contributes to extensive bone loss and acute infection often precludes a safe, clean surgery. However, it is quite common to come across failing teeth in the esthetic area, where the patient often demands a faster rehabilitation. Case presentation: In this clinical case, a fractured upper central incisor was replaced by an immediate implant, and the perimetrical area was filled with heterologous bone blended with patient-derived leukocyte- and platelet-rich fibrin (L-PRF) in the form of sticky bone. A Maryland bridge was bonded to the adjacent teeth with the intent to guarantee minimum esthetic to the patient. After 5 months, the implant was connected to a definitive abutment and crown. The facial and interdental soft tissue was maintained with appreciable success after 2 years. The pre-treatment and 2-year- post-treatment computed tomography scans revealed marginal bone preservation. Conclusion: The use of sticky bone and L-PRF in immediate implant sites helps prevent hard and soft-tissue collapse and may favor faster and sounder healing. Nature's Parkin experiment: Nix—a novel protective mechanism in Parkinson's disease

Abstract:Mitophagy, mitochondrial dysfunction, proteasome and lysosome dysfunction, and protein misfolding have key roles in the pathogenesis of Parkinson’s disease (PD). Mitophagy is a mitochondrial housekeeping mechanism mediated by PTEN-induced putative kinase 1 (PINK1) kinase and Parkin ligase. Loss-of-function PINK1 and Parkin mutations cause deregulation of mitophagy and young-onset PD. Parkin was thought to be key to selective mitochondria degradation by autophagy (mitophagy). However, recent studies have emphasized PINK1’s role; autophagy receptors nuclear dot protein 52 kDa (NDP-52) and optineurin can induce mitophagy in Parkin’s absence, but require PINK1. Koentjoro and colleagues report another novel mitophagy pathway, independent of the Parkin/PINK1 pathway by taking advantage of a natural experiment. They studied an asymptomatic woman in her 70s with a homozygous parkin mutation and her symptomatic daughter with a compound heterozygous parkin mutation. Why was the homozygous parkin mutation carrier spared parkinsonism? Through a series of experiments in human cell lines from the asymptomatic carrier, the affected daughter, 2 patients with Parkinrelated PD, and 2 PINK1-related PD, the authors found a protective mechanism in the asymptomatic mother that prevented her from developing PD despite functional Parkin loss. Although the Parkin mutation status differed between the mother and the daughter, functional Parkin loss (absence) was present in both. Mitophagy in the asymptomatic carrier (mother) cell lines was normal as Adenosine triphosphate (ATP) production rate and resistance to the mitochondrial complex 1 inhibitor-rotenone was the same in the carrier and normal control. The absence of PINK1 did not affect Parkin-independent mitophagy in the asymptomatic carrier cells treated with mitophagy inductor (CCCP). PINK1 expression was reduced in asymptomatic carrier cells, regardless of carbonyl cyanide m-chlorophenylhydrazone (CCCP), suggesting the presence of a protective PINK1/Parkin independent mechanism. The authors then studied Nix, which is the selective autophagic receptor Nip3-like protein X that mediates mitophagy in mice fibroblasts, rescues drosophila flies with PINK1 mutations, and acts as a parkin substrate in Human embryonic kidney cells 293 (HEK293A) cells. Although Nix also has a role in Parkin ligase translocation for the ubiquitination (mitochondrial labelling), the authors demonstrated Nixinduced mitophagy with Parkin functional loss. Nix levels were significantly higher in the asymptomatic mother when compared with the affected daughter and control. Mitophagy (CCCP induced) was absent when Nix was silenced. Moreover, Nix overexpression increased the ATP synthesis in Parkin and PINK1 cells, restored mitophagy (CCCP induced), and led to functional enhancement. Studies involving mechanisms affecting vesicular transport and mitochondrial clearance are ongoing. Nix may be useful in PD therapeutics once its exact action mechanism in this setting is known.
Immediately provisionalized tapered conical connection implants for single-tooth restorations in the maxillary esthetic zone: a 5-year prospective single-cohort multicenter analysis

Abstract:Abstract Objectives This open, single-cohort, multicenter, prospective study investigated the efficacy of immediately provisionalized tapered conical connection implant for single-tooth restorations in the anterior and premolar regions of the maxilla after 5 years of function. Materials and methods All implants were placed in healed sites and immediately provisionalized. MBLs, soft-tissue parameters, and oral-health impact profile (OHIP) were evaluated at implant insertion, 6, 12, 24, 36, and 60 months. Paired Wilcoxon signed-rank tests and Kaplan–Meier survival analysis was used for statistical and implant survival/success analyses, respectively. Results Seventy-seven patients (81 implants) completed the 5-year follow-up. The 5-year cumulative survival and success rates were 97.8%, and the mean MBL change from implant insertion to 5 years was − 0.80 ± 1.13 mm. Optimal papilla index scores were observed at 90.1% of sites at 5 years compared with 32.8% of sites at insertion. Pink esthetic score, modified bleeding and plaque indices, and OHIP showed statistically significant improvement at the 5-year follow-up. Conclusions Immediately provisionalized tapered conical connection implants promote marginal bone stability and excellent esthetic outcomes after 5 years of function. Clinical relevance This treatment is a viable option for patients requiring immediately provisionalized single-tooth restorations in the esthetic zone and shows favorable long-term clinical outcomes, including marginal bone stability and excellent esthetics.

Abstract:Background: In previous studies, immediate implant placement in molar sites has been widely applied. Purpose: To study the clinical effect and feasibility of immediate implant placement combined with maxillary sinus floor elevation utilizing the transalveolar approach and nonsubmerged healing for failing teeth in the maxillary molar area. Material and methods: Patients who required implantation surgry to replace a failing tooth in the maxilla molar region were selected. Patients were randomized into two groups: immediate implant placement combined with maxillary sinus floor elevation utilizing the transalveolar approach and nonsubmerged healing (test group) or delayed implant placement combined with maxillary sinus floor elevation utilizing the transalveolar approach and nonsubmerged healing (control group). The outcome criteria were the success rates of implants, Cone Beam Computer Tomography (CBCT) data and results of the subjective satisfaction survey performed with a visual analog scale (VAS). Results: All implants had good initial stability after implantation. The survival rate of implants was 100% at 1-year follow-up. At the time of permanent restoration, the differences in average horizontal shrinkage of alveolar bone (W1) on the buccal side between the test group (0.65 ± 0.12 mm) and the control group (1.23 ± 0.32 mm) were statistically significant (P < .0001); however, no statistically significant difference (P = .515) was observed on the palatal side (0.3 ± 0.10 mm vs 0.28 ± 0.08 mm). The difference in vertical resorption of alveolar bone (H1) on the buccal side between the test group (0.60 ± 0.18 mm) and the control group (1.53 ± 0.19 mm) was statistically significant (P < .0001), but no statistically significant difference (P = .190) in the reduction of palatal alveolar bone (0.24 ± 0.12 mm vs 0.29 ± 0.13 mm) was observed. After 1-year loading, no statistically significant difference (P > .05) in vertical or horizontal changes (W2, H2) were identified in the test group or control group. Patient satisfaction in both groups was similar (8.36 ± 1.01 vs 8.14 ± 1.35), and the difference between groups was not statistically significant (P = .638). Conclusion: Immediate implant placement combined with maxillary sinus floor elevation utilizing the transalveolar approach and nonsubmerged healing is feasible for the maxillary molar area, and the clinical effect is satisfactory.
Immediate versus conventional loading in short and ultra-short implants: a protocol design

Abstract:Background: After tooth extraction, inevitable resorption of the postextraction socket occurs. Placing an immediate implant does not fully preserve the bone integrity of the socket. Preservation techniques cannot avoid alveolar resorption. In residual atrophies, bone augmentation techniques are necessary for subsequent rehabilitation with implants. The aim of this study was to propose a protocol of immediate loading in short implants as an alternative to bone augmentation techniques, comparing the immediate versus conventional loading, evaluating peri-implant bone loss, the biological and prosthetic complications, the survival of the implant and prosthesis and the patient satisfaction after one year. Methods: This study consists of a randomized controlled clinical trial. For implant planning, periapical intraoral radiographs and Cone Beam Computed Tomography will be used. All patients will receive Straumann Group® (Basel, Switzerland) titanium-zirconia implants with a 4.1mm or 4.8mm diameter and a 4 mm or 6 mm length. Results: The expected results show similar or even minor bone loss, biological and prosthetic complications, identical or even better survival of the short implants with immediate loading, and greater satisfaction. Conclusions: Short implants demonstrate similar survival to conventional length implants, with comparable or even minor biological and prosthetic complications and bone loss. Immediate loading gives satisfactory results similar to conventional loading, with similar biological and prosthetic complications, and generally depends on poor hygiene levels. Bone loss is even less, and the satisfaction increases.

Abstract:Objectives To evaluate clinical, clinician‐ and patient‐reported outcomes (PROs) of two adjacent single posterior implants immediately loaded with definitive single crowns up to 1 year. Ten patients in need of two adjacent posterior single implants were included. All implants were placed applying a fully digital workflow. Definitive screw‐retained single zirconia crowns were delivered within 72 h after implant placement. Clinical parameters, patient‐ and clinician‐reported outcomes were assessed up to 1 year of follow‐up. Clinical Considerations Clinical outcomes remained stable, with no implant failures or technical and biological complications throughout the observation period. Patient satisfaction was very high at baseline (crown delivery) and remained consistently high at 6 and 12 months of follow‐up. Crown insertion 3 days after implant placement was rated as an easy procedure by clinicians. Conclusions Two adjacent single implants with immediate definitive restorations (<72 h) in the posterior region appear to be a viable and easy treatment option to shorten the overall treatment time and potentially enhancing patient satisfaction. However, randomized controlled trials are needed to confirm the advantages of this treatment protocol over a delayed loading approach. Clinical Significance In selected cases, immediate implant loading with definitive restorations in the posterior region appears a valuable and straightforward option to shorten the overall treatment time.

Abstract:Purpose To assess marginal bone level changes, implant and restoration survival, condition of the peri‐implant mucosa, and the patient's satisfaction with the single restoration supported by a 6‐mm long implant in the posterior mandible after 10 years in function. Materials and Methods Twenty‐one consecutive patients missing premolars or molars in the posterior mandible, with a bone volume consisting of a width of at least 6 mm and an estimated height of 8 mm between the top of the ridge and alveolar nerve, were included. Each patient received one or more 6‐mm implants. After 3 months, the implants were restored with custom‐made titanium abutments and cemented zirconia‐based porcelain crowns. The clinical examination and radiograph data were assessed at restoration placement and after 12, 60, and 120 months. The patients answered a questionnaire to score their satisfaction before treatment and after 12, 60, and 120 months with the restoration in function. Results A total of 31 implants were placed. Implant survival was 100%. The 10‐year mean marginal bone loss was 0.18 mm (SE: 0.08). The plaque, calculus, gingiva, and bleeding indices scores were low as was the mean pocket probing depth. The patients' satisfaction was high. Conclusion The 10‐year follow‐up data of this limited case series study reveal that 6‐mm dental implants inserted in the resorbed posterior mandible provide a solid basis for single tooth restorations.
