Abstract:Background and PurposeIschemic diffusion-weighted imaging-fluid-attenuated inversion recovery (DWI-FLAIR) mismatch may be useful in guiding acute stroke treatment decisions given its relationship to onset time and parenchymal viability; however, it relies on subjective grading. Radiomics is an emerging image quantification methodology that may objectively represent continuous image characteristics. We propose a novel radiomics approach to characterize DWI-FLAIR mismatch. MethodsIschemic lesions were visually graded for FLAIR positivity (absent, subtle, obvious) among consecutive large vessel occlusion stroke patients who underwent hyperacute MRI. Radiomic features were extracted from within the lesions on DWI and FLAIR. The DWI-FLAIR mismatch radiomics signature was built with features systematically selected by a cross-validated ElasticNet linear regression model of mismatch. ResultsWe identified 103 patients with mean age 68 ± 16 years; 63% were female. FLAIR hyperintensity was absent in 25%, subtle in 55%, and obvious in 20%. Inter-rater agreement for visual grading was moderate (Κ = .58). The radiomics signature of DWI-FLAIR mismatch included native FLAIR histogram kurtosis and local binary pattern-filtered FLAIR gray-level cluster shade; both correlated with visual grading (ρ = –.42, p < .001 and ρ = .40, p < .001, respectively). ConclusionsRadiomics can describe DWI-FLAIR mismatch and may provide objective, continuous biomarkers for infarct evolution using clinical-grade images. These novel biomarkers may prove useful for treatment decisions and future research.

DeepFLAIR: A neural network approach to mitigate signal and contrast loss in temporal lobes at 7 Tesla FLAIR images

Abstract:Background and purpose: Higher magnetic field strength introduces stronger magnetic field inhomogeneities in the brain, especially within temporal lobes, leading to image artifacts. Particularly, T2-weighted fluid-attenuated inversion recovery (FLAIR) images can be affected by these artifacts. Here, we aimed to improve the FLAIR image quality in temporal lobe regions through image processing of multiple contrast images via machine learning using a neural network. Methods: Thirteen drug-resistant MR-negative epilepsy patients (age 29.2 ± 9.4y, 5 females) were scanned on a 7 T MRI scanner. Magnetization-prepared (MP2RAGE) and saturation-prepared with 2 rapid gradient echoes, multi-echo gradient echo with four echo times, and the FLAIR sequence were acquired. A voxel-wise neural network was trained on extratemporal-lobe voxels from the acquired structural scans to generate a new FLAIR-like image (i.e., deepFLAIR) with reduced temporal lobe inhomogeneities. The deepFLAIR was evaluated in temporal lobes through signal-to-noise (SNR), contrast-to-noise (CNR) ratio, the sharpness of the gray-white matter boundary and joint-histogram analysis. Saliency mapping demonstrated the importance of each input image per voxel. Results: SNR and CNR in both gray and white matter were significantly increased (p < 0.05) in the deepFLAIR's temporal ROIs, compared to the FLAIR. The gray-white matter boundary sharpness was either preserved or improved in 10/13 right-sided temporal regions and was found significantly increased in the ROIs. Multiple image contrasts were influential for the deepFLAIR reconstruction with the MP2RAGE second inversion image being the most important. Conclusions: The deepFLAIR network showed promise to restore the FLAIR signal and reduce contrast attenuation in temporal lobe areas. This may yield a valuable tool, especially when artifact-free FLAIR images are not available.
Synthetic FLAIR as a Substitute for FLAIR Sequence in Acute Ischemic Stroke.

Abstract:Background In acute ischemic stroke (AIS), fluid-attenuated inversion recovery (FLAIR) is used for treatment decisions when onset time is unknown. Synthetic FLAIR could be generated with deep learning from information embedded in diffusion-weighted imaging (DWI) and could replace acquired FLAIR sequence (real FLAIR) and shorten MRI duration. Purpose To compare performance of synthetic and real FLAIR for DWI-FLAIR mismatch estimation and identification of patients presenting within 4.5 hours from symptom onset. Materials and Methods In this retrospective study, all pretreatment and early follow-up (<48 hours after symptom onset) MRI data sets including DWI (b = 0-1000 sec/mm2) and FLAIR sequences obtained in consecutive patients with AIS referred for reperfusion therapies between January 2002 and May 2019 were included. On the training set (80%), a generative adversarial network was trained to produce synthetic FLAIR with DWI as input. On the test set (20%), synthetic FLAIR was computed without real FLAIR knowledge. The DWI-FLAIR mismatch was evaluated on both FLAIR data sets by four independent readers. Interobserver reproducibility and DWI-FLAIR mismatch concordance between synthetic and real FLAIR were evaluated with κ statistics. Sensitivity and specificity for identification of AIS within 4.5 hours were compared in patients with known onset time by using McNemar test. Results The study included 1416 MRI scans (861 patients; median age, 71 years [interquartile range, 57-81 years]; 375 men), yielding 1134 and 282 scans for training and test sets, respectively. Regarding DWI-FLAIR mismatch, interobserver reproducibility was substantial for real and synthetic FLAIR (κ = 0.80 [95% CI: 0.74, 0.87] and 0.80 [95% CI: 0.74, 0.87], respectively). After consensus, concordance between real and synthetic FLAIR was almost perfect (κ = 0.88; 95% CI: 0.82, 0.93). Diagnostic value for identifying AIS within 4.5 hours did not differ between real and synthetic FLAIR (sensitivity: 107 of 131 [82%] vs 111 of 131 [85%], P = .2; specificity: 96 of 104 [92%] vs 96 of 104 [92%], respectively, P > .99). Conclusion Synthetic fluid-attenuated inversion recovery (FLAIR) had diagnostic performances similar to real FLAIR in depicting diffusion-weighted imaging-FLAIR mismatch and in helping to identify early acute ischemic stroke, and it may accelerate MRI protocols. © RSNA, 2022 Online supplemental material is available for this article. Abstract:Introduction Deep learning reconstruction (DLR) technologies are the latest methods attempting to solve the enduring problem of reducing MRI acquisition times without compromising image quality. The clinical utility of this reconstruction technique is yet to be fully established. This study aims to assess whether a commercially available DLR technique applied to 2D T2‐weighted FLAIR brain images allows a reduction in scan time, without compromising image quality and thus diagnostic accuracy. Methods 47 participants (24 male, mean age 55.9 ± 18.7 SD years, range 20–89 years) underwent routine, clinically indicated brain MRI studies in March 2022, that included a standard‐of‐care (SOC) T2‐weighted FLAIR sequence, and an accelerated acquisition that was reconstructed using the DLR denoising product. Overall image quality, lesion conspicuity, signal‐to‐noise ratio (SNR), contrast‐to‐noise ratio (CNR), and artefacts for each sequence, and preferred sequence on direct comparison, were subjectively assessed by two readers. Abstract:Purpose To evaluate whether deep learning reconstruction (DLR) can accelerate the acquisition of magnetic resonance imaging (MRI) sequences of the knee for clinical use. Methods Using a 1.5-T MRI scanner, sagittal fat-suppressed T2-weighted imaging (fs-T2WI), coronal proton density-weighted imaging (PDWI), and coronal T1-weighted imaging (T1WI) were performed. DLR was applied to images with a number of signal averages (NSA) of 1 to obtain 1DLR images. Then 1NSA, 1DLR, and 4NSA images were compared subjectively, and by noise (standard deviation of intra-articular water or medial meniscus) and contrast-to-noise ratio between two anatomical structures or between a anatomical structure and intra-articular water. Results Twenty-seven healthy volunteers (age: 40.6 ± 11.9 years) were enrolled. Three 1DLR image sequences were obtained within 200 s (approximately 12 minutes for 4NSA image). According to objective evaluations, PDWI 1DLR images showed the smallest noise and significantly higher contrast than 1NSA and 4NSA images. For fs-T2WI, smaller noise and higher contrast was observed in the order of 4NSA, 1DLR and 1NSA images. According to the subjective analysis, structure visibility, image noise, and overall image quality were significantly better for PDWI 1DLR than 1NSA images; moreover, the visibility of the meniscus and bone, image noise, and overall image quality were significantly better for 1DLR than 4NSA images. Fs-T2WI and T1WI 1DLR images showed no difference between 1DLR and 4NSA images. Conclusion Compared to PDWI 4NSA images, PDWI 1DLR images were of higher quality, while the quality of fs-T2WI and T1WI 1DLR images was similar to that of 4NSA images.

Deep learning reconstruction of diffusion-weighted brain MRI for evaluation of patients with acute neurologic symptoms

Abstract:Purpose: We aimed to evaluate whether the deep-learning (DL) accelerated diffusion weighted image (DWI) is clinically feasible for evaluating patients with acute neurologic symptoms, regarding its shorter study time and acceptable image quality. Materials and methods: In this retrospective study, brain images obtained at DWI with a b-value of 0 s/mm2 and DWI with a b-value of 1000 s/mm2 (DWI 1000) from 321 consecutive patients with acute stroke-like symptom were reconstructed with and without DL algorithm. We compare the diagnostic performance between DL-DWI and conventional DWI for detecting brain lesions, including acute infarction. We assessed the diagnostic accuracy of conventional DWI and DL-DWI and compared the results. Qualitative analysis based on image quality was assessed and compared using a five-point visual scoring system. Apparent diffusion coefficients (ADCs) from DWI with and without DL were also compared. Results: The mean acquisition time for the DL-DWI (49 s) was significantly shorter (P < 0.001) than conventional DWI (165 s). Both DWI with and without DL showed similar performance in diagnosing brain lesions especially sensitivity (98.8% in both DWI and DL-DWI) and specificity (99.5% in both DWI and DL-DWI). Overall image quality, gray-white matter and deep gray matter differentiation of two sequences were similar. DL DWI showed more artifacts than DWI. Lesion conspicuity, especially smaller than 5 mm, was better with DL DWI than conventional DWI (p = 0.03). ADC values of white matter, deep gray matter, and pons with DL were lower than conventional DWI. Conclusions: Compared to conventional DWI, DL-DWI achieved comparable image quality and brain lesion visualization for acute neurological symptoms, with a significantly shorter scan time.
Multicenter validation of synthetic FLAIR as a substitute for FLAIR sequence in acute ischemic stroke

Abstract:Purpose: To evaluate performance of synthetic and real FLAIR for identifying early stroke in a multicenter cohort. Methods: This retrospective study was conducted using DWI and FLAIR extracted from the Endovascular Treatment in Ischemic Stroke image registry (2017-2021). The database was partitioned into subsets according to MRI field strength and manufacturer, and randomly divided into training set (70%) used for model fine-tuning, validation set (15%), and test set (15%). In test set, five readers, blinded to FLAIR sequence type, assessed DWI-FLAIR mismatch using real and synthetic FLAIR. Interobserver agreement for DWI-FLAIR rating and concordance between synthetic and real FLAIR were evaluated with kappa statistics. Sensitivity and specificity for identification of ⩽4.5 h AIS were compared in patients with known onset-to-MRI delay using McNemar's test. Results: 1454 complete MRI sets (1172 patients, median (IQR) age: 73 years (62-82); 762 women) acquired on 125 MRI units were analyzed. In test set (207 MRI), interobserver reproducibility for DWI-FLAIR mismatch labeling was substantial for real and synthetic FLAIR (Fleiss κ = 0.79 (95%CI: 0.73-0.84) and 0.77 (95%CI: 0.71-0.82), respectively). After consensus, concordance between real and synthetic FLAIR was excellent (κ = 0.85 (95%CI: 0.78-0.92)). In 141 MRI sets with known onset-to-MRI delay, diagnostic performances for ⩽4.5 h AIS identification did not differ between real and synthetic FLAIR (sensitivity: 60/71 (85%) vs 59/71 (83%), p = .56; specificity: 65/70 (93%) vs 65/70 (93%), p > 0.99). Conclusion: A deep-learning-based FLAIR fine-tuned on multicenter data can provide comparable performances to real FLAIR for early AIS identification. This approach may help reducing MR protocol duration and motion artifacts.
Deep Learning Enables 60% Accelerated Volumetric Brain MRI While Preserving Quantitative Performance: A Prospective, Multicenter, Multireader Trial

Abstract:Background and purpose: In this prospective, multicenter, multireader study, we evaluated the impact on both image quality and quantitative image-analysis consistency of 60% accelerated volumetric MR imaging sequences processed with a commercially available, vendor-agnostic, DICOM-based, deep learning tool (SubtleMR) compared with that of standard of care. Materials and methods: Forty subjects underwent brain MR imaging examinations on 6 scanners from 5 institutions. Standard of care and accelerated datasets were acquired for each subject, and the accelerated scans were enhanced with deep learning processing. Standard of care, accelerated scans, and accelerated-deep learning were subjected to NeuroQuant quantitative analysis and classified by a neuroradiologist into clinical disease categories. Concordance of standard of care and accelerated-deep learning biomarker measurements were assessed. Randomized, side-by-side, multiplanar datasets (360 series) were presented blinded to 2 neuroradiologists and rated for apparent SNR, image sharpness, artifacts, anatomic/lesion conspicuity, image contrast, and gray-white differentiation to evaluate image quality. Results: Accelerated-deep learning was statistically superior to standard of care for perceived quality across imaging features despite a 60% sequence scan-time reduction. Both accelerated-deep learning and standard of care were superior to accelerated scans for all features. There was no difference in quantitative volumetric biomarkers or clinical classification for standard of care and accelerated-deep learning datasets. Conclusions: Deep learning reconstruction allows 60% sequence scan-time reduction while maintaining high volumetric quantification accuracy, consistent clinical classification, and what radiologists perceive as superior image quality compared with standard of care. This trial supports the reliability, efficiency, and utility of deep learning-based enhancement for quantitative imaging. Shorter scan times may heighten the use of volumetric quantitative MR imaging in routine clinical settings.
Prospective Deployment of Deep Learning Reconstruction Facilitates Highly Accelerated Upper Abdominal MRI

Abstract:Rationale and objectives: To compare a conventional T1 volumetric interpolated breath-hold examination (VIBE) with SPectral Attenuated Inversion Recovery (SPAIR) fat saturation and a deep learning (DL)-reconstructed accelerated VIBE sequence with SPAIR fat saturation achieving a 50 % reduction in breath-hold duration (hereafter, VIBE-SPAIRDL) in terms of image quality and diagnostic confidence. Materials and methods: This prospective study enrolled consecutive patients referred for upper abdominal MRI from November 2023 to December 2023 at a single tertiary center. Patients underwent upper abdominal MRI with acquisition of non-contrast and gadobutrol-enhanced conventional VIBE-SPAIR (fourfold acceleration, acquisition time 16 s) and VIBE-SPAIRDL (sixfold acceleration, acquisition time 8 s) on a 1.5 T scanner. Image analysis was performed by four readers, evaluating homogeneity of fat suppression, perceived signal-to-noise ratio (SNR), edge sharpness, artifact level, lesion detectability and diagnostic confidence. A statistical power analysis for patient sample size estimation was performed. Image quality parameters were compared by a repeated measures analysis of variance, and interreader agreement was assessed using Fleiss' κ. Results: Among 450 consecutive patients, 45 patients were evaluated (mean age, 60 years ± 15 [SD]; 27 men, 18 women). VIBE-SPAIRDL acquisition demonstrated superior SNR (P < 0.001), edge sharpness (P < 0.001), and reduced artifacts (P < 0.001) with substantial to almost perfect interreader agreement for non-contrast (κ: 0.70-0.91) and gadobutrol-enhanced MRI (κ: 0.68-0.87). No evidence of a difference was found between conventional VIBE-SPAIR and VIBE-SPAIRDL regarding homogeneity of fat suppression, lesion detectability, or diagnostic confidence (all P > 0.05). Conclusion: Deep learning reconstruction of VIBE-SPAIR facilitated a reduction of breath-hold duration by half, while reducing artifacts and improving image quality. Summary: Deep learning reconstruction of prospectively accelerated T1 volumetric interpolated breath-hold examination for upper abdominal MRI enabled a 50 % reduction in breath-hold time with superior image quality. Key results: 1) In a prospective analysis of 45 patients referred for upper abdominal MRI, accelerated deep learning (DL)-reconstructed VIBE images with spectral fat saturation (SPAIR) showed better overall image quality, with better perceived signal-to-noise ratio and less artifacts (all P < 0.001), despite a 50 % reduction in acquisition time compared to conventional VIBE. 2) No evidence of a difference was found between conventional VIBE-SPAIR and accelerated VIBE-SPAIRDL regarding lesion detectability or diagnostic confidence.
Improving the lesion appearance on FLAIR images synthetized from quantitative MRI: a fast, hybrid approach

Abstract:Objective: The image quality of synthetized FLAIR (fluid attenuated inversion recovery) images is generally inferior to its conventional counterpart, especially regarding the lesion contrast mismatch. This work aimed to improve the lesion appearance through a hybrid methodology. Materials and methods: We combined a full brain 5-min MR-STAT acquisition followed by FLAIR synthetization step with an ultra-under sampled conventional FLAIR sequence and performed the retrospective and prospective analysis of the proposed method on the patient datasets and a healthy volunteer. Results: All performance metrics of the proposed hybrid FLAIR images on patient datasets were significantly higher than those of the physics-based FLAIR images (p < 0.005), and comparable to those of conventional FLAIR images. The small difference between prospective and retrospective analysis on a healthy volunteer demonstrated the validity of the retrospective analysis of the hybrid method as presented for the patient datasets. Discussion: The proposed hybrid FLAIR achieved an improved lesion appearance in the clinical cases with neurological diseases compared to the physics-based FLAIR images, Future prospective work on patient data will address the validation of the method from a diagnostic perspective by radiological inspection of the new images over a larger patient cohort.
Deep learning-based reconstruction of highly accelerated 3D MRI

Abstract:Purpose: To accelerate brain 3D MRI scans by using a deep learning method for reconstructing images from highly-undersampled multi-coil k-space data Methods: DL-Speed, an unrolled optimization architecture with dense skip-layer connections, was trained on 3D T1-weighted brain scan data to reconstruct complex-valued images from highly-undersampled k-space data. The trained model was evaluated on 3D MPRAGE brain scan data retrospectively-undersampled with a 10-fold acceleration, compared to a conventional parallel imaging method with a 2-fold acceleration. Scores of SNR, artifacts, gray/white matter contrast, resolution/sharpness, deep gray-matter, cerebellar vermis, anterior commissure, and overall quality, on a 5-point Likert scale, were assessed by experienced radiologists. In addition, the trained model was tested on retrospectively-undersampled 3D T1-weighted LAVA (Liver Acquisition with Volume Acceleration) abdominal scan data, and prospectively-undersampled 3D MPRAGE and LAVA scans in three healthy volunteers and one, respectively. Results: The qualitative scores for DL-Speed with a 10-fold acceleration were higher than or equal to those for the parallel imaging with 2-fold acceleration. DL-Speed outperformed a compressed sensing method in quantitative metrics on retrospectively-undersampled LAVA data. DL-Speed was demonstrated to perform reasonably well on prospectively-undersampled scan data, realizing a 2-5 times reduction in scan time. Conclusion: DL-Speed was shown to accelerate 3D MPRAGE and LAVA with up to a net 10-fold acceleration, achieving 2-5 times faster scans compared to conventional parallel imaging and acceleration, while maintaining diagnostic image quality and real-time reconstruction. The brain scan data-trained DL-Speed also performed well when reconstructing abdominal LAVA scan data, demonstrating versatility of the network.
Deep Learning Accelerated Brain Diffusion-Weighted MRI with Super Resolution Processing

Abstract:Objectives To investigate the clinical feasibility and image quality of accelerated brain diffusion-weighted imaging (DWI) with deep learning image reconstruction and super resolution. Methods 85 consecutive patients with clinically indicated MRI at a 3 T scanner were prospectively included. Conventional diffusion-weighted data (c-DWI) with four averages were obtained. Reconstructions of one and two averages, as well as deep learning diffusion-weighted imaging (DL-DWI), were accomplished. Three experienced readers evaluated the acquired data using a 5-point Likert scale regarding overall image quality, overall contrast, diagnostic confidence, occurrence of artefacts and evaluation of the central region, basal ganglia, brainstem, and cerebellum. To assess interrater agreement, Fleiss' kappa (κ) was determined. Signal intensity (SI) levels for basal ganglia and the central region were estimated via automated segmentation, and SI values of detected pathologies were measured. Results Intracranial pathologies were identified in 35 patients. DL-DWI was significantly superior for all defined parameters, independently from applied averages (p-value < 0.001). Optimum image quality was achieved with DL-DWI by utilizing a single average (p-value < 0.001), demonstrating very good (80.9%) to excellent image quality (14.5%) in nearly all cases, compared to 12.5% with very good and 0% with excellent image quality for c-MRI (p-value < 0.001). Comparable results could be shown for diagnostic confidence. Inter-rater Fleiss' Kappa demonstrated moderate to substantial agreement for virtually all defined parameters, with good accordance, particularly for the assessment of pathologies (p = 0.74). Regarding SI values, no significant difference was found. Conclusion Ultra-fast diffusion-weighted imaging with super resolution is feasible, resulting in highly accelerated brain imaging while increasing diagnostic image quality.

Deep Learning k-Space-to-Image Reconstruction Facilitates High Spatial Resolution and Scan Time Reduction in Diffusion-Weighted Imaging Breast MRI

Abstract:Background: For time-consuming diffusion-weighted imaging (DWI) of the breast, deep learning-based imaging acceleration appears particularly promising. Purpose: To investigate a combined k-space-to-image reconstruction approach for scan time reduction and improved spatial resolution in breast DWI. Study type: Retrospective. Population: 133 women (age 49.7 ± 12.1 years) underwent multiparametric breast MRI. Field strength/sequence: 3.0T/T2 turbo spin echo, T1 3D gradient echo, DWI (800 and 1600 sec/mm2). Assessment: DWI data were retrospectively processed using deep learning-based k-space-to-image reconstruction (DL-DWI) and an additional super-resolution algorithm (SRDL-DWI). In addition to signal-to-noise ratio and apparent diffusion coefficient (ADC) comparisons among standard, DL- and SRDL-DWI, a range of quantitative similarity (e.g., structural similarity index [SSIM]) and error metrics (e.g., normalized root mean square error [NRMSE], symmetric mean absolute percent error [SMAPE], log accuracy error [LOGAC]) was calculated to analyze structural variations. Subjective image evaluation was performed independently by three radiologists on a seven-point rating scale. Statistical tests: Friedman's rank-based analysis of variance with Bonferroni-corrected pairwise post-hoc tests. P < 0.05 was considered significant. Results: Both DL- and SRDL-DWI allowed for a 39% reduction in simulated scan time over standard DWI (5 vs. 3 minutes). The highest image quality ratings were assigned to SRDL-DWI with good interreader agreement (ICC 0.834; 95% confidence interval 0.818-0.848). Irrespective of b-value, both standard and DL-DWI produced superior SNR compared to SRDL-DWI. ADC values were slightly higher in SRDL-DWI (+0.5%) and DL-DWI (+3.4%) than in standard DWI. Structural similarity was excellent between DL-/SRDL-DWI and standard DWI for either b value (SSIM ≥ 0.86). Calculation of error metrics (NRMSE ≤ 0.05, SMAPE ≤ 0.02, and LOGAC ≤ 0.04) supported the assumption of low voxel-wise error. Data conclusion: Deep learning-based k-space-to-image reconstruction reduces simulated scan time of breast DWI by 39% without influencing structural similarity. Additionally, super-resolution interpolation allows for substantial improvement of subjective image quality. Evidence level: 4 TECHNICAL EFFICACY: Stage 1.

Abstract:Background: Diffusion-weighted images (DWI) obtained by echo-planar imaging (EPI) are frequently degraded by susceptibility artifacts. It has been suggested that DWI obtained by fast advanced spin-echo (FASE) or reconstructed with deep learning reconstruction (DLR) could be useful for image quality improvements. The purpose of this investigation using in vitro and in vivo studies was to determine the influence of sequence difference and of DLR for DWI on image quality, apparent diffusion coefficient (ADC) evaluation, and differentiation of malignant from benign head and neck tumors. Methods: For the in vitro study, a DWI phantom was scanned by FASE and EPI sequences and reconstructed with and without DLR. Each ADC within the phantom for each DWI was then assessed and correlated for each measured ADC and standard value by Spearman's rank correlation analysis. For the in vivo study, DWIs obtained by EPI and FASE sequences were also obtained for head and neck tumor patients. Signal-to-noise ratio (SNR) and ADC were then determined based on ROI measurements, while SNR of tumors and ADC were compared between all DWI data sets by means of Tukey's Honest Significant Difference test. Results: For the in vitro study, all correlations between measured ADC and standard reference were significant and excellent (0.92 ≤ ρ ≤ 0.99, p < 0.0001). For the in vivo study, the SNR of FASE with DLR was significantly higher than that of FASE without DLR (p = 0.02), while ADC values for benign and malignant tumors showed significant differences between each sequence with and without DLR (p < 0.05). Conclusion: In comparison with EPI sequence, FASE sequence and DLR can improve image quality and distortion of DWIs without significantly influencing ADC measurements or differentiation capability of malignant from benign head and neck tumors.

Abstract:Deep learning (DL) methods for white matter lesion (WML) segmentation in MRI suffer a reduction in performance when applied on data from a scanner or centre that is out-of-distribution (OOD) from the training data. This is critical for translation and widescale adoption, since current models cannot be readily applied to data from new institutions. In this work, we evaluate several intensity standardization methods for MRI as a preprocessing step for WML segmentation in multi-centre Fluid-Attenuated Inversion Recovery (FLAIR) MRI. We evaluate a method specifically developed for FLAIR MRI called IAMLAB along with other popular normalization techniques such as White-strip, Nyul and Z-score. We proposed an Ensemble model that combines predictions from each of these models. A skip-connection UNet (SC UNet) was trained on the standardized images, as well as the original data and segmentation performance was evaluated over several dimensions. The training (in-distribution) data consists of a single study, of 60 volumes, and the test (OOD) data is 128 unseen volumes from three clinical cohorts. Results show IAMLAB and Ensemble provide higher WML segmentation performance compared to models from original data or other normalization methods. IAMLAB & Ensemble have the highest dice similarity coefficient (DSC) on the in-distribution data (0.78 & 0.80) and on clinical OOD data. DSC was significantly higher for IAMLAB compared to the original data (p<0.05) for all lesion categories (LL>25mL: 0.77 vs. 0.71; 10mL<= LL<25mL: 0.66 vs. 0.61; LL<10mL: 0.53 vs. 0.52). The IAMLAB and Ensemble normalization methods are mitigating MRI domain shift and are optimal for DL-based WML segmentation in unseen FLAIR data.

Abstract:Objective The aim of this study was to assess the utility of the combined use of 3D wheel sampling and deep learning–based reconstruction (DLR) for intracranial high-resolution (HR)–time-of-flight (TOF)–magnetic resonance angiography (MRA) at 3 T. Methods This prospective study enrolled 20 patients who underwent head MRI at 3 T, including TOF-MRA. We used 3D wheel sampling called “fast 3D” and DLR for HR-TOF-MRA (spatial resolution, 0.39 × 0.59 × 0.5 mm 3 ) in addition to conventional MRA (spatial resolution, 0.39 × 0.89 × 1 mm 3 ). We compared contrast and contrast-to-noise ratio between the blood vessels (basilar artery and anterior cerebral artery) and brain parenchyma, full width at half maximum in the P3 segment of the posterior cerebral artery among 3 protocols. Two board-certified radiologists evaluated noise, contrast, sharpness, artifact, and overall image quality of 3 protocols. Results The contrast and contrast-to-noise ratio of fast 3D-HR-MRA with DLR are comparable or higher than those of conventional MRA and fast 3D-HR-MRA without DLR. The full width at half maximum was significantly lower in fast 3D-MRA with and without DLR than in conventional MRA ( P = 0.006, P < 0.001). In qualitative evaluation, fast 3D-MRA with DLR had significantly higher sharpness and overall image quality than conventional MRA and fast 3D-MRA without DLR (sharpness: P = 0.021, P = 0.001; overall image quality: P = 0.029, P < 0.001). Conclusions The combination of 3D wheel sampling and DLR can improve visualization of arteries in intracranial TOF-MRA.

Abstract:We aimed to develop a new artificial intelligence software that can automatically extract and measure the volume of white matter hyperintensities (WMHs) in head magnetic resonance imaging (MRI) using only thick-slice fluid-attenuated inversion recovery (FLAIR) sequences from multiple centers. We enrolled 1092 participants in Japan, comprising the thick-slice Private Dataset. Based on 207 randomly selected participants, neuroradiologists annotated WMHs using predefined guidelines. The annotated images of participants were divided into training (n = 138) and test (n = 69) datasets. The WMH segmentation model comprised a U-Net ensemble and was trained using the Private Dataset. Two other models were trained for validation using either both thin- and thick-slice MRI datasets or the thin-slice dataset alone. The voxel-wise Dice similarity coefficient (DSC) was used as the evaluation metric. The model trained using only thick-slice MRI showed a DSC of 0.820 for the test dataset, which is comparable to the accuracy of human readers. The model trained with the additional thin-slice dataset showed only a slightly improved DSC of 0.822. This automatic WMH segmentation model comprising a U-Net ensemble trained on a thick-slice FLAIR MRI dataset is a promising new method. Despite some limitations, this model may be applicable in clinical practice.
