Abstract:Objective To investigate the correlation between intima-media thickness(IMT)and elasticity of carotid in patients with type 2 diabetes accompanied hyperlipidemia by quantitative intima-media thickness(QIMT)and quantitative arterial stiffness(QAS)technique.Methods Fifty-six patients with type 2 diabetes accompanied hyperlipidemia were involved into the study as case group.Forty-eight healthy volunteers were supplied as normal control group in the same period.IMT,compliance coefficient(CC),distensibility coefficient(DC)and stiffness index(β)of carotid were acquired through QIMT and QAS.Case group was divided into case group 1(IMT＜0.9mm)and case group 2(IMT≥0.9 mm),and analysis was performed furtherly.Results β,triglyceride(TG),lowdensity lipoprotein(LDL),cholesterol(Ch),glycosylated hemoglobin(GHb)and IMT had significant differences in case group 1,case group 2 and control group(P＜0.05),control group＜case group 1＜case group 2.DC and CC had significant differences in case group 1,case group 2 and control group(P＜0.05),control group＞case group 1＞case group 2.There was significant positive correlation between β and age,GHb,LDL and IMT of carotid respectively(r=0.811,r:0.764,r=0.732,r:0.729,respectively,P=0.000).Conchmions IMT of carotid could be measured accurately by QIMT.Structure imaging and functional imaging of carotid might be combined organically through QIMT and QAS.Depth study about early change of carotid in structure and function could be performed with these two techniques.

Abstract:Background: We sought to explore the potential relationship between serum levels of thyroid hormones with those of androgen and metabolic parameters in women with polycystic ovary syndrome (PCOS). Methods: Data from 1059 Chinese women with PCOS and 1015 healthy women was retrospectively collected. This data including fasting glucose and insulin, thyroid-stimulating hormone (TSH), free triiodothyronine (FT3), free thyroxine (FT4), total triiodothyronine (TT3), total thyroxine (TT4), anti-thyroperoxidase antibody (ANTI-TPO), anti-thyroglobulin (ATG), dehydroepiandrosterone sulfate (DHEAS), total testosterone (TTE), follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), prolactin (PRL), progesterone (PGN), triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL) and low-density lipoprotein cholesterol (LDL). Thyroid-related indicators were compared between PCOS and non-PCOS patients enrolled in this study. Independent variables of PCOS were compared among subgroups in accordance with the classification of TSH, homeostatic model assessment of insulin resistance (HOMA-IR), and TTE levels. To further explore the association between thyroid hormones levels and correlated metabolic parameters in PCOS, multiple regression analyses were conducted. Results: Our study found that PCOS patients had significantly higher serum TSH, FT3, TT3 and TT4 levels than non-PCOS patients. PCOS patients with TSH >= 2.5 mIU/L had significantly higher TG, fasting insulin, HOMA-IR and homeostatic model assessment of beta-cell function (HOMA-B), however, these patients also displayed significantly lower DHEAS, HDL, and quantitative insulin sensitivity check index (QUICKI) when compared to patients with TSH levels <2.5 mIU/L. PCOS patients with HOMA-IR levels >= 2.5 mIU/L demonstrated significantly higher FT3 and TSH, but lower TT3 when compared to women with HOMA-IR levels <2.5 mIU/L. Four groups divided by TTE displayed significant differences in FT3 in PCOS patients. Multiple linear regression analysis showed that TSH was significantly negatively associated with DHEAS and QUICKI. Conclusions: TSH levels are closely correlated to the metabolic and endocrine characteristics of PCOS, especially dyslipidemia and insulin resistance.

Inflammatory and endothelial markers in women with polycystic ovary syndrome

Abstract:Background: Women with polycystic ovary syndrome (PCOS) carry a pattern of cardiovascular risk factors. Endothelial dysfunction and chronic inflammation are early findings in the atherosclerotic process. The purpose of the study was to investigate the coexistence of active inflammation markers and endothelial dysfunction in young women with PCOS, and their relationship with metabolic and hormonal abnormalities of the syndrome. Materials and methods: Twenty-five young women with PCOS and 25 controls of similar age and body mass index (BMI) were studied. Endothelial function was assessed by flow-mediated dilatation (FMD) on the brachial artery and smooth muscle cells injury was excluded by nitrate-induced dilatation (NID). Plasma levels of endothelin-1 (ET-1), soluble intercellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1) and high sensitivity C-reactive protein (hsCRP) were measured. Hormonal and metabolic profiles were determined in both groups. Results: Flow-mediated dilatation (FMD) was statistically lower in PCOS (P < 0.001), whereas nitrate-induced dilatation (NID) was similar within the two groups. Polycystic ovary syndrome (PCOS) had statistically higher levels of ET-1 (P = 0.03), sICAM-1 (P = 0.01), sVCAM-1 (P = 0.02) and hsCRP (P = 0.01). Furthermore FMD was statistically higher in PCOS population with hsCRP 1 mg L(-1) when compared with PCOS population with hsCRP > 1 mg L(-1) (P = 0.02). Flow-mediated dilatation (FMD) was negatively related to hsCRP (r = -0.512, P = 0.015); ET-1 was positively related to free androgen index (r = 0.27, P = 0.05) and negatively to sex hormone-binding globulin (r = -0.465, P = 0.022); sVCAM-1 was positively related to total testosterone (r = 0.431, P = 0.036); hsCRP was positively related to BMI (r = 0.647, P = 0.001), and negatively related to FMD (r = -0.512, P = 0.015), quantitative insulin sensitivity check index (QUICKI) (r = -0.499, P = 0.018), and MATSUDA index (r = -0.445, P = 0.038). Conclusions: The present study demonstrates that endothelial dysfunction coexists and is influenced by the presence of increased serum levels of inflammation and endothelial activation markers in young women with PCOS. These parameters appear to be interrelated with hyperandrogenaemia in this insulin-resistant population.

Early Biomarkers of Subclinical Atherosclerosis in Obese Adolescent Girls with Polycystic Ovary Syndrome

Abstract:Objectives: Because in obese youth, pulse wave velocity (PWV), an early cardiovascular disease marker, is elevated, we tested if obese girls with polycystic ovary syndrome (OB-PCOS) have higher PWV and carotid intima-media thickness (cIMT) compared with obese girls without PCOS (OB-non-PCOS) and normal-weight girls without PCOS (NW-non-PCOS) and whether PWV and cIMT correlate with inflammatory and circulating endothelial function biomarkers. Study design: Cross-sectional study of PWV and cIMT in 91 OB-PCOS, 30 obese controls (OB-non-PCOS), and 19 normal-weight controls (NW-non-PCOS). Body composition, blood pressure, fasting glucose, insulin, lipid concentrations, and endothelial function biomarkers were measured. OB-non-PCOS and OB-PCOS underwent 2-hour oral glucose tolerance testing. Results: PWV was higher in OB-PCOS (664 ± 24 cm/s) and OB-non-PCOS (624 ± 37 cm/s) compared with NW-non-PCOS (468 ± 13 cm/s, P < .001), with no differences in cIMT. Systolic blood pressure, low-density lipoprotein, and non-high-density lipoprotein cholesterol were higher, and high-density lipoprotein cholesterol and indices of insulin sensitivity were lower in OB-PCOS and OB-non-PCOS compared with NW-non-PCOS. Vascular cell adhesion molecule-1 and high-sensitivity C-reactive protein were higher in OB-PCOS compared with NW-non-PCOS. PWV correlated with adiposity (rs = .46), insulin sensitivity index (homeostatic model assessment-insulin resistance rs = .31), systolic blood pressure (rs = .24; P ≤ .003 for all), and free testosterone (rs = .24; P = .03). In multiple regression analysis with PWV as the dependent variable and age, race, body mass index, PCOS, and dysglycemia as independent variables, only body mass index was an independent contributor to the model (r(2) = 0.068, P = .003). Conclusions: In adolescent girls, obesity and not PCOS appears to be associated with heightened cardiovascular disease risk. Increased PWV, vascular cell adhesion molecule-1, and high-sensitivity C-reactive protein may be the earliest subclinical atherosclerosis biomarkers in OB-PCOS.
Relationship between cardiometabolic index and early phase β-cell function in overweight or obese patients with polycystic ovarian syndrome

Abstract:Objective To explore the levels of cardiometabolic index (CMI) and its relationship with early phase β-cell function in overweight/obese polycystic ovary syndrome (PCOS) women. Method Three hundred and eighty-two overweight and obese women from Nanjing Drum Tower Hospital between April, 2016 and September, 2017 were recruited and divided into body mass index matched simple overweight/obese group (n=75) and overweight/obese PCOS group (n=307). Data of weight, height, blood pressure, heart rate and related laboratory examinations at their first visit were collected, indexes of early phaseβ-cell function (AUCIns30/AUCGlu30, DI30, Ins30/FINS) and CMI were calculated. Independent sample t test, analysis of variance and Pearson correlation coefficient were used for statistical analysis. Result Compared with simple overweight/obese women, waist-hip ratio, serum insulin levels during oral glucose tolerance test (OGTT), CMI (1.86 ± 0.69 vs 1.64 ± 0.91) and AUCIns30/AUCGlu30 (1.08 ± 0.24 vs 0.98 ± 0.32) were significantly higher respectively (t=-5.631--2.522, P<0.05), while HDL-C, 1/HOMA-IR (0.20±0.13 vs 0.28±0.16), DI30 (1.69±0.69 vs 1.88±0.72) and Ins30/FINS (5.76±3.29 vs 6.79±3.90) were significantly lower respectively (t=1.938-4.974, P<0.05) in overweight/obese PCOS women. Indexes relating early phase insulin secretion were significantly lower in overweight/obese PCOS women with higher CMI levels than those in overweight/obese PCOS women with lower CMI levels (P<0.05). In addition, CMI was negatively correlated with lgDI30 and Ins30/FINS after adjusting age, BMI, blood pressure and heart rate (r=-0.167,-0.162, P=0.004, 0.006). Conclusion Compared with simple overweight/obese women, overweight/obese PCOS women have higher cardiovascular risk and impaired early phase insulin secretion function. Early phase β-cell function indexes were negatively correlated with cardiovascular metabolic risk in overweight/obese PCOS women.

Abstract:Objective To investigate how body fat influences glucose metabolism and hormone profiles in women with polycystic ovary syndrome (PCOS), compared to women without PCOS. Methods We conducted a cross-sectional study of 166 women with PCOS and 139 age-matched control women at Peking University Third Hospital (Beijing, China) from March 2016 to December 2021. All participants underwent bioimpedance rate assessment of clinical, anthropometric, hormonal, and metabolic features. In particular, body composition parameters were assessed, based on the methods used in a previous study. Homeostasis model assessment-insulin resistance (HOMA-IR) and other indices calculated from fasting glucose and insulin were used to measure insulin resistance. The hormonal profiles [follicle-stimulating hormone (FSH), luteinizing hormone (LH), estrogen (E2), prolactin (PRL), total testosterone (T), and androstenedione (A2)] were assessed by using biochemical methods. Two subgroup analyses were conducted according to waist-to-hip ratio (WHR; < 0.85, non-central obesity and ≥ 0.85, central obesity) and body fat percentage (BFP; < 35% for lean and ≥35% for obesity). The indices above were analyzed using a two-sided t-test or Wilcoxon rank sum test. Linear regression was used to investigate the effects of body composition on metabolism and sex hormones in the PCOS and control groups. Results Compared to women without PCOS, women with PCOS and central obesity ( P =0.021), PCOS and noncentral obesity ( P <0.001), PCOS and high BFP ( P <0.001), and PCOS and low BFP ( P <0.001) had more severe glucose metabolism evaluated with HOMA-IR. Women with PCOS experienced greater insulin sensitivity impairment than did the normal population for every equal increase in BFP. LH, LH/FSH, total testosterone, and androstenedione were significantly higher in patients with PCOS than in healthy controls, regardless of WHR and BFP stratification. However, negative correlations existed between body fat indices (i.e., BFP and body mass index) and hormone indices (i.e., LH and androstenedione) in the PCOS group, but were absent in the control group. Conclusions Obese and non-obese women with PCOS have more severe insulin resistance and sex-hormone disorders than women without PCOS. The effect of body fat on sex-hormone disorders is only exist in women with PCOS. These findings suggested that PCOS clinical guidelines should be more specific to body fat. Abstract:AIMS: There are emerging data indicating an association between PCOS (polycystic ovary syndrome) and metabolic derangements with potential impact on its clinical presentation. This study aims to evaluate the pathophysiological processes beyond PCOS with particular focus on carbohydrate metabolism, ectopic lipids and their possible interaction. Differences between the two established classifications of the disease should be additionally evaluated.METHODS: A metabolic characterization was performed in 53 untreated PCOS patients as well as 20 controls including an extended oral glucose tolerance test (OGTT, to assess insulin sensitivity, secretion and ß-cell function) in addition to a detailed examination of ectopic lipid content in muscle and liver by nuclear magnetic resonance spectroscopy.RESULTS: Women with PCOS classified by the original NIH 1990 definition showed a more adverse metabolic risk profile compared to women characterized by the additional Rotterdam 2003 phenotypes. Subtle metabolic derangements were observed in both subgroups, including altered shapes of OGTT curves, impaired insulin action and hyperinsulinemia due to increased secretion and attenuated hepatic extraction. No differences were observed for ectopic lipids between the groups. However, particularly hepatocellular lipid content was significantly related to clinical parameters of PCOS like whole body insulin sensitivity, dyslipidemia and free androgen index.CONCLUSIONS: Subtle alterations in carbohydrate metabolism are present in both PCOS classifications, but more profound in subjects meeting the NIH 1990 criteria. Females with PCOS and controls did not differ in ectopic lipids, however, liver fat was tightly related to hyperandrogenism and an adverse metabolic risk profile.

The Effects of Omega-3 and Vitamin E Co-supplementation on Carotid Intima-media Thickness and Inflammatory Factors in Patients with Polycystic Ovary Syndrome

Abstract:Objectives: We sought to evaluate the effects of omega-3 and vitamin E co-supplementation on carotid intima-media thickness (CIMT) and inflammatory factors in patients with polycystic ovary syndrome (PCOS). Methods: This randomized, double-blind, placebo-controlled trial was done among 60 women with PCOS. Participants were randomly assigned into two groups (n = 30 each group) and assigned to take either 1000 mg omega-3 plus 400 IU vitamin E supplements or a placebo for 12 weeks. Results: Compared with placebo, omega-3 and vitamin E co-supplementation led to significant decreases in maximum levels of left CIMT (-0.006±0.006 vs. +0.002±0.007 mm, p < 0.001), mean levels of left CIMT (-0.005±0.006 vs. +0.002±0.010 mm, p = 0.010), maximum levels of right CIMT (-0.006±0.010 vs. +0.006±0.010 mm, p = 0.010), and mean levels of right CIMT (-0.005±0.005 vs. +0.001±0.010 mm, p = 0.020). Change in high-sensitivity C-reactive protein (hs-CRP) (-390.6±942.9 vs. +237.0±754.3 ng/mL, p = 0.006) was significantly different between the supplemented patients and placebo group. We did not observe any significant effect in plasma nitric oxide (NO) values following supplementation with omega-3 plus vitamin E compared with the placebo. Conclusions: Co-supplementation with omega-3 and vitamin E for 12 weeks among patients with PCOS had beneficial effects on CIMT and serum hs-CRP values, but unchanged NO values.

Abstract:Aim: This study was designed to examine the relationship between homocysteine (Hcy), lipoprotein levels and insulin resistance in obese and non-obese patients with polycystic ovary syndrome (PCOS). Materials and methods: Eighty-five patients (38 obese, 47 non-obese) with PCOS and 50 healthy subjects (25 obese, 25 non-obese) were included in the study. PCOS was defined according to the Homburg criterion. Serum levels of follicle-stimulating hormone (FSH), luteinizing hormone (LH), dehydroepiandrosterone sulfate (DHEA-S), insulin, 17-hydroxyprogesterone, free testosterone, androstenedione, vitamin B12 and folate were measured. Also, serum concentrations of total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), triglycerides (TG), lipoprotein (a) (Lp(a)), apoprotein B (Apo B) and apoprotein A (Apo A) were determined. Plasma Hcy levels were measured. Insulin resistance was evaluated by homeostasis model assessment (HOMA). Results: Plasma Hcy levels were significantly higher in women with PCOS than in healthy women. HOMA-R (insulin resistance) was significantly higher in women with PCOS compared with healthy women. Serum fasting TC, LDL-C, TG, Apo B, vitamin B12 and folate levels were similar between PCOS and control groups. Lp(a) levels were higher in PCOS patients than in control subjects, whereas HDL-C and Apo A levels were lower. Compared with obese PCOS subjects, non-obese PCOS subjects had low HOMA-R, TC, LDL-C, TG, Apo B, Lp(a) and androgen levels. Plasma Hcy levels, serum HDL-C and Apo A levels were similar between obese and non-obese women with PCOS. Levels of HDL-C and Apo A were lower in both obese and non-obese PCOS patients than in obese and non-obese control subjects, whereas Lp(a) levels were higher. No correlation was observed between plasma Hcy, body mass index, HOMA-R, serum androgen levels, TC, LDL-C, HDL-C, Apo A, Apo B and Lp(a) levels. Conclusion: These results showed that elevated insulin resistance and plasma Hcy levels, and changes in serum lipid profile, which are possible risk factors for cardiovascular disorders, play important roles in the development of cardiovascular disease in both obese and non-obese patients with PCOS.

Abstract:Objective To measure serum C-reactive protein (CRP) levels and analyze its associations with clinical factors in patients with polycystic ovary syndrome (PCOS).Methods Ninety PCOS women and 20 ovulating healthy women with body mass index (BMI) 25 kg/m2 as controls were recruited. PCOS women were divided to two groups: 48 BMI≥25 kg/m2 patients as obese group; 42 BMI25 kg/m2 as non-obese group. Serum CRP was assayed by immunoturbidimetry. And triglycerides (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C)and low density lipoprotein cholesterol (LDL-C) were measured. Oral glucose tolerance test (OGTT) and insulin sensitivity were also assessed in PCOS patients.Results ① Obese and non-obese PCOS group presented markedly higher logCRP than controls group (both P0.05), and the logCRP is higher in the obese PCOS group than non-obese PCOS group (P0.05). ② Univariate analysis revealed that logCRP was significantly and positively correlation with BMI and HOMA-IR(P0.05); LogCRP was significantly and negatively correlation with HDL-C and ISI(P0.05)in patients with PCOS. Partial correlation showed that logCRP was correlation with HOMA-IR(P0.05), and logCRP was significantly and negatively correlation with ISI(P0.05)in patients with PCOS. Conclusions Chronic inflammation maybe exists in patients with PCOS. CRP may be a predictor of future type 2 diabetes mellitus in PCOS patients.
Association of vascular and inflammatory markers with metabolic disorders in women with polycystic ovary syndrome

Abstract:Background/Aim. The prevalence of metabolic disorders, obesity and insulin resistance in women with polycystic ovary syndrome (PCOS) occur early in life and places this group at risk of cardiovascular disease. Hyperhomocysteinemia and increased C-reactive protein (CRP) activity have an effect on promoting atherosclerosis. This study was designed to evaluate whether high sensitivity (hs-CRP) and homocysteine (Hcy) are elevated in PCOS and to elucidate their possible relation to obesity, insulin resistance, or metabolic changes usually present in women suffering from PCOS. Methods. Serum concentration of hs-CRP and plasma levels of Hcy were evaluated in 73 PCOS women and 43 healthy women, together with clinical, anthropometric and hormonal parameters. Results. The mean of body mass index (BMI), waist circumference (WC), waist to hip ratio and mean concentration of luteinizing hormone (LH), testosterone, androstenedione, free androgen index, fasting insulin, homeostatic model assessment of insulin resistance (HOMA- IR), hs-CRP and Hcy were significantly higher in PCOS women compared to age-matched healthy women. There was a positive correlation between hs-CRP and BMI, WC, insulin, triglycerides (p < 0.001) and significant negative correlation with LH, sex hormone binding protein (SHGB), HOMA-IR, high density lipoprotein cholesterol (HDL-C) (p < 0.001). The Hcy concentration had a significant negative correlation with HDL-C level (p < 0.05). The present study demonstrated increased mean concentration of Hcy in hs-CRP women with PCOS. Conclusion. Our results support the use of these biomarkers in the evaluation of potential risk for cardiovascular diseases and early prognosis and treatment implications.

Abstract:Recently, a growing body of evidence has suggested that abnormal ovarian angiogenesis, secondary to the imbalance between various angiogenic markers, is involved in the pathogenesis of PCOS, and this has led to the use of various interventions (such as Diane-35) to restore the normal ovarian angiogenesis. Therefore, we conducted the current investigation to determine the role of such markers (endothelial growth factor (VEGF), endostatin (ES), and thrombospondin-1 (TSP-1)) in the pathogenesis of PCOS along with the associated changes in ovarian blood flow in patients with PCOS compared to healthy controls, both before and after a course of oral contraception. A total of 381 patients with PCOS and 98 healthy females of childbearing age were recruited from July 2014 to June 2017 at the Reproductive Center of the Second Affiliated Hospital of Harbin Medical University. The serum levels of VEGF, ES, and TSP-1 were determined by enzyme-linked immunosorbent assay, while ovarian perfusion was measured by the pulsatility index (PI) and resistance index (RI) by using transvaginal color Doppler ultrasound. Repeated analyses were carried out after 3 months of Diane-35 treatment. Post-treatment serum levels of luteinizing hormone (LH)/follicle stimulating hormone (FSH) ratio of patients with PCOS decreased significantly (P <0.05). The RI values of most PCOS patients increased after treatment (P<0.05), while PI was significantly increased in all patients (P<0.05). However, variable changes in the serum levels of TSP-1, VEGF, and ES after treatment were observed. Serum VEGF levels showed a negative correlation with serum LH/FSH ratio, T concentration, and ES (P <0.05), while ES levels were negatively correlated with serum T concentrations only (P<0.05). The markers of angiogenesis (VEGF, ES, and TSP-1) were expressed differently among PCOS patients, who also responded differently to the same course of Diane-35 treatment. This field still warrants further investigation to reach a more definitive conclusion.

Abstract:Objective To analyze the endocrine and metabolic characteristics of polycystic ovary syndrome(PCOS) and compare the clinical phenotype and endocrine metabolic parameters between PCOS patients with hyperandrogemia (PCOS/HA) and without hyperandrogemia (PCOS/NHA). Methods Two hundred and forty-one patients with PCOS who had not been treated and fifty healthy controls were enrolled. Height,weight,fat content (fat %),blood pressure,waist circumferences and hip circumferences were measured and recorded. Fasting blood samples were collected for measuring serum lipids,blood glucose,serum insulin and sex hormones. Homeostasis model of insulin resistance (HOMA-IR) and pancreatic β cell function (HOMA-β) were also assessed. Results Body mass index (BMI),fat content,blood glucose,insulin,lipid in PCOS patients were significantly higher than that in controls. Luteinizing hormone (LH),total testosterone (T) dehydroepiandrosterone sulfate (DHEA-S) and disturbance of sex hormone in PCOS/HA patients were significantly higher than that in PCOS/NHA patients,whereas follicular-stimulating hormone (FSH) was significantly lower (P<0.05). Normal body weight PCOS/NHA patients had higher fasting insulin [7.2(6.0-21.1) vs 6.3(3.9-9.6) mU/L,P<0.05],HOMA-IR [1.5(1.2-5.1) vs 1.4(0.8-2.0),P<0.05] and HOMA-β [130.0(102.8-304.0) vs 97.0(63.7-168.0),P<0.05] than normal body weight PCOS/ HA patients. Conclusions PCOS patients have significantly higher body mass index and blood glucose,serum lipids profile,insulin resistance and disturbance of sex hormone than the control group. Normal body weight PCOS/NHA patients have more severe insulin resistance and impaired islet β cell function than normal body weight PCOS/HA patients.

Abstract:OBJECTIVE: Polycystic ovary syndrome (PCOS) is a common endocrine condition underpinned by insulin resistance and associated with increased risk of obesity, type 2 diabetes and adverse cardiovascular risk profile. Previous data suggest autonomic imbalance [elevated sympathetic nervous system (SNS) activity and decreased heart rate variability (HRV)] as well as endothelial dysfunction in PCOS. However, it is not clear whether these abnormalities are driven by obesity and metabolic disturbance or whether they are independently related to PCOS.PARTICIPANTS AND METHODS: We examined multiunit and single-unit muscle SNS activity (by microneurography), HRV (time and frequency domain analysis) and endothelial function [ischaemic reactive hyperaemia index (RHI) using the EndoPAT device] in 19 overweight/obese women with PCOS (BMI: 31·3 ± 1·5 kg/m(2), age: 31·3 ± 1·6 years) and compared them with 21 control overweight/obese women (BMI: 33·0 ± 1·4 kg/m(2), age: 28·2 ± 1·6 years) presenting a similar metabolic profile (fasting total, HDL and LDL cholesterol, glucose, triglycerides, insulin sensitivity and blood pressure).RESULTS: Women with PCOS had elevated multiunit muscle SNS activity (41 ± 2 vs 33 ± 3 bursts per 100 heartbeats, P < 0·05). Single-unit analysis showed that vasoconstrictor neurons were characterized by elevated firing rate and probability and incidence of multiple spikes (P < 0·01 for all parameters). Women with PCOS also had impaired endothelial function (RHI: 1·77 ± 0·14 vs 2·18 ± 0·14, P < 0·05). HRV did not differ between the groups.CONCLUSION: Women with PCOS have increased sympathetic drive and impaired endothelial function independent of obesity and metabolic disturbances. Sympathetic activation and endothelial dysfunction may confer greater cardiovascular risk in women with PCOS.

Abstract:Objective: To investigate both independent and combined effects of insulin resistance and β-cell dysfunction on cardio-metabolic abnormalities in polycystic ovary syndrome (PCOS). Design: A national epidemiologic survey was performed in reproductive aged females in China from October 2007 to September 2011. Methods: A total of 824 PCOS and 2715 non-PCOS were included. The Rotterdam Criteria were applied for PCOS diagnosis. We used the homeostasis model assessment of insulin resistance (HOMA-IR) and HOMA of β-cell function (HOMA-β) to evaluate insulin resistance and β-cell dysfunction, respectively. Results: Compared with non-PCOS, PCOS showed a higher index of HOMA-IR and HOMA-β, and a higher prevalence of obesity, central obesity, and dyslipidaemia. High HOMA-IR was independently related to a high prevalence of obesity, central obesity, dyslipidaemia, and high blood glucose in PCOS. In contrast, a low index of HOMA-β index was independently correlated with a low prevalence of obesity, and central obesity, but negatively correlated with an elevated prevalence of high blood glucose in PCOS. In addition, proportion of insulin resistance was higher than that of β-cell dysfunction in PCOS with cardio-metabolic disorders. β-cell dysfunction was negatively correlated with the prevalence of central obesity and obesity. Conclusions: Insulin resistance and β-cell dysfunction independently affected cardio-metabolic abnormalities in PCOS, while insulin resistance was correlated with a higher prevalence of cardio-metabolic abnormalities than that of β-cell dysfunction. Moreover, β-cell dysfunction and insulin resistance showed divergent correlations with obesity in PCOS.
Cardiovascular Risk Predictors High Sensitivity C-Reactive Protein and Plasminogen Activator Inhibitor-1 in Women with Lean Phenotype of Polycystic Ovarian Syndrome: A Prospective Case-Control Study

Abstract:Objective Accumulating evidence suggests increased cardiovascular risk in women with polycystic ovarian syndrome (PCOS) due to a cluster of factors, such as obesity, lipid abnormalities, impaired glucose tolerance (IGT), and hypertension. Markers such as high-sensitivity C-reactive protein (hs-CRP) and plasminogen activator inhibitor-1 (PAI-1) can provide an adjunctive method for the assessment of cardiovascular risk and can indicate future coronary heart diseases in women with lean PCOS. Materials and Methods In this prospective case-control study, women clinically diagnosed with PCOS ( n = 25) with normal body mass index (BMI) and age and BMI-matched healthy controls ( n = 75) were enrolled. The quantitative data were expressed as mean ± standard deviation (SD). Unpaired Student's t -test was used to compare the values (PCOS vs. controls) and Pearson's correlation coefficient was used to elucidate the relationship between the variables. Results The mean level of fasting blood sugar, serum total cholesterol, low-density lipoprotein (LDL), thyroid-stimulating hormone (TSH), follicle-stimulating hormone (FSH), luteinizing hormone (LH), prolactin (PRL), hs-CRP, and PAI-1 were significantly increased in PCOS patients ( p < 0.000) compared with the control patients. Of the reported cases, 54.16% had hs-CRP levels greater than 3 mg/L. When the cases were further divided into normal ( n = 20) and IGT ( n = 5), hs-CRP values were significantly higher in IGT group as compared with normal glucose tolerance (NGT) group. On bivariate correlation analysis, hs-CRP had significant correlations with PAI-1 ( r = 0.41, p < 0.000), waist-to-hip ratio (WHR; r = 0.23, p = 0.02), fasting blood sugar (FBS; r = 0.26, p = 0.009), LDL ( r = 0.20, p = 0.03), TSH ( r = 0.42, p < 0.000), and LH-to-FSH ratio ( r = 0.24, p = 0.01). Conclusion Women with lean phenotype of PCOS suffer from many metabolic abnormalities such as abdominal obesity, dyslipidemia, hyperandrogenemia, and insulin resistance. The findings of the study suggest that environment of ongoing low-grade inflammation due to infiltration further exacerbates the metabolic derangements and cardiovascular risk. The investigations as hs-CRP and PAI-1 will help in early identification, diagnosis, and management of cardiovascular diseases associated with lean type of PCOS. These markers can prove to be beneficial in monitoring any unfavorable changes in cardiometabolic profile of such patients.

Abstract:Objective To evaluate the differences of the clinical manifestation and endocrine situation in patients with different ovarian morphology of polycystic ovary syndrome(PCOS).Methods A total of 234 PCOS patients were enrolled according to the ovary morphology and divided into three groups: 112 patients with B-polycystic ovary morphology(both two ovaries were PCOM, B-PCOM), 50 with U-PCOM(only one ovary was PCOM) and 72 with N-PCOM(none was PCOM). There were 39 infertile women without PCOS as control group. Data were analyzed by using SPSS 15.0 software.Results There was no statistical difference in body mass index(BMI) among the three groups of PCOS. The endometrial thickness increased in patients with B-PCOM and decreased with N-PCOM. The levels of testosterone, androstenedione and luteinizing hormone increased in PCOS groups, especially in N-PCOM patients. HOMA-IR increased, HOMA-β, disposition index(DI) and △I60/△G60 decreased in patients with NPCOM compared with in B-PCOM and U-PCOM groups. Higher level of total cholesterol(TC) and lower level of high-density lipoprotein(HDL)-C existed in PCOS patients,especially in N-PCOM. There were positive correlations between oligo-anovulation,endometrial thickness, LH/FSH ratio, fasting insulin(FINS), the area under curve of glucose(AUCGLU) and PCOM, while there was a negative correlation between HOMAIR and PCOM.Conclusion There are relationships among hyperandrogenism, hyperinsulinemia,insulin resistance(IR) and ovary morphology in PCOS patients. PCOS patients without PCOM have more serious IR and hyperandrogenism.
