Abstract:Background: Total hip arthroplasty in patients with rheumatoid arthritis is difficult and leads to worse implant survival and higher risk for complications compared to osteoarthritis. Poor periacetabular bone quality might challenge the stability of the acetabular cup. Contemporary 3D-printed highly-porous titanium cup designs have become the benchmark for cementless implants; however, their usefulness in rheumatoid arthritis is still unclear. The purpose of this study was to assess the short-term results of the use of a highly-porous 3D-printed acetabular titanium cup in patients with rheumatoid arthritis. Material and methods: We studied a consecutive series of RA patients who underwent cementless THA between 2019 and 2020 in our center with the same 3D-printed highly-porous titanium acetabular cup. All patients were evaluated prospectively for a minimum 3-year follow-up with the Harris hip score, VAS and Roles and Maudsley score. Radiographic assessment focused on evidence of cup osseointegration. Results: 37 RA patients, mean age at surgery of 51.3 years, were included. Mean follow-up was 3.2 years (3 to 4 years). By the last follow-up visit, the Harris hip score had increased significantly from 37.8 to 88.5, VAS had decreased from 6.1 to 1.0 and excellent satisfaction for 31 patients. All 5 Moore's radiographic signs were present in 35 hips, with only 4 signs present in the remaining 2 hips, thus indicating complete osseointegration of all cups. There were no complications on the acetabular side but one intraoperative proximal calcar fracture. Conclusion: 3D-printed highly porous implants used in primary THA worked well in patients with rheumatoid arthritis, showing excellent osseointegration and good clinical results in the short term.

Abstract:Purpose We prospectively evaluate the short-term clinical and radiographic outcomes of the only Chinese domestically produced trabecular titanium acetabular cup(3D ACTTM cup) in primary total hip arthroplasty (THA), aiming to provide evidence-based support for its clinical application. Methods A total of 236 patients, who underwent primary THA using 3D ACTTM cup in the Department of Joint Surgery at our hospital between January 2017 and June 2019, were included in this study. General patient data, imaging information, functional scores, and complications were collected to evaluate the early clinical efficacy. Results All patients were followed up for 33–52 months, with an average of (42.2 ± 9.2) months. At the last follow-up, the preoperative HHS score increased significantly from 43.7 ± 6.8 to 85.6 ± 9.3 points ( P < 0.01). Similarly, the preoperative WOMAC scores showed significant improvement from 59.2 ± 5.8 to 13.1 ± 3.5 points ( P < 0.01). 92.3% of the patients expressed satisfaction or high satisfaction with the clinical outcome. Furthermore, 87.7% of the acetabular cups were positioned within the Lewinnek safe zone, achieving successful reconstruction of the acetabular rotation center. The cup survival rate at the last follow-up was 100%. Conclusions The utilization of the only Chinese domestically manufactured 3D printing trabecular titanium acetabular cup in primary THA demonstrated favorable short-term clinical and radiographic outcomes. The acetabular cup exhibits excellent initial stability, high survival rate, and favorable osseointegration, leading to a significant enhancement in pain relief and functional improvement. In the future, larger sample sizes and multicenter prospective randomized controlled trials will be required to validate the long-term safety and effectiveness of this 3D ACTTM cup.

Abstract:Background: The global population, especially in the Western world, is constantly aging and the need for total hip arthroplasties has rocketed, hence there has been a notable increase in revision total hip arthroplasty cases. As time has passed, a considerable developments in science and medicine have been attained which have also resulted in the evolution of both surgical techniques and implants. Continuous improvements have allowed large bore bearings to be utilized which provide an increased range of motion, with ameliorated stability and a very low rate of wear. The trend for almost the last two decades has been the employment of porous tantalum acetabular cups. Several studies exist comparing them with other conventional methods for total hip arthroplasties, exhibiting promising short and midterm results. Methods: The Preferred Reporting Items for Systematic Reviews and a Meta-Analysis (PRISMA) were used to identify published studies in a comprehensive search up to February 2023, and these studies were reviewed by the authors of the article. Specific rigorous pre-determined inclusion and exclusion criteria were implemented. Results: Fifty-one studies met our inclusion criteria and were involved in the systematic review. Sixteen studies examined postoperative clinical and radiological outcomes of using a tantalum cup in primary and revision total hip arthroplasty, whilst four biomechanical studies proved the superiority of tantalum acetabular components. Five articles provided a thorough comparison between tantalum and titanium acetabular cups, while the other studies analyzed long-terms results and complication rates. Conclusions: Porous tantalum acetabular cups appear to be a valuable option in revision total hip arthroplasty, providing clinical improvement, radiological stability, and promising long-term outcomes. However, ongoing research, longer follow-up periods, and careful consideration of patient factors are essential to further validate and refine the use of tantalum in various clinical scenarios.

Results of third generation dual mobility total hip arthroplasties with non-cross-linked polyethylene in patients under 60 years of age: comparative study of full pressfit vs. tripod cups after 8.6 years

Abstract:Background: The concept of dual mobility of total hip arthroplasties (THA) is a revolution in the prevention of dislocations and in the treatment of instability. Its use remains controversial in patients under 60 years old, providing poorer results with first generation cups coated with alumina. This study, carried out on modern dual mobility (DM) cups with a porous bilayer coating, in active patients under 60 years of age aimed to evaluate: (1) the mechanical survival of two latest generation DM THA, with failure defined as revision of acetabular or femoral implants for mechanical loosening, by comparing a tripod cup to a full pressfit cup, (2) to evaluate the complications, (3) to compare the rate of radiographic peri-prosthetic osteolysis between the 2 implants. Hypothesis: The hypothesis was that the medium-term survival of modern DM cups placed in young patients was the same regardless of the type of acetabular fixation used. Materials and methods: This was a retrospective study of 119 latest generation DM THA with standard polyethylene, composed of 66 full pressfit cups and 53 tripod fixation cups, used in primary arthroplasty in 111 patients between 2005 and 2016. The survival study was carried out using medical records. The clinical evaluation was carried out using the modified Harris Hip Score (mHHS) as well as the self-administered Hip and Osteoarthritis Outcome Score (HOOS) and Oxford-12 questionnaires. Radiological analysis was carried out on radiographs looking for bone demineralization and acetabular and femoral lines. Results: At 8.6 years (5-16), no revision for mechanical loosening was observed regardless of the type of cup. The prosthesis dislocation rate (large joint) was 1.7%: 1 case in each group (p = 1), including one revision required in the full pressfit group. Three revisions for mechanical complications were recorded: a recurrent dislocating hip on a full pressfit cup that was not sufficiently anteverted, an acetabular peri-prosthetic fracture on a full pressfit cup and a case of iliopsoas impingement with a retroverted tripod cup. The survival rate for all-cause revision was 93.8% for pressfit (95% CI: 88.2%-99.9%) vs. 96.2% for tripod (95% CI: 91.1%-100%) (p = 0.63) and for revision due to any cause excluding infection, 96.9% for pressfit (95% CI: 92.8%-100%) vs. 98.1% for tripod (95% CI: 94.4%-100%) (p = 0.7). The rate of periacetabular osteolysis was 11%, significantly more present amongst tripod cups (24%, 9 cases) compared to full pressfit cups (2%, 1 case) (p < 0.01). Conclusion: This series demonstrates good performance of 3rd generation DM cups with porous bilayer coating in patients under 60 years of age regardless of the type of acetabular fixation, at 8.6 years of follow-up. The rate of osteolysis around full pressfit cups remains very low, unlike that of tripod cups. The use of Double Mobility prostheses for primary hip osteoarthritis is possible in young patients, preferably with full pressfit cups (i.e., without any pegs or additional screws that may promote diffusion of polyethylene debris and osteolysis). Serum gastrin concentrations in colorectal cancer patients.

Abstract:Fasting serum gastrin concentrations were shown to be elevated in colorectal cancer patients compared with controls (P = 0.0037), which was mainly accounted for by a subgroup of patients who had significantly elevated levels. In cancer patients there was no difference in gastrin concentrations in blood taken from a tumour-draining mesenteric vein and from a peripheral vein at the time of colonic resection. Serum gastrin concentrations were significantly lower after apparently curative resection for colorectal cancer (P = 0.028), suggesting that the elevated serum gastrin seen in these patients may be due, at least in part, to secretion of gastrin by the tumour.

Abstract:Objective: To explore the intermediate outcomes of third-generation alumina-on-alumina total hip arthroplasty (THA). Methods: A consecutive series of 165 primary alumina-on-alumina total hip arthroplasties were performed in 135 patients. Three patients died and 13 patients lost follow-up so that a total of 119 patients (146 hips) were available for study. The mean patient age was 53.4 ± 11.0 years old (range: 26-79). Hydroxyapatite. (HA)-coated press-fit acetabular cups were used in all cases. At femoral side, 123 cases were implanted with HA-coated uncemented stems and 23 cases implanted with high polished double taped cemented stems. All patients were evaluated clinically and radiographically after a minimal follow-up duration of 60 months. Results: The preoperative mean Harris hip score of 49.6 ± 7.9 points improved to 91.7 ± 3.0 points at the last follow-up (P < 0.05). The 5-year survival for any reason lead to revision was 96.6%. Five hips were under revision, 1 for acetabular shell loosening and migration, 1 for Vancouver B2 periprosthetic fracture, 1 for fracture of pure alumina liner and 2 for infections. All other prostheses demonstrated no radiographic evidence of loosening. No periprosthetic osteolysis was found. There were 2 cases of dislocations and 2 patients presented postoperatively with "squeaking-like" hips. Conclusion: The clinical and radiographic outcomes after primary THA with third-generation ceramic-on-ceramic bearing surfaces are favorable after a minimal follow-up duration of 5 years. Mid-term Efficacy of Standardized 3D Printed Porous Trabecular Titanium Acetabular Cup in Total Hip Arthroplasty

Abstract:Objective To evaluate the mid-term clinical efficacy of total hip replacement using a standardized 3D printed porous trabecular titanium acetabular cup. Methods Clinical data of 23 consecutive patients undergoing total hip replacement with a standardized 3D printed porous trabecular titanium acetabular cup (25 arthroplasties) between February 2016 and August 2018 were collected and retrospectively analyzed in this study. All patients were followed up for 39~69 (51.7±10.7) months. Efficacy evaluation indicators included incidence of postoperative complications and survival rate of prosthesis, as well as visual analog scale (VAS) score, Harris score, acetabular side imaging evaluation and cup survival rate at 6 months after operation and last follow-up. Results No complications were observed in all 25 hip replacements, and the survival rate of cup was 100% during the whole follow-up. The VAS scores were decreased significantly at 6 months postoperatively (1.44±0.82) and last follow-up (0.08±0.27) when compared with the scores preoperatively (7.48±0.96, P < 0.05). The Harris scores were increased significantly at 6 months postoperatively (86.88±3.52) and last follow-up (95.04±1.86) than the pre-operative scores (45.64±4.55, P < 0.05). There were no significant differences in postoperative VAS score and Harris score between the patients with and without periprosthetic radiolucent zone. Conclusion Standardized 3D printed porous trabecular titanium acetabular cup shows its favorable mid-term outcome in total hip arthroplasty, provides high acetabular survival rate, and has certain advantages in reducing aseptic loosening of the prosthesis.

Pure Alumina Bearings with Cementless Stems Versus Sandwich Bearings with Cemented Stems in Total Hip Arthroplasty.

Abstract:Background Ceramic bearing surfaces have been introduced to prevent osteolysis after total hip arthroplasty (THA), but little is known about the difference in clinical and radiological results between pure alumina and sandwich alumina bearings. The purpose of this study was to analyze the results obtained with third-generation alumina-on-alumina THA with two different designs both in liner and femoral stem fixation after a minimum follow-up of 4.2 years.Methods The results of 195 primary alumina-on-alumina THAs in 167 patients were evaluated. The procedures were performed between January 1998 and October 2006. Three patients died and 11 patients were lost to follow-up, leaving a total of 153 patients (181 hips) available for study. In the 88 group A patients, 107 hips were implanted using pure alumina bearings with cementless femoral stems. These patients were followed for (6.84 +/- 1.49) years. In the 65 group B patients, 74 hips were implanted using sandwich alumina ceramic bearings with cemented femoral stems. These patients had a follow-up period of (7.73 +/- 1.60) years. Patients in both groups were evaluated clinically and radiographically.Results One ceramic liner fracture occurred in group A and five took place in group B (P <0.05), four of them revised for liner fracture. In each group, one acetabular shell migration happened without liner breakage and two hips developed deep infections, and all these six hips received revisions. Nine femoral components loosened in group B, with seven undergoing revisions. Kaplan-Meier survivorship at 5 years for revision of any component for any reason in group A was 96.26% compared to 90.54% in group B (P <0.05). Better function was determined in group A (average Harris hip scores: 92.13 +/- 2.85) than in group B (average Harris hip scores: 86.03 +/- 4.21) and the difference was significant (P <0.05). Squeaking was not recorded in either group.Conclusions The sandwich design of the acetabular bearings can not reduce the migration rate in ceramic bearings but increase the liner fracture rate compared to pure ceramic liners. The high loosening rate in fluted and taped designed cemented stems with sandwich liners warrant caution to their use. Chin Med J 2012;125(2):244-248

Abstract:(1) Background: The aim of this study was to evaluate short- to mid-term clinical and radiological results in patients undergoing primary total hip arthroplasty (THA) with the use of a Selective Laser Melting 3D-printed highly porous titanium acetabular cup (Jump System Traser®, Permedica Orthopaedics). (2) Methods: We conducted a retrospective study and collected prospective data on 125 consecutive patients who underwent primary THA with the use of highly porous titanium cup. Each patient was evaluated preoperatively and postoperatively with a clinical and radiological assessment. (3) Results: The final cohort consisted of 104 patients evaluated after a median follow-up of 52 (38–74) months. The median Harris Hip Score (HHS) significantly improved from 63.7 (16–95.8) preoperatively to 94.8 (38.2–95.8) postoperatively (p < 0.001), with higher improvement associated with higher age at surgery (β = 0.22, p = 0.025). On postoperative radiographs, the average acetabular cup inclination and anteversion were 46° (30°–57°) and 15° (1°–32°), respectively. All cups radiographically showed signs of osseointegration with no radiolucency observed, or component loosening. (4) Conclusions: The use of this highly porous acetabular cup in primary THA achieved excellent clinical, functional, and radiological results at mid-term follow-up. A better clinical recovery can be expected in older patients. The radiological evaluation showed excellent osseointegration of the cup with complete absence of periprosthetic radiolucent lines.

Minimum 7‐year Follow‐up of A Porous Coated Trabecular Titanium Cup Manufactured with Electron Beam Melting Technique in Primary Total Hip Arthroplasty

Abstract:ObjectivesTo investigate the cup survivorship, patient satisfaction level, clinical function, and radiographic outcomes of patients who underwent total hip arthroplasty (THA) using electron beam melting (EBM)‐produced porous coated titanium cups at mid‐term follow up.MethodsA total of 32 patients (32 hips) from five hospitals in China who underwent primary THA using EBM‐produced trabecular titanium cups between May and December 2012 were retrospectively reviewed. The inclusion criteria were: (i) patients who underwent THA with the use of EBM‐produced cups with possible 7‐year follow up; and (ii) patients with follow‐up information, including the cup survivorship, patient satisfaction level, and clinical outcomes such as Harris hip score. The exclusion criteria were: (i) patients with neuropathic diseases; and (ii) patients who underwent THA due to neoplastic disease. Five (15.6%) patients were lost to follow up before the 7‐year follow‐up and, thus, were excluded; none of these patients died due to disease associated with the THA or had undergone removal of their cups as of our last evaluation. The mean age and body mass index of the patients were 59.37 (range: 38.00–69.00) years and 24.51 (range: 16.50–34.10) kg/m2, respectively. Thirteen (48.1%) of the patients were female.ResultsThe average duration of follow‐up was 93.48 (range: 89.00–99.00) months. The median Harris hip score improved from 42.00 (interquartile range: 37.00–49.00) to 97.00 (interquartile range: 92.00–97.00) at the latest follow up (P < 0.001). A total of 18 (66.7%) patients rated their satisfaction level as very satisfied, 6 (22.2%) as satisfied, 2 (7.4%) as neutral and 1 (3.7%) as dissatisfied. No intraoperative or postoperative complications were identified. At the latest follow up, all cups were considered to have achieved osteointegration fixation, with three or more of the five signs evident in the most recent X‐ray. However, three cups revealed radiolucent lines with a width of less than 1 mm. The median vertical and horizontal distances between the latest postoperative center of rotation relative to the anatomic center of rotation were 2.50 (interquartile range: −3.10, 6.94) mm superiorly and 3.26 (interquartile range: −8.12, 2.38) mm medially, respectively, at the most recent postoperative follow up. Kaplan–Meier survivorship analysis of cups, with the endpoint defined as postoperative radiolucent lines of less than 1 mm in width in at least two zones, reveals that the 8.25‐year survival was 96.3% (95% confidence interval: 76.49%–99.47%).ConclusionThe mid‐term follow‐up of patients who underwent primary THA using EBM‐produced porous coated titanium cups demonstrated favorable patient satisfaction, good clinical function, excellent survivorship, and adequate biological fixation.

Abstract:BACKGROUND The use of uncemented cups during total hip arthroplasty (THA) has gained popularity in recent years. The Robert Mathys (RM) pressfit cup, an uncemented monoblock implant is expected to preserve bone density due to its composition and external surface, while reducing backside wear with its monoblock construction. These factors should lead to a high survival rate of the implant. AIM To evaluate the mid-term survival and functional outcome of the RM Pressfit cup in a large study population. METHODS Between 2011 and 2020, we included 1324 patients receiving a primary THA using the RM pressfit cup. Final clinical follow-up was performed at 2 years postoperatively with the Dutch arthroplasty register used to assess implant status thereafter. Revision for acetabular failure and reason for revision were reported to evaluate implant survival, while the hip disability and osteoarthritis outcome score (HOOS) scores were used to assess functional outcome. RESULTS The mean age at surgery was 64.9 years. The mean follow-up was 4.6 years. Of the 1324 THAs performed, 13 needed cup revisions within 5 years after index THA: 5 due to aseptic loosening, 6 due to infection, 2 due to dislocation and 2 due to other causes. This resulted in a 5-year cup survival of 98.8% (95%CI: 98.1-99.5). Nine of the cup revisions occurred within the first year after index THA. HOOS scores increased significantly in all domains during the first year and levelled out during the second year. CONCLUSION In the present study, the RM pressfit cup demonstrated excellent clinical outcomes at mid-term follow-up; however, future studies are needed to assess the long-term outcomes of this acetabular implant.

Abstract:OBJECTIVE To evaluate the outcome of total hip arthroplasty (THA) with cementless cups and femoral head autografts for patients with hip dysplasia and osteoarthritis. METHODS Between 1995 and 2002, we implanted 23 cementless cups and femoral head autografts in 20 patients with hip dysplasia and osteoarthritis. In this study, a retrospective study was made on 21 hips in 20 patients (18 females and 2 males, aged 50 years on an average) with developmental hip dysplasia treated by THA with a cementless cup and femoral head autograft. The acetabular cup was placed at the level of the true acetabulum and all the patients required autogenous femoral head grafts due to acetabular deficiency. The average rate of the acetabular cup covered by the femoral head autograft was 31% (ranging from 10% to 45%). Eight hips had less than 25% cup coverage and thirteen between 25% and 50%. The average follow-up period was 4.7 years (range, 1-8 years). The replacing outcome was evaluated by modified Harris hip score. Preoperative and follow-up radiographs were made. RESULTS All the autografts were united to the host bones. No autograft was collapsed or no component from the hip was loosed in all the patients. According to the modified Harris hip score, the average hip score increased from 46 before operation to 89 at the final review. Before operation, the leg-length discrepancy was greater than 2 cm in all the patients except one with bilateral hip dysplasia. After operation, only 2 out of 20 patients had a leg-length discrepancy greater than 1 cm. Three hips showed minor bone resorption in the lateral portion of the graft, which did not support the cup. Three hips developed Grade 1 Brooker heterotopic ossification and one developed Grade 2. CONCLUSIONS THA with a cementless cup and a femoral head autograft for patients with osteoarthritis resulted from hip dysplasia can result in favorable outcomes. This method can provide reliable acetabular fixation and restore the acetabular bone stock in patients with developmental hip dysplasia when the cementless cup covered by the graft does not exceed 50%.

Abstract:A characteristic feature of imipenem-resistant strains of Pseudomonas aeruginosa is loss or decreased expression of the outer membrane protein (OMP) D2, whose molecular weight is 45 to 49 kDa. D2 was studied in 15 strains of P. aeruginosa with intermediate susceptibility or resistance to imipenem recovered from the sputum of 15 patients with cystic fibrosis. The OMP was extracted using Sarkosyl and separated by SDS-PAGE electrophoresis. Electrophoresis patterns were compared to those of reference strains 3B and 3C which are resistant and susceptible to imipenem, respectively. Expression of D2 was normal in three strains, weak or very weak in 11 strains and absent in one strain. For 12 strains, the alteration of the D2 protein was consistent with previous reports. However, the finding of normal D2 production in three strains is unusual and suggests the possible presence of another mechanism of resistance.

Abstract:Background: Threaded acetabular components (TACs) have been shown to offer greater initial stability compared to press-fit acetabular components (PFACs). Despite these biomechanical advantages, the use of threaded cups remains. This study compares the outcomes of TACs to PFACs in total hip arthroplasty (THA), providing evidence-based data regarding their failure rates and radiological evaluation. Methods: A meticulous research of PubMed and MEDLINE databases, following the PRISMA guidelines, was conducted, to identify all articles regarding the outcome of the use of TCAs compared to PFACs in THA. Subsequently, statistical analysis with metanalysis concerning: 1) the instances of revision due to aseptic loosening and 2) the radiological evaluations of TACs compared to PFACs and sensitivity analysis were performed. Results: This metanalysis encompassed seven studies, enrolling 7878 cases of THA utilizing PFAC, and 6684 cases using TAC. The overall odds ratio (OR) for revision due to aseptic loosening in THA using PFAC compared to TAC was 3.10 (95 % CI 0.37-25.72). Additionally, when assessing radiolucency findings across the same categories, the pooled OR was 0.53 (95 % CI 0.26-1.08). An examination of studies with larger sample sizes revealed no statistically significant variance. After adjusting for age, no substantial difference was detected. However, upon gender adjustment, it was observed that females undergoing THA with PFAC had a 5-fold risk of revision (OR = 5.26, 95 % CI 0.25-111.91, p-value = 0.29) compared to females with TAC, although without reaching statistical significance. Moreover, females exhibited a slightly elevated risk for revision due to aseptic loosening post-PFAC THA compared to males [(OR = 5.26, 95 % CI 0.25-111.91) (OR = 2.51, 95 % CI 0.01-1051.68) respectively], and for radiolucency findings [(OR 0.74, 95 % CI 0.20-1.11) (OR 0.29, 95 % CI 0.03-3.36) respectively]. Conclusions: The PFACs remain the main option for total hip reconstruction, while TACs might be a viable alternative, especially in cases of osteoporosis.

Abstract:Background and purpose: Ceramic liners may reduce early stability of uncemented acetabular components due to higher stiffness. However, the bone ingrowth capacities of porous trabecular titanium might compensate for this effect. This prospective randomized trial quantifies migration patterns of the Delta-TT cup, and compares polyethylene and ceramic liners.Patients and methods: Patients undergoing primary uncemented total hip arthroplasty with the Delta-TT cup and femoral stem with ceramic head were randomized to a polyethylene (n = 25) or ceramic (n = 28) liner. Radiostereometric analysis (RSA) radiographs, patient-reported hip function (HOOS-PS, OHS), and quality of life (EQ5D) were collected at baseline and 1.5, 3, 6, 12, and 24 months postoperatively. Model-based RSA was used to calculate 3D cup translation and rotation, and mixed models were used to compare effects over time between groups.Results: At 2 years follow-up, Delta-TT cups showed similar mean proximal translation of 0.56 mm (95% CI 0.38–0.75) in the ceramic (CE) group and 0.54 mm (0.30–0.77) in the polyethylene (PE) group, with a between group effect of 0.02 mm (–0.20–0.23). Most cup migration occurred in the first 1.5 to 3 months, stabilizing within 6 months. Any between-group effects were ≤ 0.30 mm for translation and ≤ 0.45° for rotation. Improvements in patient-reported hip function and quality of life were similar in both groups.Interpretation: Regardless of liner type, Delta-TT cups showed some initial migration and stabilized within 6 months, which seems promising for long-term fixation in both cup-liner constructs.

Abstract:Increased synthesis of insulin-like growth factor-1 is induced in murine macrophages by prostaglandin E2 (PGE2) and tumor necrosis factor-alpha (TNFalpha). Accordingly, we have investigated mechanisms regulating synthesis of PGE2 that might contribute to autocrine/paracrine effects on insulin-like growth factor-1 production. In response to zymosan, TNFalpha specifically induced a 5-fold increase in PGE2 synthesis, at the same time decreasing PGD2 production in a reciprocal fashion. Activators of cyclic AMP-dependent protein kinase (PKA), such as PGE2 itself or dibutyryl cyclic AMP, did not modify PGE2 production by themselves but potentiated the TNFalpha-induced increase in PGE2; this effect required both RNA and protein synthesis. No significant change in arachidonate release or production of other eicosanoids was observed. The inducible form of cyclooxygenase-2 (COX2) but not of the constitutive form COX1 was implicated in the generation of both PGE2 and PGD2 in these cells by use of specific inhibitors and effects of dexamethasone. Neither COX1 nor COX2 protein levels were affected by TNFalpha or PKA activators used alone, whereas in association, marked up-regulation of COX2 mRNA and protein was observed. Incubations of cells carried out with PGH2 demonstrated that PGE2 synthase activity was increased after a TNFalpha pretreatment. Taken together, our results suggest that TNFalpha induced a switch from the PGD2 to PGE2 synthesis pathway by regulating PGE2 synthase expression and/or activity and that activators of PKA markedly potentiated the TNFalpha-induced increase in PGE2 through up-regulation of COX2 gene expression.
Survival and complications of total hip arthroplasty using third-generation dual-mobility cups with non-cross-linked polyethylene liners in patients younger than 55years

Abstract:Background: In younger patients, total hip prostheses are subjected to wide motion ranges and wear-inducing forces. Dual-mobility cups (DMCs) are effective in decreasing the risk of dislocation. However, wear and osteolysis have been reported with first-generation DMCs. These complications have not been assessed in younger patients managed with third-generation DMCs associated with a lower risk of intra-prosthetic dislocation (IPD). We therefore designed a retrospective study of patients younger than 55years at THA with third-generation DMCs. Our objectives were to evaluate (1) the complication rate, and (2) the survival rate. Hypothesis: The rate of complications of THA with third-generation DMCs in patients younger than 55years of age is comparable to that in the general population of THA patients. Material and methods: This retrospective study included 79 consecutive patients (91 hips) who had total hip arthroplasty (THA) between 2007 and 2012. We included all patients younger than 55years who underwent primary THA with a third-generation DMC and a liner made of non-cross-linked polyethylene. The patients were evaluated clinically and radiologically. Results: Mean follow-up was 9.8years (range: 2-13years). At last follow-up, no patient had experienced prosthetic dislocation or IPD. Aseptic and septic cup loosening each occurred in 1 patient. In all, 7 (7.7%) patients required revision surgery with exchange of at least one component (cup loosening, n=2; neck fracture with a short stem fixed to the neck, n=1; peri-prosthetic femoral fracture, n=1; infection, n=1; and femoral-stem loosening, n=2). Peri-acetabular osteolysis developed in 2 (2.2%) hips. Cup survival to aseptic loosening was 98.9% (95% confidence interval, 97%-100%) and survival to revision for any reason except infection was 95.6% (95% confidence interval, 82%-100%). Discussion: In patients younger than 55years, third-generation DMCs were associated with absence of prosthetic dislocation, absence of IPD, and a low revision rate for cup loosening, in the medium term. These implants therefore constitute a viable treatment option in younger patients requiring THA. Level of evidence: IV, retrospective study.
The Avantage ® dual mobility cup in primary total hip arthroplasty: A registry study

Abstract:Background: Avantage Cup has been widely used in dual mobility implants. However, in Swedish Registry, the outcome of the Avantage Cup is reported with higher implants revision compared to control. The aim of our study was to verify if the same results are present in the Registry of Prosthetic Orthopedic Implants (RIPO) of Emilia Romagna (ER, Italy), as the Avantage cup was the most implanted dual mobility cup for a long follow-up reported in this Registry (2000-2012). Furthermore, we assessed the survival rate of the implant over the time. Methods: We included all patients that underwent a primary THA using the Avantage cup during the period 2000-2020 in RIPO Registry. The survivorship of the primary THA implants was calculated and plotted according to Kaplan-Meier method. Results: 886 Avantage cups were included in the analysis. During the observational period 44 hips were revised. The most common reasons for revision were: periprosthetic fractures (PPF) (n = 7, 0.8 %), deep infection (n = 7, 0.8 %), and cup aseptic loosening (n = 13, 1.5 %).The survival rate of the implant was 96.8 % (95.3-97.8) at 5 years, 95.7 at 10 years (94.0-97.0) and 92.1 at 15 years (88.5-94.6). Conclusion: In conclusion, this study has demonstrated that the Avantage cup in primary hip arthroplasty implanted with a "friendly" femoral stem granted satisfactory long-term survival. Therefore, in the Swedish Registry, the cause of the poor results presented for Avantage Cup could be the thick, rough neck stem of the widely used Lubinus stem.

Abstract:Aims: To evaluate the outcome of different types of polyethylene, bone cements and one design of uncemented fixation with porous and ceramic coating using radiostereometry, bone densitometry, conventional radiography and clinical parameters. Materials and methods: Study I: 201 patients were extracted from 5 prospective randomised studies to evaluate femoral head penetration at two years with radiostereometry in four basic designs, cemented Lubinus and Reflection cups, uncemented Trilogy and Reflection cups. Studies II and III: 60 patients (61 hips) were randomised to receive either highly cross-linked or conventional all PE cups. 32 patients with bilateral arthrosis received hybrid THA with highly cross-linked PE on one side and conventional on the contra lateral side. Femoral head penetration and the migration of the cups were evaluated with radiostereometry in the supine and standing positions. DEXA and conventional radiography were used to evaluate the bone mineral density and radiolucencies around the cemented acetabular component. Studies IV and V: 90 patients (97 and 96 hips respectively) were stratified depended on age, gender, diagnosis and preoperative BMD to create 3 main groups of socket fixation. In the first group fluoride containing cement was used, in the second group Palacos cum Gentamicin and in the third hybrid THA with porous coated HA/TCP cup. In the hybrid group the fixation of the femoral component was again randomised to either of the two cements. The results on femoral and acetabular sides are presented separately in studies IV and V, respectively. Results: Study I: Cups with polyethylene sterilized in EtO had almost twice the proximal and 3D penetration rates compared with gamma-sterilized polyethylene. Regression analysis showed that the type of sterilization, age and weight was the most important factors affecting the penetration rate. Studies II and III: In the cemented study the proximal penetration was lower in the study group independent of position at 3 years, while in the hybrid study the penetration was lower in the study group only in the supine position at 2 years. The migration of the cup did not differ between the plastics in both studies. At 2 years the periprosthetic radiolucency and BMD did not differ significantly between the 2 types of PE used in the cemented study. Study IV: The subsidence of the stem did not differ between the groups, but the periprosthetic BMD decreased more in fluoride cement group at 2 years. Conventional radiography revealed higher progression of radiolucent lines in the Palacos group, but only in one region. Study V: The proximal migration of the cup was almost similar in all three groups. The three dimensional migration was increased in patients with osteoporosis. Postoperative radiolucent lines tended to disappear with use of porous coating covered with HA/TCP. Conclusions: Study I: EtO sterilized polyethylene increased the femoral head penetration. Age and weight were also important predictors of the penetration rate. Studies II and III: The highly cross-linked polyethylene decreased the penetration rate mainly after one year probably reflecting less wear. The different mechanical properties of the two types of PE studied did not affect the early fixation of the cemented cup. Study IV: There were no obvious advantage of addition of fluoride to acrylic bone cement when used to fixate the femoral component. Study V: Use of fluoride containing cement or uncemented fixation did not improve the early stability of the socket compared to Palacos with Gentamicin.
