Abstract:Background Respiratory motion compensation is usually required in free-breathing thoracic MR imaging applications to remove the motion induced blur and ghosting artifacts. Most respiratory motion compensation techniques are based on gating in which data is acquired only at one respiratory state (RS) and therefore suffer from extended yet unpredictable scan time. Other techniques compensate for motion by correcting the phase of motion corrupted k-space data so that data acquired in multiple respiratory states can be used in reconstruction for improved scan efficiency. However, these methods are often limited to correcting rigid-body motion and thus not suitable for thoracic imaging with large field-ofview. In this work, we propose a technical strategy of correcting 3D non-rigid motion and apply it to ferumoxytol enhanced free-breathing thoracic MRA.

Abstract:Cross-sectional imaging of the lungs, or pulmonary imaging, has proven to be an incredibly valuable tool in a wide range of pulmonary diseases. The vast majority of lung imaging is done with CT, as it is fast enough to freeze respiratory motion and provides high spatial resolution to visualize fine structure of the lungs. MRI of the lungs is inherently challenging due to the presence of large local magnetic field gradients, relatively low proton density, and motion. The benefits of performing MRI for lung imaging include no ionizing radiation, opportunities for multiple contrasts, and integration with other MRI also offers the opportunity to obtain multiple tissue contrasts. The most common lung MRI techniques are structural T1-weighted scans, but also emerging are functional contrasts such as ventilation and perfusion, as well as other MRI contrast mechanisms including T2-weighting and diffusion-weighting. Finally, lung MRI can be combined with other MRI scanning techniques, including cardiac MRI, abdominal MRI, whole-body MRI, and PET/MRI, for increasing examination efficiency by only requiring a single scan session and providing more comprehensive assessment that includes evaluation of the pulmonary system. This article covers pulse sequences, motion management methods, image reconstruction, and contrast mechanisms of UTE MRI (e.g. T1-weighting, ventilation mapping) for imaging of the lung.

Abstract:Purpose: To propose and validate a respiratory motion resolved, self-gated (SG) 4D-MRI technique to assess patient-specific breathing motion of abdominal organs for radiation treatment planning. Methods: The proposed 4D-MRI technique was based on the balanced steady-state free-precession (bSSFP) technique and 3D k-space encoding. A novel rotating cartesian k-space (ROCK) reordering method was designed which incorporates repeatedly sampled k-space centerline as the SG motion surrogate and allows for retrospective k-space data binning into different respiratory positions based on the amplitude of the surrogate. The multiple respiratory-resolved 3D k-space data were subsequently reconstructed using a joint parallel imaging and compressed sensing method with spatial and temporal regularization. The proposed 4D-MRI technique was validated using a custom-made dynamic motion phantom and was tested in six healthy volunteers, in whom quantitative diaphragm and kidney motion measurements based on 4D-MRI images were compared with those based on 2D-CINE images. Results: The 5-minute 4D-MRI scan offers high-quality volumetric images in 1.2 x 1.2 x 1.6 mm(3) and eight respiratory positions, with good soft-tissue contrast. In phantom experiments with triangular motion waveform, the motion amplitude measurements based on 4D-MRI were 11.89% smaller than the ground truth, whereas a -12.5% difference was expected due to data binning effects. In healthy volunteers, the difference between the measurements based on 4D-MRI and the ones based on 2D-CINE were 6.2 4.5% for the diaphragm, 8.2 +/- 4.9% and 8.9 +/- 5.1% for the right and left kidney. Conclusion: The proposed 4D-MRI technique could provide high-resolution, high-quality, respiratory motion-resolved 4D images with good soft-tissue contrast and are free of the stitching artifacts usually seen on 4D-CT and 4D-MRI based on resorting 2D-CINE. Abstract:Background Our recently proposed MUSIC approach acquires ferumoxytol-enhanced 4D images using ECG and airway pressure signal for motion gating. It has been routinely used in our institution and provides detailed information on vascular anatomy. However, the airway pressure signal is only available in patients with general anesthesia (GA) and ECG is often problematic in high field strength. Therefore, we propose the self-gated MUSIC (SGMUSIC) where cardiac and respiratory motion is compensated by retrospective data sorting base on derived SG signals. In this study, we tested SG-MUSIC on pediatric patients under GA so that the derived SG signal can be validated against the recorded physiological signal and the images compared with the original MUSIC.

Abstract:Purpose, To develop a high scanning efficiency, motion corrected imaging strategy for free-breathing pulmonary MRI by combining a motion compensation reconstruction with a UTE acquisition, called iMoCo UTE. Methods, An optimized golden angle ordering radial UTE sequence was used to continuously acquire data for 5 minutes. All readouts were grouped to different respiratory motion states based on self-navigator signals, then motion resolved data was reconstructed by XD Golden angle RAdial Sparse Parallel reconstruction (XD GRASP). One state from the motion resolved images was selected as a reference, and motion fields from the other states to the reference were derived via non-rigid registration. Finally, all motion resolved data and motion fields were reconstructed by using an iterative motion compensation reconstruction with a total generalized variation sparse constraint. Results, The iMoCo UTE strategy was evaluated in volunteers and non-sedated pediatric patient(4-6 y/o) studies. Images reconstructed with iMoCo UTE provided sharper anatomical lung structures, and higher apparent SNR and CNR, compared to using other motion correction strategies, such as soft-gating, motion resolved reconstruction, and non-rigid motion compensation(MoCo). iMoCo UTE also showed promising results in an infant study. Conclusions, The proposed iMoCo UTE combines self-navigation, motion modeling, and a compressed sensing reconstruction to increase scan efficiency, SNR, and reduce respiratory motion in lung MRI. This proposed strategy shows improvements in free breathing lung MRI scans, especially in very challenging application situations, such as pediatric MRI studies.

Abstract:PurposeTo develop a free‐breathing, non‐electrocardiogram technique for simultaneous myocardial T1, T2, T2*, and fat‐fraction (FF) mapping in a single scan.MethodsThe MR Multitasking framework is adapted to quantify T1, T2, T2*, and FF simultaneously. A variable TR scheme is developed to preserve temporal resolution and imaging efficiency. The underlying high‐dimensional image is modeled as a low‐rank tensor, which allows accelerated acquisition and efficient reconstruction. The accuracy and/or repeatability of the technique were evaluated on static and motion phantoms, 12 healthy volunteers, and 3 patients by comparing to the reference techniques.ResultsIn static and motion phantoms, T1/T2/T2*/FF measurements showed substantial consistency (R > 0.98) and excellent agreement (intraclass correlation coefficient > 0.93) with reference measurements. In human subjects, the proposed technique yielded repeatable T1, T2, T2*, and FF measurements that agreed with those from references.ConclusionsThe proposed free‐breathing, non‐electrocardiogram, motion‐resolved Multitasking technique allows simultaneous quantification of myocardial T1, T2, T2*, and FF in a single 2.5‐min scan.

Abstract:Purpose To investigate motion compensated, self‐supervised, model based deep learning (MBDL) as a method to reconstruct free breathing, 3D pulmonary UTE acquisitions. Theory and Methods A self‐supervised eXtra dimension MBDL architecture (XD‐MBDL) was developed that combined respiratory states to reconstruct a single high‐quality 3D image. Non‐rigid motion fields were incorporated into this architecture by estimating motion fields from a lower resolution motion resolved (XD‐GRASP) reconstruction. Motion compensated XD‐MBDL was evaluated on lung UTE datasets with and without contrast and compared to constrained reconstructions and variants of self‐supervised MBDL that do not account for dynamic respiratory states or leverage motion correction. Results Images reconstructed using XD‐MBDL demonstrate improved image quality as measured by apparent SNR (aSNR), contrast to noise ratio (CNR), and visual assessment relative to self‐supervised MBDL approaches that do not account for dynamic respiratory states, XD‐GRASP and a recently proposed motion compensated iterative reconstruction strategy (iMoCo). Additionally, XD‐MBDL reduced reconstruction time relative to both XD‐GRASP and iMoCo. Conclusion A method was developed to allow self‐supervised MBDL to combine multiple respiratory states to reconstruct a single image. This method was combined with graphics processing unit (GPU)‐based image registration to further improve reconstruction quality. This approach showed promising results reconstructing a user‐selected respiratory phase from free breathing 3D pulmonary UTE acquisitions.

Abstract:Purpose: The purpose of this study was to investigate the use of a 2D rosette trajectory for breath-hold and free-breathing real-time imaging of the lungs. Methods: Eight healthy volunteers underwent breath-hold magnetic resonance imaging (MRI) using two different parametrizations of the proposed single-petal rosette (SPR) trajectory, as well as radial ultra-short echo time (UTE) acquisition combined with the tiny golden-angle acquisition scheme. The additional free-breathing acquisitions of all trajectories were performed. The proposed technique was compared with the conventional radial UTE technique regarding image sharpness, signal-to-noise ratio (SNR), regional fractional ventilation (FV) for breath-hold and retrospective image-based self-gating, and real-time imaging capabilities. Results: Image sharpness significantly increased for gated SPR images compared to radial UTE for the end-expiratory stage; no significant difference was found for the self-gated end-inspiratory stage and the breath-hold acquisitions. The SPR trajectory performs significantly better than radial UTE concerning the SNR for breath-hold imaging, whereas no significant difference was found for self-gated images. Fractional ventilation values were comparable between SPR and radial UTE. The SPR real-time data showed a marked reduction in the aliasing artifacts with no apparent streaks. Conclusion: The second half of the petal of the SPR trajectory accomplishes rephasing while acquiring non-redundant k-space data, thus leading to a more efficient sampling of the k-space than that with the radial UTE sampling scheme, with a minimal increase in TR. This leads to the need of less read-outs to achieve the same SNR values and, thus, a reduction in scan time. For real-time application, the SPR approach resulted in a marked reduction of aliasing artifacts when compared to UTE.

Abstract:Purpose: Dynamic or 4D imaging of the thorax has many applications. Both prospective and retrospective respiratory gating and tracking techniques have been developed for 4D imaging via CT and MRI. For pediatric imaging, due to radiation concerns, MRI becomes the de facto modality of choice. In thoracic insufficiency syndrome (TIS), patients often suffer from extreme malformations of the chest wall, diaphragm, and/or spine with inability of the thorax to support normal respiration or lung growth (Campbell et al., 2003, Campbell and Smith, 2007), as such patient cooperation needed by some of the gating and tracking techniques are difficult to realize without causing patient discomfort and interference with the breathing mechanism itself. Therefore (ventilator-supported) free-breathing MRI acquisition is currently the best choice for imaging these patients. This, however, raises a question of how to create a consistent 4D image from such acquisitions. This paper presents a novel graph-based technique for compiling the best 4D image volume representing the thorax over one respiratory cycle from slice images acquired during unencumbered natural tidal-breathing of pediatric TIS patients. Methods: In our approach, for each coronal (or sagittal) slice position, images are acquired at a rate of about 200-300ms/slice over several natural breathing cycles which yields over 2000 slices. A weighted graph is formed where each acquired slice constitutes a node and the weight of the arc between two nodes defines the degree of contiguity in space and time of the two slices. For each respiratory phase, an optimal 3D spatial image is constructed by finding the best path in the graph in the spatial direction. The set of all such 3D images for a given respiratory cycle constitutes a 4D image. Subsequently, the best 4D image among all such constructed images is found over all imaged respiratory cycles. Two types of evaluation studies are carried out to understand the behavior of this algorithm and in comparison to a method called Random Stacking - a 4D phantom study and 10 4D MRI acquisitions from TIS patients and normal subjects. The 4D phantom was constructed by 3D printing the pleural spaces of an adult thorax, which were segmented in a breath-held MRI acquisition. Results: Qualitative visual inspection via cine display of the slices in space and time and in 3D rendered form showed smooth variation for all data sets constructed by the proposed method. Quantitative evaluation was carried out to measure spatial and temporal contiguity of the slices via segmented pleural spaces. The optimal method showed smooth variation of the pleural space as compared to Random Stacking whose behavior was erratic. The volumes of the pleural spaces at the respiratory phase corresponding to end inspiration and end expiration were compared to volumes obtained from breath-hold acquisitions at roughly the same phase. The mean difference was found to be roughly 3%. Conclusions: The proposed method is purely image-based and post-hoc and does not need breath holding or external surrogates or instruments to record respiratory motion or tidal volume. This is important and practically warranted for pediatric patients. The constructed 4D images portray spatial and temporal smoothness that should be expected in a consistent 4D volume. We believe that the method can be routinely used for thoracic 4D imaging.

Abstract:Background MRI has a rapidly growing role in radiation therapy (RT) for treatment planning, real‐time image guidance, and beam gating (e.g., MRI‐Linac). Free‐breathing 4D‐MRI is desirable in respiratory motion management for therapy. Moreover, high‐quality 3D‐MRIs without motion artifacts are needed to delineate lesions. Existing MRI methods require multiple scans with lengthy acquisition times or are limited by low spatial resolution, contrast, and signal‐to‐noise ratio. Purpose We developed a novel method to obtain motion‐resolved 4D‐MRIs and motion‐integrated 3D‐MRI reconstruction using a single rapid (35‐45 s scan on a 0.35 T MRI‐Linac. Methods Golden‐angle radial stack‐of‐stars MRI scans were acquired from a respiratory motion phantom and 12 healthy volunteers (n = 12) on a 0.35 T MRI‐Linac. A self‐navigated method was employed to detect respiratory motion using 2000 (acquisition time = 5–7 min) and the first 200 spokes (acquisition time = 35–45 s). Multi‐coil non‐uniform fast Fourier transform (MCNUFFT), compressed sensing (CS), and deep‐learning Phase2Phase (P2P) methods were employed to reconstruct motion‐resolved 4D‐MRI using 2000 spokes (MCNUFFT2000) and 200 spokes (CS200 and P2P200). Deformable motion vector fields (MVFs) were computed from the 4D‐MRIs and used to reconstruct motion‐corrected 3D‐MRIs with the MOtion Transformation Integrated forward‐Fourier (MOTIF) method. Image quality was evaluated quantitatively using the structural similarity index measure (SSIM) and the root mean square error (RMSE), and qualitatively in a blinded radiological review. Results Evaluation using the respiratory motion phantom experiment showed that the proposed method reversed the effects of motion blurring and restored edge sharpness. In the human study, P2P200 had smaller inaccuracy in MVFs estimation than CS200. Electronic angular momentum polarizations of photofragments: a case study of ICN photodissociation from a perpendicular transition.

Abstract:A quantum treatment on ICN photodissociation from an initial perpendicular transition (Omega'=+/-1<--Omega"=0) to the asymptote CN(|Sigma+,J'M'N'1/2>)+I(2P3/2) is presented. Density matrices of both photofragments are derived and explicit expressions of the state multipoles in terms of the angular momentum coupling coefficients and the rotation-bending factors have been obtained. To perceive the physical origin of electronic angular momentum polarizations of the iodine photofragments, a correlation scheme which considers the magnetic dipolar and the electrostatic dipole-quadrupole interactions between I and CN cofragments is proposed. For ICN precursors in the vibrational ground state or in the equally populated l-type split levels, the alignment parameters of the iodine photofragments in the molecular frame can be calculated according to this long-range interaction model. For the perpendicular transition |1Pi1><--|1Sigma0+>, its alignment parameters of I(2P3/2) from the incoherent and coherent transitions to the |Omega'=1> and |Omega'=-1> components are rho(0)2(1Pi1)=0.756 and rho2(2)(1Pi1)=-0.656, respectively. For the perpendicular transition to |3Pi1>, rho(0)2(3Pi1)=-0.878 and rho2(2)(3Pi1)=0.328 are from the incoherent transition, whereas rho(0)2(3Pi1)=0.122 and rho2(2)(3Pi1)=0.328 are from the coherent transition. To analyze the photoion images of iodine photofragments, angular distributions of I+ from the 2+1 resonance-enhanced multiphoton ionization detection scheme are derived.
Free-breathing Dynamic Contrast-Enhanced MRI for Assessment of Pulmonary Lesions Using Golden-Angle Radial Sparse Parallel Imaging.

Abstract:BackgroundDynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) has been shown to be a promising technique for assessing lung lesions. However, DCE-MRI often suffers from motion artifacts and insufficient imaging speed. Therefore, highly accelerated free-breathing DCE-MRI is of clinical interest for lung exams. PurposeTo test the performance of rapid free-breathing DCE-MRI for simultaneous qualitative and quantitative assessment of pulmonary lesions using Golden-angle RAdial Sparse Parallel (GRASP) imaging. Study TypeProspective. PopulationTwenty-six patients (17 males, mean age=55.114.4) with known pulmonary lesions. Field Strength/Sequence3T MR scanner; a prototype fat-saturated, T-1-weighted stack-of-stars golden-angle radial sequence for data acquisition and a Cartesian breath-hold volumetric-interpolated examination (BH-VIBE) sequence for comparison. AssessmentAfter a dual-mode GRASP reconstruction, one with 3-second temporal resolution (3s-GRASP) and the other with 15-second temporal resolution (15s-GRASP), all GRASP and BH-VIBE images were pooled together for blind assessment by two experienced radiologists, who independently scored the overall image quality, lesion delineation, overall artifact level, and diagnostic confidence of each case. Perfusion analysis was performed for the 3s-GRASP images using a Tofts model to generate the volume transfer coefficient (K-trans) and interstitial volume (V-e). Statistical TestsNonparametric paired two-tailed Wilcoxon signed-rank test; Cohen's kappa; unpaired Student's t-test. Results15s-GRASP achieved comparable image quality with conventional BH-VIBE (P>0.05), except for the higher overall artifact level in the precontrast phase (P=0.018). The K-trans and V-e in inflammation were higher than those in malignant lesions (K-trans: 0.780.52min(-1) vs. 0.370.22min(-1), P=0.020; V-e: 0.360.16 vs. 0.26 +/- 0.1, P=0.177). Also, the K-trans and V-e in malignant lesions were also higher than those in benign lesions (K-trans: 0.37 +/- 0.22min(-1) vs. 0.04 +/- 0.04min(-1), P=0.001; V-e: 0.26 +/- 0.12 vs. 0.10 +/- 0.00, P=0.063). Data ConclusionThis feasibility study demonstrated the performance of high spatiotemporal resolution free-breathing DCE-MRI of the lung using GRASP for qualitative and quantitative assessment of pulmonary lesions. Abstract:PurposeMagnetization transfer (MT) MRI can be effective for the diagnosis of a broad range of fibrotic diseases, including liver fibrosis. However, respiratory motion, a major source of artifacts in thoracic and abdominal MR imaging, can obscure important anatomic structures, making diagnosis difficult. In this study, we explored the potential to combine free‐breathing (FB) respiratory self‐gating (RSG) methods with MT saturation for FB MT ratio (MTR) measurements of abdominal organs.MethodsA respiratory self‐gated multiple‐gradient recalled echo sequence with MT presaturation (RSG‐MT GRE) was developed and applied in a series of seven normal volunteers. We compared the MTR values of liver, pancreas, kidney, spleen, and posterior paraspinal muscle measured using our RSG‐MT GRE sequence and a conventional MT GRE sequence.ResultsRSG consistently reduced motion artifacts within MT‐weighted images acquired during FB, improved the accuracy of FB MTR measurements, and produced comparable MTRs to breath‐holding MTR measurements.ConclusionRSG approaches may offer to improve the utility of MT‐weighted imaging methods for the assessment of fibrotic diseases and tumor desmoplasia in abdominal organs. Abstract:Cardiac magnetic resonance imaging (MRI) is widely used for non-invasive assessment of cardiac morphology, function, and tissue characteristics due to its exquisite soft-tissue contrast. However, it remains time-consuming and requires proficiency, making it costly and limiting its widespread use. Traditional cardiac MRI is inefficient as signal acquisition is often limited to specific cardiac phases and requires complex view planning, parameter adjustments, and management of both respiratory and cardiac motion. Recent efforts have aimed to make cardiac MRI more efficient and accessible. Among these innovations, the free-running framework enables 5D whole-heart imaging without the need for an electrocardiogram signal, respiratory breath-holding, or complex planning. It uses a fully self-gated approach to extract cardiac and respiratory signals directly from the acquired image data, allowing for more efficient coverage in time and space without the need for electrocardiogram gating, triggering, navigators, or breath-holds. This review provides a comprehensive overview of the free-running framework, detailing its history, concepts, recent improvements, and clinical applications.

Abstract:Despite a myriad of technical advances in medical imaging, as well as the growing need to address the global impact of pulmonary diseases, such as asthma and chronic obstructive pulmonary disease, on health and quality of life, it remains challenging to obtain in vivo regional depiction and quantification of the most basic physiological functions of the lung—gas delivery to the airspaces and gas uptake by the lung parenchyma and blood—in a manner suitable for routine application in humans. We report a method based on MRI of hyperpolarized xenon-129 that permits simultaneous observation of the 3D distributions of ventilation (gas delivery) and gas uptake, as well as quantification of regional gas uptake based on the associated ventilation. Subjects with lung disease showed variations in gas uptake that differed from those in ventilation in many regions, suggesting that gas uptake as measured by this technique reflects such features as underlying pathological alterations of lung tissue or of local blood flow. Furthermore, the ratio of the signal associated with gas uptake to that associated with ventilation was substantially altered in subjects with lung disease compared with healthy subjects. This MRI-based method provides a way to quantify relationships among gas delivery, exchange, and transport, and appears to have significant potential to provide more insight into lung disease.

High-quality FLORET UTE imaging for clinical translation

Abstract:Purpose: To develop a robust 3D ultrashort-TE (UTE) protocol that can reproducibly provide high-quality images, assessed by the ability to yield clinically diagnostic images, and is suitable for clinical translation. Theory and methods: Building on previous work, a UTE sampled with Fermat looped orthogonally encoded trajectories (FLORET) was chosen as a starting point due to its shorter, clinically reasonable scan times. Modifications to previous FLORET implementations included gradient waveform frequency limitations, a new trajectory ordering scheme, a balanced SSFP implementation, fast gradient spoiling, and full inline reconstruction. FLORET images were collected in phantoms and humans on multiple scanners and sites to demonstrate these improvements. Results: The updates to FLORET provided high-quality images in phantom, musculoskeletal, and pulmonary applications. The gradient waveform modifications and new trajectory ordering scheme significantly reduced visible artifacts. Fast spoiling reduced acquisition time by 20%-28%. Across the various scanners and sites, the inline image quality was consistent and of diagnostic quality. Total image acquisition plus reconstruction time was less than 4 min for musculoskeletal and pulmonary applications with reconstructions taking less than 1 min. Conclusion: Recently developed improvements for the FLORET sequence have enabled robust, high-quality UTE acquisitions with short acquisition and reconstruction times. This enables clinical UTE imaging as demonstrated by the implementation of the sequence and acquisition on five MRI scanners, at three different sites, without the need for any additional system characterization or measurements.

Abstract:A joint image reconstruction and segmentation approach based on disentangled representation learning was trained to enable cardiac cine MR imaging in real-time and under free-breathing. An exploratory feasibility study tested the proposed method in undersampled real-time acquisitions based on an in-house developed spiral bSSFP pulse sequence in eight healthy participants and five patients with intermittent atrial fibrillation. Images and predicted LV segmentations were compared to the reference standard of ECG-gated segmented Cartesian cine in repeated breath-holds and corresponding manual segmentation. On a 5-point Likert scale, image quality of the real-time breath-hold approach and Cartesian cine was comparable in healthy participants (RT-BH: 1.99 $\pm$ .98, Cartesian: 1.94 $\pm$ .86, p=.052), but slightly inferior in free-breathing (RT-FB: 2.40 $\pm$ .98, p<.001). In patients with arrhythmia, image quality from both real-time approaches was favourable (RT-BH: 2.10 $\pm$ 1.28, p<.001, RT-FB: 2.40 $\pm$ 1.13, p<.001, Cartesian: 2.68 $\pm$ 1.13). Intra-observer reliability was good (ICC=.77, 95%-confidence interval [.75, .79], p<.001). In functional analysis, a positive bias was observed for ejection fractions derived from the proposed model compared to the clinical reference standard (RT-BH mean EF: 58.5 $\pm$ 5.6%, bias: +3.47%, 95%-confidence interval [-.86, 7.79%], RT-FB mean: 57.9 $\pm$ 10.6%, bias: +1.45%, [-3.02, 5.91%], Cartesian mean: 54.9 $\pm$ 6.7%). The introduced real-time MR imaging technique is capable of acquiring high-quality cardiac cine data in 1-2 minutes without the need for ECG gating and breath-holds. It thus offers a promising alternative to the current clinical practice of segmented acquisition, with shorter scan times, higher patient comfort and increased robustness to arrhythmia and patient incompliance.

Abstract:Magnetic resonance imaging (MRI) of the lung is technically challenging due to the low proton density and fast signal decay of the lung parenchyma itself. Additional challenges consist of tissue loss, hyperinflation, and hypoxic hypoperfusion, e.g., in emphysema, a so-called "minus-pathology". However, pathological changes resulting in an increase of tissue ("plus-pathology"), such as atelectases, nodules, infiltrates, mucus, or pleural effusion, are easily depicted with high diagnostic accuracy. Although MRI is inferior or at best equal to multi-detector computed tomography (MDCT) for the detection of subtle morphological features, MRI now offers an increasing spectrum of functional imaging techniques such as perfusion assessment and measurement of ventilation and respiratory mechanics that are superior to what is possible with MDCT. Without putting patients at risk with ionizing radiation, repeated examinations allow for the evaluation of the course of lung disease and monitoring of the therapeutic response through quantitative imaging, providing a level of functional detail that cannot be obtained by any other single imaging modality. As such, MRI will likely be used for clinical applications beyond morphological imaging for many lung diseases. In this article, we review the technical aspects and protocol suggestions for chest MRI and discuss the role of MRI in the evaluation of nodules and masses, airway disease, respiratory mechanics, ventilation, perfusion and hemodynamics, and pulmonary vasculature.
